POSTOPERATIVE nausea & vomiting , *MUSCLE relaxants , *SUGAMMADEX , *BARIATRIC surgery , *FILES (Records) - Abstract
Objective: Postoperative nausea and vomiting (PONV) is common in laparoscopic bariatric surgeries (LBS). The aim of this study is to investigate the effect of muscle relaxant antagonists on PONV seen in the recovery unit by morbidly obese patients undergoing LBS. Material and Method: With the approval of the University Ethics Committee, the files and e-archive records of morbidly obese patients who had LBS were examined, and their demographic data, PONVscore (1: No nausea-vomiting, 2: Nausea, 3: Retching, 4: Vomiting), amount of antiemetic administered in the recovery unit were recorded. According to the muscle relaxant antagonist drugs administered at the end of the surgery, the patients were divided into the neostigmine group (GrupNEO) and the sugammadex group (GrupSUG). Results: A total of 206 patients were included in the study and 80 of the patients were male and 126 were female. There was no statistically significant difference between the groups when the PONV score was evaluated (p >0,05). PONV that occurred by the patients in the recovery unit was 58,16% (57/98) in GroupNEO and 50% (54/108) in GroupSUG, and there was no statistical difference (p >0,05). Regarding the metoclopramide administered in the recovery unit, the dose was lower in GroupSUG than in GroupNEO, and the difference was statistically significant (p =0,05). Conclusion: In our study, in which we compared the effects of muscle relaxant antagonists on the incidence of PONV after LBS in morbidly obese patients, it was observed that while less PONV was observed in the postoperative recovery unit with the use of sugammadex, there was a significant decrease in the dose of antiemetic used. Aim: Binge eating disorder (BED) is the most common eating disorder among bariatric surgery candidates. BED may pose a risk to postsurgical outcomes. This study aims to determine the predictors of BED and the impact on the psychiatric comorbidity and quality of life for patients with severe obesity who underwent bariatric surgery. Material and Methods: A total of 207 patients with severe obesity who underwent bariatric surgery were included. Face-to-face psychiatric interviews were performed to diagnose BED according to the DSM-5 diagnostic criteria. A sociodemographic and clinical form, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), body image scale (BIS), and short form-36 health survey were administered to the participants. Results: The rate of BED was determined as 30.9%. BED was associated with gender (female), lifetime suicidal ideation, previous suicide attempts, age of onset of obesity and the age of onset of dieting. Patients with BED presented with poor scores in BDI, BAI, BIS, and most domains of quality of life. Conclusion: Decreased BIS score, younger ages of onset for dieting, and previous suicide attempts predicted BED. The recognition of factors involved in the development of BED in patients with severe obesity will improve the effectiveness of treatment options for these patients

INTRODUCTION[|]This study aimed to investigate depression-anxiety levels, sexual lives, childhood traumatic experiences, self-esteem, eating attitudes, and Type D personality traits in bariatric surgery candidates. [¤]METHODS[|]Our study was conducted between June 2019 and December 2019 and included a total of 100 individuals, 50 bariatric surgery candidates and 50 healthy people. A sociodemographic data form, the Beck Depression Inventory, the Beck Anxiety Inventory, the Childhood Trauma Questionnaire, the Type D Personality Scale, the Arizona Sexual Experiences Scale, the Rosenberg Self-Esteem Scale, and the Eating Attitudes Test were used in our study.[¤]RESULTS[|]The scores of Type D Personality, Arizona sexual experiences, childhood trauma, eating attitude, and self-esteem scales were significantly higher in the patient group than in the control group. There was no statistically significant difference between the anxiety and depression scores of both groups. In addition, it was found that the predictive factors of the Body Mass Index were education, self-esteem, eating attitude, and childhood traumas, explaining 63.5% of the variance. Type D personality, on the other hand, was not found to be one of the predictive factors.[¤]DISCUSSION AND CONCLUSION[|]Bariatric surgery candidates undergoe routine psychiatric examinations prior to their operations, and preoperative follow-up interviews with a psychiatrist is believed to be important for these individuals.[¤]

