Robotik hasat sistemlerinde lahana ve brokolinin derin öğrenme kullanılarak sınıflandırılması oldukça önemlidir. Derin öğrenme, yapay sinir ağları ve büyük veri setleri kullanılarak karmaşık modellerin öğrenilmesine olanak sağlayan bir makine öğrenme yöntemidir. Bu yöntem yardımıyla bitki sınıflandırmasında ve görsel tanıma problemlerinde etkili bir şekilde kullanılabilir. Lahana ve brokoli gibi bitkilerin sınıflandırılması için öncelikle bir derin öğrenme modeli oluşturulması gerekmektedir. Bu nedenle yapılan çalışmada derin öğrenme yöntemlerinden olan Inception_v3 görüntü tanıma ve sınıflandırma modellemesi kullanılmıştır. Çalışma oluşturulan 2 sınıf üzerinden yürütülmüştür. Oluşturulan sınıflar lahana ve brokoli’dir. Modelin eğitimi için Google Colab’ın sağladığı tpu donanım hızlandırıcısı kullanılmıştır. Eğitim döngüsü (epoch) sayısı 10’dur.Eğitim parametreleri olarak öğrenme hızı 0,001 tespit edilmiştir. Bu sonuçlara göre brokoli ve lahana data setin eğitimi için Inception_v3 modelinin başarılı olduğu sonucuna varılmıştır. Eğitim sürecinde modelin kayıp değeri giderek düşmüş ve doğruluk değeri artmıştır. Son aşama olan doğrulama aşamasında kayıp değeri 0,0005, doğruluk değeri 1,0000 olarak gözlenmiştir.

Objective:This study aimed to assess the impact of obesity on neuropathic pain, fibromyalgia symptoms, and quality of life in female patients diagnosed with fibromyalgia syndrome.Materials and Methods:This study enrolled 40 normal-weight and 40 obese female patients who were diagnosed with fibromyalgia syndrome. The patients enrolled in this study satisfied all of the American Collage of Rheumatology (ACR) 1990 and 2010 classification criteria. For fibromyalgia syndrome. The Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) scale and douleur neuropathique 4 questions (DN4) scale was used to assess the extent of neuropathic pain experienced by the patients enrolled in this study. The assesment of patient health status was conducted using the Fibromyalgia Impact Questionnaire (FIQ) scale.Results:Based on the results obtained from the LANSS scale, neuropathic pain was identified in 56.3% patients, while according to the DN4 scale, it was present in 78% of the patients. The analysis of data from the LANSS scale revealed that 42.5% of the patients in the normal weight group and 70% of the patients in the obese group had neuropathic pain. Furthermore, the prevalence of neuropathic pain was significantly higher among the patients in the obese group. The comparison of quality of life scores using the FIQ scale between the two groups did not reveal any statistically significant variation.Conclusion:Neuropathic pain is high in female patients with fibromyalgia syndrome (FMS). Neuropathic pain was significantly higher in the obese group. Therefore, obese women with FMS would benefit from an increased focus on managing their neuropathic pain.

Objective:Our aim was to evaluate the health-related quality of life (HRQoL), depression, fatigue, the Mediterranean fever (MEFV) gene mutations, and other disease-related variables in patients with Familial Mediterranean fever (FMF) associated spondyloarthropathies (SpA).Materials and Methods:Consecutively, 45 patients with FMF associated SpA (41 females, 4 males) and 40 healthy controls were included. Metrological measures (finger-floor distance, dorsal Schober’s test, occiput-wall distance and chest expansion) were determined. Axial and peripheral joint pain was evaluated by using visual analog scale (VAS), disease activity by Bath Ankylosing Spondylitis Disease Activity index and function by Bath Ankylosing Spondylitis Functional index. Short Form-36 and the multidimensional assessment of fatigue (MAF) scale, Beck depression index (BDI) and MAF scales were used to evaluate HRQoL, depression and fatigue.Results:Mean ages of the patients was 43.73±8.7 (24-59) years. Both physical and mental HRQoL were found to be significantly worse in FMF associated SpA patients than in controls. Twelve patients (26.6%) had clinical depression with BDI scores ≥17. MAF score was high in patients with FMF associated SpA. The most frequent mutations were M694V and E148Q in patients with FMF associated SpA. No significant effect of MEFV gene mutations was seen on QoL, fatigue, depression, pain and disease-related variables.Conclusion:FMF associated SpA significantly affects the QoL of its sufferers, as other chronic ilnesses. In this study, both physical and mental HRQoL were found to be significantly worse in FMF associated SpA patients than in controls, irrespective of the MEFV gene mutations. Depression was also seen in high rates in patients with FMF associated SpA. Further studies are needed to determine the effects of MEFV gene mutations on disease severity and QoL in FMF patients.

Our country has many species in fruit and vegetable cultivation. One of them is Gilaburu fruit (Viburnum Opulus), which has spread to different regions, especially in Kayseri. Longer preservation of products in fruits and vegetables; Drying process has been applied since ancient times to prevent deterioration caused by microbial and/or biochemical changes. One of them and the most common one is convectional drying in other words drying in a drying chamber under controlled hot air stream. In this study, Gilaburu fruits were obtained from a local producer in Kayseri. Drying processes were carried out in a convectional oven at 60, 70 and 80℃ in three repetitions. The effects of different drying conditions on the drying characteristics of Gilaburu fruits were examined. Drying time for Gilaburu fruits decreased with increasing temperature. As a color parameter total color change with respective to fresh fruit (ΔΕ) was investigated. The results indicated that it remained almost identical with temperature change. Similar trend was observed for rehydration rate. Titration acidity and pH values showed compatible changes. With temperature increase, titration acidity increased, as pH decreased. As textural properties, skin-hardness and elasticity of dried fruits were measured. As hardness increased, elasticity decreased with temperature elevation. Total phenolic content and antioxidant activity of dried samples were also examined. The results showed that there were increases in both of them with an increase in temperature.

In the period referred to as the digital era, technological products dominate almost all facets of human life. While technology facilitates human life on one hand, it threatens health with electromagnetic radiation on the other. This study investigate the effects of electromagnetic radiation on health complaints and psychological anxiety and, accordingly, shopping malls were selected as areas of application because the electromagnetic radiation is high at such venues. In this study, the health and psychological complaints of employees working at shopping malls where the electromagnetic radiation is considered to be relatively high and of employees working in similar sectors outside of shopping malls were measured and comparatively analysed. The study sample is composed of 347 employees in total, 174 of whom are employed at shopping malls in Adana province, and 173 employed in stores located outside of shopping malls. It is seen that there is no statistically significant difference between the employees working inside and outside of the shopping malls in terms of health complaints (p=.945 gt;.050) and psychological complaints (p=.122 gt;.050). But the scale points related to the health complaints of those working at shopping malls is slightly higher. The models set up in relation to the effect of the number of years worked by employees working at shopping malls on "psychological complaints" and "health complaints" were found statistically significant (F=8.234; F=4.174; p lt;.05). It is important to make measurements in shopping malls and to increase the practices to prevent personnel from having health and psychological problems.

Gömülü yeraltı yapılarının yerlerinin ve sınırlarının belirlenmesi, yerbilimlerinde önemli bir problemdir. Sınır belirleme yöntemleri olarak bilinen birçok süzgeç, potansiyel alan verileri uygulamalarında kullanılmaktadır. Bu çalışmada, önce yapay olarak oluşturulan yeraltı modeline süzgeçler uygulanmıştır. Elde edilen sonuçların yorumlanmasıyla, yöntemlerin becerisi tartışılmıştır. Gerçek arazi verisi uygulamasında, Doğu Anadolu bölgesinde yer alan, Elazığ ili ve çevresini içine alan bölgenin, Bouguer gravite anomali verisine sınır belirleme yöntemlerinin uygulanmasıyla, bölgedeki ana faylar ve fay sistemleri belirlenmeye çalışılmıştır. Bölgenin hakim tektonik yapıları olan Doğu Anadolu fayı ve Güney Doğu Anadolu fayı başarılı bir şekilde belirlenmiştir. Bunun yanında çalışma alanının kuzey kesiminde Elazığ ili ve Elazığ-Pertek-Akpazar arasında yaklaşık D-B ve GB-KD uzanımlı çizgisellikler belirlenmiştir.

The purpose of this study is to explain the factors affecting the student flow fromTurkey to different countries which takes place by means of Erasmus+ program in2015 with the gravity model. The variables reflecting the economical size of thecountries and the variable representing the distance between Turkey and the countryvisited were added to the gravity model formed and the statistical significance of themodel was tested. Moreover, at which level the student potential which wanted to havean education with Erasmus+ was affected by the quality of the visited country’suniversities, visited country’s having a Mediterranean climate, its foreign language’sbeing English and its being an OECD country were analyzed as dummy variables.

