Giriş: Bu çalışmada, obstrüktif uyku apne sendromu (OSAS)'nun değerlendirilmesinde kullanılan antropometrik ölçüm ve oranların ülkemize ait değerlerini belirlemek istedik. Materyal ve Metod: Çok merkezli prospektif olarak planlanan çalışmamıza 13 ilimizde yer alan, akredite olmuş 20 uyku merkezi katıldı. OSAS semptomları olan ve polisomnografik inceleme ile apne-hipopne indeksi (AHİ) ≥ 5 bulunan olgular OSAS çalışma grubu; AHİ < 5 ve STOP-Bang < 2 olan olgular ise kontrol grubu olarak alındı. Olgulara ait demografik özellikler [yaş, cinsiyet, beden kitle indeksi (BKİ)] ve antropometrik ölçümler (boyun, bel ve kalça çevresi, bel/kalça oranları) kaydedildi. Bulgular: Çalışmaya 20 merkezden yaş ortalaması 50.50 ± 0.21 yıl olan 2684 (%81.3’ü OSAS’lı) olgu alındı. Olgular, ülkemizin altı coğrafi bölgesinden (Akdeniz, Doğu Anadolu, Ege, İç Anadolu, Karadeniz ve Marmara Bölgesi) alındı. Olguların demografik özellikleri ve antropometrik ölçümleri; kontrol grubuna göre karşılaştırıldığında yaş, boyun, bel, kalça çevresi ve bel/kalça oranları ve BKİ özellikleri; bölgelere göre karşılaştırıldığında ise yaş, boyun, bel, kalça çevresi ve bel/kalça oranları istatistiksel olarak farklı bulundu (sırasıyla p< 0.001, p< 0.001, p< 0.05). Cinsiyetlere göre karşılaştırıldığında yaş, boyun ve kalça çevresi, bel/kalça oranı, boy, kilo ve BKİ özellikleri istatistiksel olarak farklı idi (sırasıyla, p< 0.001). Boyun çevresi ve bel/kalça oranı sırasıyla erkeklerde 42.58 ± 0.10 cm (cut-off 41; duyarlılık %61.2; özgüllük %68.3), 0.99 ± 0.002 (cut-off 0.97; duyarlılık %56.4; özgüllük %54.3); kadınlarda 39.24 ± 0.16 cm (cut-off 37; duyarlılık %66.5; özgüllük %63.4), 0.93 ± 0.004 (cut-off 0.89; duyarlılık %70.9; özgüllük %49.5) bulundu. OSAS fenotipleri; klasik OSAS, okült OSAS, REM bağımlı OSAS, pozisyon bağımlı OSAS, REM ve pozisyon bağımlı OSAS, overlap sendromu ve kompleks uyku apne sendromu olarak belirtildi. OSAS fenotiplerine göre karşılaştırıldığında yaş, boyun ve kalça çevresi, boy, kilo ve BKİ özellikleri istatistiksel olarak farklı iken (sırasıyla, p< 0.001) bel/kalça oranı farklı bulunmadı. Sonuç: Boyun çevresi erkeklerde standart değerin altında iken, kadınlarda üstünde bulundu. Bel/kalça oranı hem erkeklerde hem de kadınlarda ideal ölçülerin üzerinde idi. Bu bağlamda ülke değerlerinin tespiti OSAS olasılığı olan hastaların belirlenmesini ve polisomnografi için uyku merkezlerine yönlendirilmesini sağlayacaktır. Introduction: In this study, we aimed to determine the values of anthropometric measurements and rates used in the evaluation of obstructive sleep apnea syndrome (OSAS) in our country. Materials and Methods: Twenty accredited sleep centers in thirteen provinces participated in this multicenter prospective study. OSAS symptoms and polysomnographic examination and apnea-hypopnea index (AHI) ≥ 5 cases OSAS study group; patients with AHI < 5 and STOP-Bang < 2 were included as control group. Demographic characteristics (age, sex, body mass index-BMI) and anthropometric measurements (neck, waist and hip circumference, waist/hip ratio) of the subjects were recorded. Results: The study included 2684 patients (81.3% OSAS) with a mean age of 50.50 ± 0.21 years from 20 centers. The cases were taken from six geographical regions of the country (Mediterranean, Eastern Anatolia, Aegean, Central Anatolia, Black Sea and Marmara Region). Demographic characteristics and anthropometric measurements; age, neck, waist, hip circumference and waist/ hip ratios and BMI characteristics when compared with the control group; when compared according to regions, age, neck, waist, hip circumference and waist/hip ratios were found to be statistically different (p< 0.001, p< 0.001, p< 0.05, respectively). When compared by sex, age, neck and hip circumference, waist/hip ratio, height, weight and BMI characteristics were statistically different (p< 0.001, respectively). Neck circumference and waist/hip ratio were respectively 42.58 ± 0.10 cm, 0.99 ± 0.002, 39.24 ± 0.16 cm, 0.93 ± 0.004 were found in women. Conclusion: The neck circumference was lower than the standard value in men, but higher in women. The waist/hip ratio was above the ideal measurements in both men and women. In this context, the determination of the country values will allow the identification of patients with the possibility of OSAS and referral to sleep centers for polysomnography.

