The diagnostic criteria, treatments at the time of admission, and drugs used in patients with acute coronary syndrome are well defined in countless guidelines. However, there is uncertainty about the measures to recommend during patient discharge planning. This document brings together the most recent evidence and the standardized and optimal treatment for patients at the time of discharge from hospitalization for an acute coronary syndrome, for comprehensive and safe care in the patient's transition between care from the acute event to the outpatient care, with the aim of optimizing the recovery of viable myocardium, guaranteeing the most appropriate secondary prevention, reducing the risk of a new coronary event and mortality, as well as the adequate reintegration of patients into daily life. The paper examines the role of social work in the hospital setting and its contribution to comprehensive care for hospitalised people. The main objective of this research is to understand the position and function of hospital social workers in relation to the evolution of the concept of health and the development of health social work as an academic discipline. A qualitative methodology has been applied, using exploratory interviews from a Spanish tertiary level hospital with social workers, one of whom has the coordination functions. The results obtained reveal that hospital social work challenges the hegemonic biomedical model, gradually seeing its level of institutional recognition increase. Particularly noteworthy is the effort to focus on addressing the social factors that interfere (and are interfered) with the health status of hospitalised people, in addition to the concern of moving towards proactive and early planning of hospital discharge that ensures responsible management of patient flows, an adequate transition after discharge and continuity of care. However, some challenges are also identified, such as the restrictive association of social work with discharge management or the limitations resulting from the increase in administrative tasks and austerity policies, which may affect the ability of professionals to develop functions of utmost importance, such as the accompaniment or the defense of rights. In conclusion, the need to continue building hospital social work that allows questioning the dominant structures of hospital care is highlighted, placing the ethics of care and social justice at the centre. In addition, the importance of fully acknowledging and valuing the unique contributions that social workers make is emphasised in order to promote their visibility and recognition to ensure professional practice that addresses the biopsychosocial as a whole. Introduction: The WHO reports an increase in chronic non-communicable diseases. Given the complexity of these diseases, patients are constantly hospitalized; therefore, it is necessary to strengthen the competence of both patients and caregivers at home to take care of the disease using the program "discharge plan." Objective: To determine the change in care competency following the implementation of the tool "discharge plan" in a second-level care facility for in-home care of the chronically ill person and family caregiver dyad. Materials and Methods: Quantitative, quasi-experimental study, with pre-test and post-test design, using the "Characterization of the patient-caregiver dyad" form and the "CUIDAR" instrument. The population consisted of 62 dyads. Non-probabilistic convenience sampling was used. Pre- and post-delta was calculated, and paired Student t-test was used for comparison. Results: The "discharge plan" program increased the patients' dimensions of Knowledge by a delta of 3.2, Uniqueness by 3.0, and total care competency by 10.3. Family caregivers' competencies improved in the dimensions of Knowledge by a delta of 3.34, Uniqueness by 1.5, Enjoyment by 1.7, and Reciprocityrelationship by 1.61, for a total competency score of 9.2. Discussion and conclusions: These findings suggest that the discharge plan program must be strengthened during hospitalization and discharge to improve the homecare competency of chronically ill patients and their family caregivers. Introduction. Hospital readmission of patients with mental illness is mainly caused by worsening of the underlying disorder; therefore, interventions focused on rehabilitation and social reintegration should be considered, being the transition to home care a strategy that contributes to the reduction of gaps, and an improved access to services and rehospitalizations. The present study seeks to identify and expose, from nursing care, which are the characteristics of the transition plan of the hospitalized patient in the mental health unit reported in the literature synthesizing the findings. Methodology. Scoping review according to the guidelines of the JBI Manual for Evidence Synthesis and the PRISMA report. The databases PubMed, Biblioteca Virtual en Salud, Nursingovid and ScienceDirect were consulted, including publications in Spanish, English and Portuguese in the period 2015-2021. Results. Fourteen articles were selected, identifying the following topics: Discharge Planning and Preparations; Continuity of Health Care; Social and Family Support for Discharge; and Therapeutic Adherence and Implications for Nursing Practice. Discussion. In line with other studies, the interventions that deal with good interpersonal relationships, social support and the generation of the link between the hospital and the community stand out. Conclusions. The retrieved literature