Amaç: Bu çalışmanın amacı distal tibia kırığı nedeniyle minimal invaziv yöntem ve biyolojik fiksasyon prensipleri ile plak kullanarak osteosentez uyguladığımız olgularda klinik, radyolojik sonuç ve komplikasyonların değerlendirilmesidir.Gereç ve Yöntem: Bu çalışmada distal tibia kırığı nedeniyle minimal invaziv yöntem ve biyolojik fiksasyon prensipleri ile ameliyat olan 58 hasta (35 erkek, 23 bayan, ortalama yaş 32) ortalama 28 aylık takibleri sonucunda değerlendirildi. Hastaların kırıkları AO/OTA sınıflamasına göre değerlendirildi. Buna göre 16 hastada A1 (%27.6), 13 hastada A2 (%22.4), 14 hastada B1 (%24.1), 11 hastada B2 (%19), 4 hastada ise C1 (%6.9) tipi kırık mevcuttu. Tüm hastalarda kırıkların kaynama sonuçları, aktiviteye dönüşleri, ayak bileği hareket açıklıkları, enfeksiyon ve komplikasyonlar Johner ve Wruhs kriterlerine göre değerlendirildi.Bulgular: Hastalarda kırıkların ortalama kaynama süresi 13 hafta (11-24 arası) idi. Tüm olgularımızda kaynama elde edilirken bir hastada gecikmiş kaynama görüldü. Hiçbir olgumuzda enfeksiyon ve yara yeri problemi görülmedi. İki olguda kaynama sonrasında 5 derece varus deformitesi tespit edildi. Dört olgumuzda diğer ayağa kıyasla 5 derece, 4 olguda ise 10 derece dorsifleksiyon kaybı görüldü. Sonuç olarak Johner ve Wruhs değerlendirme kriterlerine göre 48 hasta mükemmel (%82.75), 10 hasta (%17.25) iyi olarak değerlendirildi.Sonuç: Yumuşak doku azlığı, zayıf vaskülarizasyon gibi nedenlerden dolayı komplikasyonla karşılaşma oranının yüksek olduğu distal tibia kırıklarında, minimal invaziv yöntem ve biyolojik plaklama prensipleri ile başarılı sonuçlar alınabilmektedir.r.

Objective: The aim of this study was to determine whether vitamin D deficiency in patients with musculoskeletal pain who were admitted to our hospital and to establish the difference between vitamin D levels according to gender and age groups. Materials and Methods: Among 28.702 patients, 857 (3%) patients whose vitamin D levels were studied were included in the study. Serum 25(OH) D level was determined by liquid chromatography-mass spectrometry (LC-MS). These fibers areused in production of various yarn blends, woven, knit and nonwoven fabrics to manufature apperal andhome textiles such as t-shirts, bedding, sweater and baby dress due to these superior properties. This review,about SPF, is divided into two sections. In the first part; structure and production stages of SPF and itsenviromental effects had been described. In the second part of this review, properties and application areasof SPF have been described.

Soybean fiber (SPF) is a protein based botanic fibre. These fibers exhibit very good physical properties such as brightness, softness and drape. Moreover, SPF has a variety of health functionalities and anti-bacterial properties. Fibers were first produced in the 20th mid-century. However due to the significant challenges encountered during the production of SPF, interest for these fibers was decreased. At the end of the 20 th century, SPF re-captured attention due to an increased awakening on ecological, renewable and sustainable fiber concept. Soybean is cheap and abundant. Tenacity of SPF was improved by including polyvinyl alcohol (PVA). Therefore, the production and the usage of SPF are increasing rapidly because of these key advantages. Soybean fibers usually is used in blends with other fibers. In Turkey, a variety of different products are produced from this special fiber. This review, about SPF, is divided into two sections. In the first part; structure and production stages of SPF and its enviromental effects have been described. In the second part of this review, properties and application areas of SPF have been described. The purpose of this review is to fill a gap in the Turkish literature about this bio-degradable, renewable and sustainable SPF.

Posteriyor Lökoensefalopati Sendromu (PLES) mental bozukluk, baş ağrısı, epileptik nöbet ve görme bozukluğu ile karakterize, sıklıkla artmış kan basıncı ile birliktelik gösteren akut nörolojik bir acildir. Fizyopatolojik mekanizması kesin olarak bilinmemekte, ancak hipertansiyona yanıt olarak ortaya çıkan vazospazma bağlı olarak kan beyin bariyerinin etkilendiği ve sonuçta serebral ödemin oluştuğu ileri sürülmektedir. Tanı klinik ve nöroradyolojik görüntüleme yöntemleri ile sıklıkla konulabilmektedir. Ancak yanlış tanı veya tanıdaki gecikme nedeni ile tedavinin yapılamadığı durumlarda sendrom serebral hemoraji, koma ve ölümle sonuçlanabilmektedir. Bu yazıda PLES'li bir olgu sunularak konuya dikkat çekilmek istenmiştir.

Summary Özet Orijinal Araştırma / Original Investigation 77 Aim: Complex regional pain syndrome (CRPS) is characterized by pain, vasomotor and sudomotor changes and trophic disturbances. It may develop as a result of trauma, especially extremity fractures and surgery. Although the pathogenesis of CRPS is not exactly clear explained, it has been suggested that central and periferal mechanisms play role and neurogenic inflamatuar and microvasculer functional impairmensts are accompanying to the process. Not all but some of the patients with past trauma or with other possible etiological factors develop CRPS. This suggests the presence of an individual predisposition. In this article, we searched post fractüred CRPS-I patients demographic characteristics, current systemic diseases and symptoms that accompany a CRPS in our records. Materials and Methods: In this retrospective study conducted in Ankara Atatürk Education and Research Hospital, medical records of 356 patients admitted to physical medicine and rehabilitation outpatient clinics with fractures between January 2011 and June 2011 were evaluated and 34 patients diagnosed as CRPS-1 were included in the study. Results: 34 of 356 patients (9.56%) with fractures were diagnosed as CRPS-1 in our outpatient clinics. Mean age of the patients was 46.05 years and 10 patients were females (29.4%) and 24 patients (70.6% ) were males. Fractures were in upper extremities in 18 patients (52.9%) and in lower extremities in 16 (47.1%) patients. Neuropsychiatric disorders and other systemic diseases that may have a role in etiology of CRPS were found in lower rates in medical records of our patients. Conclusion: CRPS does not develop in every patients after travma who has etiologic risk factors, so it reminds that there exists a tendency to CRPS. We have observed that CRPS-1 risk was increased in male patients and in upper extremity fractures. We did not observe any other impertant factor which increases tendency to CRPS. Aim: Osteoporosis is a skeletal disease characterized with decreased bone mass and microarchtitectural deterioration of bone tissue which increases bone fragility and fracture risk. Osteoporosis and osteoporotic fractures constitute an important health problem in general population. This study aimed to determine the incidence of osteoporosis, chronic diseases accompanying osteoporosis and incidence of falls in male and female patients admitted to our out patient clinics retrospectively. Material and Methods: Patient records of the 11624 patients admitted to Ankara Atatürk Education and Research Hospital Physical Medicine and Rehabilitation Outpatient clinics between January 2010 and July 2010 were retrospectively reviewed and 644 patients diagnosed as osteoporosis according to femoral neck and/or lumbar dual energy x ray absoptiometry measurements were included in the study. Ages of the patients, sexes, chronic ilnesses, musculoskeletal sytem complaints and fall histories were also recorded. Results: The incidence of osteoporosis was found to be 7.61% in female patients and it was determined that incidence was 5-fold increased in women than in men. Besides, chronic ilnesses and fall history were accompanying in higher ratios in osteoporotic patients. Conclusion: Heart diseases, hypertension, diabetes, neurological diseases leading to impairment in balance and musculoskelatal system complaints were quite frequent in patients with osteoporosis and these diseases should be taken seriously since they increase the risk of falling. It is important to avoid using drugs which lead to balance impairment, to use walk aids like canes or walkers, to perform exercises including balance and coordination training and endurance exercises in order to prevent falls. Aim: Osteoporotic vertebral fractures and increased kyphotic angle may cause disturbances in pulmonary functions. Our aim was to evaluate pulmonary function and quality of life in osteoporotic patients. Material and Methods: Fifty-one patients with osteoporosis and 19 osteopenic subject, followed in our outpatient clinic, were included. Their demographics were recorded. All the subjects were submitted to pulmonary function test (PFT) and dorsal lateral radiography was obtained to measure Cobb angle. Global body pain was assessed using Visual Analogue Scale (VAS) and quality of life was assessed using Short Form-36 (SF-36) questionnaire. Results: The mean ages were 63.3±8.7 in osteoporosis and 55.3±7.3 in osteopenia groups (p=0.001). Body mass index of the osteoporosis group was significantly lower than osteopenia group (28.8±5.7 and 32.2±5.5, respectively, p=0.029). In osteoporosis group compared to osteopenia group, the absolute forced expiratory volume in 1 second (FEV1) (1.8±0.4 and 2.1±0.5, respectively, p=0.018) and absolute forced vital capacity (FVC) (2.2±0.5 and 2.6±0.6, respectively, p=0.005) were significantly lower. No differences were found between two groups concerning other PFT parameters. The mean Cobb angle was higher in osteoporosis group than osteopenia group, but the difference was not significant (32.7±12.4 and 28.5±9.5, respectively, p=0.194). When all the subjects were considered, no correlation was found between Cobb angle and PFT parameters (p>0.05). No significant difference was found between two groups in respect to VAS and SF-36 scores (p>0.05). Conclusion: We found absolute FEV1 and absolute FVC significantly lower in osteoporosis group compared to osteopenia group. But we observed no differences in % predicted FEV1 and % predicted FVC. We supposed that the differences were because the subjects in osteopenia group were younger and fatter. The pulmonary functions and quality of life of the osteoporotic patients with a kyphotic angle up to studied are not worse than the osteopenic people. (From the World of Osteoporosis 2009;15:11-5)

