3. A Pilot Study: The Effect of COVID-19 on Sonographic Optic Nerve Sheath Diameter Measured for Critical Patient Management

Objective: The aim of this study is to evaluate the effects of the Coronavirus disease 2019 on sonographic optic nerve sheath diameter measurement and thus avoid possible misleading results in clinical practice. Material and Method: Each volunteer was first evaluated using carotid system color Doppler ultrasonography. Patients with a history of PCR-confirmed Coronavirus disease 2019 infection were classified as group 1 and patients without a history of Coronavirus disease 2019 infection were classified as group 2, and sonographic optic nerve sheath diameter values of both groups were analyzed. Results: Of the 123 patients included in the study, 70 (56.9%) were female and 58 (43.1%) were male. 83 (67.5%) of the patients included in the study were in group 1 and 40 (32.5%) were in group 2. The mean sonographic optic nerve sheath diameter values for the groups were 3.53 mm and 3.46 mm, respectively. The sonographic optic nerve sheath diameter differences between the two eyes for the groups were determined to be 0.203±0.139 mm and 0.282±0.2 mm. Conclusion: Due to the variable effects of Severe Acute Respiratory Syndrome Coronavirus-2, the use of sonographic optic nerve sheath diameter measurement in current standards for critical patient management may lead to false-positive or false-negative results.

5. OPTIC NERVE AND RETINAL LAYER MEASUREMENTS WITH OPTICAL COHERENCE TOMOGRAPHY IN PCR POSITIVE AND NEGATIVE COVID-19 PATIENTS

Objective and Aim Covid-19 targets all tissue and organ systems, not just the lungs. The optic nerve and retina with extensive microvascular nutrition are prone to viral involvement. Optical coherence tomography is a technology that provides detailed information about both optic nerve and retinal structure. The study was carried out to investigate possible changes in the optic nerve and retinal structure of patients with Covid-19 infection, dividing PCR positivity or negativity. Methods Thirty PCR positive Covid-19 patients with different ages and varying admission complaints were included in the study. Twenty-five Covid-19 patients who were PCR negative with similar age and gender were selected as a secondary group for comparison. All patients underwent ophthalmologic examination, including slit-lamp biomicroscopy, funduscopy, and OCT. These examinations were performed four weeks after the diagnosis of Covid-19 for full compliance with the mandatory isolation. In addition, RNFL, retinal thickness, and retinal volume measurements were performed. Results No statistical significance was observed in any parameter between the PCR positive or negative patients when the comparative analysis for both eyes in RFNL measurements. There was a significant difference in retinal thickness measurements between the PCR positive and negative groups regarding left eye central retinal thickness (p=0.047). However, there was no statistical difference in retinal volume measurements. Conclusion Retinal imaging with optical coherence tomography is a non-invasive, reproducible, and rapid technique in which subclinical or overt retinal pathologies can be detected during COVID-19. Therefore, management of COVID-19 patients should include retinal assessment with close follow-up, especially in patients with headaches and optic pain.

Background: We aimed to compare optic nerve elasticity and central retinal artery resistive index values in Nonproliferative diabetic retinopathy and Proliferative diabetic retinopathy. Materials and Methods: The degree of optic nerve hardness of 31 patients with nonproliferative diabetic retinopathy and proliferative diabetic retinopathy was evaluated using the Shear-Wave Elastography technique. Simultaneously, the resistive index values of the central retinal artery were measured using Color Doppler Ultrasonography and compared with the control group. Results: Optic nerve Shear-Wave Elastography values were significantly higher in patients with retinopathy compared to healthy volunteers and diabetic patients without retinopathy(p<0.05). Central retinal artery resistive index values were significantly higher in patients with retinopathy than those without retinopathy (p <0.05). RI values were measured as 0.57 ± 0.88 for those without retinopathy and as 0.67 ± 0.53 for those with retinopathy. Conclusions: The etiopathogenesis diabetic retinopathy is still controversial. Significant changes in the degree of stiffness of the optic nerve and ocular blood flow parameters in Color Doppler Ultrasonography examination can be observed in Nonproliferative diabetic retinopathy and Proliferative diabetic retinopathy with Shear Wave Elastography. 14. Optic Nerve Sheath Diameter Measurement in Children Presenting to a Pediatric Emergency Department with Head Trauma

Introduction:Optic nerve sheath diameter (ONSD) measurement is a non-invasive and easy imaging technique for the estimation of intracranial pressure in children and adults. We aimed to measure the ONSD in pediatric patients with head trauma.Methods:In this study, 40 patients with head trauma and age- and sex-matched 40 healthy children were included. Nerve sheath diameter was measured by transorbital sonography.Results:Diameter of the mean optic nerve sheath of the right and left eye of the patients were 0.408±0.064 mm and 0.417±0.065 mm, respectively; both results were higher in patients than in controls. In the right eyes of the patients, the ONSD values had a negative correlation with Glasgow Coma Scale scores. There was a positive correlation between systolic and diastolic blood pressures and optic nerve sheath diameters of patients. Clinical status of the patients at admission and blood pH levels were associated with ONSD.Conclusion:The result of our study indicated that ONSD measurement is a non-invasive technique and can be used easily to estimate increased intracranial pressure in pediatric emergency departments.

Objective In this radioanatomical study, we aimed to obtain data on the morphometric properties of the optic strut as it is an important reference point for optic nerve surgery. Material and Methods Seven adult human dry skulls, present in the inventory of Faculty of Medicine Department of Anatomy were included in this study. Direct anatomical measurements were obtained with a digital caliper and radiological measurements with computed tomography. The position of the optic strut was evaluated according to the preciasmatic sulcus. Results The length and width of the optic strut were 4.53 ± 0.74 mm and 4.83 ± 1.34 mm in direct anatomic measurements. On computed tomography, the length and width of the optic strut were 4.13 ± 1.27 mm and 4.31 ± 0.82 mm. The distance between the two struts was 16.83 ± 2.56 mm and 15.91 ± 1.81 mm in direct anatomic measurements and computed tomography, respectively. Computed tomography and direct anatomic evaluation revealed that the optic strut was sulcal in 4 dry skulls, postsulcal in 2 dry skulls, and asymmetric in 1 dry skull. Conclusion Considering the fact that optical strut is a landmark for surgeons and may be damaged during anterior clinoid process resection, our numerical data may be useful during surgical applications around the optic nerve. Ophthalmologic examination of a ten-year-old girl, who presented with erythema in both eyes for three days revealed conjunctival hyperemia, small petechial hemorrhages and valve edema, was diagnosed with adenoviral conjunctivitis. Congenital optic disc drusen was detected in fundus examination. Ten days later, OCT scans showed increased optic disc nerve fiber layer thickness and disc edema. Parainfection optic neuritis was considered in the patient, who had no decrease in visual acuity. In the follow-up, edema developed in the optic disc resolved spontaneously without systemic treatment. Currently Terson’s Syndrome(TS) is used to describe all types of bleeding seen in the posterior segment associated with intracranial hemorrhage and intracranial pressure elevation. During intracranial hemorrhage rising intracranial pressure is thought to be transmitted through the optic nerve sheath, resulting in venous hypertension in the optic nerve, rupture of the retinal veins and bleeding. In this study we present a case of TS who applied initially to our clinic. A seventy-seven-years-old male patient was admitted to our clinic with decreased vision in both eyes and complaints of headache. Best corrected visual acuity(BCVA) of the patient was found to be 0.2 and 0.3 in the right and left eye respectively. In fundus examination there was papilledema, intraretinal hemorrhages around the optic disc and intravitreal. The patient, who was thought to be TS in the differential diagnosis, stated that he had a head trauma 2 months ago. The patient underwent emergency cranial computerized tomography and was diagnosed with subdural hematoma which he underwent surgery for. During the third month of follow-up, increased BCVA and decreased intraocular hemorrhage were noted. This case shows us vital importance of differential diagnosis of TS in any case of intraocular hemorrhage. [ABSTRACT FROM AUTHOR]

Optic nerve avulsion is a rare pathology with poor prognosis usually seen after blunt trauma. The optic nerve is separated from the sclera by indirect forces due to the relatively weak structure of the lamina cribrosa area. Here we describe an 11-year-old boy who experienced optic nerve avulsion and retinal detachment after penetrating ocular trauma.

19. İntravitreal ranibizumab enjeksiyonunun optik sinir üzerine etkisinin patern VEP testi ile değerlendirilmesi.

Aim: To evaluate the effect of intravitreal injection of ranibizumab, an anti-VEGF agent commonly used for the treatment of choroidal neovascularization, on optic nerve by pattern VEP test. Material and Method: Pattern VEP test was applied to 20 eyes of 20 patients who received 0.5 mg of ranibizumab injection because of age related choroidal neovascularization maculopathy and also to other non-injected eyes of the same patients in 1st and 6th weeks. Results are compared with 10 control patients of same age distribution. Results: Mean age was 73.45 in 20 age related maculopathy patients ranging between 64 and 82. Mean best corrected visual acuity of eyes receiving injection was 9/100 before injection and 14/100 finally on 6th week. Mean intraocular pressure was 13.85mmHg in eyes receiving before the injection, 16.75mmHg at 1st week and finally 14mmHg at 6th week. There was no statistically significant difference between the VEP wavelengths and latencies of the patient group and the control group (p> 0.05). There was no statistically significant difference between VEP wavelengths and latencies in 1st and 6th weeks between injection receiving eyes and the other eyes (p> 0.05). Conclusion: As a result of VEP evaluation it is concluded that 0.5 mg intravitreal ranibizumab injection does not have an indirect toxic effect to the optic nerve. [ABSTRACT FROM AUTHOR]

