Introduction: the more frequent diabetes's micro vascular complications involve mainly to the retina and the kidney. A narrow relationship has been recognized between diabetic retinopathy and micro albuminuria. Objective: to determine the behavior of the micro albuminuria in patient with diabetes type 2 and diabetic retinopathy. Methods: an observational, descriptive, cross-sectional study in patients with diabetes type 2 and diabetic retinopathy at the Arnaldo Milián Castro Hospital in the period between 2018 and 2019. Results: the women bigger than 60 years old were more affected with diabetic retinopathy (43, 2%). The proliferative retinopathy diabetic predominated (47, 7%) in patients with more than 10 years of evolution. The 38, 6% of patients had high micro albuminuria. The 32, 4% of patients with high micro albuminuria had a proliferative diabetic retinopathy and bad metabolic control. The number of patient with proliferative diabetic retinopathy, arterial hypertension associate and high micro albuminuria was smaller than we hoped to find. Conclusions: a little more than a 1/3 of the patients with diabetes and some grade of severity of diabetic retinopathy present high micro albuminuria. More than half of the patients with high micro albuminuria have a bad metabolic control, of these the majority with proliferative diabetic retinopathy. Recent longitudinal studies have revealed a cumulative incidence of proteinuria between 30 and 50% in Type 2 (non-insulin-dependent) diabetic patients of different ethnic origins. Introduction: diabetic nephropathy is an important complication of diabetes mellitus, and because of its repercussion, it is a must to investigate it in the initial stages. Objective: to determine the impact of an educational intervention on the metabolic control of diabetic patients and on the progression of kidney damage. Method: an intervention study was conducted in a medical office from January 2017 to January 2018. The sample consisted in 21 diabetic patients. The variables were: age, sex, type and time of diabetes mellitus, degree of metabolic control, associated vascular risk factors and microalbuminuria. The study was divided into stages. An initial diagnosis was made, and on this basis, an educational intervention strategy was designed and carried out, later the variables were evaluated. Results: female patients and diabetes mellitus type 2 predominated. A 42.9% of patients had more than 10 years of evolution of the disease. Before the intervention, 71.2% of patients had poor metabolic control of their disease. The vascular risk factors that were most frequently associated with a poor metabolic control were hypertension (87.5%), dyslipidemia (31.3%) and ischemic heart disease (25%). Before the intervention, 38% of patients presented positive microalbuminuria. After the intervention, there was improvement in metabolic control and very significant and favorable changes in microalbuminuria values. Conclusions: the educational intervention was effective; it was shown by the favorable changes in the metabolic control and the markers of kidney damage in the patients of the study. BACKGROUND: There are precedents that the strict control of the blood pressure figures can have a favorable impact avoiding the development of microalbuminuria, and therefore preventing the appearance of nephropathy, as well as significantly reducing fatal outcomes due to cardiovascular and cerebrovascular disease. OBJECTIVE: To show that strict control of tension figures can impact decreasing the microalbuminuria. MATERIAL AND METHOD: An observational and descriptive study was done from January to December 2017 with patients with an established diagnosis of type 2 diabetes mellitus and/or systemic hypertension, with random selection. These patients were incorporated into a Self-Commissioning Protocol to loan blood pressure. Patients with chronic kidney disease were not included. RESULTS: There were included 200 patients. The correlation between multiple variables and the presence of microalbuminuria was measured finding a statistically significant correlation when segmenting patients according to the ACC/AHA 2017 classification and after adjusting the antihypertensive dose with p = 0.00001. CONCLUSIONS: Treatment should be adjusted based on the patient's blood pressure, regardless of whether the treatment is with IECAS or ARA II. [ABSTRACT FROM AUTHOR]

