Afganistan genel popülasyonunda HBV, HCV ve HDV epidemiyolojik durumu ve moleküler profilini belirlemek için beĢ ana bölgeden rasgele 492 birey ve daha önce rapor edilen 234 HBsAg ve 44 anti-HCV pozitif hasta çalıĢmaya dahil edilerek HBsAg, Anti-HBc, Anti-HBs, Anti-HCV ve Anti-Delta pozitifliğini eliza testi ile ve popülasyonundaki genotipleri, alt genotipleri ve alt türleri HBV Pre S, HCV NS5B ve HDAg bölgelerinin dizilenmesi ve filogenetik analizi yöntemi ile araĢtırılmıĢtır. Ayrıca, HBV pol, HBsAg ve NS5B genlerindeki klinik açıdan önemli olan varyantları da incelenmiĢtir.Anti-HBc ve HBsAg prevalensi sırasıyla % 30.28 ve % 6.3 olarak belirlenmiĢtir. Katılımcıların % 27.64'ü, Anti-HBs ≥ 10.0 mIU / mL titresi gösterirken, % 59,95'i HBV'ye karĢı duyarlıydı. Anti-HCV pozitifliği ise % 1,62 olarak tespit edilmiĢtir. HBsAg pozitif bireyler arasında Anti-HDV prevelansı % 2.5 tespit edilmiĢtir. HBV genotip D ve HDV-I'in ilgili enfeksiyonların yaklaĢık % 100'ü iken HCV genotip dağılımı % 75.86 1b, % 20.69 3a ve % 3.44 3b gösterilmiĢtir. HBsAg MHR'de Y100 (3/20), Q101 (1/20), G102 (3/20), T115 (9/20), P120 (1/20), T131 (1/20) mutant varyantlar gözlenmiĢtir. Benzer Ģekilde HBV pol geninde S213T (2/20), Q215P (1/20) ve N248H (20/20) mutasyonları saptanmıĢtır. Ayrıca C316N mutasyonu HCV 1b hastalarının % 72.7‟sınde bulunmuĢtur.Sonuç olarak, Afgan hastalarında çok az genotipik varyasyon olduğu söylenebilir. In addition, clinically important variants of HBV pol gene, HBsAg and HCV NS5B regions respectively were considered.The Anti-HBc and HBsAg prevalence were 30.28% and 6.3% respectively. Totally 27.64% of samples presented titers of Anti-HBs ≥ 10.0 mIU/mL while 59.95% was susceptible. Only 1.62% of persons were Anti-HCV positive. The Anti-HDV prevalence among HBsAg positive patient was 2.5%. The HBV genotype D and HDV-I are approximately 100% of the relevant infections, while the HCV genotype distribution is shown as 75.86% 1b, 20.69% 3a and 3.44% 3b. The observed mutant variant in HBsAg were Y100 (3/20), Q101 (1/20), G102 (3/20), T115 (9/20), P120 (1/20), T131 (1/20). Likewise, S213T (2/20), Q215P (1/20) and N248H 100% (20/20) mutations were detected in the HBV pol gene. The C316N mutation was also found in 72.7% of HCV 1b patients.However there is little genotypic variation in Afghan patients. Hepatit C Virüsü (HCV), kronik karaciğer hastalığı, siroz ve hepatosellüler karsinomaya yol açabilen önemli bir enfeksiyon etkenidir. Dünya nüfusunun yaklaşık %3?ünün HCV ile enfekte olduğu ve hastaların %30?unun da sonunda ileri karaciğer hastalığı geliştirdiği tahmin edilmektedir. Virüs, 9.6 kilo baz (kb) uzunluğunda tek iplikli ve pozitif anlamlı bir RNA?ya sahiptir. Hepatit C tanısı vücutta virüsün varlığının gösterilmesi ile konur. Bunun için en sık antikor gösterilmesine dayalı serolojik testler, viral RNA gösterilmesine dayanan nükleik asit testleri ve karaciğer biyopsisi kullanılmaktadır. HCV RNA tayini HCV enfeksiyonu tanısında en duyarlı yöntemdir ve altın standart olarak kabul edilmektedir. Viral yükün tespit edilmesinde kantitatif PZR (kompetatif PZR veya RT-PZR) tekniği kullanılır. HCV RNA?nın plazmada belirlenmesiyle HCV replikasyonu en iyi şekilde değerlendirilebilir. ?Real Time? PZR cihazını kullanarak farklı primer-prob setlerini Taqman EZ-RT PZR Kiti ile çalışarak denedi ve HCV-RNA?yı kantitatif olarak saptamaya yönelik en uygun, yüksek kesinlik ve duyarlılıkta tek aşamalı `Real Time? PZR koşullarını belirlemeyi amaçlandı. Çalışmada farklı HCV izolatları Mega 3.1 yazılımı ile karşılaştırılarak HCV genomunun korunmuş bölgeleri belirlendi ve bu bölgelere özgü, Primer Express 3.0.1 yazılımı ile primer-prob tasarımı yapıldı( set 1, 4, 5 ve 6 ). Ayrıca Ronald E Engleand ve arkadaşları `Development of a TaqMan Assay for the Six Major Genotypes of Hepatitis C Virus: Comparison With Commercial Assays? çalışmasında tasarlandığı primer prob seti ( set 2) de kullanıldı.Laboratuvarımızda daha önce `COBAS® AmpliPrep? cihazında çalışan ve yüksek viral yüke (107 kopya/?l) sahip olan plazma örnekleri seçilerek ve DNA/RNA?sız insan plazması (CTM(-) C Human plasma roch molecular system) kullanarak 1/10 şeklinde seri seyreltme ile 102?e kadar yapıldı. Yapılan denemeler sonucunda 5´UTR bölgesini hedef alan üç farklı primer-prob seti ( set 1, set 2 ve set 3 ) `Light Cycler Real Time? PZR sisteminde sırayla 1000, 100 ve 1000 kopya/?l?ye kadar, saptanabildiği gözlendi. Set 1 primer prob sistemi (taqman prob), `COBAS® AmpliPrep? cihazın sonucuna göre kantifikasyonu bir log düşük hesaplandı ve tekrarlanabilirliği sağlanamadı. Set 2 ve set 3 primer-prob sistemlerinin (MGB?li prob) tekrarlanabilirliğini göstermek amacıyla, sırayla 107 ile 102 ve 107 ile 103 arası seri seyreltmeye sahip standartlarin deney içi ve deneyler arası Ct değerinin değişkenliği değerlendirildi. Set 2 için CV değeri deney içi 0.02 ile 6.49 ve deneyler arası 0.21 ve 4.39 arasında değişebilmektedir. Set 3 ise Ct değerinin CV değeri deney içi 0.50 ? 