Amaç: Bu çalışma, akademisyenlerin beslenme alışkanlıkları ve fiziksel aktivite düzeylerinin incelenmesi amaçlanmıştır.Yöntemler: Araştırma tanımlayıcı tipte bir araştırmadır. Marmara Üniversitesi Haydarpaşa Kampüsü’ndeki Tıp Fakültesi Temel Bilimleri, Eczacılık Fakültesi, Hemşirelik Yüksekokulu ve Hukuk Fakültesinde Aralık 2013-Nisan 2014 tarihleri arasında görev yapan 225 öğretim elemanına yüz yüze anket uygulanmış ve boy ile kiloları ölçülmüş ve öğrencilerin Vücut Kitle İndeksleri-VKİ (ağırlık (kg)/ boy² (m²) hesaplanmıştır. VKİ değeri 30 üstü olanlar obez olarak değerlendirilmiştir.Bulgular:Öğretim elemanlarının %66,2’si kadın, %33,8’i erkektir. Katılımcıların, %23,6’sının Tıp Fakültesi’nde, %28,0’ının Eczacılık Fakültesi’nde, %13,7’sinin Hemşirelik Yüksekokul’ unda ve %34,7’sinin Hukuk Fakültesi’nde görev yapmakta olduğu bulunmuştur. Katılımcıların ; %4,9’u zayıf , %58,6’sı normal kilolu , %29,8’i fazla kilolu ve %6,7’si obez olarak bulunmuştur. Fazla kilolu ve obezler birlikte değerlendirildiğinde öğretim elemanlarının %41,4’ünün kilosunun normalin üzerinde olduğu görülmektedir.Obezite oranı en yüksek bölüm; Eczacılık Fakültesinde iken (%9,6), bu oran Tıp Fakültesi öğretim üyelerinde %3,7 ile daha düşüktür. Hukuk Fakültesinde hiç zayıf katılımcı yoktur. Kilosu normalin üzerinde olanların oranı %33,3 ile Eczacılık Fakültesinde en yüksek iken, Hemşirelik Yüksekokulunda bu oran %12,9 ile en düşüktür. Obezite bakımından en yüksek oran %9,6 ile Eczacılık Fakültesinde iken, en düşük oran %3,7 ile Tıp Fakültesindedir. Katılımcıların öğün atlama durumlarına göre dağılımları incelendiğinde; öğretim elemanlarının önemli bir oranının %72,0’ı sürekli öğün atladığı görülmektedir. Bazen öğün atlayanlarda dikkate alındığında bu oran % 94,7’ye çıkmakta, öğün atlamayanların oranı çok düşük kalmaktadır.Tıp Fakültesi (71,7 ±35,29) en yüksek yağ alımına sahip bölüm iken, Hukuk Fakültesi (57,8±17,72) günlük en düşük yağ alımına sahip bölümdür. Eczacılık Fakültesi’nin günlük kalori (1420,53±337,26) ve protein alımı (85,5±33,66), diğer bölümlere göre daha yüksek iken, Tıp Fakültesinin günlük karbonhidrat alımı (123,6 ±46,11) diğer bölümlere göre daha yüksektir.Sağlık alanında görev yapanlar en yüksek günlük kalori (1388,9±332,65) ve yağ alımına (68,5± 26,02) sahip grup iken, Hukuk alanında görev yapanlar en düşük günlük kalori (1282,5± 326,44) ve yağ alımına (57,8±17,85) sahiptir. Sağlık alanında görev yapanların günlük karbonhidrat (117,4± 37,82) ve protein alımları (79,7± 31,61) Hukuk alanında görev yapanlara göre daha yüksek iken bunlar arasında istatistiksel olarak anlamlı fark yoktur (p>0,05).Öğretim üyelerinin aktivite düzeyi değerlendirildiğinde % 66,7’sinin in aktif olduğu görülmüştür. Minimal düzeyde aktif olanların oranı % 30,7 iken çok aktif olanların oranı yalnızca % 2,6’dır.Sonuç: Sağlık alanında görev yapan öğretim elemanları en yüksek günlük kalori ve yağ alımına sahip iken Hukuk Fakültesinde görev yapanlar en düşük günlük kalori ve yağ alımına sahiptir. Öğretim elemanların %6,7’si obezdir ve obezite oranı en yüksek bölüm; Eczacılık Fakültesinde bulundu(%9,6). Sağlıklı ve dengeli beslenme ile ilgili eğitim verilebilir.

