Introduction: Sepsis is a syndrome of physiologic, biochemical and pathologic abnormalities induced by infection and has been associated with high mortality and morbidity. This study aimed to compare Systemic Inflammatory Response Syndrome (SIRS) criteria, Pitt bacteremia score and quick Sequential Organ Failure Assessment (qSOFA) for the prediction of mortality in patients with sepsis due to Gram-negative bacteremia. Materials and Methods: Patients with sepsis due to Gram-negative bacteremia admitted to emergency medicine clinic were observed retrospectively from March 2016 to March 2018. Clinical data, laboratory results, co-morbidities, antimicrobial treatment, Pitt bacteremia score, qSOFA score, SIRS criteria of patients were evaluated. Results: A total of 106 patients with sepsis due to gram-negative bacteremia according to Sepsis-2 definition were included. Thirty-day mortality was 20.8\%. Multivariate analysis determined Pitt bacteraemia score (OR:1.63, 95\% CI 1.29-2.05, p< 0.001) as predictors of mortality among septic patients. The area under the ROC curve was 0.885 (95\% CI 0.737-0.933) for Pitt bacteremia score 0.808 (95\% CI 0.713-0.904) for qSOFA and 0.623 (95\% CI 0.492-0.753) for SIRS. Pitt bacteremia score showed the highest specificity (\%71.4) and positive predictive value (\%40.0) as compared to other scores. Conclusion: As the result of the analyses, the mortality rate in patients with sepsis due to gram-negative bacteremia was better predicted with the Pitt bacteremia score than qSOFA score and SIRS criteria.

Amaç: Bu çalışmanın amacı, sistemik yangısal cevap sendromlu yenidoğan buzağılarda serum demir seviyesi ve hematolojik parametrelerdeki değişimlerin ortaya konulmasıdır. Gereç ve Yöntem: Bu araştırmada hayvan materyali olarak, sistemik yangısal cevap sendromlu 19 buzağı ile kontrol amaçlı 8 sağlıklı buzağı kullanıldı. Tüm buzağıların rutin klinik muayeneleri yapıldı. ÖZET SIRS, sepsis, ağır sepsis, septik şok ve MODS yoğun bakım hekimlerinin sık karşılaştıkları ayırıcı tanıda zorlandıkları bunun yanında tanının acilen konulup tedavinin başlanmasının hastanın mortalite morbiditesinin önemli derecede etkilediği bir hastalık gurubudur. Kasım 2002-Ocak 2004 tarihleri arasında DÜTF Çocuk Sağlığı ve Hastalıkları Kliniği 6 yataklı Yoğun bakım ünitesinde takip edilen 77'si kız, 80'i erkek olmak üzere 157 hasta çalışmaya alındı. Bu hastaların en küçüğü 1 en büyüğü 182 aylık olduğu tespit edildi. Bu hastalar SIRS,sepsis,ağır sepsis ve septik şok kriterleri için ACCP/SCCM (American College of Chest Physicians/Society of critical Care Medicine) Consensus Conference Commitee tanımlamaları temel alındı. Çalışmada hastaların en sık 25-48 aylık olduğu, en sık kış mevsiminde başvurdukları, sepsisin en sık nedeninin respiratuar kaynaklı olduğu saptandı. Ayrıca yoğun bakım ünitemize kabulde % 42.6'sında MODS saptandığı, MODS ile mortalite arasında belirgin bir ilişki olduğu, yoğun bakıma yatırılıp ölen hastalarda % 95 oranında MODS olduğu, MODS gelişen ve ölen hastalara bakılığında ölen hastalarda en sık iki ve üç organ yetersizliği geliştiği tespit edildi. Sepsisten ağır sepsis ve septik şoka, ağır sepsisten septik şoka ilerleyen hastalarda mortalitenin çok yüksek oranlarda olduğu ve çalışma sonucunda mortalitenin kızlarda daha fazla olduğu, kızlarda en sık mortalitenin (% 23.3) ağır sepsiste, erkeklerde ise en sık mortalitenin (% 23.3) septik şokta gerçekleştiği tespit edildi. Multipl organ disfonksiyonun çocuklarda erken geliştiğini ve sepsisin mortaliteyi artırdığı görüldü. Bunların erkenden tanınıp tedavi edilmesi prognoz açısından büyük önem taşıyabileceği ve yüksek olan mortalite oranlarını düşürebileceği kanısına varıldı. 53 SUMMARY SIRS,Sepsis,severe sepsis,septic shock and MODS are a group of diseases that is frequently seen and hardly diagnosed by physicians working in intensive care units and also in which immediate diagnosis and initializing treatment may significantly affect the morbidity and the mortality. A total of 157 children (77 female, 80 male) aged between 1 to 182 month admitted to a 6 bedded Intensive Care Unit of DÜTF Pediatrics Clinic between November 2002 and January 2004 were studied. In this study patients were evaluated as SIRS, sepsis, severe sepsis, septic shock, based on descriptions of ACCP/SCCM (American College of Chest Physicians/Society of Critical Care Medicine) Consensus Conference Committee. In our study we determined that most patients were between 25-48 months, commonly admitted in winter and respiratory infections were the most frequent reason for sepsis. It is also determined that 42,6% of patients at admission to our PICU were MODS; there was a significant association between MODS and mortality and that MODS was the most frequent reason in our PICU deaths (95%). In these patients lost because of MODS we mostly determined 2 or 3 organ failure. It is determined that mortality is higher when sepsis ameliorates to severe sepsis and septic shock; the mortality is higher in females, and most frequent mortality reason is severe sepsis in females(23.3%) and septic shock in males (23.3%). We determined that MODS develops earlier and sepsis increases mortality in children. It is concluded that in these cases early diagnosis and treatment may be important for prognosis and may decrease these high mortality rates. 54 66
