2. Relationship between positive mental health and quality of work life in nurses

INTRODUCTION: This study was conducted to evaluate positive mental health and quality of work life levels of nurses working in hospitals in terms of personal and work-related factors and to examine the relationship between positive mental health and quality of work life. METHODS: This descriptive and correlational study was conducted with 247 nurses working in three public hospitals. The data were collected using an introductory information form, the Positive Mental Health Scale and the Quality of Nursing Work Life Survey. Statistical analyses were performed using a software package (Kruskal–Wallis H test, Independent Sample-t test, ANOVA test, Spearman Correlation Analysis). RESULTS: The mean total Positive Mental Health Scale and the Quality of Nursing Work Life Survey scores were 73.27 +- 14.86 and 103.42 +- 13.01, respectively. It was determined that the work environment, work conditions, and job perception positively affected positive mental health, whereas the increase in relations with executive nurses negatively affected positive mental health and the perception of quality of work life was related to both positive mental health and quality of work life. DISCUSSION AND CONCLUSION: It was determined that nurses perceived their positive mental health at a level close to good and their quality of work life at a moderate level, and both positive mental health perception and quality of work life perception differed according to some variables (age, hospital type, employment type and perception of work life). In addition, it was determined that the perception of quality of work life differed according to having children, the clinic and the length of service in the clinic, and the perception of positive mental health differed according to the length of service. Practices aimed at improving the leadership and management skills of nurse managers can contribute to improving the quality of work life and mental well-being of nurses.

5. 'Süper Çocuk' Karakter ve Hikâyesinin Kanül Uygulanan Çocukların Kaygısı Üzerine Etkisi

Amaç: Bu çalışmada ayaktan tedavi hizmetleri için başvuran, damar yolu açılan ve kan alınan çocukların kaygısını azaltmak için illüstre edilmiş ve bilişsel davranışsal teknikle hazırlanmış "Süper Çocuk" girişiminin etkinliğinin belirlenmesi amaçlandı. Yöntemler: Çalışma bir çocuk hastanesinin ayaktan tedavi hizmetlerine başvuran ve intravenöz kanül uygulanan 5-10 yaş grubu çocuklarla yürütüldü. Kontrol gruplu, ön-test, ara-test ve son-test şekilde tasarlandı. Çalışmaya 72 çocuk dahil edildi (deney grubu = 36, kontrol grubu = 36). Çalışmada çocukların kaygısı Çocuk Anksiyete Skalası-Durumluluk ile değerlendirildi. Girişim olarak bilişsel davranış tekniklerle hazırlanmış, süper kahraman analojisini içeren "Süper Çocuk" kartları ve posterleri kullanıldı. Bulgular: Deney ve kontrol grubundaki çocukların yaş (t = 0,571; p = 0,570) ve cinsiyetleri (X2 = 0,225; p = 0,635) benzerdi. Çocukların %50'sinden fazlasının ağrılı işlemlerle ilgili bilgi almadığı tespit edildi. İntravenöz kateter işlemi sırasında girişim grubundaki çocukların kontrol grubundaki çocuklara kıyasla daha az kaygı yaşadıkları belirlendi (p < 0,05). Sonuç: Bilişsel davranışsal teknikle hazırlanan, çocuğa işlemlerle ilgili bilgiler veren illüstüre "Süper Çocuk" posterlerinin çocuğun prosedürel kaygısını azaltmada etkili olduğu bulundu.