Purpose: To investigate the usefulness of macular volume analysis calculated by optical coherence tomography (OCT) for glaucoma diagnosis and follow-up. Materials and method: Fifty glaucoma patients who were examined in ophthalmology department of Ondokuz Mayıs University and 50 healthy volunteers were included in the study. Glaucomatous eyes were divided into two groups based on visual field test as early stage glaucoma (35 patients) and advanced stage glaucoma (15 patients). The measurements of optic nerve head (ONH), retinal nerve fiber layer (RNFL) thickness and macular volume were performed by Stratus-OCT. The relationship between the macular volume and the presence and stage of glaucoma was investigated. ROC analysis was performed to compare the sensitivity of macular volume, RNFLthickness, and C/D ratio in the diagnosis of glaucoma. Results: Mean macular volume was 6.59±0.44mm3 in early stage glaucoma, 6.05±0.38mm3 in advanced stage glaucoma and 6.98±0.34mm3 in control group. In the comparison of early and advanced glaucoma groups with each other and with the control group, significant differences were found in all macular volume parameters except for foveal volume, all RSLT parameters, and all OSB parameters except for disc area (P<0.05). AUROC values were calculated as 0.786, 0.814 and 0.912 for the macular volume, RSLT, and C/D ratio, respectively. Conclusions: Macular volume analysis with OCT provides objective and quantitative data for evaluation of glaucomatous structural defects. The use of macular measurements in conjunction with OSB and RSLT parameters will be an appropriate approach for the diagnosis and follow-up of glaucoma. [ABSTRACT FROM AUTHOR]

Sheehan sendromu postpartum kanama ve hipovolemiye sekonder olarak ortaya çıkan hipofiz nekrozuna bağlı parsiyel veya tam hipofiz yetmezliğidir. Klinik bulgular laktasyonun kesilmesi, sekonder amenore, libido kaybı ve hipofiz rezerv kaybı ile ilişkili bulgular ile karakterizedir. Tedavide hormon replasmanı kullanılır. 44 yaşındaki kadın sürekli uyuma isteği, halsizlik, vücutta şişlik ve görme bulanıklığı yakınması ile kliniğe kabul edildi. Fizik muayenede hipotansiyon ve ödem saptandı. Özgeçmişinde 11 yıl önce doğum yaptığı, aşırı kanamasının olduğu, kan transfüzyonu yapıldığı ve sonrasında bebeğini emziremediği bildirildi. Hali hazırda amenoresi olduğu belirlendi. Sheehan sendromu olarak değerlendirilen olguya glukokortikoid ve levotiroksin replasmanı başlandı. Hipofiz magnetic rezonans görüntülemede empty sella, orbital magnetik rezonans görüntülemede ise bilateral optik sinir kılıflarında ödem saptandı. Empty sella subaraknoid mesafede basınç artışına yol açarak optik sinir kılıfında ödeme neden olabilir. Bu olgu nedeniyle klinik pratiğimizde ender görülen bu birlikteliğe dikkat çekilmek istenmiştir.

Sheehan syndrome is partial or complete hypophyseal insufficiency due to hypophyseal necrosis secondary to postpartum hemorrhage and hypovolemia. Clinical findings are cessation of lactation, secondary amenorrhea, loss of libido and hypophyseal reserve loss. Treatment is hormone replacement. Forty-four years old women admitted with somnolence, malaise, edema and blurred vision. Physical examination revealed edema and hypotension. She had massive bleeding during her birth 11 years old ago; therefore she had received blood transfusion and could not breast-feed then. She had amenorrhea when she admitted. She was diagnosed to be Sheehan syndrome while glucocorticoid and levothyroxine replacement was started. Hypophyseal and orbital magnetic resonance imaging revealed empty sella and edema in bilateral optical neural sheaths respectively. Empty sella may cause edema in optical neural sheaths by increasing pressure in subarachnoidal space. We present this case to point out this coincidence. [Cukurova Med J 2013; 38(2.000): 299-304]

Pur po se: To investigate the clinical and laboratory features of patients with traumatic optic disk pallor. Ma te ri al and Met hod: We retrospectively reviewed the records of 45 patients (47 eyes), who were diagnosed with traumatic optic disc pallor in our clinic between 2005-2010. Best-corrected visual acuity (BCVA), biomicroscopic and fundoscopic findings were recorded. Analysis of retinal nerve fiber layer (RNFL) and macular thickness maps, computerized visual field tests and visual evoked potentials (VEP) were evaluated. We compared the data of pathologic eyes with the data of healthy eyes. Re sults: The mean age was 22.3±3.8 (20-37) years. Twenty (44.4%) patients had traffic accident history, 9 (20%) - object trauma, 8 (17.8%) experienced work accident, and 8 (17.8%) had home accident history. BCVA was at the level of 20/40 (0.5) and above in seven eyes, between 20/200 (0.1) and 20/50 (0.4) in 20 eyes, counting fingers in 15 eyes, hand motion and light perception in three eyes, and no light perception in two eyes. Peripapillary staphyloma (PPS) is a non-hereditary congenital optic disc (OD) anomaly. Herein, we aimed to describe a case of PPS and discuss the clinical, orbital magnetic resonance imaging (MRI) and ultrasonography (USG) findings. A one-year-old healthy girl was referred to our hospital for absence of fixation in her left eye. In addition to ophthalmological examinations, MRI and ocular USG were also performed. Sciascopic examination revealed -2.0 diopters in her right and -8.0 diopters in her left eye. Slit-lamp examination showed normal findings in both eyes. At fundus examination, peripapillary staphyloma was detected in the left eye, while the right eye showed normal findings. MRI and USG also supported the diagnosis. As a result, MRI and USG findings can support the clinical observations in the diagnosis of PPS, which should be kept in mind especially in the differential diagnosis of unilateral absence of fixation in children. Objectives: Evaluation of structural alterations of the optic nerve head (ONH) and the retinal nerve fiber layer (RNFL) in patients with ocular hypertension (OHT) and early-stage glaucoma and assessment of the discriminatory diagnostic performance of spectral-domain optical coherence tomography (SD-OCT) Copernicus (Optopol Technology S.A.). Materials and Methods: This study included 59 eyes of a total of 59 patients, 29 of whom were diagnosed with OHT (Group 1) and 30 with early-stage glaucoma (Group 2). The differentiation of early-stage glaucoma and OHT was carried out on the basis of standard achromatic visual field test results. Analysis of the ONH and RNFL thickness of all cases was made using SD-OCT. Group 1 and Group 2 were compared with respect to the ONH parameters and RNFL thickness. The diagnostic sensitivity of the OCT parameters was evaluated by the area under the receiver operating characteristics curves (AUC). Results: The average, superior, inferior, and nasal RNFL thicknesses in early-stage glaucoma cases were approximately 10% (12-14 μm) less compared to the OHT eyes, with differences being highly significant (p⩽0.001). However, there was no statistically significant difference in the temporal RNFL thicknesses. The most sensitive parameter in the early diagnosis of glaucoma was average RNFL thickness corresponding to AUC: 0.852, followed by AUC: 0.816 and AUC: 0.773 values in superior and inferior RNFL thickness, respectively. In localized RNFL defects, the highest sensitivity corresponded to superior and superonasal quadrants (ACU: 0.805 and ACU: 0.781, respectively). There were not any statistically significant differences between the ONH morphological parameters of the two groups. Conclusion: RNFL analysis obtained using SD-OCT Copernicus is able to discriminate early-stage glaucoma eyes from those with OHT. Cerebrolysinin Sıçanlarda Sisplatinle İndüklenen Oksidatif Optik Sinir Hasarına EtkisiGiriş ve amaç: Sisplatin erişkinlerde ve çocuklarda birçok malign hastalığın tedavisinde kullanılan platin türevi genis spektrumlu antikanser bir ilaçtır. Sisplatinin hastalarda antikanser etki oluşturan dozları, sağlıklı doku ve organlar üzerinde toksik etki oluşturmaktadır. Sisplatin ilişkili önemli toksik etkilerden birinin optik nöropati olduğu ifade edilmektedir ve tedavinin sonlandırılmasına neden olmaktadır. Literetürlerde, sisplatin tedavisi esnasında, antioksidan azalmasına, reaktif oksijen türlerinin (ROS) artmasına ve hücre hasarı ile sonuçlanan lipidperoksidasyonuna (LPO) yol açmaktadır. Literetürlerden edinilen bilgiler, sisplatin ilişkili optik sinir hasarının tedavisinde antioksidan ve nörotrofik ilaçların yararlı olabileceğini düşündürmektedir. Çalışmamızda, sisplatine bağlı oksidatif optik sinir hasarına karşı koruyucu etkisini araştıracağımız cerebrolysin, domuz beyin dokusundan elde edilen düşük molekül ağırlıklı peptidler ve aminoasitlerin bir karışımı olup, potansiyel nöroprotektif ve nörotrofik özelliklere sahiptir. Nöroprotektif ve nörotrofik etkisini antioksidan aktivitesi ile ilişkilendiren çalışmalar bulunmaktadır. Edinilen bu bilgiler cerebrolysininsisplatinle indüklenen oksidatif optik sinir hasarının tedavisinde yararlı olabileceğini ortaya koymaktadır. Literetürlerde, cerebrolysininsisplatin ilişkili oksidatif optik sinir hasarına etkisini araştıran klinik ve preklinik çalışmalara rastlanmadı. Bu nedenle, çalışmamızın amacı, cerebrolysinin sıçanlarda sisplatinle oluşturulan oksidatif optik sinir hasarına etkisini biyokimyasal ve histopatolojik olarak araştırmaktır.Materyal ve metot: Albino Wistar türü sıçanlar deney öncesi sağlıklı (SG), tek başına 2,5 mg/kg sisplatin verilen (CIS) ve 2,5 ml/kg cerebrolysin + 2,5 mg/kg sisplatin (CCG) uygulanan gruplara ayrıldı. CCG (n=10) hayvan grubuna 2,5 ml/kg dozda cerebrolysinin hazır solusyonuintraperitoneal (ip) yoldan enjekte edildi. CIS (n=10) ve SG (n=10) grubuna ise çözücü olarak distile su aynı hacimde ip yoldan sevk edildi. Cerebrolysin ve distile su uygulandıktan bir saat sonra CCG ve CIS hayvan gruplarına sisplatin 2,5 mg/kg dozda ip yoldan enjekte edildi. Sisplatin ve distile su belirtilen doz ve hacimde günde bir defa 14 gün boyunca aynı yöntemle uygulandı. Bu süre sonunda, yüksek doz (50 mg/kg) tiopental anestezisi ile öldürülen hayvanların optik sinir dokuları çıkartıldı. Daha sonra çıkarılan optik sinir dokularında malondialdehid (MDA), myeloperoksidaz (MPO), total glutatyon (tGSH), superoksiddismutaz (SOD), total antioksidan status (TAS) ve total oksidanstatus (TOS) düzeyleri ölçüldü. Objective: In this study we evaluated the optic nerve head (ONH) cupping parameters and thickness of retinal nerve fibre layers (RNFL) with Heidelberg retinal tomography (HRT) and optical coherence tomography (OCT) in patients with the diagnosis of physiological cupping whose intraocular pressure (lOP) and visual fields (VF) are normal but ONH cupping is more than 0.6. We then compared them with early glaucomatous (EG) patients and normal cases as control (NK). Material and Methods: The study enclosed 57 eyes of 57 patients having no previous diagnosis of glaucoma but with physiological cupping C/D≥0.6 on routine ophthalomological evaluation and 56 eyes of 56 early glaucomatous patients followed in our glaucoma unit. Control group (NK) consisted of 55 eyes of 55 cases with C/D≤0.4. All the patients were evaluated with whole ophthalmologic examination, intraocular pressure measurements, with gonioscopy and tests for thickness of central cornea (TCC), OCT, RNFL analyses, HRT, ONH parameters and VF tests. Only one eyes of each patient were randomly chosen. Results: All of the three groups were comparable with regards to TCC and refraction values (p=0.437 and p=0.478). But there was significant differences regarding OCT and HRT evaluations (p<0.001). Disc areas of PC cases were greater than those of other groups (p<0.001). The cupping area of PC cases was similar to those of EG cases (p=0.085) whereas it was significantly greater than those in control group (p<0.O01). There was no difference in Rim area of PC and C groups as was expected (p=0.355) and Rim area of both groups was measured thicker than that of EG cases (p=0.002). It was seen that the most important OCT parameter differentiating PC and EG cases from each other was mean RNFL thickness. When the sensitivity and specificity of OCT and HRT measurements in differentiating PC cases were compared it was seen that OCT was superior. Conclusion: Optic coherence tomography and Heidelberg retinal tomography might be valuable in differentiating between large optic nerve head cupping cases from glaucoma. [ABSTRACT FROM AUTHOR]

