Akut pyelonefrit geçirmiş 0-5 yaş arası çocuklarda renal skar tanısında prokalsitoninin prediktiv değeri Amaç: Akut pyelonefrit (APN) geçirmiş çocuklarda prokalsitonin yüksekliğinin nükleer görüntülemeye gerek kalmadan böbrekte pyelonefrite bağlı oluşabilecek erken böbrek hasarının teşhisini ve geç dönemde oluşabilecek geri dönüşümsüz böbrek hasarının önlenmesini incelemeyi amaçladık.Gereç ve Yöntem: Çalışma Haziran 2015-Haziran 2016 tarihleri arasında Ankara Üniversitesi Tıp Fakültesi Cebeci Çocuk Hastanesi akut pyelonefrit tanısıyla yatarak tedavi görmüş 30 hasta grup 1, idrar yolu enfeksiyonu tanısıyla izlenen 30 hasta grup 2, ayrıca kontrol grubu olarak sağlıklı gönüllülerden oluşan 30 hasta grup 3 olmak üzere toplam 90 hasta çalışmaya dahil edildi.Her üç grupta ilk tanı anında, tanıdan 3 ay sonra ve 6 ay sonra eritrosit sedimantasiyon hızı (ESH) , C-reaktiv protein (CRP), prokalsitonin (PCT) ve tam kan bakıldı. Grup 1'de ek olarak ilk tanı anında Böbrek Kortikal Sintigrafisi (Tc-99m DMSA Sintigrafisi) yapıldı ve tanıdan 6 ay sonra geç dönemde oluşabilecek böbrek hasarının teşhisi için Böbrek Kortikal Sintigrafisi (Tc-99m DMSA Sintigrafisi) tekrarlandı. Bulgular: Toplam 90 hasta çalışmaya dahil edilmiştir.Bu hastalardan 30'u APN tanısıyla hospitalize edilmiştir.Bunlardan 48 erkek (%53.3) ve 42 kız (%46.7) hasta olmuş.İlk tanı anında PCT,CRP ve ESH için gruplara baktığımızda:- PCT için ortalama değer APN grubunda 0,58 (0,23-1.1) , İYE grubunda 0,27 (0,13-0.5) ve kontrol grubunda 0,11 (0,05-0,21) bulunmuş. CRP için ortalama değer APN grubunda 46 (10,7-165,9) , İYE grubunda 34,95 (5,7-121,9), Kontrol grubunda 0,9 (0,1-33,6) olmuş. ESH içn ortalama değer APN grubunda 35 (14-98),İYE grubunda 37,5(16-72),Kontrol grubunda 7,5 (1-19) olmuş.3.ayda görülen PCT,CRP ve ESH için gruplara baktığımızda:- PCT için ortalama değer APN grubunda 0,31 (0,12-0,55) ,İYE grubunda 0,16 (0,12-0,31) , Kontrol grubunda 0,11 (0,1-0,11) olmuş. CRP için ortalama değer APN grubunda 22,45 (4-103,9),İYE grubunda 16,15 (3,10-76,80), Kontrol grubunda 0,62 (0,1-13,7) bulunmuş. ESH için ortalama değer APN grubunda 17 (8-78),İYE grubunda 14,5 (8-34),Kontrol grubunda 5 (1-14) olmuş.6.ayda görülen PCT,CRP ve ESH için gruplara baktığımızda:-PCT için ortalama değer APN grubunda 0,13(0,12-0,17),İYE grubunda 0,11 (0,1-0,11),Kontrol grubunda 0,11(0,1-0,11) bulunmuş. CRP için ortalama değer APN grubunda 3,95 (0,1-24,9),İYE grubunda 3,85(0,4-12,7),Kontrol grubunda 3,6(0,3-12,5) olmuş. ESH için ortalama değer APN grubunda 8 (1-31),İYE grubunda 5,5(1-18),Kontrol grubunda 5(1-14) olmuş. Tüm gruplarda ilk başvuru anında,3.ay ve 6.ayda görülen tam kan sonuçları arasında statistikel olarak anlamlı fark olmamıştır.APN grubunda ilk tanı anında çekilen DMSA sintigrafide hiçbir hastada renal skar saptanmamıştır.Fakat,tanıdan 6 ay sonra geç dönemde APN grubunda çekilen DMSA sintigrafisinde toplam 26 hastada renal skar saptandı ve bunlardan 19 hasta kız,7 hasta erkekti,4 hastda ise renal skar izlenmedi.Sonuç: APN grubuna baktığımızda prokalsitonin için cut-off değer 0.42 ng/mL olarak alındığında değer altında kalan 0.972 oldu. APN grubu için spesifite %96.7,sensifite ise %86.7 olarak olduğu görüldü.Ayrıca APN grubunda CRP için cut-off değer 22 mg/ml olarak alındığında, bu grup için sensivite %73,3, spesifitesi ise % 66,7 olmuştur.Bu sonuçlara dayanarak serum PCT düzeyinin renal parankimal hasarı göstermede CRP'ye göre daha hassas bir parametre olabileceğini düşünmekteyiz.Anahtar sözcükler: Akut pyelonefrit, C-reaktiv protein, Prokalsitonin, Tc-99m DMSA Sintigrafisi ABSTRACTPredictive value of serum procalcitonin in the diagnosis of renal scarring in children aged 0-5 years with acute pyelonephritisPurpose : İn this study we aimed to assess of procalcitonin elevation in children with acute pyelonephritis may occur