Turkey is among the endemic countries in terms of tularemia. Tularemia, a zoonotic disease, is an important public health problem that occasionally causes epidemics and sporadic cases in our country. Transmission can often be via infected/contaminated animal contact, consumption of contaminated water, or vectors. The disease most commonly presents with lymphadenopathy in the neck and is often misdiagnosed. Since serological methods are necessary in the diagnosis of the disease, if it is not considered, it creates a problem in the diagnosis of the disease. The biggest known outbreak of the disease in our country was in Antalya. Many variables, such as seasonal changes and increased migration, have caused the epidemiology of the disease to change. In this review study, we aimed to evaluate the province of Antalya in terms of tularemia., Türkiye tularemi açısından endemik ülkeler arasında bulunmaktadır. Zoonotik bir hastalık olan tularemi, ülkemizde zaman zaman salgınlara ve sporadik vakalara neden olan önemli bir halk sağlığı sorunudur. Sıklıkla bulaş yolu, enfekte/kontamine hayvan teması, kontamine suların tüketilmesi veya vektörler aracılıklı olabilir. Hastalık en sık boyunda lenfadenopati ile kendini göstermekte ve çoğu zaman yanlış tanı almaktadır. Hastalığın tanısında serolojik yöntemlerin gerekli olması nedeniyle akla gelmemesi durumunda hastalık tanısının konulmasında sorun oluşturmaktadır. Hastalığın ülkemizdeki bilinen en büyük salgını Antalya ilinde olmuştur. Mevsimsel değişiklikler ve artan göçler gibi birçok değişken hastalık epidemiyolojisinin değişmesine neden olmuştur. Biz de bu derleme çalışmasında, Antalya ilinin tulareminin açısından değerlendirmesini yapmayı amaçladık.

Amaç: İnfantil kolik, aile ve hekim için oldukça kaygı verici 4 aylıktan küçük bebeklerde sık olarak acile başvuru nedeni olan ve etiyolojisi net bilinmeyen ağlama krizleridir. Yoğun ağlama krizleri sırasında çaresiz kalan aileler, çevredekilerin de etkisi ile farklı tedavi yöntemleri arama eğilimindedirler. 28 yaşındaki annenin 1. gebeliğinden normal spontan vaginal yol ile 39 gebelik haftasında doğan iki aylık erkek bebeğin, son 24 saattir uyandırılamaması şikayeti, acil servise başvuru nedenidir. Acil servise başvuru öyküsünde ailenin, bebeğin yoğun ağlama nöbetleri nedeniyle eczaneden alınan farklı farmakolojik ürünleri ardı ardına denediği, ancak bebeğin şikayetlerinin devam etmesi nedeniyle, çevredekilerinin de önerisi ile “elma yağı” adı ile satın aldıkları ve içeriğini bilmedikleri bir ürünü bebeğin göbek bölgesine masaj yoluyla uygulandığı ve sonrasında bebeğin ağlamalarının azaldığı ve uykuya meyilli olduğu bilgisi yer almıştır. Hastanedeki tedavi sırasında bebeğin sadece uyarılınca uyandığının belirlenmesi sonucu aileden kullandıkları ürün istenmiştir.Gereç ve Yöntem: Piyasadan satın alınan ve bebeğe masaj yoluyla uygulanan “elma yağı” adı ile satışa sunulmuş ürünün, GC-FID ve GC/MS yöntemleri ile içerik analizi yapılmıştır. Uçucu bileşiklerin teşhisi için çeşitli bilimsel veri kütüphaneleri kullanılmıştır.Sonuç ve Tartışma: Bu çalışma, son yıllarda doğala yönelme kavramının bilinçsiz ellerde geri dönüşümü olmayacak kadar olumsuz sonuçlara neden olabileceğini göstermekte ve aynı zamanda ilaçlar ve tıbbi doğal ürünler alanında yetkin olan eczacılık mesleğinin önemini göz önüne sermektedir., Objective: Infantile colic is crying crises that a cause of frequently emergency application in infants younger than 4 months and whose aetiology is unknown. Families who are desperate during intense crying crises tend to seek different treatment methods with the influence of those around. The complaint of not waking up for the last 24 hours of a two-month-old male baby born at 39 weeks of gestation with normal spontaneous vaginal delivery from the first pregnancy of a 28-year-old mother is the reason for admitting to the emergency department. In the history of applying to the emergency department, the family tried different pharmacological products purchased from the pharmacy one after another due to the intense crying attacks of the baby, but the complaints of the baby continued, the family bought a product called "apple oil" and applied the product that they did not know the content to the baby by massaging the oil to the umblicus area with the suggestion of the people around. It was reported that the baby's cries were reduced and he was susceptible to sleep. As a result of the detection that the baby woke only when stimulated during the hospital treatment, the product they used was requested from the family.Material and Method: The content analysis of the product, which was bought from the market and put up for sale under the name of "apple oil" applied to the baby by massaging, was performed using GC-FID and GC/MS methods. Various scientific data libraries have been used to identify volatile compounds.Result and Discussion: This study shows that the concept of turning to nature in recent years can cause irreversible negative consequences in unconscious hands.and also highlights the importance of the pharmacy profession, which is highly competent in the field of medicines and medicinal natural products.

OBJECTIVE Klebsiella pneumoniae, a Gram-negative pathogen, especially which produces carbapenemase, is seen as a major threat to public health due to rapid plasmid-mediated spread of resistance and limited therapeutic options available for treatment. Although colistin has been recognized as a "last resort" antimicrobial for multidrug-resistant K. pneumoniae infections, these isolates have developed resistance to colistin as a result of its intensive use. The aim of this study was to evaluate the efficacy of double-carbapenem treatment of colistin-resistant K. pneumoniae experimental sepsis in mice. METHODS In the study, 8-10-week-old Balb-c mice were divided as control groups (positive and negative) and treatment groups (colistin, ertapenem+meropenem, and ertapenem+meropenem+colistin). Sepsis was developed in mice by an intraperitoneal injection of colistin resistant K. pneumoniae. Antibiotics were given intraperitoneally 3 h after bacterial inoculation. Mice in each subgroup were sacrificed with overdose anesthetic at the end of 24-48 h and cultures were made from the heart, lung, liver, and spleen. Furthermore, homogenates of lung and liver were used to detect the number of colony-forming units per gram. Bacterial clearance was evaluated in lung and liver at different time points. RESULTS When the quantitative bacterial loads in the lung and liver tissues are evaluated, no statistically significant difference was observed between different antibiotic treatments (p>0.05). All three treatment options were not effective, especially in 24 h. Duran, Ali (Balikesir Author), Autoantibodies targeting nuclear and cytoplasmic autoantigens are used as markers in the diagnosis and classification of systemic autoimmune rheumatic diseases (SARD). The dense fine speckled (DFS) pattern is characterized by the fine-granular fluorescence of the nuclei in the interphase and the metaphase chromatin. DFS70 antibodies have been reported in healthy individuals, various autoimmune disorders, infection, cancer and inflammatory conditions. But there is still lack of information about its clinical significance. This study aimed to investigate the clinical significance of anti-DFS70 autoantibodies and the determination of accompanying pathologies. A total of 5710 serum samples routinely requested for ANA screening were tested between 2017 and 2019. Antinuclear antibody (ANA) and dsDNA were performed by indirect immunofluorescence method (IIF) (Euroimmun, Germany). Immunoblot (IB) method was used for the extractable nuclear antigen profile (ENA) (Euroimmun, Germany). Demographic and clinical data, were investigated from the medical records. Among 5710 samples tested for ANA, 23.7% were ANA positive by IIF. Mean age of the patients were 47.9 and 79.5% were female. Only 8.1% of the study group had SARD. The frequency of DFS pattern by ANA-IIF was 6.0% (342/5710), (mean age ± SD= 44.4 ± 16.7, 88% female). DFS70 pattern-positive patients were sub-grouped according to their diagnosis. SARD were detected 10.8% (mean age ± SD= 55.12 ± 14.10) in DFS70 pattern positive patient group (RA 6.1%, SS 2.6%, SLE 0.9%, SSc 0.6%, UCTD 0.6%). Autoantibodies accompanying anti-DFS70 antibody were determined as Ro-52, SS-A, nucleosome, histone, AMA-M2, dsDNA, respectively. Non-SARD diseases were determined in 89.2% of the patients with positive DFS70 pattern. Non-SARD diseases were detected as musculoskeletal complaints (47.4%), other rheumatic diseases like fibromyalgia (14.3%), dermatological diseases (9.4%), gastrointestinal system diseases (5.6%), hematological disorders (3.8%), thyroid /parathyroid diseases (3.5%), allergic diseases (2.3%), neurological diseases (2.3%) and neoplasia (breast cancer) (0.6%). The anti-DFS70 autoantibody is widely used to exclude the diagnosis of SARD in the absence of concomitant SARD-related autoantibodies. It has been observed that anti-DFS70 autoantibody may be associated with non-SARD rheumatic diseases and in many diseases (dermatological, gastrointestinal system, hematological, thyroid diseases) related to other systems. Therefore it is essential to evaluate these pathologies in patients positive for anti-DFS70 antibodies., Nükleer ve sitoplazmik otoantijenleri hedefleyen otoantikorlar, sistemik otoimmün romatizmal hastalıkların (SORH) tanı ve sınıflandırılmasında belirteç olarak kullanılmaktadır. Dense fine speckled (DFS) paterni interfaz evresinde nükleusda ve metafaz kromatininde yoğun ince benekli tarzda boyanma gösterir. Anti-DFS70 antikorları sağlıklı kişilerde, çeşitli otoimmün bozukluklarda, enfeksiyonda, kanserde ve enflamatuvar durumlarda bildirilmiş olmakla birlikte klinik önemi hakkında bilgiler hala yetersizdir. Bu çalışmada, anti-DFS70 otoantikorlarının klinik öneminin araştırılması ve eşlik eden patolojilerin belirlenmesi amaçlanmıştır. 2017-2019 yılları arasında rutin ANA taraması için gönderilen toplam 5710 serum örneği test edilmiştir. IIF-ANA testine göre DFS70 paterni sıklığı %6.0 (342/5710) olarak belirlenmiştir (Ortalama yaş ± Ss= 44.4 ± 16.7,%88 kadın). DFS70 paterni saptanan hastalar tanılarına göre alt gruplara ayrılmıştır. DFS70 paterni pozitif hasta grubunda %10.8 oranında (ortalama yaş ± Ss= 55.12 ± 14.10) SORH saptanmıştır (RA %6.1, SS %2.6, SLE %0.9, SSc %0.6, UCTD %0.6). Anti-DFS70 antikoruna eşlik eden otoantikorlar sırasıyla Ro-52, SS-A, nükleozom, histon, AMA-M2, dsDNA olarak belirlenmiştir. Anti-DFS70 paterni pozitif hastaların %89.2’sinde SORH dışı hastalıklar saptanmıştır. Kas iskelet sistemi şikayetleri (%47.4), fibromiyalji gibi diğer romatizmal hastalıklar (%14.3), dermatolojik hastalıklar (%9.4), gastrointestinal sistem hastalıkları (%5.6), hematolojik bozukluklar (%3.8), tiroid/paratiroid hastalıkları (%3.5), alerjik hastalıklar (%2.3), nörolojik hastalıklar (%2.3) ve neoplazi (meme kanseri) (%0.6) SORH dışı hastalıklar olarak belirlenmiştir. Anti-DFS70 otoantikoru, eşlik eden SORH ile ilişkili otoantikorların yokluğunda SORH teşhisini dışlamak için yaygın olarak kullanılmaktadır. Anti-DFS70 otoantikorunun SORH dışı romatizmal hastalıklarla ve diğer sistemlerle ilgili birçok hastalıkla (dermatolojik, gastrointestinal sistem, hematolojik, tiroid hastalıkları) ilişkili olabileceği gözlenmiştir. Bu nedenle, anti-DFS70 antikorları pozitif hastalarda bu patolojilerin değerlendirilmesi önemlidir.

