The intervention of the stomatologist in these transdisciplinary teams is fundamental, since this treatment is longitudinal, biopsychosocial, and equitable, according to the oral-maxillary health needs of people with CLP, through different stomatological treatment protocols. Introduction: folic acid plays a crucial role in the epigenetic regulation of embryofetal development and its deficiency has been shown to be related to the appearance of different congenital defects. Objectives: to determine the time trend curves of five types of non-syndromic folatesensitive birth defects and to identify risk factors associated with them. Methods: an observational, analytical, retrospective, population-based case-control study was conducted in Villa Clara Province between 2013 and 2018. Prevalence rates of congenital defects among live births, stillbirths and elective terminations of pregnancy were determined and the analysis of the polynomial trend line of prevalence rates for the different types of congenital defects was performed. Results: a decreasing or stable trend was observed for cleft lip and palate, gastroschisis and Down syndrome, while a slightly increasing trend was observed for congenital conotruncal heart defects and neural tube defects. Conclusions: the discrete upward trend identified in the prevalence rates of congenital congenital heart disease and neural tube defects has a multifactorial origin, in which genetic predisposition and different risk factors such as deficient consumption of folic acid in critical periods of embryonic development are intertwined. - Abstract
Background and objective. Lip and nose symmetry are the goal of repair of unilateral cleft lip and different pre and postoperative treatments have been developed with this purpose. The objective of this study was to perform a review of the literature to evaluate the effects of postoperative nasal conformers on cleft lip and palate patients. Methods. We conduct a systematic review of the literature for studies published until March 2021 to evaluate aesthetic effect using nasal conformers after primary cheilorhinoplasty in patients with cleft lip and palate. Results. We identified 6 studies for the final analysis, which included 195 patients. The overall study quality according to Oxford CEBM and Newcastle-Ottawa scale was low. Conclusions. The results obtained from this study provides that placement of postoperative nasal conformers did not improve nasal symmetry in patients with unilateral cleft lip and palate. Based on available scientific evidence, definitive conclusions about the effectiveness of these devices on nasal symmetry after unilateral cleft lip nose repair cannot be drawn. Orofacial clefts represent a heterogeneous group of congenital malformations affecting different structures of the oral cavity and face. Overall, infants with these disorders have a higher lifetime morbidity and mortality compared to unaffected individuals. Therefore, advances in biomedical research are unavoidable. Thus, the overall objective of this work was to conduct a literature review to narratively analyse the 10 main primary studies on orofacial clefts carried out in Spain, published from 2018 to date. According to this review, at an institutional level, the Complutense University of Madrid (UCM) is notable with 4 articles published by the UCM 920202 research group. The Rey Juan Carlos University of Madrid also stands out, with three papers related to different aspects of the personality and quality of life of cleft patients, as well as many other cognitive-emotional variables. In relation to the University of Valencia, we found two studies carried out on large samples of cleft patients. Finally, in Barcelona, an observational study on otorhinolaryngological problems in cleft palate patients is noteworthy. In conclusion, although several studies have been published in recent years on different aspects related to clefts, there is still much work to be done. Spain should craniofacial development. Large, multicenter and collaborative studies are needed to delve deeper into the aetiological mechanisms and, ultimately, into the possible tools for their prevention. Similarly, support is needed to better elucidate questions related to treatments in all dimensions of health, preferably randomised controlled clinical trials, which facilitate the transfer of knowledge and its universal accessibility within the Spanish public health system. Palate formation occurs between the fifth and eleventh week of intrauterine life as a result of the union of the primary and secondary palate. On the other hand, the formation of the upper lip occurs between the fifth and sixth week of development, and is configured in its middle part by the fusion of the medial and lateral nasal processes, at the expense of the maxillary processes. The prevalence of cleft lips and / or cleft palate varies according to the different ethnic groups, with figures ranging from 0.7 to 1.1 cases per 1000 live births. The aim of this work was to carry out a literature review on epidemiological aspects, molecular and environmental genetic mechanisms that influence the occurrence of cleft lip, cleft palate and its embriology. The literature search was carried out in the databases PUBMED, SCOPUS, SPRINGER, SCIENCEDIRECT using the English terms "cleft lip and palate", "cleft lip", "cleft palate" and "embryology". Inclusion criteria included studies carried out in humans and animals, published between 2015 and 2021. The search yielded a total of 407 works, of which after a filter by title and abstract, a total of 38 articles remained, in which a text analysis was carried out complete to finally select 26 articles that cover genetic-molecular, environmental, epidemiological and syndromic topics. In addition, 6 documents associated with textbooks and scientific articles were incorporated by manual search, without considering the inclusion criterion of time. This review describes the strong association between oral fissures and mutations of genes Msx1, sonic hedgehog, bone morphogenetic proteins and fibroblast growth factor during migration of neural crest cells and palate shaping and formation. Lack of folic acid during palae development dar and the presence of hypoxia due to exposure to smoke, are the environmental factors most frequently observed in orofacial malformations. Introduction. Cleft lip and/or palate are malformations of the stomatognathic system, characterized by interruption of continuity of the tissues of the upper lip, alveolar ridge, bony palate and lateral or bilateral palatine mucosa, which can lead to abnormal aesthetics and phonetics. defective. Objective. To identify the prevalence of risk factors associated with the presence of lip and palate clefts in patients who attended the Hospital para el Niño Poblano. Methodology. A descriptive cross-sectional observational study was carried out, by collecting data in physical medical records of patients with cleft lip and palate (CLP) for the interpretation of the prevalence of associated risk factors. The main variables to be determined were analyzed in Excel tables: sex, place of birth, father's age, mother's age, gestation time, hereditary family history, associated syndrome, smoking, alcoholism and drug addiction. This information is presented in graphs. Results. In the analysis of the information, the relationship between the presence of the different risk factors associated with cleft lip and palate was identified; in the first analysis, it was found that the male sex had a higher percentage of appearance of this alteration with 55%, compared to the female sex with 45% of the studied population. Discussion. It was reported that the male sex presents 55%, compared to the female sex with 45%; similar results were informed by other authors, in which the male sex was 59% of the studied population showing a greater appearance of this alteration. Conclusions. The male sex showed a higher prevalence in this study (55%), term patients represented 89% of the population, hence this is a prevalence factor. [ABSTRACT FROM AUTHOR]