Optical aberrations of the eye are the errors of the optical system that limit the resolution, contrast and amount of detail in the image formed on the retina. Wavefront technology allows us to measure these optical aberrations, calculate mathematically, and transfer this information into excimer laser system to perform customized treatment on the cornea. Two treatment algorithms developed to create low aberration-corneal profile are wavefront-optimised (WF-O) and wavefront-guided (WF-G) treatments. WF-O treatment, aims not to increase the existing spherical aberration while treatment is based on manifest refractive error as in conventional laser treatments. By increasing the number of laser spots applied peripherally in order to optimize the corneal asphericity, the preoperative central:peripheral keratometry ratio is preserved and optic zone shrinkage is prevented. On the other hand, WF-G treatment is based on aberrometry measurements and aims to correct the existing high-order aberrations in the eye. Thus, retinal image with high spatial details can be achieved. However, presence of postoperative defocus can abolish the successful results obtained with WF-G treatment. Clinical randomized controlled trials showed that in patients with preoperative RMS value of <0.3 μm, higher order aberration outcomes are similar after WF-G and WF-O treatments, but WF-G treatment yields better results when it is ≥0.4 μm. In normal eyes, very limited visual advantage can be achieved with WF-G treatment and preservation of asphericity value with WF-O treatment carries greater importance. On the other hand, in case of high astigmatism or higher order aberrations other than spherical aberration, decreasing aberrations with WF-G treatment becomes more important. Purpose: To investigate the relationships between the birth parameters such as birth weight, gestational age, and birth type and the parameters of macula, optic nerve and retinal nerve fiber layer (RNFL) in preschoolers. Material and Method: After a broad ophthalmologic examination was performed for every healthy child at age between 3 and 6, the children were scanned by optical coherence tomography (OCT) for the evaluation of macula, optic disc, and RNFL parameters. The relationships between the birth parameters and OCT parameters were analyzed by Spearman correlation test and Mann Whitney U test. Results: One hundred and seventy-two eyes of 111 healthy children were evaluated in the study. The mean age of the children was 57.48±11.87 months (range: 30-80 months). Forty-eight percent of the children were delivered vaginally and the rest of them - by cesarean section. The negative correlations between gestational age and thicknesses of superior inner and temporal inner macula, and temporal and inferior RNFL were statistically significant(r=-0.204, p=0.019; r=-0.203, p=0.020; r=-0.230, p=0.009; r=0.175, p=0.047; respectively). While the parameters that showed negative correlations with birth weight were superior outer macular thickness, temporal, inferior, and average RNFL thicknesses and disc area, the parameters that demonstrated positive correlations with birth weight were cup/disc-area and horizontal cup/disc ratios (r=-0.196,p= 0.025; r=-0.317, p< 0.001; r=-0.269, p= 0.002; r=-0.228, p=0.009; r=-0.189, p=0.032; r=0.181, p=0.04; r=0.197, p= 0.025,respectively). Optic disc area of the children who were delivered vaginally was larger than that of the children who were delivered by cesarean section and this difference between these two groups was statistically significant (p=0.034). Discussion: There were significant effects of the birth parameters such as birth weight and gestational age on some parameters of macula and optic disc obtained by OCT. Purpose: We aimed to compare the analysis of retinal nerve fiber layer (RNLF) thickness of the patient with ocular hypertension (OHT) and open angle glaucoma (OAG) with the analysis in normal population. Material and Methods: The 167 eye of 87 subjects were examined after pupillary dilation with optical coherence tomography (Stratus OCT , model 3000).There were 3 groups; OHT group 1 (n=36), OAG group 2 (n=85), control group, group 3 (n=46). The results of RNLF thickness (mean, superior, inferior, nasal, temporal) were recorded and compared statistically. Results: Central corneal thickness (CCT) was 577.7±24.8 μ in group 1 whereas 557.0±34.6 μ in group 2 and 541.8±32.3 μ; in group 3. The measurement of CCT was significantly thicker in group 1 than that of group 2 and 3 (p<0.05). RNLF thicknesses (mean, superior, inferior, nasal, temporal) were 103,1±9,3, 124.3±15,8, 134.6±14.5, 85.4±19.6, 67.9±11.8 μ; respectively in group 1, whereas 84.9±17.0, 101.5±25.6, 107.8±26.0, 67.8±18.9, 61.1±16,6 μ in group 2 and 98.3±11.3, 117.3±25.6, 127.9±24.1, 77.2±16, 66.2±11.4 μ in group 3. Although there was no statistically significant difference between group 1 and group 3 regarding the mean and all quadrant thicknesses, there was a significant difference amoung groups at all quadrants except temporal quadrant (p<0.001). Conclusion: RNLF thickness analysis with Stratus OCT is an effective and reliable method in detection and evaluation of glaucoma patients. Purpose: To compare of the optic nerve head topographic parameters between patients with rheumatoid arthritis (RA) and an age- and sex-matched control group, by using Heidelberg retinal tomography (HRT) III. Materials and Methods: Optic disc topography analysis was done in 15 eyes of 15 patients with RA and 15 eyes of age- and sex-matched control subjects by HRT III. Differences between the groups were analysed statistically with the Mann-Whitney U test. Results: The mean age of the 15 nonglaucomatous RA patients (6 male, 9 female) was 53.8±9.6 and that of the 15 normal subjects (6 male, 9 female) was 53.7±8.5. No statistically significant differences were found in cup volume (p=0.5813), cup area (p=0.6295), rim area (p=0.6384), rim volume (p=0.8183), disc area (p=0.4764), mean cup to disc ratio (p=0.9632), mean cup depth (p=0.7650), or mean retinal nerve fiber layer (p=0.8530) thickness of eyes between the two groups. Post-traumatic optic nerve transection is a rare condition. It is usually complete and can be accompanied by orbital bone fractures and globe luxation. The management of such cases is quite challenging. In this report, we aimed to present the case of a 49-year-old male patient with orbital trauma, globe luxation, medial rectus muscle and optic nerve transection on the left side. Globe reduction and medial rectus muscle reposition were performed 19 days after the trauma. Five month later, the patient had no light perception in the left eye, however, he had eye movements in all directions of gaze and was satisfied with his left eye aesthetically. Aim: The RNFL thickness value of the patients who had established lacunar infarct on their MRI (Magnetic Resonance Imaging), compared by OCT with healthy volunteers, which is a noninvasive examination and has a high reliability. Material and Methods: The patients who had applied to neurology polyclinic for any reason and established lacunar infarct on their MRI were included in the study. Patients' optic nerve fiber thickness was measured. The control group was formed by 50 randomized healthy volunteers. Results: 20 of patients were female (40%) and 30 of them were male (60%), and the average age was 62.88±1.25 of control group were female (50%) and 25 of them were male (50%), and the average age was 62.64±1.06. When the regional RNFL thickness of the patients and the control group was compared on superior, inferior, nasal and temporal quarters, it was determined that significant differences had demonstrated (p<0.001). Conclusions: When the studies with optic atrophy were examined, the noticing feature was that; these diseases or pathologies had effected the brain diffusely in all. But in our study, we satisfied that in the patients with lacunar infarct whose central nervous system has been effected locally; the RNFL might become thinner on the all face of optic nerve's in OCT. Purpose: To investigate the safety and effectiveness of Trabecular Bypass Micro Stent (GlaukosTM i-stent) in open-angle glaucoma patients via a prospective, controlled study. Materials and Methods: The study was carried out in three groups. In the first group, one or two stents was/were randomly inserted into 23 medically uncontrolled, open-angle glaucoma patients. In the second group, either two stents were applied or latanoprost therapy was started in 21 newly diagnosed open-angle glaucoma patients randomly. In the third group, either phaco+stent or only phaco surgery was performed in 12 cataract patients with openangle glaucoma. Surgery was performed under local anesthesia. An L-shaped stent, preloaded onto a special inserter was put into the anterior chamber through a temporal clear corneal incision with a 20 G MVR knife. The stent was inserted into the Schlemm's canal using a Swan Jacob surgical goniolens. The changes in the number of antiglaucomatous medications and intraocular pressure (IOP) were analyzed. Results: In patients with one stent, the preoperative mean IOP dropped from 20.7±2.4 mmHg with a mean number of 1.7±1.2 drugs to 18.7± 2.5 mmHg with 0.7±1.3 drugs, whereas in patients with two stents, the preoperative mean IOP dropped from 21.1±2.2 mmHg with 2.0±0.9 drugs to 18.7±1.8 mmHg with 0.8±1.0 drugs. The decrease in both IOP and the number of drugs was statistically significant within each subgroup, but the difference between the two subgroups was not significant (one stent versus two stents). Mean baseline IOP in the latanaprost group was 22.9 ±3.3 mmHg and it decreased to 17.6±2.7 mmHg in the last examination. Furthermore, the preoperative mean IOP in patients with two stents was 23.5±1.5 mmHg and it decreased to 17.8±2.9 mmHg in the last examination. The decrease in IOP was not different between the two subgroups (latanaprost versus two stents). In the phaco group, the preoperative mean IOP dropped from 22.9±4.7 mmHg with a mean number of 0.6±0.9 drugs to 16.4±1.8 mmHg with 0.8±1.1 drugs, whereas, in the phaco+stent group, it dropped from 23.1±4.9 mmHg with a mean number of 0.6±0.5 drugs to 14.6±2.8 mmHg without any drug in the last control. The decrease in IOP was statistically significant in both subgroups. The phaco+stent combined procedure allowed discontinuation of antiglaucomatous drugs in all patients. An IOP spike was seen only in one case on the first postoperative day. Minimal anterior chamber hemorrhage was detected in only 4 eyes. Refractive and visual acuity changes were not detected in any patient except in those who underwent phaco surgery. Conclusion: Glaukos i-stent was safe and effective in open-angle glaucoma treatment. Colored Doppler ultrasonography revealed variations in the flow rate and indexes of ophthalmic artery and central retinal artery when compared with the contralateral normal eye, and these variations changed during the follow-up. A 55 year-old presented to our clinic for routine eye examination. Her visual acuity was 0.7 in right eye and 1.0 in left eye. Anterior segment examination and intraocular pressure was normal in both eyes. Dilated fundus examination revealed a vascularizated mass involving temporal half of the optic nerve head and shown exudation in the inferor retinal region adjacent to the optic nerve head. Fundus fluorescein angiograpy demonstrated early hyperfluorescence of lesion and the late pooling of the dye with leakage from the optic nerve capillaries. The retina in the periphery was normal. Central nervous system and other systems were normal. The patient who had been diagnosed with capillary hemangioma of optic nerve head were not given any treatment regimen and was followed regularly. One year after the evaluation of lesion, the visual acuity was 0.9 in right eye at the last follow-up examination and dilated fundus examination showed that there was no variation the size of lesion on optic disc and retinal region with exudation became small. Periphery retina of right eye and left eye were normal. Capillary hemangioma of optic nerve head rarely occurs and can be detected at the routine eye examination. Capillary hemangioma can be treated by alternative methods. But, capillary hemangioma seldomly may regress spontaneously. The association of capiller hemangioma of optic nerve head with various systemic disease should be considered and systemic investigations should be made. [ABSTRACT FROM AUTHOR]