6.99 ve deneyler arası 1.85 - 4.29 arasında değişebilmektedir. Set 2 ve 3 Ct değerini karşılaştırılması sonucunda deney içi ve deneyler arası set 2 primer-prob sistemi set 3?e göre tekrarlanabilirliği daha fazlaydı. Metodun verimliliği (?efficiency?) yaklaşık olarak sırasıyla %100 ve %90 olarak hesaplandı. Optimum koşullarda set 2 primer-prob sistemi set 3?e göre daha duyarlı ve daha spesifiktir.Sonuç olarak çalışmada set 2 (HCV Con259F, HCV Con312R ve HCV-278MGB) ile Taqman EZ-RT PZR Kiti kullanarak `Light Cycler 480 Real Time? PZR cihazinde yüksek özgünlük, yüksek duyarlılık ve yüksek verimliliğe sahip olan ve 100 kopya/?l?e kadar HCV RNA virüsü saptanabilen bir yöntem geliştirildi. It infects nearly 3% of the world?s population and it is estimated that 30% of patients eventually develop end stage liver disease. HCV genome is a positive single strand RNA being 9.6 kilo bases in length. Hepatitis C diagnosis is made by demonstrating the existence of the virus in the body. The most common tests used on HCV diagnosis are antibody-based serological tests, viral RNA-based nucleic acid tests and liver biopsy. Determination of HCV RNA, considered as the most sensitive method and the gold standard for diagnosis of HCV infection. Quantitative PCR (competitive PCR or RT-PCR) techniques are the most reliable methods to detect HCV viral load. Replication of HCV can be evaluated rightly with determination of HCV RNA in plasma samples. Real-time PCR system is a rapid system that determines the amplification with high sensitivity and specificity.In this study, improving an HCV quantification method had been aimed, which is more convenient and sensitive, by using various primer probe systems. Experiments were performed using Taqman EZ-RT PCR kit based on light cycler 480 real time PCR systems.In the study, different HCV isolates had been aligned by Mega 3.1 software and highly conserved primers and probes were designed by using Primer Express 3.0.1 software (set 1, 4, 5 and 6 ). In addition, the primer-probe system, designed by Ronald E Engleand and his colleagues on their study (Development of a TaqMan Assay for the Six Major Genotypes of Hepatitis C Virus: Comparison With Commercial Assays) had been tested (set 2). High viral load samples (107 copies/?l) have been selected that were routinely studied at our laboratory by using COBAS ® AmpliPrep system. Ten fold serial dilutions of the samples was obtained from 107 to 102 by using viral DNA/RNA free plasma samples Results of the experiments showed that three different primer-probe systems (set 1, set 2 and set 3) that targets the 5´ UTR of HCV genome could detect HCV RNA up to 1000, 100 and 1000 copies/?l, respectively. According to the results, HCV viral load has been calculated one log lower than the results from COBAS ® AmpliPrep when the set 1 (TaqMan probe) primers and probes were used. In addition to its low sensitivity the results were not reproducible. To demonstrate the reproducibility of the set 2 and the set 3 primer-probe systems (MGB probe), the variability of Ct value of the standards with serial dilution was evaluated. The CV value of intra- and inter-assay of experiments varies between 0.02 - 6.49 and 0.21 - 4.39 respectively for set 2 primer-prob system. The CV of the intra-assay and inter assay of Ct value can vary from 0.50 to 6.99 and 1.85 up to 4.29 in experiments with set 3 of primer-prob system.?By comparing the Ct value of the Set 2 and 3 primer-prob systems, intra-assay and and inter assay reproducibility on the set 2 primer-probe was more high then set 3. The efficiency of the method, had been calculated 100% and 90%, respectively. At optimum conditions set 2 primer probe system showed results with more sensitivity and specificity when compared to the set 3 primer-probe system.In conclusion, by using HCV Con259F, HCV Con312R and HCV-278MGB probe system with EZ-RT PZR Kit based on light cycler 480 real-time PZR system a high specificity, high sensitivity up to 100 copies/?l and high efficiency was achieved.