reports five characteristics of the transition plan with nursing interventions that represent the success of the transition process for the patient. Introduction: Birth, hospitalization, transition from hospital to home and care for preterm infants are critical events for parents. Materials and Methods: A descriptive qualitative study was conducted with four fathers and 12 mothers who had experienced preterm infant care. Semi-structured interviews were conducted between March and October 2020 and grounded theory tools were used for analysis. Results: The following four categories were identified: coping with preterm birth, experiencing day-to-day prematurity in neonatal units, empowering parents to care for preterm infants and caring at home. The category for empowering parents to care for preterm infants was identified as the central category in the experience. Discussion: Experiencing preterm birth is complex and dynamic as it involves a high emotional burden for parents. Caring for preterm infants requires developing specific skills before discharge in which empowerment plays a central role. Parental empowerment facilitates this transition supported by nursing accompaniment, information provided, involvement in the care of their child in the neonatal unit, family support and individual conditions of parents. Conclusions: Experiencing parental empowerment in the care of their preterm infants is a key element that facilitates the transition from hospital to home and encourages the development of home care skills. Introduction: Heart failure (HF) is a disease with a high impact on people's quality of life. Therefore, it is necessary to develop strategies that promote self-care from hospitalization and support the transition process. Objective: To determine the effect of a nursing discharge plan on the level of self-care of patients with HF who are hospitalized from January to November 2019. Methodology: Controlled clinical trial with a sample of 64 patients. They were randomized to discharge plan or usual follow-up. To measure the level of self-care, the European Self-Care Scale was used with a Cronbach's alpha of 0.7. A descriptive analysis was performed with tables of frequencies and percentage distribution and the non-parametric Mann Whitney U test. Results: Before applying the discharge plan, it was determined that 90.2 % of the patients in the control and intervention groups had a low level of self-care. After applying the discharge plan, a statistically significant improvement (p: 0.000) was observed in the level of self-care. Conclusions: The discharge plan had a beneficial effect on the level of self-care thanks to its educational, motivational, behavioral reinforcement and symptom management components, allowing patients to assume an active role in modifying their behaviors to promote health. Klebsiella pneumoniae (Kp) is a member of the Enterobacteriaceae family, it is an opportunistic pathogen that affects immunosuppressed individuals. K1 and K2 strains are hyper mucous that is considered an hypervirulent factor. We report a case of a female patient who developed a hepatic abscess, bacteremia and meningoencephalitis due to an hypervirulent Kp strain requiring mechanical ventilation, vasopressors, antibiotic treatment and percutaneous drainage of the liver abscess in the intensive care unit at Hospital Cayetano Heredia. The patient survived and was discharge from the hospital. This case illustrates the presence of an hypervirulent Kp strain in Peru. [ABSTRACT FROM AUTHOR]

Objective. To describe the clinical features of patients with post-COVID-19 syndrome who have recently been discharged from intensive care units (ICUs) included in a chronic care program in Colombia. Methods. Descriptive case series study of a cohort of patients with post-COVID-19 syndrome who entered the Remeo® chronic care program between July 2020 and May 2021. Clinical features, complications, and treatments are described. Results. Among patients in the program discharged from an ICU, 122 cases of post-COVID-19 syndrome were identified. These patients continued in the program. The mean age was 66.9 years (CI 64-68); 62.29% were men, 88.9% (109) had a tracheostomy, 72.8% (90) had a gastrostomy, and 99% required supplemental oxygen. In the first four months, 9,518 interventions were carried out, including physical therapy (...:20.7), occupational therapy (...:10.9), respiratory therapy (...:41.4), and psychology (...:4.8). Conclusions. The chronic care program was an option for patients with post-COVID-19 syndrome recently discharged from an ICU, with a view to minimizing ICU occupation rates and facilitating patients' return to their homes. Objective: to identify the available evidence regarding the post ICU syndrome in patients leaving the intensive care unit. Methods: it is a review of scope according to the Joanna Briggs Institut methodology, in 5 data bases with the search strategy ([PostIntensive AND Care AND Syndrome] [Nursing AND Care AND PICS]). Primary studies published between 2010 and 2020 with full text available were included. Results: having analyzed 48 primary studies, 6 thematic categories were created: cognitive aspect in post ICU syndrome, musculoskeletal, psycho-emotional, prevention strategies with models of ICU survivors, post ICU syndrome in the family and evaluation tools. Conclusions: To improve the health care in patient in post ICU syndrome, to prevent the syndrome emergence and increase the quality of life provided to the ICU survivors and their families. Evidence shows that caregivers can develop different disorders that impact their quality of life. Nursing plays a crucial role in preventing the syndrome advent by using interdisciplinary and ICU strategies. This charge and follow-up plans can make these patients to achieve a faster rehabilitation and avoid impairment or sequels in the future. [ABSTRACT FROM AUTHOR]