Postpartum spinal osteoporosis (PPSO) is a rare disease entity presenting with back/low back pain and is characterized by osteoporosis during late pregnancy or puerperium. It generally leads to one or more vertebral fractures. Herein, we present a young female patient who had multiple vertebral compression fractures due to PPSO in the postpartum period. Objective: We investigated the prevalence and distribution of depressive symptoms in individuals aged 15 and older, as well as the relationship between depressive symptoms and sociodemographic characteristics, chronic diseases, health status and health behaviors., Method: The analysis of data collected from the Türkiye Health Surveys conducted by Turkish Statistical Institute (TURKSAT) in 2016 and 2019 involved using the Chi-Square independence test. Effect sizes were evaluated using Phi or Cramer's V coefficients. Patient Health Questionnaire-8 (PHQ-8) module was used to scan the depressive symptoms., Results: The adult population's point of prevalence of depressive symptoms was 4.7%±0.24 in males and 8%±0.19 in females, with a population total of 6.3%±0.21. The yearly prevalence of depressive symptoms was 6.1%±0.45 in males, 13.2%±0.53 in females and a population total of 10%±0.49. Age, gender, income, education, social support, health status, disability, and chronic illnesses were found to be significant predictors of the incidence of depressive symptoms (p<0,05). The results indicated that the rate of people with depression getting help from psychologists, psychotherapists,and psychiatrists was low., Conclusion: Age, gender, income, education, marital status, self-rated health status, social support, number of chronic illnesses, and disability were the most important risk factors for depressive episodes. In addition to such self-reported research completed before the pandemic and Kahramanmaraş earthquakes in Türkiye, further studies based on structured diagnostic interviews are required.

Objective: The objective of this study is to determine the prevalance of Hepatitis C Virus (HCV) in patients with Opioid Use Disorder and to investigate both the sociodemographic and psychological differences between patients with or without Hepatitis C Virus., Method: Blood samples were taken from patients diagnosed with Opioid Use Disorder (OUD) who applied to Mersin Provincial Health Directorate Toros State Hospital Alcohol and Drug Addiction Treatment Center (AMATEM) between 01.09.2019-01.09.2020 and hepatitis virus markers, complete blood count and basic biochemistry were assessed. Sociodemographic Evaluation Form, Barratt Impulsivity Scale, Childhood Traumas Scale, Drug Use Disorders Identification Test (DUDIT), Temps-A Temperament Scale, Difficulty in Emotion Regulation Scale were applied to 107 patients with HCV and 101 patients without HCV who were selected by systematic sampling and the findings obtained were compared., Results: Of the 1190 patients diagnosed with Opioid Use Disorder, 340 (28.5%) were found to be HCV positive. 107 HCV-positive and 101 HCVnegative patients who were selected for comparison were similar in terms of sociodemographic characteristics. Among HCV positive patients, the rate of intravenous drug use and needle sharing were significantly higher (p<0.001, p<0.001). Overall duration of substance use and intravenous substance use were significantly higher in HCV positive patients (p=0.024 and p=0.017). Similarly, HCV positive group were significantly more likely (p<0.001) to have heard of HCV before and significantly more likely (p=0.009) to know that HCV causes cirrhosis and liver cancer. Regarding the scores of Barratt Impulsivity Scale, Childhood Traumas Scale, Substance Use Disorder Recognition Test, DUDIT, Temps -A Temperament Scales applied to HCV positive and HCV negative patients with Opioid Use Disorder, no difference was found. The scores of the 'Non-Acceptance' factor of the Difficulties in Emotion Regulation Scale were found to be significantly higher in HCV positive patients with Opioid Use Disorder (p=0.020)., Conclusion: Patients diagnosed with Opioid Use Disorder (OUD) have higher rates of HCV positivity compared to general public. Intravenous drug use significantly increases this risk. Half of HCV-positive patients were unaware that they have the virus and only a very small proportion received treatment. Therefore, examining each patient with OUD for HCV, treating positive patients in an effective referral system seems to be the an important step in the eradication of this disease in this population.

Bacteria causing hospital-associated infections continue to become resistant due to antibiotic resistance, which has become a global problem worldwide and accordingly, the antibiotic options used in the treatment of infectious diseases caused by these bacteria are limited. In the light of the data obtained from experimental studies on plants, it is thought that plant extracts may be a promising option in the treatment of infectious diseases. In this study, it was aimed to determine the antibacterial activity of Taraxacum officinale extracts on Bacteroides fragilis ATCC 25285 standard strain by broth microdilution method and to pioneer different studies that will investigate the antibacterial effects of plant extracts on resistant B.fragilis strains that cause hospital-acquired opportunistic infections after invasive interventions and trauma. In this study, the T.officinale plant collected as a result of field work was divided into root, leaf and flower parts and dried at 70 °C for 24 hours and then turned into powder. Dried plant samples were extracted in ethanol and methanol for 24 hours. The obtained extracts were stored at -80 °C to be used in the broth microdilution method. B.fragilis ATCC 25285 standard strain was used as the bacterial strain. As a result of the experiments performed with broth microdilution method, the MIC (minimum inhibitory concentration) value of root, leaf and flower extracts with ethanol was determined as 200 μg/ mL, the methanolic root extract as 100 μg/mL and the methanolic leaf and flower extracts as 200 μg/mL. As a result, ethanol and methanol plant extracts were found to be effective on B.fragilis strain.

Introduction: Lung cancer is the most common solid organ malignancy requiring intensive care unit (ICU) admission. For many years, lung cancer patients were not considered in the priority patient category for admission to ICU because of their high mortality rate and poor response to therapy. Considering the developments in treatment modalities, we aimed to reevaluate the prognosis of patients with lung cancer in the ICU., Materials and Methods: Patients characteristics, date of diagnosis, the reason for ICU admission, the stage of cancer, histopathological type, history of chemotherapy, radiotherapy, or surgery for cancer, and APACHE-II and Charlson comorbidity index (CCI) were recorded retrospectively., Result: A total of 100 patients had a mean age of 69.7 ± 9.0 years. Among these patients, 18% had small cell lung cancer, while 82% had non-small cell lung cancer. The in-hospital mortality rate was 69% for all patients, while among those discharged from the ICU, the first 6-month mortality rate was 58.1%. The median survival time was 8.2 months. Advanced age, the need for mechanical ventilation, the need for vasopressors, a high APACHE -II, and the CCI all reduced survival in multivariate analysis, whereas chemotherapy and surgical history improved survival., Conclusions: Patients admitted to the ICU with lung cancer continue to experience a high mortality rate. However, identifying the factors that are associated with survival can be crucial in establishing care plans and prioritizing ICU admission for further therapy.

Objective: The aim of this study is to analyze the validity and reliability of the Turkish form of Massachusetts General Hospital Hairpulling Scale (MGH-HPS), which is used to measure the severity of Trichotillomania (TTM)., Methods: Fifty patients diagnosed with TTM according to the DSM-5 diagnostic criteria and fifty healthy controls participated in the study. The participants were asked to complete a sociodemographic questionnaire, the MGH-HPS-TR, the Clinical Global Impression (CGI), the Beck Depression Inventory (BDI), the Beck Anxiety Inventory (BAI) and the Barratt Impulsiveness Scale (BIS-11). The construct validity and the criterion validity of the MGH-HPS-TR were determined by means of exploratory factor analysis (EFA) and confirmatory factor analysis (CFA), respectively. The reliability analysis of the MGH-HPS-TR was assessed by calculating the Cronbach's α coefficient and the item total correlation coefficient. The values for the area under the curve (AUC), sensitivity and specificity were based on the ROC analysis., Results: AFA and CFA results indicated a single factor structure with 7 items explaining 82.5% of the variance. The item/factor loadings were satisfactory with the best fit indeces. Correlations were found between the scores on the MGH-HPS-TR and the other scales used for criterion validity analyses. The internal consistency and the item-total correlation coefficients of the scale were found to be satisfactory. Based on a cut of point of ≥ 9, the scale had high power for discriminating between the patient and the control groups and high sensitivity and specificity., Conclusion: This study showed that the MGH-HPS-TR can be used as a valid and reliable psychometric tool in Turkey.

