Pyoderma gangrenosum is a neutrophilic dermatosis which is characterized by chronic, non-infectious ulcers. There is no standard treatment or algorithm for pyoderma gangrenosum. The choice of treatment varies depending on the type of ulcer, growth rate of ulcer, general condition of the patient, and the underlying disease. Thrombocyte-rich fibrin which contains platelets and leukocytes is an autologous membrane. Through this feature, it is used to repair soft tissue defects. In this case, a 56-year-old female patient with pyoderma gangrenosum who did not see any benefit from systemic therapies is presented for having significant improvement on her ulcer after thrombocyte-rich fibrin application. The case is presented in order to emphasize that the platelet-rich fibrin therapy is easy to be prepared and administered and also it is autologous and inexpensive at the same time. Background: Behçet's disease is a chronic systemic vasculitis. The prevalence and clinical findings differ among various ethnic and regional groups. Our study aims to present the data of Behçet's disease, which is endemic in our country, in Adıyaman province of Southeastern Anatolia. Materials and Methods: The clinical and demographic characteristics of the patients were retrospectively evaluated according to the criteria of the International Study Group (1990) and followed in the rheumatology clinic. Results: Our study included a total of 150 patients, including 90 (60%) males, and 60 (40%) females. The mean age was 37.76 ± 9.09 years. Clinical findings included oral aphthae (100%); genital ulcers, 82 (59.4%); eye involvement, 47 (35.6%); skin findings, 34 (26%); and vascular involvement, 16 (12.3%). Although genital ulcers were more common in women and eye involvement and skin findings were more common in men, there was no statistically significant difference (p = 0.543, p = 0.233, and p = 0.066, respectively). Vascular involvement was significantly higher in men (p = 0.048). Conclusion: The most common finding in our study is mucocutaneous lesions. In our country's data, it was observed that Behçet's disease was more severe in young men. In our study, the fact that vascular involvement was significantly higher in men might explain the poor prognosis in men. As a result, our resuts suggest that Behçet's disease in the Southeast has similar clinical features as those reported from other regions in our country. [ABSTRACT FROM AUTHOR]

Hiatal hernias are very common and result in gastroesophageal reflux when they become symptomatic. Sliding type hiatal hernias are frequently observed; however, paraesophageal or giant hernias characterized by herniation of the fundus or whole stomach inside the chest are seen in only 1-5% of all patients. In those patients, ulcers, bleeding and obstructions can be seen. Resistance to medical treatment of gastroesophageal reflux or very large esophageal hiatus is an indication for surgical repair in hiatal hernias. Symptomatic paraesophageal hernias should be operated on as well. In the treatment of both conditions, laparoscopic fundoplication and hernia reduction are applied. [ABSTRACT FROM AUTHOR]

Orbital infections are seen most commonly in childhood. They require urgent treatment because of worse outcomes such as sepsis, meningitis, loss of vision and death. Although these infections are usually seen as complication of trauma and paranasal sinusitis, nasolacrimal agenesis is the most common etiology in the newborns. Herein a case of intraorbital abscess occuring at the background of dacriocystocele in a 10 day-old-female newborn is reported. The culture of abscess material revealed S. aureus and the patient was successfully treated with efficient antibiotic treatment and abscess drainage. [ABSTRACT FROM AUTHOR]

This report was described generalized amyloidosis in a male gazelle, 1.5 years old. In necropsy, hyperaemia on all the serosal surfaces in abdomen, a hemorrhagic ulcer, 7 cm in diameter, on mucosa of rumen, and whitish colored foci; 1-2 mm diameter, on the surfaces of both kidneys werepresent. Microscopically, there were homogenous pink depositions in the glomeruli, intertubular interstitium, tubular basement membranes and walls of blood vessel in kidney, lymphoid follicles in spleen, space of Disse in liver and lamina propria of the small intestine. These deposits were seen brick-red color stained with Congo red on tissue sections, but these were not stained with Congo red after treated with potassium permanganate. For this reason, AA (secondary) amyloidosis was evaluated. When stock tissue samples stored in 10% formalin were treated with Lugol and acid sulphuric, amyloid depositions