Purpose: To investigate the effects of age, sex, glaucomatous optic nerve change and pre-treatment intraocular pressure level on the ocular hypotensive effect of latanoprost in primary open-angle glaucoma cases. Materials and Methods: Primary open-angle glaucoma cases 99 in number who latanoprost treatment had been begun between January 2003 and January 2005, were included to our retrospective study. The cases were divided into groups according to their ages (40-59 year-old and ≥60 yearold), pre-treatment intraocular pressure (22-26 mmHg and 27-32 mmHg) and cup/disc ratios (0.2-0.4 and 0.5-1.0). The eyes with higher intraocular pressure; in the case of equality, the right eyes were enrolled. Post treatment amount of decrease in intraocular pressure and percentage of decrease and necessity of change the therapy between the groups were compared statistically by non-parametric t-test, chi-square test and Mann-Whitney U test. Results: Percentage of decrease in intraocular pressure was 28.02%(0-53%) at the post-treatment third month and 32.4%(0-56%) at the post-treatment first year. The change or addition of other anti-glaucoma drops were necessary in 20 cases (20.2%) within the first year of treatment. In respect to ocular hypotensive effect of latanoprost no statistically significant differences were detected between the groups according to their ages, sex and cup/disc ratios (P=0.15, P=0.35 P=0.95.) But pre-treatment intraocular pressures were found to have statistically significant effect on the decrease in intraocular pressure after latanoprost therapy (Pª0,001). The necessity of the change or addition of other anti-glaucoma drops were found to be more frequent in male cases and cases with higher cup/disc ratios. Conclusion: It was concluded that ocular hypotensive effect of latanoprost is more in patients with high pre-treatment intraocular pressure; but age, sex and optic nerve changes had no statistically significant effect. AMAÇ: Çalışmamızda İİH'un radyolojik bulguları olarak kabul edilen optik sinir çevresinde sıvı artşı, posterior sklerada düzleşme ve/veya çentikleşme, optik sinir tortiozitesi, Meckel kovuğunda genişlemenin sayısal olarak dökümente edilebilmesi için İİH semptomları olmayan sağlıklı olgularda optik sinir çapı (OSÇ), optik sinir kılıfı çapı (OSKÇ), posterior sklerada düzleşme ve/veya çentikleşme, optik sinir tortiozitesi, Meckel kovuğu genişliği ve bu düzeyde İCA transvers çapı ölçümleri yapılarak sayısal ve orantısal değerler elde olundu.GEREÇ VE YÖNTEM: Çalışmamızda, hastanemiz radyoloji anabilim dalımızda 25.09.2018 tarihinden itibaren geçmişe doğru taranarak, 350 hastanın beyin MRG tetkikleri değerlendirilmeye alındı. Görüntüsü değerlendirilmeye uygun olmayan ve dışlanma kriterlerini barındıran 167 olgu çalışma dışı bırakıldı. Çalışmaya yaşları 18 ile 50 arasında değişen 79 erkek (%43,2), 104 (%56,8) kadın olmak üzere toplam 183 sağlıklı olgu dahil edildi. Olgularda; optik sinir çapı ve optik sinir kılıfı ölçümleri, optik sinir kılıfındaki BOS mesafesinin optik sinire oranı, posterior sklerada düzleşme ve/veya çentiklenme, optik sinir tortiozitesi, Meckel kovuğunda BOS mesafe genişliği ve bu düzeyde İCA çapı sayısal olarak ölçüldü.BULGULAR: Çalışmaya dahil edilen 183 olgunun (79 erkek, 104 kadın) yaş ortalaması 35,30 ±9,64 olarak saptandı. Olgularda OSÇ en düşük ortalama değeri 1,05±0,38 (ort. ± SD, mm), en büyük ortalama değeri ise 2,04±0,49 olarak tespit edildi. OSKÇ en düşük ortalaması 3,39±0,55, en büyük ortalama değeri 5,56±0,55 idi. OSÇ/OSKÇ oranı en düşük ortalama değeri 0,25±0,06, en büyük ortalama değeri 0,40±0,08 olarak ölçüldü. İCA transvers çapı solda 3,51±0,56, sağda 3,38±0,55 olup sol İCA çapı sağa göre daha büyük bulundu. Meckel kovuğu transvers çapı sağda 4,78±0,87, solda 4,81±0,86 idi. İCA/Meckel oranı 0,74±0,17 idi. Hipofiz bezi yüksekliği ortalama değerlerinin yaş gruplarına göre yaş ilerlerdikçe azaldığı ortaya konmuş olup 18-26 yaş aralığında 4,23±1,34, 27-35 yaş aralığında 3,95±1,05, 36-44 yaş aralığında 3,52±0,90, 45-50 yaş aralığında 3,33±1,18 olarak tespit edildi.SONUÇ: Sağlıklı olgularda normotif değerlerin ve oranların tespit edildiği çalışmamızda, elde edilen verilerin, tanısı radyolojik olarak teyit edilmesi gereken İİH olgularında referans oluşturacağı kanaatindeyiz. OBJECTIVE: In our study, healthy patients without any symptoms of IIH, underwent measurement of optic nerve diameter (OND), optic nerve sheath diameter (ONSD), Meckel's cavity width, and transverse diameter of ICA as well as each of them were evaluated for the presence of flattening and / or notching in the posterior sclera and optic nerve tortuosity.MATERIALS AND METHODS: 350 patients' brain MRI images were evaluated retrospectively and 167 patients were excluded from the study whom were not suitable for evaluation or exclusion criteria. A total of 183 healthy subjects 79 men (43.2%) and 104 women (56.8%), aged between 18 and 50, were included in the study. In each case; optic nerve diameter and optic nerve sheath diameter measurements, the ratio of CSF distance to optic nerve in optic nerve sheath, CSF distance in Meckel's cavity and at this level ICA transverse diameter were measured numerically as well as posterior scleral flattening and / or notching, optic nerve tortuosity were evaluated for each patient.RESULTS: The mean age of 183 patients (79 male, 104 female) was 35.30 ± 9.64. The lowest mean value of OND was found to be 1.05 ± 0.38 (mean ± SD, mm) and the highest mean value was 2.04 ± 0.49. The mean average of the ONSD was 3.39 ± 0.55 and the highest mean value was 5.56 ± 0.55. The lowest mean value of the OND / ONSD was found as 0.25 ± 0.06 and the mean value of the maximum was 0.40 ± 0.08. ICA transverse diameter was 3.51 ± 0.56 on the left and 3.38 ± 0.55 on the right and left ICA diameter was found to be larger than right. The transverse diameter of the Meckel's cavity was 4.78 ± 0.87 on the right and 4.81 ± 0.86 on the left. The ICA / Meckel ratio was 0.74 ± 0.17. The mean pituitary gland height values were found to decrease depending on the age. In this context, the value of age range of 18-26 years was found to be 4.23 ± 1.34, 27-35 age range value was found to be 3.95 ± 1.05, 36-44 age range value was found to be 3.52 ± 0.90, 45-50 age range value was found to be 3.33 ± 1.18.CONCLUSION: In our study, we established normative values and ratios in healthy cases, we believe that this data will constitute practical reference values /interval for indeterminate cases of IIH in cases whose diagnosis is needed to be confirmed radiologically. 80

