Amaç: Kardiyak problemi ve herhangi bir sistemik hastalığı olmayan hipertiroidili hastalarda beyin natriüretrik peptid (BNP) düzeyini ve tedavinin BNP düzeyine etkisini değerlendirmeyi planladık. Gereç ve Yöntemler: Çalışmaya 29 hipertiroidili ve 30 sağlıklı olgu dahil edildi. Hastalarda total T3, total T4, serbest T3, serbest T4, TSH, anti TPOAb, Anti TgAb ve BNP düzeylerine tedavi öncesinde ve ötiroid olduktan sonra bakıldı. Bulgular: Hipertiroidili hastalarının tedavi öncesi BNP düzeyleri kontrol grubuna göre anlamlı olarak daha yüksek bulunmuştur (P0,05). Ayrıca hastalarda Anti TgAb, ST3, ST4 ve TT4’de tedavi öncesi pozitif bir korelasyon gözlenmiş (P0,05). Sonuç: Hipertiroidili hastalarda BNP düzeyi artmıştır. Antitiroid tedavi BNP düzeyini anlamlı olarak düşürmektedir. Objective: We aimed to evaluate the Brain Natriuretic Peptide (BNP) levels in patients with hyperthyroidism who do not suffer any cardiac problems or systemic diseases and the effect of the treatment on the BNP levels as well. Material and Methods: 29 hyperthyroid patients and 30 healthy subjects participated in the study. The patients were checked for their total T3, total T4, free T3, free T4, TSH, anti TPOAb, Anti TgAb, and BNP levels before and after treatment period. Results: BNP levels of patients with hyperthyroidism before treatment were found to be significantly higher than the control group (P 0.05). In addition, a positive correlation was observed in Anti TgAb, ST3, ST4 and TT4 before treatment (P 0.05). Conclusion:BNP level is increased in patients with hyperthyroidism. Antithyroid therapy significantly reduces the BNP level.

The Turkish Hypertension Consensus Report was prepared for the first time in 2015 to adapt the European and American international guidelines to our clinical practice and to create a practical report that could be a basic reference for all physicians dealing with hypertensive patients. This report, which was prepared by a committee with representation from 5 leading hypertension associations, has been accepted and is widely used. New clinical studies in hypertension literature and updated international guidelines since 2015 have demanded an update of the Turkish Hypertension Consensus Report as well. In this updated 2019 report, blood pressure levels were classified as Normal, Elevated, Stage 1, and Stage 2 hypertension. A new section was added for secondary hypertension. It was specified that drug treatment may be initiated with any 1 or a combination of 4 groups of drugs (diuretics, calcium channel blockers, angiotensin-converting enzyme [ACE] inhibitors, and angiotensin receptor blockers [ARBs]), except a combination of an ACE inhibitor and an ARB. It was emphasized that beta-blockers may be a first choice for hypertension treatment in diseases such as atrial fibrillation, heart failure, and coronary artery disease. The initial recommendation for hypertension treatment is a combination therapy in patients with a blood pressure level >= 150/90 mmHg. Target blood pressure values were redefined according to age and the presence of comorbidities. The hypertension treatment algorithm was renewed; it is proposed that drug therapy can also be initiated with a risk-based approach for the group with an elevated blood pressure (systolic blood pressure: 120-139 mmHg, diastolic blood pressure: 80-89 mmHg). The threshold clinic systolic blood pressure level was reduced from 160 mmHg to >= 150 mmHg for the initiation of drug therapy in individuals 80 years of age or more. The section on the treatment of special groups has now been expanded to include pregnancy and lactation. As in the previous report, in this update, practical recommendations for the most common cases seen in the clinic were the goal, rather than a comprehensive report that addresses all aspects of hypertension. This report has evidence-based recommendations for most patients; however, it should be kept in mind that there may be differences from 1 patient to another and that physicians should take an individualized approach according to a good clinical evaluation.