Objective: To describe prenatal and post-natal clinical features of neonates born from mothers with COVID-19 including 14-day follow-up post discharge. Methods: Case series of neonates attended at Hospital Guillermo Almenara born from mothers infected with COVID-19 during the lockdown implemented by the Peruvian government. Neonates were divided in two groups based on positivity of the nasopharyngeal PCR test. Results: 201 pregnant women with COVID-19 diagnosed by rapid tests who delivered 206 neonates were identified; 4 of these neonates had positive nasopharyngeal PCR tests. Pregnants infected with COVID-19 were younger than non-infected, but no differences were observed in route of delivery, sex of the newborn and body weight at birth. Two hundred and five neonates were discharged one week after birth ;181 neonates with negative nasopharyngeal negative PCR tests and 4 neonates with a positive PCR test were followed for 14 days, none presented symptoms and received mixed lactation. Conclusions: Neonates born from mother infected with COVID-19 had minimal risk of getting the infection and their pre and post-natal development will not be affected despite of having a positive PCR test. Introduction: Background: the techniques for screening and nutritional intervention in hospitalized patients are a cost-effective measure. In clinical practice the problem lies in their administration. Objective: to evaluate the implementation of a nutritional screening tool in patients admitted to our Medicine Ward (M). Material and methods: a single-center, prospective, observational study in patients admitted to M. Exclusion criteria: end-stage cancer patients, inpatient stay less than 72 h, and hospital readmission of previously included patients. One out of every 2 hospital admissions was selected. We assessed the following: whether nurses performed the screening test, the Short Nutritional Assessment Questionnaire (SNAQ); the score obtained; consultations with dieticians (ICD); prescribed nutritional support; and coding of malnutrition in the discharge report. Dieticians repeated the SNAQ for one in every 3 patients. Results: during the study period 726 patients were admitted, 377 were selected, and 315 were included. The nursing staff administered the SNAQ to 93.6% of patients and malnutrition was present in 19%. Dieticians detected malnutrition in 37.8%. In 41.9% of patients with severe malnutrition an ICD was performed, whereas in 40% of them no nutritional intervention was implemented. The diagnosis of malnutrition was recorded in the discharge report of 42.1% of our cases. Conclusion: Compliance with screening test administration was good, but there is disagreement with the assessment made by dieticians. For most malnourished patients nutritional support fals to be prescribed, no ICDs are performed, and a malnutrition diagnosis is not included in the discharge report. Background: Post-operative and intrahospital infections generate greater morbidity and mortality and increase the costs of care. The abuse of antibiotics and the increasing prevalence of postoperative infections by gram-negative bacteria have led to the use of alternative regimens of antimicrobial prophylaxis. Objectives: To describe the behavior of nosocomial infections and compare two schemes of pre-surgical antimicrobial prophylaxis. Material and methods: Retrospective study, in which the rates of nosocomial infections are described, and their relationship with two schemes of pre-surgical prophylaxis (PEP) from March to June; EPP 1 (year 2017) and EPP2 (year 2018), as well as with additional measures to the PPE as use of chloraprep / Lysol (2017) and hand washing (2018) during these periods, in order to evaluate the effectiveness of the same. Results: The number of episodes of infections per 100 discharges was 10.67 (2017) and 11.42 (2018) without statistical differences. The infection rate of central venous catheter was 17.4 vs 9.1 (p = 0.001), urosepsis of 10.75 vs 9.27 (p = 0.03,) and bacteraemia of 2.59 to 1.88 (p = 0.04,). During the EPP1 scheme there were 125 episodes of infection, while EPP2, only 119, as well as decrease in urosepsis, bacteremia and infection of the gastrostomy site. Conclusions: There was no significant increase in neurosurgical infections with both schemes, nor in nosocomial infections. It is important to mention that the use of chloraprep for presurgical asepsis and Lysol for decontamination of inert surfaces, as well as the improvement in handwashing techniques have been determinant for the control of nosocomial infections. Objectives: To determine the efficacy of the Manheim´s index in predicting mortality in patients with secondary peritonitis. Methods: A retrospective diagnostic like study was carried-out with data extracted from clinical charts. A non-probabilistic sampling of 294 patients of whom 40 had died and 254 were discharged alive after being surgically intervened from secondary peritonitis in the General Surgery service at Hospital Daniel Alcides Carrion in Huancayo was calculated using EPIDAT 4.1. Sensitivity, specificity, predictive values, likelihood ratios and ROC curves and area under the ROC were calculated. Results: A Manheim´s index of ≥26 showed a sensitivity of 0.85, specificity of 0.972, positive predictive value of 0.829 and negative predictive value of 0.976. The optimal threshold was ≥21 attaining a sensitivity of 0.975. The area under the ROC curve was 0.976. Conclusions: Manheim´s index is a good predictor of mortality among patients with secondary peritonitis, the optimal threshold was ≥21. [ABSTRACT FROM AUTHOR]