Leyşmanyazis, hücre içi bir parazit olan Leishmania türlerinin etken olduğu bir hastalıktır. Parazitintürlerine ve konakçı immün tepkisine bağlı olarak, hastalıkların üç temel klinik formu; kutanöz, mukokutanözve viseral leyşmanyazis olarak adlandırılmaktadır. Kutanöz leyşmanyazis kronik bir hastalık olup ülserleşmişlezyonların varlığı ile karakterizedir. Hastalık sırasında oluşan cilt patolojisi, kısmen enfekte edenLeishmania türleriyle, aynı zamanda çeşitli klinik sonuçlara neden olan enflamatuvar ve anti-enflamatuvarkonak immün yanıt faktörlerinin kombinasyonu ile belirlenmektedir. Bu çalışmada, Leishmania major veLeishmania braziliensis’e karşı doğal dirençli ve duyarlı olan farelerde hastalığın patogenezi ile bağışıklıkyanıtında rolü olan ve konak-parazit etkileşimlerini belirleyen genleri, moleküler sinyal mekanizmalarınıve moleküllerin biyolojik fonksiyonlarını belirlemek amaçlanmıştır. Bunun için, L.major/L.braziliensis ileenfekte ve sağlıklı farelerden elde edilmiş yirmi dört doku örneğinin ekspresyon profilini içeren GSE56029kodlu transkriptomik veri seti “Gene Expression Omnibus” (GEO) veri tabanından elde edilmiştir. Dahasonra, R betiğinde limma paketi ile diferansiyel olarak eksprese edilmiş genler tanımlanmıştır. AnalizlerdeFDR q< 0.05 ve mutlak log2FC> 2 eşik değer olarak kabul edilmiştir. Ardından diferansiyel olarak ekspreseedilmiş genler için fonksiyonel ve yolak zenginleştirme analizleri “Ingenuity Pathway Analysis” (IPA) ilegerçekleştirilmiştir. Anlamlı derecede farklı seviyede eksprese edilen genlerin her biri için p< 0.01, FDR qdeğeri< 0.01 ve mutlak log2FC> 1 kullanılarak IPA 8.0 yazılım programı ile analiz edilmiştir. Bu analizlerile en yüksek oranda zenginleştirilmiş yolakların enflamasyon, dendritik hücre olgunlaşması ve TREM-1“Triggering Receptor Expressed on Myeloid Cells 1” (TREM-1) sinyal mekanizmaları olduğu ve bağışıklık sisteminin düzenlenmesiyle ilgili diferansiyel olarak eksprese edilmiş genlerin kutanöz leyşmanyazis seyriile yakından ilişkili olduğu belirlenmiştir. Akım-yukarı regülatör analizi ile dokularda biyolojik aktivitelereneden olan gen ekspresyon değişikliklerini açıklayan regülatörün TNF-?, IFNy, IL-1 ?, IL-10RA ve “SignalTransducer and Activator of Transcription-1” (STAT-1) olduğu öngörülmüştür. Çalışmada ayrıca, antileishmanialetkiye sahip olabilecek kimyasal bileşikler de tanımlanmıştır. Bu çalışma ile direnç/duyarlılıkfenotipini belirleyen parazit türlerine ve konağa ait mekanizmalar aydınlatılmaya çalışılmıştır. L.major veL.braziliensis ile enfekte olan BALB/c ve C57BL/6 farelerinde farklılık gösteren gen ekspresyon paternleri,sitokin/kemokinler ve sinyal yolaklarının değerlendirilmesi, enfeksiyonun altında yatan potansiyel mekanizmalarıgenetik açıdan daha iyi anlamamızı sağlayacaktır. Bu sonuçların kutanöz leyşmanyazisin tanıve prognoz tahmini için potansiyel biyobelirteçlerin geliştirilmesi ve yeni tedavi hedefleri hakkında bilgisağlaması açısından gelecek çalışmalar için yol gösterici nitelikte olabileceği düşünülmektedir.

Trichophyton verrucosum genç sığırlarda gelişme geriliği, süt sığırlarında verim kaybı ve derinin ekonomik değerinin azalması gibi önemli sorunlara neden olmaktadır. Etken teşhisinde konvansiyonel yöntemlerin zaman alıcı olması ve kesin sonuçlar vermemesinden dolayı duyarlılığı ve özgüllüğü yüksek, hızlı yeni tanı yöntemlerine ihtiyaç duyulmaktadır. Bu çalışmada sığırlardan T. verrucosum’un kültür ve MALDI-TOF MS yöntemleri ile identifikasyonu amaçlanmıştır. Bu amaca yönelik olarak Şanlıurfa ilinde dermatofitoz şüpheli 100 adet sığırdan deri kazıntısı ve kıl örnekleri alındı. Alınan örnekler direkt mikroskopi ile incelendi ve 52 (%52)’si pozitif bulundu. Örneklerin yapılan kültürü sonucu 63 (%63)’ünde dermatofit izole edildi ve bunların 52 (%52)’si T. verrucosum olarak identifiye edildi. Örnek alınan hayvancılık işletmelerinin %100 nün T. verrucosum ile infekte olduğu saptandı. İdentifikasyonu yapılan T. verrucosum izolatları 7950-7954 arasında MALDI-TOF MS spektrum pikleri saptanarak doğrulandı. T. verrucosum’un doğrulanması amacı ile MALDI TOF tekniğinin geleneksel yöntemlere kıyasla daha başarılı bir şekilde kullanılabileceği, etkenin zoonotik önemi ve ülke hayvancılığına verdiği ekonomik zarardan dolayı hastalığa karşı gerekli koruma kontrol önlemlerinin alınması gerektiği sonucuna varıldı.

Mirror neurons were discovered in macaque monkeys in the early 1990s by Rizzolatti and his colleagues at the University of Parma. In studies investigating mirror neurons in humans, noninvasive methods of neuroimaging and electrophysiology have been used because the invasive methods used on macaque monkeys are not available. Several complex functions, including identification, evaluation, and imitation of action, empathy, learning, and memory, have been attributed to this neuron group since its discovery. Thus, the mirror neuron system is thought to be the bridge between cognition and action. This possible role in cognition shows the contribution of this system in some neurological and psychiatric diseases. With this perspective, studies on the pathology of the mirror neuron system in Parkinson’s and Alzheimer’s diseases, amyotrophic lateral sclerosis, and autism-spectrum and psychotic disorders are mentioned in this review, together with research on the possible involvement of the mirror neuron system in congenital mirror movement disorder. A re-evaluation of neurological and psychiatric diseases in light of new opinions on the organization of the brain following the discovery of mirror neurons may be helpful in understanding clinical features and establishing new methods of treatment and rehabilitation., Mirror neurons were discovered in macaque monkeys in the early 1990s by Rizzolatti and his team at the University of Parma. Amaç: Bruselloz dünyada en sık görülen bakteriyel zoonotik enfeksiyon hastalığıdır. Çalışmamızda hastalığın endemik olarak görüldüğü Güneydoğu Anadolu bölgesinde ikinci basamak bir hastaneye başvuran ve bruselloz tanısı ile takip edilen 111 bruselloz olgusunun değerlendirilmesi amaçlandı. Yöntem: Çalışmada ikinci basamak bir hastanenin Enfeksiyon Hastalıkları ve Klinik Mikrobiyoloji polikliniğine Aralık 2018 – Aralık 2019 tarihleri arasında başvuran bruselloz tanısı ile ayaktan veya yatarak takip edilen 111 olgunun demografik özellikleri, klinik bulguları, olası bulaş yolları, tedavi rejimleri, komplikasyon verileri ve relaps oranları retrospektif olarak incelendi. Bulgular: Hastaların 77’si (%69.3) kadın ve olguların yaş ortalaması 36.4 (yaş aralığı 14-94) idi. Meslek dağılımı açısından, hastaların 69’u (%62.1) ev hanımıydı, 17’si (%15.3) hayvancılıkla uğraşmaktaydı. Olguların 88’inde (%79.2) taze peynir yeme öyküsü vardı. Hastalarda en sık görülen semptomlar, halsizlik (%92.7) ve artralji (%87.3) olarak saptandı. Çalışmaya alınan hastalarda CRP yüksekliği %39.6, anemi %37.8 oranı ile en sık görülen laboratuvar bulguları olarak tespit edildi. Etkilenen organ ve sistemler incelendiğinde olguların 49’unda (%44) hematolojik tutulum, 23’ünde (%20.7) karaciğer tutulumu, 11’inde (%10) osteoartiküler tutulum görüldü. Bir (%0.9) hastada brusellozun nadir bir komplikasyonu olarak izole perikardit, bir (%0.9) hastada da brusella epididimoorşiti saptandı. Çalışmaya dahil edilen hastaların 12’sinde (%10.3) tedavileri bittikten sonraki bir yıllık izlemde relaps saptandı. Sonuç: Bruselloz birçok organ ve dokuyu etkileyebilmektedir. Bu nedenle çok değişik klinik belirti ve bulgularla ortaya çıkabilmekte ve birçok hastalıkla karışabilmektedir. Endemik bölgelerde yaşayan nonspesifik semptom ve bulgular ile başvuran hastalar mutlaka bruselloz açısından değerlendirilmelidir., Aim: Brucellosis is the most common bacterial zoonotic infection disease in the world. In our study, we aimed to evaluate 111 brucellosis cases who were admitted to a secondary-line hospital by followed up with the diagnosis of brucellosis in the Southeastern Anatolia region where the disease was seen as endemic. Method: In this study, demographic characteristics, clinical findings, possible transmission routes, treatment regimens, complication data and relapse rates of 111 patients who were admitted to an infectious diseases and clinical microbiology clinic of a second-line hospital between December 2018 and December 2019 were examined retrospectively. Results: Seventy-seven of the patients (69.3%) were female and the mean age of the patients was 36.4 (age range 14-94). In terms of profession distribution, 69 (62.1%) of the patients were housewives, 17 (15.3%) of the patients were engaged in animal husbandry. There was a history of eating fresh cheese in 88 (79.2%) patients. The most common symptoms of the patients were malaise (92.7%) and arthralgia (87.3%). The most common laboratory findings in patients included in the study were 39.6% CRP elevation and 37.8% anemia. When the affected organs and systems are examined, hematological involvement was observed in 49 (44%) patients, liver involvement in 23 (20.7%) patients, osteoarticular involvement in 11 (10%) patients. There was found to one patient (0.9%) had isolated pericarditis that was a rare complication of brucellosis, and brucella epididymoorchitis in one (0.9%) patient. In the one-year follow-up after treatment, relapses were detected in 12 patients (10.3%). Conclusion: Brucellosis can affect many organs and tissues. For this reason, it can appear with many different clinical signs and symptoms and can be confused with many diseases. In patients presenting with nonspecific symptoms and signs, who were living in endemic regions, brucellosis infection should definitely be considered, and patients should be evaluated for brucellosis.

Mastitis is a great cost for the dairy industry in terms of production loss, breast damage and associated treatment costs. Somatic cell count (SCC) is widely used as an indicator of mastitis. In this study, it was aimed to compare biochemical indicators such as milk amyloid A (MAA), electrical conductivity (EC) and pH with SCC. A total of 50 cow milk was collected from cow milk producers in Afyon province. The collected milk is Group I (GI) below 200 thousand cells/ml according to SCC and Group II (GII) between 200-400 thousand cells mL-1 and those above 400 thousand cells mL-1 are Group III (GIII). is divided into groups. Kist hidatik hastalığı, Echinococcus granulosus etkenine bağlı paraziter bir enfeksiyondur. En sık karaciğer ve akciğerde görülür. Kistlerin çoğu asemptomatiktir ve spontan olarak gerileyebilir. Araştırma Mart 2016-Şubat 2017 ayları arasında mevsimsel olarak Antalya İli Döşemealtı İlçesi sınırlarında doğan Kırkgöz Kaynakları’nda yaşayan ve endemik bir tür olan Pseudophoxinus antalyae Bogutskaya, 1992 (Çiçek Balığı)’deki Helmint parazitlerinin teşhis edilmesi amacı ile yapılmıştır. P. antalyae Bogutskaya, 1992, anılan su kaynağının akış güzergâhı üzerinde bulunan üç istasyondan sepet ağlar kullanılarak yakalanmıştır. Örneklenen balıkların önce boy, ağırlık, yaş ve cinsiyetleri ölçülmüş ve tespit edilmiştir. Kırkgöz kaynağında 120, Kepez I HES Yükleme Göleti ve kanallarında 136 ve Çırnık Köprüsü’nden ise 95 adet olmak üzere toplam 351 adet birey incelenmiştir. İncelenen balık örneklerinden 95 adet konağın çeşitli parazitlerle enfekte olduğu saptanmıştır. Çalışma sonucunda Paradiplozoon homoion, Dactylogyrus sp., Rhabdochona denudata, Contracaecum sp. ve Ligula sp. parazitleri tespit edilmiştir. Araştırmada P. antalyae Bogutskaya, 1992’deki parazitlerin mevsimsel değişimi, cinsiyet, boy ve yaş gruplarına göre; yaygınlık (%), ortalama yoğunluk ve bollukları saptanmıştır., This study was carried out between March 2016 and February 2017 to determine the helminth parasites on Pseudophoxinus antalyae Bogutskaya, 1992 (Flower Fish), an endemic fish born in the border of Doşemealtı Province (Antalya), living in Kırkgöz Resources. P. antalyae, were caught using pots from three stations located on the flow route of the mentioned water source. Firstly, the length, weight, age, and sex of the sampled fish were measured and determined. Then, external, and internal examination was performed. A total of 351 individuals which including 120 in Kırkgöz Spring, 136 in Kepez I HES’s forebay dam and canals and 95 from Çırnık Bridge, were examined. Paradiplozoon homoion, Dactylogyrus sp., Rhabdochona denudata, Contracaecum sp. and Ligula sp. parasites were identified in the result of this study. In the research, prevalence (%), mean density and abundance parameters were evaluated according to seasonal variation, sex, length, and age groups of parasites of P. antalyae Bogutskaya, 1992.