Amaç: FFA'da izole optik disk hiperfloresansı saptanan Behçet hastalarında, optik sinir fonksiyonunda, subklinik tutulumların erken tanısında yardımcı olabilecek değişikliklerin, kontrast sensitivite, VEP ve OCT-RNFL ile değerlendirilerek araştırılması amaçlanmıştır.Gereç ve Yöntem: FFA'da OD hiperfloresansı saptanan hastalar, FFA'sı normal Behçet hastaları ve kontrol grubu olmak üzere 3 grup oluşturuldu. Toplamda 45 hastanın 88 gözü çalışmaya dahil edildi. Gruplar ortalama OCT-RNFL kalınlığı, kontrast sensitivite, ortalama VEP genlik-latansı açısından karşılaştırıldı. Kontrast duyarlılık Pelli-Robson kontrast sensitivite kartıyla değerlendirildi. OCT-RNFL, Zeiss Cirrus HD-OCT Model-4000 ile değerlendirildi ve ortalama retina sinir lifi kalınlığı esas alındı. Amaç: Bu çalışmada, primer açık açılı glokom (PAAG) hastalarında optik sinir başının optik koherens tomografi anjiografi (OKTA) ve retina sinir lifi tabakası (RSLT) analizi sonuçları ile değerlendirilmesi amaçlanmıştır.Gereç ve Yöntem: Bu çalışma Haziran 2015 - Haziran 2018 tarihleri arasında Dicle Üniversitesi Tıp Fakültesi Göz Hastalıkları Polikliniği'nde takipli hasta dosyaları taranarak gerçekleştirildi. Çalışmaya PAAG tanısı almış ve takibi yapılan 35 hasta ve oftalmolojik olarak patolojisi olmayan 35 olgu (kontrol grubu) dahil edildi. Tüm olguların dosyaları incelenerek OKTA ile peripapiller kapiller ve sinir başı vaskuler yoğunluk ölçümleri ve optik koherens tomografi (OKT) ile optik sinir başının RSLT analiz sonuçları kaydedildi. Bulgular: Çalışmaya 35 PAAG tanılı olgunun 62 gözü ve 35 kontrol olgusunun 70 gözü dahil edildi. PAAG grubunda olguların yaşları 7-82 (yıl) aralığında olup yaş ortalaması 40,21±20,10 (yıl) idi. Kontrol grubunda olguların yaşları 15-49 (yıl) aralığında olup yaş ortalaması 35,84±8,51 (yıl) idi. PAAG hastalarında 18 kadın, 17 erkek, kontrol grubunda 15 kadın, 20 erkek bulunmaktaydı. Bu çalışmada gruplar demografik özellikleri bakımından istatistiksel olarak birbirine benzerdi. OKTA ile elde edilmiş vaskuler dansite sonuçları kontrol grubu ve PAAG' li gözler arasında karşılaştırılmıştır. Vaskuler dansite değerleri, PAAG tanılı hastalarda kontrol grubuna göre cihazın ölçümünü aldığı bütün segmentlerde istatistiksel olarak daha düşük bulunmuştur. OKT ile elde edilmiş RSLT kalınlıkları gruplar arasında karşılaştırıldı. RSLT kalınlığı bütün segmentlerde PAAG tanılı hastalarda kontrol grubuna göre istatistiksel olarak daha düşük bulunmuştur. PAAG ve kontrol gruplarındaki olguların görme keskinliği ve göz içi basınç (GİB) değerleri karşılaştırıldı. PAAG grubunda görme keskinliği değerleri kontol grubuna göre istatiksel anlamlı olarak daha düşük bulundu. GİB değerleri karşılaştırıldığında PAAG grubunda ortalama GİB değerleri 16,41±6,74 mmHg, kontrol grubunda 14,55±2,63 mmHg olarak saptandı ancak bu farkın istatiksel olarak anlamlı olmadığı izlendi. OKTA ve RSLT ölçüm sonuçlarının PAAG tanılı hastaları ayırt etmede iyi bir sınır değeri (cut-off) olup olmadığını belirlemek amacıyla Receiver Operator Characteristics Curve (ROC) analizi yapılmıştır. Değerlendirme sonucunda OKTA ölçüm sonuçlarının PAAG'yi öngörmede tanısal değerleri olduğu görülmüştür. Sonuçlar değerlendirildiğinde optik sinir başı (OSB) tümü, peripapiller, temporal ve RSLT temporal superior sonuçlarının daha da iyi bir tanısal değere sahip olduğu görülmüştür.Bu çalışmada PAAG grubunun OKTA ve RSLT ölçümlerinin görme keskinliği değerleri ile ilişkisi incelendi. RSLT inferior temporal ve nasal alan sonuçları ile görme keskinliği arasında anlamlı bir ilişki olmadığı izlenmiştir. OSB inferior nasal ve inferior temporal vaskuler dansite değerlerinin görme keskinliği ile ilişkisi saptanmamıştır. OSB Superior temporal vaskuler dansite değerleri ve görme keskinliği arasında ise istatistiksel anlamlı olsa da zayıf bir ilişki tespit edilmiştir. OSB tüm imaj, disk içi, peripapiller, nasal, superior nasal ve temporal vaskuler dansite ölçümlerinin görme keskinliği değerleri ile arasında istatistiksel olarak anlamlı orta derecede pozitif yönlü bir ilişki saptanmıştır. Buna göre bu çalışmada, OSB tüm imaj, disk içi, peripapiller ve nasal ölçümleri yüksek olan glokomlu hastaların görme keskinlikleri de ilişkili bir şekilde yüksek bulunmuştur.Sonuç: PAAG hastalarının seyrinde ortaya çıkabilecek OSB hasarında OKTA ile vaskuler yoğunluklarının ölçümleri ve OKT ile RSLT kalınlıklarının analiz edilmesi yardımcı tanı yöntemi olarak kullanılabilir.Anahtar Kekimeler: Primer açık açılı glokom, optik koherens tomografi anjiografi, optik koherens tomografi, retina sinir lifi analizi, optik sinir başı Purpose: This study was aimed to evaluate the optic nerve head (NH) by optic coherence tomography angiography (OCTA) and retinal nevre fiber layer (RNFL) thickness analysis in primary open angle glaucoma (POAG) patients.Materials and Methods: This study was performed between June 2015 and June 2018 in Dicle University Medical Faculty Opthalmology Outpatient Clinic. Thirty-five subjects with POAG and 35 healty subjects (control group) were included in the study. Radial peripapillary capillary (RPC) and NH vascular density measurements and RNFL analysis results with optical coherence tomography (OCT) were recorded in all cases. Results: 35 POAG patients' 62 eyes and 35 control subjects' 70 healthy eyes were included in the study. In the POAG group, the ages of the patients were between 7-82 (years) and the mean age was 40,21 ± 20,10 (years). The age of the patients in the control group was between 15-49 (years) and the mean age was 35.84 ± 8.51 (years). In the POAG group,18 patients were female and 17 were male. There were 15 females and 20 males in the control group. In this study, groups were statistically similar in terms of their demographic characteristics. Vascular density results obtained by OCTA were compared between the control group and eyes with POAG. Vascular density values were found to be statistically lower in patients with glaucoma than control group. RNFL thicknesses obtained by OCT were compared between groups. RNFL thicknesses were found to be statistically lower in POAG group compared to the control group in all segments.Visual acuity and intraocular pressure (IOP) values of POAG and control groups were compared. Visual acuity values were significantly lower in patients with POAG than control group. The mean IOP values were found to be 16.41 ± 6.74 mmHg in patients with POAG and 14.55 ± 2.63 mmHg in the control group. This values were compared and the difference was not statistically significant. Operator Characteristics Curve (ROC) analysis was performed in order to determine a good cut-off value for distinguishing between patients with POAG and healthy subjects. It was seen that the OCTA and RNFL measurement results had diagnostic values for predicting POAG. NH whole ımage, NH peripapillary, NH temporal and RNFL temporal superior results had better diagnostic values.In this study, the relationship between POAG patients OCTA, RNFL measurements and visual acuity values was investigated. RNFL inferior temporal and RNFL nasal results were not associated with visual acuity. Accordingly, NH inferior nasal and NH inferior temporal vascular density values were not associated with visual acuity. There was a weak but statistically significant correlation between NH superior temporal vascular density and visual acuity. There was a statistically significant positive relationship between the visual acuity and NH whole image, inside disc, peripapillary, nasal, superior nasal, temporal vascular density measurements. The POAG patients whose NH whole image, peripapillary, nasal and inside disc vascular density measurements were high also had high visual acuities.Conclusion: The use of OCTA imaging to measure vascular density and OCT to measure RNFL thicknesses can be used as an adjunctive diagnostic method in the injury of the optic nerve head that may occur in the course of patients with POAG. Amaç: Bu çalışmada, acil serviste hiperglisemi tespit edilen hastaların tedavi ile normoglisemi sağlandığında optik sinir kılıf çapı (OSKÇ) değişkenliğini oraya koymak amaçlanmıştır.Gereçler ve yöntem: Prospektif, kesitsel, gözlemsel olarak planlanan çalışmamıza, çalışma süresince acil servise başvuran 18 yaş ve üzeri, başvuruda kan glikoz düzeyi 250 mg/dl ve üzeri olan hastalar çalışmaya alınmıştır. Acil serviste hiperglisemi tespit edildiği andan sonra, tedavi öncesi ve sonrası OSKÇ ölçümleri yapıldı. Oküler ultrasonografi (USG) ile her hasta için papillanın 3 mm posteriorundan transvers ve longitudinal olarak ardışık iki ölçüm yapılıp ortalaması alındı. Hastalardan eş zamanlı kan gazı, üre, sodyum, potasyum, klor, glikoz, idrar keton analizi için kan ve idrar örnekleri alınıp, ozmolariteleri hesaplandı.Bulgular: Çalışmaya 18 hasta dahil edildi. Tedavi öncesi sağ OSKÇ ölçümleri 3,10-5,50 mm aralığında (ortalama: 4,10±0,59 mm), sol OSKÇ ölçümleri 3,10-5,70 mm aralığında (ortalama: 4,24±0,73 mm) olup, tedavi sonrası sağ OSKÇ ölçümleri 3,10-5,50 mm aralığında (ortalama: 4,16±0,65 mm), sol OSKÇ ölçümleri 3,20-5,70 mm aralığında (ortalama: 4,34±0,53 mm) tespit edildi. Delta sağ OSKÇ ölçümleri -2,00–1,30 mm aralığında (ortalama: -0,01±0,89 mm), delta sol OSKÇ ölçümleri -1,40–1,20 mm aralığında (ortalama: -0,18±0,73 mm) olarak bulundu. Bağımlı değişkenler ile yapılan bu çalışma sonucunda hiperglisemik durumdaki OSKÇ ölçümleri ile, normoglisemik durumlarındaki OSKÇ ölçümleri arasında istatistiksel olarak anlamlı fark saptanmadı (p>0.05).Sonuç: Bu tez çalışmasında ortaya çıkan bulgular sonucunda, erişkin hiperglisemik hastalarda tedavi ile normoglisemi sağlanması sonucu oküler USG ile OSKÇ ölçümlerinde istatistiksel anlamlı fark olmadığı ve dolayısıyla ultrasonografik OSKÇ ölçümlerinin hiperglisemik hastalarda tedavi izleminde kullanımının yararlı olmayacağı ortaya konmuştur.Anahtar kelimeler: acil servis, hiperglisemi, optik sinir kılıf çapı, oküler ultrasonografi, tedavi Objective: To investigate the optic nerve sheath diameter (ONSD) differences in patients presented with hyperglycemia and treated in the emergency department.Materials and methods: The prospective, cross-sectional, observational study included adult patients with an admission blood glucose level of 250 mg/dl or more. Following detection of hyperglycemia in the emergency department, pre- and post-treatment ONSD measurements were performed. Each patient underwent two consecutive measurements performed transversely and longitudinally at the point 3 mm posterior of the papilla by ocular ultrasonography (USG). Blood and urine samples were also drawn for simultaneous blood gases, urea, sodium, potassium, chlorine, glucose, urine analysis for ketone bodies and osmolarities were calculated.Results: A total of 18 patients were included in the study. Right ONSD measurements were between 3,10 and 5,50 mm (mean: 4,10±0,59) and left ONSD were between 3,10 and 5,70 mm (mean: 4.24±0.73) before treatment. Following the treatment, right ONSD were found between 3,10 and 5,50 mm (mean: 4.16±0.65 mm), while left ONSD were between 3.20 and 5.70 (mean: 4.34±0.53 mm).Delta right ONSD was found to be between -2.00 and 1.30 mm (mean: -0.01±0.89 mm), delta left ONSD was found to be between 1.40 and 1.20 mm (mean: -0.18±0.73 mm). As a result of this study, there was no statistically significant difference between ONSD measurements in hyperglycemia and in normoglycemic conditions (p>0.05).Conclusion: The findings of this study revealed that there is no statistical difference in ocular USG with ONSD measurements in adult hyperglycemic patients as a result of provision of normoglycemia, and therefore it would not be useful to employ ultrasonographic ONSD measurements in the treatment follow-up in hyperglycemic patients.Key words: hyperglycemia, optic nerve sheath diameter, ocular ultrasonography, emergency medicine, treatment, 81

