Amaç: Laparaskopik kolesistektomi (LK), semptomatik kolelitiazis için en çok uygulanan cerrahi yöntemdir. LK sonrası trokar giriş yeri ağrısı ameliyat sonrası dönemde stres ve morbiditede artışa, hastanede kalış süresinde uzamaya neden olmaktadır. Ameliyat sonrası ağrı kontrolünde intravenöz opioid ve non-opioid analjezik kullanımı yaygındır. Bununla birlikte intravenöz ağrı kesici kullanımının yan etkileri de mevcuttur. Lokal anestezik infiltrasyonu ile ağrı kontrolü intravenöz ağrı kesici kullanımını ve ameliyat sonrası stresi azaltabilir. Bu çalışmanın amacı LK sonrası yapılan lokal anestezik infiltrasyonunun etkinliğini göstermektir. Materyal ve Metod: Çalışmaya ASA skoru 1-2 olan 100 hasta dahil edildi. Tüm hastalara standart LK uygulandı. Her biri 50 hastadan oluşan 2 ayrı grup oluşturuldu. İki grup Mann-Whitney U testi kullanılarak karşılaştırıldı. Grup-1 LK’ den sonra kesi yerlerine lokal bupivakain enjekte edilmiş 50 hastadan, grup-2 ise lokal anestezik enjekte edilmemiş 50 hastadan oluşmaktaydı. Grup-1’ deki her hastaya 10 cc %1’ lik bupivakain infiltrasyonu uygulandı. Dördüncü. ve 12. saat vizüel analog skala (VAS) skorları kaydedildi. İstatistiksel değerlendirme için SPSS 15.0 programı kullanıldı. Bulgular: Grup 1’ de ortalama VAS skoru dördüncü saatte 2, onikinci saate 3.4 bulunurken; grup 2’ de dördüncü saat değeri 5.1 ve onikinci saat değeri 4.6 olarak bulundu. Ortalama VAS skoru grup-1’ de istatistiksel olarak anlamlı şekilde grup 2’ den daha düşük bulundu(p, Aim: Laparoscopic cholecystectomy (LC) is the most frequently performed elective surgery for symptomatic cholelithiasis. Trocar site pain after LC causes increased stress, morbidity and hospital stay. Consumption of intravenous opioid and nonopioid analgesics for control of postoperative pain is common. Intravenous analgesics have also side effects. Pain control with local anaesthetic infiltration can reduce intravenous analgesics consumption and postoperative stress. The aim of this study is point the efficiencies of local anaesthetic infiltration after LC. Methods: A total of 100 patients with ASA Class 1-2 were included to the study. All of the patients underwent to standard LC. Patients were divided into two groups of 50 each. Group-1 consisted of local bupivacaine enjected 50 patients and group-2 was consisted of non-local enjected 50 patients, after LC. Two groups were compared with the Mann-Whitney U test. A total of 10 cc sterile 1% bupivacaine infiltration was performed each patient in group-1. The 4th and 12th hour visual analogue scale (VAS) scores, were recorded. SPSS 15.0 program was used for statistical evaluation. Results: In group-1, the mean VAS score was 2 in the fourth hour and 3.4 in the twelfth hour; in group-2 the fourth hour was 5.1 and the twelfth hour was 4.6. Amaç: Adjuvan tedaviden fayda görebilecek kolorektal kanserli hastaları seçebilmek için histopatolojik evrelemeye ek olarak yeni prognostik belirteçlere ihtiyaç duyulmaktadır. Kolorektal kanserlerde ise anjiogenezin prognoz üzerine etkisi halen tartışmalıdır. Yöntem: Modifiye-Astler-Coller Evre A-B olarak histopatolojik tanı almış 119 hasta belirlenmiş ve düzenli takipleri olan, 41 hasta çalışmaya alınmıştır. Bu hastaların 16 tanesinde lokal rekürrens veya uzak metastaz tespit edilmiştir. VEBF ekspresyonu anti-rabbit immunoglobulin boyaması yöntemiyle belirlenmiştir. Bulgular: VEBF ekspresyonu negatif olan grupta 5 yıllık yaşam %83.33, VEBF ekspresyonu pozitif olan grupta ise 5 yıllık yaşam %82,13 olarak bulunmuştur. VEBF ekspresyonu ile tümör yerleşimi, tümör derecesi, pT evresi, angio-nöral-lenfatik invazyon, lokal nüks ve uzak metastaz ve 5 yıllık sağ kalım arasında anlamlı bir bağlantı tespit edilememiştir. Ancak kitle dışı klinik bulgular ile başvuran hastalarda, sağ kalım açısından anlamlı fark bulunmuştur (p=0.003). Sonuç: Bu çalışma sonucuna göre evre A-B kolo-rektal kanserlerde immünhistokimyasal yöntemle anjiogenez belirlenmesinin prognostik önemi gösterilememiştir.

Gebelik sırasında mide kanseri oldukça nadir görülmektedir. Gebeliğe bağlı bulantı, kusma ve abdominal ağrı gibi gastrointestinal semptomlar hastalığın tanısını güçleştir¬mektedir. Tanı konduğunda ise hasta ileri evre tümör olarak karşımıza çıkmaktadır ve prognozu olumsuz yönde etkilemektedir. Mide kanserinde erken tanı ve tedavi sağ kalım oranını artırmaktadır. Gebelik sırasında tanı ve tedavisi oldukça güç olan mide kanserinin tedavisinde kadın doğum, genel cerrahi ve medikal onkoloji kliniklerinin ortak çalışmasıyla başarılı sonuçlar alınabilir. Bu çalışmada gebeliğinde hiçbir gastrointestinal şikayeti olmadığı halde ileri evre mide kanseri tespit edilen 39 yaşında bayan hasta sunulmuştur. Hastanın özgeçmişinde 5 yıl önce meme kanseri nedeniyle tedavi gördüğü öğrenildi. Postoperatif dönemde yapılan tetkik ve tedaviler sonrasında tedaviye tam yanıt alındı., The gastric cancer is rare in pregnancy. Gastrointestinal symptoms due to pregnancy such as nausea, vomiting and abdominal pain make difficult at the diagnosis of this disease. At diagnosis, the patient presents with the late stage tumors and this situation negatively affects prognosis. Early diagnosis and treatment increases the survival rate for gastric cancer. We can have successfull conclusions by the collaboration of the clinics such as gynecology, general surgery and medical oncology in the treatment of gastric cancer which requires both a difficult diagnosis and treatment during pregnancy. In this study, A 39 -year-old female patient who is determined in late stage gastric cancer is presented, although she had no gastrointestinal symptoms in pregnancy. The medical history revealed that she was treated for breast cancer 5 years ago . After the tests and treatments in the postoperative period, a positive response was taken to treatment.

Meme kanseri gebelik sırasında nadir görülür ancak gebelikte en sık görülen kanserdir. Gebelikte ve laktasyon döneminde oluşan memelerdeki hassasiyet ve büyüme nedeni ile bu dönemde ortaya çıkan bir kitle ve dolayısı ile meme kanserinin tanısı gecikebilir. Kadın doğum, genel cerrahi ve onkoloji kliniklerinin birlikte takip etmeleri gereken bir patolojidir. Gebelikle ilgili meme kanseri terimi, gebelik sırasında ya da gebeliği takiben 1 yıl içinde görülen meme kanserlerini kapsamaktadır. Gebelik ve laktasyonda olan fizyolojik değişiklikler nedeniyle fizik muayene bu hastalarda klinisyen için problem oluşturabilir. Bu dönemde kitle saptanırsa ultrasonografi ve ince iğne aspirasyon biyopsisi yapılarak kitle araştırılmalıdır. Gebelikte meme kanseri tanısı ileri evrelerde konulmaktadır. 25 Yaşında G3P2 olan hasta son gebeliğinin yedinci ayında sağ memesinde gittikçe büyüyen kitle nedeniyle hastaneye başvurmuş. Yapılan meme ultrasonografisinde sağ memede BIRADS 5 kriterlerine uyan kitle tespit edilmiş ve yapılan biyopsi sonucuna göre invaziv duktal karsinom tanısı almıştır. Öncelikle kadın doğum uzmanı görüşü alınıp genel cerrahi tarafından sağ modifiye radikal mastektomi ve aksiller diseksiyon uygulandı. Operasyon sonrası medikal onkoloji ve kadın doğum uzmanlarının takibine alınan hasta 33. gebelik haftasında sezeryanla doğum yaptı ve adjuvan kemoterapi programına alındı., The breast cancer is rare in pregnancy but it is the most frequent malignant tumor in pregnant women. The natural tenderness and enlargement of the breasts of pregnant and lactating women may hinder detection of discrete masses and therefore, early diagnosis of breast cancer may be delayed. It is a pathology that must be followed by obstetrics and gynecology, general surgery and oncology clinics. The term of the breast cancer associated pregnancy contains the breast cancer diagnosed during pregnancy or within one year after birth. The physical examination in these patients may pose a problem for the clinician because of the physiological changes during pregnancy and lactation. If the mass is detected during this period, it should be investigated by ultrasound and fine-needle aspiration biopsy. Diagnosis of the breast cancer in the pregnancy is diagnosed at a late stage. 25 years old patient admitted to the hospital because of a mass in her right breast being steadily growing at the end of the seventh month of her gestation. In her right breast a mass was determined with BRIADS 5 criteria by breast ultrasound and the result of the biopsy revealed invasive ductal carsinoma. First, the right modified radical mastectomy and axillary dissection was performed by general surgery, with obstetrics and gynecology's opinion. Then she was followed by oncology, obstetrics and gynecology clinics and gave birth by cesarean section in the 33rd week of pregnancy. Then, she was received adjuvant chemotherapy regimen.