Whooping cough (pertussis) is a contagious upper respiratory tract infection primary causative agent of which is gram-negative coccobacillus Bordetella pertussis. This disease, which may be fatal especially in infants, is characterized by repeated, violent coughing spasms caused by infection and may results in death. In this study, selected immunogenic proteins, namely Leucin/Isoleucin/Valin binding protein (LivJ), Fimbria type 2 (Fim2) and members of Type 3 Secretion System (TTSS) Putative Outer Protein D (BopD) and Putative Outer Protein N (BopN) expressed recombinantly. Genes of selected proteins were first cloned into pGEM®-T Easy vector and transformed to into E. coli DH5α cells, then cloned into the expression vector pET-28a(+) and transformed into E. coli BL21(DE3) cells to express the proteins. Expressed proteins were also purified by using His-tag columns. The molecular weights of the expressed BopD and Fim2 proteins were higher than expected on SDS-PAGE. Leakage was observed during BopD and BopN protein expression. Recombinant expression of these proteins, which take place in pathogenicity, is really important because of their potential as a vaccine candidate. Aim: Oxidative stress caused by untargeted anticancer drugs in tissues causes serious side effects such as cardiotoxicity, neurotoxicity, and hepatotoxicity. Paclitaxel is one of the taxane-derived chemotherapeutic drugs used in the treatment of solid tumors. Such chemotherapeutics are known to cause nerve damage. Epigallocatechin gallate (EGCG), which is the most active component of catechins in tea, is an important antioxidant and has an important role in ensuring DNA stability and healthy life. In this study, possible protective effects of epigallocatechin gallate against paclitaxel-induced oxidative stress were investigated. Method: 32 rats forming our study groups; divided into 4 groups with 8 rats in each group. As a result of the experimental protocol, the animals were sacrificed and sciatic nerve tissue was isolated. After nervous tissue homogenization, superoxide dismutase (SOD), catalase (KAT) enzyme activities and glutathione (GSH), malondialdehyde (MDA) and nitric oxide (NO) levels were examined. Results: According to the data obtained, paclitaxel administration decreased SOD, CAT activity and GSH levels and increased MDA levels, however, EGCG treatment increased SOD, CAT activity and GSH levels and decreased MDA levels. Conclusion: As a result of our study, it was shown that EGCG can prevent the oxidative stress caused by paclitaxel in sciatic nerve tissue., Amaç: Hedeflenmemiş antikanser ilaçlardan kaynaklı oksidatif stres çeşitli dokularda kardiyotoksisite, nörotoksisite ve hepatotoksisite gibi önemli yan etkilere neden olmaktadır. Paklitaksel, solid tümörlerin tedavisinde kullanılan taksan türevi kemoterapötik ilaçlardan biridir. Bu tür kemoterapötiklerin sinir hasarı oluşturduğu bilinmektedir. Çaydabulunan kateşinlerin en aktif bileşeni olan epigallokateşin gallat (EGCG) önemli bir antioksidan olup, DNA stabilitesinin sağlanılması ve sağlıklı yaşamda önemli bir role sahiptir. Çalışmamızda, paklitaksel ile oluşturulmuş oksidatif strese karşı epigallokateşin gallatın olası koruyucu etkileri incelenmiştir. Yöntem: Çalışma gruplarımızı oluşturan 32 sıçan; her grupta 8 sıçan olacak şekilde 4 gruba ayrılmıştır. Deney protokolünün sonunda sıçanlar sakrifiye edilerek siyatik sinir dokusu izole edilmiştir. Sinir dokusu homojenize edildikten sonra süperoksitdismutaz (SOD), katalaz (KAT) enzim aktiviteleri ileglutatyon (GSH), malondialdehid (MDA) ve nitrik oksit (NO) seviyeleri incelenmiştir. Bulgular: Elde edilen verilere göre paklitaksel uygulamasının SOD, KAT aktivitesi ve GSH düzeylerini azalttığı MDA seviyesini artırdığı, bununla birlikte EGCG tedavisinin SOD, KAT aktivitesi ve GSH düzeylerini arttırdığı ve MDA seviyesini azalttığı bulunmuştur. Sonuç: Çalışmamızın sonucunda EGCG’in paklitakselin siyatik sinir dokusunda neden olduğu oksidatif stresi engelleyebileceği gösterilmiştir.

The allergens of food-origin mainly cause some symptoms such as dermatitis and gastrointestinal disorders, even leading to potentially life-threatening reaction called anaphylaxis. Fish and fish products are not only a valuable source of nutrients, but also a potential source of ß-parvalbumins responsible for allergic reactions, leading to severe life-threatening health risks. Fish and fish products are among the first eight foods responsible for 95% of the foodborne allergic cases all over the world. All the food codexes has listed the fish and fish products as the common causes of allergic reactions, and included on the list of foods and ingredients that must be declared on food labels. The European Academy of Allergy and Clinical Immunology has determined the upper level of ß-parvalbumine in fish and fish products as 0.1 mg/100 g fish meat. The objective of this study was to inform the reader about fish and fish products allergens that are considered as a food safety and health issue.

İnsülin direnci, glikozun kas ve diğer dokulara taşınmasında hücrelerin insülininin etkisine yanıt verme yeteneğinin azalması olarak tanımlanır. Obezitenin gelişmesiyle birlikte yağ dokusundan salınan esterlenmemiş yağ asitleri, gliserol ve proinflamatuar sitokinler insülin direncine neden olur. Özellikle MCP-1 ve TNF-α gibi sitokin ve kemokinler adipositlerde trigliseridlerin hidrolizine neden olur. Insulin resistance is defined as a decrease in the ability of cells to respond to the effect of insulin in the transport of glucose to muscle and other tissues. With the development of obesity, nonesterified fatty acids, glycerol, hormones, pro-inflammatory cytokines released from adipose tissue are known to play a role in the development of insulin resistance. Especially cytokines and chemokines such as MCP-1 and TNFα cause hydrolysis of triglycerides in adipocytes, causing high levels of free fatty acids in circulation. These free fatty acids are then stored by the muscle and liver and beta cells again as DAG, TAG, and ceramide, blocking the IRS receptor causing its resistance. In addition, as a result of excessive fat nutrition, the accumulation of toxic lipid derivatives causes the inactivation of the intracellular energy sensor AMPK. Depending on the inactivation of AMPK, the glucose carrier protein (GLUT4) translocation decreases, and insulin resistance improves. Also, reduced activation of AMPK causes lipid accumulation in tissues, cellular dysfunctions, and, consequently, many chronic diseases. Research on the role of inflammation and initiating pathogens in psychiatric diseases has been increasing in recent years. Borrelia burgdorferi infection is transmitted to animals and humans with Ixodes group hard ticks. Borrelia burgdorferi bacteria cause Lyme Disease (LD). There are over 250 peer-reviewed scientific articles addressing the causal relationship between tick-borne diseases and mental disorders. Infection with LD has been associated with various psychiatric conditions. Studies show that many psychiatric disorders are seen in LD, including schizophrenia, bipolar disorder, panic attack, major depression, anorexia nervosa and obsessive compulsive disorder. It is important to identify infectious agents and associated inflammatory and immunological factors in treatment-resistant mental disorders. More research is needed, especially in the child and adolescent population. In these case report the relationship between borrelia burgdorferi infection and psychotic symptoms in a child who presented to the emergency department with restlessness, agitation and psychotic symptoms is discussed. Hastalığın viremi döneminde genellikle baş, boyun, genitalbölge, perineum, scrotum, meme ve bacaklarda deri nodülleri görülür. This review gives information about situation of the LSD in the world and in Turkey, etiology, epidemiology, pathogenesis and pathology, clinical presentation, diagnosis, treatment, the protection and control.

Staphylococci are pathogenic microorganisms that have various virulence factors. One of the most important of these virulence factors is biofilm production. Some staphylococci become resistant to antibiotics by producing biofilms and treatment of infections becomes more difficult. The aim of this study was to determine the effect of vancomycin, erythromycin, oxacillin antibiotics on planktonic and biofilm forming forms of staphylococci isolates which were isolated from mastitis cow's milk in a previous study. Minimum inhibitory concentration (MIC), minimum biofilm inhibitory concentration (MBIC), minimum biofilm eradication concentration (MBEC) values of antibiotics were determined. When the MIC, MBIC and MBEC values of antibiotics were compared; the highest range of MIC values was determined for erythromycin (32-64 μg ml-1). MBIC and MBEC values were similar for all test antibiotics. As a result of this study, staphylococci isolates were found to be more resistant to antibiotics by producing biofilm and were affected by higher doses of antibiotics than their free-living forms., Stafilokoklar çeşitli virülans faktörlere sahip olan patojen mikroorganizmalardır. Bu virülans faktörlerden birisi biyofilm üretimidir. Bazı stafilokoklar, biyofilm üreterek antibiyotiklere karşı dirençli hale gelmektedir ve enfeksiyonların tedavisi zorlaşmaktadır. Bu çalışmanın amacı, daha önce bir çalışmada mastitisli inek sütlerinden izole edilmiş olan stafilokok izolatlarının planktonik ve biyofilm oluşturan formlarının üzerine vankomisin, eritromisin, oksasilin antibiyotiklerinin etkisini belirlemektir. Antibiyotiklerin minimum inhibitör konsantrasyonu (MİK), biyofilm minimum inhibitör konsantrasyonu (MBİK) ve minimum biyofilm eradikasyon konsantrasyonu (MBEK) değerleri belirlenmiştir. Antibiyotiklerin MİK, MBİK ve MBEK değerleri karşılaştırıldığında; en yüksek MİK değeri (32-64 μg ml-1) eritromisin için olduğu belirlenmiştir. MBİK ve MBEK değerleri ise tüm antibiyotikler için benzerlik göstermiştir. Bu çalışmanın sonucunda, stafilokok izolatlarının biyofilm oluşturarak antibiyotiklere karşı daha dirençli hale geldikleri ve serbest yaşayan formlarına göre daha yüksek dozda antibiyotiklerden etkilendikleri saptanmıştır.

Peritonitis is a common complication in patients undergoing continuous ambulatory peritoneal dialysis. Majority of cases are usually due to a single agent. Besides, polymicrobial peritonitis develops in approximately 1-4% of patients. A gram-negative pathogen with another bacteria or fungus and rarely more than one gram- positive pathogen are responsible for this polymicrobial peritonitis. It is recommended to investigate intraabdominal focus in patients with polymicrobial peritonitis. In this article, we present a 29-year-old patient with chronic renal disease for 7 years who underwent continuous ambulatory peritoneal dialysis for the last 1 year and had polymicrobial peritonitis due to Serratia fonticola, Enterococcus faecium and Cryptococcus laurenti and resulted with exitus., Peritonit, sürekli ayaktan periton diyalizi uygulanan hastalarda sık görülen bir komplikasyondur ve vakaların çoğunluğunda genellikle tek bir etkene bağlı olarak gelişir. Bununla birlikte, hastaların yaklaşık %1-4’ünde polimikrobiyal peritonit gelişmektedir. Bu hastaların çoğunda gram negatif bir etken ile birlikte mantarlar, nadiren de birden fazla gram pozitif patojen rapor edilmektedir. Polimikrobiyal peritonit gelişen olgularda intraabdominal odak araştırılması önerilmektedir. Bu yazıda 29 yaşında 7 senedir kronik böbrek hastalığı olan ve son bir senedir sürekli ayaktan periton diyalizi uygulanan periton kültüründe Serratia fonticola, Enteroccus faeicum ve Criptococcus laurenti üreyen ve mortal seyreden bir hasta sunuldu.