OVERHEAD costs , *PAYMENT , *UNIVERSITY hospitals , *BARIATRIC surgery , *COST analysis , *GASTRIC bypass - Abstract
Objective: The aim of this study is to determine the service cost of bariatric surgery within the scope of diagnosis-based package price payment which is one of the reimbursement methods in Turkey, and to determine to what extent the reimbursement amount specified in the Health Application Notification covers the service cost. Material and Method: The study is a descriptive and the relevant data were obtained retrospectively. All patients who had bariatric surgery in two university hospitals in Konya/Turkey in 2019 constitute the study population. It was determined that only the services with the P609871 (Obesity, By-pass), P609872 (Obesity, Sleeve) Health Application Notification codes were provided to 70 patients, but the patients who were determined to have different operating codes were excluded from the scope of the study and the study was conducted with 66 patients. Results: In terms of university hospitals, the income-to-expense ratio for obesity bypass surgery is 48.90%, and 39.19% for obesity sleeve surgery. In general, except for the fixed costs for bariatric surgery, the amount paid by Social Security Agency (SSA) within the scope of Health Application Notification covers approximately 40.05% of the cost of university hospitals. It has been determined that the material cost has the highest share among the cost elements. Conclusions: As a result of the cost analysis, it was determined that university hospitals made a loss for the relevant services. It has been concluded that the diagnosis-based package price payment method in university hospitals cannot cover the costs and that threatens the financial sustainability of university hospitals serving with Health Application Notification prices. Aim:Obesity has become a rapidly increasing public health problem all over the world. Obesity itself is a risk factor for cholelithiasis, and the fast weight loss period after bariatric surgery is a situation that increases this risk. The fact that both obesity and surgical treatment of obesity increases the formation of stones in the gall bladder has made it even defendible that cholecystectomy should be performed routinely during the bariatric surgery at one stage, even if there is no cholelithiasis. In this study, we aimed to evaluate our gallbladder approach in patients who were decided to undergo bariatric surgery in our center, together with the literature reviews. Methods:In our study, the data of 185 patients who underwent bariatric surgery due to obesity in the University of Health Sciences Kanuni Sultan Suleyman the in Istanbul Training and Research Hospital Clinic of, Department of General Surgery, between 2018 and 2020 were retrospectively obtained. A total of 185 patients were included in the study. The patients were divided into group 1 (sleeve gastrectomy) and Group 2 (gastric bypass) according to their operation techniques. Results:The average age was 36.43 ± 9.52, the ratio of women/men was 151 (81.6%)/34 (18.4%). Mean body mass index (BMI) was determined as 44.16±5.09. In the postoperative period, gallstones were formed in 27 (14.6%) patients, whereas gallstones were not detected in 158 (85.4%) patients. There was no significant difference between the groups in terms of postoperative gallstone formation. Conclusion:In the light of the literature and after our clinical experience, we do not require routine imaging of the gallbladder before bariatric surgery in asymptomatic patients, and we recommend performing concomitant cholecystectomy only in symptomatic patients.

Obesity, which continues to be widespread from past to present, is a disease to have physical, social, environmental and psychological effects. The presence of many causes of obesity and its comorbid diseases results in serious financial consequences in the diagnostic and treatment process. The methods chosen for the treatment of obesity varies according to the person's body mass index and the presence of comorbid disease. One these methods is bariatric and metabolic surgery (obesity surgery), and it is applied in the case of having additional diseases to patient’s obesity and preferences of surgeon in addition to obesity. The most important thing to know about this issue is that obesity surgery is involved in a very comprehensive treatment. After surgery, the person should be followed with a multidisciplinary approach by an internal medicine specialist, a dietician specializing in bariatric surgery, a psychiatrist or psychologist specializing in nutrition and eating disorders. One of the most important problems encountered before and after the operation, which may adversely affect the success of the operation, is binge eating disorder. This article focuses on how binge eating disorder and mood influence obesity surgery.

Objective: Psychiatric evaluation of candidate patients before bariatric surgery (BS) has an important place in the success of the treatment. In this study, it was aimed to examine the relationship between childhood trauma (CT) and body image, self-esteem and eating attitudes of individuals who applied for BS., Method: A total of 87 BS candidate patients with morbid obesity, 57 women and 30 men, were included in the study. Sociodemographic Information Form, Childhood Trauma Questionnaire (CTQ), Body Perception Scale (BPS), Rosenberg Self-Esteem Scale (RSES), and Eating Attitude Test (EAT-40) were used as data collection tools in the study., Results: CT was detected in 47.1% of the cases. RSES (t=3.296; p<0.01) and BPS (t=3.267; p<0.01) scores were found to be significantly higher in those with a history of CT. A positive and significant relationship was found between EAT-40 and CTQ -sexual abuse (SA) sub-dimension (r=0.570; p<0.01). A significant relationship was found between all subdimensions of CTQ and RSES. A significant relationship was found between CTQ physical neglect (PN), emotional neglect (EN), and emotional abuse (EA) sub-dimensions and BPS. In addition, CTQ total score was found to significantly and negatively predict self-esteem (β=- 4.432; p<0.001) and body image (β=-3.700; p<0.001)., Conclusion: In our study, it was found that those with CT were dissatisfied with their bodies and had lower self-esteem. Questioning CT in the psychological evaluation of pre-BS cases may contribute to the understanding of the etiology of obesity and may play an important role in planning the follow-up after BS.