Amaç: Akut apandisit, günlük uygulamada en sık karşılaşılan cerrahi hastalıkların başında gelir. Konvansiyonel laparoskopiye göre daha minimal invazif bir yöntem olan tek insizyonlu laparoskopik cerrahi, son zamanlarda yaygınlaşmaya başlamıştır. Bu çalışmada tek insizyonlu laparoskopik apandektomide edindiğimiz tecrübeyi paylaştık. Gereç ve Yöntem: Nisan 2010 – Nisan 2012 tarihleri arasında kliniğimizde, toplam 15 tek insizyonlu laparoskopik (TİL) apandektomi gerçekleştirdik. SILS (Single incision laparoscopic surgery) port (Covidien) direkt görüş altında göbekten batın içine yerleştirildi. Açı verilebilir endograsper, endodisektör ile apandiks ortaya konuldu. Apandektomi materyali SILS port ile birlikte göbekten çıkarıldı. Ameliyat süresi, ağrı skoru, hastanede kalış süresi ve komplikasyonlar kayıt altına alındı. Bulgular: Hastaların 11'sı (% 73) erkek, 4'ü (% 27) kadın idi. 1 hastada (% 6) 3 port laparoskopiye dönüldü. Ortalama yaş 35,25 (28-43)±7,36 yıl ortalama vücut kitle indeksi 30,75 (23-41)±8,34 kg/m2 idi. Anestezi skoru 1,25(1-2)±0,5 idi. Ortalama ameliyat süresi 59,33(40-90)±19,09. dakikaydı. Ameliyat sırasında ve sonrasında komplikasyonla karşılaşılmadı. Tüm hastalar ameliyat sonrası 24.saatte taburcu edildi. Ortalama ağrı skoru 2 (1-3) ±0,81 idi. Sonuç: TİL apandektomi, 3 port laparoskopik apandektomi kadar etkin bir yöntemdir. Laparoskopik cerrahi konusunda tecrübeli ellerde uygulandığında güvenli bir girişimdir. Erken dönem tecrübemizde, mükemmel kozmetik sonuçlar elde ettik, ancak yararlar, ağrı düzeyi ve iyileşme açısından daha geniş serilere ve prospektif çalışmalara ihtiyaç bulunmaktadır., Aim: Acute appendicitis is one of the most commonly encountered surgical problems in everyday practice. Single-incision laparoscopic operations have recently emerged as a less invasive alternative to conventional laparoscopy. The aim of this study was to describe our initial experience with single-incision laparoscopic appendectomy. Patients and methods: Between April 2010 – April 2012, we performed 15 single incision laparoscopic appendectomy in our clinic. SILS port (Covidien) was inserted under direct vision into the peritoneal cavity. Appendix was dissected with articulated endograsper and endodissector. Appendectomy was performed and appendix removed from umbilicus with SILS port. Outcome measures include operative morbidity, operative time, pain score, hospital stay. Results: Eleven of patients (73%) were men, four (27%) were women. Conversion to open surgery were required for one case. The mean age was 35,25 (28- 43)±7,36 years and mean body mass index was 30,75 (23-41)±8,34 kg/m2. Anestesia score was 1,25(1- 2)±0,5 and mean operating time was 59,33(40- 90)±19,09 minutes. No intraoperative or postoperative complications were encountered. Conclusion: Single incision laparoscopic appendectomy is an effective alternative to standard three -incision laparoscopic appendectomy in selected patients. This early experience suggests that outcomes are comparable to standard laparoscopic surgery but with improved cosmesis, however, a larger series is necessary to confirm these findings and to determine if there are any benefits in pain or recovery.

Perkütan bilier balon dilatasyonu (PBBD) tedavisi benign patolojiler nedeniyle gelişen darlıklara 1980' li yıllardan itibaren uygulanan bir tedavi yöntemidir. Kolanjit şikayetleri ve hiperbilirübinemisi bulunan ve yapışıklıklar nedeni ile parsiel kolesistektomi operasyonu hikayesi bulunan olgunun endoskopi eşliğinde uygulanan işlemlere cevap vermeyen darlığına perkütan balon dilatasyonu ile tedavisi gerçekleştirildi. Dirençli darlığı bulunan olgumuzdaki PBBD tedavisini literatür ışığında sunmayı amaçladık., Percutaneous biliary balloon dilatation (PBBD) treatment is applied to the benign biliary stenosis since the early 80's. In this patient with cholangitis, hyperbilirubinemia and a history of partial cholecystectomy, PBBD treatment was applied to the stenosis unresponsive to endoscopic procedures due to adhesions. Herein, we present a case undergoing PBBD due to resistant stenosis with a review of the literature.

Amaç: Çalışmamızın amacı oral yoldan beslenemeyen ancak gastrointestinal sistem fonksiyonları normal olan hastalarda perkütan endoskopik gastrostomi açılması işleminin mortalite ve morbiditesini ortaya koymak ve hastaların vücut kitle indeksi ile işlem süresi ve komplikasyon oranları arasındaki ilişkiyi belirlemektir. Yöntemler: Kliniğimizde midazolam ile sağlanan sedasyon sonrası lokal anestezi ile cilt uyuşturularak çekme metodu ile perkütan endoskopik gastrostomi açılan 24 hastanın işlem kayıtları ve şu anki sağlık durumları incelendi. Komplikasyon oranları belirlendi. Bulgular: Perkütan endoskopik gastrostomi açılan 24 hastada işleme bağlı mortalite gelişmedi. Hastaların vücut kitle indeksi ortalama 18.96±3.85 kg/m2 olarak hesaplandı. İşlem süresi ortalama 750±130 sn bulundu. Hastaların 15'inin malnutrisyon sınırı olarak belirlenen 18.5kg/m2 lik vücut kitle indeksi değerinin altında olduğu ancak bu gurup ile beslenme problemi olmayan 9 hasta arasında işlem süresi açısından anlamlı farklılık olmadığı saptandı. İşlem sonrası bir hastada giriş yerinde enfeksiyon gelişti ve antibiyotik tedavisi verildi. Hastaların ortalama 17,3±9,2 aylık takiplerinde 2 hastanın yutma refleksindeki düzelmeye bağlı olarak gastrostomi kateterleri çekildi. İki hastada yıpranma sebebi ile kateter değişimi yapıldı. Altı hasta primer hastalığına bağlı olarak kaybedildi. Sonuç: Perkütan endoskopik gastrostomi oral yoldan beslenemeyen hastalar için ideal beslenme yoludur. Bu teknik ile gastrostomi açılması klasik cerrahi yönteme göre hem daha hızlı hem de daha güvenilirdir., Objective: We aimed to evaluate the mortality and morbidity of percutaneous endoscopic gastrostomy applications and to investigate the relationship between body mass index and duration of the procedure and complication rates. Methods: We retrospectively analyzed the clinical data and complication rates of 24 patients to whom Percutaneous endoscopic gastrostomy was applied by the pull method under midazoloam superficial anesthesia. Results: No mortality occured due to the procedure. The BMİ of the patients were 18.96±3.85 kg/m2. The mean time of duration of the procedure was 750±130 sec. 15 of the patients BMİ was under and nine above the malnutrition level (18.5kg/m2) but the BMİ had no corelation between the duration of the procedure. In only one case wound infection occured and antibiotherapy was needed. During a follow up period of 17.3±9.2 months two patients patients catheter was changed because of malfunction and two patients were able to tolarate oral intake and their catheters were removed. Six patients died because of their primery disease during the follow up. Conclusions: PEG is easy to handle, effective and safe for patients who can not feed orally. From our experience, we confirm the advantages of PEG from classical surgical gastrostomy as a long-term nutritional support showing a low incidence of complications; and zero mortality.

Purpose: The aim of this study was to determine the diagnostic value of colonoscopy in patients with nonspecific abdominal pain Patients and Methods: A prospective observational study was performed at the department of general surgery between March 2006 and March 2009. All patients with abdominal pain (n: 2.604) were evaluated carefully. After a detailed anamnesis, physical examination, laboratory and radiological evaluation, the patients with nonspecific abdominal pain were invited for a colonoscopic examination. Results: Colon cancer was detected in three of the patients aged 59, 63 and 68 years old. Other nine had pedunculated polyps with dysplasia in three of them. Neither had pedicle invasion. Millimetric hyperplastic polyps were detected in 16 other patients. A hundred and ninety eight patients had colonic diverticula. Seventeen of the 25 patients with polyps and 127 of the patients with diverticula were over age 50 years. Conclusion: The value of colonoscopic examination is still controversial in patients with nonspecific abdominal pain. Although the detection rate of malignant and pre-malignant lesions are low, the high detection rate of diverticula and the low complication rate due to the procedure are in favour of colonoscopic examination in these patients.