4. Teaching Practicum in English Language Teaching (ELT): A Systematic Review

As a three-handed social process including pre-service teachers, teacher educators from academia, and mentor teachers from schools of practice, teaching practicum, with its reflective and collaborative notions, has recently gained more interest in the field of English language teaching (ELT). In this regard, this systematic review aimed to find out recent research based on teaching practicum in ELT. Regarding the PRISMA guidelines and clear eligibility criteria, the review included a total of 24 research articles based on the ELT teaching practicum. Of the articles, the majority were qualitative studies and mostly conducted in EFL (English as a foreign language) contexts such as Türkiye and Indonesia. MAXQDA (v.2020) software-based content analysis was conducted to analyze data, and the findings were demonstrated via tables including themes, categories, and codes. The study concluded that teaching practicum has several professional (e.g., theoretical and practical awareness, classroom management, and reflective practice) and personal gains (e.g., positive beliefs and attitudes toward ELT, reflectivity, more interaction and communication, and self-confidence) for pre-service ELT teachers. On the other hand, the selected studies also stressed negative professional aspects (e.g., the incompatibility between theory and practice, lack of feedback, and inefficient mentoring) and personal (e.g., lack of reflectivity, adaptation problems, and lack of social contact). The review lastly included implications revealed by these studies to shed light on the issue of ELT practicum for supporting further research.

9. Cloud Based WEB Application Design for Automatic Turkish Business Card Recognition and Its Performance Evaluation

In this study, digital-business card holder software was developed that digitally stores physical business cards prepared in Turkish in a cloud-based database. In the proposed software, the information on the physical business card is converted into text by optical character recognition method (OCR) using business card photos, and then the texts obtained with the help of developed algorithms are separated and grouped. Finally, the digitally obtained business card data is stored in the cloud-based database for later use. Considering the Turkish business cards, it is known that there are a wide variety of complex business cards unique to the country as well as the characters specific to the Turkish language. In this context, first of all, a method that correctly recognizes Turkish characters has been determined in the study. Later, name, mobile phone, e-mail address, company title, position and similar meaningful information were separated from the data read. In order to make these decompositions, special methods have been developed for each field and more accurate and meaningful data has been obtained with field-based algorithms. Thanks to the developed cloud-based platform-independent interface, it is possible to access data from more than one device with a single user over the internet. The study also offers a layered service architecture and database infrastructure that can be used by multiple accounts and multiple users connected to it simultaneously from a single platform. In addition, in the analyzes performed with the developed software, it was determined that 15 business cards with different features were read with an accuracy rate of over 80%.

Amaç: Bu çalışmada; hemşirelerin klinik örnekler hakkındaki bilgi düzeylerinin değerlendirilmesi ve hizmet içi eğitim ihtiyaçlarının belirlenmesi amaçlandı.Gereç ve Yöntem: Çalışmaya dahili ve cerrahi kliniklerinde çalışan toplam 102 hemşire dahil edildi. Katılımcılara klinik örneklerle ilgili bilgi düzeylerinin ve sosyodemografik özelliklerinin belirlenmesi için iki bölümden oluşan anketler yüz yüze görüşme ile uygulandı. Genel laboratuvar bilgisi, klinik örnekler, örnek alım teknikleri ve örneklerin taşınma/saklanma koşulları bilgileri ana başlıkları altında toplam 20 soru soruldu. Hemşirelerin sosyodemografik özelliklerine göre anket sorularına verdikleri cevaplar istatistiksel olarak değerlendirildi.Bulgular: Görev yeri ile doğru cevap sayısı arasında anlamlı istatistiksel fark (p=0.007) saptandı, Dahili kliniklerde çalışan hemşireler daha başarılı bulundu. Yaş ve klinik örnekler ile ilgili bilgi düzeyi arasında anlamlı istatistiksel fark (p=0.009) saptandı ve 35 üstü yaş grupları başarısız bulundu. Eğitim düzeyi ile bilgi düzeyi arasında anlamlı fark (p=0.645) saptanmadı. İş tecrübesi grupları arasında bilgi düzeyi bakımından anlamlı istatistiksel fark (p=0.289) saptanamadı. Hemşirelerin 20 sorudan 8'ine anlamlı istatistiksel fark oluşturacak şekilde sıklıkla doğru yanıt verdikleri (F gt; 4.679; p lt; 0.05) gözlendi. Sorulara verilen yanıtların dağılımı incelendiğinde hemşirelerin klinik örneklerle ilgili bilgi düzeylerinin yetersiz olduğu saptandı.Sonuç: Hastanemizde hemşirelik hizmetleri kalite programları çerçevesinde uygulanan hizmet içi eğitimlerin klinik örneklerle ilgili kısmının yetersiz kaldığı görüldü. Hizmet içi eğitim programları düzenlenirken klinik örneklerle ilgili bilgi eksiklerinin dikkate alınarak mikrobiyoloji, biyokimya ve patoloji ile ilgili konuların kapsamının genişletilmesi ve performans değerlendirilmelerinin de bu kapsamda tekrar gözden geçirilmesinin yararlı olabileceği düşünüldü.
