Anterior Cruciate Ligament (ACL) rupture is one of the most common injuries in the current context, and reconstruction by autograft is the most used rehabilitation method along with physiotherapy for physical rehabilitation; However, certain biomechanical alterations have been detected in the gait pattern of these patients due to the effects that the injury produces on the surrounding structures and this has resulted in the development of degenerative pathologies secondary to the reconstruction of the ligament. The objective of the present investigation is to analyze the alterations in gait biomechanics present in patients with anterior cruciate ligament reconstruction through different autograft techniques. A systematic review was carried out based on the PRISMA method guidelines in the Google Scholar, PubMed, Scopus and Springerlink databases, including a total of 11 articles. Several gait alterations were detected in the patients analyzed in parameters such as angles of movement, joint range, tibial translation, moments of force and vertical ground reaction; being the knee flexion angle the most evaluated. However, the studies show discrepancies in several of the results found and, in addition, there is little or no information on the biomechanics of the hip and ankle; Therefore, it is difficult to reach a conclusion about a specific gait pattern for these patients. The objective of this field work was to describe and analyze the verbal performance with ineffective motor behaviors or motor patterns in a group of children attending schools of pedagogical risk, in the southern zone of the province of Buenos Aires with the subsequent purpose of cognitive stimulation and strengthening programs. motor. The design was descriptive-correlational and cross-sectional. The selection of the sample was intentional, not probabilistic; it was composed of 507 children between 4 and 7 years of age from four public schools in the Province of Buenos Aires, Argentina, from the Initial and Primary levels. The intellectual level, the types of gait and the achievements obtained according to age and sex were studied. The results showed that the degree of difficulty in the jumping and balance tests exceeded 50%, being 79% at the age of 7 years. Existing also at this age, primacy of motor clumsiness. In general, the intellectual level detected was borderline normal and slow, and few expected linguistic achievements for age. In summary, there would be a relationship between motor clumsiness, motor development and intellectual level. The article presents the design of a new model of functional transtibial prosthesis that simulates the anatomical behavior of the tibioperoneo-astragalus joint. The design aims to improve gait biomechanics and reduce impact on the stump and gait alterations. The design of a new transtibial prosthesis is proposed that meets the requirements of human body functioning and improves body biomechanics. The model includes a spring system that allows for energy displacement during gait and has hydraulic, pneumatic, and electric actuators. Introduction: conventional therapy has been shown to be effective in treating stroke patients, but its techniques are sometimes monotonous, reducing patient adherence. In recent years, technologies have been developed that provide these patients with key elements of an adequate neurorehabilitation program, such as robotic systems for gait reeducation. The aim of the present work is to compare the effects of robotic therapy and conventional therapy in gait reeducation in stroke patients. Material and method: a search was carried out in the Pubmed and Medline databases using the MeSH term “Robotics” and the keywords “physical therapy modalities”, “stroke” and “gait”. Randomized clinical trial, published in the last five years in Spanish, English, French, Italian or Portuguese were included whose sample consisted of stroke patients and whose intervention made a comparison between robotic therapy and conventional therapy. Results: the search strategy yielded a total of 39 results of which a total of 8 randomized clinical trials were included in this review. The devices analyzed are the Lokomat, used in 4 of the studies. Devices such as Gait Exercise Assist Robot, Stride Management Assist, i-Walker, and Robowalk have been used in the remaining studies. Conclusion: the application of robotic therapy can be a useful support in the rehabilitation of people with stroke for the recovery of functionality while walking, but no real and superior efficacy compared to conventional therapy has been demonstrated. Introduction: it is estimated that 2 out of every 1000 live births suffer from cerebral palsy worldwide. Many of these people will not be able to develop a gait pattern, which is why robotics in neurorehabilitation is of great importance, in this case the Lokomat, which will facilitate a gait pattern more suited to the characteristics of the user. This repeated practice provides access to motor learning that together with neuroplasticity will be responsible for generating changes in these people. The objective of the case study is to analyze the difference in joint ranges prepost intervention with Lokomat in patients with cerebral palsy. Likewise, to assess whether there are changes during the gait of these patients. Material and method: the intervention consisted of a total of 17 training sessions with the Lokomat lasting 35 minutes each. The main variables studied have been the joint ranges of both hips and knees in all phases of gait. Results: positive results have been obtained in joint ranges, in the gait cycle and in spasticity, although they have not been statistically significant. 15. Rhythmic auditory stimulation promotes gait recovery in Parkinson's patients: A systematic review and meta-analysis.