were seen as dark blue. Generalized reactive amyloidosis with ulceration in the rumen was diagnosed. Objectives: CagA positive H. pylori strains are considered to be more virulent than other strains. In this study, we aimed to investigate the rate of CagA positivity in duodenal ulcer (DU) and functional dyspepsia (FD), and its effect on H. pylori eradication response. Materials and methods: The study was performed on H. pylori positive 60 patients with DU and 50 patients with FD, who underwent upper gastrointestinal endoscopy. H. pylori infection was identified by histology. All patients received a quadriple therapy consisted of esomeprazole 20 mg b.i.d., colloidal bismuth subcitrate 600 mg b.i.d., tetracycline 500 mg q.i.d. and metronidazole 500 mg t.i.d. for 7 days. H.pylori status was rechecked using C14-urea breath test 6 weeks after the end of treatment to confirm cure. Specific IgG antibodies for CagA status were determined by enzyme linked immunosorbent assay. Results: CagA positivities in the patients with DU and FD were calculated respectivily 70% and 68% (P>0.05). H. pylori was eradicated in 85.5% of the patients infected with CagA (+) strains, in 50% of those infected with CagA (-) strains (P=0.001). The eradication rates were 95.2% and 55.6% in CagA positive and negative DU subgroups (P=0.001), and 73.5% and 43.8% in CagA positive and negative FD subgroups (P=0.04). Conclusion: CagA positivities were not different in duodenal ulcer and functional dyspepsia. CagA (+) strains was susceptible to the eradication treatment. The titres of serum anti-CagA antibodies may be used in the prediction of eradication outcome, and the modification of eradication therapy. [ABSTRACT FROM AUTHOR]

Background and Aims: Acute upper gastrointestinal bleeding (UGB) is one of the most important causes of mortality and morbidity in hospitalized patients. In this study we aimed to retrospectively analyze the gastrointestinal endoscopy results in patients that refer to our clinic with UGB. Materials and Methods: Patients with UGB admitted to our clinic were retrospectively evaluated for location of bleeding and the laboratory findings. Results: One hundred-ten patients with UGB; of the patients, 42.7% were female, 57.2% were male with average age 60.4± 17.3 (ranged between 18 and 85). Upper endoscopy performed in 96% of patients with in the first 24 hours of bleeding. The most frequently observed endoscopic lesions were erosive gastritis (33.5%) and duodenal ulcus (29.0%). Endoscopic treatment performed in 9 of the patients (in 3 patients argon plasma coagulation, in 5 patient scleroteraphy with 1/10000 adrenalin solution and in 1 patient varices band ligation). One patient death with abondan bleeding. Conclusions: Erosive gastritis has been found to be the most frequent cause of UGB. The mortality rate of 0.9% was very low. The diagnosis of Behcet's disease is made on the basis of the criteria proposed by the International Study group for Behcet's disease in 1990. According to the criteria, recurrent oral ulceration must be present as well as at least two of the following: Recurrent genital ulceration, eye lesions, skin lesions and a positive pathergy test. Acute/chronic oral ulcers and genital ulcers should be regarded in the differential diagnosis of Behcet's disease. Behcet disease is multisystemic disorder which is characterized by a vasculitis. Gastrointestinal involvement is seen in 0-5% of the patients with Behcet's disease. The most common places for the involvement are terminal ileum and cecum. However, it may be seen anywhere of the gastrointestinal system from mouth to anus. The lesions are seen as a deep or localized ulcer. Behcet Disease may radiologically imitate inflammatory bowel disease or neoplastic disorders. Recurrent oral aphthous ulcerations, genital ulcerations and cutaneous manifestations including erythema nodosum like lesions, superficial thrombophlebitis and papulopustular lesions are the most common manifestations of Behcet's disease. The pathergy reaction reflects skin hypersensitivity to trauma in Behcet's disease. Pathergy test which is characterized by the formation of a sterile pustule 24-48 h after intradermal needle prick has a high diagnostic value especially in Middle East and Mediterranean countries. Due to their high sensitivity, mucocutaneous lesions and pathergy test constitute four out of the five criteria in the International Behcet's Disease study group classification criteria. Behcet's disease has a wide clinical spectrum and since there are no specific diagnostic laboratory tests, diagnosis depends on clinical acumen of the physician. In the majority of patients