In this study, it was aimed to determine the possible factors affecting the clinical importance of Corynebacterium striatum isolates, which were accepted as infectious or contamination/colonization agents, by comparing their clinical and microbiological characteristics, antimicrobial susceptibility results, biofilm forming abilities and genotypic characteristics. The patients with C.striatum growth in the clinical samples sent to the laboratory were evaluated as infection or contamination/colonization with the evaluation of the examination findings and other laboratory parameters by the relevant physician. This study included 58 isolates, 29 of which were considered as infection and 29 as contamination/colonization. Length of hospital stay, presence of underlying disease [diabetes mellitus (DM), neurological disease, ischemic heart disease, chronic kidney disease, solid tumor], surgical operation status in the last month, and antibiotic use in the last three months of the patients were examined. Identification of the bacterial type was made with MALDI-TOF MS (Bruker/Germany) system. Antimicrobial susceptibility tests were performed by disc diffusion and gradient diffusion method and evaluated according to EUCAST standards. Biofilm production was determined in 96-well microtiter plates on negatively charged polystyrene surfaces. Clonal analyzes were performed by PFGE method using Xba1 enzyme. It was observed that there was no difference in terms of demographic characteristics in the two patient groups included in the study. It was observed that C.striatum strains isolated from outpatients were mostly found in the contamination/ colonization group. The presence of diabetes mellitus, ischemic heart disease, chronic kidney disease and solid tumor was not statistically different in the two patient groups. It was observed that C.striatum strains grown in the samples of patients with neurological disease were mostly found in the infectious agent group (p= 0.025). It has been observed that C.striatum strains grown purely in culture were mostly found in the infectious agent group (p= 0.001). Biofilm production was found to be significantly higher in the infectious agent group (p= 0.015). In antimicrobial susceptibility tests, it was observed that there was widespread multidrug resistance (MDR) in both groups and there was no significant difference between the groups in terms of antimicrobial susceptibility. In our study, it was determined that the strains showed very different PFGE patterns and were not clonally related to each other. In this study, it was determined that the demographic characteristics and comorbidities of the patients were not helpful in evaluating the clinical significance of C.striatum. Biofilm production was observed to be a common virulence factor in C.striatum strains. It was thought that there may be difficulties in the treatment of C.striatum in the future due to the widespread MDR detection among this bacterium strains. Our study contributes to draw attention to the increase in C.striatum infections, resistance and virulence factors.

Objective: This study has aimed to investigate the validity and reliability of the Borderline Personality Features Scale for Children-Short Form (BPFSC-SF-TR) in Turkish adolescents., Method: The study was carried out with adolescents between the ages of 12-18 from clinical (N=168) and community (N=181) backgrounds. All participants were asked to complete the BPFSC-SF-TR, the Personality Belief Questionaire - Short Form (PBQ-SF), the Brief Symptom Inventory (BSI) and the Personality Inventory for DSM-5- Short Form (PID-5-SF) scales. Also, the Kiddie Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime Version-Turkish Version (K-SADS-PL-TR) was administered to the participants in the clinical group and their parents were asked to complete the Pediatric Quality of Life Inventory (PEDsQL). Test-retest correlations and the Cronbach's α coefficients were calculated., Results: BPFSC-SF-TR scores of both groups of participants positively correlated with the PBQ-BF borderline subscale, the PID-5-SF borderline related facets and the BSI scores, and negatively correlated with the PedsQL in the clinical group. Furthermore, the clinical group had higher total BPFSC-SF-TR scores than the community group, and their scores positively correlated with the number of diagnoses and psychiatric symptoms determined by using the KSADS-PL-TR. Exploratory and multi-group confirmatory factor analyses of the data of both groups supported a single factor structure. The Cronbach's α of the scale was 0.84 in the clinical group, and 0.79 in the community group. The test-retest reliability correlation coefficient of the scale was 0.71., Conclusion: The BPFSC-SF-TR is a valid and reliable tool for Turkish adolescents.

The most realistic approach in recent years is researching the resistance inhibition rather than synthesizing new compounds. In this study, we aimed to determine i) the effect of phenylalanine-argininebeta-naphthylamide (PAβN), on minimum inhibition concentrations (MICs) of ciprofloxacin (CIP), ii) to obtain the CIP+PAβN concentration that inhibits CIP resistance and iii) to show that this inhibition is caused by the effect of PAβN on the expression of efflux pump (EF) system genes. Acinetobacter baumannii isolates were collected from Trakya University Hospital. In 67 isolates determined to be resistant to CIP, CIP susceptibility was investigated in presence of PAβN once again. Isolates determined to have four or more fold decrease in ciprofloxacin MIC values were included in checkerboard assay and quantitative real-time reverse transcriptase polymerase chain reaction (qrRT-PCR). Fractional inhibition concentrations (FICs) were calculated through the PAßN concentrations that inhibit ciprofloxacin resistance, by the checkerboard assay results. With the combination of CIP+PAβN, the effect of the concentrations at which inhibition occurs, to the expression levels of EF system genes (adeA, adeB, adeR, adeS, adeF, adeG, adeH, adeL) was investigated by qrRT-PCR. By the checker board assay, a synergistic effect was determined between PAßN and CIP in 11 isolates, while in other isolates the effect was determined to be additive. In some isolates resistant to CIP, CIP + PAβN combination inhibited the resistance and increased CIP susceptibility. In the presence of 25 mg/L and 100 mg/L PAβN, 22 (32.83%) and 27 (40.3%) of 67 isolates became sensitive to CIP, respectively. In seven isolates, 12.5 µg/ml PAβN concentration eliminated CIP resistance by decreasing CIP MIC value to 1 µg/ml. Also, in one isolate the MIC value was 0.5 µg/ml in the presence of 25 µg/ml PAβN and 1 µg/ml in the presence of 1.5625 µg/ml PAβN. After analyzing the expression levels of EF genes (adeA, adeB, adeC, adeF, adeG, adeH, adeL, adeR and adeS) by the qRt-PCR method, it was determined that with the addition of PAβN to media containing CIP, the expression levels of the genes decreased (p<0.05). The aim of the study has been achieved with the results obtained. These results highlighted the importance of research on the inhibition of resistance, as well as the synthesis of new antimicrobial compounds. Combined use of inhibitors and antibiotics should be considered as an alternative treatment method. Thus, existing antibiotics can be included in the treatment again, saving time and money. It will be possible to use these findings in further studies to elucidate the mechanism of action of new inhibitor candidate compounds and associate them with the expression of DAP genes, also by investigating mutations in the regulatory gene regions in isolates with over-expression levels.

Idiopathic orbital myositis is an inflammatory disease of the extraocular muscles and is a rare disease of orbit. Diagnosis is based on clinical and radiological findings. Demonstration of edema and thickening of extraocular muscles in orbital magnetic resonance imaging and dramatic response to corticosteroids are considered a pathognomonic finding for orbital myositis. In this case, we aimed to discuss a rare case and emphasize the importance of differential diagnosis in headaches.

Biofilms are often responsible for the difficulties in the treatment of infectious diseases due to their properties that facilitate escape from antibiotic effect and their antiphagocytic effects. At least 65% of all infectious diseases are associated with biofilm-forming bacteria. As Staphylococcus aureus and Staphylococcus epidermidis are among the most common agents of hospital infections and the infections are mostly biofilm-related, they pose an important problem. In infectious isolates, the minimum biofilm eradication concentration (MBEK) values of biofilm forms are much higher than the minimum inhibition concentration (MIC) values of planktonic forms. This situation requires the use of much higher doses of antibiotics in the treatment of infections and causes an increase in antibiotic resistance. The N-acetylcysteine (NAC) molecule is known to be effective against biofilm by disrupting mature biofilms and reducing the adhesion of bacteria to surfaces. In this study, it was aimed to demonstrate i) the biofilm-forming abilities ii) the change in ampicillin and vancomycin MIC values in the presence of NAC molecules, iii) the change in the MBEK values of these antibiotics in the presence of NAC molecule and iv) the change in the expression levels of genes thought to be related to biofilm formation in the presence of the NAC molecule among S.aureus (n= 38) and S.epidermidis (n= 12) isolates isolated from various clinical specimens in Trakya University Health Research and Application Center. In this study, microplate crystal violet method was used to demonstrate the biofilm formation in staphylococci. Broth microdilution and checkerboard method were used to demonstrate the change in the presence of NAC molecule of the MIC and MBEC values of ampicillin and vancomycin. The effect of NAC on the expression of intercellular binding proteins A and D (icaA, icaD) and Staphylococcus regulatory protein A (sarA) genes, which are the genes involved in biofilm formation in staphylococci, was determined by quantitative real-time Polymerase Chain Reaction (qRt-PCR) method. The Student-t test was used to compare the control and experimental groups (concentrations detected with synergy and additive effect); p˂ 0.05 was accepted as the limit value of significance. In this study, when the NAC molecule was used together with ampicillin and vancomycin, it was determined that this combination lowers the MIC values of staphylococcus isolates and staphylococcal biofilm MBEK values; and also the expression levels of icaA, icaD and sarA which were effective in biofilm formation in staphylococci have not changed and decreased. As a result, in this study, it has been determined that the NAC molecule can be a new alternative for combined drug therapy and is promising in terms of bringing a new approach to treatment. In addition, it is thought that it is possible to use the NAC molecule together with different microorganisms and antimicrobial agents, and the results obtained in this study are considered to be guiding for further studies on this subject.