Giriş ve Amaç: İskemik inme; fokal serebral, spinal veya retinal infarkt sonucu meydana gelen nörolojik fonksiyon bozukluğudur. Akut inmede en etkili tedavi yaklaşımı trombolitik tedavidir. İnme ve komplikasyonlarına bağlı olarak kafa içi basınç artabilir. Çalışmamızda inmeye ve komplikasyonlarına ikincil olarak görülen kafa içi basınç artışını öngörmek amacıyla beyin tomografisinde optik sinir kılıf çapı ölçümünün yararlılığını değerlendirmeyi amaçladık.Gereç ve Yöntem: Çalışmamıza Dokuz Eylül Üniversitesi Acil Servisine 01.01.2015-01.06.2018 tarihleri arasında akut inme ile başvuran ve trombolitik tedavi alan 93 hastanın başvuru şikâyeti, yaş ve cinsiyetleri, komorbid hastalıkları, vital bulguları ve GKS, trombolitik sonrası komplikasyonları, geliş NIHSS, 24 saatlik ve 7 günlük mortaliteleri, giriş ve 7. günde MRS sonuçları değerlendirildi. Optik sinir kılıf çapları (OSKÇ) trombolitik öncesi (OSKÇ-0) ve trombolitik sonrası (OSKÇ-24) çekilen beyin tomografilerinde karşılaştırıldı.Bulgular: Hastaların 49'u kadındı (%52,7) ve yaş ortalaması 76 (Aralık: 37-94) idi. Hastaların 86'sında (%92,4) en az bir tane komorbid hastalık mevcuttu. En sık karşılaşılan komorbid hastalık hipertansiyondu (n=65, %69,9). Sinir sisteminde nöronal sağkalım, nörogenez ve plastisite gibi önemli görevleri olan nörotrofik faktörlerin varlığı nöronal hücreler için vazgeçilmezdir. Nörotrofik faktörlerin yokluğu hatta seviyelerindeki farklılıklar bile bir çok nörodejeneratif hastalığı tetiklemektedir. Serebral Dopamin Nörotrofik Faktör (CDNF) ve Mezensefalik Astrosit-Kökenli Nörotrofik Faktör (MANF) adlı iki nörotrofik faktör yapısal özellikleri ile diğer nörotrofik faktörlerden ayrılmaktadırlar. Bu farklılık sonucunda ise diğer nörotrofik faktörlerle kıyaslandıklarında daha farklı sinyal yolakları üzerinden farklı etkilere yol açtıkları düşünülmektedir. Bu sebeplerden dolayı bu iki nörotrofik faktörün araştırılması nörodejeneratif hastalıkların işleyişlerinin daha iyi anlaşılabilmesi ve sonucunda yeni tedaviler üretilebilmesi için uygun bir seçenek olarak gözükmektedir. Bu tezde CDNF ve MANF tedavilerinin optik sinir hasarı sonrası in vivo şartlarda nöronal sağkalım, protein sinyal iletim yolakları ve ek olarak protein profil analizi üzerine incelemeler yapılmıştır. Tüm bu mekanizmaları inceleyebilmek adına optik sinir aksotomisi modeli ile farelere hasar verilmiştir. Bu hasar modeli ile hasar kan beyin bariyerini bozmayarak bölgesel kalmakta ve retinal gangliyon hücrelerine in vivo çalışılabilme olanağı sağlamaktadır. Hasar sonrası CDNF tedavisinin MANF tedavisine oranla daha fazla olacak şekilde iki tedavinin de hücre ölümünü azalttığı, yeni nöron oluşumunu arttırdığı ve protein profil analizi ile de hücrelerin hasara karşı verdiği tepkileri arttırarak hücreleri hayatta kalmaya yönlendirdikleri gözlemlenmiştir. Elde edilen bulguların bir çok nörodejeneratif hastalığın tedavisine yönelik yeni yaklaşımlar bulunmasına yardımcı olacağı düşünülmektedir. Neurotrophic factors are a must for the central nervous system. They are important factors for neuronal survival, neurogenesis and also neuronal plasticity. The absence of neurotrophic factors or even their low levels can trigger many neurodegenerative diseases. Two factors named Cerebral Dopamine Neurotrophic Factor (CDNF) and Mesencephalic Astrocyte-Derived Neurotrophic Factor (MANF) differ from all other neurotrophic factors because of their unique structures. Because of this difference it is believed that they work along different signalling pathways and so have different outcomes. As a result, deeper understanding of these two factors is an appropriate option in order to find new approaches for better understanding and possibly treating various neurodegenerative diseases. This thesis focuses on the effects of CDNF and MANF treatment in vivo on optic nerve axotomy. Neuronal survival, protein signalling pathways and protein profile analysis were investigated. In order to study these mechanisms, optic nerve axotomy was performed. Both CDNF and MANF had restorative effects on neuronal survival and increased neurogenesis and also had a positive effect on increasing the cellular stress responses of retinal ganglion cells. It is believed that the acquired results will lead to finding new approaches of therapeutic solutions for neurodegenerative diseases. 67