Objective: Using rhythmic auditory stimulation (RAS) to improve gait disturbance in Parkinson's disease (PD) is an available treatment option, yet a consensus on its effectiveness remains controversial. We summarized the effects of RAS on gait, functional activity and quality of life in PD patients through a systematic review and meta-analysis. Methods: PubMed, Embase, Web of Science, Medline, and Cochrane Library databases were initially searched to identify relevant literature up to August 2021. Next, the methodological quality of eligible comparative studies was assessed by the Physiotherapy Evidence Database Scale. The treatment effects to clinical outcome in relation to gait, motor activities, and quality of life were analyzed. Results: A total of 18 studies consisted of 774 subjects were included in this meta-analysis. Comparing with the control group, RAS had significantly increased stride length (p < 0.001), accelerated gait speed (p < 0.001), reduced the occurrence of freezing events during walking (P = 0.009), achieved an improvement in Unified Parkinson's Disease Rating Scale (UPDRS) II (P = 0.030), UPDRS-III (P < 0.001) and Parkinson's Disease Quality of Life Questionnaire (PDQL) (p = 0.009) scores over an interval of 1-26 months. Conclusion: In this meta-analysis of 18 randomized controlled trials, we have demonstrated that RAS improves the general motor functions (UPDRS-III), particularly in gait, mobility and quality of life, in patients with Parkinson's disease. The human gait is a series of movements of the lower extremities and trunk to move the body forward and can be affected using prostheses and orthoses. A hands-free crutch was built as an alternative to conventional crutches, which presents problems in some parts of its design and requires a gait analysis, which is carried out using the Kinovea software. The current study compared three situations: normal gait, using the orthosis, and accompanying the device with a Canadian cane. It is noted that there are abnormalities when using the orthosis compared to normal walking which is slightly corrected with the auxiliary support of the cane. It is concluded that the orthosis is deficient in its walking cycle, so it is proposed to make structural changes. Developing this hands-free crutch will improve the quality of life for patients with lower limb injuries. [ABSTRACT FROM AUTHOR]

Introducción: La esclerosis múltiple (EM) es la causa de discapacidad neurológica no traumática más frecuente en población joven. Su tratamiento rehabilitador se centra en mejorar los síntomas y restablecer la funcionalidad. Uno de los métodos de rehabilitación estudiados recientemente es el trabajo de la fuerza muscular, simultáneo con ejercicios de equilibrio, mejora de la marcha y coordinación. El objetivo de esta revisión es analizar la aplicación de dichos programas de entrenamiento en sujetos con EM. Métodos: En febrero de 2020 se consultaron las bases de datos CINAHL y Medline, utilizando los descriptores MeSH "multiple sclerosis" y "resistance training". La búsqueda arrojó 89 resultados, de los cuales se analizaron 17 estudios que se adaptaban al objetivo de revisión. Resultados: Se analizaron los efectos de programas de entrenamiento aislado de la fuerza mediante protocolos de alta intensidad aeróbicos continuos o interválicos, con incrementos de la carga de trabajo o resistencia progresiva, así como entrenamientos de suspensión (TRX), resistencias con el peso corporal, hatha yoga, y su combinación con ejercicio cardiovascular, actividad física autoguiada, electroestimulación neuromuscular o entrenamientos funcionales de funcionalidad y marcha. Se analizaron las variables de fuerza y función neuromuscular (espasticidad, propiocepción), funcionalidad (movilidad, capacidad motora, equilibrio, fatiga y fatigabilidad), parámetros metabólicos (tolerancia a la glucosa, factor neurotrófico del cerebro (BDNF), esfingosina-1-fosfato (S1P), composición corporal, cortisol y DHEA, mediadores inflamatorios, marcadores inmunomoduladores, capacidad aeróbica), y parámetros como calidad de vida, satisfacción, adherencia y participación. Conclusiones: Los protocolos de