Healthcare workers are the group with the highest risk of COVID-19 transmission. The illness of healthcare workers poses a risk to patients admitted to the hospital, colleagues and households besides their own health. In this study, it was aimed to determine the risk assessment and the factors associated with risk status of an university hospital healthcare workers after risky COVID-19 contact. The data of the descriptive study were obtained from 773 follow-up records of 555 healthcare workers who applied with COVID-19 risky contact between 06.04-10.05.2020. Employees who were positive for RT-PCR evaluated as "patients" and others as "risky contact". Risk assessment was classified as no risk, low, medium and high risk contact according to the "Algorithm of Assessment of Health Workers with COVID-19 Contact" of the Ministry of Health. The relationship between the risk levels of the participants and their demographic and workplace characteristics and their usage of personal protection were evaluated. Mean, standard deviation, percentage, chi-square and ANOVA tests were used in the analysis of the data. The average age of the healthcare workers was determined as 34.4 ± 7.6 years. It was determined that 56.2% of those who had contact were female, 62.9% were married and 17.5% had an additional disease. It was determined that 45.6% of the risky contacts were nurses, 18.4% were supportive personnel and 16.9% were doctors. While 46.5% of the contacts were found as medium, 28.0% low, 17.1% high risk and 8.4% risk free. 38.2% of risky contacts occurred while working in internal/surgical clinics. While 66.0% of the employees had risky contact during patient care and treatment, 25% had risky contact with colleagues in social settings. High-risk contact was higher in social relations between employees. The source of the contact was a colleague in 73.2% of the employees. The average age of high-risk employees was smaller than those of low-risk. While 54.5% of the employees wore surgical masks during contact, 67.8% of the patients did not have a mask. Of 555 employees followed, 37 (6.7%) were diagnosed as COVID-19; 48.6% of the patients were nurses and 18.9% were doctors. It was determined that 48.6% of the healthcare workers were working in the COVID-19 service, outpatient clinic or intensive care unit at the time of diagnosis. The source of the infection was thought to be a colleague in 51.6% of the patients. COVID-19 was more common in nurses and doctors. It was determined that risky contact also occurred in services other than the units where COVID-19 patients were treated and risky contact often took place while providing healthcare to the patients and during social relations between the employees. Unprotected contact of the employees with each other in the workplace was identified as an important risk source. Inadequate use of personal protective equipment by healthcare workers led to an increase in medium and high risk contacts. The use of masks by patients and their relatives during the health service delivery and the proper use of personal protective equipment by healthcare professionals will reduce the risk. With the physical improvement of the rest areas of the employees and the arrangement of the breaks, the risky contact between colleagues in the social areas will be reduced.

Introduction: The patients in the intensive care unit have a higher risk of infections because of the poor general condition of these patients and the frequent application of invasive procedures with longer hospitalization length. Also, this group of patients tend to have resistant infections due to empirically widespread and uncontrolled use of broad-spectrum antibiotics. Hence, data are needed to determine appropriate empirical antibiotic therapy in intensive care patients. In this study, it was aimed to assess the distribution of microorganisms and antibiotic resistance profile from the samples taken from the patients in the intensive care unit., Materials and Methods: Patients who were hospitalized in Ankara University Faculty of Medicine Chest Diseases Intensive Care Unit for more than 24 hours between December 2016 and December 2017 were included in our study. Demographic characteristics, comorbid diseases, clinical findings, results of sputum, tracheal aspirate, blood, urine, gaita and pus culture samples and antibiotic susceptibility test results were recorded prospectively., Result: A total of 287 patients, 150 males and 137 females, were included in the study. The mean age of all patients was 69.96 ± 14.4 years. Two hundred twenty-three positive cultures were detected in 1053 samples taken from 287 patients. Gram-negative bacilli constituted 55.1% of the 223 positive cultures. The most common microorganisms were Acinetobacter (16.6%), Staphylococcus (14.8%) and Klebsiella (10.8%). Colistin resistance was found to be 8.3% in the Acinetobacter strains and resistance rates of 97-100% were observed to other antibiotic groups. Thirty-three staphylococcus were isolated, 17 were S. aureus and 16 were coagulase-negative Staphylococcus. While 29.4% of S. aureus were resistant to methicillin (MRSA), vancomycin resistance was not detected. Meanwhile, the MRSA ratio was 62.5%, there was no vancomycin resistance among the coagulase-negative Staphylococcus. Klebsiella was the third most common microorganism and beta-lactamase producing Klebsiella strain was 62.5%. Gentamycin was found to be the most susceptible antibiotic in Klebsiella strains with a resistance rate of 20.8%. Pseudomonas aeruginosa constituted 9.4% of the cultures. While the resistance to colistin was not detected, resistance to piperacillin/tazobactam 42.8%, tobramycin, imipenem and meropenem 50% and ceftazidime 61.9%. The duration of hospitalization in patients with Acinetobacter isolated (23 [10-34] days vs 12.5 [5-24] days, p= 0.011) and the mortality rate (62.5% vs 37.5%, p= 0.008) were significantly higher than those who were not Acinetobacter isolated., Conclusions: In conclusion, gram-negative bacilli constitute the majority of the patients in intensive care unit. Acinetobacter, the most common microorganism, has a high resistance rate and has been associated with prolonged hospitalization and mortality.

Objective: The present study aims to determine the effects of anxiety sensitivity, anxiety level, perceived stress and coping strategies on peritraumatic dissociation in post-earthquake acute stress disorder (ASD) patients., Method: Sociodemographic data form, Beck Anxiety Index (BAI), Perceived Stress Scale (PSS), Anxiety Sensitivity Index-3 (ASI-3), Coping with Earthquake Stress Scale, and Peritraumatic Dissociation Scale (PDEQ) were applied to 477 patients diagnosed with ASD., Results: Anxiety sensitivity cognitive sub-dimension explained 31.5%, anxiety explained 7%, and perceived stress explained 1% of the variation in peritraumatic dissolution development. A moderate positive correlation was determined between peritraumatic dissolution and anxiety, a weak positive correlation was found between peritraumatic dissolution and perceived stress, a weak positive correlation was determined between peritraumatic dissolution and positive thinking, and a very weak negative correlation was determined between peritraumatic dissolution and seeking social support. A moderate positive correlation was determined between peritraumatic dissolution and physical, cognitive and social sub-dimensions of anxiety sensitivity., Conclusion: The most important finding in the study was the fact that the highest contribution to the development of peritraumatic dissolution was by the cognitive sub-dimension of anxiety sensitivity. It could be suggested that individuals with high anxiety sensitivity may experience higher peritraumatic dissolution and these individuals could have a higher risk of PTSD later on.

The Acinetobacter calcoaceticus-Acinetobacter baumannii (Acb) complex consists of phenotypically very similar nosocomial species; A.baumannii, Acinetobacter nosocomialis, Acinetobacter pittii, Acinetobacter seifertii and Acinetobacter djikshoorniae and one environmental species A.calcoaceticus. The rapid and accurate identification of the members of Acb complex is critical as these nosocomial pathogens can show differences in antimicrobial susceptibility and clinical outcomes. The conventional phenotypic methods are slow, unreliable and less efficient for the differentiation of Acb complex species, including the A.baumannii species within the Acb complex. Although various molecular methods are available, such as amplified ribosomal DNA restriction analysis (ARDRA) and blaOXA-51-like gene specific PCR, they are usually inconvenient for the routine diagnostic laboratories. Recently, matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) offers an opportunity for rapid, cost-effective and convenient bacterial identification in routine diagnostic procedures conducted in clinical laboratory. In this study, we aimed to evaluate the diagnosis performance of MALDI-TOF MS system to identify blood isolates of A.baumannii. A total of 180 nonduplicate carbapenem resistant Acb complex (strain numbers; TR1-TR60) and A.baumannii (TR61-TR180) blood isolates were collected from the intensive care units of the three university hospitals in Turkey from January 2016 to December 2016. All isolates were evaluated by using blaOXA-51-like gene specific real time (Rt-PCR) analysis, ARDRA (restriction enzymes-AluI, CfoI, MboI, MspI, RsaI) method and MALDI-TOF MS (VITEK® MS, bioMérieux, France) system. All the strains except TR10, TR31, TR35 and TR52 were identified as A.baumannii by ARDRA method. Out of 177 of all the isolates, presence of blaOXA-51-like gene was found except for TR10, TR31 and TR52 isolates. However, TR31 without the presence of blaOXA-51-like gene was identified as A.pittii using the ARDRA. Totally 176 isolates which were identified as A.baumannii by both of the methods, ARDRA and Rt-PCR- blaOXA-51-like, were accepted as a reference for the evaluation of the diagnosis performance capacity of the MALDI-TOF MS. Overall, for all 176 isolates tested, the sensitivity obtained with the MALDI-TOF MS were 99.4% with 75% specificity. The accuracy value of the method was determined as 98.9% for the identification of A.baumannii to the species level. MALDI-TOF MS is increasingly used in diagnostic microbiology for the routine identification of bacteria to the genus, species or subspecies level with high rates of sensitivity and specificity. In future, by expanding the database, MALDI-TOF MS system would possibly become the ideal method for routine diagnostic laboratories that could potentially identify more species and even determine some characteristics of antimicrobial resistance and virulence determinants.

