Introduction: Compliance with the guidelines for intervention in acute malnutrition by the Local Health Directorates must guarantee comprehensive care processes. However, there are serious deficiencies reflected in comprehensive care. Objective: To evaluate compliance with the comprehensive care guide for acute malnutrition in children under five years of age in the municipalities of Córdoba. Method: Descriptive study in which Local Health Directorates of municipalities in the Department of Córdoba participated. Through interviews, observation and audit of evidence, information was taken to evaluate the percentage of compliance with the Resolution's reagents. Results: The overall level of compliance is medium-low (69%). A wide range of the reagents (87.5%) of event code 113 have moderate to low degrees of compliance, including the control and monitoring of identified cases. Conclusions: Compliance with the guidelines established in the Resolution is deficient. [ABSTRACT FROM AUTHOR]

This document exposes the current health problem in which, due to government policies, there is no adequate distribution of antiviral drugs to the population, despite having guidelines where the criteria for receiving them are established, as well as the medical units in the country that must grant them to the population. In the same way, a criticism is made of the vaccination policy in which monovalent vaccines that do not have verifiable efficacy before the omicron strains are preferred to be given to the population instead of bivalent vaccines that are fully indicated in this pandemic stage. Introduction. A high-quality and timely medical forensic examination can potentially validate and address the crime to restore victims' rights, minimize the trauma they may experience, and promote healing. Aim. To evaluate compliance with the forensic criteria of the comprehensive health care protocol for victims of sexual assault according to resolution 0459 of 2012 in four hospitals in southwestern Colombia. Methods. Retrospective descriptive study, a checklist based on Resolution 0459 of 2012 was used to evaluate the process of care for victims of sexual assault in hospital institutions of low and medium complexity. Descriptive analysis of the data was performed. Categorical variables were presented as absolute and relative frequencies, a Z score test was used with a value of p < 0.05 as statistical significance. Results. 131 cases of sexual assault treated in the emergency services of low and medium complexity institutions were documented. 87% of the cases occurred in women and 50.4% in people under 14 years of age. In most cases, the standard of comprehensive care for victims of sexual violence was not met. Conclusion. The comprehensive care process with a forensic approach in Colombia is deficient; It violates the rights of victims, transgresses the gender equity policy and resolution 0459 of 2012. These situations could be having a significant impact on the administration of justice, the restitution of victims' rights and the impunity of the crime. [ABSTRACT FROM AUTHOR]

This article describes the importance of public-private partnerships (PPPs) and public-private coordination to address antimicrobial resistance (AMR) through the One Health approach. These partnerships are developed between governmental actors, private companies, and social organizations to build agendas, decisionmaking, and the management of projects of common interest. A case study from Colombia is presented. It describes examples of response from the public sector when the World Health Organization (WHO) reported a colistin resistance alert in 2016; from the animal protein production sector under an industry partnership for the rational use of antimicrobials; and, finally, from a public-private partnership in the swine sector, with governmental institutions and international cooperation, to take action to mitigate the risk of AMR. In Colombia, the establishment of partnerships with organizations representing agricultural producers has generated impactful actions such as strengthening ongoing communication channels between the public and private sectors; characterizing the conditions of the agricultural production chain; establishing mechanisms for consultation and validation of health policies related to AMR; obtaining a baseline of indicator pathogens and identification of possible flows of AMR spread; and, finally, achieving knowledge transfer and capacitybuilding with national and international experts, with actions to raise awareness about the problem of AMR and its impact on public health. The strategic model developed in Colombia through public-private collaboration can inspire other low- and middle-income countries to optimize their use of resources to obtain results that contribute to the national AMR mitigation plan. The objective of the investigation was to obtain the level of relationship between health policies and the administration of financial resources of Hospital Belén de Trujillo by 2021, the sample consisted of 53 people, the chosen method obeys the qualitative-quantitative paradigm, I use the descriptive scalar technique organized in a set of items according to their indicators, the hypotheses were tested, calculating correlational Student-T using the SPSS (r of H1.1: 0.499, r of H1.2: 0.686, r of H1.3 : 0.576, r of H1.4: 0.527, r of H1.5: 0.546, r of H1.6: 0.633, r of H1.7: 0.434, r of H1.8: 0.577, r of H1.9: 0.619 ), with a 95% reliability