entrenamiento de la fuerza, de uso aislado o combinado con otros métodos, mejoran la fuerza muscular y funcionalidad de la marcha de los sujetos con EM, así como de sus parámetros metabólicos. Objetivo: Analizar si existe asociación entre las alteraciones de la marcha y la incidencia de deterioro cognitivo en un seguimiento a tres años de adultos mayores de población abierta. Material y métodos: Estudio observacional analítico de diseño longitudinal, se tuvo como base el estudio 10/66, se seleccionó 804 participantes sin trastorno neurocognitivo, se identificaron las características de la marcha las cuales se valoraron pidiendo al participante que camine 10 metros a velocidad normal, se registró la velocidad, estabilidad, balaceo y equilibrio; los datos sociodemográficos y comorbilidades también se extrajeron del mismo cuestionario. Se utilizó regresión lineal multivariable para examinar las asociaciones las alteraciones de la marcha iniciales y el desarrollo de trastorno neurocognitivo. Resultados: La edad media fue de 72.9 años, predominó el género femenino (66.5%), 45.5% no tenía una pareja actual, 64.9% tenían escolaridad baja o nula; 45.2% provenían de zonas rurales, 28.5% tenían marcha lenta, 29.3% disminución del balanceo, 11% inestabilidad y 13% movimientos lentos. La incidencia de demencia fue de 9.2% y de deterioro cognitivo leve 4.2%; las alteraciones de la marcha se asociaron con demencia en los modelos bivariados p <0.010, estos resultados no fueron significativos cuando se asoció a trastorno neurocognitivo menor. Al ajustar con las covariables la demencia se asoció con disminución de la velocidad en la marcha regular IC del 95%=1.05-3.81; mala IC del 95%= 1.01-4.18; balanceo reducido IC del 95%= 1.05-3.50, inestabilidad IC del 95%= 1.08-3.83 y disminución de la velocidad de los movimientos IC del 95%= 1.15-3.84. Conclusiones: El uso combinado de variables de la marcha permitieron una asociación con la incidencia de demencia, las pruebas empleadas son fáciles de realizar y evaluar, sin embargo, esta interpretación se debe realizar con cautela ya que las alteraciones de la marcha no pueden explicarse utilizando un enfoque de factor único. Este texto aborda y hace explícitos aspectos biomecánicos de la marcha integrando la cinética y la cinemática de esta al estudio del movimiento humano desde estados de normalidad. El abordaje biomecánico de la marcha humana es una parte fundamental en las investigaciones en bioingeniería del movimiento humano y en clínica de rehabilitación, para las cuales es necesario comprender tanto el control del movimiento desde los centros integradores, como el desarrollo de la postura y de la dinámica entre los segmentos corporales, los cuales, en posición bípeda, forman una columna que soporta todo el peso corporal dentro de un campo gravitacional. El conocimiento desarrollado en este texto es básico en el estudio del movimiento corporal, en el que la evaluación de la marcha desde los laboratorios de análisis de movimiento a través de los sistemas de captura del movimiento humano permiten no solo conocer el movimiento en las tres dimensiones, sino también la cinética, la cinemática, e incluso datos electromiográficos, que demuestran los cambios dinámicos en la longitud del músculo, así como el registro de la actividad muscular de la persona cuando está en movimiento. Herramientas de alta utilidad en la recolección de datos cuantitativos que permitan de manera objetiva determinadas condiciones de alteración y normalidad.El conocimiento desarrollado en este texto es básico en el estudio del movimiento corporal, en el que la evaluación de la marcha desde los laboratorios de análisis de movimiento a través de los sistemas de captura del movimiento humano permiten no solo conocer el movimiento en las tres dimensiones, sino también la cinética, la cinemática, e incluso datos electromiográficos, que demuestran los cambios dinámicos en la longitud del músculo, así como el registro de la actividad muscular de la persona cuando está en movimiento. 23. Development of a Wearable Sensor System for Dynamically Mapping the Behavior of an Energy Storing and Returning Prosthetic Foot.