Amaç: Streptococcus milleri grubunda yer alan Streptococcus constellatus nadir görülen apse etkenlerinden bir tanesidir. Olgumuz 68 yaşında bayan hasta sağ böğür bölgesinde meydana şişlik nedeniyle hastanemiz cerrahi servisine başvurmuştur. Apseden drenajla alınan örnekten saf kültür halinde S. constellatus üretilmiştir. Olgu perkütan drenaj ve uygun antibiyotikle tedavi edilmiştir. Apse örneklerinde S. constellatus gibi nadir ve zor üreyen bakterilerin de etken olabileceği göz önünde bulundurulmalıdır., Objective: Streptococcus constellatus is included in the Streptococcus milleri group which is a rare patogen bacteria isolated from abscess. We report a case of 68-yearold women who presented to our emergency ward with swollen in her right psoas area. S. constellatus as a pure culture grew in the culture of abscess specimen which is obtained by percutaneous drainage. The patient was succesfully treated with local drainage and appropriate antimicrobials. S. constellatus is a rarely isolated bacteria from abscess sample. It should be considered that slowly groving bacterias such as S. constellatus can be a potential pathogen of abscess.

Amaç: BI-RADS (Breast Imaging Reporting and Data System) kategori 3–5 olarak değerlendirilen lezyonlara kliniğimizde yapılan ultrasonografi (US) eşliğinde tru-cut biyopsi sonuçları retrospektif olarak değerlendirildi. US rehberliğinde tru-cutmeme biyopsisinintanı değeri araştırıldı. Ve kliniğimizdeki sonuçlar güncel literatür ile birlikte yorumlandı. Gereç ve Yöntem: Bu çalışmada Maltepe Üniversitesi Tıp Fakültesi Radyoloji Anabilim Dalında Eylül 2006 ve Mayıs 2009 tarihleri arasında US eşliğinde tru- cutbiyopsisiuygulanan97 olgunun bulguları sunulmaktadır. Bu çalışmada US, mamografi ve manyetik rezonans görüntüleme ile BI-RADS kategori 3–5olarak değerlendirilen ve US'de görülebilenmeme lezyonlarına US eşliğinde tru-cut biyopsi yapıldı. Tüm lezyonların boyutları, lokalizasyonları, BIRADS kategorileri, yapılan girişim sayısı ve histopatolojik tanıları kaydedildi. Bulgular: Histopatoloijk değerlendirme sonucu toplam 61 olgu (%62.9) benign, 35 olgu (%36.1) malign ve 1 (%1) olguda atipik duktal hiperplazi tanısı aldı. Toplam 97 olgunun 94'ünde (%96.9) patolojik tanı tru-cut ile konulmuş olup metodunsensitivitesi%96.9, spesifitesi%100 bulundu. Örneklemeiçin olgulara 4–6 arasında girişim (ortalama 4.3) yapılmasına rağmen sadece 3 olguda meme cildinde ekimoz şeklinde minör komplikasyon oluştu. Sonuç: Meme hastalıklarının tanısı için US rehberliğinde yapılan tru-cut biyopsi yöntemi; sterotaksive MRG rehberliğinde yapılan tru-cutveya vakum destek likör biyopsilerine göre sadece daha hızlı ve ucuz değil aynı zamanda kolaytolere edilebilen ve uygulanabilen bir tanı yöntemi olup son dönemlerde cerrahi biyopsinin etkin bir alternatifi haline gelmiştir., Aim: The breast biopsy results of lesions, which were assessed as BIRADS (Breast Imaging Reporting and Data System) category 3-5 in our clinic, are, evaluatedretrospectively. Thediagnostic valueof USguided breast biopsy is investigated and the results of the method in our clinic are interpreted with the current literature. Material and Methods: In this study, the fi ndings of 97 cases that underwent US guided breast biopsy at Radiology department in Maltepe University Hospital; during the time interval between September 2006 to May 2009 are presented. In this study, tru-cut biopsies with ultrasonography (US) guidance were performed to the breast lesions which were assessed as BI-RADS category 3 to 5 with US, magnetic resonance imaging (MRI) or mammographyandallof whichwere visiblein US. Thedimensions, localizations, BI-RADS categories, the number of attempts for sampling and the histopatological results of all lesions included in the study were documented. Results: Histopatological assessments revealed, 61 cases as benign (%62,9), 35 cases as malignant (%36.1), and 1 case as atypical ductal hyperplasia. 94 (%96.9) of 97 cases have been diagnosed by tru-cut biopsy, and as a result the sensitivity and specifi city were found %96.9 and %100 accordingly. Although forsampling 4-6(meanaverage4.3) attempts applied to patients. Only in 3 cases a minor complication as ecchymosed occurred on the breast skin. Conclusion: For the histological diagnosis of breast diseases US guided tru-cut biopsy is not only faster and cheaper but also an easily tolerated and applicable method when compared to MRI guided biopsy or stereotactic biopsy or vacuum assisted core biopsy procedures. It also became eff ective alternative surgical excision biopsies recently.

Purpose: The aim of this study is to evaluate the efficacy of endoscopy training at an university hospital surgery clinic and to find an answer to the question; when and how surgical endoscopy training should be. Materials and Methods: At the surgery clinic of our university gastrointestinal endoscopies were performed by residents under supervision and the success rates, complications and duration were recorded. An endoscopy was accepted to be succesfull if the caecum or duodenum was entubated. Results: When the residents files were examined the success rate for the first 50 gastroscopios was 58%. With an increasing experience the time of the procedure and the success rates increased. For an 100% success rate an experience of 100 gastroscopios was enough. AHough colonoscopy success rate also increased by experience, even after performing 200 colonoscopies still caecum entubation rate was not sufficient. Conclusion: Our study showes that colonoscopy is a harder procedure compared with gastroscopy and should be performed under supervision . in our country the courses designed by The Turkish Society of Surgery is a good choice to learn endoscopy for residents. But stil endoscopy training should spread along the 5 years of residency and maybe endoscopy rotation should be mandatory every year.

Bu çalışmada tanısal amaçlı üst gastrointestinal sistem endoskopisi yapılan hastalarda işlem öncesi sedasyon amaçlı sistemik midazolam uygulanmasının etkileri incelendi. Çalışmaya Maltepe Üniversitesi Genel Cerrahi Anabilim Dalı Endoskopi Ünitesinde 2007 Ocak- 2009 Nisan tarihleri arasında tanısal amaçlı üst gastrointestinal sistem endoskopisi yapılan 600 hasta alındı. İşlem öncesi hastalara 0,1mg/kg iv midazolam verildi.Tüm hastalarda pilor geçildi ve duodenum 1 kıta görüldü. Hastaların bazal, sedasyon sonrası ve işlem sonrası nabız ve oksijen saturasyonu, işlem süresi,anksiyete skoru ve hastanın işlem sırasındaki sedasyonu durumu endoskopist tarafından değerlendirildi. Altı yüz hastadan 562'si tıbbi gereklilik olması halinde tekrar üst gastrointestinal sistem endoskopisi yaptıracaklarını beyan ettiler.Üst gastrointestinal sistem endoskopisinde midazolam kullanılmasının, gerek işlemin endoskopist tarafından rahatça gerçekleştirilmesi, gerekse hastanın konforunun sağlanması ve daha sonraki endoskopik işlemler öncesi gelişecek anksiyetenin önüne geçilmesi için gerekli ve güvenli bir yöntem olduğu sonucuna varıldı., The aim of this study is to determine the role of midazolam used as a sedative agent in upper gastrointestinal system endoscopy. We evaluated the results of 600 patients. All of them were medicated by 0.1 mg/kg iv midazolam preoperatively. The duration of the procedure,basal and postgastroscopic pulse and 02 saturation, anxiety score and the sedation score of the patients were noted. Five hundred and fifty two of the patients were willing to undergo the same procedure if needed. Our study suggests that the use of midazolam in upper gastrointestinal system endoscopy makes the procedure easier and more comfortable both for the patient and the endoscopist.

Purpose: Oxygen radicals which are produced during reperfusion phase play a central role for hepatocellular damage during ischemia/reperfusion (l/R) injury. Antioxidant strategies appear a promising approach to prevent I/R injury. In the present study, the effect of glutathione (GSH) preconditioning established by whey protein feeding on hepatocellular injury markers was assessed in a rat hepatic normothermic l/R injury model. The interaction between intracellular GSH content and heme oxygenase-1 [HO-1] enzyme expression was also determined in order to understand the level of upgrading of HO-1 system in a high GSH content environment after preconditioning. Material and Methods: Livers of female Sprague-Dawley rats were subjected to 45/45 minutes of normothermlc l/R injury. A group of rats were fed by standard chow for a three week duration before I/R procedure (CONTROL l/R: n«16), whereas another group was fed by "whey" protein for preconditioning (WHPR l/R; n=16) during the same period. Two SHAM groups were constituted accordingly but rats were not subjected to l/R injury (STD SHAM and WHEY SHAM; n=6 in each). Following 45 minutes of reperfusion, serum transaminase levels as well as GSH, malondialdehyde (MDA) levels, apoptotic index (Al) and HO-1 gene expression In liver tissue were determined. Half of the animals In both injury groups were surveilled for a week without sampling. Results: Intracellular GSH levels in WHEY SHAM group were significantly higher than those of STD SHAM group which indicated a successful preconditioning. Preconditioning by "whey" protein feeding prior to l/R injury (WHPR l/R) significantly ameliorated the increased levels of serum transaminase as well as liver MDA levels and Ai in CONTROL l/R group. Survival of the rats In both injury groups were not different. "Whey" preconditioning also significantly increased GSH levels, whereas HO-1 expression was lower when compared to rats subjected to l/R Injury without preconditioning. Conclusion: Feeding with "whey" protein successfully resulted high GSH content in rat liver. Whey preconditioning ameliorated hepatic l/R Injury. HO-1 enzyme seems to have a less important role in antioxidant defense system when GSH production was induced prior to l/R injury.