cup, using the non-parametric Spearman statistic (ρ); concluding that the correlation ranges from positively weak to positively strong, a fact that leads to rejecting the hypotheses from H01.1 to H01.9; accepting the alternative hypotheses from H1.1; up to H1.9. The interpretation of the statistical parameter, consistent with the findings, is based on the behavior of the study variables that systematizes the proposal: psycho-educational courseworkshop "Work-emotional preparation" that neutralizes the abnormal political-administrative behavior derived from the policies health and the administration of financial resources. In 1962, Spain sought entry into the EEC, in this context, the Spanish health authorities promoted the National Plan for the Eradication of Tuberculosis (PNET) between 1965 and 1972, according to the massive campaign model that had triumphed in Europe, in a sociopolitical context modulated by propaganda, the need to open the regime and the search for international recognition. The main objective of this study is to analyze the approaches, strategies and final assessment of the PNET. The study shows the disputes around the right to health. It begins in 2008, year in which health was declared an autonomous fundamental right by the Constitutional Court. It ends in 2015, the moment in which the Statutory Health Law is enacted. The research was carried out through the documentary analysis of sources from the archives of Congress, the Constitutional Court, the press and academic texts. Likewise, semi-structured interviews with key actors in the conflict and categories of analysis related to the conception of the right to health were conducted and implemented. From this, advances in the jurisprudence of law were evidenced; the dichotomy of a State that guarantees health as a fundamental right and at the same time defender of the market; positions of the actors that transit between the defense of a cost-effective system and groups inclined to defend health as a fundamental right in charge of the State. The COVID-19 pandemic is a social and especially health problem whose appearance has made it possible to highlight the circulation of other epidemics affecting the population and the socioeconomic and structural crisis present in different countries. Attention is aimed at counteracting the disease through prevention measures at the community level to reduce the spread of the virus and prevent new infections. This context proposes health promotion as the basis to rethink the education processes of the population for the assessment of their health. This document reviews the evolution of the health and different approaches to health promotion; orientations that the health policies developed during the appearance of pandemics that occurred in the world through times; the challenges that countries are facing, and the social behaviors observed during the current pandemic. With this background, some lessons learned from the political, scientific, and social aspects outlined and some general considerations raised, from the scenario presented by COVID-19, on the importance of assuming health as a social right and duty and health promotion of health. Health is a process that allows individuals and communities to develop learning strategies to protect their health and act on its social determinants; Likewise, elements of the communication process directly related to health promotion to guide the lives of citizens. This paper aims to provide theoretically elements to develop health models and systems in Latin America. Its objective is to analyze the attention models and the most relevant configurations of health systems, with a special focus in Primary Health Care (PHC) based models. Methodologically, a narrative no systematic review was made in databases and Google Scholar. For this purpose, it was used the following keywords: models, configurations, paradigms, and theories/health. Papers with holistic information considered relevant and appropriate were selected. For the analysis, first, a hierarchic classification of the distinct approaches identified was made. Second, classic models defined in the literature were studied. Finally, a critical analysis about some models related to PHC in Latin America and distinct modalities of attention in the 21st century was made. With regard to the structured relationships necessary to reorient the health system, it was concluded that these must be grounded in PHC approaches, and oriented toward a "communitarian attention model" to organize services. Latin American countries have been facing an important social and demographic phenomenon known as South-South migration, which differs from the classic South-North migration pattern due to the lower social and governmental response capacity and similar social and health conditions in both populations (migrants and natives). Oral health is not excluded from this situation and must be analyzed from an integrative proposal that involves demographic, social, and economic factors and those of the migratory process. This article reviews the existing literature in different geographical contexts, identifying conceptual and methodological gaps, and proposing an analysis of migration as a social determinant with an impact on oral health inequalities and inequities. This is the starting point for presenting a conceptual framework that explains the stages and moments faced by migrant groups, identifying the factors that affect the