Introduction: Intensive care physicians are increasingly involved in decision making about the prognosis of intensive care unit ICU patients. With this study; we aimed to evaluate the power of clinician foresight at prediction of mortality in patient at triage to intensive care and patient follow-up., Materials and Methods: This study was conducted in ICUs located in various geographical regions of Turkey between January 1, 2017-April 30, 2017.The clinical research was planned as observational, multicenter, cross-sectional., Result: A total of 1169 intubated patients were followed in 37 different ICU. At the beginning of the follow-up we asked the physician who will follow the patient in the ICU to give a score for the probability of survival of the patients. Scoring included a total of 6 scores from 0 to 5, with the "0" the worst probability "5" being the best. According to this distribution, only 1 (0.9%) of 113 patients who were given 0 points survived. Three (6.1%) of 49 with the best score of 5 died. Survival rates were significantly different in each score group (r: -0.488; p<0.001). After the combined mortality estimation scores based on the clinical observations of the physicians (0 and 1 point score was combined as non-survive, 4 and 5 score was combined as survived) 320 of the 545 patients were estimated to be dead and 225 were predicted survival. Sensitivity and spesifity of scoring system to predict mortality was 91.56% (95% CI: 87.96-94.37), 76.89% (95% CI: 70.82-82.23) respectively., Conclusions: In this study, we concluded that the physicians who follow the patients in the ICU can predict the poor prognosis at the time of admission and the high mortality rate. The physician's opinion on mortality estimation should be considered in intensive care mortality scoring in addition to other laboratory and clinical parameters.

This study was aimed to investigate the anti-leishmanial effects of bee products (honey and propolis) by using the causative agent of cutaneous leishmaniasis Leishmania tropica promastigotes, in in vitro culture. In vitro anti-leishmanial efficacy of honey (pine, flower and chestnut) and propolis used in the study were evaluated using the microdilution method. Honey, which is a bee product, was dissolved with RPMI medium containing fetal calf serum (FCS) and diluted in the same medium, and serial dilutions were prepared in concentrations between 62.5-1000 mg/ml. Propolis, on the other hand, was dissolved with ethyl alcohol and only 2.5 μl was used from all these concentrations since the alcohol content was more than 50% in these concentrations prepared and we thought that this rate would negatively effect the parasite development. Then, RPMI containing FCS was diluted in the medium and serial dilutions were prepared at concentrations between 50-800 μg/ml. To the dilutions prepared, the promastigot suspension was added so that their final concentrations in the wells were 1 x 106 promastigot/ml and then the medium was incubated for 24 and 48 hours in 26°C. After the incubation, promastigotes were determined microscopically for morphology, mobility and live parasite density, and cell viability was determined by MTS method and 50% inhibitor concentrations (IC50) were compared with control groups. Anti-leishmanial activity of propolis (50, 100, 200, 400 and 800 μg/ml) and honey (62.5, 125, 250, 500 and 1000 mg/ml) on promastigotes was evaluated in vitro. In microscopic examinations, pine honey showed anti-leishmanial activity starting from 62.5 mg/ml, flower honey 250 mg/ml, and chestnut honey 125 mg/ml, and pine honey was more effective on promastigotes (p< 0.05), and propolis was effective from 100 μg/ml concentration. It has been determined that very low concentrations of propolis caused changes in the morphological structure of the parasites and were more effective than the other bee products. The prevention of cell proliferation and decreasing of the IC50 values according with the time of pine honey (IC50= 109.28 mg/ml), flower honey (IC50= 248.07 mg/ml), chestnut honey (IC50= 147.65 mg/ml) and propolis (IC50= 82.98 μg/ml) applied on L.tropica promastigot cell culture was determined by MTS method. In this study, it was found that various concentrations of pine, flower, chestnut honey and propolis showed anti-leishmanial activity on L. tropica promastigotes. It has been observed that pine honey is more effective on promastigotes after 48 hours of incubation period, and propolis is more effective in both morphology and cell inhibition of the parasites even at very low concentrations. It is believed that these data can be used as an alternative treatment method against cutaneous leishmaniasis infections and further studies are required.

Myiasis is defined as follows; some flies lay their eggs and larvae on live organs and tissues or on cavities of nose, eye, ear and etc of humans or animals and the larvae develop there and cause pathological disorders. One of the rare types of myiasis is the urogenital myiasis. This condition has been associated with covering urogenital area with clothes in almost all populations and therefore, less chance for flies to reach this site and lay their eggs. Low socio-cultural level and poor hygiene conditions are stated as the most important risk factors of myiasis. On the other hand, toilets within the human living spaces are ideal environments for fly larvae that are fed on bacteria as they contain moisture and a microbial environment. It is stated that especially in case of the presence of an underlying infection, flies are more prone to lay their eggs or larvae on this area due to the bad smell. In this case report, a patient with a moderate sociocultural level with no underlying chronic disease, no open wound in the genital area and no urinary tract infection was presented. A 53-year-old female patient living with her family in an apartment flat in the city center of Konya admitted to the outpatient clinic of infectious diseases with the complaint of moving worms in her urine. The patient stated that she had seen maggots while urinating for two weeks and that she passed her urine into a container to be sure that the maggots were coming from her urine, upon seeing the maggots there she has admitted to the hospital. The Larvae were defined as the fourth stage larvae of Psychoda spp. No open lesions or wounds were seen in genitourinary area of the patient who had no complaints other than mild urinary burning. Complete blood count test results of the patient were normal and there was no growth in her urine culture. It was stated that other family members whom she lived with did not have similar complaints. The patient was diagnosed as genitourinary myiasis caused byPsychoda spp. and informed about the relationship between hygiene and the disease. And urinary antiseptics were initiated as the treatment for the patient. After the treatment, no larvae were seen in the urine of the patient. In conclusion, many regions are suitable for Psychoda type of adult flies to live in our country due to the climate conditions. It is considered that infestation may develop in patients with less risk factors in terms of urogenital myiasis, that larvae may increase in number of patients who are not treated or followed up and that the infestation may become chronic and results in more serious clinical conditions. The necessary treatment and follow-up must be performed in such cases and the patients should be educated about the relationship between infestation and hygiene.

Introduction: Obesity has been considered to be one of the important risk factor for obstructive sleep apnea (OSA). We aimed to investigate the clinical and polysomnographic differences of obese and non-obese OSA patients and how these differences, if any, can be reflected in our daily clinical practice., Materials and Methods: The polysomnographic data of 157 consecutive patients that underwent a sleep study were analyzed. Ninety-nine adult patients with a diagnosis of OSA [apnea-hypopnea index (AHI) ≥ 5/hour] were retrospectively assesed in two groups in respect of obese (BMI ≥ 30 kg/m2) or non-obese (BMI <30 kg/m2) according to body mass index (BMI). The clinical and polysomnographic results of the patients in both groups were compared., Result: Between 2010 and 2011, 99 (31 K/68 M) of 157 patients underwent polysomnography were diagnosed with OSA (AHI ≥ 5/hour). Thirthy six patients with OSA (36.4%) were non-obese and 63 patients (63.6%) were obese. There were no differences in the symptoms related to OSA between two groups (p> 0.05). Although there was no statistical significance, the age of the obese patients (50.5 ± 12.7 years) was lower than those of the non-obese (54.9 ± 10.0 years) in polysomnography their total sleep time was shorter and sleep latency later, and sleep efficiency, arousal index, N3%, and REM% were lower and N1% was higher (p> 0.05). While in the obese OSA patients the AHI and Epworth Sleepiness sclae were higher (respectively; p= 0.028, p= 0.01), average oxygen saturation at sleep was lower (p<0.001). While obese OSA patients had similar ratio of hypertension and atherosclerotic heart disease with non-obese OSA patients, whereas diabetes mellitus was higher in obese OSA group (p= 0.036). The use of antidepressant drug was more common in the non-obese patients than the obese patients (p= 0.011)., Conclusions: Although obese OSA patients have a more severe disease compared to nonobese OSA patients, there were no significant differences in clinical and polysomnographics findings between two groups. Should be noted that OSA may occur in non-obese patients.

