Sendromik obezite farklı gen ya da kromozom bozukluklarıyla ortaya çıkar. Obeziteye dismorfik bulgular, mental retardasyon ve gelişimsel anomaliler eşlik eder. Prader-Willi sendromu, Bardet-Biedl sendromu ve Alström sendromu klinik pratikte en sık karşılaşılan obezite sendromlarıdır. Prader-Willi sendromu hipotoni, hiperfaji, hipogonadizm ve boy kısalığı ile karakterize genomik imprinting hatasından kaynaklanan bir obezite sendromudur. Bardet-Biedl sendromu retinal distrofi, trunkal obezite, postaksiyel polidaktili, öğrenme güçlüğü, renal anomaliler ve erkeklerde hipogonadotropik hipogonadizm ile karakterize otozomal resesif geçişli, genetik olarak heterojen bir siliopati sendromudur. Alström sendromu ilerleyici kon-rod distrofisine, obezite ve sensörinöral işitme kaybının eşlik ettiği çoklu organ tutulumu ile karakterize, otozomal resesif geçişli bir sendromdur. Ekzojenik ve monojenik obezitelerin dışlandığı durumlarda sendromik obeziteye yaklaşırken hormonal değerlendirmenin yanında hasta ek dismorfik özellikleri, oftalmolojik, dental, kardiyak, renal, nörolojik sistem yönünden de değerlendirilmelidir. Tanının doğrulanması ve aileye genetik danışmanlık hizmeti verilebilmesi için genetik tanı yöntemlerinden yararlanılmalıdır., Syndromic obesity occurs with different genetic or chromosomal disorders. Prader-Willi syndrome is an obesity syndrome, characterized by hypotonia, hyperphagia, hypogonadism and short stature due to genomic imprinting defect. Bardet-Biedl syndrome is a genetically heterogeneous ciliopathy syndrome caused by autosomal recessive genes, characterized by retinal dystrophy, truncal obesity, postaxial polydactyly, learning difficulties, renal anomalies, and hypogonadotropic hypogonadism only in males, Alström syndrome is an autosomal recessive syndrome, characterized by progressive cone-rod dystrophy, obesity and sensorineural hearing loss accompanied by multi-organ involvement. If exogenous and monogenic obesity is excluded, not only hormonal evaluation but also additional dysmorphic features, ophthalmic, dental, cardiac, renal, and neurological systems should also be evaluated to approach syndromic obesity. Genetic diagnostic analysis should be utilized for confirming the diagnosis and providing genetic counseling to families.

Joubert Sendromu (JS) beyin sapı ve serebellar malformasyonlarla karakterize santral sinir sisteminin nadir gelişimsel bir bozuk- luğudur. Göz (örneğin, retinal displazi gibi), okulomotor, kas-iskelet ve böbrek (örneğin, kistik displazi, nefronofitizis) gibi diğer sistemik bozukluklarla ilişkili olabilir. Biz burada distal renal tübüler asidozis ile birlikte olan JS bir olguyu bildiğimiz kadarı ile tıp literatüründe ilk olması nedeni ile sunduk., Joubert syndrome (JS) is a rare developmental disorder of the central nervous system, characterised by brainstem and cerebellar mal- formations, hypotonia, episodic hyperapnea and apnea and mental retardation. It may be related other systemic abnormalities like ocular (e.g, retinal dysplasia, etc.), oculomotor, musculoskeletal and renal (e.g., cystic dysplasia, nephronophthisis) with renal failure. We describe a case of JS with distal renal tubular acidosis, a relation that has never been described earlier in the medical literature to the best of our knowledge.

The genus Acomys is represented by 19 species in a vast area in the African and eastern Mediterranean regions. Acomys cilicicus, belonging to the genus Acomys, is an endemic species in Turkey. In this study, by evaluating the reproductive data in laboratory conditions and the natural habitat of A. cilicicus, the reproductive biology of the species was researched in detail for the first time. The data concerning the reproduction of 1404 samples, reproduced from 1 male and 2 females in the laboratory and 34 samples obtained from 6 localities near Silifke, were recorded. For A. cilicicus, it was determined that the pregnancy period was 38-40 days, litter size was 1-3 (n = 321, mean = 2.05), and time to maturity was 2-4 months. The newborns' eyes were closed, their auricles were bonded to the head, they had short hair, their abdomens were hairless, and their backs were dark gray. Dentition was completed within 9-10 days and therefore the newborn could easily take food. Suckling was under control of the mother and carried on for approximately 2-3 months. Furthermore, the reproductive, infanticidal, and cannibalistic behaviors of this species were monitored periodically. These data were compared with A. cahirinus, russatus, nesiotes, minous, and dimidiatus, whose reproductive biologies are known and who are close to A. cilicicus geographically. Finally, it was determined that some reproductive findings of A. cilicicus were compatible with the findings of other species, and some findings were different from those of the species above.

