Giriş ve Amaç: Bilateral ve unilateral balon kifoplasti operasyonu geçirmiş torakolomber vertebra fraktürlü hastaların klinik ve radyolojik sonuçlarını karşılaştırmakMateryal Metod: Ocak 2016 ile Aralık 2018 tarihleri arasında İzmir Kâtip Çelebi Üniversitesi Atatürk Eğitim ve Araştırma Hastanesi Beyin ve Sinir Cerrahisi Kliniğinde balon kifoplasti işlemi yapılan hastalardan telefonla çağırılıp poliklinik kontrolüne gelen toplam 77 hasta çalışma grubuna alındı. Giriş ve Amaç: Bu çalışma osteoporoz ve tümör nedenli vertebra korpus fraktürleri olan hastalara yapılan perkütanöz vertebroplasti (PVP) veya balon kifoplastiden (BKP) sonra preoperatif ve postoperatif klinik ve radyolojik sonuçlarını değerlendirmek için yapılmıştır. Omurga fraktürlerinde konservatif ve cerrahi metodlar tedavide tek başına ya da birlikte kullanılabilirken, tedavide amaç anatomik redüksiyon sağlamak, spinal deformiteyi önlemek, ağrıyı azaltmak, vertebra yüksekliğini yeniden sağlamak ve erken mobilizasyon ile hastanın günlük aktivitelerine geri dönmesini sağlamaktır.Gereç ve Yöntem: Çalışmada 2011-2016 yılları arasında Çanakkale 18 Mart Üniversitesi Beyin ve Sinir Cerrahisi Kliniği' nde perkütan vertebroplasti ve balon kifoplasti uygulanan toplam 65 hasta değerlendirmeye alındı. Olgular tedavi amacıyla kliniğe yatırıldıklarında Visual Analog Skala (VAS), Oswestry Dizabilite Sorgulaması (ODS) değerleri kaydedildi. Bu çalışmaya alınan 65 olgunun 44'i kadın (%67,7), 21'i erkek (%32,3) olup, PKP/BKP işlemi uygulanmıştır ve bu hastaların 63' ü osteoporotik, 2'si tümör nedeniyle gelişen vertebra kompresyon kırıklarıydı. Olguların radyolojik değerlendirmeleri ise T1, T2, STIR sekans torakolomber MRG, BT ve radyografi ile preoperatif ve postoperatif olarak değerlendirildi. İşlem yapılan 65 seviyenin dağılımı; Th8 seviyesine 1 (%1,5), Th10 seviyesine 3 (%4,6), Th11 seviyesine 4 (%6,2), Th12 seviyesine 19 (%29,2), L1 seviyesine 11 (%17), L2 seviyesine 13 (%20), L3 seviyesine 7 (%10,8), L4 seviyesine 6 (%9,2), L5 seviyesine 1 (%1,5) seviye şeklindeydi.Bulgular: Olguların başlıca semptomu ağrı olup PVP/BKP ile ortalama VAS skorları; 7,9' dan 2' ye gerilemiştir (p0,05), verilen sement miktarı ile ağrı iyileşmesi arasında belirgin korelasyon saptanmıştır (p0.05).viiConclusion: Kyphoplasty is a reliable and effective method in pain relief, providing vertebral body height restoration and kyphosis correction. This study revealed that the more amount of the injected cement, the higher rate of the cement leakage. The current study also demonstrated that there was no correlation between the amount of the cement and kyphosis correction rate. 112

Vertebral compression fracture is a significant problem associated with pain and functional impairment. Traditional open surgery has been problematic in the management of patients in this population because of poor bone quality, frequent and extensive medical comorbidity. Nonoperative management, including medications for pain relief and bracing, has been used traditionally; however, many patients are left with incapacitating residual pain and are unable to return to their previous level of activity. Limitations in the management of vertebral compression fractures have provided impetus for the development and refinement of percutaneous treatment methods suh as vertebroplasty and kyphoplasty. Vertebroplasty is a percutaneous procedure in which polymethyl methacrylate (PMMA) is injected into the fractured vertebral body. Percutaneous vertebral body augmentation with vertebroplasty has now gained widespread acceptance for the treatment of osteoporotic vertebral body fractures, as well as a number of other pathologic conditions. Principally used for their effectiveness in pain relief and subsequent acceleration of patients’ functional status, they have proved to be significant advancements in the treatment of patients with osteoporotic compression fractures. The most common complications associated with these procedures are pain and tenderness at the site of needle insertion, extravasation of cement and its secondary effects. Aim: Vertebroplasty (VP) and kyphoplasty (KP) are minimally invasive methods for the treatment of vertebral compression fractures (VCF). In this study, we aimed to present our experience with VP and KP performed under local anesthesia and percutaneously. Material and Methods: Between 2002 and 2008, 91 patients (total 112 vertebrae) with VCF due to osteoporosis, tumor and trauma underwent VP or KP. After radiological and routine evaluations, all patients were operated under local anesthesia. Mean operation time was 20 minutes per vertebrae. We never stopped the operation because of intolerance of any patient and all of them were discharged on the day of operation or the next day except one. Results: All patients had severe back pain. Mean preoperative Visual Analogue Scale (VAS) score for pain was 8.3 and the mean postoperative value was 2.7. Mean anterior wall and mid-vertebral body heights increased by 32% and 43%, respectively. The mean kyphotic angle decreased from 15.4 to 11.2. Cement leakage was observed in 4 patients and one of them underwent an open operation (decompression and stabilization) because of cement leakage to the epidural space. There was no other complication. Conclusion: Vertebroplasty and kyphoplasty are safe, effective and minimally invasive procedures. Elderly patients can thefore avoid the potential complications of general surgery and be mobilized and discharged early.