Objective: One of the METHODS used to assess autonomic nervous system dysfunction in the etiology of panic disorder (PD) is heart rate variability (HRV). HRV is controlled by the sympathetic and parasympathetic (vagal) branches of the autonomic nervous system and reflects the capacity of autonomic stimulation by the parasympathetic system. The aim of this study was to evaluate heart rate variability (HRV) time domain parameters based on twenty four hour holter ECG analysis among drug-naive patients with panic disorder (PD) without any other medical and psychiatric comorbidity., Method: The study group consisted of 41 patients with PD and 46 healthy controls. Participants were evaluated with SCID-1 for psychiatric diagnoses. Then Hamilton Depression Rating Scale (HDRS), Hamilton Anxiety Rating Scale (HARS), Panic Disorder Severity Scale (PDSS) and Clinical Global Impression Scale (CGI-S) were applied to participants. Twenty four hour Holter ECG outcomes were analyzed on a computer program and time domain parameters were evaluated., Results: Among the parameters analyzed from HRV, SDANN was significantly higher (p <0.001); duration of RMSSD, NN50 and pNN50 were lower in PD group than the control group (p = 0.003, p = 0.005, p = 0.047, respectively). In the correlation analysis, there was a moderate negative correlation between CGI-S and NN50 and pNN50. In logistic regression analysis, the increase in SDNN was found to increase the probability of PD by 1.11 (95% CI, 1.010-1.209); the increase in SDANN was found to decrease the probability of PD by 0.892 (95% CI, 0.818-0.973), and the increase in pNN50 was found to decrease the probability of PD by 0.523 (95% CI, 0.342-0.801)., Conclusion: The data obtained in our study confirm that there is a decrease in some HRV parameters like RMSSD, NN50 and pNN50 reflecting parasympathetic activity among patients with PD.

Introduction: In this study, we aimed to determine the values of anthropometric measurements and rates used in the evaluation of obstructive sleep apnea syndrome (OSAS) in our country., Materials and Methods: Twenty accredited sleep centers in thirteen provinces participated in this multicenter prospective study. OSAS symptoms and polysomnographic examination and apnea-hypopnea index (AHI) ≥ 5 cases OSAS study group; patients with AHI < 5 and STOP-Bang < 2 were included as control group. Demographic characteristics (age, sex, body mass index-BMI) and anthropometric measurements (neck, waist and hip circumference, waist/hip ratio) of the subjects were recorded., Result: The study included 2684 patients (81.3% OSAS) with a mean age of 50.50 ± 0.21 years from 20 centers. The cases were taken from six geographical regions of the country (Mediterranean, Eastern Anatolia, Aegean, Central Anatolia, Black Sea and Marmara Region). Demographic characteristics and anthropometric measurements; age, neck, waist, hip circumference and waist/ hip ratios and BMI characteristics when compared with the control group; when compared according to regions, age, neck, waist, hip circumference and waist/hip ratios were found to be statistically different (p< 0.001, p< 0.001, p< 0.05, respectively). When compared by sex, age, neck and hip circumference, waist/hip ratio, height, weight and BMI characteristics were statistically different (p< 0.001, respectively). Neck circumference and waist/hip ratio were respectively 42.58 ± 0.10 cm, 0.99 ± 0.002, 39.24 ± 0.16 cm, 0.93 ± 0.004 were found in women., Conclusions: The neck circumference was lower than the standard value in men, but higher in women. The waist/hip ratio was above the ideal measurements in both men and women. In this context, the determination of the country values will allow the identification of patients with the possibility of OSAS and referral to sleep centers for polysomnography.

Pulmonary metastases are usually seen as parenchymal nodules, lymphatic-interstitial spread and pleural effusion, however cavitary and cystic metastatic lesions are rare. While breast cancer, gastrointestinal tumors, kidney tumors, malignant melanoma, sarcomas, lymphoma and leukemia mostly metastasize to the lung, endometrial adenocarcinoma rarely metastasizes. A 73-year-old woman with multiple cystic-cavitary lung lesions was referred to our clinic because of chronic cough. She had endometrial adenocarcinoma metastasis to the lung. We wanted to present our case because of the rare occurrence of cystic-cavitary lung metastases and the rare presentation of metastasis of endometrial carcinomas to the lung.

Leptomeningeal metastasis is a very rare complication of infiltration of leptomeninges and subarachnoid space with malignant cells. It is an indicator of poor prognosis. Its incidence is 3.8% in non-small cell lung carcinoma (NSCLC). This rate is higher in patients with epidermal growth factor receptor (EGFR) mutation. Brain magnetic resonance imaging (MRI) is the first choice in the diagnosis. The diagnosis of leptomeningeal metastasis is difficult and often bypassed because it is rare and does not cause gross mass lesions such as brain metastasis. Systemic chemotherapy, intrathecal therapy, cranial radiotherapy and targeted treatment agents are an option in the treatment. It has been shown that targeted therapies can be promising because of the ability to switch to cerebrospinal fluid in appropriate patients. We present the case with EGFR positive lung adenocarcinoma whit leptomeningeal metastasis (LM) due to its rarity, difficulty in diagnosis and its association with EGFR mutation.

IgG4-related disease (IgG4-RD) is a set of diseases that can affect multiple organs, produce an immune-mediated fibroinflammatory response, and lead to tissue destruction and organ failure. Hemophagocytic syndrome is a life-threatening hyperinflammatory fatal disease caused by defect and excessive macrophage activity in natural killer cells. The disease can often be confused with other immune-mediated diseases such as cancer, infection, interstitial lung disease, sjogren's syndrome, wegener's vasculitis, or temporal arteritis. Hemophagocytic syndrome is defined as hemophagocytic lymphohistiocytosis (HLH) which is a pathological and clinical condition caused by phagocytosis of erythrocyte, leukocyte, platelet and precursor cells which are the cellular elements of the macrophages which are activated due to various reasons. Although there are two types as primary (familial) and secondary (depending on infections), the clinical findings are the same. Presence of familial disease/known gene defect and/or at least 5 of 8 clinical and laboratory diagnostic criteria is required for diagnosis. The first target is the suppression of hyperinflammation urgently, the second is the elimination of the stimulus triggering the event. In secondary HLH, treatment should be planned according to the underlying cause. As a rare complication of a rare disease, a case with the diagnosis of IgG4-related disease complicated with hemophagocytic syndrome is presented with the literature.

Introduction: Asthma is a heterogeneous and chronic inflamatory disease that can accompany many comorbidities during the course. It is thought that different comorbidities are seen in different phenotypes of asthma. Our aim is to establish the relationship between systemic comorbidities seen in asthmatic patients and asthma control, severity and phenotypes., Materials and Methods: Patients who were followed up with asthma diagnosis in Polyclinics of Ankara University Faculty of Medicine Department of Immunology and Allergy Diseases were questioned about demographic characteristics. Their control status and disease severity were determined. They were classified whether r atopic, eosinophilic or obese. Systemic comorbidities were questioned and Charlson comorbidity indices (CCI) were calculated. The difference between the groups in the terms of CCI was evaluated., Result: Two hundred and twenty-nine patients (29E/200K, 12.7%/87.3%) with a mean age of 51.25 ± 12.02 were included in the study. CCI was significantly higher in patients without asthma control than in those with partial control and well control (CCI: 2.22, 1.69, 1.50, respectively) (p= 0.03). There was also a linear correlation between asthma severity and CCI (CC: 0.22, p= 0.001). Allergic comorbidities were more frequent in the eosinophilic phenotype (p= 0.01) (OR: 2.20, 95% CI: 1.2-3.8) but did not increase the likelihood of accompanying systemic comorbidity (OR: 1.05, 95% CI: 0.8-1.2) (p> 0.05). The likelihood of systemic comorbidity, especially HT, coroner artery diseases, depression, was higher in nonatopics than in atopic patients (OR: 2.039 95% CI: 1.04.11) (p= 0.03). Obesity was found to be a risk factor for systemic comorbidities (OR: 1.36 %95 C1: 1.09-1.84) (p= 0.04)., Conclusions: Severe, uncontrolled, obese or nonatopic asthma patients should be examined for systemic comorbidities. There is a need for further study to assess the relationship between treatment of established comorbidities and asthma course.

Organ donation is a matter of concern in critically ill patients who need intensive care after carbonmonoxide (CO) intoxication. A 26-year-old female patient was unconscious after having spent 20 minutes in the bathroom with a water heater (70% butane and 30% propane mixture). In the CT of the patient with Glasgow Coma Scale (GCS) 3, ventricular system was erased, white-gray matter separation was lost and fissures were observed as erased. Torax CT showed conspicuous areas showing air bronchograms in both lung bases and posterior areas and diffuse frosted glass densities in other areas. In a patient with no improvement in consciousness; apnea test, neurological examination and CT angiography showed that there was no blood flow in the brain and brain death was confirmed. On the second day of hospitalization, the relatives gave their approval for the donation. It may also be advantageous to use oxygen at high concentrations in carbonmonoxide poisoning as it may allow recruitment of closed alveoli. Successful kidney, heart, lung, liver and pancreas transplants from donors exposed to CO have been reported. Carbonmonoxide intoxication may be appropriate for transplantation for patients who are waiting for organs of brain death cases where the organ preservation is well done. Although carbonmonoxide intoxication has failed in the literature, it shows that there is no definite contraindication.