Objective: Prolonged intensive care unit (ICU) stay prevents the use of ICU equipment by other patients and increases hospital cost. This retrospective study aimed to investigate the risk factors for prolonged ICU stay in patients undergoing open heart surgery. Methods: The medical records of 513 patients who underwent coronary artery bypass grafting and valvular heart surgery were retrospectively evaluated. Patients were divided into two groups based on their ICU stay: groups I (, 15.03.2012, Ethics Committee Approval: Ethics committee approval was received for this study from the Clinical Research Ethical Committee of Ege University School of Medicine (Date: 06.04.2015, No: 15-3/12).

Amaç: Çalışmamızın amacı astımlı çocuklarda, düzenli egzersizin astım semptomları, astım tedavi ihtiyacı skoru, yaşam kalitesi ve solunum fonksiyonları üzerindeki etkilerini araştırmaktır.Gereç ve Yöntem: Bu çalışmaya 17 farklı okuldan 73 (32 erkek, 41 kız) astımlı öğrenci seçildi. İki yüz otuz iki öğrenciye çalışmanın başlangıcında solunum fonksiyon testi yapılıp, yaşam kalite skoru, astım tedavi ihtiyacı ve semptom skoru tespit edildi. Ortalama yaş 12±2,3 (7-16 yaş) idi. Yetmiş üç öğrencinin 27’si yüzme, 26’sı sokak basketbolu,11’i atletizm, 9’u ise badminton grubunda çalışmışlardı. Bağımsız değişkenlerin karşılaştırılmasında Student’s t–test kullanıldı.Bulgular: Sokak basketbolu ve badminton grubundaki hastaların semptom skorunda anlamlı azalma saptandı. Yüzme ve atletizm grubunda olan hastaların yaşam kalite skorlarında artış belirlendi. Yüzme, sokak basketbolu ve badminton grubundaki hastaların birinci saniye zorlu ekspirasyon volümü değerlerinde istatistiksel olarak anlamlı yükselme saptandı. Sonuç: Solunum fonksiyonları ve yaşam kalitesi üzerindeki etkileri nedeni ile düzenli egzersiz, çocukluk çağı astım tedavisinin bir parçası olmalıdır., Purpose: The aim of our study was to investigate the effect of regular exercise on asthma symptoms, score for asthma medication requirement, quality of life and pulmonary function tests in children with asthma.Materials and Methods: Seventy three children (32 males and 41 females) with asthma were participated from 17 different schools. At the beginning of the study, pulmonary function test was performed to the subjects along with the questionnaire of their life quality, current medication and symptom scores. Mean age of the participants was 12±2,3 years (7-16). Among these 73 students, 27 had practiced in the swimming group, 26 in the street basketball group, 11 in the athleticism group and 9 in the badminton group before participating in the study. Student’s t–test was used to compare the independent variables.Results: Symptom scores were significantly decreased in patients who practiced in the street basketball group and badminton group. Quality of life was significantly increased in patients who practiced in the swimming group and athleticism group. There was a significant increase in forced expiratory volume in 1 second at the end of the study when compared from baseline measurements in swimming, badminton and street basketball groups. Conclusion: We observed beneficial effects of exercise on quality of life and pulmonary function in children with asthma. Regular exercise should be encouraged as part of childhood asthma management.