Amaç: Obstrüktif Uyku Apne Sendromu (OUAS) olan hastalarda optik sinirin fonksiyonunun VER, bilgisayarlı otomatik perimetri ve Farnsworth Munsell 100 Hue testi ile değerlendirilmektir.Hastalar ve Yöntem: Erciyes Üniversitesi Tıp Fakültesi Nöroloji polikliniğine başvuran ve Polisomnografi testi ile OUAS tanısı alan hastalarda detaylı göz muayenesini takiben optik sinir fonksiyonlarını ve olası patolojileri değerlendirmek amacıyla VER (Görsel uyarıya kortikal cevap), bilgisayarlı otomatik perimetri ile görme alanının taranması ve Farnsworth Munsell 100 Hue testi ile renkli görme testleri uygulandı. VER testinde P100 dalgasının amplitüd ve latansı, bilgisayarlı otomatik perimetri ile varsa görme alanı defektleri, Mean Deviasyon (MD), Loose Variance (LV) değerlerini ve Renkli Görme testinde Total Hata Skoru değerlendirildi. Bu parametreler hasta ile kontrol gruplarında karşılaştırıldı. Testlerdeki parametreler araştırmacı tarafından ölçülüp istatiksel fark ölçüldü.Bulgular: Kontrol grubunun yaş ortalaması 40.3 ± 13.1 yıl, hafif OUAS grubunun yaş ortalaması 50.3 ± 8.0 yıl, orta OUAS grubunun yaş ortalaması 46.0 ± 9.8yıl, ağır OUAS grubunun yaş ortalaması 49.6 ± 7.2 yıl idi. Gruplar arasında yaş dağılımında anlmalı farklılık yoktu (P>0.05).Bilgisayarlı otomatik perimetride Ortalama MD( Mean Deviasyon)değeri kontrol grubunda 5.6± 4.0 db, hafif OUAS grubunda 5.4 ± 3.1 db, orta OUAS grubunda 5.5 ± 5.0 db, Ağır OUAS grubunda 5.6 ± 3.5 db olarak hesaplandı. Kontrol grubu, hafif OUAS, orta OUAS, ağır OUAS grubunda ortalama MD (db) değeri anlamlı (p ˃ 0.05) farklılık göstermemiştir. Kontrol grubu, hafif OUAS, orta OUAS, ağır OUAS grubunda ortalama sLV(PSD) (db) değeri anlamlı (p ˃ 0.05) farklılık göstermemiştir. Hafif OUAS, orta OUAS, ağır OUAS grubunda ortalama VER p100 latans değeri kontrol grubundan anlamlı (p ˂ 0.05) p= 0.000 olarak daha yüksekti. Kontrol grubu, hafif OUAS, orta OUAS, ağır OUAS grubunda, ortalama p100 Amp (Uv) değeri anlamlı (p ˃ 0.05) p= 0.129 farklılık göstermemiştir. Hafif OUAS, orta OUAS, ağır OUAS grubunda ortalama HUE Total Hata Skoru(THS) değeri kontrol grubundan anlamlı (p ˂ 0.05) olarak daha yüksekti.Sonuç: OUAS (Obstrüktif Uyku Apne Sendromu) ile ilişkili hipoksi, hiperkarbi, asidoz ve vasküler otoregülasyon bozulması, optik sinirin hemodinamiğini bozarak nörodejenerasyona neden olabilir. Ayrıca hipoksemiye bağlı vasküler direnç artışı optik sinir perfüzyonunu bozarak optik sinirinin fonksyonel değişikliğne neden olabilmektedir. Bu değişiklik VER testinin p100 dalgasının latansında uzaması ve amplitüd değişikliğine aynı zamanda renkli görme problemlerine yol açabilmektedır.Anahtar Kelimeler:Obstrüktif Uyku Apne Sendromu, optik sinir, VER ( visually evoked response=Görsel uyarıya cevap), bilgisayarlı otomatik perimetri, Farnsworth Munsell 100 Hue testi (Renkli Görme testi) Objective: To evaluate the function of the optic nerve in patients with obstructive sleep apnea syndrome (OSAS) by VER, computerized automatic perimetry and Farnsworth Munsell 100 Hue test.Materials and Methods: To evaluate the optic nerve functions and possible pathologies following detailed eye examination in patients who were admitted to the Neurology outpatient clinic of Erciyes University Faculty of Medicine and diagnosed with OSAS, VER (Visual cortical response), computerized automated perimetry, visual field screening and Farnsworth Munsell 100 Color vision tests were performed with Hue test. In the VER test, amplitude and latency of the P100 wave,İn the computerized automatic perimetry and visual field defects, Mean Deviation (MD), Loose Variance (LV) values and Total Error Score in Color Vision test were evaluated. These parameters were compared in patient and control groups. The parameters in the tests were measured by the researcher and the statistical difference was measured.Results: The mean age of the control group was 40.3 ± 13.1 years, the mean age of the mild OSAS group was 50.3 ± 8.0 years, the mean age of the moderate OSAS group was 46.0 ± 9.8 years, and the mean age of the severe OSAS group was 49.6 ± 7.2 years. There was no significant difference in age distribution between the groups (P> 0.05).MD value in computerized automatic perimeter was 5.6 ± 4.0 db in control group, 5.4 ± 3.1 db in mild OSAS group, 5.5 ± 5.0 db in moderate OSAS group, 5.6 ± 3.5 db in severe OSAS group.The mean MD (db) values of the control group, mild OSAS, moderate OSAS, and severe OSAS did not differ significantly (p ˃ 0.05). In the control group, mild OSAS, moderate OSAS, and severe OSAS, the mean sLV (PSD) (db) values did not differ significantly (p ˃ 0.05).The mean VER p100 latency was significantly higher in the mild OSAS, moderate OSAS, and severe OSAS groups than the control group (p ˂ 0.05), In the control group, mild OSAS, moderate OSAS, and severe OSAS, the mean p100 Amp (Uv) value did not differ significantly (p ˃ 0.05) The mean HUE Total Error Score (TES) of the mild OSAS, moderate OSAS, and severe OSAS groups were significantly higher than the control group (p ˂ 0.05).Conclusion: Hypoxia, hypercarbia, acidosis and vascular autoregulation disruption associated with OSAS (Obstructive Sleep Apnea Syndrome) may disrupt the hemodynamics of the optic nerve and cause neurodegeneration. In addition, increased vascular resistance due to hypoxemia may impair the optic nerve perfusion and cause functional changes of the optic nerve. 47. 4,5 G cep telefonu radyasyonunun görme sistemi üzerine etkileri

Farklı frekans bandındaki cep telefonlarının yaydığı radyofrekans (RF) dalgaların, insan ve çevre sağlığı üzerine etkilerine ilişkin çok sayıda çalışma mevcuttur. Tüm bu çalışmalar; elektromanyetik alan (EMA)'ların, canlılar üzerine olumsuz etkileri olabileceğini göstermektedir. Geniş frekans bandında çalışan 4,5 G uyumlu cep telefonları internet, haberleşme v.b. birçok işlevi yerine getirmektedir. Bu nedenle; bu tür telefonların kullanım süresinin uzaması, EMA'lara maruziyeti arttıracak bir faktördür. Bu tez çalışmasında 4,5 G uyumlu cep telefonlarının görme sistemi üzerine etkileri; elektrobiyofiziksel, biyokimyasal ve histolojik yöntemler kullanılarak araştırılmıştır. Çalışmada; görsel uyarılmış potansiyel (GUP) ölçümü, optik sinir antioksidan enzim aktiviteleri ölçümü ve optik sinir ultra yapısı görüntülenmiştir. Deneylerde; 16 sham ve 16 deney grubu olmak üzere 32 adet Wistar albino erkek sıçan kullanılmıştır. Deney grubundaki sıçanlar 45 gün boyunca, günde 2 saat 4,5 G uyumlu cep telefonuna, maksimum 5,4 V/m'lik elektrik alan, maksimum 0,009W/kg'lık SAR ve 1800 MHz'lik EMA'ya maruz bırakılmıştır. Çalışma sonunda; ortalama deri sıcaklığında uygulama öncesi ve sonrası sıcaklık değerleri arasında, gruplar arasında istatistiksel olarak anlamlı bir fark gözlenmemiştir (p˃0,05). MDA düzeyi; deney grubunda, sham grubuna göre önemli oranda artarken (p=0,002), antioksidan enzimlerden SOD ve CAT düzeyleri deney grubunda, sham grubuna göre önemli oranda azalmıştır (p0,05), N2 ve P3 genlikleri ile P2-N2 ve N2-P3 tepeden tepeye genlikleri istatistiksel olarak önemli oranda azalmıştır (p0,05); P3 latansı, deney grubunda sham grubuna göre daha kısa bulunmuştur (p0,05). While MDA levels were significantly increased in the experimental group compared to the sham group (p=0,002), antioxidant enzymes, SOD and CAT levels were significantly decreased in experimental group compared to sham group (p0,05), the amplitudes of N2 and P3 and the peak-to-peak amplitudes of P2-N2 and N2-P3 decreased significantly (p0,05). Amaç: Bu çalışmada aksiyel miyopi hastalarında optik sinir başı (OSB); retina sinir lifi tabakası (RSLT) kalınlığının optik koherens tomografi (OKT) ve vasküler dansite yoğunluğunun optik koherens tomografi anjiografi (OKTA) ile değerlendirilmesi amaçlanmıştır.Gereç ve Yöntem: Bu çalışma miyop tanısı ile Dicle Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı Retina Birimi'nde izlenen ve tedavi edilen olgularda Haziran 2017 – Mayıs 2019 tarihleri arasında çekilen OKT, OKTA ve muayene sonuçlarının bulunduğu hasta dosyaları taranarak elde edilen verilerle geriye dönük olarak yapılmıştır. Çalışmada daha önce miyopi tanısı almış ancak retina muayenesinde miyopiye bağlı patolojik bulgusu olmayan 23 hastanın 37 gözü ve dejeneratif miyopisi olan 24 hastanın 34 gözü incelenmiş, OSB'nın kan akımı ve RSLT kalınlığı değerlendirilip oftalmopatolojisi olmayan gözlerle karşılaştırılmıştır.Bulgular: Çalışmaya katılan hastaların aksiyel uzunluklarının ve refraksiyon kusurlarının ortalamaları sırası ile kontrol grubunda 22,510±0,559 mm, -0,236±0,363 D; birinci grupta 26,495±0,702 mm, -8,567±2,885 D ve ikinci grupta 30,025±1,126 mm, -15,838±3,903 olarak saptanmış ve tüm karşılaştırmalarda istatistiksel olarak anlamlı farklılık olduğu izlenmiştir. OKT ile yapılan RSLT ölçümlerinde ise ikinci grubun hem birinci grup hem de kontrol grubuyla karşılaştırıldığında temporal kadran dışındaki diğer kadranlarda RSLT kalınlıklarında istatistiksel olarak anlamlı bir incelme olduğu görülmüştür. Ayrıca birinci grup; kontrol grubuyla karşılaştırıldığında temporal ve temporal inferior kadran dışındaki kadranlarda RSLT kalınlıklarında istatistiksel olarak anlamlı bir incelme olduğu izlenmiştir. RSLT değerleri ile aksiyel uzunluk ve refraksiyon kusuru arasında ise herhangi bir ilişki saptanmamıştır. OKTA ile OSB vasküler dansite yoğunluğu değerlendirilmiştir. Kontrol grubunda OKTA parametrelerinin birinci grup ile istatistiksel olarak anlamlı farklılık göstermiyorken ikinci grup ile ise anlamlı farklılık gösterdiği tespit edilmiştir. Ayrıca birinci ve ikinci grup arasında da NH ve RPC segmentlerinde aynı bölgelerde benzer şekilde istatistiksel olarak anlamlı farklılık olduğu saptanmıştır. Hem birinci hem de ikinci grupta aksiyel uzunluk arttıkça OKTA ile ölçülen OSB perfüzyonunda azalma olduğu yani negatif korelasyon olduğu izlenmiştir. OKTA ve RSLT parametreleri arasında korelasyonun farklı segmentlerde değişiklikler gösterdiği tespit edilmiştir.Sonuç: Aksiyel miyopisi olan hastaların özellikle optik disk ve peripapiller alanda görülebilecek komplikasyonların değerlendirilebilmesi açısından OKTA ile yapılan vasküler dansite yoğunluk ölçümleri; noninvaziv de olması nedeniyle bu hastaların tanı ve takibinde kullanılabilir.Anahtar Kekimeler: Miyopi, aksiyel miyopi, aksiyel uzunluk, optik koherens tomografi, retina sinir lifi analizi, optik koherens tomografi anjiografi. Purpose: The aim of this study was to evaluate the retinal nerve fiber layer (RNFL) thickness in optic nerve head (ONH) with optical coherence tomography (OCT) and vascular density of ONH with optical coherence tomography angiography (OCTA) in axial myopia patients.Materials and Methods: This study was performed retrospectively with the data obtained from OCT, OCTA and patient files taken between June 2017 and May 2019 in patients who were followed up and treated in the Retinal Unit of Ophthalmology Department of Dicle University Medical Faculty with the diagnosis of myopia. In this study, 37 eyes of 23 patients who were previously diagnosed with myopia but no pathological findings due to retinal examination and 34 eyes of 24 patients with degenerative myopia were evaluated. Blood flow and RNFL thickness of ONH were evaluated and compared with eyes without ophthalmopathology.Results: The mean axial lengths and refraction defects respectively of the patients were 22,510 ± 0,559 mm, -0,236 ± 0,363 D in the control group; 26,495 ± 0,702 mm, -8,567 ± 2,885 D in the first group and 30,025 ± 1,126 mm, -15,838 ± 3,903 D in the second group and significant differences were observed in all comparisons. In the RNFL measurements performed with OCT, it was observed that there was a statistically significant decrease in RNFL thickness in other quadrants other than temporal quadrants in the second group when compared with the first and control groups. In addition, the first group; when compared with the control group, there was a statistically significant decrease in RNFL thickness in the quadrants other than the temporal and temporal inferior quadrants. No correlation was found between RNFL values and axial length and refractive error. The vascular density of ONH was evaluated with OCTA. In the control group, OCTA parameters did not show statistically significant differences with the first group, but show statistically significant differenceswith the second group. There was also a statistically significant difference between the first and second groups in NH and RPC segments in the same regions. As the axial length increased in both the first and second groups, there was a decrease in the ONH perfusion measured by OCTA, ie there was negative correlation. It was found that the correlation between OCTA and RNFL parameters varied in different segments.Conclusion: In patients with axial myopia, vascular density density measurements can be performed with OCTA, which is a noninvasive method and the optic disc and peripapillary area can be evaluated for complications, diagnosis and follow-up.Key Words: Myopia, axial myopia, axial length, retinal nerve fiber layer, optical coherence tomography, optical coherence tomography angiography. 86