Amaç:Kist hidatik hastalığı, ülkemizde, özellikle Doğu ve Güneydoğu AnadoluBölgesi'nde ve ilimizde ciddi bir sağlık problemidir. Hidatik hastalık,Echinococcus granulosus ve Echinococcus multilocularis tarafından oluşturulanbir protozoal hastalıktır. İnsanların karaciğer, akciğer, dalak ve beyin gibiiç organlarında kistler oluşturarak ölüme kadar giden istenmeyen sonuçlara yolaçabilmektedir. Bu çalışmada kist hidatik hastalığı nedeniyle ameliyat yapılanhastalarda anestezi deneyimlerimizi değerlendirmeyi amaçladık.Materyal ve Metot:Bu çalışmada; Ağustos 2015- Ağustos 2018 tarihleri ​​arasında,Harran Üniversitesi Araştırma ve Uygulama Hastanesi ameliyathanesindekist hidatik ön tanısıyla ameliyat olan hastalarındemografik verileri, uygulanılan anestezi yöntemi, anaflaksi oluşup oluşmadığı, hangi organ tutulumu olduğuretrospektif olarak incelendi. Bulgular:Çalışmamıza, kist hidatik tanısıyla hastanemizde cerrahi tedavisi yapılan 321hasta dahil edildi. Hastaların % 33,33’ü (n = 107) erkek, % 66,66’sı (n = 214)kadındı. Bu hastaların % 25,85’i (n = 83) 18 yaş altı pediatrik hasta, %74,17’ü (n = 238) yetişkin hasta idi. Hastaların % 55,76’sında (n = 179)karaciğer, % 36,44’ünde (n = 117) akciğer, % 3,11’inde (n = 10) akciğer vekaraciğer, % 2,18’inde (n = 7) böbrek, % 1,24’ünde (n = 4) kalp, % 0,93’ünde (n= 3) beyin, % 0,31’inde (n = 1) pelvik tutulum vardı. Hastalarda intraoperatifalerji ve anaflaktik reaksiyon bulgusu not edilmemiştir.Sonuç:Kist hidatik hastalığında operasyon sırasında anaflaktik reaksiyon ciddi birkomplikasyon olup nadiren görülmektedir. Kist hidatik ameliyatlarındaintraoperatif antihistaminik ve steroidin rutin kullanımının alerji veanaflaksiyi azalttığını düşünmekteyiz., Background: Hydatid cyst disease is aserious health problem in our country, especially in Eastern and SoutheasternAnatolia Region and in our province. Hydatid disease is a protozoal diseasecaused by Echinococcus granulosus and Echinococcus multilocularis. It canproduce cysts in the internal organs of humans such as liver, lungs, spleen andbrain, leading to undesirable consequences. In this study, we aimed to evaluateour anesthesia experience in patients who underwent surgery for hydatid cystdisease.Materials and Methods: In this study; BetweenAugust 2015 and August 2018, demographic data, anesthesia method, anaphylaxisand organ involvement were investigated retrospectively in patients operatedwith pre-diagnosis of hydatid cyst in Harran University Research andApplication Hospital operating room.Results: 321 patients who underwent surgicaltreatment in our hospital with the diagnosis of hydatid cyst were included inthe study. 33.33% (n = 107) of the patients were male and 66.66% (n = 214) werefemale. We think thatintraoperative antihistaminic and steroid use in cyst hydatid surgery decreasesallergy and anaphylaxis.

Amaç: Orta Hat kapanma Defekti (OHD) yüksek morbidite ve mortalite ileilişkili doğumsal spinal omurgaların birleşme kusurudur. OHD’li olan hastalardaFetuin-A, Osteopontin, Total antioksidan ve oksidan seviyeleri bilinmemektedir.Bu çalışmada bu paramatrelerin OHD’li olan hastalarda düzeyleri ve hastalıklaolan ilişkisi araştırıldı.Materyal veMetot: Çalışmaya, 0-5 yaş grubuarasında OHD olan hastalar (Çalışma grubu, n=80) ve benzer yaşta sağlıklıçocuklar (Kontrol grubu, n=85) olmak üzere 165 olgu alındı. OHD olan olgularSpina Bifida Occulta (Grup I, n=22), Spinal Meningosel veya Miyelomeningosel(Grup 2, n=26) ve Hidrosefalisi olan (Grup 3, n=31) olmak üzere 3 grubaayrıldı. Dağılımların normal olup olmadığı tek yönlü Kolmogorov–Smirnov testiile değerlendirildi. Gruplar arası karşılaştırmalarda, ki-kare, student ttesti, Mann-Whitney U testi ve Kruskal-Wallis varyans testleri kullanıldı. Fetuin-A, Osteopontin, total antioxidant andoxidant levels are unknown in patients with midline closure defects. In thisstudy, the levels of these parameters in patients with NTD and theirrelationship with the disease were investigated.Methods:The study included 165 patients (0-5 age group) with OHD (Study group, n = 80)and healthy children of similar age (Control group, n = 85). So metabolomics is the cheap and correct separation, definition and measurement of all metabolites in cells, tissues or biological fluids in short amounts of time with high throughput technologies such as NMR, GC-MS, and LC-MS. It is the quantitative measurement of the metabolic profile of the living being to characterize the genetics and the phenotypic response to the nutritional status of it. Environmental factors, such as feeding patterns and exposure to toxins, play an important role in the prevention/development of many diseases, such as obesity, type 2 diabetes, and therefore in the health status of individuals. There are many studies in the field of food science and nutrition, food quality and food safety and food processing. Being healthy is a combination of good genetics, good lifestyle and good nutrition. Metabolomics allows the analysis of physiological processes by monitoring metabolite concentration, they can vary according to metabolic information and to a substantial extent the molecular content of the food and its health properties and characterize the chemical changes as a result of food processing. In summary, a better understanding of the effects of genes, proteins, enzymes, metabolites at the cellular level and microenvironments can aid in metabolomic approach to the design of nutritional programs in order to process cell functions and improve overall health. Today, scientists are working to prevent obesity, diabetes, cardiovascular and chronic diseases and find new bioactive food components that prolong life, lose weight, improve physical and mental health, and prevent diseases such as heart attacks and cancer. In this context, the use of metabolomics techniques, the development of food consumption patterns, the benefits of any diet and improvement of physiological responses are provided and new components can be discovered. In summary, a better understanding of the effects of cell-level genes, proteins, enzymes, metabolites, and micro-environments can help metabolic approaches to the design of nutritional programs to process cell functions and improve overall health and to improve our understanding of the role of health and disease, nutrition and nutrients. Girişve Amaç: Etat mamillone endoskopikolarak gözlenen ve Helicobacter pylori pozitifolan hastalarda mukozal nodüler tutulum ile karakterizekolaylıkla tanınabilen karakteristik bir bulgudur. Bizbu çalışmada etat mamillone görüntüsü nedeniyle endoskopik biyopsi alınanhastalarda bu görüntünün histopatolojik tanıya göre doğruluğunu araştırdık. Gereç ve Yöntem: Bu çalışma Ocak2016-Aralık 2017 tarihleri arasında hastanemiz gastroenteroloji kliniğindeçeşitli sebeplerle endoskopi yapılan hastaların hasta kayıtları incelenerekretrospektif olarak yapılmıştır. Toplamda 5200 endoskopi kaydı incelenmiş, etatmamillion veya etat mamillone tabiri geçen 53 hasta çalışmaya dahil edilmiştir.Bulgular: Araştırma popülasyonu 41kadın (%77) ve 12 erkek (%23) erkekhasta olmak üzere 53 kişiden oluştu. Hastaların ortalama yaşı 29±11.2 (18-56)yıldı. Hastaların endoskopik biyopsi sonuçlarına bakıldığında 4 (%8) hastadaatrofik gastrit mevcut iken 49 (%92) hastada atrofi yoktu. Hastaların biyopsiörnekleri incelendiğinde 53 hastanın 45’inde Helicobacter pylori pozitifsaptandığı görüldü (%85). Bu hastaların Sydney sınıflamasına göre 12’sinde 1+ Helicobacterpylori (%23),17’sinde 2+ Helicobacter pylori (%32) ve 16’sında 3+ Helicobacter pylori (%30) saptanmıştır. In the present study, we investigated the accuracy of this appearanceon the basis of histopathological diagnosis in patients who underwentendoscopic biopsy for etat mamillone. Materialand Methods: The present study was performed retrospectively betweenJanuary 2016 and December 2017 in the gastroenterology clinic of our hospital.A total of 5200 endoscopy records were examined, and 53 patients with etatmamillion or etat mamillone were included in the study. Results: The study population consisted of 53 patients, 41 of whomwere female (77%) and 12 male (23%). The mean age of the patients was 29±11.2years (range: 18–56 years). Atrophic gastritis was observed in four patients (8%),while 49 patients (92%) had no atrophy. Upon biopsy, 45 of the 53 patients(85%) were found to be Helicobacter pylori positive.According to the Sydney classification, 12 patients had 1+ Helicobacterpylori (23%),17 had 2+ Helicobacter pylori (32%),and 16 had 3+ Helicobacter pylori (30%).There was 85% correlation between the endoscopic etat mamillone appearance andbiopsy results. Conclusion: Endoscopic etat mamillone appearance is anon-invasive and highly accurate method for diagnosis of Helicobacterpylori gastritis.

