Introduction: Teaching evidence-based practice is a recognized worldwide strategy for improving health care quality. Objective: To identify the level of evidence-based practice competencies of nursing students according to the characteristics of the curriculum followed in a Colombian university. Materials and Methods: Cross-sectional study measuring nursing students in three periods (N=125): Curriculum-1 without research training (n=24); Curriculum-2 with research training (n=55); Curriculum-3 with evidence-based practice training (n=46). The primary outcome was evidence-based practice competencies measured by attitude, knowledge, and skills. Results: The global competence score was 4.08 (interquartile range = 3.84-4.32), and no statistically significant differences were observed between the scores obtained by students in the three curricula. However, knowledge competencies for evidence-based practice were significantly higher in the Curriculum 3 group. Discussion: Some authors have described the relationship between the timing of curricular transformations and the development of progressive competencies as a staggered process whose results are reflected in at least one complete curricular cycle after its implementation. Conclusions: The curricular adjustments made at this institution have improved the perception of knowledge in evidence-based practice among nursing students; however, it is necessary to continue working on effective pedagogical strategies that enhance the skills for its implementation. The article describes a descriptive process study that evaluates the implementation of the good practices guide for vascular access in the Regional Host Euskadi of Osakidetza-Basque Health Service. The results show an increase in the use of alternative catheters and in the early removal of short peripheral venous catheters. It is concluded that the implementation of the guide improves the results in the use of alternative catheters and the decrease in days of peripheral venous catheters. The study evaluates the results of the implementation of the clinical practice guide for vascular access in the organizations of the Regional Host Euskadi. An increase in the use of alternative catheters to peripheral venous catheters and a decrease in the days of use of central venous catheters are observed. However, conclusive results have not been obtained in reducing complications of vascular access devices. [Extracted from the article]

This report presents the work done on the design, publication, and impact of updates on evidence-based COVID-19 interventions, in order to support decision-making with updated evidence syntheses based on living systematic reviews of therapeutic interventions. To this end, a specific working group was created within the National Commission for Health Technology Assessment (CONETEC) of the Ministry of Health of Argentina, in collaboration with the Pan American Health Organization (PAHO). A methodology was designed to develop and publish assessment documents aimed at supporting decision-making. These update reports were based on the exploration of three areas: health effects, feasibility of implementation, and evidence-based recommendations. A color-coded system was used to present the conclusions according to the balance between their positive and negative effects in different clinical scenarios, in order to improve their interpretation and implementation. In 2021, 16 evidence synthesis reports were published (13 original reports and three full updates). These were downloaded many times from the CONETEC website and the Regional Database of Health Technology Assessment Reports of the Americas (BRISA), highlighting the need for robust, up-to-date, reliable evidence summaries adapted for implementation in the country's health system. Other challenges include constant updating of information, appropriate dissemination, and sustained rigorous preparation of the reports. [ABSTRACT FROM AUTHOR]

Background: Students often have difficulty engaging with evidenced-based practice (EBP) courses that are largely theoretical and lack clinical relevance. This may result in graduates lacking vital skills to critique practice and participate in quality improvement. Approach: This paper describes the development and implementation of a collaborative academic and healthcare industry-based course that linked theoretical aspects of EBP. Students undertook a quality improvement project during clinical practice experience. Course assessments were designed to provide feedback on the progression of projects, the integration of theoretical and practical aspects of EBP and dissemination of findings. Conclusions: The course required substantial collaboration between the university and industry partners to identify clinically relevant projects and coordinate academic and clinical mentorship to support the students. The course demonstrated a significant level of clinical and academic partnership that resulted in relevant clinical quality improvements and increased student engagement with learning about, producing and implementing EBP. Introduction. The Best Practice Guidelines of the Registered Nurses' Association of Ontario (RNAO) is a comprehensive approach that includes issuance of guidelines, active support for their implementation, and an international data system for assessing results. Objective. Reflect on the process of transformation in nursing through knowledge, taking into consideration the past, present and future of the Best Practice Guidelines of the RNAO. Summary. Initially, background is provided on the development of the Best Practice Guidelines, and their dissemination, implementation and sustainability. The program has been expanded at three levels: 1) upward expansion, or through increased coverage; 2) outward expansion, or through the adaptation of policies, laws and guidelines; 3) inward expansion, or through changes in norms and culture. In the large-scale dissemination of the Best Practice Guidelines, several success factors can be observed, such as: localization, comprehensiveness, solidity, proven