oral health of this population. In conclusion, migration and its relationship with oral health in the Latin American context are in a scientific construction stage and it implies the need to generate research proposals based on the social reality lived by those directly involved and from different methodologies. Similarly, action strategies are required at different levels and with different actors from interdisciplinary approaches, recognizing oral health as a fundamental right linked to general health, for all citizens, regardless of their origin. The Direct-To-Consumer Genetic Tests (TGDC) provide access to genetic information without necessarily involving a physician or any other health personnel in the process.TGDC can promote public awareness of genetic origin about some diseases, as, by bringing this type of information to general population, and not only to a specific patient, society becomes more aware of them. Therefore, a healthier lifestyle, personalized medicine and promotion of self-care are also increased 8Ars Pharm. 2019; 60(supl. 1): 7-14Fernández-Rodríguez M.Nevertheless, being the principle of autonomy the main argument alluded to in favor of the unlimited access to TGDC, it is not possible to affirm that the consumer acquiring them, is truly autonomous. The companies commercializing them do not currently have a quality track on the predictive value of their tests. Therefore, the precautionary principle should also operate, by preventing damage to whoever is exposed, without having the necessary information.The non-regulation of TGDC may pose a risk to the consumer for two reasons: firstly, the absence of pre and post test genetic counseling by qualified personnel in accredited centers - as established by the Biomedical Research Law 14 / 2007-, and secondly, the absence of a correct interpretation of the results by a professional can pose a medical and psychological risk to the user If a consumer decides to acquire one of these genetic tests online, the test is directly sent to home, and he/she will receive the results of the test and deciding what to do with them. These new possibilities for medical-health information are associated to these new ethical-legal approaches to which it is necessary paying attention in Spain.The community pharmacy, as a health center and organized structure, next to general population and involved in education, professional advice, and participation in health programs, become the perfect allies in matters of possible health regulation of the TGDC. The protocols and screening studies carried out in Spain make the current community pharmacy an advantageous option for the regulation of TGDC, integrating with the pharmacy as a private health facility of public interest, within the National Health System. Congenital heart diseases are considered the most frequent malformations. In Mexico every year between 12 and 16 thousand children are born with some type of congenital heart disease and it is the second cause of mortality in children under 1 year of age and in children aged between 1 and 4 years. The problem of the care of congenital heart diseases is analyzed from a perspective of health policies in the country with an emphasis on regionalizing their care in order to rationalize the use of available resources and with the aim of serving the largest number of patients and achieve the best clinical results. It emphasizes the need to promote cooperation among the different civil society organizations and institutions to exchange ideas to create consensus and joint projects for a better use of available resources to achieve universal and quality care of children with congenital heart disease in all the country. - Abstract
According to ENCOVI (2018) and the Bengoa Foundation (2019), our country is going through a complex humanitarian crisis, characterized by a situation of multidimensional poverty, which is 48%. In this context, the School is the ideal place and one of greater responsibility for the achievement of a comprehensive health in children and allows the consolidation of habits that improve their quality of life, present and future. The research was based on a study by survey, during 2018, to teachers, in 24 Basic Education institutions: public (70.6%), private-subsidized (29.4%), medium-low socioeconomic stratum, located in the Miranda-Venezuela state. The information was collected through three questionnaires with emphasis on the frequency and Likert scale: (a) nutritional evaluation, (b) the teacher as nutritional guardian, and (c) the school as a health promoter. Based on the indicators proposed by De Tejada and Col (2013) and WHO (2006). The internal consistency indices were 0.880; 0.794 and 0.842, respectively (SPSS, version 21). The results indicate: (a) teachers perceive nutritional deficiencies, not generalized, with affection of the physical condition in general, emotional and psychomotor health in children; (b) little presence (< 50%) of programs promoting integral health in schools; (e) The increase in diseases is observed in a worrying way: diarrhea (29% ↑), and influenza (36% ↑), caries (46% ↑), pediculosis (lice) (64% ↑), stress, aggression and irritability (27%↑) and malnutrition (55%↑). This work explores the management models for hospitals. It is an exploratory research with a documentary design applied in the review of the different management models throughout the history and including an in-depth inquiry on the new public management model