Bariatric surgery is considered to be a highly effective treatment in obese individuals, as it reduces weight loss, obesity-related concomitant diseases and mortality, and improves the quality of life. All Bariatric procedures alter, to varying degrees, the anatomy and physiology of the gastrointestinal tract; These changes make patients more susceptible to deficiencies in macro and micronutrients that can lead to diseases such as anemia, osteoporosis, protein malnutrition. Most obese patients have some nutritional deficiencies before surgery. For this reason, patients should be subjected to a full nutritional assessment before the operation and these deficiencies that exist before the operation should be corrected. Nutritional complications associated with bariatric surgery can be prevented by lifelong nutrition monitoring by administering multivitamins and mineral supplements according to the patient's needs. In this study, literature about long-term complications occurring after bariatric surgery is compiled. [ABSTRACT FROM AUTHOR]

Obesity is a common health problem with serious complications. Medical and surgical methods can be used in treatment. Laparoscopic sleeve gastrectomy (LSG) is a widely used surgical procedure. LSG is a high-risk surgery because of the effects of both existing obesity and the applied laparoscopic technique. Anesthesia plan from preoperative examination to perioperative management and postoperative care should be done carefully. We aimed to share our experience in the anesthetic management of a patient who underwent a LSG operation which was performed for the first time in our hospital.

Intragastric band migration (IGBM) is one of the major complications of gastric banding. In this report, we aimed to present a case of IGBM, which was diagnosed intraoperatively, and to review the relevant literature. A 59-year-old male patient was admitted to our outpatient clinic due to epigastric pain persisting for the past three months. The patient had a history of gastric banding surgery owing to obesity with open surgery nine years ago. Postoperative follow-up was not done properly and the patient had started to gain weight in the third postoperative year. Incisional hernia was found in physical examination and operation for gastric band removal and hernia repair was planned. During surgery, the band could not be found around the stomach, therefore, gastroscopy was performed and it was found that the majority of the band was placed in the stomach. The patient was intraoperatively diagnosed with IGBM and the band was removed through gastrotomy, and hernia repair was performed. The patient was discharged at postoperative 6th day without any complication. Although IGBM is rarely seen, it should be considered as a long-term complication in cases with dysfunctional gastric band and in patients who started to gain weight after operation. Treatment is the removal of the band review.

Aim: Although bariatric surgery for elderly obese patients is having high complication risks and with indefinite results, can be still beneficifial. The aim of this study is to evaluate the results of sleeve gastrectomy in elderly patients. Material and Method: All 65 years or older patients underwent laparascopic sleeve gastrectomy operation between April 2013-September 2017 included in this retrospective study. Demographic data, comorbidities due to obesity, body mass index, pre and post operative weight, recovery from comorbidities, followup time, postoperative complications, and duration of hospital stay were recorded. Elderly patients and patients under 65 years underwent sleeve gastrectomy were comparised according to results. Results: Fourteen patients (mean age 66.9±1.9 years) operated on for laparascopic sleeve gastrectomy. Mean 52.9% BMI loss obtained in the patients after 19-month follow-up. In the elderly patients, significant improvements were obtained for the comorbidities due to obesity. Complication rates were similiar among the two groups. Conclusion: Laparascopic sleeve gastrectomy is an effecitive and safe operation in elderly obese patients, but weight loss is more tolerable compared to younger patients. A selective group of elderly patients may benefit from bariatric surgery. [ABSTRACT FROM AUTHOR]