results, accessibility, being at the forefront and collective identity. Conclusions. The program enables healthcare institutions and systems to focus on the patients and clinical excellence, using the most recent research as basis for the practice and to optimize results. Introduction. The patient care model at the Salvation Army Toronto Grace Health Centre Hospital is based on principles of patient- and family-centered care provided by the interprofessional team. In this framework, different programs are designed and implemented to constantly improve and keep a good level of care and quality of life of the patients, within the limitations involved in hospitalization, once they have become a Best Practice Spotlight. Objective. Present the experience of the interprofessional wound rounds before and after being recognized as a Best Practice Spotlight. Summary. Before becoming a BPSO, the policy and procedure documents guiding the hospital's practices on ulcers or pressure wounds used as reference the evidence-based guidelines of the Registered Nurses' Association of Ontario (RNAO). However, the implementation or application of these practices had not been formalized directly at the point of patient care. This article presents the transition and the relevant aspects for making patient- and family-centered care a reality for the interprofessional team in the component of interprofessional wound rounds. Conclusions. The success in the effective implementation of the guidelines of the BPSO program has become a collective identity for the organizations that work towards the best results in healthcare organizations in national and international institutions. Introduction: Knowledge is a necessary tool for scientific research and progress in any discipline. But scientific knowledge and information dynamics are not only sustained by individuals but are produced and maintained by groups of people working in the same environment where links and relationships can influence the process. Aim: To analyze the social networks of information source utilization, help/advice for knowledge transfer and the places where nursing professionals share information. Method: Analysis of social networks through a validated questionnaire. Professionals from 6 hospital units were recruited. Results: 77 professionals participated with a mean age of 42.9 (SD:11.48). Peers were the most frequently used source of information (76 choices) compared to databases and scientific articles, which were the least selected (63 choices). Horizontal homophilous networks (professionals with similar status/interests) are the most frequent for obtaining information on research results (74 choices). The care unit is the most pointed environment for sharing information (50 choices). Conclusions: Professionals consider the knowledge of their peers as the main source for obtaining information on research results. Units with a certain degree of specialization use clinical practice guidelines and protocols as the main source of information. Nursing professionals use homophilichorizontal networks to obtain information. The work environment in its different settings (unit, office, meetings) is the most used for sharing information on research results. [ABSTRACT FROM AUTHOR]

Introduction and objectives: The naturalistic outcomes assessment in psychotherapy is a requirement for the clinician to evaluate the effectiveness of his work and, in this way, correct possible deficiencies in his performance. The present study evaluated the outcomes of a psychotherapeutic intervention carried out from the solution-focused therapy of a clinical psychologist in his private practice for five years. In addition, the relationship of the outcomes with different sociodemographic, clinical and model variables was evaluated, in search of a profile that would benefit especially from the psychotherapeutic process. Material and methods: The sample was comprised of 249 people aged 11 and 78 years. A naturalistic evaluation was carried out implementing a protocol for assesing outcomes in the routine clinical work. The severity of the symptoms and the impairment of the cases were measured by the Global Assessment Functioning Scale (GAF). Sociodemographic, clinical and model variables were also evaluated. Results: 75.5% of the sample had a successful outcome in therapy. This percentage rose to 85.8% if only people who were considered to have completed psychotherapeutic treatment were taken into account. Age, life stage, symptomatic severity, and having previously been in therapeutic treatment were related, although weakly, to the results. Conclusions: These outcomes support the hypothesis of the effectiveness of psychotherapy and its strength in different sociodemographic and clinical profiles. [ABSTRACT FROM AUTHOR]

Evidence-based Dentistry allows clinical decisions based on current research supported by the best available evidence, integrating the clinical experience, with the preferences and needs of the patients. Objective: the objective of this article was to determine the initial state of knowledge, attitudes, access, and confidence about the principles of Evidence Based on Practice of first-year students at Dental School at the San Simón University. Methods: descriptive, prospective cross-sectional study that applied the KACE form validated by Hendricson in 2012, consists of 35 items divided into four categories: Knowledge, Attitudes, Access, Confidence. Results: the students who participated (n=239) registered in Knowledge category 62.76% correct answers about case study knowledge. In Attitudes category 67.37 % indicated they agree with the evidence-based patient care. Access to Evidence category 40.17% of the students' access textbooks in search of information; Confidence category in critical evaluation skill 45.61% indicated having some confidence about the adequacy of the sample size. Conclusion: according to the results, the training of knowledge about Practice Based Dentistry should be deepened. Regarding attitudes, the results indicate student interest; in access to evidence, students registered a fair value in confidence in critical assessment. [ABSTRACT FROM AUTHOR]