m ÖZET Ankara İli Bezirhane Beldesi'nde 1999-2000 yıllan arasında gerçekleştirilen alan çalışmaları sırasında, hibernasyondan çıkış zamanındaki eşeysel farklılık, hibernasyona giriş ve hibernasyondan çıkış ağırlığı, hibernasyona giriş zamanı ve Küçük Asya yer sincabı tarafından tüketilen bazı bitki türleri değerlendirilmiştir. Ergin bireylerin, juvenillerden daha önce hibernasyona girdiği, tüm erkek ve dişilerin Mart ayı içerisinde hibernasyondan çıktığı ve toprak üzerinde ilk görülen bireyin erkek olduğu tespit edilmiştir. Ortalama çıkış tarihinin erkeklerde 15 Mart ± 3 gün, dişilerde 23 Mart ± 4 gün olduğu ve erkeklerin dişilere göre 8 gün önce hibernasyondan çıktığı saptanmıştır. Bunun yanı sıra hibernasyondan çıkışta ergin erkeklerin, ergin dişilerden, bir yaşındaki dişi ve erkeklerden daha ağır olduğu ancak son üç grup arasında ağırlık açısından bir farklılık bulunmadığı tespit edilmiştir. Kontrolsüz laboratuvar koşullarında gerçekleştirilen çalışmalar sırasında, heterotermal dönemden çıkış zamanındaki eşeysel farklılık, torpor ve ara uyanışlar sırasındaki vücut sıcaklığı, heterotermal dönem uzunluğu, torpor uzunluğu, en uzun kesintisiz torpor süresi ve heterotermal dönem boyunca ağırlık kaybı değerlendirilmiş bunun yam sıra torpor ve aktif döneme ait bazı serum değişkenleri karşılaştınlmıştır. Dişilerin heterotermal dönem uzunluğunun erkeUerinkine göre daha fazla olduğu, ancak heterotermal dönem içerisinde yüzde olarak torporda geçirilen süreler açısından eşeyler arasında anlamlı bir farklılık olmadığı saptanmıştır. Laboratuvar sıcaklığındaki düşüşe paralel olarak, her iki eşey grubunda da aylara göre yüzde olarak torporda geçirilen sürenin düzenli olarak arttığı saptanmıştır. Ayrıca en uzun kesintisiz torpor süresinin erkeklerde dişilere göre daha kısa olduğu tespit edilmiştir. Aktif dönemle karşılaştınldığmda, torpor sırasında serum kolesterol, trigliserit, amilaz, ürik asit, üre azotu, sodyum ve klor seviyelerinin azaldığı, potasyum, TSH, T4, ve FT4 seviyelerinin değişmediği, kreatinin, total kalsiyum, inorganik fosfat, T3 ve FT3 seviyelerinin arttığı bulunmuştur. IV ABSTRACT In this study, data were gathered on sexual difference in timing of emergence from hibernation, emergence weight, time of immergence to hibernation, immergence weight and some plant species consumed by Asia Minor ground squirrels (S. xanthoprymnus) during 1999-2000 in Bezirhane Town in Ankara province. It was determined that all males and females emerged from hibernation in March and males were the first animals to appear above ground. Mean dates of emergence were 15 March ± 3 days and 23 March ± 4 days for 9 males and 18 females respectively. Adult males emerged 8 days earlier than did conspecific females. At emergence, older males were heavier than yearling males, older females and yearling females but no differences in the weight of the latter three groups were found. In the uncontrolled laboratory conditions, data were gathered on sexual difference in timing of ending of heterothermal period, body temperature during torpor and arousals, duration of heterothermal period, duration of torpor bout, the longest torpor bout, and weight loss during heterothermal period. It was determined that the heterothermal period of females was longer than that of males. In contrast, no differences in the torpor bout duration (as per cent of heterothermal period) between males and females were found. While laboratory temperature decreases, it was found that torpor bout duration (as per cent of heterothermal period) increases gradually in each sex groups. In addition, it was found that the longest torpor bout duration in females was longer than that of males. Some serum parameters was compared between torpor and active period, and it was found that during torpor, serum cholesterol, triglyceride, amylase, uric acid, üre N, sodium and chloride levels decreased, potasium, TSH, T4 and FT4 levels did not change, and creatinine, total calcium, inorganic phosphate, T3 and FT3 levels increased. Moebius sendromu, diğer adıyla Moebius sekansı, doğuştan yüz ve göz sinirlerinde felç ile belirgin, ilerleyici olmayan bir kraniyal disinnervasyon bozukluğudur. Emmede zayıflık ve yüz felci nedeniyle izlenen, in vitro dölleme ile oluşan gebelik sonucu doğan, beş günlük kız hastaya iki taraflı pitoz ve dışa bakış kısıtlılığı, dilde sola kayma, dismorfik yüz görünümü, sol el parmak ve tırnaklarında hipoplazi bulguları ile birlikte Moebius sendromu tanısı konuldu. Bu sendroma üç, dört, beş, dokuz, 10 ve 12 gibi ek kraniyal sinir tutulumu, kol ve bacak gelişim anomalileri de eşlik edebilir. Etiolojide bir çok etmen öne sürülmekle beraber yardımcı üreme tekniklerine bağlı nadir olgular bildirilmiştir. Hastalarda beslenme güçlüğü ve aspirasyon sorunları süt çocukluğu döneminde karşılaşılan başlıca sorunlardır. Yüz felci ile doğan yenidoğan diğer kraniyal sinirler bakımından da ayrıntılı muayene edilmeli, ayırıcı tanıda Moebius sendromu ile birlikte diğer kraniyal disinnervasyon bozuklukları göz önünde bulundurulmalıdır., Möbius' syndrome, also known as Möbius' sequence, is a nonprogressive cranial dysinnervation disorder characterized by congenital facial and abducens nerve paralysis. Here, we report a 5-day-old girl who was conceived after in vitro fertilization with poor suck and facial paralysis. She had bilaterally ptosis and lateral gaze limitation, left-sided deviation of the tongue, dysmorphic face, hypoplastic fingers and finger nails on the left hand, and was diagnosed as having Möbius' syndrome. Involvement of other cranial nerves such as three, four, five, nine, 9 and 12, and limb malformations may accompany this syndrome. However, several factors have been proposed for the etiology, some rare cases have also been reported with artificial reproductive technologies. Feeding difficulties and aspiration are the main problems encountered in infancy. The other cranial nerves should be examined further in newborns who present with congenital facial palsy, and other cranial dysinnervation disorders should be considered in the differential diagnosis.