Bu araştırma, sağlık çalışanlarının psikolojik sağlık ile Tanrı algılarının meslek, kıdem, cinsiyet, yaş, medeni durum ve eğitim düzeylerine göre karşılaştırılmasını; psikolojik sağlıkları ile Tanrı algıları arasındaki ilişkinin incelenmesini amaçlamaktadır. Araştırma nicel araştırma yöntemlerinden birisi olan ilişkisel tarama modeli kullanılarak gerçekleştirilmiştir. Çalışma grubu Sincan Dr. Nafiz Körez Devlet Hastanesi'nde görev yapan toplam 198 sağlık çalışanından oluşmaktadır. Katılımcıların %79,29'u (n = 157) kadın, %20,71'i (n= 41) erkek çalışanlardan oluşmaktadır. Araştırmanın verilerini toplamak için Güler (2007) tarafından geliştirilen 'Tanrı Algısı Ölçeği', Türkçeye uyarlanan "Kısa Semptom Ölçeği (KSÖ)" olarak bilinen ölçek ve araştırmacı tarafından hazırlanan kişisel bilgi formu kullanılmıştır. Araştırmanın sonucunda, sağlık çalışanlarının Tanrı algısı, sevgi yönelimli algıların yüksek, korku yönelimli algıların düşük olduğunu göstermektedir. Ayrıca, sağlık çalışanlarının Tanrı algısı düzeyleri, yaş, cinsiyet, medeni durum, kıdem ve eğitim düzeyi gibi faktörlere bakılmaksızın benzerlik görülmektedir. Sağlık çalışanlarının psikolojik sağlık düzeylerinde en düşük olduğu boyut somatizasyon iken en yüksek olduğu boyutlar depresyon ve hostilite olarak tespit edilmiştir. Katılımcıların yaşları, cinsiyetleri, eğitim düzeyleri ve medeni durumları gibi faktörlerin psikolojik sağlık düzeylerini etkilemediği görülmüştür. Ayrıca, katılımcıların sevgi yönelimli Tanrı algısı düzeylerinin artmasıyla anksiyete, depresyon ve olumsuz benlik algısı düzeylerinde hafif bir azalma olduğu tespit edilmiştir.

- Abstract
Objective The aim of the research is to investigate the relationship between perceived hospital value and hospital loyalty in patients applying to a university hospital. Material and Method Quantitative research design and easy sampling method were used in the study. The hospital perceived value scale developed by Moliner (2006), the Turkish validity and reliability study conducted by Akbolat and Durmuş (2021), and the hospital loyalty scale developed by Price and Arnould (1999), the Turkish validity and reliability study conducted by Ünal (2016) were used as data collection tools in the study. The Cronbach Alpha value of the hospital perceived value scale was found to be 0.921 and the Cronbach Alpha value of the hospital loyalty scale was found to be 0.732. The sample of the study consists of patients receiving outpatient outpatient clinic services from a university hospital in Kahramanmaraş. Within the scope of the research, 321 patients were reached. Descriptive statistics, t test in independent groups, analysis of variance and correlation analyses were performed on the data. Results As a result of the findings obtained from the research, hospital value perceptions of married individuals were found to be higher than single individuals. The hospital value perceptions of individuals with chronic illness and continuous medication use were found to be higher than those without chronic illness and continuous medication use. Hospital value perceptions of illiterate participants were found to be higher than primary school, high school and undergraduate graduates. It was concluded that monetary costs are important in the perceived value of the hospital. The effect of non-monetary costs on perceived value was found to be low. It has been determined that the dimensions of functional value and perceived value of the hospital organization have a significant positive effect on hospital loyalty. Conclusion As a result of the results obtained from the study, it was concluded that hospital loyalty increases as the perceived hospital value increases. [ABSTRACT FROM AUTHOR]