An observational, descriptive and serial cases study, to determine the presence of microalbuminuria in 80 patients with type 2 diabetes mellitus, classified in the doctor's office No. 42 of "José Martí Pérez" Teaching Polyclinic was carried out in Santiago de Cuba, from October, 2016 to the same month in 2017. In the series, the 60-64 age group was the one with higher epidemiological connotation, related to the microalbuminuria of high range (86.7%). The course time of the diabetes was closely linked with the detection of high microalbuminuria; also, the different stages of the chronic renal disease bore no relation with the course time of the illness, that is to say, the stage 1 was in correspondence with the beginning of the diabetes and the stages 4 and 5 with the patients with more than 10 years of clinical course. In diabetes mellitus (type 1 and type 2) chronic macrovascular and microangiopathic complications can occur; leading to diabetic nephropathy, the most frequent cause of renal failure. Microalbuminuria is the first clinical characteristic, which is at least in part, of tubular origin. Basic and clinical studies suggest that tubular dysfunction has a significant role in the development of diabetic nephropathy. The tubular dysfunction is characterized by alteration of renin angiotensin system (RAS), production of AGE (advanced glycosylation products), development of inflammation and increase of circulating adiponectin levels. Furthermore, the involvement of the RAS in tubular damage, suggests that drugs such as inhibitors of angiotensin converting enzyme (ACE) inhibitors and AT1 receptor antagonists (angiotensin ARA II) have a protective effect in patients with diabetic nephropathy. Objectives: To establish the prevalence of microalbuminuria in a group of patients with systemic arterial hypertension (SAH) to analyze the association between this parameter and cardiovascular risk factors as well as with SAH treatment. Methods: This is a sub-analysis of 564 patients of Mexico, extracted from an international, observational, and cross-sectional study followed by specialists, The study included patients with SAH without any other causes of microalbuminuria. Results: Microalbuminuria in these patients had a prevalence of 63.8% (95% IC 58.4, 69,3) and correlated with a wide variety of risk factors and concomitant cardiovascular diseases Most patients with microalbuminuria already received treatment with angiotensin II receptor antagonists (50%), without pretending to establish the impact of the drugs on the microalbuminuria values. Conclusion: The prevalence of patients with SAH and high cardiovascular risk is high in this study and justifies their management and care with multifactorial strategies aimed to adequately control their blood pressure and to modify other current cardiovascular risk factors. Background: The detection of microalbuminuria is the earliest clinical sign of renal damage in patients with diabetes. The hic.org.mx 24-hour urine is the gold standard for measurement of albuminuria, it is accepted that screening can be used a spot urine sample, it is better to relate the concentration of albuminuria with creatinuria. Objective: T o determine the prevalence of incipient renal damage through the albumin/creatinine in patients attending the Clinic in Guanajuato ISSSTE Hospital, diagnosed with diabetes and/or hypertension. Methods: We conducted a longitudinal descriptive study, using the instrument Clinitek Status and Clinitek Microalbumin test strips for casual urine sample. The results of the albumin/creatinine were associated with the diagnosis of patients with creatinine clearance. Results: Patients with diabetes: normoalbuminuria = 55%, microalbuminuria = 32% and macroalbuminuria = 13%. Patients with hypertension: normoalbuminuria = 88%, microalbuminuria = 6% and macroalbuminuria 6%. Diabetic patients with hypertension normoalbuminuria = 38%, microalbuminuria = 33% and macroalbuminuria = 29%. Pregnancy is a physiological event with important influence in several organs, particularly the kidneys. The objective of this study was to correlate values of microalbuminuria in 70 apparently healthy pregnant patients at the third gestation trimester with the corresponding urine assay. Microalbuminuria (mean ± standard deviation) assayed by turbidimetry was 15,7 ± 12,8 mg/L, being lightly above the reference limit; on the other hand, 45,7% of patients presented a microalbuminuria above 15 mg/L, revealing significant differences (p < 0,05) between this group and patients in the group with normal albumin excretion. In the group with above normal microalbuminuria, 75,7% of them showed alterations in their urine analysis, particularly related to urinary infection. There were not significant differences in the microalbuminuria found among the patients with or without alteration of corresponding urinary sediment analysis. Introduction: Few data exist of the presence of microalbuminuria as a prognosticating factor for organ failure and death in acute pancreatitis. Objective: Determine the possible association of microalbuminuria as a risk factor for organ failure and death in acute