Amaç: Bu çalışmada ileri yaş hastalarda çimentolu ve çimentosuz hemiartroplasti sonuçlarını ve birbirine üstünlüklerini değerlendirdik. Yöntemler: Bu çalışma femur boyun kırığı nedeni ile takip edilen ve hemiartroplasti cerrahisi uygulanan toplam 86 hastadan düzenli kontrollere gelip kayıtları tutulan 46 hastanın dahil edildiği retrospektif bir çalışmadır. Bu hastaların 28'ine çimentosuz, 18'ine çimentolu hemiartroplasti yapılmıştı. Bulgular: Çimentosuz hemiartroplasti operasyonu yapılmış hastaların ortalama yaşı 71,3 (yaş aralığı 50-94), çimentolu hemiartroplasti yapılmış grupta ise 86 (yaş aralığı 58-104)' tü. Broker'in myoziti değerlendirmek için tarif ettiği sınıflandırmaya göre çimentolu bipolar hemiartroplasti yapılan 10 (%55,5) hastada myozit görüldü. Çimentosuz bipolar hemiartroplasti yapılan 6 (%21,4) hastada myozit görüldü. Çimento uygulanan 18 hasta ile uygulanmayan 28 hasta arasında harris skoru açısıdan anlamlı fark yoktu (p=0.339). Ayrıca iki grup arasında femoral stem gevşemesi ve stem migrasyonu açısından anlamlı fark yoktu (p> 0.05). Heterotropik ossifikasyon açısından iki grup arasındaki fark ise anlamlı idi (p< 0.05). İki grup arasındaki perioperatif mortalite oranları karşılaştırıldığında sementli hemiartroplasti yapılan grupta mortalite oranları daha yüksek bulunmuştur. Sonuç: İleri yaştaki hastalarda sementsiz kalça protezinin özellikle ameliyat esnasında hastaya minimum travma yaşatması, mortaliteyi anlamlı bir şekilde düşürmesinden dolayı bu hastalarda en uygun tedavi yöntemi olduğunu düşünmekteyiz. Objective: In this study, we evaluated advantages of each and cemented and cementless hemiarthroplasty results in older patients. Method: This study is a retrospective study included 46 patients came to regular controls from a total of 86 patients who underwent hemiarthroplasty surgery and followed because of femoral neck fracture. Of these patients, 28 cementless and 18 cemented hemiarthroplasty were enrolled for the study and were then classified into two groups. Results: The average age of patients with cementless hemiarthroplasty operations was 71.3 years , while the average age of patients with cemented hemiarthroplasty was 86 years . According to the Broker classification described to evaluate myositis, in 10 (55.5%) patients who made cemented bipolar hemiarthroplasty was seen myositis. In 6 (21.4%) patients who made cementless bipolar hemiarthroplasty was 6 (21.4%) was seen myositis. Among 18 patients who applied cement with 28 patients who not applied cement was no significant difference in Harris score (p=0.339). In addition, between the two groups was no significant difference in terms of femoral stem loosening and stem migration (p< 0.05). The difference between the two groups in terms of heterotopic ossification was significant (p< 0.05). Perioperative mortality rate was significantly higher in cemented hemiarthroplasty. Conclusion: The cementless hip prosthesis in elderly patients has minimal trauma during surgery. Mortality is also low. Therefore, we believe that the cementless hip prosthesis in elderly patients is most appropriate therapy.