with systemic findings, mucocutaneous lesions precede the onset of systemic involvement and therefore prompt recognition of mucocutaneous manifestations is of utmost importance in the early diagnosis and the outcome of the disease. Autoimmune progesteron dermatitis (AIPD) is a rare disorder which is characterized by cyclical premenstrual flares of cutaneous or mucocutaneous manifestations. We present a 36-year-old woman with a 2-year history of oral ulcers and a rash which presents a week before every menstruation. She also had a history of recurrent herpes labialis infection; however herpes simplex virus could not be detected by PCR in oral lesions. Routine laboratory investigations, as well as serum complement levels were normal, except for elevated ANA titer (1/160). Histopathological examination of one of the papules was consistent with erythema multiforme and immunobullous diseases were ruled out by negative immunofluorescence studies. Based on the history and the clinical features, AIPD was suspected and the diagnosis was confirmed by flare of the lesions after progesterone challenge test. Subsequently, oral tamoxifen was started which controlled her flares significantly and no major side effects except amenorrhea was observed during treatment. Next she was given spironolactone which did not control her symptoms, therefore oral prednisolone had to be introduced. Due to its rare occurence, clinical features, pathogenesis, diagnostic and treatment alternatives of AIPD are discussed in this study based on the review of the medical literature. Aim: In this study, we aimed to compare the clinical and demographic features of pediatric Behçet's patients with those of adult age. Materials and Method: We examined the clinical and demographic features of the 18 and under 18-year-old children who were followed-up in the Behçet's Center of Uludağ University Medical Faculty between 1992 and 2007, retrospectively. The information of age, sex, presence of oral aphthous ulcers, genital ulcers, erythema nodosum, papulopustular eruption, pathergy test positivity, ocular, articular, neurological, gastrointestinal, renal involvements and HLA-B5 positivity were obtained from the medical charts of the patients. Results: There were 30 pediatric Behçet diagnosed according to the diagnostic criteria of International Study Group for Behçet's Disease. The mean age of the patients was 15.9. At the time of the diagnosis, most of the patients were aged between 17-18 (43.3%). There was only one patient under 10. Oral aphthae was observed in all, and genital ulceration or the scar of the ulcer was observed in 23 patients; 15 females, 8 males (76.6%). Ocular involvement was present in 18 patients; 8 females, 10 males (60%); erythema nodosum in 13 patients; 8 females, 5 males (43.3%), papulopustular eruption in 16 patients; 10 females, 6 males and pathergy test positivity in 12 patients; 7 females, 5 males (36.6%). Vascular symptoms were observed in 2 patients, arthralgia-arthritis in 15 patients and neurological involvement in 4 patients. Conclusion: In this study, it was concluded that ocular involvement was more frequent in Behçet's patients aged between 0-18 than adult Behçet's patients, but mucocutaneous findings had similar frequency. Objective: The aim of this retrospective study is to investigate the safety and efficacy of endovascular repair of aortic lesions with the Medtronic ®Talent stent-graft system and to present mid-term results of endovascular aortic repair performed in our center. Methods: Between December 2002 and March 2007, 54 patients (6 women) with aortic (14 thoracic and 40 abdominal) lesions underwent treatment with ®Talent stent-graft. The average age of the patients was 64.8 (20-88) years. Duration of follow-up period ranged from 1 to 49 months (average 21 months). Indications for endovascular repair were degenerative aneurysm in 45, degenerative aneurysm and penetrating ulcer in 2, only penetrating ulcer in 1, traumatic thoracic isthmic transsection in 4 and vasculitic aneurysm secondary to Behcet's disease in 2 patients. Results: Repair was performed with the tubular (16), aortouniiliac (1) or bifurcated (37) stent-grafts. Technical success rate was 100%. No death, major complication or need of immediate conversion to open repair was seen. Endoleak rate was 18.5% at 1 month follow-up period. Thirty-day mortality was 1.8% and morbidity (other than endoleaks) rate was 12.9%. During the follow-up period, secondary intervention was required in 12.9% of patients. Iliac limb occlusion was detected in 1 patient (1.8%). Graft migration causing type 1 endoleak and requiring open surgical treatment was seen