Objectives: To determine the predictive performance of fetal growth restriction by Maternal Fetal Medicine Society (MFMS) definition of ultrasound, the Delphi consensus (DC) and the Barcelona Fetal Medicine (BFM) criteria for adverse perinatal outcomes, and to identify whether there is an association between the diagnosis of fetal growth restriction (FGR) and adverse perinatal outcomes. Material and methods: A retrospective cohort study was conducted including women with singleton pregnancies between 24 and 36 weeks of gestation seen at the maternal fetal medicine unit for ultrasound assessment of fetal growth and delivery care in a public referral hospital in Popayán, Colombia. Pregnancies with ultrasound findings of congenital abnormalities were excluded. Convenience sampling was used. Sociodemographic and clinical variables were measured on admission; additional variables were gestational age, FGR diagnosis and adverse composite perinatal outcome. The predictive ability of three fetal growth restriction diagnostic criteria for poor perinatal outcomes was analyzed and asociation between FGR and adverse perinatlal outcomes. Results: Overall, 228 pregnant women with a mean age of 26.8 years were included; FGR prevalence according to the three criteria was 3.95 %, 16.6 % and 21.9 % for DC, BFM and MFMS, respectively. None of the criteria resulted in an acceptable area under the curve for the prediction of the composite adverse neonatal outcome; FGR diagnosis by DC and MFMS were associated with adverse perinatal outcomes with a RR of 2.6 (95 % CI: 1.5-4.3) and 1.57 (95 % CI: 1.01-2.44) respectively. No association was found for BFM RR: 1.32 (95 % CI: 0.8-2.1). Conclusions: Given a positive result for FGR, the Delphi method is significantly associated with adverse perinatal outcomes. The proportion of false negative results for a poor perinatal outcome is high for the three methods. Prospective studies that reduce measurement and attrition bias are required. Introduction. Extreme neonatal morbidity refers to any event that occurs in the first twenty-eight days of life with imminent vital risk. It is multifactorial and delays in treatment are relevant. It was described for the first time as a public health indicator in Brazil in 2010. Methodology. This was a cross-sectional study in a high complexity hospital in 2013. Sociodemographic and assistance characteristics, morbidity events and their outcomes, the indicator itself, neonatal mortality, and delays in treatment (types I - IV) were reviewed. Results. 1,190 neonates were entered, 120 cases occurred and the first 60 were analyzed due to data saturation. The extreme neonatal morbidity rate was 59 for every 1,000 children born alive and the mortality rate was 13.9. This was similar to the figures in Brazil and greater than national statistics (33 for every 1,000). The most significant events with respect to mortality were weight lower than 1,500 grams, mechanical ventilation, an Apgar score lower than 7 at 5 minutes and congenital anomalies. Delays from mothers were predominantly type I (45.4%) due to poor prenatal control. The most frequent neonatal delay was type IV (40%) due to bad adherence to institutional protocols. Discussion. The local indicator was greater than the national indicator due to the institutional complexity and delays in treatment, which cause statistical saturation. This can be improved with better adherence to existing strategies, such as prenatal control and institutional protocols. Conclusion. Perinatal death from an institutional view is a matter of difficult confrontation for both, parents who experience the loss, as for the health personnel present at the event. Kristen Swanson theory considers that the goal of nursing is to provide care, for women and her family, significantly to help in the acceptance of the death of a loved one. The support health personnel provide for parents and their family, is significant in the acceptance of the loss and the development of the grieving process. This article tells the experience of a nurse in her interaction with a woman who faced the death of his daughter at the time of birth. - Abstract
Objective: research article from a qualitative perspective, whose purpose is to deepen the impact that perinatal death generates in a couple of parents, and the professional assistance provided. The identified conceptual elements were enriched by an in-depth interview with a young couple who work in the social area, from which the researchers generate reflections of second order as contributions to the construction of knowledge. Materials and methods: the findings of the documentary review, scarce in relation to the subject, were contrasted with an in-depth interview with the parents of a deceased perinatal baby. After that, the researchers contrasted the documentary, testimonial components and generated reflections from a multidimensional and interdisciplinary perspective. Results: in the handling of the case there is little sensitivity on the part of health professionals, inadequate treatment and absence of accompaniment in the situation of grief. Although conceptually there is mobilization in front of the topic by health professionals, especially nurses and doctors, there are considerable gaps that do not encourage accompaniment; the image offered is of little sensitivity to the families that experience the loss. Conclusions: faults in health care are identified when dealing with this human phenomenon that involves the entire family and health context, which demonstrates the need to implement differentiated protocols that respond appropriately to the needs of these families. - Abstract
Background: Caring for and supporting parents whose infant has died is extremely demanding, difficult and stressful. In some situations nurses may experience personal failure, feel helpless, and need to distance themselves from bereaved parents because they feel unable to deal with the enormity of the parental feelings of loss. Interventions to address bereavement after neonatal death are increasingly common in Spain hospitals. However, more research is needed to assess the encounter between parents and health professionals during hospitalization after a fetal loss. Aim: The purpose of this research was to explore the experiences of midwives and nurses to care for families experiencing perinatal death. Method: Literature search in the following databases: CINAHL, Medline, Index, Crochane, Google Académico and PubMed (2005-2015). Inclusion criteria were: sample of nurses / midwives who have faced a fetal death or women with perinatal loss to explain their experience with nursing staff. Results: We included 7 articles of 68 reviewed. The included studies are written in English. In these studies, nurses and midwives emphasized their need for increased knowledge and training on how to cope with bereaved parents and requested greater support from team members and the hospital. Conclusion: Training programs in obstetrics need to better address the bereavement needs of parents following a stillbirth, and research is needed to evaluate effective bereavement interventions. A bibliographical review focused on the maternal mourning due to perinatal death is performed, in order to understand a devastating event for parents and particularly for the mother, for the loss of the child. It also describes the emotional adaptive changes within women during and after pregnancy and frustrations of motherhood, which involves the loss. The mourning process must be understood through the various stages that characterize it, and that can manifest in a linear manner or behave as a dynamic process changing from one phase to another. In these circumstances, a health professional's approach is important and should be based on a relationship of empathy with the grieving mother, which encloses ethical, philosophical and sociological connotations that should be taken into consideration in any management of health. For mothers in this situation, it is useful to have self-help or support groups in order to overcome this tragic experience and in turn, allow them to interact and be supported by other mothers who have had the same experience.