It has been recognized that that the design and prescription of Energy Storing and Returning prosthetic running feet are not well understood and that further information on their performance would be beneficial to increase this understanding. Dynamic analysis of an amputee wearing a prosthetic foot is typically performed using reflective markers and motion-capture systems. High-speed cameras and force plates are used to collect data of a few strides. This requires specialized and expensive equipment in an unrepresentative environment within a large area. Inertial Measurement Units are also capable of being used as wearable sensors but suffer from drift issues. This paper presents the development of a wearable sensing system that records the action of an Energy Storing and Returning prosthetic running foot (sagittal plane displacement and ground contact position) which could have research and/or clinical applications. This is achieved using five standalone pieces of apparatus including foot-mounted pressure sensors and a rotary vario-resistive displacement transducer. It is demonstrated, through the collection of profiles for both foot deflection and ground contact point over the duration of a stride, that the system can be attached to an amputee's prosthesis and used in a non-laboratory environment. It was found from the system that the prosthetic ground contact point, for the amputee tested, progresses along the effective metatarsal portion of the prosthetic foot towards the distal end of the prosthesis over the duration of the stride. Further investigation of the effective stiffness changes of the foot due to the progression of the contact point is warranted. [ABSTRACT FROM AUTHOR]

Objective: To describe the clinical evolution of patients with total knee replacement surgery due to grade IV gonarthrosis secondary to varus angular deformity. Material and methods: A descriptive trial where we included patients who underwent total knee arthroplasty due to gonarthrosis secondary to varus angular deformity. We used the following variables, sex, age, affected side, type of approach, type of prosthesis, type of gait post-surgically, infection, vascular status, post-surgical pain, rejection of prosthetic material, varusflection angular deformity pre and post-surgically. Results: We reviewed 13 files, 69.2% men and 30.8% women, the mean age was 72.38, minimum 56, maximum 82, DE 7.11 years. Six (46.2%) left knees and 7 (53.8%) right knees. All patients had a medial parapatellar approach and bone balancing, 23.1% had ligament balancing; 7.6% had a total knee replacement stabilized posteriorly and 93.3% not stabilized posteriorly; 1 (7.6%) patient had infection, none had vascular involvement, 76.9% had to walk with a cane, 15.4% walked on their own and 7.6% had no gait. The average grade of the angular deformity pre-surgery was 15.77 and the average post-surgery was less than or equal to 5°, the difference of average grades pre and post-surgery was 36.719 and in flection 0.439, p= 0.00 and p= 0.669 respectively (paired t). Conclusion: We can conclude that the clinical evolution of patients with gonarthrosis secondary to varus angular deformity who underwent TKA is good, complications are similar to those in the literature. Objective: To assess the functional efficacy of the RIMAG unconventional hip prosthesis in cancer and non-cancer patients. Material and methods: Functional analysis of 38 patients with RIMAG arthroplasty, using the Musculoskeletal Tumor Society Scoring System and including the following: functionality versus histologic diagnosis (5 malignancies, 3 metastases, 14 giant cell tumors (GCT), 8 with other benign tumors, and 8 had no tumor), age (40.4 ± 19.1 years), sex (24 females, 14 males), postoperative follow-up (34.7 ± 31.3 months). Analysis of intraoperative, immediate and late complications, time to weight bearing, gait and work or social reintegration were also assessed. Results. Global mean of ideal functionality was 65.6 ± 23.2%. Age was negatively correlated with the functionality rate (r = -0.507, p = 0.001); significant difference with the histologic diagnosis: malignant tumors 54.8, metastasis 58.6, giant cell tumor 25.6, other benign tumors 34.2, non-tumor causes 56.6 (p = 0.001). When functionality means were adjusted for age with a covariance analysis, better percentages were found for benign tumors (79.2 ± 6.8%) and giant cell tumors (76.4 ± 6.3%), less functionality for non-tumor causes (45.7 ± 7.6%), and intermediate percentages for malig- nant tumors and metastasis (p = 0.03). Intraoperative complications occurred in 4 cases (28.6%), immediate in 3 (7.8%), late in 15 (39.4%); 62.5% of patients started immediate weight bearing, 92.1% resumed their usual activities after surgery. Conclusions: The reconstruction of the proximal femur with RIMAG is a safe and reliable treatment option, with complication rates below the ones reported in the literature. Satisfactory results were obtained, with high functional rates for benign tumors, and low rates in older patients, malignant tumors and metastasis. [ABSTRACT FROM AUTHOR]