by the Latin-American Development Center (CLAD). It is concluded that in the State-owned hospitals this model can be successfully implemented whenever the individuals in the top management become professionals. This effort should be taken as a strategic measure by the State in addition to the support and mobilization towards a managerial reform process. At the end of the 90s in Peru, after determining the resistance to antimalarial drugs, a change in antimalarial treatment schemes was decided; this change included the combined therapy for P. falciparum, mefloquine/artesunate in the Amazon region, and sulfadoxine pyrimethamine/artesunate in the North coast. After two decades, and aimed at assessing the impact of these schemes on the malaria endemic, a review was conducted of malaria reports in three departments accounting for more than 70% of cases reported in the country. The major impact of the sulfadoxine-pyrimethamine/ artesunate scheme in the North coast was evident since it reduced the number of cases of P. falciparum to virtually zero four years after implementation of the combined therapy. The single dose and the ability to limit the development of sporozoites were crucial in order to achieve this goal. The mefloquine/artesunate scheme had the limitation of ensuring supervised treatment in the health service facilities and the need for three doses. It is important to select an effective and easy-to-administer scheme when choosing the first line of treatment for malaria. This experience is significant for the malaria eradication goals in Peru. This article analyses the history of the emergence of generic medications in Colombia during de 1960's. It exposes that this emergence was the result of a merging between an industrial local sector, an industrial group from the United States and a pharmaceutical policy of the State. From the analysis of documents retrieved from the Ministry of Health, from the national and foreign press and some interviews with stakeholders, it is clear that the Ministry of Health and some of its workers stablished a pioneering policy within the continent. This policy strengthened the quality control of generic medications and it supported its diffusion. The Colombian State acted upon as an actor capable to regulate the harmonization between the benefits for the industry and the society seeking access to health. A first step taken by the Government and the industry toward the pharmaceuticalization of the society was the strategy of generics' consumption. [ABSTRACT FROM AUTHOR]

Health care promotion in the university context materializes when a healthy lifestyle of the educational community is evident in its environment. The purpose of this article is to share the experience of Universidad de Antioquia as a health care promoting university. This process started in the University Wellbeing area, by means of the management of three services: 1) Promotion of Sexual Health and Affectivity; 2) Health Self-care; and 3) Psycho-physical health program. In 2005, the Universidad Saludable proposal was formalized by means of the Ruling of the Dean 20535 of 2005. In 2010 the concept of a health care promoting university was introduced alongside its three axes: research, cross-sectional curriculum, and student participation. These axes materialized in three projects: the first, Safe and Healthy Faculty, headed by the Public Health National Faculty, allowed the creation of strategies for a healthy work environment and cross-sectional health promotion in academic and research curricula; the second project, Wellbeing Promoters, had student participants working with their peers on health care promotion; and the third project, Smoke-free University, had the Medicine and Dentistry faculties to be declared as smoke-free environments. Finally, it is concluded that Universidad de Antioquia, as a health care promoting university, materialized by means of three projects. The willingness of the institutional policies and the actors involved is a key success factor to guarantee its continuity. [ABSTRACT FROM AUTHOR]

Objective: describing the work conditions of the health personnel associated with public health programs. Methodology: we carried out a descriptive cross-section study on a representative sample consisting of 672 people associated with health institutions approved by the Ministry of Health in 2013, which provide public health programs and services. Results: findings suggest most of the personnel associated with public health services and programs had medicine, nursing, and dentistry training; had problematic working conditions - as short-term contracts are prevailing - and the public health training coverage was low. The most common form of adequacy in public health of the people surveyed was associated with approving the knowledge test necessary to become associated with the institution and with the performance assessments. Conclusions: it is possible to see limitations on training and education, as well as deficient working conditions for people who work in public health programs. [ABSTRACT FROM AUTHOR]