Introduction. It is recognized that the evidence-based nursing (EBN) contributed to enhancing the quality of care and that there are organizations such as the Registered Nurses' Association of Ontario (RNAO) that have developed guides on clinical practice and methodological strategies for their implementation. This article describes the experience and reflections on the implementation process of RNAO's EBN guides, focusing on the sustainability and adhesion results of three guides implemented in a tertiary institution. Reflection topics. RNAO has created an implementation tool according to the knowledge-to-action process model which consists of six phases: problem identification, local context adaptation, assessment of facilitating agents and of obstacles for the use of knowledge, adjustment, deployment of interventions / implementation strategies, monitoring, evaluation and sustainability. All these phases were advanced to initially implement three guides ("Assessing and managing pain", "Preventing falls and injuries from falls" and "Assessing skin lesions risk due to pressure") at a tertiary institution. Conclusions. The periodic audit and verification of the strategies implemented promotes compliance by the service personnel as regards institutional goals and the sustainability of the change achieved. In this study, seven important personalizing factors are reviewed: patient reactance level, stage of change phase, culture, coping style, attachment style, therapeutic preferences, and religious and spiritual dimension. Meta-analytic studies suggest that these factors notably affect the efficacy of psychological treatments, and that their application will allow for further improvement in psychotherapy efficacy. Multiple treatment comparison meta-analyses are systematic reviews that allow to make direct or indirect comparisons of different interventions from the data collection results of randomized clinical trials, grouping them in a common outcome or conglomerate which is expressed quantitatively. Other names given are network meta-analyses, or mixed treatment comparison meta-analyses or indirect meta-analyses. The use of the comparison of several treatments allows a greater estimation of the intervention effect. This is a guide for the physician to make more accurate decisions and to apply better care criteria to their patients. This literature review should help the reader to perform an organized and adequate critical reading (according to the JAMA recommendations) of the multiple treatment comparison meta-analyses. The current landscape of psychotherapy is as prosperous as it is disconcerting. Different therapies show comparable efficacy. Evidence- based practice has given way to a backwash of different movements. Psychotherapy process research leads to an archipelago of "niches of researchers" . In order to clarify this panorama, we have turned to the philosophy of science. The philosophy of science has made it possible to identify different epistemologies, reopen the distinction between positive natural science and human sciences and resituate psychotherapy on the human science side. As a human science, psychotherapy is, above all, a contextual holistic relational activity, person-centered and values-based, rather than a scientific-technical activity centered on "faulty mechanisms". Wampold's contextual model and a phenomenological-existential approach are milestones of this "new life" of psychotherapy, which is more human than technological-impersonal. [ABSTRACT FROM AUTHOR]

This study reports the experience obtained by the National Institute of Health of Peru in the use of deliberative dialogues for the formulation of recommendations as one of the stages of the process of adaptation of evidence-based clinical practice guidelines using the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) system. Using a multidisciplinary approach, this study describes: 1. the public health problems addressed in the dialogues; 2. the role of the involved parties; 3. the parameters that were considered before and during the discussions; 4. the challenges for optimizing these dialogues and stimulating the interaction between scientific evidence and the added value provided by the involved parties through their experiences, perspectives, and knowledge for the development of recommendations in health. Introduction: The incorporation of Registered Nurses' Association of Ontario Best Practices Guidelines is recognized as an invaluable contribution to nursing, and its contribution to the reduction of variability in patient care and safety has also been proven. It is a challenge to include in nursing training processes the use of learning strategies "E-learnings" that allow expanding coverage in training and obtaining adequate results. Objective: To present the experience and reflections regarding the training of champions of the evidence in the nursing staff in "las Condes de Chile" Clinic on the implementation of clinical practice guidelines. Discussion and Conclusions: Based on continuous education by phases, the Clinical Practice Guideline for Pressure Ulcer Prevention was initially taken into account and subsequently the Clinical Practice Guideline Registered Nurses' Association of Ontario for the Management of Vascular Accesses. Approximately 900 employees attended and 550 of them were nurses. It was observed that after the training, using the E-learning teaching platform, there were no differences in learning between the groups instructed under the E-learning methodology versus the face-to-face methodology. Objective: To present the socio-political context in which the best practices guidelines program of the Registered Nurses' Association of Ontario (RNAO) is situated. Reflection Topics: The discussion focuses on the key health and healthcare challenges faced by different countries around the world, and the threats and opportunities they present for Nursing. Emphasis is placed