3. Competencias legales de la Psicología Clínica en España: Actualización y guía práctica.

- Abstract
The practice of a profession takes place in the context of the assignment of a set of legal competencies. Since its recognition as a regulated profession, clinical psychology has undergone a process of evolution and consolidation. The aim of this work was to review the legal competencies of clinical psychology in Spain. A non-systematic narrative review of legislation, jurisprudence and bibliography has been carried out. The results were grouped into: 1) Assessment, diagnosis and psychological treatment; 2) Issuance of reports; 3) Referral to services/programs, indication of admission and discharge, and request for testing; 4) University teaching; and 5) Legal responsibilities. Clinical psychologists are recognized as having legal capacity for a wide range of clinical, management, teaching and research activities. Some activities are limited in practice and others have no established legal framework for their performance. [ABSTRACT FROM AUTHOR]

- Abstract
The evaluation of healthcare quality is a mandatory requirement for improving the services offered. However, there are few valid and standardized measures, especially in the field of clinical psychology care. The objective of this study is to develop and validate a quality-of-care measurement index from the perspective of clinical psychology specialists that allows for agile and easy interpretation of evaluation results. In this study, 83 clinical psychology specialists from the Andalusian Health Service participated. Through psychometric analysis, an index composed of four essential indicators was obtained, with excellent values of internal consistency and adequate fit indices for a unidimensional model. The resulting index is a useful, reliable, valid, and sensitive tool for monitoring the quality of clinical psychology interventions and evaluating improvement programs based on empirical evidence. The second activity is a series of clinical sessions from the clinical psychology, health, and psychotherapy section, where cases and work themes in psychotherapy will be presented. These sessions will be presented by various professionals in the field of psychology and will take place on different dates. In addition, the Official College of Psychology of Madrid is also organizing other events such as personal conversations, colloquiums, film forums, and conferences on topics such as the psychology of aging, neuropsychology, and sports psychology. In Mexico, there is a lack in academic research about bisexual Health-Disease-Attention process. This paper analyzes the main problems about health-disease for bisexual people with a Social Medicine and a Collective Health approach. In order to do this analysis, this paper offers a systematization of research's discoveries related to the Health-Disease-Attention process in bisexual people. Problems on health-disease for bisexual people include worse mental health conditions in comparison to gay and lesbian people. As psychologists have few contextualized knowledge about bisexual health-disease situations, then they cannot adequately respond to bisexual people needs. Bisexual Health-Disease-Attention process is an historical and social product where the lack of visibility and the negative valuation functions as mechanisms to regulate people's sexuality, which can be reproduced in Health-Attention places, such as Mental Health ones. Introduction: Mindfulness training has demonstrated positive effects on psychological health and is being used in a variety of clinical and therapeutic settings. Materials and Method: This quasi-experimental study examined the impact of Mindfulness practice on self-compassion, empathy, self-esteem, stress, anxiety, and Mindfulness awareness of undergraduate psychology students. Results: The results showed a significant increase in self-compassion and Mindfulness awareness, as well as a decrease in stress, depression and anxiety after the intervention. Positive correlations were found between Self-Compassion, Self-Esteem, empathy and Mindfulness awareness, and negative correlations with stress, anxiety and depression. Conclusions: This study highlights the importance of self-compassion and supports the incorporation of Mindfulness programmes in professional training for the development of essential therapeutic skills, such as self-esteem, empathy and Mindfulness awareness. Being overweight and obese have been frequently observed in women with infertility, which is a constant in more than 20 Latin American countries. Several studies propose that stress is essential for modifying body weight since it causes an increased food intake. In the investigation, 250 infertile women were evaluated and associated with the Body Mass Index (BMI). 80.4% of the participants were overweight or obese without sig-nificant stress levels. The regular BMI group has a higher risk in the healthcare subscales than overweight and obese women. Traditional clinical psychology can change social representations of overweight or obese women; they can become aware of risk factors and emphasize health care for women with infertility who present risks to their health. This paper is a theoretical reflection resulting from a case study presented in order to obtain the postgraduate diploma in Clinical Psychology with Psychoanalytic Orientation from Universidad San Buenaventura, Cali (Colombia). Its purpose is to inquire about the relationship and the difference between maternal ravages and maternal insanity, terms that arise when analyzing a case in which the participation was as a psychotherapeutic companion in a school context. The hypothesis is that maternal insanity refers to the opaque jouissance of the woman behind the mother, and the maternal ravages to the subjective effects that the maternal insanity left in the child. In this journey, it is argued that, although maternal insanity and maternal ravages are not comparable, both are united as a symptom in the mother-child relationship, inasmuch as they correspond to the unlimited feminine jouissance and the precarious or non-existent function of a third party that could mediate between such relationship. The literature referring to the perception of risk as a determining aspect for the consumption of marijuana, but there are no instruments for its objective evaluation. Aim. To design a battery of evaluation of the perception of risk towards the consumption of marijuana, for Colombian university students. Method. Instrumental study, a table of specifications was designed, which guided the construction of the items, then the validation was carried out by 12 expert judges, five in psychometry and seven in clinical psychology, the adjustments to the items, the application of the test to 520 university students (mean = 21.31; SD = 3.59) and the exploratory, confirmatory factor analyzes, and analysis of the internal consistency of the test. Results. A battery with three subtests (severity, absolute vulnerability, and relative vulnerability) was elaborated, with high variance explained by each scale, with models that present adequate goodness-of-fit indicators, high Cronbach's alphas (in .96, for each test) and correct correlations between item-item and between item-test. Conclusions. The battery developed for evaluating the perception of risk towards marijuana use is a valid and reliable battery for Colombian university students. [ABSTRACT FROM AUTHOR]