Aim: Vertebral compression fractures VCF can lead to severe acute and chronic pain, impaired mobility, reduced quality of life, and an increased risk of mortality due to limited mobility. Percutaneous vertebroplasty VP and kyphoplasty KP are two vertebral augmentation procedures VAPs which are minimally invasive surgical options in the treatment of painful VCFs. Vertebral fractures may result in acute pain around the fracture site, loss of vertebral height due to vertebral collapse, spinal instability, and kyphotic deformity. The main goal of the VP and KP procedures is to provide immediate pain relief for patients and restore the vertebral height lost due to the fracture. This study aims to evaluate KP in terms of pain, disability, and quality of life. Costeffective radiological variables i.e., vertebral height restoration and KP for cement extravasation and complications profile were analyzed. Unipedicular or bipedicular approaches were compared. Methods: Following an injury, the patients had conservative treatment of VCFs including bed-rest bracing and analgesics for four weeks. In 55 patients, a total of 83 vertebral fractures were present. The main causes were falling from a height, traffic accident, and malignancies. We used spinal anesthesia for all patients. Vertebral augmentation was done under fluoroscopy. Results: The mean age was 59.18 years SD16.13 . The rate of female population 56.4% and osteoporotic patients 54.5% was higher. Thoracolumbar junction T10-L2 fractures were the most common fractures of spinal column. L1 was the most often affected vertebra 24.09% . After VAP, pain scores of the patients statistically significantly decreased. Unipedicular or bipedicular approaches provided the same pain reduction. The pain scores of osteoporotic patients and preoperative and postoperative pain scores were higher than non-osteoporotic groups. Preoperative and postoperative pain scores of the female patients were higher than male patients. Seven patients 12.7% had cement leakages. Conclusion: Our study result suggest that the use of the optimal material to provide the optimal result for patient is at the discretion of the surgeon. Clinicians can take the advantage of increased working time. Creating cavity by using drill and curve curette or a balloon and high-viscosity cement under low pressure can minimize leakage complications., Amaç: Vertebra sıkışma kırıkları VSK ; şiddetli akut ağrı, kronik ağrı, hareketliliği kısıtlama, hayat kalitesini azaltma ve hareket kısıtlanmasından ötürü mortalite riskini artırmaya öncülük edebilir. Perkütan Vertebroplasti VP ve Kifoplasti KP , ağrılı VSK’da minimal girişimsel cerrahi seçenekleri olan iki vertebra yükseltme yöntemidir. Vertebra kırıkları; kırık etrafında akut ağrı, vertebra çökmesine bağlı yükseklik kaybı, omurga instabilitesi ve kifotik deformiteye sebep olabilir. VP ve KP işlemlerinin hedefi hızlıca hastaların ağrıdan kurtulması ve kırık nedeniyle oluşan yükseklik kaybını onarılmasıdır. Bu çalışma kifoplastinin; ağrı, hayat kalitesi ve yetersizlik gibi faktörler üzerine olan etkilerini değerlendirmek amacındadır. KP ve radyolojik değişkenler vertebra yükseklik onarımı, vb , çimento kaçakları ve komplikasyon profilleri ile analiz edildi. Tek veya iki taraflı pediküler yaklaşımlar karşılaştırıldı. Materyel ve Metod: Yaralanma sonrası VSK hastaları; 4 haftalık yatak istirahati, korse ve analjezik tedavi gibi konservatif tedaviler aldılar. 55 hastada toplam 83 vertebra kırığı mevcuttu. Yüksekten düşme, trafik kazası ve malignensiler ana nedenleriydi. Tüm hastalar için spinal anestezi uyguladı. Vertebra yükseltme işlemleri floroskopi altında yapıldı. Bulgular: Ortalama yaş 59.18 ±16.13 idi. Kadın hasta populasyonu % 56.4 ve osteoporotik hasta populasyonu % 54.5 yüksekti. Omurga sütununun kırıklarının en sık olduğu yer torakolomber kesişim noktasıydı. Lomber birinci vertebra en çok etkilenen vertebraydı %24.9 . Vertebra yükseltme işlemi sonrası hastaların ağrı skorları istatiksel olarak belirgin azaldı. Tek veya iki taraflı pediküler yaklaşımlar, aynı ağrı skoru düzelmesi sağladı. Osteoporotik hastaların operasyon öncesi ve sonrası ağrı skorları osteoporotik olmayan hasta grubundan daha yüksekti. Bayan hastaların operasyon öncesi ve sonrası ağrı skorları erkek hastalardan daha yüksekti. Yedi hastada çimento kaçağı oluştu. Sonuç: Çalışmamızın sonucu; hasta için en uygun sonuçları sağlamak için en uygun material ve yöntemin kullanılmasının cerrahın tercihinde olmasını önermektedir. Klinisyen, çalışma zamanını azaltmanın avantajlarını alabilir. Vertebrada balonla, dril ve küret yardımıyla kavite oluşturmak ve yüksek viskositeli çimentoyu düşük basınçla vermek, çimento kaçağı komplikasyonlarını minimuma indirebilir.
