In this study we aim to evaluate the effect of prophylactic 'Granulocyte colony-stimulating factor' (G-CSF) administration on ventilation associated pneumonia (VAP) developing. In comparison of group I (n:35) receiving G-CSF and the control group II (n:35) there were no statistical differences between demographic values, APACHE II scores, feeding procedures, erythrocyte and platelet counts (p > 0.05). The leucocyte and neutrophil counts were increased significantly in group I at 24. hours and 2., 3., 4. days (p < 0.001). There was no important difference between the serum levels of SGOT, SGPT, LDH, BUN and creatinin of two groups (p > 0.05). VAP was determinate in 17 (48.5%) patients in group I and 30 (85.7%) patients in group II. (p < 0.05). During the 7-day period, the numbers of VAP patient were significantly higher in group II in every individual evaluation after 3. days on mechanical ventilation (p < 0.05 and p < 0.001). In conclusion we decided that to enhance the anti inflammatory response with prophylactic C-CSF (0.5 MU/kg) administration is an effective alternative for limiting VAP in intensive care patients with prolonged ventilatory support.

In this study we aim to determine the safety and efficacy of patient controlled analgesia (PCA) in pediatric patients for postoperative analgesia and compare with conventional IM meperidine administration. 20 pediatric patients (ASA I-II), aged 8-14 years who were undergoing urologic surgery were divided in two groups. Group I received PCA (Abbott Life care infuser Pumper) with intravenous fentanyl a loading dose of 3 µg/kg and bolus doses of 1 µg/kg and lockout interval of 30 minutes. Group II received meperidine 1 mg/kg IM with 6 hour periods. Vital signs; heart rate, mean arterial pressure (MAP), respiratory rate, periferal arterial oxygen saturation; Visual pain scores (VAS) and comfort scores (6-Points scores) were recorded with 2 hours interval for the first 12 hours. Side effects; nausea, vomiting, itching, dry mouth, respiratory depression, hypotension were evaluated. The comparison of the demographic and vital values were not significant between two groups (p>0.05). In this study we investigated the potential mutagenic effects of isoflurane in clinical concentrations. The study was performed on 20 patients who underwent different types of surgical intervention. Anaesthesia was induced with thiopentone and maintained with 1 % isoflurane. Vecuronium was used for muscular relaxation. The anaesthesia period was limited to 60 minutes. Venous blood samples were taken before anaesthetic inhalation, at the 15th and 60th minutes of anaesthesia, ten days after anaesthesia. The values of blood just before anaesthetic inhalation were used as the control. The venous samples were cultured within 72 hours and mutagenicity tests, mitotic index, sister chromatid exchange and replication index, were used for examination of the exchange of chromosomes and cell cycle. The values of mitotic index and replication index were significantly depressed (p0.05). In conclusion, we decided that short-term anaesthesia administration with isoflurane in clinical concentration has reversible mutagenic effect.

Group I received a loading dose of propofol of 1-2 mg/kg and intermittant 0.5 mg/kg IV bolus doses as necessary. Group II received ketamine 1-2 mg/kg initially and 1 mg/kg intermittantly during MRI. In both group vital signs; HR, RR, SaO2 and sedation scores were obtained every 15 minutes during MRI. Duration to arousal, recovery and oral feeding were determined in both groups. Adverse effects were evaluated during procedure. There were no significant differences in demographic values and sedation scores between two groups (p > 0.05). In this study we evaluated the effects of bolus and continuous infusion of propofol as a sedative for MRI in adult patients. 20 adult patients (ASA I-II) undergoing MRI were randomly divided to two groups. Initial dose of propofol (1-2 mg/kg IV bolus) was followed by maintenance continuous infusion (1 mg/kg/hour) in group I. Group Il received intermittent bolus doses (1 mg/kg) following the same initial dose when required. Vital signs; HR, RR, SaO2, sedation scores were recorded in every 15 minutes during MRI.