Human gait is the way how a human can move in biped position, in which bipodal and monopodal supports are present and requires a development process and automation. It is necessary to understand the bases of this process and that is the objective of the present work. Information from database PUBMED as well as historical articles, texts and references cited in public published papers to date were analyzed. Pertinent information related with the objectives proposed in the present review was found and analyzed. It was then divided into three sections as follows: gait development, normal gait, and characteristics of gait which influence the gravity center line. It can be concluded that the human gait is a locomotion process which presents efficiency and functionality, then each individual present a determined pattern when walking, performing the minimal effort and lesser energy expenditure with an adequate stability and propulsion. The gait pattern is acquired in childhood and after practicing, the neurosensory system converts it in an adaptation process generating repetitive commands of motor controls, which permit to the individual to walk without a conscious effort. Freezing is characterized by an inability to produce effective steps and includes an arrest of the march and festination. Clinical evaluation presents several difficulties. Objective: to translate into Spanish and validate the freezing of gait questionnaire in Mexican patients with Parkinson's disease. Methods: following the translation into spanish of the freezing of gait questionnaire; we included 63 patients diagnosed with Parkinson's disease treated at the Clinic for abnormal movement of the National Institute of Neurology and Neurosurgery in Mexico City. The freezing of gait questionnaire and the Short and the Short Parkinson's Evaluation Scale were applied to all patients. Clinimetric properties of the instrument were assessed. Results: the internal consistency using Cronbach's alpha was adequate for the whole instrument (0.81). 51% of the sample had freezing of gait. Conclusions: the freezing of gait questionnaire, spanish version, implemented in Mexican patients clinimetric possesses adequate clinimetric properties which are comparable to other populations. Objective: To know the community dwelling elderly gait speed in Lima Metropolitana and the slow gait speed associated factors. Material and methods: Transversal and descriptive study of a probabilistic, multiphase and cluster stratified sample of 246 non institutionalized persons over 60 years of the six areas in Lima Metropolitana. Gait speed was calculated as the time to walk a distance of eight meters of ten in total and the Fried's criteria were used to determine frailty status. Results: Mean gait speed was 0.92 ± 0.24 m/s with a percentile distribution between 0.77 m/s (p25) and 1.06 m/s (p75). A slow gait speed was associated with older age (0.95 ± 0.24 m/s in persons of 60 to 69 years vs. 0.84 ± 0,25 m/s in persons older than 80 years, p<0,05), female sex (0,86 ± 0,22 m/ s vs. 1,01 ± 0,23 m/s in male sex, p<0.01), and the presence of frailty (0.67 ± 0.24 m/s vs. 1,04 0,22 m/s in non frail persons, p<0.01). Conclusions: The mean gait speed was 0.92 ± 0.24 m/s. We found an statistic association between a slow gait speed and older age, female sex and the presence of frailty. [ABSTRACT FROM AUTHOR]

This paper introduces a device for the gait kinematic analysis. It is based on videogrametry methods and image processing in order to record the human movement. It was used a digital camcorder, located to a distance of four meters from the person, capturing its gait in the sagital plane. Also, a kinematic model was developed of lower limbs; this model takes the position data of the bony landmarks and calculates the joint angle of the hip, knee and ankle, besides the pelvic tilt during gait. The identification and tracking of the points was carried out by a special reflective coated in the superficial markers. Later, it was designed and developed an algorithm in order to record and process the data gathered, through of the camcorder in the develop language C# .NET. The application developed captures and processes the video, tracking every marker in the frame, applies the kinematic model and shows the graph of the joint angle during gait. Finally, a protocol was designed and a test was carried out with a person in order to compare the data gathered for the software and the theoretical data described in the literature. In spite of the limitations of the system related to gait bidimensional analysis, articular curves very similar to those theoretical were obtained with commercial methods for the tridimensional movement analysis.