- Abstract
Background: Postpartum post-traumatic stress disorder (PTSD) has a prevalence of 3-4% in women, rising to 15-19% in the presence of risks during pregnancy or childbirth, and reaching 39% in the case of neonatal death. Perinatal complications can trigger a real or perceived threat to maternal or neonatal life, which can evoke intense emotional reactions equivalent to a traumatic stressor according to PTSD criteria. Four symptom clusters have been identified: re-experiencing, avoidance, negative cognitions and mood, and hyperarousal symptoms. Despite its high comorbidity with depression and anxiety, postpartum PTSD remains underdiagnosed in maternal settings. The Postpartum Posttraumatic Stress Disorder Scale, originally developed in England, is in the process of translation and global validation. Objective: To validate the Spanish adaptation of the City Birth Trauma Scale. Methods: Quantitative, nonexperimental, cross-sectional, descriptive-correlational study in a sample of 677 Argentine women with children under 12 months of age. Results: Exploratory factor analysis revealed two dimensions that explained 49.56% of the total variance. Confirmatory factor analysis supported the two-factor structure. The scale showed high internal consistency (total α = 0.903), with α = 0.872 for ‘birth-related symptoms’ and α = 0.886 for ‘general symptoms’. Construct validity of the City BiTS was demonstrated by moderate associations with the DASS-21 subscales and a negative correlation with the BIEPS-A. Conclusions: The Spanish adaptation of the City Birth Trauma Scale is confirmed as a reliable and valid instrument, consistent with original research findings and subsequent validations, articulating postpartum PTSD in two symptom categories: birth-related and general. 28. Diseño de un modelo de regresión para predecir la presencia de la depresión durante el embarazo a partir de la inteligencia emocional, el cuidado parental, la ansiedad y el estrés.

- Abstract
Background: Emotion regulation involves the modulation of emotional experiences to facilitate goal attainment. Conversely, emotional difficulties are a pattern of emotional experiences and expressions that interfere with goal-directed behavior. Objectives: Design a new model to predict the presence of depression in women during pregnancy. Methods: Non-experimental, cross-sectional, explanatory study of depression in women during pregnancy (logistic regression) considering the variables emotional intelligence, parental care, anxiety and stress. The sample consisted of 273 pregnant women-mothers between 14 and 38 weeks pregnant, aged between 18 and 38 years, for a mean of 25.67 years (SD=5.8). Results: The regression model is valid and significant in predicting the probability of occurrence of depression, explaining 82.4% of the variance of DV (Presence of depression) by the variables age, clarity and repair of depression dimensions. emotional intelligence, the maternal and paternal overprotection dimensions, and paternal care of the parental style variables; stress, work and single marital status. There is a 95.2% probability of success in the depression result when each of the model variables is incorporated. Conclusions: The best predictors of depression in pregnancy would be, on the one hand, higher levels or values of the variables and indicators age, reparation, maternal overprotection, paternal care, and stress, and on the other hand, low scores in the dimensions and values of clarity variables, and paternal overprotection; added to whether the woman works and is single. This combination of variables would be the individual and contextual conditions that influence said appearance. [ABSTRACT FROM AUTHOR]