pancreatitis. Material and Methods: We prospectively studied all adult patients with acute pancreatitis admitted in our Hospital in a one-year period. The factors studied were age, gender, etiology, time of onset of acute pancreatitis, Ranson's criteria, organ failure (Atlanta criteria) and death. Microalbuminuria was measured upon admission and sequentially at 24, 48, 72 and 96 hrs, using Clinitek® Microalbumin strip (Clinitek® 50 Analyzer). Statistical analysis: Continuous variables are expressed as mean, range, and standard deviation. The comparisons between the complicated and non-complicated groups were done with Chi-Square and Fisher exact tests, as well as a t Student, according to which one was more appropriate. ROC curves to detect microalbuminuria with the most predictive value for organ failure and death were done. Results: One hundred patients were included, 71 were female, the rest males and the mean age was 42 ± 19 years old (16 - 96). Etiology was biliar in 76 patients, alcohol related in 12 patients and other etiologies in the 11 patients. Organ failure was diagnosed in 13 patients, local complications in 4 patients and 5 patients died. Microalbuminuria levels in patients with > 3 versus < 3 Ranson 's criteria was (69 vs. 37 mg /L, p<0.001) and in patients with and without organ failure (98 mg/L vs. 38 mg/L. p <0.001) respectively. The microalbuminuria at 24 hrs ≥ 80 mg/l was found to be a risk factor for organ failure (OR 9, IC 95% 2.5-32, p=0.0023) with a sensitivity of 54%, specificity of 89%, positive predictive value of 41%, negative predictive value of 93% and accuracy of 84%, but did not reach statistical significance for mortality. Conclusion: The presence of <80 mg/l of microalbuminuria at 24 hrs showed to be a risk factor for organ failure but no for mortality in patients with acute pancreatitis. [ABSTRACT FROM AUTHOR]

Hypertensive retinopathy is related to the time of exposure of the arteries to increased blood pressure, and its pattern is useful to estimate hypertensive disease evolution. The retinal arterial changes start with an initial acute vasoconstrictor phase, followed by a chronic sclerotic stage. Clinical studies have demonstrated a relationship between hypertension and left ventricle diastolic dysfunction. On the other hand, urinary albumin excretion is linked to subclinical organs damage, such as left ventricle hypertrophy and thickening of carotid artery wall; microalbuminuria is considered as a marker of morbidity and mortality in hypertensive patients. This study included 45 male hypertensive patients (blood pressure > 140/90 mm Hg, systolic/diastolic), age range 18-65 years old. Eye funduscopy and retinography was found normal in 19 (42.2%) patients hypertensive retinopathy was found in 2 (57.8%) of patients. Stage I retinopathy was found in 1 (35.5%)patients; Stage II in 9 (20.0 %) patients and Stage III in 1 (2.2%) patient. No patients showed Stage IV retinopathy. Left ventricle (LV) mass was found normal in 25 (55.6%) patients and left ventricle hypertrophy in 20 (44.4 %) of them. LV diastolic dysfunction (DD) was found in 21 (46.6%) patients; Type I DD was observed in 20 (44.4%) patients and Type II DD in only one (2.2%) case. No patients showed Type III DD. Microalbuminuria was found in 12 (26.7%) patients. When hypertensive retinopathy was matched to microalbuminuria, 19 (73.07 %) of patients did not have microalbuminuria and only 7 (26.9%) patients presented this disorder (Cramer coefficient of association= 0.007, p= 0.96). Background: Microalbuminuria is considered a marker of endothelial dysfunction, as well as generalized vasculopathy, which can increase the atherogenic states. Objective: To determinate if value of microalbuminuria is associated with severity of the acute coronary syndrome. Material and methods: An analytical, cross-sectional study was made in the Internal Medicine Service, in Hospital General de Ticomán, that included a total of 40 patients with acute coronary syndrome, 20 with unstable angina and 20 with acute myocardial infarction. Patients had ischemic symptoms and ST elevation in at least two leads or new left bundle-branch block. A urine sample was collected, at any time of the seven later days to acute coronary syndrome, and microalbuminuria levels were measured by dipstick. The statistical analyses were run on the software package SPSS v13 and included Student unpaired t test. Results: An albuminuria of 45 ± 38 mg/L was present in subjects with unstable angina compared with 147 ± 131 mg/L of the acute myocardial infarction group (p = 0.002). Conclusions: The microalbuminuria level is associated with the severity of acute coronary syndrome, observing more elevated values in acute myocardial infarction that those observed in unstable angina.
