CANBOLAT, A. CyberKnife® Robotik Kollu Lineer Hızlandırıcı Cihazında kVGörüntü Alma Sıklığının Tüm Vücudun Aldığı Radyasyon Dozuna Etkisi,Hacettepe Üniversitesi Sağlık Bilimleri Enstitüsü Radyoterapi Fiziği Programı,Yüksek Lisans Tezi, Ankara, 2012. Bu çalışmada CyberKnife® cihazına aitgörüntüleme sistemlerinden gelen doz katkısı incelendi. X-ışını tüplerinin dört farklıenerji ve pozlama (mAs) parametreleri için görüntü başına dozlar ölçüldü. Bu dozlarAlderson Rando fantomun yüzeyine TLD ve Gafkromik film yerleştirilerek ölçüldü vebirbirleriyle karşılaştırıldı. Ölçüm sistemlerinin duyarlılığını arttırmak için her ölçüm40 ve 80 görüntü alınarak tekrarlandı ve tek bir görüntü dozu elde edildi. Görüntüsayısındaki iki katlık artışa karşı ölçüm sistemlerinin duyarlılığı incelendi. 90 kVenerji ile alınan TLD ölçümlerinde görüntü başına dozlar 10 ve 90 mAs için sırasıyla0.12 ve 0.82 mGy' dir. 120 kV enerji ile alınan TLD ölçümlerinde görüntü başınadozlar 10 ve 90 mAs için sırasıyla 0.41 ve 2.68 mGy' dir. 90 kV enerji ile alınanGafkromik film ölçümlerinde görüntü başına dozlar 10 ve 90 mAs için sırasıyla 0.17ve 0.84 mGy' dir. 120 kV enerji ile alınan Gafkromik film ölçümlerinde görüntü başınadozlar 10 ve 90mAs için sırasıyla 0.46 ve 2.73 mGy' dir. Her iki ölçüm sisteminde deenerji arttıkça görüntü başına doz değerleri artmaktadır. Görüntü sayısı iki katarttığında elde edilen görüntü başına doz değerlerinde anlamlı fark yoktur. Kullanılanölçüm sistemleri düşük dozlara duyarlı cevap vermektedir. Gafkromik filmin uzaysalçözünürlülüğü daha iyi olması ve doku eşdeğeri olmasından dolayı TLDölçümlerinden daha yüksek değerler elde edilmiştir. Bu fark düşük enerjilerde dahabelirgindir. Ölçümlerde mAs değerinin üç kat artması görüntü başına doz değerlerindeortalama 2.45 (2.54-2.32) katlık bir artışa sebep olmaktadır. Sonuç olarak, X-ışınıgörüntülemesinden kaynaklı yüzey dozu, kV ve mAs parametrelerine bağlıdır. Gerekligörüntü sayısı hastalık bölgesinin yeri ve CKS tedavi seçeneklerine göre, yani nefes(Syncrony Solunum Takibi), omur kemikleri (Xsight Omurga Takibi), ve kranialkemiklerin (6D-Skull Takibi) takibi ile değişir. Bölümümüzde, hasta için kullanılankümülatif gerçek zamanlı görüntü sayısı genellikle 40 ile 150 aralığında değişmektedir.Bu durum görüntüleme işlemi tarafından oluşan ek yüzey dozunun dikkate alınmasıgerektiğini gösterir. Bu nedenle, görüntü kalitesinden ödün vermeden optimum düşüktüp parametreleri kullanarak, tedavinin kalitesinden ödün vermeden az sayıda görüntükullanılması düşüncesindeyiz. CANBOLAT, A. The Effect of Kilovoltage (kV) Imaging Frequency on Total BodyIntegral Dose at CyberKnife® Robotic Arm Linear Accelerator Machine,Radiotherapy Physics Program of Oncology Institute at Hacettepe University,Thesis in Radiotherapy Physics Program, Ankara, 2012. In this study, dosecontribution result from the imaging systems for CyberKnife® were analyzed. Doseper image values were measured for four different energy and exposure (mAs)parameters. These doses were measured and compared with each other by placing TLDand Gafkromik film on the Alderson Rando phantom surface. To increase the precisionof measuring systems, each measurement was repeated two times on 40 and 80 imagesets, and a single image dose was obtained. The sensitivity of measurement systemswere evaluated against two-fold increase in the number of the images. Doses per imagefor TLD measurements taken by 90 kV energy are 0.12 and 0.82 mGy, respectively, for10 and 90 mAs. For 120 kV energy , these values are 0.41 and 2.68 mGy, respectively,for 10 and 90 mAs. Doses per image for Gafchromic film measurements taken by 90kV energy are 0.17 and 0.84 mGy, respectively, for 10 and 90 mAs. For 120 kV energy, these values are 0.46 and 2.73 mGy, respectively, for 10 and 90 mAs. The dose perimage values, for both measurement systems, were increasing in case of the energyincrease. When the energy was increased, the dose per image values were gettingcloser to each other. No significant change in dose per image even if the number ofimages were doubled. Measurement systems responded more sensitively in low doses.The higher values were obtained from measurements of the Gafchromic film becauseof the spatial resolution and the tissue equivalent was better than the TLD. Thisdifference was more pronounced in low energies. The dose per image for each energyincreased with the factor of 2.45 (2.54-2.32) while shifting the mAs parameter with thefactor of three. As a result, the surface dose of X-ray imaging depends on theparameter couple kV and mAs in CKS. The number of required image for each casevaried with the location of a disease and CKS treatment options, i.e. breath (SyncronyRespiratory Tracking), vertebra bones (Xsight Spine Tracking), and cranial bones (6DSkullTracking) tracking. For our department, cumulative real-time images used forpatient has changed in the range of 40 and 150. This situation revealed the requirementof consideration of additional surface dose exposed by imaging process. Therefore,while we choose the optimal lowest tube parameters without compromising from thequality of imaging, also we use minimum number of images without compromisingfrom the quality of treatment. 92

In this study it is aimed to design and develop an instrumental device which is capable of measuring some key parameters of X-ray tubes employed in many medical imaging devices including conventional X-ray imaging, fluoroscopy, mammography and dental X-ray imaging and those measured parameters which is critical for both imaging quality and patient safety aspects are acquired and evaluated extremely precise and very reliable with the help of latest technological advancements. Furthermore the designed instrumental device is supplemented with a Bluetooth connection capability which allows both remote controlling of the device and data transmission between the device and PC. Also a graphical user interface which is capable of recording multiple exposure data acquired via Bluetooth module and plotting comparison graphs in demand of user is also programmed in .NET environment. Remote controlling of the device and data transmission via Bluetooth avoided possible risks of working close to high voltage transformer of X-ray tube, hence gives an extra protection to medical device technicians. Taken measurements can be used to make periodic calibrations of medical imaging devices and this also helps reducing possible risks may be caused by overdosing during imaging.