Aim: To investigate the role of non-contrast magnetic resonance imaging (MRI) and findings of quantitative MRI in the evaluation of pancreatic lipomatosis in patients undergoing bariatric surgery. Methods: Pre-operative and post-operative MRI findings and laboratory test results of 44 obese patients, who underwent sleeve gastrectomy, were reviewed retrospectively. Liver craniocaudal length, hepatic and pancreatic region of interest (ROI) ratios at in-phase and out-of phase sequences (in-phase/outof phase), pancreas/spleen ROI ratio on T2-weighted sequence (T2 ratio) and thickness of subcutaneous fat were estimated. The relationship between pre-operative and postoperative findings was analyzed. Results: A total of 44 (41 female, three male) patients with a mean age of 42 were included in the study. Liver length, hepatic and pancreatic in-phase/out-of phase ratios, pancreas/spleen T2 ratio and thickness of subcutaneous fat in the post-operative period were found to be lower than those in the pre-operative period (p<0.05). When pancreas/spleen T2 ratio ≥1 was considered abnormal for the diagnosis of pancreatic lipomatosis, high specificity (100%), positive predictive value (100%) and accuracy (75%) were obtained. Conclusion: Non-enhanced MRI with quantitative measurements, including pancreas/spleen T2 ratios, can be performed efficiently for the diagnosis of pancreatic lipomatosis and patient follow-up. Aim: The aim of this study is to determine the possible nutrient and protein deficiency in the first six months after sleeve gastrectomy and to determine its effect on some biochemical parameters. Material and Methods: This study is an intervention study including 102 patients (75 female and 27 male) aged between 18-65 years, who had undergone sleeve gastrectomy operation at the General Surgery Clinic of Şişli Florence Nightingale Hospital between 13.07.2017 and 06.01.2018 and been followed up by a dietician for at least six months postoperatively. Anthropometric measurements, three-day food intake, and biochemical parameters were followed pre-operatively and during the first six months. Results: There was a 27.4% decrease in body weight of the patients in first six months after surgery. Both decrease in body weight, and decrease in body fat ratio and body mass index were found statistically significant (p<0.001). A statistically significant difference was found between preoperative and postoperative 6th month biochemical parameters; serum HbA1c, insulin, uric acid, total protein, total cholesterol, HDL-cholesterol, LDL-cholesterol, triglyceride, iron, iron binding capacity, ferritin, zinc, vitamin D, calcium, parathormone, folate and vitamin B12 levels of the patients (p<0.001). The difference between 6th month serum total protein levels of patients using and not using whey protein powder in the first month after sleeve gastrectomy was found statistically significant (p=0.002). Conclusion: It should be kept in mind that regulating patients' lifestyle with post-operative multidisciplinary team and regular follow-up, and supporting with active life are necessary for obesity treatment to be sustainable. Objective: Obesity is a worldwide public health problem, which increases morbidity and mortality. Bariatric surgery has become increasingly popular in the last decade and is a preferred treatment for obesity. The aim of this study is to evaluate the obese patients who are candidates for bariatric surgery by psychiatric clinical interview and psychometric tests. Material and Methods: 48 obese patients who were consulted to evaluate the psychiatric suitability of bariatric surgery were included in the study. Sociodemographic data form, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Rosenberg Self-Esteem Scale (RSES), Coping Strategies with Stress Inventory (COPE), Dutch Eating Behavior Questionnaire (DEBQ) and Short Form of Barratt Impulsiveness Scale (BIS-11) were applied to the patients included in the study. Results: Of the patients included in the study, 32 (66.7%) were female and 16 (33.3%) were male. The mean body mass index (BMI) of the patients who are bariatric surgery candidates was found to be 45.23 ± 8.97, there was no difference between the two groups. There was no statistically significant difference in BDI scores between genders, and BAI scores were higher in women. COPE nonfunctional coping subgroup scores were found to be significantly higher in women. There was no significant difference between the sexes in the evaluations of DEBQ, RSS-self-esteem subscale and BIS-11. When the patients were divided into two groups as obese and morbidly obese, and mean scores of BDI and BAI were compared, it was found that both of the scores in the obese group were higher than the morbid obese group. Conclusion: Obesity is a multisystem disorder with mental components. Considering the mental components of obesity and their differences between genders should be evaluated as an important step in facilitating the patient's compliance with treatment with both surgical and non-surgical methods and in preventing recurrences. [ABSTRACT FROM AUTHOR]