in 1 patient (1.8%). Four patients (7.4%) are still under follow-up for type-2 endoleaks that do not require intervention. No graft infection or death due to aneurysm rupture was detected. Conclusion: Endovascular treatment of aortic lesions in selected patients with comorbid conditions using the ®Talent stent-graft exhibits a high degree of technical success with a low perioperative morbidity and mortality rate. Objective: Reynoud phenomenon (RP) and digital ulcers (DUs) negatively affect hand functions and quality of life in patients with systemic sclerosis (SSc). The aim of this study was to investigate the efficacy of ozone therapy in SSc patients with DUs resistant to medical therapy. Methods: The participants' demographic data, medical treatment, organ involvement, and clinical parameters (duration and number of RP attacks, ulcer pain and diameter) were recorded. Twentyfive SSc patients with DU were randomized to the ozone group (I) (n=13) treated with medical treatment plus oxygen-ozone therapy and the control group (II) (n=12) treated with medical treatment only. The efficacy of the treatment was evaluated using the scale. Hand functions were assessed with the Health Assessment Questionnaire (HAQ) and Modified Hand Mobility in Scleroderma (HAMISm) test. Ulcer grade, clinical parameters, HAQ, mHAMIS scores were re-evaluated in all participants at the end of the treatment. Results: There was no statistically significant difference between the ozone and control groups in terms of clinical and demographic data. At the end of the month, the effective healing rate was significantly higher in group (I) than in group (II), where it represented 92% in ozone group versus 42% in control group (p=0.010). Morever, ulcer size, ulcer pain, duration and number of RP attacks, HAQ, and HAMISm scores were significantly improved in the treatment group compared to the control group (p<0.05). Conclusion: Local ozone therapy is effective in the treatment of resistant DUs in SSc patients and improves clinical parameters and functional disability. [ABSTRACT FROM AUTHOR]

Objective: Helicobacter pylori is implicated in gastric and duodenal carcinoma, gastric and duodenal ulcers, atrophic gastritis, dyspepsia, gastric adenocarcinoma and gastric lymphoma. Eradication of Helicobacter pylori has a major role in the treatment of the mentioned diseases. Methods: Two thousand and sixty nine gastro endoscopic examinations performed in the İstanbul Research and Education Hospital Endoscopy Unit during July-October 2010 were retrospectively analyzed. During each examination, specimens were obtained and sent to the Pathology department. Each specimen was dyed using the Modified Giemsa technique. The status of Helicobacter pylori was made as -(negative), +(positive first level), ++ (positive second level), +++ (positive third level). Results: Out of 2069 gastroendoscopic examinations performed in the İstanbul Research and Education Hospital Endoscopy Unit during July- October 2010, 301 were reported by the performing endoscopist to be totally normal. In 175 of these, Helicobacter pylori was at least + (58%), whereas in 126 patients’ biopsy results Helicobacter pylori was - (42%). Conclusion: In this study, in patients with normal gastroendoscopic findings, Helicobacter pylori is present in at least + in 58% of patients. Considering the wide range of diseases caused by this bacteria, in every endoscopic examination at least one specimen should be obtained. [ABSTRACT FROM AUTHOR]

Behçet's disease (BD) is a systemic disease characterized by oral aphthosis, genital ulcers, ocular lesions and also gastrointestinal, musculoskeletal, neurological and vessel involvements may develop. Venous manifestations are more common than arterial involvement. Venous involvement often occurs in the veins of lower extremity. Upper extremity venous involvement is rare. Eosinophilic ulcer of the oral mucosa is an uncommon, benign and self-limiting disease. Although exact pathogenesis of the disease is unknown, trauma is thought to play a role in the etiology. Eosinophilic ulcer is characterised by an ulcer with an indurated border, commonly occurring on the tongue but also on any region of the lips and oral mucosa. The ulcerated lesion with acute onset may heal spontaneously within 1 week to 8 months. A 23 year-old-man man was admitted for painful ulceration on the lower lip for seven years which şares up during summers and heals spontaneously. Clinical and histopathological findings of the case, consistent with eosinophilic ulcer, is presented because of its rare occurrence as well as clinical resemblance to squamous cell carcinoma of the lower lip.
