Abstract

Results of the hemostasis conduction in conditions of revascularization in 106 patients, оperated on for atherosclerotic affection of aorta and the main arteries of the lower extremities, were adduced. Syndrome of hypercoagulation of traumatic stage of surgical intervention in early postoperative period is developing due to thrombinemia on background of a fibrinolytic system depression. There was proved a necessity to impact on thrombin-fibrinous factor (factor ІІа) of hemocoagulant cascade by application of nonfractionized heparins immediately after conclusion of operative intervention with thromboprophylaxis prolongation, using low-molecular heparins (impact on Ха factor) in accordance to the branch standards.

Abstract

The experience of the hybrid surgical interventions performing in 24 patients with diabetes mellitus type II of complicated ischemic form of diabetic foot syndrome was adduced. According to the results of preoperative examination of patients showed a multilevel occlusive—stenotic lesions of the lower limb arteries. The hybrid arterial reconstruction operations performing has improved the results of surgical treatment of such patients.

Abstract

Lymphorrhea have had occur after performance of 1381 reconstructive operative interventions on arteries of the lower extremities in 42 (3.04%) of patients. While application of 76% solution of triombrast and further elastic compression in 83.3% observations a good result was achieved, in 16.7%--satisfactory.

Abstract

The influence of the prolonged perineural femoral nerve blockade to restoration of blood flow in operated patients with occlusive disease of the lower limb arteries using an ultrasound was stadied. As for the results of the study, the use of proposed technique, reduces recovery of Vs in lower limb vessels for 3 days, Vd for 7 days and IPgk for 3 days in comparison with control group.

Abstract

Main reasons and frequency of early postoperative thrombotic complications were analyzed. The frequency of early postoperative thrombosis in vessels of femoro-popliteal-tibial segment composed 19.1%. The most frequency of early reocclusion observed after the femoro-tibial reconstructive operations (in 32.4% patients), the least--after femoro-popliteal grafting at above knee level (in 14.1%). The main reason for early reocclusion is an inadequate estimation of distal arteries damage.