Amaç: Bu çalışmada, multiple skleroz hastalarında fovea ve optik sinir başının optik koherens tomografi anjiografi görüntüleme yöntemi ile değerlendirilmesi amaçlanmıştır.Gereç ve Yöntem: Bu çalışma Şubat 2018 -Mayıs 2018 tarihleri arasında Dicle Üniversitesi Tıp Fakültesi Göz Hastalıkları Polikliniği'nde gerçekleştirildi. Çalışmaya Nöroloji Kliniği'nde daha önceden multiple skleroz tanısı almış ve takibi yapılan 40 hastanın (daha önce görme problemi bulunan GRUP 1; 15 hasta ve görme problemi olmayan GRUP 2; 25 hasta) ve multiple skleroz hastası olmayan normal 40 hasta (kontrol grubu) çalışmaya dahil edildi. Tüm olguların dosyaları incelenerek optik koherens tomografi anjiografi ile peripapiller alan; total, üst yarı alan, alt yarı alan, süperior, inferior, nazal ve temporal kadranlarda, 12 kadranda peripapiller retina sinir lifi tabakası kalınlığı ve sinir başı ölçümleri kaydedildi. Ayrıca damar yoğunlukları ölçüldü. Bu ölçümler üç grup arasında karşılaştırıldı.Bulgular: Çalışmada yer alan grup 1 hastaların %35'i kadın ve %2.5'i erkek olurken, grup 2'de yer alan hastaların %50'si kadın ve %12,5'inin erkek ve kontrol grubunda yer alan hastaların %50'si kadın ve %50'si erkek idi. Grup 1'deki hastaların yaş ortalaması 35,73±6,79 yıl olurken grup 2'deki hastaların yaş ortalaması 37,44±9,88 yıl ve kontrol grubundaki hastaların ise 35,12±9,05 yıl olarak saptanmıştır Kontrol grubunda yer alan hastaların bölgelere göre sağ ve sol göz Optik Sinir Başı (OSB) vasküler yoğunluk değerlerinin grup 1 ve grup 2'de yer alan hastalarınkinden daha yüksek değerlere sahip oldukları saptandı. Grup 1 ile kontrol grubu arasında Whole Image, Inside Disc Inferior Temporal ve Temporal bölgelerde istatistiki yönden anlamlı olarak yüksek bulundu. Amaç: Optik nörit (ON) çocukluk çağı demiyelinizan hastalıkları arasında sıkça görülen, genellikle düzelme sağlanabilmekle birlikte bazı olgularda geri dönüşümsüz hasar ve görsel fonksiyonel kayba neden olabilen önemli bir sağlık sorunudur. Dikkatli bir ayırıcı tanı ve iyi planlanmış bir tedavi gerektiren bu hastalığın değerlendirildiği bu çalışmanın amacı; ON'li çocukların tüm özellikleri ile retrospektif olarak değerlendirilmesidir.Yöntem: Kliniğimizde Ocak 2005-Temmuz 2017 tarihleri arasında tanı alan, tedavi edilen ve izlenen optik nöritli hastaların bilgileri hastane bilgi yönetim sistemi ve hasta kayıtlarından edinildi. Olguların demografik ve klinik özellikleri ile tedavi kayıtları ve sonuçları retrospektif olarak değerlendirildi.Bulgular: Ortalama yaşları 11,4±4,4 yıl olan 28 hastaya ait veriler incelendi. K/E oranı 16/12 idi. 17 hastada tek taraflı, 11 hastada bilateral tutulum vardı. Ensık başvuru şikayeti görme keskinliğinde azalma idi (%57,1). Bilateral hastalıkta, görme kaybı oranları anlamlı derecede yüksek idi. Enfeksiyon öyküsü olguların %28,6'sında mevcuttu. En sık muayene bulgusu disk ödemi idi (%50). 2 hastada magnetik rezonans görüntülemede demyelinizan plak vardı. 15 hasta iv+oral steroid (%53,6), 5 hasta sadece iv steroid (%17,9), 1 hasta oral steroid (%3,6) aldı.1 hastaya plazmaferez uygulandı. 11 hasta remisyona girdi (%53,6), 3 hastada (%10,7) rekürrens oldu, unilateral hastalığı olan 4 olgunun, bilateral yerleşimli hastalığı olan 1 olgunun görme düzelmesi olmadı, diğer olguların görmelerinde düzelme sağlandı.Sonuç: Çalışmamızda elde ettiğimiz bulguların literatür ile genel bir uyum içinde olduğu görüldü. Sonuçlarımız çocukluk çağının bu önemli hastalığı için önemli bir veri kaynağı oluşturacaktır. Daha uzun süreli prospektif takipler ile hastalığın uzun dönem prognozu hakkında daha verimli sonuçlar elde edilebilecektir.Anahtar Kelimeler: Optik nörit, demografik özellikler, klinik özellikler, tedavi sonuçları, OndokuzMayısÜniversitesi Background and aim: Optical neuritis (ON) is a frequent symptom among the demyelinating diseases of childhood. Most of the cases can be recovered, but some patients may have irreversible damage and loss of visual functionality. A detailed diagnosis and well-managed treatment is necessary in this disease. The aim of current study is to evaluate the demographic and clinical features of ON cases retrospectively.Methods: The records of patients that diagnosed and followed-up at our clinic between Jan 2015 and Jul 2017 with a diagnosis of ON were obtained from patients' records and hospital database. The demographic and clinical characteristics of patients, treatments and outcomes were retrospectively evaluated.Results: Mean age of 28 patients was 11.4±4.4 years. F/M ratio eas 16/12. 17 patients had unilateral, and 11 had bilateral disease. Most frequent complaint on admission was decreased visual acuity (57%). Visual loss was significantly higher in bilateral disease. 28.6% of cases had a history of infection. Most frequent finding in examinations was disc edema (50%). 2 patients had demyelinating lesions on magnetic resonance imaging. 15 cases received iv+pulse steroid (53.6%), 5 patients had iv only steroid (17.9%), and 1 patient had oral steroid (3.6%). 11 patient had remission (53.6%), 3 patients had recurrence (10.7%). 4 patients with unilateral disease, and 1 patient with bilateral disease had no improvement with treatment, remaining cases had recovered visual functions.Conclusion: Our results were generally in accordance with the literature data. Our results will constitute an important data source for this important disease of childhood.