AMAÇ: Gebelikte meydana gelen fiziksel ve hormonal değişiklikler, anksiyete, büyüyen fetusun diafragmaya yaptığı bası sonucu artan abdominal rahatsızlıklar vb. nedenlerden dolayı uyku kalitesi etkilenmektedir. Buna bağlı olarak çalışmamız, gebe kadınlarda uyku kalitesini ölçmeyi, uyku kalitesi ve trimesterler arasındaki ilişkiyi değerlendirmeyi, gebelerin uyku kalitesini etkileyen et-kenleri saptamayı ve tanımlamayı amaçlamıştır. GEREÇ VE YÖNTEM: Bu çalışma kesitsel ve tanımlayıcı bir çalışmadır. Afyon Kocatepe Üniversitesi Ahmet Necdet Sezer Araştırma ve Uygulama Hastanesi Kadın Hastalıkları ve Doğum Polikliniğinde Aralık 2015-Şubat 2016 tarihleri arasında 152 gebe kadın üzerinde yapıldı. Veriler yüz yüze görüşme yöntemiyle toplandı. Katılımcılara çalışma ile ilgili bilgi verildi ve sözlü onamları alındı. Çalışmada veriler kişisel bilgi formu, Pittsburg Uyku Kalitesi İndeksi (PUKİ), Berlin Uyku Anketi kullanılarak toplandı. BULGULAR: Yaş ortalaması 27,59 ±5,59 olan gebe kadın-ların %70,4 ü ev hanımı idi. Çalışmamıza katılan gebelerin yarısından fazlasının eğitim düzeyi lise ve üzeredir. Beden Kitle İndeksine (BKİ) göre gebelerin %20,4 ü obezdir. PUKİ ye göre %55,92’in uyku kalitesinin kötü olduğu saptan-dı(?5). Araştırmamızda PUKİ puanları 0-20 arasında de-ğişmekte idi. PUKİ ortalaması 7,38±4,91 dir. Berlin Uyku Anketine göre %44,07 sinin uyku kalitesinin kötü olduğu saptandı (?2). SONUÇ: Gebelerin genel uyku kalitesinin trimesterlere göre değiştiği, en kötü uyku kalitesinin 3.trimesterde ol-duğu, sosyo-demografik özelliklerin uyku kalitesi üzerine etkili olduğu bulunmuştur. Uyku bozuklukları gebelikte ciddi maternal ve fetal komplikasyonlara neden olabildi-ği için erken tanı ve gerekli tedaviyle bu kötü sonuçların azaltılabileceği düşünülebilir. OBJECTIVE: Sleep quality is affected during pregnancy due to the reasons such as physical and hormonal chan-ges, anxiety, and increased abdominal discomfort due to growing fetus putting pressure on the diaphragm. In the present study, we aimed to assess the sleep quality in pregnant women, to evaluate the correlation betwe-en sleep quality and trimesters, and to identify possible factors which affect the sleep quality during pregnancy. MATERIALS AND METHODS: This is a cross-sectional and descriptive study included a total of 152 pregnant women who were admitted to Afyon Kocatepe Univer-sity, Ahmet Necdet Sezer Research and Training Hospital, Gynecology and Obstetrics outpatient clinic between December 2015 and February 2016. Data were collected in face-to-face meetings. Data were collected using Per-sonal Data Form, Pittsburgh Sleep Quality Index (PSQI), and Berlin Sleep Questionnaire (BSQ). RESULTS: The mean age of the patients was 27,59 ±5,59 years, and 70,4% of them were housewives. Educational level of the pregnant women participating in the study was secondary school or higher. According to body mass index (BMI) values, 20,4% of the pregnant women were obese. According to the PSQI scores, 55,92% had poor sleep quality (?5). The mean PSQI score was 7,38±4,91. According to the BSQ scores, 44,07% of the pregnant wo-men had poor sleep quality (?2). CONCLUSIONS: Our study results showed that overall sleep quality of the pregnant women changed based on the trimester, and the poorest quality of sleep was du-ring the third trimester. In addition, sociodemographic characteristics have an impact on the sleep quality. As sleep disturbances can lead to serious maternal and fe-tal complications during pregnancy, early diagnosis and appropriate treatment are required to reduce these poor outcomes.