Thyroid cancer is a fataldisease has a high incidence. Therefore, the determination of moleculesinvolved in thyroid cancer is very crucial for early diagnosis and treatmentstrategies of the disease. In this study, high-dimensional functional genomicdata were integrated with system biology tools and the molecular targets andsignatures in thyroid cancer were determined. As a result of enrichmentanalysis, it was determined that important cancer pathways, metabolic pathwaysand immune system related pathways were activated. The protein- proteininteraction network was reconstructed using differential gene expression isdetermined by advanced statistical analysis and the molecular targets and signaturesin thyroid cancer were determined as JUN, LRRK2, BCL2, CCND1, TLE1, MET, ICAM1,DDB2 and RXRG. It was determined that these genes can differentiate tumorsamples and normal thyroid tissues via independent data analysis. Among theseproteins, JUN, TLE1 and DBB2 were found to be novel molecular targets. It ispredicted that these molecular targets can be used in the diagnosis andtreatment strategies of papillary thyroid cancer. Bağımsız birveri setinin analizi ile, bu genlerin tümör ve normal dokuları ayırt edebileceğibelirlenmiştir. Bu proteinler arasından JUN, TLE1 ve DBB2’nin yeni molekülerhedef ve biyoişaretçi aday olabileceği bulunmuştur. Belirlenen hedeflerinpapiller tiroid kanserinin teşhis ve tedavi stratejilerinin oluşturulmasındakullanılabileceği öngörülmektedir. Amanita species are the most common cause of mushroom poisoning with a high rate of mortality. A 58 years old woman who had a mushroom intake history 3 days ago was presented with nausea, vomiting and decrease in urine volume. The patient was evaluated to have acute kidney, liver and cardiac injury in the setting of rhabdomyolysis due to mushroom poisoning. Besides medical support therapy she received hemodialysis and plasmapheresis treatments, which resulted in complete clinical and laboratory improvement. We emphasized the significance of mushroom poisoning in the context of their wide clinical presentation spectrum and potentially lethal feature., Mantar zehirlenmelerinin mortalitesi yüksektir, sıklıkla amanita türlerine bağlı gelişir. Elli sekiz yaşında kadın hasta üç gün önce mantar yeme öyküsü ve bulantı, kusma, idrar miktarında azalma şikayetleriyle başvurdu. Amaç:Serebrovasküler hastalıkların kardiyak fonksiyonlar üzerine negatif etkileriolduğu bilinmektedir. Ek olarak, inme kliniğinde uzun dönem izlemde en sık ve en önemli ölüm nedeni kardiyakdisfonksiyondur. Bu çalışmanın amacı hemorajik inmeli hastalarda kardiyak enzimve inflamasyon belirteçlerinin değişimini incelemek ve bu belirteçlerin inmeprognozuna etklerini araştırmaktır. Materyal ve Metot: Bu çalışmaya intraserebral hematom (ISH) tanısı ile yatırılan ve hastalığın başlangıcının ilk 24 saati içindeolan 53 hasta ile 48 kontrol dahil edildi. Her iki grubun da 1., 5. ve 10. günkreatinin kinaz (CK), kreatinin kinaz myokardiyal band (CKMB), troponin-I(Trop-I), C reaktif protein (CRP) ve lökosit değerleri incelendi.Bulgular: Çalışmaya dâhil edilen 53 hasta ile 48 kontrolgrubu arasında yaş açısından anlamlı farklılık tespit edilmedi (P=0.062). 1.günCK, CRP ve lökosit değerleri ile 5. ve 10 gün CRP ve lökosit değerleri, ISH grubunda kontrol grubuna göre anlamlıolarak daha yüksek idi. Bununla beraber, ISH grubunda CK-MB ve troponindeğerlerinin giderek azaldığı, CRP değerlerinin ise giderek arttığı tespitedildi. 53 kişilik hasta grubunun 13 tanesi takipte exitus oldu. Yaşayanhastalar ile karşılaştırıldığında, exitus olan hastalarda 1. gün CK, CRP velökosit, 5. gün CK, CK-MB ve CRP ile 10. gün CK, CK-MB ve troponin değerlerianlamlı olarak daha yüksek idi.Sonuç:Çalışmamızda, intraserebral hematom tanılı hastalarda CK-MB vetroponin değerlerinde 1.günden itibaren bir azalma olduğunu, CRP değerlerindeise 1. günden itibaren bir artma olduğunu tespit ettik. In addition, cardiac dysfunction is themost common and most important cause of death in the stroke clinic duringlong-term follow-up. The aim of this study was to investigate the changes incardiac enzyme and inflammation markers in patients with hemorrhagic stroke andto investigate the effects of these markers on stroke prognosis.Methods: This study included 53 patients and 48 controls who were hospitalizedfor intracerebral hematoma (ICH) and were within the first 24 hours of theonset of the disease.Creatinine kinase (CK), creatinine kinase myocardial band (CKMB), troponin-I(Trop-I), C-reactive protein (CRP) and leukocyte values ​​of both groups were evaluated on the 1st, 5thand 10th days.Results: There were no significant differences in agebetween 53 patients and 48 control groups (68.50 ± 10.22 versus 71.70 ± 6.02, P= 0.062). CK, CRP and leukocyte values ​​on day 1 and CRP and leukocyte values ​​on day 5 and 10 were significantly higher inthe ICH group than in the control group. However, CK-MB and Troponin levelsdecreased and CRP values ​​increased gradually in ICH group. Thirteenpatients of 53 patients died during follow-up. Compared with the survivingpatients, CK, CRP and leukocytes on day 1, CK, CK-MB and CRP on day 5 and CK,CK-MB and Troponin on day 10 were significantly higher in patients with exitus.Conclusions: Inour study, we found a decrease in CK-MB and troponin values ​​from day 1 and an increase in CRP values ​​from day 1 in patients with intracerebralhematoma. In addition, we foundthat cardiac enzymes and inflammatory markers were higher in patients withexitus.Therefore, we believe that early recognition of myocardial damage andinflammation in hemorrhagic stroke patients may significantly reduce morbidityand mortality.

Blackhairy tongue (Lingua villosa nigra) is a benign and asymptomatic clinicalcondition characterized by marked accumulation of keratin on the filiformpapillae. It is rare in childhood. Although the etiology of the disease is notclear, various drugs, infections and poor oral hygiene are consideredresponsible. Here, we present a case of black hairy tongue which is thought tobe due to systemic antibiotic use and candida infection in a nine year old malepatient., Siyahkıllı dil (lingua villosa nigra), filiform papilla üzerinde belirgin keratinbirikimi ile karakterize, benign ve asemptomatik bir klinik durumdur. Çocukluk çağında ender görülür. Hastalığınetiyolojisi net olmamakla birlikte çeşitli ilaçlar, enfeksiyonlar ve kötü ağız hijyeni sorumlu tutulmaktadır. Burada,dokuz yaşında bir erkek hastada sistemik antibiyotik kullanımına ve kandida enfeksiyonuna bağlı geliştiğidüşünülen siyah kıllı dil olgususunulmaktadır.

Objective: In this study, it was aimed to determine the frequency of night eating syndrome (NES) risk in adults who appealed to a family health center. Method: The research was structured as a cross-sectional study at Gümüşhane Kürtün Family Health Center between February 2018 and August 2018. In this study, sociodemographic information questionnaire and the Night Eating Questionnaire (NEQ) were administered to 403 volunteers aged 18 years and over who appealed to the family health center for any reasons. Participants who received 25 points and above from the NEQ were considered positive for the risk of NES. Also, the height and weight of the participants were recorded. Results: The average age of participants was 40.5±15.4 years. Of these, 45.4% (n=183) were male and 54.6% (n=220) were female. A total of 11.7% (n= 47) of the sample screened positive for NES with a total NEQ score of ≥25. No statistically significant association was found between a positive NES screening result and some of the sociodemographic parameters (gender, marital status, working status, place of residency) (p>0.05). Significant positive associations emerged between a positive NES screening and body mass index, age and smoking status (p0,05). Q humması dünyada birçok ülkede görülen, Gram negatif, zorunlu hücre içi, bir bakteri olan Coxiellaburnetii’nin neden olduğu zoonotik bulaşıcı bir enfeksiyon hastalığıdır. Bu çalışmanın amacı, Kahramanmaraş ilinde risk gruplarında C.burnetii ve Q ateşi seropozitifliğininaraştırılmasıdır. Çalışmaya, Kahramanmaraş ilinde çalışan veteriner hekim,celep ve mezbaha kasabı gibi meslek gruplarından toplam 40 gönüllü birey (20-60yaş aralığında; 34’ü erkek, 6’sı kadın) katılmıştır. Çalışma grubuna ait serumlarda,ELISAyöntemiyle C.burnetii FazII IgG antikorları araştırılmıştır. Sonuç olarak, örneklerin 4 (%10 )’ünde C.burnetii seropozitifliği belirlenmiştir. C.burnetii seropozitifbireylerin tamamı (n:4) erkek (bir veteriner hekim, üç celep) idi. Buaraştırmanın sonuçları, ilimizde C.burnetii ve Q ateşi seroprevalansınındüşük olduğunu göstermiştir. The purpose ofthis study was to search the seroprevalence of C.burnetii and Q fever in risk groups inKahramanmaraş city, Turkey. A total of 40 voluntary individuals (34 male, 6 female;age range, 20-60years) including veterinarians, cattle-dealers, and butchers workingin Kahramanmaraş city were included in the study. C.burnetii phase IIImmunoglobulin G (lgG) antibodies were investigated by, Enzyme-Linked ImmunoSorbent Assay method in the sera samples of the study group. As a result, C.burnetiiseropositivity was determined in four (10%) of the samples. All positive samples (n=4)are male (one veterinarian, three cattle-dealers). These results of the study showed thatthe seroprevalence of C.burnetii and Q fever was low in the city. C.burnetii, which canbe transmitted from infected animals to humans, is of great importance for publichealth as it can cause pneumonia, hepatitis and endocarditis in humans.

Leyşmanyazis, enfekte dişi tatarcıkların ısırmasıyla insanlara bulaşan paraziter bir hastalıktır. Kutanöz leyşmanyazisin (KL) tanısında dermal kazıntıdan alınan örneklerde, mikroskopta parazitin gösterilmesi altın standart bir yöntem olarak yerini korumaktadır. Ancak bir yıl ve daha uzun süreli lezyonu bulunan kronik olgularda amastigot sayısı çok az sayıda olduğundan saptamak zordur. Hastaların öncelikle vücutlarında çıkan bu lezyonları dikkate almamaları, sağlık kurumlarına başvurmamaları veya geç başvurmaları, yanlış tedavi almaları gibi birçok nedenden ötürü tanı ve tedavi gecikmektedir. Ayrıca, lezyonlar üzerine sekonder enfeksiyon eklenerek prognoz kötüleşmekte ve yaralar kronikleşmektedir. Bu nedenle, kronik KL şüpheli olgularda mikroskop ile inceleme yöntemine ek olarak moleküler yöntemlerden faydalanılmaktadır. Bu çalışmada, mikroskop incelemesi sonucunda Leishmania amastigotları görülmeyen ancak klinik olarak KL açısından değerlendirilmesi rapor edilmiş şüpheli kronik KL Türkiye başlangıçlı olgulara ait dermal kazıntı preparatları seçilerek polimeraz zincir reaksiyonu (PCR) yöntemi kullanılarak leyşmanyazis tanısında moleküler yöntemin tanı değerinin ortaya konması amaçlanmıştır. Hatay Mustafa Kemal Üniversitesi Tıp Fakültesi Parazitoloji Laboratuvarına farklı polikliniklerden gelen KL şüphesi ile başvurmuş ve mikroskop incelemesi sonucunda endemik bölgelerden geldikleri kaydedilmiş (Hassa, Altınözü, Yayladağı gibi), klinik olarak kronik KL açısından değerlendirilmesi rapor edilmiş Türk hastaya ait dermal kazıntı preparatları (n= 50) çalışmaya dahil edilmiştir. Örneklerden DNA izolasyonu yapılarak tüm Leishmania türlerine özgü kinetoplastit DNA (kDNA) bölgesini hedefleyen 13A, 13B primerleri kullanılmıştır. PCR ile pozitif bulunan örnekler internal transcribed spacer (ITS-1) bölgesini hedefleyen LITSR ve L5.8S primerleri kullanılarak “polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP)” yöntemi ile tür tayini yapılmıştır. Toplam 50 adet dermal kazıntı örneğinden 17 (%34)’si kDNA bölgesini hedefleyen 13A, 13B primerleri ile pozitif olarak bulunmuştur. Pozitif bulunan örnekler ITS-1 bölgesini hedefleyen LITSR ve L5.8S primerleri ile de pozitif olarak saptanmıştır. ITS-1 gen bölgesiyle yapılan PCR sonucunda elde edilen ürünler BsuRI (HaeIII) enzimi kullanılarak kesilmiştir. PCR-RFLP analizi sonucunda 17 örneğin 11’i Leishmania tropica, biri Leishmania major ve beşi Leishmania infantum/donovani olarak tanımlanmıştır. Kronik KL; sarkoidoz, tüberküloz, malign tümörler gibi deri hastalıklarıyla karışabilmektedir. Özellikle, kronik KL olguları doğru tanı konulamaması, mikroskop deneyiminin yeterli olmaması, parazit sayısının az olması nedeniyle görülememesi gibi pek çok sebepten gözden kaçabilmektedir. Bu nedenle kronik KL şüphesi olan ancak mikroskop inceleme yöntemi ile parazit saptanamayan örneklerde moleküler yöntem olan PCR’nin kullanılmasının gerektiği sonucuna varılmıştır, Leishmaniasis is a parasitic disease that is transmitted to humans by the bites of infected female phlebotomine sandflies. In the diagnosis of cutaneous leishmaniasis (CL), in the smear samples, the demonstration of the parasite by microscope remains a gold standard method. However, it becomes difficult to diagnose the parasite since the number of amastigotes in chronic cases with a lesion of one year or longer is very low. Due to many factor such as patients primarily do not to take any notice these lesions in their bodies, do not apply to health institutions or late applied, receive wrong treatment; the diagnosis and treatment are delayed. In addition, it is been worse prognosis by add secondary infection to lesions and wounds become chronic. For this reason, molecular methods are used in addition to microscopic examination in chronic suspected CL cases. It was aimed to reveal of the molecular diagnostic value in chronic suspected CL cases by polymerase chain reaction (PCR) in the smear belonging to Turkish patients that reported to be evaluated clinically because it can not be seen Leishmania amastigotes in microscopic examination. Smear of 50 Turkish patients who were clinically reported of the evaluation of chronic CL were selected. These samples were smears belonging to suspected CL patients that applied Hatay Mustafa Kemal University, Faculty of Medicine, Parasitology laboratory from different polyclinics and were decided to be evaluated clinically as a result of microscopic examination because they came from endemic regions (such as Hassa, Altınözü, Yayladağı). DNA was isolated from selected samples and PCR was performed using 13A, 13B primers targeting the kinetoplastid DNA (kDNA) region. The samples found positive by PCR were typed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) analysis using LITSR and L5.8S primers targeting internal transcribed spacer (ITS-1) region. Of the 50 smear samples, 17 (34%) were determined positive with 13A, 13B primers targeting the kinetoplastid DNA (kDNA) region. Positive samples were also found to be positive with LITSR and L5.8S primers targeting ITS-1 region. The PCR products obtained from PCR with ITS-1 gene region were digested with the restriction endonucleases BsuRI (HaeIII). As a result of PCR-RFLP analysis, it was determined that 11 of Leishmania tropica, one of Leishmania major and five of Leishmania infantum/donovani out of 17 samples. Chronic CL can be confused with skin diseases such as sarcoidosis, tuberculosis, malignant tumors. In particular, chronic CL cases can be escaped the attention for many reasons such as failure to diagnose correctly, insufficient microscope experience, fail to see due to low number of parasites. For this reason, it was concluded that PCR, which is a molecular method, should be used in chronic suspected CL samples which are negative for the parasite by microscopic examination.