Background. Early readmissions (between 24 hours and 30 days after discharge) can be disruptive for psychiatric patients and their families. The aim of this study is to determine the factors influencing this early readmission. Methods. A retrospective case-control study was carried out from 2015 to 2017. Cases were matched with controls with a similar age (± 10 years), admission date (± 30 days), ICD-10 diagnosis and sex. Sociodemographic, clinical and pharmacological factors were examined. Results. 86 cases and 86 controls were chosen. The largest ICD-10 group was F20-29 (70,9%). Statistically significant (SS) factors according to the bivariate analysis were: urban vs. rural living environment (p=0.011); poor subjective pharmacological adherence (AdFa, p=0.048); poor objective AdFa (p=0.023); poor social or family support (p=0.019); poor follow-up in psychiatric outpatient consultations (p=0.021); unemployment (p=0.046); and a higher number of readmissions during the previous year (p<0.001). In the multivariate analysis, SS factors were: urban living environment (OR=2.791; 95% CI, 1.017--7.663; p=0.046); poor social or family support (OR=2.255; 95% CI, 1.160--4.384; p=0.017); poor follow-up in psychiatric outpatient consultations (OR=2.156; 95% CI, 1.101--4.223; p=0.025); and a higher number of readmissions during the previous year (OR=1.536; 95% CI, 1.174--2.009; p=0.002). Conclusions. Living in an urban environment; poor AdFa; poor social or family support; poor follow-up in psychiatric outpatient consultations; unemployment; and a higher number of readmissions during the previous year were all directly related to early readmissions for psychiatric patients. Clinical psychology requires continuous research to encourage integrative explanations for the complexity of mental disorders and their underlying causes. Biological evolutionary approaches have shown particular heuristic power for this endeavor. Life history theory (LHT) is an evolutionary model that incorporates novel and significant theoretical and empirical advances. However, there is a growing need for the incorporation of other successful evolutionary approaches. Thus, the goal of the present paper is to propose potential integrative connections between evolutionary causal modes, behavior systems, and LHT. For this, borderline personality disorder is used as an example of a condition that can be understood as an interaction between stress and attachment psychobiological systems (proximate causes), within the framework of ultimate causes clarified by LHT. La posibilidad de realizar un diagnóstico y proponer un tratamiento psicoterapéutico, psiquiátrico o mixto, basándonos exclusivamente en la aplicación de pruebas psicológicas, resulta imposible; es necesaria una entrevista detallada y cuidadosa, realizada por un clínico que alcance a escuchar y no se concrete a oír, que observe y no se conforme con ver y además, que se permita sentir; resonar afectivamente con el paciente es necesario para comprender. Buscar una posición de absoluta objetividad, aleja al clínico de la subjetividad del paciente. Durante muchos años la enseñanza de la técnica en la entrevista psicológica se omitió, porque se consideraba un'don'llevarla a cabo con habilidad, destreza y conocimiento. En la actualidad, la entrevista forma parte del método clínico y resulta fundamental en todo programa académico de psicología, psicoterapia y psicoanálisis. Este libro es una guía para el aprendizaje de los estudiantes y una herramienta para los psicólogos dedicados a esta área de trabajo. La manipulación afectiva es moneda corriente y son muchos los que aún sostienen que, para lograr sus objetivos, el fin justifica los medios. Y esto se ha vuelto una constante utilizada sin considerar el daño que ocasiona ni sus consecuencias a mediano y largo plazo. Cuando esta manipulación se convierte en chantaje emocional, estamos hablando de una situación aún más tóxica en la que hay un dúo involucrado y ambos son partícipes necesarios para que esa dinámica se lleve a cabo. El chantaje emocional es una manipulación devastadora que tiñe de dolor, angustia, tensión permanente, sentimiento de culpa y obligación, involucra amenazas acompañadas de una lacerante violencia psicológica, emocional, muchas veces económica y hasta física. Sin embargo, no hay víctimas ni victimarios en estas situaciones siempre y cuando el vínculo se sostenga en el tiempo, ya que, para que eso suceda, se necesita de la participación de ambos. El proceso terapéutico va haciendo foco en que el paciente recupere su autoestima y su poder personal para hacerse responsable de su participación en ese circuito, desarme viejas creencias funcionales a ese tipo de vínculos, deje de sentirse una víctima de las circunstancias y se convierta en protagonista, creador y autor de su destino. Este libro es una aproximación para comprender y poner luz sobre este tipo de vínculos en los que el amor se confunde con apego, dependencia y necesidad. Luego de años de terapia y horas de trabajo con sus pacientes, la autora comparte información esclarecedora para aquellos que se encuentran atrapados en circuitos disfuncionales como estos. Los relatos que sus pacientes han compartirdo (de quienes se han cambiado los nombres para preservar su identidad) suman testimonios muy valiosos para comprender el origen, el desarrollo y la resolución de este tipo de circuitos patológicos.

Estudios en clínica psicodinámica: aportes al razonamiento clínico y a la intervención psicoterapéutica recoge los resultados de varias investigaciones del Grupo de Investigación en Estudios Clínicos y Sociales en Psicología de la Facultad de Psicología de la Universidad de San Buenaventura, Medellín. El lector podrá encontrar en un primer momento un modelo integrador de la clínica psicológica y un sustento al razonamiento clínico construido durante años de experiencia por la psicoterapeuta Mónica Schnitter Castellanos. Posteriormente, hallará discusiones en torno a: la noción de psicopatología del carácter, la incidencia del razonamiento clínico en el cambio psíquico, el lugar de la autonomía en el cambio psíquico, una propuesta comprensiva de intervención con adolescentes con deprivaciones afectivas tempranas, el uso de la metáfora como canal de resolución del conflicto psíquico y los cambios físicos que llevan a cabo algunas mujeres en los procesos psicoterapéuticos en relación con los movimientos psíquicos. Además de los temas centrales de cada capítulo, el libro puede leerse como un todo. Aquí, la calidad de la relación psicoterapéutica destaca como la estrategia más importante en la eficacia de los procesos psicoterapéuticos y se entrelaza al razonamiento clínico como método que permite la construcción entre lo general (la suma de la estructura psíquica, su caracterología y sus síntomas), lo particular (la subjetividad del paciente) y la intersubjetividad (la relación psicoterapéutica misma). Los compiladores consideramos que este libro constituye una obra de referencia destinada a aportar al ejercicio comprensivo de la psicoterapia y a la reflexión académica, que debe enriquecerse cada vez más en lo inagotable de la cuestión psíquica.