WOS: 000406926000015, Asymptomatic double aortic arch associated with tetralogy of Fallot (TOF) is a very rare congenital anomaly. Echocardiography plays a decisive role in the imaging of these patients as in several congenital heart diseases. In suspected double aortic arch diagnosis as in the current case, computed tomography or cardiac catheterization is of diagnostic importance for confirming complex anomalies such as this. In our case, these imaging techniques were performed and the patient underwent a one-stage total correction with ligation of an aortic arch via median sternotomy. Following an uneventful recovery, the patient was discharged on the postoperative fifth day.

Nowadays, high system performances in the management of robust processes that require great attention were increased using the computer-aided analysis methods and the developments of the diagnosis system. Analysis of the respiratory sounds with computer-aided systems allows objective and useful assessments. In this study, a brief description of the abnormal respiratory sounds was presented. The main aims of the study are performing a systematic review about methods and the machine learning algorithms that are used to classify the abnormal respiratory sounds for diagnosis of cardio-pulmonary disorders and evaluating the development of possible methods on respiratory sounds in the future.

From February 1992 to October 1995, 504 patients aged 3 to 69 years (median, 36 years) received 663 Ultracor prostheses (103 aortic, 259 mitral, 12 tricuspid, and 130 multiple). Early mortality was 2.77% with a rate of 3.88% for aortic valve replacement, 3.47% for mitral, 0% for tricuspid, and 0.99% for double valve replacement. Follow-up was 100% (719.5 patient-years) and the overall mortality (early and late deaths) was 3.76% with a rate of 4.85% after aortic valve replacement, 3.85% after mitral valve replacement, 8.33% after tricuspid valve replacement, and 2.97% after double valve replacement. There were no deaths among 29 patients who had triple valve replacement. All patients including 33 children (3.5 to 18 years of age) received sodium warfarin. The linearized risk per patient-year for all embolic events (major and minor) was 0.69%, 1.11%, and 0.69% for aortic, mitral, and multiple valve replacement respectively. When only major events were considered, the linearized risks were 0.27%, 0.13%, and 0% per patient-year respectively. Freedom from major systemic embolism was 99% ± 0.07% after aortic valve replacement, 99% ± 0.07% after mitral valve replacement, and 99% ± 0.06% after multiple valve replacement. Five patients suffered valve thrombosis, 4 of whom definitely received inadequate anticoagulation therapy. Sixteen patients had 25 episodes of anticoagulant-related hemorrhage. No structural failure was recorded and no significant hemolysis was found in the absence of periprosthetic leak. This experience encourages us to continue using the Ultracor prosthesis.

International audience; Chronic obstructive lung disease (COPD) and diffuse parenchymal lung diseases (DPLD), including idiopathic pulmonary fibrosis (IPF) and sarcoidosis, are associated with a high incidence of pulmonary hypertension (PH), which is linked with exercise limitation and a worse prognosis. Patients with combined pulmonary fibrosis and emphysema (CPFE) are particularly prone to the development of PH. Echocardiography and right heart catheterization are the principal modalities for the diagnosis of COPD and DPLD. For discrimination between group 1 PH patients with concomitant respiratory abnormalities and group 3 PH patients (PH caused by lung disease), patients should be transferred to a center with expertise in both PH and lung diseases for comprehensive evaluation. The task force encompassing the authors of this article provided criteria for this discrimination and suggested using the following definitions for group 3 patients, as exemplified for COPD, IPF, and CPFE: COPD/IPF/CPFE without PH (mean pulmonary artery pressure [mPAP] = 25 mm Hg); PH-COPD, PH-IPF, and PH-CPFE); COPD/IPF/CPFE with severe PH (mPAP >= 35 mm Hg or mPAP >= 25 mm Hg with low cardiac index [CI