The study aims to evaluate the emergency department performance of a university hospital within the scope of Health Quality Standards indicators. Descriptive and cross-sectional research methods are used to design the study. The population of the study, which is followed up in a university hospital between January 1st, 2021, and December 31st, 2021, within the scope of the Ministry of Health emergency service indicators, include the rate of re-admissions to the emergency department, the average time of reaching the emergency department by the consultant physician, the average observation time in the emergency department, and the rate of patient referral. No sample is selected for the study, and the entire universe is evaluated. The tool used for collecting data is the emergency department indicator card. In the analysis of the data, the calculation methods specified in the indicator cards by the Ministry of Health are used. According to the study, the rate of revisits to the emergency department with the same complaint is 0.002%, and the performance target of 1% is reached. It is observed that the average observation time in the emergency department is 93 minutes, and the target of 720 minutes is reached. It is found that the time for the consultant physician to reach the emergency department is 32 minutes in 2021, and the target for this indicator is exceeded by 30 minutes. It is found that the patient referral rate (0.001%) is well below the target (1%). As a result of the study, the target is reached for the rate of revisits to the emergency department, the average hospitalization time in the emergency department, and the patient referral rate, but the target for the consultant physician's time to reach the emergency department could not be reached, and in this regard there is a need for an improvement. Aim: This research was conducted to determine the relationship between the work alienation level and quality perceptions of nurses working in the hospital. Method: The population of this descriptive, correlational, and cross-sectional study consisted of 300 nurses working in a training and research hospital in a province of Turkey, and the sample consisted of 134 nurses who volunteered to participate in the study. Results: The nurses' mean score from the work alienation scale was 66.6 (SD=13.6), and their mean score from the quality perception scale was 257.7 (SD=50.6). In this study, it was aimed to examine the relationship between the managerial performance and decisionmaking competence of healthcare managers working in hospitals in Trabzon and to investigate the effect of socio-demographic characteristics on managerial performance and decision-making competence. The "Managerial Skills Scale" developed by Sperry (2003) and adapted into Turkish by Şimşek (2006) was used to collect data in the study. As a result of the research, managers who are graduates of Health Management and Faculty of Economics and Administrative Sciences and who work in hospitals with a bed capacity of 100 or more were also found to have higher problem-solving and decisionmaking competencies. It has been determined that increases and decreases in the level of managerial performance significantly and highly affect problem-solving and decision-making competence. In addition, high levels of conflict management and compliance with corporate and human resources in managers are important variables in increasing decisionmaking competence (p < 0.05). This research was carried out to determine the effect of perceived organizational justice and diversity management on human resource efficiency among health personnel. for this purpose, organizational justice, diversity management and human resources productivity scales were used in the study. The research type is descriptive-relation-seeking type of research. The study was carried out by reaching 437 people out of 2215 employees working actively in 3 public and 1 private hospitals located in a city center. In the analysis of the research, the SPSS package program and the AMOS application were used to verify the scale factors. In line with the research data, a high and positive relationship between organizational justice and human resource efficiency; It was concluded that there is a moderate and positive relationship between diversity management and human resources productivity. As a result of the regression analysis between the variables, it was determined that perceived organizational justice and diversity management positively affected human resources productivity. This study was designed within the framework of concepts that have gained importance at the organizational level in recent years and aims to examine the effect of ethical leadership perceptions of healthcare professionals on the level of psychological ownership. The study is a cross-sectional study designed with a causal screening model. The sample of the study consists of 292 health workers. The data collection tool includes the "Ethical Leadership Scale" and the "Psychological Ownership Scale" and questions about the demographic characteristics of the participants. Analysis of the data was carried out using the SPSS 22 program. Descriptive analysis, factor analysis, reliability analysis, correlation analysis, and regression analysis were used in the statistical analysis. As a result of the analysis, the participants' ethical leadership scale average score was 2.695; among the subdimensions of psychological ownership, the average