Background: In this study, it was planned to examine the clinical findings that may be caused by thyroid nodules of 6 cm and above, which are rarely seen, and the rates of postoperative complications (hematoma, cord paralysis, permanent hypocalcemia, tracheostomy) of these nodules. Materials and Methods: The study was conducted on 23 patients with nodules of 6 cm and above among all patients who underwent total thyroidectomy between 2020-2021. Demographic information of the patients was recorded and computed tomography was performed on all patients. The patients were followed up in the short and long term in terms of complications in the postoperative period, and all complications were recorded. Results: Twenty-three patients with a thyroid nodule size of 6 cm or more were analyzed retrospectively. In 9 (39.1%) of 23 patients, signs of airway obstruction were observed in neck CT (Computerized tomography). It was determined that the thyroid gland showed substernal extension in 2 (8.6%) patients. While no permanent complications were observed in any of the patients, temporary complications were observed in 5 patients (21.7%). Conclusions: Thyroid nodules of 6 cm and above are less common in the thyroid gland. In patients with thyroid nodules of 6 cm or more, thyroidectomy is essential for reasons such as airway obstruction, risk of malignancy, and aesthetic concerns. Increasing the size of the thyroid gland increases the risk of complications after surgery, but this rate can be minimized with appropriate surgical technique and close follow-up. Objective: Our aim was to analyze the clinical and surgical aspects of the patients with substernal goiter. Materials and Methods: A total of 34 patients who underwent surgical treatment with the diagnosis of substernal goiter at Atatürk Chest Diseases and Chest Surgery Center between January 1996 and July 2005 were evaluated retrospectively. The patients were analyzed with respect to age, gender, presenting symptoms, findings, surgical procedures utilized, postoperative histopathology and follow-up. Results: Of the 34 patients, 19 were female and 15 were male with an age average of 57.5 years (range; 25-71). The most common symptom was related with respiratory system (dyspne and cough). A total of 28 patients (82.3%) were managed via standard "Collar" incision whereas the remain was required a total of 6 additional surgical procedures (partial/complete median sternotomy, thoracotomy). Of the 34 patients, 29 (85.2%) were diagnosed as diffuse/nodular colloidal goiter postoperatively; malignancy was detected in 2 (5.8%) of the patients. There was no operative mortality and our morbidity rate was 17.6% Conclusion: The management of choice in patients with substernal goiter is to be surgical. Although servical incision is adequate in most of the patients, abnormal thyroid tissue can be resected via additional partial or complete mediansternotomy when indicated in patients with intrathoracic localisation with low morbidity and mortality rates. Introduction: Substernal thyroid goiter is defined as extension of the thyroid gland into the mediastinum. Vocal cord paralysis, vena cava superior syndrome due to the compression on major vessels, and Horner syndrome can be rarely seen in patients. Case Report: A 77-year-old female patient presented to our emergency department with complaints of dyspnea, flushing, and edema on her face. Her vital signs were as follows: BP: 140/90 mm Hg, HR: 135 bpm, respiratory rate: 28 per minute, and SpO2: 88%. On her physical examination, there was increased jugular venous distension, both thyroid lobes were palpable, and breath sounds decreased in the right lung. A contrast-enhanced pulmonary computed tomography (CT) was planned, and substernal goiter was seen on CT. Then, 4 L/min supplemental O2, 2 puffs of 200 mcg budenoside 3 times a day and 250 mg IV methylprednisolone therapy were started. Conclusion: In patients presenting with dyspnea, retrosternal (substernal) goiter should also be considered. [ABSTRACT FROM AUTHOR]