- Abstract
With the rising prevalence of morbid obesity, the numbers of bariatric surgical procedures are also increasing, thus resulting in more neurological complications to be recognized. Among these complications, the most well-defined are Wernicke's encephalopathy and peripheral neuropathies. Guillain Barre Syndrome (GBS) has been reported as a very rare complication of bariatric surgery and reported cases are mostly axonal types. An 18-year-old female patient applied to our outpatient clinic with complaints of pain and weakness in her arms and legs. She had bariatric surgery 3 months ago. The electrophysiological findings of the patient who had proximal muscle strength loss in the lower extremities were consistent with subacute anterior root anterior horn involvement in the L3-S1 innervated muscles. After the intravenous immunoglobulin therapy, the patient improved almost completely. We think that in neurological complications after bariatric surgery, nutritional deficiencies should be taken into consideration as well as inflammatory damage and these cases should be followed up and treated in a multidisciplinary manner in order to prevent permanent neurological damage. - Abstract
Obesity is a complex, multifactorial, and largely preventable disease. Today, all countries in the world have reached the alarm level. For this reason, many treatment modalities have been developed in the fight against obesity today. One of them is a bariatric surgery. Individuals may experience serious adjustment problems due to significant changes in eating, lifestyle and body image after bariatric surgery. Roy's Adaptation Model (RAM), widely used in nursing, focuses on determining the adaptation needs of the individuals in the four areas of integration and the changcs that occur in and around the human adaptation system. In this ease study, two patients who underwent bariatric surgery in a university hospital in 2017 were evaluated prcopcrativcly one time and postoperatively four times according to RAM. In this study, it was determined that the first case adapted according to RAM while the second case could not adapt and have physiological and psychological complications. In the study, RAM facilitated the systematic evaluation and monitoring of patients and made it possible to identify problems. [ABSTRACT FROM AUTHOR]

Objective: In this study the effects of intraabdominal high pressure application on perfusion and hemodynamics in bariatric surgery was planned to investigate. Method: Fifty-one patients which are planned for obesity surgery were included to the study. Patients were monitorized with cerebral-oximetry (NIRS) to follow cerebral oxygenation (rSO2) and bispectral index (BIS) to follow depth of anesthesia near the standard monitorization in operating room. Measurements were performed in seven different times and 'before induction' named as T1 time, and 'extubation' as T7 time. Results: Evaluation with T1, when the carbondioxide (ETCO2) and BIS values at the end of inspiration were constant, heart rate was found significantly low in T2, T4, T5, T6 when high in T7; mean arterial pressure (MAP) was found significantly low in T2, T3, T4, T5, T6; NIRS right values was found significantly low in T3, T4, T5 and NIRS left values was found significantly low in T2, T3, T4, T5, T6. In a correlation with T1 based total 64 measurement 20% decreases in NIRS values were found. Conclusion: Application of intraabdominal high pressure in bariatric surgery effects cerebral oxygenation and hemodynamics significantly. For a safe anesthetic process in patients which were applied high intraabdominal pressure it is useful to follow the cerebral oxygenation and close monitoring for hemodynamic parameters. - Abstract
Aim: Elevated red blood cell distribution width (RDW) and mean platelet volume (MPV) levels which are closely associated with chronic inflammation and platelet aggregation are suggested as independent predictors of obesity and cardiovascular diseases. However influence of significant weight loss following bariatric surgery on these parameters is unknown. Therefore we aimed to find out the effect of significant weight loss following laparoscopic sleeve gastrectomy (LSG) on RDW and MPV levels. Methods: The medical data of 98 morbid obese subjects (25 male, 73 female) who were operated between February 2015 and June 2017 according to indications of bariatric surgery in current guidelines including body mass index (BMI) >40 kg/m² or BMI= 35-40 kg/m² with additional comorbidities were recorded. The difference between baseline and values at one year was expressed as a delta (Δ). Results: The mean age of our study population was 41.89±11.99 years and mean weight loss in one year after LSG was 45.41±13.13 kg (36.5%). BMI decreased from 46.60±7.11 kg/m² to 29.58±4.63 kg/m². Compared to the baseline, significant decreases in RDW (14.61±1.69% vs. 13.71±2.10%; p<0.01) and MPV (8.63±1.45 vs. 7.92±1.24 fL, p<0.001) levels were found in the postoperative oneyear values. In addition; ΔVKI was positively correlated with ΔRDW (r=0.343, p<0.01) and ΔMPV (r=0.322, p<0.01). Conclusion: We found that morbid obese subjects have significantly decreased RDW and MPV levels which are correlated to their weight loss in one-year follow-up after LSG. If we think that several factors may affect these parameters, conduction of further prospective large-scale studies are needed. [ABSTRACT FROM AUTHOR]