ÖZET Karaciğer cerrahisinde en önemli klinik problemlerden birisi iskemi/reperfiizyon (l/R) hasarının oluşmasıdır. Sıcak İ/R hasarında Kupffer hücreleri tarafından salınan serbest oksijen radikalleri akut hepatosellüler hasar oluşturur. Bir antioksidan olan glutatyon (GSH)'nun bu hasan engellediği bilinmektedir. Çahşmamızda yüksek sistein içeren whey proteini ile önkoşullandırma sonrası artan GSH seviyelerinin sıçanlarda oluşturulan sıcak parsiyel İ/R modelindeki koruyucu etkinliği ve hem oksijenaz-1 (HO-1) sistemi ile olan etkileşimi araştırıldı. Sprague-Dawley cinsi sıçanlardan dört grup oluşturuldu. İki SHAM grubundan birinde sıçanlar whey proteini ile (WHEY SHAM; n=6) diğerinde ise standart yem (STT> SHAM; n=6) ile üç hafta süre ile beslendi. Kontrol grubunda standart yem ile beslenen sıçanlara (KONTROL İ/R; n=16) ise 45 'er dakikalık sürelerle selektif klempaj yöntemi ile sıcak İ/R hasan oluşturuldu. Aynı cerrahi işlem üç hafta whey proteini ile önkoşullandırılan gruba da uygulandı (WHÖK İ/R; n=16). İ/R hasan oluşturulan her iki guptaki sıçanların yansı (n=8) öldürülmeyerek yedi günlük sağkalımlan takip edildi. Sıçanların kanlarında SGOT, SGPT, karaciğer dokularında ise GSH, malondialdehit (MDA), HO-1 gen ekspresyonu ölçümü ile apoptotik indeksin (Aİ) de hesaplandığı histopatolojik incelemeleri yapıldı. WHEY SHAM grubunda GSH değerlerinin STD SHAM grubuna göre anlamlı olarak yükseldiği saptandı. KONTROL İ/R grubunda STD SHAM grubuna göre serum SGOT ve SGPT değerleri ile karaciğer dokusundaki MDA seviyesi ve Aİ'deki gözlenen artışların istatistiksel olarak anlamlı olduğu saptandı. Yine bu parametrelerin WHÖK İ/R grubunda KONTROL İ/R grubuna kıyasla anlamlı olarak azaldığı gözlendi. Ancak her iki grup arasında bir haftalık sağkalım açısından fark olmadı. WHÖK İ/R grubunda KONTROL İ/R grubuna kıyasla GSH miktarının arttığı ancak buna karşın HO-1 gen ekspresyonu düzeylerinin ise anlamlı olarak düşük olduğu saptandı. Sonuç olarak karaciğer İ/R hasarında whey proteini ile önkoşullandırma dokuda GSH miktarım arttırarak koruyucu etki göstermektedir. Ancak GSH miktanndaki artış diğer bir antioksidan olan HO-1 enzim sisternini baskılamaktadır. Objective: Streptococcus constellatus is included in the Streptococcus milleri group which is a rare patogen bacteria isolated from abscess. We report a case of 68-yearold women who presented to our emergency ward with swollen in her right psoas area. S. constellatus as a pure culture grew in the culture of abscess specimen which is obtained by percutaneous drainage. The patient was succesfully treated with local drainage and appropriate antimicrobials. S. constellatus is a rarely isolated bacteria from abscess sample. It should be considered that slowly groving bacterias such as S. constellatus can be a potential pathogen of abscess., Amaç: Streptococcus milleri grubunda yer alan Streptococcus constellatus nadir görülen apse etkenlerinden bir tanesidir. Olgumuz 68 yaşında bayan hasta sağ böğür bölgesinde meydana şişlik nedeniyle hastanemiz cerrahi servisine başvurmuştur. Apseden drenajla alınan örnekten saf kültür halinde S. constellatus üretilmiştir. Olgu perkütan drenaj ve uygun antibiyotikle tedavi edilmiştir. Apse örneklerinde S. constellatus gibi nadir ve zor üreyen bakterilerin de etken olabileceği göz önünde bulundurulmalıdır.

Aim: Acute appendicitis is one of the most commonly encountered surgical problems in everyday practice. Single-incision laparoscopic operations have recently emerged as a less invasive alternative to conventional laparoscopy. The aim of this study was to describe our initial experience with single-incision laparoscopic appendectomy. Patients and methods: Between April 2010 – April 2012, we performed 15 single incision laparoscopic appendectomy in our clinic. SILS port Covidien was inserted under direct vision into the peritoneal cavity. Appendix was dissected with articulated endograsper and endodissector. Appendectomy was performed and appendix removed from umbilicus with SILS port. Outcome measures include operative morbidity, operative time, pain score, hospital stay. Results: Eleven of patients 73% were men, four 27% were women. Conversion to open surgery were required for one case. The mean age was 35,25 28- 43 ±7,36 years and mean body mass index was 30,75 23-41 ±8,34 kg/m2. Anestesia score was 1,25 1- 2 ±0,5 and mean operating time was 59,33 40- 90 ±19,09 minutes. No intraoperative or postoperative complications were encountered. Conclusion: Single incision laparoscopic appendectomy is an effective alternative to standard three -incision laparoscopic appendectomy in selected patients. This early experience suggests that outcomes are comparable to standard laparoscopic surgery but with improved cosmesis, however, a larger series is necessary to confirm these findings and to determine if there are any benefits in pain or recovery., Amaç: Akut apandisit, günlük uygulamada en sık karşılaşılan cerrahi hastalıkların başında gelir. Konvansiyonel laparoskopiye göre daha minimal invazif bir yöntem olan tek insizyonlu laparoskopik cerrahi, son zamanlarda yaygınlaşmaya başlamıştır. Bu çalışmada tek insizyonlu laparoskopik apandektomide edindiğimiz tecrübeyi paylaştık. Gereç ve Yöntem: Nisan 2010 – Nisan 2012 tarihleri arasında kliniğimizde, toplam 15 tek insizyonlu laparoskopik TİL apandektomi gerçekleştirdik. Apandektomi materyali SILS port ile birlikte göbekten çıkarıldı. Ameliyat süresi, ağrı skoru, hastanede kalış süresi ve komplikasyonlar kayıt altına alındı. Bulgular: Hastaların 11'sı % 73 erkek, 4'ü % 27 kadın idi. 1 hastada % 6 3 port laparoskopiye dönüldü. Ortalama yaş 35,25 28-43 ±7,36 yıl ortalama vücut kitle indeksi 30,75 23-41 ±8,34 kg/m2 idi. Anestezi skoru 1,25 1-2 ±0,5 idi. Ortalama ameliyat süresi 59,33 40-90 ±19,09. dakikaydı. Ameliyat sırasında ve sonrasında komplikasyonla karşılaşılmadı. Tüm hastalar ameliyat sonrası 24.saatte taburcu edildi. Ortalama ağrı skoru 2 1-3 ±0,81 idi. Sonuç: TİL apandektomi, 3 port laparoskopik apandektomi kadar etkin bir yöntemdir. Laparoskopik cerrahi konusunda tecrübeli ellerde uygulandığında güvenli bir girişimdir. Erken dönem tecrübemizde, mükemmel kozmetik sonuçlar elde ettik, ancak yararlar, ağrı düzeyi ve iyileşme açısından daha geniş serilere ve prospektif çalışmalara ihtiyaç bulunmaktadır.