Amac: Periodontitis kaynakli dis kaybi olan ve mevcut implantlarinda peri-implantitis gelisen olgunun, cerrahi tedavi asamalari ve 3 yillik takip surecinin sunulmasi amaclanmistir. Olgu: Sistemik olarak saglikli ve sigara kullanmayan 43 yasindaki kadin hasta klinigimize peri-implant bolgede agri ve ciddi kanama sikayetiyle basvurdu. Hastanin dislerini periodontal nedenlerle kaybettigi ve sonrasinda dental implantlarin yapildigi; mandibuler implantlarin 6 yil icerisinde kaybedildigi ogrenildi. Klinik muayenede 7 peri-implant bolgede yumusak dokuda odem, ≥10 mm sondlama derinligi ve sondlamada kanama bulundu. Radyografik incelemede ise ileri duzeyde peri-implant kemik kaybi saptandi. Tum degerlendirmeler sonucunda 7 adet implantin cikarilmasina karar verildi. Operasyonlara hazirlik asamasinda implant uzeri sabit protezlerin sokumu planlandi ve bu sirada 3 implant kaybedildi. Ilk operasyonda, 13-14 bolgesindeki implantlar cikarilarak bolgeye rejeneratif tedavi uygulandi. Takip eden operasyonda, 23-27 bolgesindeki implantlar cikarildi. Bu operasyon sirasinda molar bolgede maksiller sinus membraninin, alveoler kret uzerinde agiz icine acik oldugu goruldu. Bu bolge kolajen membranla kapatilip, tum bolgelere rejeneratif tedavi uygulandi. Ucuncu operasyonda, sag molar bolgedeki implant cevresi tam kalinlik flep kaldirilarak, peri-implantitis tedavisi uygulandi. Sonuc: Periodontitis hikayesi olan hasta, 2008-2016 yillari arasinda 13 implantini kaybetti. Bu da, periodontitis hikayesine sahip bireylerin, peri-implantitis acisindan artmis risk altinda oldugu fikrini desteklemektedir. A CASE REPORT WITH 3 YEARS FOLLOW-UP ABSTRACT Aim: The aim of this case report is to present the surgical treatment stages and 3 years follow-up period of a case who has peri-implantitis lesions and tooth loss due to periodontitis history. Case: 43-year-old woman who was systemically healthy and non-smoker, referred to our department with peri-implant problems. It was learned that she had lost her teeth because of the periodontal reasons, then dental implants had been placed and then she lost mandibular implants in 6 years. In the clinical examination, peri-implant soft tissue edema and ≥10mm probing depth measurements with bleeding on probing were observed. Advanced peri-implant bone loss was detected on radiographs. After the examination, removal of implants was decided. In the curse of removal of fixed denture, 3 implants were lost. Firstly, implants 13-14 were extracted and regenerative treatment was performed. Later, implants 23-27 were extracted. In molar region, it was observed that maxillary sinus membrane was exposed to the oral cavity on the alveolar crest. This site was covered with resorbable collagen membrane; regenerative treatment was performed in all regions. Finally, peri-implantitis treatment was performed on implant 16. Result: The patient with a history of periodontitis lost 13 implants between 2008-2016. This supports the idea that individuals with a history of periodontitis are at greater risk for peri-implantitis. Key Words : periodontitis, peri-implantitis, dental implants.

Tokman, Hrisi Bahar/0000-0002-2205-5120; Demirci, Mehmet/0000-0001-9670-2426; WOS:000463858800003 Objective: The aim of this study were to determine the amount of Lactobacillus acidophilus and Lactobacillus casei, which are considered to have protective roles among the host-gut microbiota due to their probiotic properties, by using real-time polymerase chain reaction (PCR) and to compare between adult patients with type 1 diabetes mellitus (T1DM) and healthy individuals. Methods: Between January 2014 and October 2014, a total of 106 stool samples comprising those of 53 adult patients who were diagnosed with T1DM and those of 53 healthy individuals who presented to an endocrinology clinic were included in the study. DNA was isolated from the stool samples and analyzed with specific primers for L. acidophilus and L. casei by real-time PCR. Statistical analyses, such as Mann-Whitney U test, were used to determine the difference among patients with T1DM and healthy individuals. Results: The mean age of patients with T1DM and healthy individuals participating in the study was 32.87 +/- 12.68. A statistically significant difference was observed in the body mass index (BMI) and levels of HbA1c and fasting blood sugar of patients with T1DM when compared to those of the healthy individuals (p0.05). Conclusion: This study was the first to determine and compare the amounts of L. acidophilus and L. casei in the gut of patients with T1DM and healthy individuals. The amount of these bacteria with known probiotic activities is not different among patients with T1DM and healthy individuals, and there is no change in the amount of these bacteria in both the reciprocal triggering cycle of T1DM and gut microbiota. This situation needs to be clarified with comprehensive studies. Bu çalışmada, sığır ELISA-PAG (gebelik ilişkili glikoprotein) kitinin Konya Merinosu koyunlarda; 1) gebeliğin teşhisi ve 2) PAG'lerin erken gebelik plazma profilinin belirlenmesindeki kullanılabilirliği değerlendirildi. Bu amaçlar için, kan örnekleri gebeliğin 21-70. günleri arasında haftalık olarak alındı. Kit üreticisinin talimatına göre, PAG’lerin plazma seviyeleri belirlendi ve değer 0.3 OD’nin (optimal yoğunluk) üzerinde ise gebelik tanısı pozitif olarak kabul edildi. Gebeliğin 21 ve 70. günleri arasında PAG'lerin profilinde lineer benzeri bir artış olduğu belirlendi. PAG'lerin pozitif değer oranları 28, 35 ve 42. günlerde sırasıyla %30.8, %69.2 ve %66.7 olarak belirlendi. Bu sonuçlara göre, ticari sığır ELISA-PAG kitinin Konya Merinosu koyunlarda erken gebeliğin tespitinde ve fetoplasental yapının sağlığının izlenmesinde güvenilir olduğu belirlendi. Ancak, plazma örneklerinde gebelik tanısı için pozitif değer 0.3 OD kabul edilirse erken gebelikte yüksek bir hata payı ortaya çıkmaktadır In this study, the usability of bovine ELISA-PAGs (Pregnancy Associated Glycoproteins) kit was evaluated for 1) the diagnosis of pregnancy and 2) the determination of early pregnancy plasma profile of PAGs in Konya Merino ewes. For these purposes, the blood samples were taken from 21 to 70 days of pregnancy by weekly. According to the manufacturer’s instruction, the plasma level of PAGs was determined and accepted as the positive for pregnancy, if it was above than 0.3 OD (optimal density). An approximately linear increase in plasma profile of PAGs was determined for the period from 21 to 70 days of pregnancy. Percentages of the positive value of PAGs for on days 28, 35 and 42 were recorded as 30.8%, 69.2%, and 66.7%, respectively. According to these results, it can be concluded that the commercial bovine ELISA-PAGs kit is good for detecting early pregnancy and monitoring fetoplacental unit well-being in Konya Merino ewes. However, there is still a high margin of error for ewes if considering 0.3 OD for the positive.

Prevalence of non-communicable diseases has shown an increase in rapidly urbanizing population. It introduces us with a new urban epidemic which shapes by lifestyle and working patterns of urban residents. Physical inactivity, unhealthy diet, smoking and alcohol consumption are four key risk factors which lead the development of non-communicable diseases. Socio-economic, environmental and occupational influence which are brought together with urban settlement increases the impacts of these factors. Air pollution, problems in the food system, lack of green space and living in poor neighbourhoods are important urban features which can trigger noncommunicable diseases. Type 2 diabetes, cardio-vascular diseases, depression, anxiety, chronic obstructive pulmonary disease, cancer, and arthritis are among the noncommuncable disease which can be associated with urban features. Improving urban features, strengthening the city's residents, ensuring community involvement, adapting urban planning and primary health care according to these interventions and implementation of the multi-sectoral strategy may be useful for control and prevention of these diseases.Bulaşıcı olmayan hastalıkların prevalansı hızla kentleşen nüfusta artış göstermektedir. Bulaşıcı olmayan hastalıkların gelişiminde öncülük eden dört temel risk faktörü; fiziksel inaktivite, sağlıksız beslenme, sigara içimi ve alkol tüketimidir. Kentsel yerleşimin beraberinde getirdiği sosyo-ekonomik, çevresel ve mesleksel etkiler bu risk faktörlerinin etkisini artırmaktadır. Hava kirliliği, gıda sistemindeki sorunlar, yeşil alanların azlığı ve yoksul semtlerde yaşam bulaşıcı olmayan hastalıkları tetikleyebilen önemli kentsel özelliklerdendir. Tip 2 Diyabet, kardiyo-vasküler hastalıklar, depresyon, anksiyete, kronik obstrüktif akciğer hastalığı (KOAH), kanserler ve artritler kentsel özellikler ile ilişkili olabilen bulaşıcı olmayan hastalıklardandır. Bu hastalıkların kontrolü ve önlenmesinde sürdürülebilir kalkınma hedefleri doğrultusunda kentsel özelliklerin iyileştirilmesi, kent sakinlerinin güçlendirilmesi, toplumsal katılımın sağlanması, kentsel planlamanın ve birinci basamak sağlık hizmetlerinin bu müdahale alanlarına göre uyarlanması ile çok sektörlü stratejilerin uygulanması yararlı olabilir.

Objective: The objective of this study is to define novel biomarkers for Prostate Cancer (PCa) via in silico analysis that takes PCa-specific miRNAs, finds their combinatorial target genes (potential ceRNAs), selects ones containing Transcribed Ultra Conserved Region (T-UCR) among them and potentiates their relevance with PCa. Methods: Thirty-four miRNAs of which clinical relevances with PCa were proved experimentally were exported via miRWalk database.Using the ComiR database, 859 genes targeted by these 34 miRNAs simultaneously were identified. Genes with ComiR score above 0.911 were taken into account. Genes containing T-UCR and showing potential ceRNA activity were extracted. Among PCa-associated ceRNAs including T-UCR, we identified genes with significant expression differences between PCa and normal prostate tissue using the GEPIA database. The statistical evaluation of the association of NFAT5 and PTBP2 genes with PCa was performed by Spearman correlation test in GEPIA database. Results: PCa-associated ceRNAs cross-matching with genes including T-UCR in their exonic regions were NFAT5, CLK3, PTBP2, CPEB4, MIPOL1 and TCF4. We identified genes with significant expression differences between PCa and normal prostate tissues among PCa-associated ceRNAs including T-UCR. According to this analysis, NFAT5 and PTBP2 genes were significantly less expressed in PCa than in normal prostate tissue while the others didn’t show any significant differential expression pattern. NFAT5 and PTBP2 genes were found to be significantly associated with PCa (p=0.000012; R=0.72). Conclusion: All in all, this is the study associating NFAT5 and PTBP2 genes with PCa and giving them tumor suppressive potential for PCa. Still, larger and more comprehensive studies are needed on this issue.

