In this paper, energy communities are examined as an arena within which the possibilities for a just and democratically oriented climate transition are currently being enacted. More specifically, it explores how energy communities (ECs) in Sweden are taking shape against the background of the EU's Clean Energy Package, wherein ECs have been identified as an important instrument for increased citizen participation and deepened democratization. With our theoretical basis in critical social research, we examine the Swedish landscape of energy communities as a discursive battlefield, where sociomaterial arrangements and actor interests are under formation. Special attention is paid to actors' political drivers for engagement, the ideological motivations that are expressed, the transformative strategies that are put into play, and the alliances that are formed. We argue that this is of sociological importance to scrutinize how laudable ambitions and visions of a climate-neutral society are put into practice, so as to illuminate power relations that otherwise risk being made invisible. We find that Swedish energy communities are generally forced to relate to a dominant innovation-oriented discourse with a depoliticizing effect. At the same time, we note that a variety of other ways of enacting community energy abound, which reflect values that deviate from the dominant discourse. [ABSTRACT FROM AUTHOR]

Cardiovascular disease is the most important cause of death and life-years lost in Sweden today. Cardiovascular risk prediction is a cornerstone in primary prevention; the use of antihypertensive and lipid-lowering therapy is guided by absolute cardiovascular risk. The Systematic COronary Risk Evaluation (SCORE) model has been the most widely applied model in Sweden for almost two decades. Recently, an updated model called SCORE2 was published. The new risk prediction model is based on contemporary data, predicts the risk of incident cardiovascular disease in addition to cardiovascular mortality, and accounts for competing risks, thus overcoming some major limitations with SCORE. Sweden is classified as a moderate-risk country according to the new model; here we report the risk chart for moderate-risk countries translated into Swedish.

Early diagnosis and correct treatment of acute scaphoid fractures is mandatory and of great importance in terms of the long-term prognosis for these young patients. Missed acute scaphoid fractures tend to lead to pronounced symptoms later on. A SNAC wrist, with the gradual progression of degenerative osteoarthritis is most often debilitating, with pain, limited and restricted range of motion, and loss of strength.

In an international perspective, Sweden has a very low case fatality after acute myocardial infarction (AMI). The aim of this study was to present trends and regional differences in case fatality within 28 days of the first AMI for males and females in Sweden after adjusting for co-morbidity. Adjustments in order to remove random effects on the rank order of county councils were made. The study was based on national data on more than 500,000 cases of AMI. Between 1987 and 1999, case fatality after AMI decreased from 47% to 37% among men and from 44% to 34% among women. The case fatality in the individual counties varied from 29% to 37% for men and from 31% to 40% for women. Further analysis is needed in order to explain these variations.

Prostate cancer is the most common malignancy among males in the West. Any reduction in mortality would require early detection of cases in which curative treatment is possible. From 1994 through 1998, 105 patients with clinically localised T1-T2 tumours were operated via radical prostatectomy at Karolinska sjukhuset in Stockholm. Three patients were lost to follow-up. We obtained clinical data from patients' records and used pathologist reviews to characterise tumours regarding grade and histopathological stage. We used PSA in serum as a surrogate endpoint, with a value above 0.6 ng/ml designated as treatment failure. Outcome was examined with respect to tumour grade, histopathological stage and pre-operative PSA level. Altogether 22% of patients showed PSA failure during follow-up. No mortality due to prostate cancer was recorded. We found that both tumour grade and pre-operative serum PSA level correlated well with treatment failure (P < 0.01). Our findings illustrate the diagnostic difficulties involved, since a relatively large percentage of patients were registered for treatment failure shortly after radical prostatectomy.

The main objective of neurointensive care is to counteract the development of secondary brain ischemia. The management is focused on preventing, detecting and correcting secondary insults that are likely to produce ischemic brain damage. This requires intensive multimodality monitoring of the brain. There is no reliable technique available for continuous monitoring of cerebral blood flow and therefore intermittent methods have to be used in some cases for objective measurement of cerebral perfusion. PET allows measurements of both cerebral blood flow and metabolism, which is essential for the assessment of the energy metabolic state of the brain tissue. PET may be used to guide therapeutic intervention, to evaluate the effect of treatment, to validate new techniques for monitoring of the brain and to determine the efficacy of potential neuroprotective drugs. The possibilities that PET offers in neurointensive care and research are illustrated. The experiences from the application of PET in the evaluation of pharmacological treatment of increased intracranial pressure in head injured patients and the use of PET in combination with intracerebral microdialys, in an MCA-occlusion-reperfusion primate model and in patients with subarachnoid hemorrhage, to validate intracerebral microdialysis as an instrument for chemical monitoring of the brain during neurointensive care, are reported.

Recent years have witnessed a reduction in the frequency of infectious complications both in allogeneic and in autologous bone marrow transplantation (BMT). With a frequency of 30-40 percent, septicaemia is the predominant problem during the aplastic phase. Owing to the introduction of prophylaxis with quinolones, there has been a shift in the aetiological spectrum toward Gram-positive infections, and Gram-negative infections now account for less than 10 percent of all cases. In turn, this has been accompanied by reduced mortality. Early institution of treatment based on viraemia diagnosis has radically reduced the substantial morbidity and mortality formerly due to cytomegalovirus infection during the first three months after BMT. Antibiotic prophylactic treatment for Pneumocystis carinii infection during the first year, and against pneumococcal infections thereafter, has also been highly successful. The remaining problems are the diagnosis and treatment of invasive fungal infections and the increased susceptibility to infection among those who develop graft-versus-host disease.

Epstein-Barr virus infection (EBV) was discovered 25 years ago in tumour cells from Burkitt's lymphoma. EBV causes infectious mononucleosis and contributes to the pathogenesis of Burkitt's lymphoma and nasopharyngeal cancer. Atypical courses of the primary infection are attracting increasing attention and may present problems of diagnosis at the same time as no clear diagnostic or laboratory-based definition of the reactivation of this herpes virus is available. The article reports on the clinical course, the diagnosis, and the possibilities of treatment.