Objective: We aimed to evaluate the mortality and morbidity of percutaneous endoscopic gastrostomy applications and to investigate the relationship between body mass index and duration of the procedure and complication rates. Methods: We retrospectively analyzed the clinical data and complication rates of 24 patients to whom Percutaneous endoscopic gastrostomy was applied by the pull method under midazoloam superficial anesthesia. Results: No mortality occured due to the procedure. The BMİ of the patients were 18.96±3.85 kg/m2. The mean time of duration of the procedure was 750±130 sec. 15 of the patients BMİ was under and nine above the malnutrition level 18.5kg/m2 but the BMİ had no corelation between the duration of the procedure. In only one case wound infection occured and antibiotherapy was needed. During a follow up period of 17.3±9.2 months two patients patients catheter was changed because of malfunction and two patients were able to tolarate oral intake and their catheters were removed. Six patients died because of their primery disease during the follow up. Conclusions: PEG is easy to handle, effective and safe for patients who can not feed orally. From our experience, we confirm the advantages of PEG from classical surgical gastrostomy as a long-term nutritional support showing a low incidence of complications; and zero mortality., Amaç: Çalışmamızın amacı oral yoldan beslenemeyen ancak gastrointestinal sistem fonksiyonları normal olan hastalarda perkütan endoskopik gastrostomi açılması işleminin mortalite ve morbiditesini ortaya koymak ve hastaların vücut kitle indeksi ile işlem süresi ve komplikasyon oranları arasındaki ilişkiyi belirlemektir. Yöntemler: Kliniğimizde midazolam ile sağlanan sedasyon sonrası lokal anestezi ile cilt uyuşturularak çekme metodu ile perkütan endoskopik gastrostomi açılan 24 hastanın işlem kayıtları ve şu anki sağlık durumları incelendi. Komplikasyon oranları belirlendi. Bulgular: Perkütan endoskopik gastrostomi açılan 24 hastada işleme bağlı mortalite gelişmedi. Hastaların vücut kitle indeksi ortalama 18.96±3.85 kg/m2 olarak hesaplandı. İşlem süresi ortalama 750±130 sn bulundu. Hastaların 15'inin malnutrisyon sınırı olarak belirlenen 18.5kg/m2 lik vücut kitle indeksi değerinin altında olduğu ancak bu gurup ile beslenme problemi olmayan 9 hasta arasında işlem süresi açısından anlamlı farklılık olmadığı saptandı. İşlem sonrası bir hastada giriş yerinde enfeksiyon gelişti ve antibiyotik tedavisi verildi. Hastaların ortalama 17,3±9,2 aylık takiplerinde 2 hastanın yutma refleksindeki düzelmeye bağlı olarak gastrostomi kateterleri çekildi. İki hastada yıpranma sebebi ile kateter değişimi yapıldı. Altı hasta primer hastalığına bağlı olarak kaybedildi. Sonuç: Perkütan endoskopik gastrostomi oral yoldan beslenemeyen hastalar için ideal beslenme yoludur. Bu teknik ile gastrostomi açılması klasik cerrahi yönteme göre hem daha hızlı hem de daha güvenilirdir.

Percutaneous biliary balloon dilatation PBBD treatment is applied to the benign biliary stenosis since the early 80's. In this patient with cholangitis, hyperbilirubinemia and a history of partial cholecystectomy, PBBD treatment was applied to the stenosis unresponsive to endoscopic procedures due to adhesions. Herein, we present a case undergoing PBBD due to resistant stenosis with a review of the literature., Perkütan bilier balon dilatasyonu PBBD tedavisi benign patolojiler nedeniyle gelişen darlıklara 1980' li yıllardan itibaren uygulanan bir tedavi yöntemidir. Kolanjit şikayetleri ve hiperbilirübinemisi bulunan ve yapışıklıklar nedeni ile parsiel kolesistektomi operasyonu hikayesi bulunan olgunun endoskopi eşliğinde uygulanan işlemlere cevap vermeyen darlığına perkütan balon dilatasyonu ile tedavisi gerçekleştirildi. Dirençli darlığı bulunan olgumuzdaki PBBD tedavisini literatür ışığında sunmayı amaçladık.

The breast cancer is rare in pregnancy but it is the most frequent malignant tumor in pregnant women. The natural tenderness and enlargement of the breasts of pregnant and lactating women may hinder detection of discrete masses and therefore, early diagnosis of breast cancer may be delayed. It is a pathology that must be followed by obstetrics and gynecology, general surgery and oncology clinics. The term of the breast cancer associated pregnancy contains the breast cancer diagnosed during pregnancy or within one year after birth. The physical examination in these patients may pose a problem for the clinician because of the physiological changes during pregnancy and lactation. If the mass is detected during this period, it should be investigated by ultrasound and fine-needle aspiration biopsy. Diagnosis of the breast cancer in the pregnancy is diagnosed at a late stage. 25 years old patient admitted to the hospital because of a mass in her right breast being steadily growing at the end of the seventh month of her gestation. In her right breast a mass was determined with BRIADS 5 criteria by breast ultrasound and the result of the biopsy revealed invasive ductal carsinoma. First, the right modified radical mastectomy and axillary dissection was performed by general surgery, with obstetrics and gynecology's opinion. Then she was followed by oncology, obstetrics and gynecology clinics and gave birth by cesarean section in the 33rd week of pregnancy. Then, she was received adjuvant chemotherapy regimen., Meme kanseri gebelik sırasında nadir görülür ancak gebelikte en sık görülen kanserdir. Gebelikte ve laktasyon döneminde oluşan memelerdeki hassasiyet ve büyüme nedeni ile bu dönemde ortaya çıkan bir kitle ve dolayısı ile meme kanserinin tanısı gecikebilir. Kadın doğum, genel cerrahi ve onkoloji kliniklerinin birlikte takip etmeleri gereken bir patolojidir. Gebelikle ilgili meme kanseri terimi, gebelik sırasında ya da gebeliği takiben 1 yıl içinde görülen meme kanserlerini kapsamaktadır. Gebelik ve laktasyonda olan fizyolojik değişiklikler nedeniyle fizik muayene bu hastalarda klinisyen için problem oluşturabilir. Bu dönemde kitle saptanırsa ultrasonografi ve ince iğne aspirasyon biyopsisi yapılarak kitle araştırılmalıdır. Gebelikte meme kanseri tanısı ileri evrelerde konulmaktadır. 25 Yaşında G3P2 olan hasta son gebeliğinin yedinci ayında sağ memesinde gittikçe büyüyen kitle nedeniyle hastaneye başvurmuş. Yapılan meme ultrasonografisinde sağ memede BIRADS 5 kriterlerine uyan kitle tespit edilmiş ve yapılan biyopsi sonucuna göre invaziv duktal karsinom tanısı almıştır. Öncelikle kadın doğum uzmanı görüşü alınıp genel cerrahi tarafından sağ modifiye radikal mastektomi ve aksiller diseksiyon uygulandı. Operasyon sonrası medikal onkoloji ve kadın doğum uzmanlarının takibine alınan hasta 33. gebelik haftasında sezeryanla doğum yaptı ve adjuvan kemoterapi programına alındı.

Objective: To determine the need of medical therapy in asymptomatic, Helicobacter pylori positive antral gastritis patients after a triple eradication therapy. Material and methods: Gasatroscopically detected symptomatic antral gastritis patients were evaluated. 248 of 321 were Helicobacter pylori + .These patients were treated by amoxicilin 1 gr 2x1 clarithromycin 500 mg 2x1 for forteen days and pantoprazol 40 mg daily for two months. After the therapy 74 patients were still Hp + according to the stool analyzes and eradication was unsuccesfull.. 42 of those patients whom erradication therapy failed were completely asymptomatic. Asymptomatic patients were seperated into two groups. First group was treated by tetracycline 2x500mg and metranidazole 4x500 mg. The second group did not use any drugs and only dietary regulations were suggested. Six moths later both groups were controlled again. Results: The stool analyzes of 21 patients in the first group were repeated. Only 9 were positive but the symptoms reoccured in 9 4 Hp - patients. In the second group 10 patients were symptomatic after the dietary regulations. Conclusion: The need of second line medical therapy in asymptomatic, Hp positive antral gastritis patients after a triple erradication therapy is controversial, Amaç: Üçlü tedavi sonrası semptomları kaybolan ancak Helikobakter pilori Hp pozitifliği devam eden hastalara ek tedavi gereksinimini ortaya koymak. Yöntem: Kliniğimizde yapılan gastroskopilerinde yalnızca antral gastrit saptanan semptomatik 321 hastanın 248'inde biyopsi ile Hp + gösterildi. Hp + hastalara amoksisilin 1 gr 2x1 klaritromisin 500mg 2x1 14 gün boyunca ve pantoprazol 40 mg iki ay boyunca verildi. Tedavi sonrası yapılan gaita analizlerinde 74 hastada Hp eradikasyonunun başarısız olduğu görüldü. Eradikasyonun başarısız olduğu bu hastalardan 42'si semptomlarının tümü ile yok olduğunu ifade etti. Asemptomatik hastalar iki gruba ayrıldı. Birinci gruptakilere tetrasiklin 2x500mg ve metranidazol 4x500mg'lık ikinci basamak tedavisi verildi. İkinci gruptaki hastalara sadece beslenme önerilerinde bulunuldu. Her iki grup hasta 6 ay sonra tekrar kontrole çağrıldı. Bulgular: Birinci gruptaki 21 hastanın gaitada Hp analizleri tekrarlandı. Sadece 9 hastada pozitifliğin devam ettiği ancak bu gruptaki 9 dördü H p- hastada semptomların tekrar başladığı görüldü. Sadece diyet verilmiş olan ikinci gruptaki hastaların ise 10'u şikayetlerinin tekrarladığını ifade etti. Sonuç: Antibiyotik tedavisi sonrası semptomları düzelen ancak Hp pozitifliği devam eden hastalarda Hp eradikasyonu amacı ile ikinci basamak tedavi vermenin klinik yararı tartışmalıdır.