Objective: Describing the contextual elements involved in the development of sexual and re- productive health public policies of the municipalities of the metropolitan area. Methodology: We performed a systematic review of the public policies, scientific journals, and interviews to experts on sexual and reproductive health topics. The analysis of the information was performed through a codification and categorization process. Results: The findings show that srh is a topic being discussed in different social groups and brings about public opinion. It has undergone social and cultural changes in approach, there is recognition of the actors and the relationship between public policies and the media as elements of transformation of citizens. The national policies of srh are a pattern to follow for municipal entities, but their guidelines must adapt to the particular necessities of each municipality as to improve the rendering of the services according to the context of the inhabitants. Context: The access to health services is an increasingly worrying topic in the research and public policies agendas. In the case of Colombia, this topic becomes more important and attains supremacy opposite to assurance. Objective: Building a concept of access and identifying barriers, as well as strategies and alternatives to overcome them from the perspective of the actors of the health system in Medellin. Materials and Methods: qualitative research, focused on three areas of Medellin. We used techniques such as semi-structured interviews; social cartography workshops; observation and result questionnaires of the services. Results: The vision on the access is heterogeneous between actors, although some similarities and differences were identified, that allowed us to build a proposal of the concept of access. The barriers and factors that facilitate access can be attributed to factors on the offer side as well as on the demand side. Most of the barriers happen during real access, during the search and continuity of the treatment. The social characteristics of the population, such as knowledge of the system and social and community support are also determinant factors of access. Discussion and Conclusions: All actors use strategies to solve the access barriers and are affected by them on different levels. The qualitative analysis of the concept of access, of barriers and facilities, allow the identification of alternatives that may serve as components for health policies. [ABSTRACT FROM AUTHOR]

In different countries medical error is considered one of the first lawsuits causes against health care providers, which makes it necessary to pay attention in identifying the reasons of its increase, for health is a law protected value. There are several causes for medical error; it arises when the doctor performs with negligence, unskilfulness or imprudence. If any procedure or medical care does not adhere to what is established in the normative, the doctor will have to respond legally, that is to say, s/he will have to face legal liability, be it civil, criminal and/or administrative. The possible sanctions may be cautionary warning, notice of termination, disqualification, fines, jail and/or damage repair. Public and private institutions, healthcare staff, judges and legislators must all be committed with the system. They should also act with the will to guarantee safety and control in the healthcare services, providing ongoing training to the professionals in the area and adequate work conditions, emphasising the importance of communication with the patients and their family, and establishing security policies to comply with quality the provision of health services. An intense process of state development, which generated social intervention and new capacities to produce public policies, took place since the forties in Argentina. This phenomenon was accompanied by trends towards an increasing centralization of the political power in the Executive branch of the government. Within this context, this paper attempts to explore these processes from a perspective based on a provincial jurisdiction, constructing an account that locates the history of this political entity within the more comprehensive story of the structuring of the Interventionist State. In order to achieve this purpose, the history of one provincial health department and its relationships with the Federal State organizations are analyzed, trying to unravel the reactions that the provincial state elaborated to the process of centralization of social policies, which accompanied the emergence of Peronism. Objectives: To describe the supply and demand of clinical fields for undergraduate students of Peru. Materials and methods. A descriptive study was considering as supply of clinical fields the total number of existing hospital beds in Peru. The demand was calculated using the total number of alumni registered in health science careers following the clinical years or the internship. We calculated the number of beds per student and the coverage of clinical fields nationally and in some selected regions (Lima, Arequipa, La Libertad and Lambayeque). Results. In 2009, Peru had 34,539 hospital beds, 78.5% of which pertained to the public sector and 48.4% are from Lima. We estimated that in 2008 44,032 alumni needed clinical fields, 70% from private universities, which grew 65% since 2005. The coverage of clinical fields, considering only interns from four careers (medicine, nursery, obstetrics and dentistry) was only 31.5% at the national level. The number of beds per student oscillated between 0.5 in La Libertad to 0.82 in Lima with a national mean of 0.45. Conclusions. The supply of clinical fields for teaching undergraduates is insufficient to satisfy the demand, which continues to grow because of private universities, and hence requires urgent regulation.
