Serebral palsili (SP) çocuklarda yetersiz kas gücü ve duyusal entegrasyon bozukluğu sebebiyle postüral kontrol bozulmuştur. Vücudun merkezinde yer alan gövde postürün kontrolünde önemli rol oynamaktadır. Daha iyi gövde kontrolüyle elde edilen proksimal stabilitenin artması üst ekstremite fonksiyonlarında iyileşmeye yol açabilmektedir. Likra bazlı kompresyon giysi ortezleri ekstra proprioseptif girdi sağlayarak vücut farkındalığında artışa yol açar. Bu giysi ortezlerinin aynı zamanda proksimal stabiliteyi ve üst ekstremite fonksiyonlarını arttırdığı öne sürülmektedir. Bu çalışmada amaç SPIO yelek adlı bir çeşit likralı kompresyon giysi ortezi kullanımın oturma sırasında denge ve postürde, oturma fonksiyonunda, gövde asimetrisinde, kalçanın lateralizasyonunda ve kaba el becerisinde iyileşmeye yol açıp açmayacağını araştırmaktı.Bu tek kör randomize kontrollü çalışmaya gövde kontrolü bozuk olan 3-7 yaşlarında 24 serebral palsili (SP) çocuk dahil edildi. Oturma fonksiyonu ve oturma sırasında postür ve dengenin değerlendirilmesinde Oturmanın Değerlendirilmesi Skalası (Sitting Assessment Scale-SAS) ve Kaba Motor Fonksiyon Ölçümü (Gross Motor Function Measure-GMFM)'nün oturma bölümü kullanıldı. Kaba el becerisinin değerlendirilmesinde Kutu Blok Testi (KBT) kullanıldı. Değerlendirmeler tedavi öncesinde (TÖ), ortez giyildikten hemen sonra tedavi sonrasında (TS), TS 1. ve 3. aylarda yapıldı. Gövde asimetrisinin ölçümü ve ortezin kalça lateralizasyonuna olan etkisinin değerlendirilmesi için TÖ ile TS 6. ay arasında Cobb (CA), Modifiye Cobb açılarındaki (MCA) ve Migrasyon İndeksi (Mİ)'deki değişim saptandı.Oturmanın Değerlendirilmesi Skalası'nda SPIO gruplarına göre daha az iyileşme gösteren kontrol grubu haricinde tüm gruplarda benzer iyileşmeler gözlendi. Aynı zamanda ortez giyildikten hemen sonra oturma sırasında postür ve dengenin kontrolü ve kaba el becerisi de iyileşme gösterdi. TS 6.ayda CA ve MCA anlamlı olarak azalırken, Mİ istatiksel anlamlı değişiklik göstermedi. CA hariç radyografik ölçümlerdeki değişimler gruplar arasında benzerdi. CA değerleri kontrol grubunda daha az değişim gösterdi.Oturma sırasında postür ve dengede ve üst ekstremite fonksiyonunda SPIO yeleğin anlık etkisi mevcuttur. SPIO yeleğin tedavi sırasında kullanımı oturma postüründe, gövde asimetrisinde, oturma fonksiyonunda ve üst ekstremite fonksiyonlarında iyileşmeye yol açmaktadır.ANAHTAR KELİMELER: Giysi tedavisi, gövde kontrolü, likralı giysiler, ortezler, oturma, serebral palsi. Postural control in children with cerebral palsy (CP) is deteriorated due to inappropiate muscle force and lack of sensory integration. The trunk which is found in the centre of the body plays a crucial role in postural control. Improved proximal stability obtained by a better trunk control may lead to improvements in upper extremity function. Lycra based compression garments provide extra proprioseptive information which enhances body awareness. It is proposed that these orthotic garments improve proximal stability and upper extremity function. In this study, the aim was to investigate if the use of a lycra based compression garment called SPIO vest would lead to improvement in posture and balance during sitting, trunk asymmetry, hip lateralization and gross manuel dexterity.Twenty-four children with CP between the ages of 3-7 with impaired trunk control were enrolled in this single blind randomized controlled trial. To assess sitting function and posture and balance during sitting Sitting Assessment Scale (SAS) and sitting domain of Gross Motor Function Measure (GMFM) were used. Box and Block Test was used to evaluate gross manuel dextrity. Assessments were done before treatment (BT), immediately after garment was worn, at posttreatment, at 1 month posttreatment (MPT) and at 3 MPT. To measure trunk asymmetry and effect of orthosis in hip lateralization, changes in Cobb (CA) and Modified Cobb angles (MCA) and Migration index (Mİ) between BT and 6 MPT were determined. All groups showed smiliar improvements except control group showing less improvement in SAS scores than SPIO groups. Posture and balance during sitting and gross manuel dexterity also improved immediately after orthosis was worn. While CA and MCA decreased significantly at 6 MPT, Mİ did not show statistically significant difference. Changes in radiographic assessments were smiliar between groups except CA values. CA values showed less change in control group.SPIO vest has an immediate effect on posture and balance during sitting and upper extremity function. SPIO vest can be used during therapy to improve posture and balance, trunk assymetry, sitting function and upper extremity function. 4. Assessment of the Relationships Between Spinopelvic Radiologic Parameters and Balance, Gait and Falls Efficacy in Patients with Postmenopausal Osteoporosis

Objective: The aim of this study is to investigate the relationship between sagittal balance, spinopelvic parameters, bone mineral density, balance, and falls efficacy in postmenopausal osteoporosis patients. Materials and Methods: Forty patients between the ages of 40-80 with a diagnosis of postmenopausal osteoporosis were included in the study. Spinopelvic parameters (sagittal vertical axis (SVA), thoracic kyphosis, lumbar lordosis, pelvic tilt, pelvic incidence, sacral slope) were calculated from spine radiographs. Patients were applied Tinetti Balance and Gait Assessment and Falls Efficacy Scale. Relationship between bone mineral density measurements, clinical parameters, spinopelvic parameters, balance and gait, and falls efficacy were investigated. Results: Mean age of the patients was 63.42 +/- 8.98 and the mean L1-L4 T score was 2.5 +/- 0.68. Statistically significant negative correlation was found between the L1-L4 and L2-L4 vertebrae bone mineral density and T score, and sacral slope. Negative statistical correlations were found between SVA and Tinetti Balance and Gait Assessment balance subtest and total scores. There was no statistically significant relationship with other spinopelvic parameters. There was no significant correlation between SVA and spinopelvic parameters and Falls Efficacy Scale, while a statistically significant negative correlation detected between Falls Efficacy Scale and Tinetti Balance and Gait Assessment balance and gait subtests and total scores. Among the patients with normal, mild and severe deformity sagittal balance, there was statistically significant difference in terms of Tinetti Balance and Gait Assessment balance and gait subtests scores and total scores, while there was no difference with regards to Falls Efficacy Scale. Conclusion: In postmenopausal women with osteoporosis, SVA, sagittal balance parameters, and balance and gait functions were found to be associated.