Objectives: The aim of this study was to investigate patient satisfaction with nursing practices regarding postoperative pain management., Methods: A systematic review of the literature published between 2005 and 2015 was conducted, comprising a search of 7 databases: ScienceDirect, Turkish Medline, PsycINFO, Ovid, Prequest, Google Scholar, and the ULAKBIM Turkish Medical Database. Five articles were identified as eligible for review. The methodological quality of the studies was assessed using the Turkish version of the Joanna Briggs Institute-Meta Analysis of Statistics Assessment and Review Instrument (JBI -MAStARI) Critical Appraisal Checklist. The data were extracted using a standard data extraction form developed for this study, and subsequently evaluated., Results: In the articles eligible for this review, the study samples comprised patients with blunt or penetrating trauma sent to the emergency room (n=418) and those who underwent total knee replacement (n=120); heart, lung, or gastrointestinal system surgery (n=559); or open heart surgery (n=52). The maximum pain score was 7.20±1.95 (min-max: 0-10) and the maximum satisfaction score was 100%. The findings revealed that pharmacological methods were most frequently used for postoperative pain management and non-pharmacological methods were not used sufficiently by nurses. Nevertheless, the patients were satisfied with the postoperative pain relief nursing care., Conclusion: In this systematic review, scientific evidence indicated that patient satisfaction with nursing practices related to postoperative pain management was high; however, non-pharmacological methods were used insufficiently in the last decade.

Objective: There is a lack of evidence regarding clinical predictors for the treatment response to lithium, which is the main stay treatment option for bipolar disorder. Studies that examined the mechanistic action of lithium revealed that glycogen synthase kinase 3β (GSK-3β) enzymeinhibition was important in regard to treatment responses. Based on this background, we aimed to investigate the association between responses to lithium treatment and five different polymorphisms of GSK-3β., Method: Lithium treatment response scale (LTRS) scores for 100 patients diagnosed with bipolar disorders type I were calculated according to the hospital records. Blood samples were collected and genomic DNA was obtained using the MagNA Pure Compact automatic isolation method. The GSK-3β: rs17183904, rs17183897, rs34009575, rs34002644, and rs17183890 polymorphisms were analyzed by real time PCR., Results: In this cohort, the mean age of patients was 41.1±10.3 years, the mean age of disease onset was 24.5±8.2, and the mean LTRS score was 4.9±1.8. There was no statistically significant difference for LTRS scores between groups in terms of gender, marital status, level of education, and the type of first episode. LTRS was significantly higher in only the patients harbouring GSK-3β rs17183890 AG genotype (p=0.008, t:2.71). Interestingly, no differences were found for the remaining polymorphisms., Conclusion: The specific GSK-3β polymorphism that associated with lithium-response in our study may help to predict lithium responses and to develop individualized treatment. We presume that our pharmacogenomic findings may also provide important contributions to the clinical practice in regard to future evaluation of the treatment adherence and side effects. To obtain these goals, further genome-wide scanning studies conducted on larger sample cohorts are required.

Objective: In this study, it is aimed to evaluate the reliability and validity of the Turkish version of Bipolar Prodrome Symptom Scale., Method: The study was carried out with subjects who participated the study on the epidemiology of bipolar disorder among the students in the 1st grade of Celal Bayar University. The sample consisted of 30 volunteers diagnosed as bipolar disorder and 122 healthy controls. For concurrent validity, Hypomania Checklist-32-Revised was used. In the statistical analysis, internal consistency coefficient, item-total score correlation coefficients, exploratory factor analysis, correlation with concurrent scale and ROC curve were calculated., Results: Translation into Turkish and back-translation into English of were performed and thus the semantic harmony of the scale was obtained. In the internal consistency, Cronbach alpha coefficient was between 0,969-0,979 and item-total score correlations were between 0,767-0,929 and 0,725-0,890. In factor analysis, for the severity subscale a one-factor solution representing 78,9% of the variance and for the frequency subscale one-factor solution representing 71,7% of the solution are obtained. Correlation of Bipolar Prodrome Symptom Scale with Hypomania Checklist-32-Revised was r=0,513 and 0,530. In the ROC analysis, area under the curve was 0,977 and 0,999. The scale discriminates well between the bipolar group and healthy control group., Conclusion: Bipolar Prodrome Symptom Scale developed for screening hypomania is reported to be reliable and valid in Turkish.

Demodicosis is an ectoparasitic cutaneous infestation of pilosebaceous unit caused by Demodex mites. Demodex spp. are considered to be pathogenic when they increase in number and infect dermis.They can be the cause of diseases such as pityriasis folliculorum, papulopustular rosacea, granulomatous rosacea, inflammatory papule, acne vulgaris, perioral dermatitis, blepharitis and folliculitis. These mites can be found in most mammals including human. Canine and feline demodicosis is a well-known example of severe dermatitis caused by the proliferation of Demodex mites. Dogs are one of the most popular pets worldwide with well-being feeling to their owners. However, they may cause a health risk to humans due to their zoonotic diseases potential. Demodicosis is a severe and prevalent dermatologic disease in dogs. In this case report, a Demodex case thought to be related with a dog infestation was presented. A twenty-year-old girl student who feeds a Miniature Pinscher crossbred dog in her house for a while noticed common itchy papulopustular lesions, incrustation and some alopecic loci in her dog and admitted to Selçuk University Faculty of Veterinary Medicine, Department of Parasitology. Itchy papules and pustules were also observed in dog's owner face and arm about three or four weeks later. Skin scrapings and hair samples were taken from the patient and the dog and examined with light microscope. Adult and egg forms of Demodex spp. were detected in both patient and dog specimens in microscopic examination. Ivermectin treatment was recommended as 0.2 mg/kg for the dog. The owner of the dog was sent to a dermatology clinic and topical permethrin was recommended for the treatment. At the end of the first month the dog was controlled again, a whole clinical and microscopic improvement was observed and all of the lesions were disappeared for both the dog and its owner. This demodicosis case, thought to originate from a dog, is a rare condition because of Demodex mites are known to be highly host specific and not a zoonotic parasite. Although host specific cross infections between humans and animals have been rarely reported, D. canis was determined to be more transmissible across species than the other Demodex mites as indicated in the literature. As a result, it is necessary and important to keep in mind about Demodex spp. infestation in patients with skin complaints and pet feeding.

Introduction: Oral nutritional supplement (ONS), an important part of multidisciplinary pulmonary rehabilitation (PR) program, is indicated according to body composition abnormalities in the patients with chronic obtructive pulmonary disease (COPD). We evaluated efficacy of oral nutritional supplement (ONS) during 5-8 months in patients with COPD who completed PR program., Materials and Methods: The data of 41 patients with stable COPD were recorded after the approval had been taken. Patients were grouped according to ONS duration. Group 1 consisted of patients who took ONS during 2 months, group 2: 5 months, group 3: 8 months. In all patients, pulmonary function tests, quality of life, exercise capacity and body composition datas were recorded before and after PR program, at 3rd, 6th month and in 20 patients 1 year follow-up visit., Result: In all groups, improvements after PR in exercise capacity, dyspnea, and body composition sustained at 3 and 6 month. Quality of life was protected in group 1 during six month, in other groups it was over values before PR despite loss at 6. month. All improvements after PR were similiar between groups. In 20 patients with one year follow up, while the improvements in dyspnea and body composition were preserved, both exercise capacity and quality of life decreased, exercise capacity was lower than values before PR., Conclusions: In patients with COPD, after multidisplinary PR program, the improvements in dyspnea, exercise capacity, quality of life were maintained during six months regardless of body composition and ONS duration. Furthermore, while the improvements in body composition, dyspnea, quality of life were protected, exercise capacity was found to be lower than baseline.

Lung cancer still remains the leading cause of cancer death among all the cancer types. Early diagnosis is the most important factor for efficient treatment and disease management. Nowadays, several new methodologies are being used in clinical practise for diagnosis, staging and treatment of disease. Therefore, survival is prolonged even in patients who are not eligible for surgery. This has led to increase in the acceptance of lung cancer patients in intensive care units (ICU) due to both the disease and the treatments applied and also due to the comorbidity of the patients. However, it is unclear which lung cancer patient will benefit from intensive treatment. In this review, we shared the ICU admission reasons and prognosis of the early stage and advanced stage lung cancer patients and when these patients were referred to ICU and treatment modalities in ICU were discussed.