Aim: The breast biopsy results of lesions, which were assessed as BIRADS Breast Imaging Reporting and Data System category 3-5 in our clinic, are, evaluatedretrospectively. Thediagnostic valueof USguided breast biopsy is investigated and the results of the method in our clinic are interpreted with the current literature. Material and Methods: In this study, the fi ndings of 97 cases that underwent US guided breast biopsy at Radiology department in Maltepe University Hospital; during the time interval between September 2006 to May 2009 are presented. In this study, tru-cut biopsies with ultrasonography US guidance were performed to the breast lesions which were assessed as BI-RADS category 3 to 5 with US, magnetic resonance imaging MRI or mammographyandallof whichwere visiblein US. Thedimensions, localizations, BI-RADS categories, the number of attempts for sampling and the histopatological results of all lesions included in the study were documented. Results: Histopatological assessments revealed, 61 cases as benign %62,9 , 35 cases as malignant %36.1 , and 1 case as atypical ductal hyperplasia. 94 %96.9 of 97 cases have been diagnosed by tru-cut biopsy, and as a result the sensitivity and specifi city were found %96.9 and %100 accordingly. Although forsampling 4-6 meanaverage4.3 attempts applied to patients. Only in 3 cases a minor complication as ecchymosed occurred on the breast skin. Conclusion: For the histological diagnosis of breast diseases US guided tru-cut biopsy is not only faster and cheaper but also an easily tolerated and applicable method when compared to MRI guided biopsy or stereotactic biopsy or vacuum assisted core biopsy procedures. Gereç ve Yöntem: Bu çalışmada Maltepe Üniversitesi Tıp Fakültesi Radyoloji Anabilim Dalında Eylül 2006 ve Mayıs 2009 tarihleri arasında US eşliğinde tru- cutbiyopsisiuygulanan97 olgunun bulguları sunulmaktadır. Bu çalışmada US, mamografi ve manyetik rezonans görüntüleme ile BI-RADS kategori 3–5olarak değerlendirilen ve US'de görülebilenmeme lezyonlarına US eşliğinde tru-cut biyopsi yapıldı. Tüm lezyonların boyutları, lokalizasyonları, BIRADS kategorileri, yapılan girişim sayısı ve histopatolojik tanıları kaydedildi. Bulgular: Histopatoloijk değerlendirme sonucu toplam 61 olgu %62.9 benign, 35 olgu %36.1 malign ve 1 %1 olguda atipik duktal hiperplazi tanısı aldı. Toplam 97 olgunun 94'ünde %96.9 patolojik tanı tru-cut ile konulmuş olup metodunsensitivitesi%96.9, spesifitesi%100 bulundu. Örneklemeiçin olgulara 4–6 arasında girişim ortalama 4.3 yapılmasına rağmen sadece 3 olguda meme cildinde ekimoz şeklinde minör komplikasyon oluştu. Sonuç: Meme hastalıklarının tanısı için US rehberliğinde yapılan tru-cut biyopsi yöntemi; sterotaksive MRG rehberliğinde yapılan tru-cutveya vakum destek likör biyopsilerine göre sadece daha hızlı ve ucuz değil aynı zamanda kolaytolere edilebilen ve uygulanabilen bir tanı yöntemi olup son dönemlerde cerrahi biyopsinin etkin bir alternatifi haline gelmiştir.

The gastric cancer is rare in pregnancy. Gastrointestinal symptoms due to pregnancy such as nausea, vomiting and abdominal pain make difficult at the diagnosis of this disease. At diagnosis, the patient presents with the late stage tumors and this situation negatively affects prognosis. Early diagnosis and treatment increases the survival rate for gastric cancer. We can have successfull conclusions by the collaboration of the clinics such as gynecology, general surgery and medical oncology in the treatment of gastric cancer which requires both a difficult diagnosis and treatment during pregnancy. In this study, A 39 -year-old female patient who is determined in late stage gastric cancer is presented, although she had no gastrointestinal symptoms in pregnancy. The medical history revealed that she was treated for breast cancer 5 years ago . After the tests and treatments in the postoperative period, a positive response was taken to treatment., Gebelik sırasında mide kanseri oldukça nadir görülmektedir. Gebeliğe bağlı bulantı, kusma ve abdominal ağrı gibi gastrointestinal semptomlar hastalığın tanısını güçleştir¬mektedir. Tanı konduğunda ise hasta ileri evre tümör olarak karşımıza çıkmaktadır ve prognozu olumsuz yönde etkilemektedir. Mide kanserinde erken tanı ve tedavi sağ kalım oranını artırmaktadır. Gebelik sırasında tanı ve tedavisi oldukça güç olan mide kanserinin tedavisinde kadın doğum, genel cerrahi ve medikal onkoloji kliniklerinin ortak çalışmasıyla başarılı sonuçlar alınabilir. Bu çalışmada gebeliğinde hiçbir gastrointestinal şikayeti olmadığı halde ileri evre mide kanseri tespit edilen 39 yaşında bayan hasta sunulmuştur. Hastanın özgeçmişinde 5 yıl önce meme kanseri nedeniyle tedavi gördüğü öğrenildi. Postoperatif dönemde yapılan tetkik ve tedaviler sonrasında tedaviye tam yanıt alındı.

The aim of this study is to determine the role of midazolam used as a sedative agent in upper gastrointestinal system endoscopy. We evaluated the results of 600 patients. All of them were medicated by 0.1 mg/kg iv midazolam preoperatively. The duration of the procedure,basal and postgastroscopic pulse and 02 saturation, anxiety score and the sedation score of the patients were noted. Five hundred and fifty two of the patients were willing to undergo the same procedure if needed. Our study suggests that the use of midazolam in upper gastrointestinal system endoscopy makes the procedure easier and more comfortable both for the patient and the endoscopist., Bu çalışmada tanısal amaçlı üst gastrointestinal sistem endoskopisi yapılan hastalarda işlem öncesi sedasyon amaçlı sistemik midazolam uygulanmasının etkileri incelendi. Çalışmaya Maltepe Üniversitesi Genel Cerrahi Anabilim Dalı Endoskopi Ünitesinde 2007 Ocak- 2009 Nisan tarihleri arasında tanısal amaçlı üst gastrointestinal sistem endoskopisi yapılan 600 hasta alındı. İşlem öncesi hastalara 0,1mg/kg iv midazolam verildi.Tüm hastalarda pilor geçildi ve duodenum 1 kıta görüldü. Hastaların bazal, sedasyon sonrası ve işlem sonrası nabız ve oksijen saturasyonu, işlem süresi,anksiyete skoru ve hastanın işlem sırasındaki sedasyonu durumu endoskopist tarafından değerlendirildi. Altı yüz hastadan 562'si tıbbi gereklilik olması halinde tekrar üst gastrointestinal sistem endoskopisi yaptıracaklarını beyan ettiler.Üst gastrointestinal sistem endoskopisinde midazolam kullanılmasının, gerek işlemin endoskopist tarafından rahatça gerçekleştirilmesi, gerekse hastanın konforunun sağlanması ve daha sonraki endoskopik işlemler öncesi gelişecek anksiyetenin önüne geçilmesi için gerekli ve güvenli bir yöntem olduğu sonucuna varıldı.