Objective: In the present study, we aimed to retrospectively evaluate the preoperative characteristics, intraoperative and postoperative results of patients who underwent laparoscopic obesity surgeries. Methods: After obtaining the approval of the Ethics Committee, records of patients who underwent laparoscopic obesity surgery from January 2013 to December 2016 were reviewed. Demographic characteristics, medications used in anaesthesia and analgesia, the duration of recovery unit/hospital stay, intensive care unit/mechanical ventilation requirements and complications were recorded. Results: A total of 329 ASA II-III patients over a 3-year period were operated. Thiopental and propofol were administered at induction, sevoflurane, isoflurane and desflurane were administered for the maintenance, and vecuronium and rocuronium were administered to aid in neuromuscular blockage. The mean durations of recovery unit and hospital stays were 30.80±6.01 minutes and 4.27±1.68 days, respectively. The hypnotic agent, muscle relaxant or inhalation anaesthetics used did not have a significant effect on the duration of recovery unit and hospital stay. Mask ventilation and intubation were noted to be difficult in 5.5% and 8.5% of the cases, respectively. The presence of obstructive sleep apnoea syndrome and high body mass index and Mallampati scores significantly increased difficult mask ventilation and difficult intubation rates. Four patients were transferred to intensive care unit for close monitoring. Two patients were re-operated on, two patients had rhabdomyolysis, one patient had Wernicke's encephalopathy and two patients had peripheral neuropathy. Perioperative mortality did not occur in any patient. Conclusion: We believe that appropriate patient selection, the use of well-designed anaesthesia and surgical protocols play important roles in increasing the success rate of patient outcomes and early and late complications in laparoscopic obesity surgery. Objective: Obesity is a chronic metabolic disorder that leads to the increased risk of cardiovascular diseases. This study aims to investigate the effect of weight loss on the platelet count and volume, which is associated with cardiovascular diseases. Methods: In total, 56 obese patients were recruited for the study. The parameters were retrospectively evaluated before and after 6 months of surgery. Results: The mean weight of the patients was 126.2±23.1 kg before surgery and 91.8±20.5 kg after surgery (p<0.001). The mean platelet counts were 292.5±58.6×103/µL before surgery and 246.8±59.1×103/µL after surgery (p<0.001). The mean platelet volumes were 10.4±1.0 fL and 11.6±0.9 fL before and after surgery, respectively (p<0.001). The mean platelet counts before surgery were correlated with the mean platelet volume, mean weight, and mean body mass index (p<0.01, r=-0.39, p<0.01, r=0.35, p<0.01, r=0.41, respectively). The mean platelet counts after surgery were correlated with the mean platelet volume (p<0.001, r=-0.68). Conclusion: This study demonstrated decreased platelet counts and increased platelet volume at 6 months after surgery in obese patients. Further long-term and prospective studies are warranted to clarify these results and pathopsychological mechanisms involved. [ABSTRACT FROM AUTHOR]

Objective: Obesity is also seen to cause psychosocial problems as well as medical problems. The aim of this study is to examine the level of life satisfaction, body perception and self-esteem of morbid obese patients as a candidate for bariatric surgery and the effect of body perception and selfesteem on life satisfaction. Material and Method: In this study, 136 morbid obese patients with body mass index (BMI) of at least 40 who applied to the Endocrinology Department of Inonu University and 69 healthy individuals with BMI of less than 30 were included between Jan-Dec 2016. Those with any psychiatric disorder were excluded from the study. Coopersmith Self-Esteem Scale, Body Sense Scale, Life Satisfaction Scale were applied to the groups. Results: Statistical analyzes revealed that the life satisfaction and body image scale scores of the morbid obesity patients were lower than the control group. It was remarkable that self-esteem scale score was higher in patients than control group. In the correlation analysis, a positive correlation between life satisfaction and body perception was found. Conclusion: According to the findings of the study, morbid obesity patients had lower life satisfaction and higher dissatisfaction with body appearance than the control group but higher self esteem. In addition, as the satisfaction of the person's body image increased accordingly with the increase of life satisfaction. In some societies, the concept of weakness is appreciated, while obese individuals are exposed to exclusion. If self-esteem is thought to be influenced by interpersonal relationships, the higher self-esteem in morbid obese patients can be explained by the variation in social structure and number of patients.