of work-based ownership is 4.666, and the average of emotional ownership is 3.991. According to the results of the correlation analysis, a positive and significant relationship (r=0.674, p<0.01; r=0.427, p<0.01) was found between ethical leadership, emotional ownership, and work-based ownership. As a result of the regression analysis, it was determined that the level of ethical leadership explained 45.4% of the total variance for the emotional ownership attitude of the participants and 18.2% of the total variance for the work-based ownership attitude. As the perception of healthcare professionals that their managers display ethical leadership behaviors increases, their levels of emotional ownership and work-based ownership also increase. In line with the findings of the study, it can be said that if the administrators exhibit ethical behavior, health workers will embrace both their work and their organizations more. This study was carried out to investigate the effect of occupational health hands a fety training on the knowledge level of hospital staff. Pre-test and post-test quasi-experimental designs were used in the study. The sample of the study consists of 702 staff working in Hatay Mustafa Kemal University Health Application and Research Hospital. Before the training, the sample group was performed a test about occupational health and safety, which consisted of 20 multiple-choice questions, each correct answer was 5 points, and the wrong answer didn't eliminate the correct answer. After 15 days following the end of the training, the same questions were given to the participants, and they were asked to answer again. As a result of the statistical assay, the average score of the participants before the education was 60.48±13.55, while this score increased to 79.34±11.89 after the education. The score obtained by 37 of the participants before the training is higher than the score obtained after the training. The score received by 629 participants after the training was found to be higher than before the training. The scores of the 36 participants before and after the training are equal. A statistically significant difference was found between pretraining and post-training (p <0.05). It can be said that the training has a positive effect on the knowledge level of the participants. - Abstract
The aim of this study is to examine the effect of service experience on negative patient behaviors and the mediating role of corporate reputation on this effect. The research data were obtained from the adult individuals using health services in Turkey in the last year, by using the online questionnaire, using the easy probabilistic sampling method. The questionnaire form consists of four parts. The first part consists of three statements (age, gender, hospital type) containing the demographic characteristics of the participants. In the second part, negative behavioral responses scale developed by Zeithaml et al. was used. In the third part, the service experience scale developed by Boakye et al. was used. In the last part, the corporate reputation scale developed by Torres et al. was used. According to the research findings, service experience affects the behaviors of complaining (β=-0.147), negative word of mouth (β=-0.233) and switching (β=-0.165), but has no effect on interia. In addition, perceived corporate reputation has a mediating role in the effect of service experience on patient behavior. As a result, when patients experience a negative experience, they may complain, change or verbally disparage the health institution. In addition, healthcare providers can prevent negative patient behaviors if they strengthen their reputations. Amaç: Bu çalışmanın amacı Türkiye Cumhuriyeti Merkez Bankası (TCMB) verileri ile özel sağlık hizmeti sunan işletmelerin 2009-2020 yıllarına ait finansal durumunu ve performansını oran analizi yöntemiyle analiz etmektir.Yöntem: Çalışmada TCMB web sitesinden elde edilen 2009-2020 yıllarına ait Q-861 Hastane Hizmetleri Alt Sektörünün bilançoları ve gelir tabloları kullanılmıştır. Hastane hizmetleri alt sektörü retrospektif olarak 2009-2020 yılları için bilanço ve gelir tablolarından elde edilen veriler Microsoft Office Excel programı ile oran analizine tabi tutulmuş ve sağlık sektörü için literatürde genel kabul görmüş oranlar doğrultusunda yorumlanmıştır.Bulgular: Özel sağlık hizmeti sunan işletmelerin likidite oranları olması gereken değerlerden düşük olup 2009-2010 yıllarında ideal seviyededir. Finansal yapı oranları da olması gereken değerin altında hesaplanmış olup borçlanma oranının yıllar itibariyle giderek yükseldiği görülmüştür. Varlıkları kullanım oranları bakımından ise stok devir hızı ve alacak devir hızından yüksek bulunmuştur. Diğer bir ifadeyle stokların elden çıkması ile alacakların tahsilinden hızlı olmaktadır. Karlılık oranlarından net kar marjı, öz sermaye karlılık oranı ve varlık karlılık oranları 2011, 2016 ve 2018 yıllarında negatif değer alarak zarar görülmektedir.Sonuç: Sağlık hizmetleri sunumunda yüksek teknoloji ürünü ve oldukça yüksek maliyetli cihazlar ve malzemeler kullanılmaktadır. Bu cihazların ve malzemelerin yerli üretiminin maliyetleri ve dışa bağımlılığı azaltacağı dolayısıyla finansal durumu olumlu yönde etkileyeceği düşünülmektedir. Sağlık hizmetleri finansmanında Sosyal Güvenlik Kurumu (SGK) önemli yer tutmaktadır. Bu durumun özel sağlık işletmelerinin alacakların tahsil süresini, satış fiyatını ve karlılığını etkilediği sonucuna varılmıştır.