depending on pyelonephritis the diagnosis of early kidney damage prevention and investigation of irreversible kidney damage in later periods without nuclear imaging.Materials and Methods: Between June 2015-June 2016 in Ankara University Medical Faculty ,in Cebeci Children's Hospital children who have received treatment with the diagnosis of acute pyelonephritis in 30 patients in group 1, with the diagnosis of urinary tract infection in 30 patients in group 2, as well as a control group consisting of healthy volunteers in 30 patients in group 3, for a total of 90 patients included in this study . İn all three groups, in the first of diagnosis , 3 months after diagnosis and 6 months after diagnosis, investigated the blood values of erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), procalcitonin (PCT) and complete blood counts.Addition in Group 1 the first time of diagnosis was performed renal cortical scintigraphy (Tc-99m DMSA scintigraphy) and 6 months after diagnosis to expose the late kidney damage the renal cortical scintigraphy (Tc-99m DMSA scintigraphy) were repeated.Results: A total of 90 patients included in this study . 30 patients hospitalized with the diagnosis of APN . Of these patients 48 patients were male (53.3%) and 42 female (46.7%) respectively.At the first time of diagnosis when we look at this groups for the PCT, CRP and ESR:- The mean value for PCT was 0.58 (0,23-1.1) in APN group, in UTİ group 0.27 (0,13-0.5) and 0.11 in the control group (0.05- 0.21) were found. The mean value for CRP was 46 (10.7 to 165.9) in APN group, 34.95 in the UTİ group (5.7 to 121.9), 0.9 in the control group (0.1-33.6), respectively. The average value of ESR was 35 (14-98) in APN group,in the UTİ group 37.5 (16-72) in the control group 7.5 (1-19) respectively.3 months after diagnosis when we look at this groups for the PCT, CRP and ESR:- The mean value for the PCT was 0.31 (0.12 to 0.55) in APN group, in the UTİ group 0.16 (0.12-0.31), in the control group 0.11 (0.1- 0.11) . The mean value for CRP was 22.45 (4-103.9) in APN group ,in the UTİ group was 16.15 (3.10 - 76.80), 0.62 in the control group (0.1 to 13.7) were found. The mean value for the ESH was 17 (8-78) in APN group,in the UTİ group was 14.5 (8-34) in the control group 5 (1-14) respectively.6 months after diagnosis when we look at this groups for the PCT, CRP and ESR:The mean value for the PCT was 0.13 (0.12 to 0.17) in APN group , in the UTİ group was 0.11 (0.1-0.11), 0.11 in the control group (0.1- 0.11) were found . The mean value for CRP was 3.95 (0.1 to 24.9) in APN group ,in the UTİ group was 3.85 (0.4-12.7), in the control group was 3.6 (0.3 to 12.5), respectively. The mean value for the ESH was 8 (1-31) APN group ,in the UTİ group was 5.5 (1-18), in the control group was 5 (1-14) respectively.In all groups, in the first admission, 3 months and 6 months after diagnosis there is no statistically significant difference between the results of complete blood counts.İn APN group at the first time of diagnosis, were not detected renal scarring in any patient which is performed DMSA scintigraphy. But, 6 months after diagnosis whom performed DMSA scintigraphy a total of 26 renal scarring were indentified in APN group. 19 of these patients were female, 7 male patients, and in 4 patients was not observed the renal scarring.Conclusion: Looking at the APN group if the cut-off value for procalcitonin is taken as 0.42 ng/ mL, the remaining value was under 0972. For the APN group specificity was seen 96.7% but the sensifity was 86.7%.Also in APN group, if the cut-off value is taken for CRP as 22 mg / ml , the sensitivity was 73.3% , but specificity was 66.7%.Based on these results , we think that the serum PCT levels a more sensitive parameter according to CRP in the diagnosis of renal parenchymal damage.