Fiziksel aktivite ölçeğinden hesaplanan değerlere göre hastaların %56,8'i inaktif, %33,7'si normal ve %9,5'i aktif düzeyde fiziksel aktiviteye sahiptir. Bariatrik cerrahi obeziteden kurtarsa dahi fiziksel inaktivite devam etmektedir. Sonuç olarak çalışmamızdan aldığımız veriler doğrultusunda bariatrik cerrahi sonrası fiziksel aktivite artırılmalı, egzersiz reçeteleri oluşturulmalı ve bu konuda eğitimler verilmelidir., The purpose of this research was to examine the association between healthy eating attitudes and physical activity levels in individuals after bariatric surgery using biochemical and anthropometric data. The research included 95 12-36 months post-op patients between the ages of 18 and 65. "Healthy Eating Attitude Scale" and "Physical Activity Questionnaire for Primary Care" were applied to the subjects. According to the values calculated from the physical activity scale, 56.8% of the patients had inactive, 33.7% normal and 9.5% active physical activity. Physical inactivity continiues even though bariatric surgery saves people from obesity. When attitudes toward healthy nutrition are considered, 58.9 % report having a high level of nutrition, 28.4 % report having a high level of ideal nutrition, and 12.6 % report having a moderate level of healthy nutrition. Before and after surgery, BMI, weight, AST, ALT, triglyceride, HOMA IR, fat mass, lean body mass, muscle mass, and fat percentage values were substantially reduced (p.05), HDL and LDL cholesterol levels were substantially higher than they were before surgery (p0.05). In conclusion, data obtament from the study shows that there must be enhancement in physical activity, exercise prescriptions and trarning rovided on this issue.

Obesity is a serious health problem facing the modern world, and its incidence has increased at an alarming rate in recent years. Treatment options include lifestyle changes such as diet and exercise and pharmacological and surgical procedures. Bariatric surgery is a method applied to patients with advanced obese and comorbidities. Surgical procedures are generally classified into restrictive, malabsorptive, or a combination of restrictive and malabsorptive elements. Some complications such as micronutrient deficiencies can occur in patients after surgery. Among the reasons for the occurrence of these deficiencies are variables depending on the patient and the surgical method. While among the patient-dependent variables came to the fore noncompliance with the nutrition program created specifically for the individual and the surgical procedure and not using the recommended dietary supplements, variables related to surgery differ according to procedures that restrictive and malabsorptive. Thiamine, vitamin B12, folate, iron, vitamins D, A, and K, zinc, and copper are the most common micronutrient deficiencies after bariatric surgery. Besides, have deficiencies of these micronutrients in the preoperative period occurs more serious complications in the postoperative period. Lifelong follow-up of patients is the most important step in the management of these deficiencies as a result of bariatric surgery. In particular, patients need to adhere to the recommended dietary supplements after surgery and to comply with medical nutrition therapy at a high level. Also, because some of these nutritional supplements interfere with each other's absorption, appropriate dosage and time adjustments must be made correctly. The purpose of this review is; to describe the micronutrient deficiencies observed in the postoperative period in bariatric surgery patients and to present the current information in the literature about the reasons for the occurrence of these deficiencies and their prevention.

In this study, which was conducted to observe how the nutritional and behavioral attitudes occurring one month after the surgery affect the short-term biochemical parameters of patients undergoing bariatric surgery, the "nutrition and behavioral attitude scale in bariatric surgery patients" was applied to individuals aged 18-64 years who had undergone bariatric surgery, preoperatively and 1 month after the operation, and biochemical parameters were requested before and 1 and 6 month after the operation. The research was carried out as a prospective study on 39 individuals who applied to Samsun Anadolu Hospital General Surgery clinic between 16.02.2021-24.09.2021 for bariatric surgery. Shapiro-Wilk, KolmogorovSmirnov and Anderson-Darling tests, Paired test, Wilcoxontest, Mcnamer test, McNemar-Bowker test, Friedman (Repeated Measures ANOVA (Non-parametric)) test and Durbin-Conover test were used in statistical analysis; the significance level was taken into account as 0.05 (p-value) in the analyzes. Fasting glucose, ALT, AST, CRP, creatinine, TSH, B12, calcium and iron levels of the patients were evaluated. Although there is no significant difference in the nutritional behaviors of obese individuals, it has been determined that there are positive changes in glucose (hunger), ALT, creatinine, CRP and TSH levels due to changes such as the number of meals and water consumption caused by the surgery, but there is no significant change in AST, B12, calcium and iron levels. As a result, as advocated in the literature, it was determined that the nutritional and behavioral changes that had to be made within the first month due to surgery had positive effects on biochemical parameters. As a result, as advocated in the literature for long-term (3,6,12 months) follow-up, it was determined that the effects on biochemical parameters were positive with the changes in nutrition and behavioral attitude observed in the first 6 month