Objective: laboratory diagnosis of Brucella canis infections cannot be made by classical serological methods as readily as infections of other species of Brucella pathogenic for humans. Therefore, the information about B. canis infections in Turkey is limited. In this study, we aimed to detect presence of B. canis coinfections in humans who were diagnosed as brucellosis. Methods: study has been designed as retrospective cross-sectional. A total of 566 serum samples collected from patients who had confirmed brucellosis and were living in cities of the Eastern and Southeastern Anatolia regions of Turkey were tested with rapid slide agglutination test (RSAT), 2-mercaptoethanol RSAT (2ME-RSAT) and microplate agglutination test (MAT) using B. canis M-, a less mucoid variant, as the antigen. Results: out of the samples tested, 142 (25%) and 49 (8.7%) were positive by RSAT and 2ME-RSAT, respectively, and this difference was significant (p= 0.05). In total, 55/ 566 (9.7%) samples were MAT-positive. Differences between 2ME-RSAT and MAT were not statistically significant (p>0.05). Conclusions: our B. canis coinfection prevalence data demonstrates that serological testing for B. canis should be performed in Turkey. It was also concluded that there are some advantages for using MAT, and this technique may be preferable over 2ME-RSAT. / Amaç: Brucella canis infeksiyonlarının laboratuvar tanısı, insanlarda hastalık yapan diğer Brucella türlerinin infeksiyonlarındaki gibi klasik serolojik yöntemlerle konulamamaktadır. Bu nedenle Türkiye’de B. canis infeksiyonları hakkındaki bilgiler kısıtlıdır. Bu çalışmada, Türkiye’de bruselloz tanısı almış hastalarda B. canis koinfeksiyonu varlığının saptanması amaçlanmıştır. Yöntemler: çalışma retrospektif kesitsel olarak tasarlandı. Türkiye’nin Doğu ve Güneydoğu Anadolu bölgelerinde bruselloz tanısı almış 566 hastanın uygun koşullarda saklanmış serum örneği, zayıf mukoid B. canis M- varyant suşundan elde edilen antijenin kullanıldığı hızlı lam aglütinasyon testi (LAT), 2-merkaptoetanol lam aglütinasyon testi (2ME-LAT) ve modifiye plak aglütinasyon testi (MAT) ile analiz edildi. Bulgular: test edilen örneklerden 142 (%25)’si LAT, 49 (%8.7)’u 2ME-LAT ile B. canis seropozitif olarak saptandı ve aradaki fark istatistiksel olarak anlamlı bulundu (p≤0.05). MAT ile örneklerin 55 (%9.7)’i pozitif olarak saptandı. 2ME-LAT ve MAT arasında istatistiksel olarak anlamlı bir fark saptanmadı (p>0.05). Sonuçlar: B. canis koinfeksiyonu prevalansına ilişkin bulgularımız, Türkiye’de B. canis için serolojik testin yapılması gerektiğini göstermektedir. Amac: Periodontal hastaliga sahip bireylerde implant tedavisinin prognozu hakkinda farkli bulgulara sahip cok sayida calisma mevcuttur. Bu calismanin amaci; periodontal hastalik gecmisi olan hastalarda ve periodontal olarak saglikli bireylerde ortalama 29 aylik takip suresinde implant basarisinin tespiti ve implant cevre kemik doku kaybinin; sigara kullanimi, yas, cinsiyet ve Diyabetes Mellitus (DM) gibi hastaya bagli parametrelere gore karsilastirilmasidir. Gerec ve Yontem: Bu calismada; 14 periodontal olarak saglikli bireye ve 12 periodontal hastalik gecmisi olan hastaya toplamda 139 adet implant yerlestirildi. Tum implantlar 8-10 hafta iyilesme surecini takiben yuklendiler. Implantlarin cevresinde marjinal kemik kayiplari baslangic ve ortalama 29 ay sonrasinda olculdu. Periodontal hastalik gecmisi olan hastalarda ve periodontal olarak saglikli bireylerde implant basarisi degerlendirildi. Iki hasta grubu arasinda implant cevre kemik doku kaybi hastaya bagli parametrelere gore karsilastirildi. Bulgular: Sigara kullanimi ve DM; implant cevresi marginal kemik kaybini her iki hasta grubunda istatistiksel olarak anlamli sekilde etkilemistir (p≤ 0,05). Periodontal hastaliga sahip bireyler grubunda kadin hastalarda marjinal kemik kaybi degerleri erkek hastalara kiyasla daha dusuktur. Buna karsilik saglikli bireyler grubunda; kadin hastalarda takip suresi icerisindeki ortalama kemik kaybi erkek hastalardan daha fazladir. Ancak her iki gruptaki bu farklilik istatistiksel olarak anlamli bulunmamistir (p≥ 0,05). Takip suresi icerisindeki ortalama kemik kaybi degerleri her iki grup icinde genc bireylerde, orta ve yasli bireylere gore daha az bulunmustur. Ancak bu farklilik istatistiksel olarak anlamlilik ifade etmemistir (p≥ 0,05). Calismada 29 aylik ortalama basari orani tum implantlarda, Periodontal hastaliga sahip bireylerde ve Periodontal olarak saglikli bireyler gruplarinda sirasiyla; %95, %93,8 ve %96,6 olarak tespit edilmistir. Sonuc: Implantlarin takip suresi icerisindeki marjinal kemik kayiplarina diabet ve sigara kullaniminin etkisi aciktir. Cevre marjinal kemik kayiplari periodontal hastalik gecmisi olan bireylerde, periodontal olarak saglikli bireylere gore istatistiksel olarak anlamli artis gostermistir. Bu acidan periodontal hastalik gecmisi olan bireylerde implant tedavisinde ikincil basarisizlik faktorlerinin dikkatli degerlendirilmesi gerekmektedir. Anahtar Kelimeler: Periodontitis, Dental Implant, Diabet THE SUCCESS RATES OF IMPLANTS IN PERIODONTALLY COMPROMISED AND PERIODONTALLY HEALTHY PATIENTS- A 29 MONTHS RETROSPECTIVE EVALUATION ABSTRACT Aim: The aim of this study is the evaluation of the success rate of implants in periodontally compromised and periodontally healthy patients in average 29 months follow-up time. And comparison of the marginal bone loss around implants according to patient related parameters as smoking, age, gender and diabetes mellitus. Materials and Methods: One hundred and thirty nine implants were placed in 14 periodontally healthy and 12 periodontally compromised patients and loaded with final restorations 8-10 weeks of healing time. Marginal bone loss around implants was determined radiographically at initial and after a mean observation time average of 29 months. The smoking habit, Diabetes Mellitus, age and gender on the marginal bone loss was evaluated. Results: Marginal bone loss was affected statistically significantly by smoking and DM, in both groups (p≤ 0,05). In PHB group marginal bone loss was found higher for men, in PSB group marginal bone loss was found higher for women but for both of the groups these differences were not statistically significant (p≥ 0,05). Also in both of the groups age was not the decisive factor for marginal bone loss. The overall cumulative survival rate for 29 months was % 95 and for PHB and PSB groups consecutively, %93,8, %96,6. Conclusion: The affect of smoking and DM is obvious for the marginal bone loss for both of the groups. And for periodontally compromised patients bone loss increases statistically significantly higher in comparison to healthy patients. During the implant treatment, for periodontally compromised patients secondary factors must be followed carefully. Key words: Periodontitis, Dental Implant, Diabetes Mellitus

Lyme disease, which is caused by spirochete Borrelia burgdorferi sensu lato, is a tick-transmitted, multisystem infectious disease. It occurs in stages, with different clinical manifestations at each stage. Erythema migrans is the most frequent manifestation which occurs at the site of the tick bite. Borrelial lymphocytoma and acrodermatitis chronica atrophicans are late-stage cutaneous manifestations. Extracutaneous signs of infection most often involve the musculoskeletal, cardiovascular, and nervous systems. Serologic assays remain the mainstay of diagnosis. All stages of the disease are curable with appropriate antibiotic therapy.

Objectives: Among woman, breast cancer is the most prevalent cancer worldwide. It is composed of various cell types that are classified into different subtypes such as triple negative breast cancer (TNBC), triple positive breast cancer (TPBC). TNBCs and TPBCs represent distinct genetic background, thereby leading to therapeutic diversity in breast cancer. It is critical to know their tumorigenic properties to overcome the diversity. Methods: TPBC cell lines; BT474, HCC1954 and TNBC cell lines; MDA-MB-435, Hs578T, MDA-MB-231 were used to measure intracellular iron levels via a fluorescent probe, calcein-AM, utilizing flow cytometry. Breast cancer stem cells (BCSCs) are detected by surface expression of CD44+/CD24- markers in the lines. Results: Here, it has shown that TNBCs have higher basal levels of iron and population of BCSCs than TPBCs. Iron addition provides enrichment of BCSCs in TPBCs. Conclusion: Iron is an important element for maintenance of BCSCs. Giriş: Meme kanseri dünya çapında kadınlarda görülen en yaygın kanserdir. Üçlü negatif ve meme kanseri (TNBC) ve üçlü pozitif meme kanseri (TPBC) gibi farklı alt tiplerde kategorilere ayrılan heterojen bir hastalıktır. TNBC’ler ve TPBC’ler farklı genetik altyapıları gösterirler ve bu meme kanserinde terapötik çeşitliliğe yol açar. Bu çeşitliliğin üstesinden gelmek için hücrelerin tümörijenik özelliklerini bilmek çok önemlidir. Yöntemler: Flowsitometriden faydalanılarak floresan bir prob olan calcein-AM yardımıyla hücreiçi demir seviyelerini ölçmek için TPBC hücre hatları BT474, HCC1954 ve TNBC hücre hatları MDA-MB-435, MDA-MB-231, Hs578T kullanıldı. Meme kanseri kök hücreleri (BCSCs) hücre hatlarında CD44+/CD24- ‘ün yüzey ekspresyonu ile tanımlandı. Bulgular: Burada TNBC’lerinTPBC’lerden daha fazla bazal demire ve BCSC’ye sahip olduğu gösterilmiştir. Demir ilavesi TPBC’lerde BCSC populasyonunun artmasını sağlamıştır. Sonuç: Demir BCSC’lerin canlılığını idame etmesi bakımından önemli bir elementtir.

Amaç: Otitin komplikasyonu olarak beyin absesi sıklığı azalmışıtır. Sinüzitten kaynaklanan beyin absesi ise hem yetişkinlerde hem çocuklarda önemli bir yer teşkil etmektedir. Bu çalışmada otojenik ve sinojenik kaynaklı 6 beyin absesi vakası sunuyoruz.Gereç ve yöntem: Ocak 2013 ile Mayıs 2017 tarihleri arasında intrakraniyal abse ile yatırılan çocukların verileri geriye dönük olarak değerlendirildi. Önceden belirleyici faktörler, klinik bulgular ve prognoz belirlendi.Bulgular: İntrakraniyal abse bulunan 8 çocuk arasında 6’sı otogenik ve / veya sinogenik enfeksiyona sahipti. En sık görülen semptom baş ağrısıydı. İntrakraniyal abse 3 olguda epidural, 3 olguda parankimaldi. Etken patojenler olarak Streptokok intermedius, Streptococcus pyogenes, Provotella ile Propionibacterium acnes tespit edildi. Tüm çocuklara antibiyoterapi ve cerrahi drenaj uygulandı. Nörolojik sekel ve ölüm gerçekleşmedi. Sonuç: Sinüzit ve otitis media; antibiyotik kullanımının artmasına rağmen, beyin abseleri için hala önemli birer predispozan faktördür., Background and aims: Brain abscess as a complication of ear infections has decreased in frequency. By contrast, brain abscess arising from a sinus infection remains an important consideration in both adults and children. Here we present 6 cases with brain abscess due to otogenic or sinogenic complications.Methods: Data of children hospitalized with intracranial abscess between January 2013 and May 2017 were evaluated retrospectively. Predisposing factors, clinical manifestations, and prognosis were identifiedResults: Among 8 children with intracranial abscess 6 had otogenic and/or sinogenic infection. The most frequent symptom was the headache. The intracranial abscess was epidural in 3 cases and parenchymal in 3 cases. Causative pathogens were Streptococcus intermedius, Streptococcus pyogenes, and Propionibacterium acnes with Provotella loescheii. All children received antibiotics and surgical drainage. Neurological sequelae and death did not occur Conclusion: Despite the increased use of antibiotics, sinüsitis and otitis media is an important predisposing factor for brain abcess.