on the need to use evidence as a central tool, but not the only one to optimize the health outcomes of citizens. Conclusions: The four main components that facilitate an effective care model correspond to adequate human resources, positive inter professional work, scope expansion of the role of registered nurses (RNs), and evidence-based practice. Introduction: Sedation is a practice frequently used in multiple settings outside the operating room. Currently, it is employed by different interdisciplinary groups. It is imperative toensure safe, standardized practices and proper training of the personnel involved. Objectives: To update available information, create minimum standards of care for performing high-quality sedation procedures based on the best available evidence. These standars lead to safe and effective care practices, reducing unjustified variability. Materials and methods: A group of different health professionals with experience in sedationoutside the operating room and representatives of scientific and patient societies developedthe guideline. Through a non-formal consensus of thematic and methodological experts, the content, scope and objectives, study populations and questions to be examined wereall defined. A systematic review of the literature was performed to formulate the recommendations with graduated levels of evidence in accordance with GRADE methodology. Results: A summarized version of the Clinical Practice Guideline with recommendations for the administration of sedation as part of diagnostic or therapeutic procedures outside the operating room in patients over the age of 12. Conclusions: The Clinical Practice Guideline presents evidence-based recommendations in order to standardize the procedure, improving the quality of care and reducing morbidity of patients that require sedation outside of the operating room. Introduction: The implementation or Clinical Practice Guidelines (CPG) is a strategy that whose goal is to increase the use of evidence in the context of decision making and building health policies. Nurses of The “Cardioinfantil-IC” Foundation (FCI-IC) established a collaborative agreement with the Registered Nurses Association of Ontario, Canada (RNAO), in order to implement and evaluate three of its GPC in adult inpatients. Objective: This paper describes the design, process and preliminary evaluation of an implementation plan between July 2012 and December 2014. Methodology: The selection of the CPG was made after an initial needs diagnosis and a feasibility analysis. Two CPGs were based on prevention: one about fall and another one about pressure ulcers (PU); and one more based on direct care related to assessment and management of diabetic foot ulcers. The indicators selected for CPG about prevention were frequency of evaluation and confirmation of risk and event rate (falls and UP). In the CPG about care was evaluated the percentage of assessed patients (lower limbs and feet) in admissions, the percentage of patient/family education, and the percentage of patients with foot injury reduction (50%). Results: The evaluation of the risk for falls and pressure ulcers with a reassessment ranging from 24 to 72 hours was performed in all patients that were admitted. There was a reduction in the use of guardrails, and a low reduction in falls. There was no change in the rate of pressure ulcers but the incidence of ulcers was limited to grade II. The assessment in lower limbs, the education, and care about foot ulcers in diabetic patients increased. Conclusions: The implementation process of GPC, as a result of the networking with the RNAO, has guided nursing care practice based on the assessment of risk and events. Some limitations in the level of evidence of some recommendations have promoted the development of a new research. Objectives: Efforts have been made to standardise evidence-based practice, but clinical practice guidelines do not always follow strict development methods. The objective of this review is to identify the current guidelines, analyse the variability of its recommendations and make a synthesis for clinical practice. Material and methods: A systematic review of clinical practice guidelines was made in electronic databa- ses and guidelines databases; using "fibromyalgia" AND ["guideline" OR "Clinical Practice guideline"] as terms, from January for 2003 to July of 2013. Guidelines were selected according to the following criteria: a) aimed to fibromyalgia treatment in adults; b) based on scientific evidence, systematically searched; c) evidence levels and strength of recommendation included; d) written in English or Spanish. Results: From 249 initial results, six guides fulfilled the inclusion criteria. Clinical practice guidelines analysed in this review show great variability both in the presence and level of evidence and in the strength of recommendation of many treatments. Physical exercise and cognitive-behavioural therapy are first-line treatments, showing high level of evidence. Amitriptyline, used for short periods of time for pain control, is the pharmacologic treatment with the most solid evidence. The multimodal approach reported better results than the isolated application of any treatment. Conclusions: Final recommendations in this review identify optimal treatments, facilitating the translation of evidence into practice and enabling more efficient and effective quality care. In 2002, the School of Nursing at the Pontificia Universidad Católica in Ecuador introduced the teaching of evidence-based practice as part of the Curriculum Reform and Career Diversification Project. Now, over a decade later, the effectiveness of its implementation is being analyzed. Objective: Explore the beliefs, opportunities for implementation and support provided by the institution and its organizational culture to the teaching of evidence-based nursing practice (EBN). Materials and Methods. This is a qualitative and quantitative exploratory study involving 18 teachers and 77 students. The instruments included two surveys (Likert scale): one for