Çalışmada katılımcıların %73,9'u kadın, %52,1'i lisans ve üzeri eğitime sahip, %55,6'sı ameliyattan 3-4 yıl sonra kilo aldığı, %21,0'i kronik hastalığa sahip, %40,3'ü sağlıklı beslendiği, %68,9'u egzersiz yaptığı belirlendi. Yaş ortalaması 39,8±9,6, Beden Kitle İndeksi 30,2±5,9'dir. Yaşam kalitelerinin biyopsikososyal alt boyutu (cinsel yaşam, fiziksel işlev, psikososyal alan puan toplamı; 59,5±6,7) orta, komplikasyon alt boyutu düşük (14,1±4,7) saptandı. Sağlık çalışanlarının özellikle mahremiyete gösterilen özen ve dikkat (%95), iletişimde kullandıkları yöntem-dil (%92,4, bilgilerin açık ve anlaşılır (%84,0) olduğunu belirtmelerine karşın eğitimde kullanılan broşür (%57,1), kilo takibi (%61,3), eğitim destek ve yönlendirmeleri (%71,4) daha düşük bulundu. Biyopsikososyal alt boyutunda yaş (OR:-1,904), kronik hastalık (OR:-4,305) ve tekrar kilo alma (OR:-2,437) etkili faktörler olduğu saptandı. Komplikasyon gelişiminde etkili faktör bulunmadı. Çalışmada katılımcıların tekrar kilo almaya başlamalarına karşı yaşam kalitelerinin etkilendiği, 40 yaş ve üzeri ve kronik hastalığı olanların risk grubunda olduğu, sağlık çalışanlarının kilo kontrolüne yönelik sağlıklı yaşam davranışlarına eğitim ve takip hizmetinin geliştirilmesi sonucuna ulaşıldı. This study was carried out in a descriptive and cross-sectional type with the aim of increasing the life quality of the individuals who have undergone bariatric surgery by determining their late-term life quality and their expectations from healthcare professionals. The population of the study consisted of 648 participants, aged between 18-65 years, who underwent bariatric surgery four years ago in a private clinic. 119 participants whose sample was calculated with 80% power and 0.05 type1 error were included in the research. Data were collected by phone calls by the researcher between January and April 2021. "Socio-Demographic Data Form", "Expectations of Patients Who Have Undergone Bariatric Surgery From Health Professionals" and "Quality of Life of the Patients Who Have Had Obesity Surgery" scales were used to collect data. In the study, it was determined that 73.9% of the participants were women, 52.1% had bachelor's degree or higher education, 55.6% put on weight 3-4 years following the operation, 21.0% had chronic diseases, 40.3% of them ate healthily, and 68.9% exercised. The mean age is 39.8 ± 9.6, and the Body Mass Index is 30.2 ± 5.9. The biopsychosocial sub-dimension of their life quality (sexual life, physical function and psychosocial score were totally 59.5±6.7) was found to be moderate, and the complication sub-dimension was low (14.1±4.7). Despite the fact that the participants noted that the health workers pay particular attention to privacy (95%), the method-language they use in communication (92.4%), the information is clear and understandable (84.0%), brochures used in education (57.1%), weight tracking (61.3%), educational support and referrals (71.4%) were found to be lower. Age (OR:-1,904), chronic disease (OR:-4,305) and regaining weight (OR:-2,437) were found to be effective factors in the biopsychosocial sub-dimension. No effective factor was found in the development of complications. In the study, it was concluded that the life quality of the participants was affected by the fact that they started to gain weight again, that those aged 40 and over and those with chronic diseases were in the risk group, and that education and follow-up services upon healthy life behaviours regarding weight control were to be developed.