Objective: Implement a Nursing Process (NP), exclusive of Escuela Militar de Enfermeras that consists of a structure that covers the needs, that allows to achieve optimal results on the effectiveness of care; the procedure was carried out with 31 cadets of the 4th year generation 2012-2016; a diagnosis of the former graduates theses was made; based on this analysis; a teaching manual was designed, a providing training to the cadets; who worked with case studies relating them to the diagnoses (NANDA), results (NOC) and interventions (NIC) for better effectiveness in the process. Material and Method: Transversal, prospective, mixed and descriptive study. A Likert scale (Cronbach's alpha 0.70) was applied to the cadets and a test of knowledge about the PAE (Richardson Kudder 0.49). SPSS program version 22 was used for data descriptive analysis and inferential statistics. Results: The findings indicate that the cadets had not implemented the NP, they had limited interaction with the patient; however, they were doing assessment activities based on the diagnosis (p = 0.023). After the training, the 31 cadets made the formulation of nursing diagnoses, interventions and evaluation through NANDA, NOC, NIC, and the teaching manual. Conclusions: The cadets showed a positive attitude about the process and taxonomies. It is useful because it gives them the scientific fundaments to the professional practice, provides a comprehensive view of patients, helps recovery and evaluates of the quality of care. Objective. Determine the spatial distribution of hospital discharges of viral fever caused by mosquito bites in Mexico and characterize its historical trend in the period 2004-2014. Methods. An exploratory cross-sectional inferential analysis was performed. The primary data were obtained from the hospital discharge registries of the National Health Information System (SINAIS) for the period 2004-2014. In order to identify clusters of reported cases, indicators of geographical association were estimated and risk areas established on the basis of environmental determinants validated with estimated relative concentration measurements. In order to determine the temporal trend and age structure of the cases, growth rates were estimated and density curves constructed. Results. The geographic distribution of these hospital discharges in Mexico follows a focalized pattern. In total, a high concentration was detected in clusters made up of 20 municipalities, 9 of which were located in the state of Sinaloa. In 37% of states in the country, the concentrations of hospital discharges of viral fever cases were higher than those for the country as a whole, and their localization indices ranged from 5.79 in Sinaloa to 1.17 in Campeche. Conclusion. Geographic information, information on the link found between areas at high risk for transmission of infection due to the presence of the transmitting vector, and information on social conditions such as poverty are useful for designing strategies for the prevention and control of emerging arthropod-borne viral infections, such as the Zika virus, in Mexico. Background: the quality and continuity of nursing care depend on the quality of information flows between different contexts of care. Objectives: to analyze information relating to the nursing diagnosis and its dimensions, on the role of the care-giver who shared in the transition from hospital to community health care, and the quality of information flows established. Methods: we carried out a content analysis of data from 97 cases derived from the structure of nursing information sharing between the hospital and primary health care at the time of hospital discharge and first contact with nursing primary health care. Results: the documentation at the time of hospital discharge indicates the existence of a significant number of cases where the «Caregiver» emerged as a focus of the nurse, with implications for the process of continuity of care in the transition from hospital to community. However, the continuity of information flow in shared documentation on such cases is on many situations, reduced or nonexistent. Conclusion: the documentation focused on the development of knowledge and skills to carry out the role of career and there was no continuity in information flow in half the cases analyzed. In Ciudad Juarez, Chihuahua, Mexico, morbidity and mortality from injuries have increased alarmingly since 2008. This paper aims to examine the changes in the number of hospital discharges for external injuries recorded during the 2008-2010 period in a hospital in Ciudad Juarez. A descriptive retrospective study conducted at the Ciudad Juarez General Hospital looked at the incidence of external injuries as the reason for hospital discharges during the period under analysis. The average proportion of hospital discharges attributed to external injuries was 27%, with the 25-44-year-old age group being the most affected. More than half of the discharges were for fractures. The incidence rate of hospital discharges attributed to injuries in Ciudad Juarez was almost four times greater than that reported at the national level. [ABSTRACT FROM AUTHOR]