professors and another for students. Results: The results of the surveys were analyzed with different data stratification: professors, students, the five steps in the EBN method and other criteria. A weighted mean was applied, as was a one-way and two-way ANOVA, with a significance level of α = 0.05, and Tukey-Kramer's multiple comparison method. Discussion: A comparison of the responses of both groups showed considerable importance was attributed to beliefs about EBN, but not to implementing changes in EBN practice or teaching, which is contradictory. Conclusion: The perception of teachers and students on the dimensions of EBN beliefs and implementation was quite similar in both groups. In recent years, applications of advanced technologies have led to the emergence of a research line in the behavioral sciences. The growing body of publications that report the efficiency and effectiveness of intervention programs that address a range of psychological disorders raises the need to strengthen them, assessing their generalizability to other cultures in order to assess the external validity. Through the collaboration of two research groups, a cross-cultural validation was carried out in Mexico, assessing two treatment programs using virtual reality to treat fear of flying, agoraphobia and fear of public speaking with a self-applied program, which previously demonstrated efficacy in Spain. In this paper we report the results of effectiveness and efficiency in the Mexican population. To carry out clinical practice based on evidence (CPBE), it is necessary to acquire some skills, including systematic literature search and application of formal rules for evaluation of the literature found, thereby obtaining the best evidence available. Thus, one could summarize the CPBE process includes several steps, one of which is critical appraisal of the evidence found in literature search. It is essential to understand that not all the information from scientific journals is reliable or valid, and therefore suitable to be instituted in clinical practice. For this reason, the information must be critically analyzed; i.e. carefully and systematically examining research to judge its validity and reliability, or stated another way, its value and relevance in a particular context. Objective: To review the clinical evidence on subcutaneous (sc) abatacept and to formulate recommendations in order to clear up points related to its use in rheumatology. Method: An expert panel of rheumatologists objectively summarized the evidence on the mechanism of action, practicality, effectiveness, and safety of abatacept sc and formulated recommendations after a literature review. Results: The efficacy and safety of abatacept sc was studied in 7 clinical trials, 3 double-blind, 3 open, and one mixed, with the following endpoints: comparison against abatacept iv, impact on immunogenicity, effect of replacing iv by sc, abatacept sc in monotherapy, and non-inferiority to adalimumab. No significant differences were found between sc and iv abatacept on efficacy or safety. The development of sc abatacept has allowed a complementary study to the iv, formulation, thus making the abatacept profile better defined. Conclusions: This is a practical document to supplement the summary of product characteristics. In summary, abatacept sc is presented asan effective and safe drug and, therefore, as an alternative for use within the broad armamentarium the rheumatologist has to treat RA. It also has the advantage of being the only biological agent that can be administered iv and sc which can facilitate its use in certain patients. [ABSTRACT FROM AUTHOR]

- Abstract
Given the large number of publications, it is a problem to choose a study to respond to clinical problems with a good level of evidence. This requires following a scientific methodology, defining and limiting the problem, transforming it into a question, applying a search strategy in an appropriate source of information and finally, analyzing the information and summarizing the evidence found. Specific tools for analize the evidence had been develop. They are known as reading guides, Reader's Guides or Users' Guides, of which there are distinct formats, but in general, they give the necessary guidelines for the critical analysis of the different tipes of biomedical articles. These tools assess three key areas of study: internal validity, impact and external validity. The areas above mentioned are practically evaluated applying a user's guide to articles relating to treatment, as an example to a study published in a prestigious scientific journal. 49. GUÍA DE PRÁCTICA CLÍNICA PARA LA DETECCIÓN TEMPRANA DE LAS ANOMALÍAS DURANTE EL TRABAJO DE PARTO, ATENCIÓN DEL PARTO NORMAL Y DISTÓCICO.
- Abstract
Objective: To make recommendations for early detection of abnormalities during labor, and care of normal and dystocic delivery. Materials and methods: The Guideline Developer Group (GDG) prepared this CPG during 2011-2012 in accordance with the Methodology Guideline for the development of Comprehensive Care Guidelines of the Colombian General System of Social Security, on the basis of the available scientific evidence, and with the active participation of patient groups, scientific societies and stakeholders. In particular, the evidence for this section was adapted from the "Clinical practice guideline for normal delivery care" (País Vasco -- 2010) and adapted through systematic procedures for the search and assessment of the evidence as well as for the generation of recommendations. The level of evidence and the power of the recommendations were expressed using the Scottish Intercollegiate Guidelines Network (SIGN) system. Results: We present the recommendations for labor and delivery care associated with the highest probability of a successful outcome for the mother and the baby. Conclusions: It is expected that the recommendations contained in this CPG will be used by practitioners in pregnancy care programs in order to reduce morbidity and mortality attributable to labor and delivery complications.