Brown Adipose Tissue (BAT) present only in mammals play a significant role in maintaining body temperature through non-shivering thermogenesis during the early days of extra-uterine life. “Metabolic Winter Hypothesis”, suggests that cold exposure is one the most powerful and physiological stimulus for BAT activation resulting in weight loss through increased thermogenesis. The objective of this study was to review the relationship between cold stress (metabolic winter hypothesis) and related factors in energy metabolism through activation of brown adipose tissue. Literature search was conducted using Science direct, Medline, Scopus data bases, 59 studies were included in this review. Mitochondrion rich BAT are unique in the sense that they bypass adenosine triphosphate (ATP) production and instead create heat by activating uncoupling protein 1 (UCP-1). Repeated stimulation by cold (16-18◦C) few hours per day for a period of 6 weeks in human adults resulted in hyperplasia of brown adipocytes and increased tissue mass. Increased stimulation from sympathetic nerve endings also resulted in increased UCP-1 activity as found in studies performed with rodents and humans. Melatonin, a hormone associated with sleep, acting to lower the core body temperature, increased BAT activity and improved glucose metabolism. New studies suggest that Sirtuins (Sır-2) proteins and exercise-induced production of Irisin causes an increase in BAT activity by stimulating mitochondrial biogenesis in brown adipocytes leading to increased energy expenditure. Further studies are required to elucidate the novel relationship between cold stress and altered energy metabolism, promising a solution to obesity related progressive metabolic diseases.

In this study, we aimed to investigate the incidence of Dientamoeba fragilis with different diagnostic methods in patients with gastrointestinal symptoms and determine the sensitivity and specificity of existing diagnostic methods. Fecal samples collected from 101 patients with gastrointestinal complaints (especially upper abdominal pain, abdominal and pelvic pain, nausea and vomiting, gastroenteritis and colitis, unexplained fever and diarrhea) and 20 control cases from various clinics were included in the study. Samples were first examined with native-Lugol (N-L) method and cultured in Robinson medium. All 121 stool and culture samples were stained with iron hematoxylin stain (IHS) and trichrome stain (TS) methods and examined by PCR and QPCR for D.fragilis. Among 121 stool samples 13 (10.7%), 2 (1.7%), 7 (5.7%) 13 (10.7%), and 7 (5.8%), 4 (3.3%), 2 (1.7%), 3 (2.5%) of cultured samples were determined positive with IHS, TS, PCR, QPCR respectively. Fifteen of the 121 stool samples were determined as diarrheal. All diarrheal stool samples were negative with IHS and TS. One of the diarrheal stools and 6 (4.9%) of the non-diarrheal stools were positive by PCR. All of the diarrheal stools were negative. Thirteen of the non-diarrheal stool samples (10.7%) were positive by QPCR. When the QPCR method was considered as gold standard, sensitivity and specificity values were determined as 46% and 93% in IHS, 0% and 99% in TS, 54% and 100% by PCR and sensitivity and specificity values were 67% and 96% in IHS, 33% and 98% in TS, 67% and 100% by PCR among cultured stool samples. As a result, it was determined that there was a statistically significant difference between the samples of the patients and the control groups and the sensitivity and specificity of the conventional and molecular methods (IHS, TS, PCR and QPCR) determined in this study supported the results of other compared studies. It has been determined that staining methods used for the diagnosis of D.fragilis gave false positivite or negativite results. In addition, the QPCR method is more advantageous in terms of time saving for the diagnosis and initiation of the treatment and in cases where QPCR is not available, IHS and conventional PCR methods should be used together. In our opinion, this study will contribute to the results of epidemiological and scientific studies on D.fragilis in Turkey.

Amaç: Demir eksikliğine bağlı anemi (IDA), dünyada özellikle yeni doğanlarda, çocuklarda ve kadınlarda en sık görülen besinsel eksikliktir. Düşük demir içeriği olan besinlerin tüketilmesi ve ince bağırsaktan yeterli demirin emilememesi demir eksikliğine bağlı anemiye neden olmaktadır. Memelilerde fizyolojik olarak aktif demirin vücuttan atılımı için bir sistemin olmamasından dolayı bağırsak enterosit hücreleri, vücuttaki demir metabolizmasının dengeli bir şekilde devamlılığında hayati bir role sahiptirler. Yeterli bağırsak demir emilimi anemi riskini azaltmak için gereklidir. Diyetten alınan ve kanda bulunan demir insan vücudu için bulunan iki demir kaynağıdır. Bu çalışmada demir eksikliğine bağlı anemi durumunda bağırsak demir metabolizmasının moleküler ve genetik düzenlenmesinde diyetten gelen ve kanda bulunan demir mineralinin etkisinin incelenmesi amaçlanmıştır. Gereç ve Yöntem: İnsan kolon epitelyal hücreleri (CaCo-2) insan ince bağırsak sistemini modellemek için özel hücre kültürü membran sistemlerinde büyütülmüştür. Kolon hücrelerin apikal ve bazolateral polarizasyonu trans epitelyal rezistans (TEER) ölçümüyle test edilmiştir. Demir eksikliğine bağlı anemi Deferoxamine (DFO) ile indüklenmiş olup, demirin IDA üzerindeki etkisi RT-qPCR tekniği ile gen düzeyinde araştırılmıştır. Bulgular: Marker gen (TFR, DMT1) mRNA ekspresyon düzeyleri analiz edildiğinde, hücrelerin bazolateral kısmına demir verildiğinde apikal kısma verilene oranla IDA fenotipinin kaybolduğu gösterilmiştir. Sonuç: Sonuçlar kan demirinin bağırsak demir metabolizmasının düzenlenmesi için gerekli olduğunu göstermiştir. Kan demir düzeyi diyetten gelen demirle regüle edilir. Böylece, günlük demir alımı kan demiri düzeylerini düzenlemek için önemlidir. Bağırsak enterosit hücrelerinin bazolateral kısım boyunca demir mineralini algılama kabiliyetine sahip olabileceğini desteklemektedir., Aim: Iron deficiency anemia (IDA) is the most common global nutritional deficiency especially in women, children and infants. Consumption of low dietary iron and inadequate intestinal iron intake cause IDA. Intestine of enterocyte cells play a vital role to maintain body iron homeostasis since mammals do not have active physiological iron excretion mechanism. Efficient intestinal iron absorption is essential to decrease risk of anemia. Diet and blood derived iron are two source for human body. Thus, the aim of this study was to investigate the effects of dietary and blood iron minerals on the molecular and genetic regulation of intestinal iron metabolism during IDA.Material and Method: The human colon epithelial cells (Caco-2 cells) were grown on special bicameral cell culture insert systems to mimic the human small intestine. The apical and basolateral polarization of the cells were tested by trans epithelial electrical resistance (TEER) measurement. IDA was induced by Deferoxamine (DFO) and the effect of iron on IDA was investigated at the gene regulation levels by RT-qPCR technique. Results: Our main result showed that when iron was given into basolateral side of cells, iron deficient phenotype was disappeared compared to apical side iron treatment by analyzing mRNA expression levels of marker genes (TFR and DMT1). Conclusion: This study suggests that blood iron is essential to maintain intestinal iron metabolism. The blood iron level is regulated through dietary iron. Therefore, daily iron intake is important to maintain blood iron levels. Moreover, our results also suggest that enterocyte cells might have ability to sense iron mineral through basolateral side.

WOS: 000464106200018 Gram-negative bacterial meningitis can usually be seen in neonates and elderly patients, after head trauma or interventions such as prostate biopsy and brain surgery. Typical findings of acute bacterial meningitis such as fever, headache and meningeal irritation signs may not be observed, and mortality rate is high in these patients. In this report, we present a case of Gram-negative bacterial meningitis secondary to bacteremia probably due to urinary tract infection. The patient died despite appropriate antibiotic therapy. Central nervous system infection should be kept in mind especially when elderly patients with comorbid diseases do not respond to antibiotic treatment and changes in mental status develop.

Giriş: Tüberküloz genel olarak akciğerlerde görülmekle birlikte tüm organları etkileyebilmektedir. Ancak akciğer dışı tutulum olduğunda tanı koymak, akciğer tutulumundan daha zordur. Mycobacterium tuberculosis’in izole edilmesi, ekstrapulmoner tüberküloz (EPTB) tanısında altın standart olmasına karşın, özellikle EPTB’de bakteriyolojik izolasyon oranı düşüktür. Eğer M. tuberculosis elde edilebilirse, hastalığın epidemiyolojik özellikleri ve ilaç duyarlılığı hakkında bilgi vermektedir. Materyal ve Metod: Bu çalışmada, 2009-2016 yılları arasında mikobakteriyoloji laboratuvarında M. tuberculosis izole edilen ekstra pulmoner örnekler değerlendirilmiştir. İzole edilen bakterilerin genotiplerin belirlenme işlemi ve ilaç duyarlılık testleri yapılmıştır. Spoligotipleme, daha önce tarif edilen standart bir teknik kullanılarak gerçekleştirilmiştir. Bulgular: Çalışma süresince 165 hastaya ait 171 ekstrapulmoner örnekte M. tuberculosis (75 erkek, 90 kadın, ortalama yaş: 53.35 ± 19.92) kültürde üretilmiştir. Birden fazla ekstrapulmoner organ tutulumu 6 hastada mevcuttur. En yaygın EPTB şekli 60 hastada lenf nodu, 32 hastada plevral tüberküloz ve 25 hastada kemik tüberkülozdur. Yedi hasta insan immün yetmezlik virüsüyle infekte olmak üzere 44 hastada immün yetmezlik saptanmıştır. Tüberküloz ilaç direnci 175 örneğin 21’inde tespit edilmiştir. Rifampisin direnci 7 örnekte, yüksek düzeyde izoniazid direnci 11 örnekte ve rifampisin ve izoniazid direnci (çoklu ilaç direnci) 6 örnekte gösterilmiştir. Tüberküloz hastalarının farklı örneklerinden elde edilen 135 klinik izolatın genetik profili Spoligotyping kullanılarak belirlenmiştir. Majör Spoligotipler, T (n= 62; %45.9), LAM7-TUR (ST41) (n= 11; %8.1) ve H1 (n= 9; %6) genotipleridir. Sonuç: En yaygın EPTB şekli, kültürle kanıtlanmış hastalarda lenf nodu tüberkülozudur. Bu hasta grubunda, çoklu ilaç direnci oranı düşüktür (%3.6). Spoligotip T (%45.9) en sık görülen genetik profil olarak tespit edilmiştir., Introduction: Tuberculosis is seen generally in the lungs. Besides, all organs in the body can be affected by tuberculosis. Diagnosis of extra pulmonary tuberculosis (EPTB) is more difficult than pulmonary tuberculosis (PTB). Although, the isolation of Mycobacterium tuberculosis is gold standard of diagnosis of EPTB, the rate of bacteriologic isolation is low especially in EPTB. If M. tuberculosis is detected, it gives some information about the epidemiological features of the disease and drug susceptibility. Materials and Methods: in this study, extra pulmonary samples isolated M. tuberculosis in mycobacteriology laboratory were evaluated between 2009-2016. the identification of the genotype of isolated bacteria and drug sensitivity tests were conducted. Spoligotyping was accomplished using a standard technique as described previously. Results: During the study period, M. tuberculosis were cultured in 171 extra pulmonary samples of 165 patients (75 male, 90 female, mean age: 53.35 ± 19.92). Initial direct microscopically examination was revealed M. tuberculosis in 44 patients. There were more than one extra pulmonary organ involvement in six patients. the most common EPTB forms were lymph node TB in 60 patients, pleural tuberculosis in 32 patients and bone tuberculosis in 25 patients. Immunosuppression was detected in 44 (%26.6) patients. Among these, seven patients were infected with HIV. in 21 of 175 samples, drug resistance was detected. Rifampicin resistance in 7 samples, high level isoniazid resistance in 11 samples and rifampicin plus isoniazid resistance (multiply drug resistance) in 6 samples were demonstrated. One hundred thirty-five clinical isolates were cultured from tuberculosis patient’s different samples, of which the genetic profile was determined by using Spoligotyping. the major Spoligotypes were T (n= 62; 45.9%), LAM7-TUR (ST41) (n= 11; 8.1%) and H1 (n= 9; 6%) genotypes. Conclusion: the most common EPTB form was lymph node tuberculosis in culture proven patients. in these patients group, multiply drug resistance rate was low (3.6%). Spoligotypes T (45.9%) was detected as most common genetic profile.