40. Investigation of Occupational Accidents in Hospitals: An Example of a Training and Research Hospital

INTRODUCTION: It is the examination of the causes, frequency, and consequences of occupational accidents in hospitals. METHODS: In this descriptive study, conducted in a single center, all occupational accidents in a hospital in 2019 were examined. Data on the causes of occupational accidents, occurrence time, the injury caused by the accident, and the labor loss were shown in frequency tables. The differences between the groups according to gender and education level for labor loss due to the accidents were analyzed with Yates Chi-square and Kruskal–Wallis Analysis. SPSS v.22 statistics program was used in the analysis of the data. RESULTS: A total of 131 occupational accidents occurred in the hospital and the annual occupational accident rate was 9%. The mean age of the employees exposed to the accident was 33.3+-9.41 years, the majority was women (58%) and nurses/midwives/laboratory workers (29.8%), and their working experience was between 1 and 5 years (42.7%). In terms of education level, most accidents were seen in undergraduate education (24.4%). Most of the accidents occurred during day shifts (71%), in inpatient clinics (27.5%), and the most common was with sharp-piercing injuries (35.9%). The labor loss after the accidents was 19.8% and 98 days in total. The labor loss was higher for males than females and for employees with associate degree education compared to others. DISCUSSION AND CONCLUSION: Occupational accidents not only cause loss of workforce, but also negatively affect the health of employees. Identification of high-risk personnel will ensure that the protective approaches to prevent accidents will be more effective.

44. Adaptation of the Intrahospital Transport Safety Scale into Turkish

Objective: The aim of this study is to adapt the Intrahospital Transfer Safety Scale into Turkish. Methods: It is a methodological study. One hundred twenty-nine surgical intensive care nurses participated in the study between May and June 2021. Data were collected online using the Sociodemographic and Occupational Characteristics Form and the Intrahospital Transfer Safety Scale. After the language validity was ensured, the data were analyzed with the Content Validity Index, Split Half Method, Cronbach alpha coefficient, Item Total Score Correlation, and Explanatory Factor Analysis values. Results: The mean age of the surgical intensive care nurses is 28.8+-6.8 years, 78.3% of them are undergraduate graduates and 42.6% of them work in the Anesthesiology and Reanimation Intensive Care Unit. The Content Validity Index of the scale was found to be 0.95. The Cronbach alpha coefficient of the scale was 0.93 and it was determined that it had a high degree of reliability. In the explanatory factor analysis, four factors with an eigenvalue exceeding one were obtained and the total variance explained by these four factors was found to be 63.59%. Factors were determined as organization, tools and technologies, environment and team collaboration. Cronbach's alpha values were found to be between 0.73 and 0.92. Conclusion: The Intrahospital Transfer Safety Scale was found to be a valid and reliable tool.

45. Changes in imaging patterns of geriatric trauma patients in emergency department

Purpose: We aimed to analyze imaging tests and findings in a series of geriatric trauma patients admitted to emergency department (ED) at different time intervals. Materials and Methods: Two groups of 300 randomly selected patients over consecutive five-year periods were compared. Patients admitted in the first and second five-year periods were recorded as group I and II, respectively. A comprehensive comparison was carried out between two groups regarding the age, sex, reason for admission, comorbidities, Revised Trauma Score (RTS), Glasgow Coma Scale (GCS), radiological findings and number of radiological examinations, dose length product (DLP) values and duration of hospital stay. Results: The number of patients who underwent X-ray and CT examinations and total number of X-rays and CT scans in group II was higher than those in group I. CT scans were negative for a trauma-related finding in 49% of patients in group I and 55% of patients in group II. In patients with radiological evidence of trauma, no significant difference was observed between two groups regarding the major trauma related change. However, the trauma-related minor findings on CT were more common in group II than in group I. Conclusion: Despite the increasing use of imaging tests, there was no difference in imaging findings suggesting major trauma or requiring a longer inpatient stay. This may be related to the increasing use of radiological examinations over the years as a result of the orientation towards defensive medicine.

Aim: This study aims to determine the income and expense items of the health care provided for Epidermolysis Bullosa patients in the home health services unit of a public hospital in Kayseri. Method: The data of this study were provided as a result of interviews with the hospital's corporate information management system, trustee, relevant warehouse, purchasing, billing, and home health services unit. It is to determine the expense of the health service by activity-based cost analysis method. Results: As part of the study, it was found that costs were very high, especially labor costs, and as a result of the related services, income was quite low, despite these costs. As part of the health service, it was found that the prices of the Health Implementation Communiqué did not cover expenses, and the expenses were significantly excessive. Conclusion: In this direction, the costs of the relevant public hospital are high compared to their income, and multifaceted proposals were made to reduce expenses and increase their income.