Aim: Laparoscopic cholecystectomy LC is the most frequently performed elective surgery for symptomatic cholelithiasis. Trocar site pain after LC causes increased stress, morbidity and hospital stay. Consumption of intravenous opioid and nonopioid analgesics for control of postoperative pain is common. Intravenous analgesics have also side effects. Pain control with local anaesthetic infiltration can reduce intravenous analgesics consumption and postoperative stress. The aim of this study is point the efficiencies of local anaesthetic infiltration after LC. Methods: A total of 100 patients with ASA Class 1-2 were included to the study. All of the patients underwent to standard LC. Patients were divided into two groups of 50 each. Group-1 consisted of local bupivacaine enjected 50 patients and group-2 was consisted of non-local enjected 50 patients, after LC. Two groups were compared with the Mann-Whitney U test. A total of 10 cc sterile 1% bupivacaine infiltration was performed each patient in group-1. The 4th and 12th hour visual analogue scale VAS scores, were recorded. SPSS 15.0 program was used for statistical evaluation. Results: In group-1, the mean VAS score was 2 in the fourth hour and 3.4 in the twelfth hour; in group-2 the fourth hour was 5.1 and the twelfth hour was 4.6. The mean VAS score in group-1 was statistically lower p, Amaç: Laparaskopik kolesistektomi LK , semptomatik kolelitiazis için en çok uygulanan cerrahi yöntemdir. LK sonrası trokar giriş yeri ağrısı ameliyat sonrası dönemde stres ve morbiditede artışa, hastanede kalış süresinde uzamaya neden olmaktadır. Ameliyat sonrası ağrı kontrolünde intravenöz opioid ve non-opioid analjezik kullanımı yaygındır. Bununla birlikte intravenöz ağrı kesici kullanımının yan etkileri de mevcuttur. Lokal anestezik infiltrasyonu ile ağrı kontrolü intravenöz ağrı kesici kullanımını ve ameliyat sonrası stresi azaltabilir. Bu çalışmanın amacı LK sonrası yapılan lokal anestezik infiltrasyonunun etkinliğini göstermektir. Materyal ve Metod: Çalışmaya ASA skoru 1-2 olan 100 hasta dahil edildi. Tüm hastalara standart LK uygulandı. Her biri 50 hastadan oluşan 2 ayrı grup oluşturuldu. İki grup Mann-Whitney U testi kullanılarak karşılaştırıldı. Grup-1 LK’ den sonra kesi yerlerine lokal bupivakain enjekte edilmiş 50 hastadan, grup-2 ise lokal anestezik enjekte edilmemiş 50 hastadan oluşmaktaydı. Grup-1’ deki her hastaya 10 cc %1’ lik bupivakain infiltrasyonu uygulandı. Dördüncü. ve 12. saat vizüel analog skala VAS skorları kaydedildi. İstatistiksel değerlendirme için SPSS 15.0 programı kullanıldı. Bulgular: Grup 1’ de ortalama VAS skoru dördüncü saatte 2, onikinci saate 3.4 bulunurken; grup 2’ de dördüncü saat değeri 5.1 ve onikinci saat değeri 4.6 olarak bulundu. Mide içeriğinin retrograd olarak özofagusa geçmesine gastroözofageal reflü denir. Gastroözofageal reflü hastalık olmayıp, fizyolojik bir olaydır. Pirozis, regürjitasyon ve yutma güçlüğü gibi klinik belirtilere, alt özofagusta erozyon, stenoz, ülser ve metaplastik epitel gibi komplikasyonlara yol açtığı zaman gastroözofageal reflü hastalığından bahsedilir. Gastroözofageal reflü hastalığı tedavisinde; yaşam düzeninde değişiklikler, antiasitler, proton pompa inhibitörleri, histamin 2 reseptör blokerleri gibi medikal tedaviler ve ileri evrelerde ise Nissen fundoplikasyonu gibi çeşitli cerrahi girişim seçenekleri bulunmaktadır.Çalışmamızda; gastroözofageal reflü ve takiben özofajit oluşturduğumuz deneysel modelde; yara iyileşmesi üzerine olumlu etkileri ve antioksidan etkinliği bulunan whey proteinlerinin yararlarını göstermeyi amaçladık. Bu amaçla ratlarda mide fundusunun ligasyonu ve pilorun parsiyel daraltılması şeklinde sıklıkla yapılan bir reflü modeli kullandık.Deneysel çalışmamızda; 32 Winstar-albino cinsi rat randomize olarak eşit sayıda (n: 8) 4 gruba ayrıldı; 1. gruba laparotomi yapılıp, reflü oluşturmadan standart yem verildi; 2. grup reflü modeli oluşturulmasını takiben standart yemle beslendi; 3. grup ise aynı şekilde reflü modeli oluşturulmasını takiben whey proteini ile beslendi. 4. grup ise laparotomi yapılmaksızın whey proteini ile 3 hafta süresince beslendi. Deney sonunda kan örnekleri ve özofagus ½ distal kısmı ile mide girişinden oluşan doku örnekleri histopatolojik ve biyokimyasal inceleme için alındı.Özofagus distal kısmının ve mide girişinin histopatolojik incelemesinde; reflü oluşturulan gruplara ait özofagus ve mide preparatlarında, kontrol gruplarına göre özofajiti ve gastriti destekleyen bulgular gözlenmiş olup, reflü oluşturulması sonrası standart yemle beslenen gruba kıyasla, whey proteinle beslenen grupta doku hasarının daha az olduğunu saptadık. Ayrıca, dokuların biyokimyasal analizinde; reflü oluşturulan gruplardaki özofagusun oksidatif hasarının göstergeleri olan MDA ve katalaz düzeyleri diğer iki kontrol grubu hayvanlarına göre istatistiksel olarak anlamlı düzeyde artış gösterdi. Yüksek MDA ve katalaz düzeyi, çalıştığımız dokularda belirgin bir inflamasyon ve hasar olduğunu göstermektedir. Reflü oluşturulan gruplar kendi aralarında karşılaştırıldığında ise; whey proteini ile beslenen grubun MDA ve katalaz düzeylerinin, standart yemle beslenen grubun aksine daha düşük kaldığı saptandı. Yani whey proteini ile beslenen ratlarda antioksidan etkinliğin artmış olduğunu saptadık. Kan serumunun biyokimyasal analizinde reflü oluşturulan gruplarda laktat düzeyi belirgin artış gösterdi. Reflü oluşturulan grup 2 ve grup 3 karşılaştırıldığında ise whey proteini ile beslenen ratların serum laktat düzeyleri anlamlı farklılık göstermedi.Histopatolojik ve biyokimyasal sonuçlarımız; genel olarak, whey proteinleri ile beslenmenin doku inflamasyonunu ve özofajit gelişimini azaltıcı etkileri olduğunu göstermiştir. Serbest oksijen radikallerini, reflü özofajit gelişimde en önemli mediyatörler olarak gösteren çeşitli literatür bilgileri ışığında, whey proteinlerinin yüksek antioksidan etkinliğinin doku hasarını azalttığı ve yara iyileşmesi üzerine olumlu etkileri olduğunu göstermiş olduk. Sonuç olarak; antireflü tedavide kullanılan, medikal tedavi seçeneklerinden biri olan antisekretuar tedavinin yanında, antioksidan ilaçların da kullanımının yararlı olacağını düşünmekteyiz.Anahtar Kelimeler: gastroözofageal reflü, gastroözofageal reflü hastalığı, özofajit, whey proteini, antioksidan Gastroesophageal reflux is defined as the retrograde movement of the stomach content back to the esophagus. Gastroesophageal reflux is not a disease, it is a physiological event. When clinical symtoms, such as difficulty in swallowing, heartburn, regurgitation and complications such as the lower esophageal erosion, stenosis, ulcers and metaplastic epithelium develops; gastroesophageal reflux disease is mentioned. In treatment of gastroesophageal reflux disease; medical treatment, changes in the lifestyle, intake of antiacids, histamine 2 receptor blockers, proton pump inhibitors and, several surgical operations such as Nissen fundoplication are used in advanced stage. In our study, gastroesophageal reflux was surgically created, gastric fundus ligation and partial narrowing of pylor was used as a model. Positive effects of whey protein on wound healing and antioxidant activity was detected by various laboratory parameters.In our study; 32 Winstar-albino rats was randomised into 4 groups (n: 8). Rats in the first and second groups were normally feed and a simple laparotomy for the first and a reflux procedure was performed for the second group, rats in the third group were feed by enriched whey protein and reflux procedure was performed and, rats in the fourth group were feed by enriched whey protein alone during 3 weeks. At the end of the procedure, blood samples and distal portion of the esophagus and proximal portion of the stomach were withdrawn for histopathological and biochemical investigation.In histopathological investigation of distal portion of the esophagus and proximal portion of the stomach; esophagitis and gastritis were observed in groups 2 and 3, compared to groups 1 and 4. We detected less tissue damage in group 3 compared to group 2. Also, in the biochemical analysis, MDA and catalase levels in groups 2 and 3 were increased significantly compared to groups 1 and 4.When the groups 2 and 3 were compared, MDA and catalase levels were decreased in the whey protein group. Thus, we obtained an increased activity of antioxidant status in rats feed by enriched whey protein. Lactate level was significantly increased in groups 2 and 3.Generally, our histopathological and biochemical results demonstrates that feeding by whey protein is positively effective on reflux models condition and diminishes esophagitis. In the literature; free oxygen radicals are most important mediators on development of reflux esophagitis, we demonstrated that the high antioxidant capacity of whey proteins decrease the tissue injury and facilitate the recovery. Amaç: Meme ağrısı şikayeti ile polikliniğe başvuran hastalarda depresyon ve anksiyete skorlaması yapmak ve antidepresan kullanımının mastodini tedavisi üzerindeki etkinliğini belirlemek.Yöntem: Genel Cerrahi Kliniğine Kasım 2009-Mart 2010 tarihleri arasında meme ağrısı şikayetiyle başvuran, anamnez, fizik muayene ve radyolojik tetkiklerinde patoloji saptanmayan 80 kadın hasta çalışmaya dahil edildi. Tüm hastalar başlangıçta ayrıntılı anamnez alınmasını takiben muayene edildi ve Belirti tarama testi (SCL-90-R), Hamilton Depresyon Derecelendirme Testi (HAM-D), Hamilton Anksiyete Değerlendirme Testi (HAM-A) ve Görsel Analog Ağrı Skalası (VAS) anketleri uygulandı. HAM-D