INTRODUCTION: Ralstonia mannitolilytica is a causative agent of hospital-acquired infections. The species of the Ralstonia genus are difficult to identify and differentiate. Our objective was to identify and typify R. mannitolilytica isolates obtained from blood cultures of pediatric patients. METHODS: We collected 10 isolates that were identified with an automated system, a Matrix-assisted laser desorption/ionization time of flight mass spectrometry (MALDITOF-MS) system and 16S rRNA gene analysis. The resistance profile was determined and the carbapenemases were typed through PCR and sequencing. The clonal relationship of the isolates was established by Pulsed-field gel electrophoresis (PFGE). RESULTS: The MALDI-TOF-MS system and 16S rRNA gene analysis identified the isolates as R. mannitolilytica; all belonged to a single clone and were resistant to meropenem. The carbapenemases OXA-443 and OXA-444 were detected in all strains. CONCLUSIONS: This is the first report of R. mannitolilytica isolates with clonal origin and harboring the carbapenemases OXA-443 and OXA-444 in Mexican pediatric patients. [ABSTRACT FROM AUTHOR]