ölçeği 10 puanın üzerinde olan hastalar psikiyatrik değerlendirmeye alındı ve depresyon tanısı konan 32 hastaya 3 ay süre ile Venlafaxine 75 mg/gün başlandı. Depresyon tanısı almayan 48 kişi ise kontrol grubunu oluşturdu. Hastalarla 3 aylık ilaç tedavisi ve takip süresi boyunca her ay bir kez görüşüldü ve Belirti tarama testi, Hamilton Depresyon derecelendirme testi , Hamilton Anksiyete Değerlendirme Testi ve Görsel Analog Ağrı Skalası anketleri tekrar uygulandı. Sonuçların karşılaştırılmaları SPSS 12.0 istatistik programı ile independent samples T-test kullanılarak yapıldı.Bulgular: Bu çalışmaya yaşları 16 ile 48 (ortalama 37,05 ± 7,31 yaş) arasında değişen 80 kadın hasta dahil edildi. Hastaların tümü pre-menopozal dönemdeydi. Kadınların 33' ü 35 yaş ve altı, 47 si 35 yaş üstündeydi. Ağrı 51 hastada tek memede, 29 hastada her iki memede lokalizeydi ; 22 hastada sağ memede, 29 hastada sol memedeydi; 51 hastada (% 63,8) siklik, 29 hastada (% 36,3) nonsiklik karakterde ağrı mevcuttu. Siklik mastodinisi olan kadınların yaş ortalaması 37,3 iken nonsiklik mastodinisi olan kadınların yaş ortalaması 36,1 idi.Radyolojik inceleme sonucunda 41 hasta BİRADS 1, 20 hasta BİRADS 2, 19 hasta BİRADS 3 kategorisinde yer aldı. 25 hasta ağrının koltukaltı, kol ve boyuna doğru yayıldığını ifade ediyordu, 55 hastada ise yalnız memede ağrı mevcuttu.68 hastanın fizik muayenesi normal olarak değerlendirildi; 12 hastada ele gelen kitle saptandı. Lokalizasyona göre meme ağrısının dağılımına baktığımızda ise retroareolar bölgede ağrısı olan 9, tek kadranda ağrısı olan 22 hasta, yaygın ve bir bölgeden daha fazla ağrısı olan 49 hasta tespit edildi. Yapılan psikiyatrik değerlendirmeler sonucunda 32 hastaya depresyon tanısı konarak anti-depresan tedavisine başlandı. Depresyon tanısı konmayan 48 hastaya ise klinik gözlem uygulandı.Hastalar 3 ay sonra tekrar değerlendirildiğinde;Antidepresan ilaç kullanan 32 kişiden 22 sinin ağrısının geçtiği,10 kişinin ise ağrısının geçmediği; ilaç kullanmayan 48 kişilik grupta ise 16 kişinin ağrısının kendiliğinden geçmesine rağmen 24 kişide ağrının geçmediği görülmüştür. Depresyon tanısı alan ve antidepresan ilaç başlanan 32 kişiden 26 sının depresyonu geçmesine rağmen 6 kişide depresyonun devam ettiği görülmüştür.Depresyonu geçen 26 kişinin 6 sında meme ağrısı devam ederken 20 sinde meme ağrısı geçmiştir.Depresyonu geçmeyen 6 kişinin 3 ünde meme ağrısı devam ederken, 3 hastada meme ağrısı şikayeti geçmiştir. Bir başka deyişle depresyon saptanmayan hastaların %66,6'sınde ağrı devam ederken, depresyon saptanan hastaların %31.2'sinde ağrının devam ettiği görüldü, ve aradaki fark istatistiksel olarak anlamlı bulundu (p< 0.05).Bu çalışmada ağrılı tek yada iki taraflı olmasıyla ağrının remisyonu arasında anlamlı ilişki saptanmadı. Bilateral meme ağrısı olan 29 hastadan ağrısı geçen 13 kişi (%44,8) iken tek taraflı meme ağrısı bulunan 51 hastadan ağrısı geçen 24 kişi (%47) idi.Çalışmamızda nonsiklik meme ağrısı olan kadınların % 44,8 inde ağrı devam ederken, siklik meme ağrısı olanlarda % 56,8 inde ağrı devam etmekteydi.Ağrının koltukaltına yayılımıyla ağrının remisyonu arasında anlamlı ilişki saptanmadı. Koltukaltına yayılan meme ağrısı bulunan 24 hastadan ağrısı geçen 12 kişi (%50) iken koltukaltına yayılımı olmayan meme ağrısı olan 56 hastadan ağrısı geçen 26 kişi (%46) tespit edildi.Sonuç: Mastodini, depresyon ile yakın ilişkilidir. Mastodini yakınmasıyla gelen hastalarda, eğer klinik şüphe varsa mutlaka tanı koyularak depresyon tedavisi de başlanılmalıdır. Semptomatik tedavinin yanı sıra hastaların iyi bir psikiyatrik değerlendirmeye tabi tutulması ve gerek görüldüğü durumlarda düzenli bir antidepresan kullanımı mastodini tedavisinde yüz güldürücü sonuçlar vermektedir. Aim: To access the factors affecting mastodini in patients who were admitted to the clinic with complaints mastodini and research the efficiency of antidepressant treatment by clinical evaluation and measuring depression and anxiety.Method: 80 patients who were admitted to Maltepe University Medical Faculty General Surgery Clinic between November 2009-March 2010 with mastodini and who don't have any pathology by anamnesis, physicial examination and radyological investigation included to this study. Detailed history has taken from all patients and then they have been examined. Symptom screening test (SCL-90-R), Hamilton depression evaluation test (HAM-D), Hamilton anxiety evaluation test (HAM-A) and visual analog scale questionnaire(VAS) were applied to all patients. Ones on the HAM-D scale over 10 points were taken into psychiatric evaluation and 32 patients who were diagnosed as depression started to use Venlafaksin 75mg/day for 3 months. 48 patients who weren't diagnosed as depression constituted control group. During 3 montly medical treatment and observation time all patients were seen every month and Symptom screening test (SCL-90-R),Hamilton depression evaluation test (HAM-D), Hamilton anxiety evaluation test (HAM-A) and visual analog scale questionnaire were applied again. All this statictical evaluation made by using SPSS 12.00 package programme and staticticaly meaningful value admitted at least 0.05. Firstly Statisticaly frequency and dissociation of variables were found. To determine the difference between the variables Independent Samples T ? Test and to determine the first and last test evaluation Paired Samples T ? Test were applied. Results were interpreted with tables.Findings: 80 female patients between age of 16-48 (mean 37,05 ± 7,31 age) were included to this study. All patients were in premenopausal term. 33 women were younger than 35, 47 women were older than 35. 51 patients have unilateral mastodini, 29 patients have bilateral mastodini; 22 patients have mastodini at right breast, 29 patients have mastodini at left breast; 51 patients (63,8%) have siclic mastodini, 29 patients (36,3%) have nonsiclic mastodini. While mean age who have siclic mastodini was 37,3 mean age who have nonsiclic mastodini was 36,1.As a result of radiological investigation 41 patients were BIRADS 1, 20 patients were BIRADS 2, 19 patients were BIRADS 3. 25 patients decribed pain localization along armpit, arm and neck, 55 patients described pain localization only in breast.Physical examination was normal at 68 patients; and 12 patients had a mass which was determined by physical examination. According to localization of mastodini dissociation in 9 patients retroareolar zone, in 22 patients single quadran pain, in 49 patients widespread and more than one quadran pain were determined. As a result of psychiatric evaluation depression diagnosed in 32 patients and antidepression treatment was started. 48 patients who didnt have depression were clinically observed.When we evaluate patients after 3 months;22 of 32 patients who were using antidepression drug mentioned that their pain come to an end, 10 of 32 patients who were using antidepression drug mentioned that their pain still existed. Although 16 of 48 patients who weren't using drug mentioned that their pain come to an end automatically, rest of 24 patients mentioned that their pain still existed.Although 26 of 32 patients who had depression and been using antidepression drug mentioned that depression recovered, 6 of 32 patients mentioned that depression still existed. 6 of 26 patients whose depression recovered still had mastodini, 20 of 26 patients whose depression recovered didn't have mastodini any more. 3 of 6 patients who had depression still had mastodini, 3 of 6 patients who had depression didn't have mastodini any more. In other words 66,6% of patients who didn't have depression had continuous mastodini, 31,2 % of patients who had depression didn't have mastodini any more. And the difference was staticticaly meaningful (p< 0.05).In this study, any relation between unilateral or bilateral mastodini and pain remission weren't determined. 13 of 29 patients (44,8%) who had bilateral mastodini didn't have mastodini any more, 24 of 51 patients (47 %) who had unilateral mastodini didn't have mastodini any more. In our study, in 44,8% of patients who had nonsiclik mastodini still had mastodini, and in 56,8% of patients who had siclic mastodini still had mastodini. We didn't determine meaningful relationship between dissociation of pain to armpit and pain remission.12 of 24 patients (50%) who had mastodini in the direction of armpit didn't have mastodini any more, 26 of 56 patients (46%) who didn't have mastodini in the direction of armpit didn't have mastodini any more.Results: Mastodini has a close relationship with depression. If there is clinical suspect in patients who have mastodini, depression must be diagnosed and depression treatment must be started. Beside symptomatic treatment, patients should be evaluated psychiatricly and if needed regular antidepression usage has positive outcomes in mastodini treatment. 132

Purpose: Oxygen radicals which are produced during reperfusion phase play a central role for hepatocellular damage during ischemia/reperfusion (I/R) injury. Antioxidant strategies appear a promising approach to prevent I/R injury. In the present study, the effect of glutathione (GSH) preconditioning established by whey protein feeding on hepatocellular injury markers was assessed in a rat hepatic normothermic I/R injury model. The interaction between intracellular GSH content and heme oxygenase-1 (HO-1) enzyme expression was also determined in order to understand the level of upgrading of HO-1 system in a high GSH content environment after preconditioning.