17. Factores de riesgo en pacientes con fisura de labio y paladar en México. Estudio en 209 pacientes.

- Abstract
Background and objective. Cleft lip and palate are associated with several factors including demographic and genetic factors among others. The estimated prevalence of the disease is 1-600 worldwide. Our aim is to study the relationship between the prevalence of cleft lip and palate and several factors associated to the disease in Mexico. Methods. A total of 209 patients with cleft lip and palate (isolated or combined) were included. Patients were referred from both public and private hospitals. Data were collected through direct interview and questionnaire with patientes' mother or parent. Results. In our study, 47.8% patients were female. Mean age was 8.9 ± 7.3 years old; 163 patients (78%) had a combined cleft-lip and palate, and 46 (22%) had an isolated cleft lip or palate. Mother's age was directly associated with the prevalence of cleft lip and palate. The history of facial clefts in the family was also directly related to the presence of a combined cleft, in contrast to an isolated cleft. (41.1% vs 26.1% respectively). Conclusions. In our study, both cleft lip and palate were directly related to the age of the mother. Thus, we believe that future studies should address the importance of preventive measures and treatment in the woman with a high-risk pregnancy (such as adolescent teens with pregnancy and elderly women). - Abstract
Cleft lip and palate is the most common craniofacial deformity, representing an international and national problematic well known, but treated by few surgeons. Due to recent negative experiences due to COVID-19, the Plastic Surgery medical staff of the Dr. Salvador Bienvenido Gautier in Santo Domingo (Dominican Republic), made it its task to return hope and positivity by returning smiles. Our main duty was to restore happiness and hope through surgical procedures to our population, greatly affected by the recent pandemic. The VII Surgical Operative for Cleft and Palate took place during November 5th -8th, 2020 with a multidisciplinary team conformed by: plastic surgeons, pediatricians, anesthesiologist, nurses, an organization and coordinating team, the administrative hospital staff, the public health department, the Dominican Plastic Surgery National Society, and the Dominican Medical College. We had 5 international surgeons from Mexico, 1 national invited surgeon, 14 staff members form the Plastic and Reconstructive Surgery Department, 12 residents from the Plastic and Reconstructive Surgery Department. Fifty three patients were evaluated and 37 were operated. The most common pathology was the combination of cleft lip and palate with a total of 15 patients, followed by cleft lip with 13 patients and cleft palate with 9 patients. [ABSTRACT FROM AUTHOR]

- Abstract
Background and objective. Cleft lip and palate are the most common craniofacial defects that require multidisciplinary management in a long term. Cleft lip presents in Mexico in 1 of every 750 new born. According to the National Health Secretary by 2004 there were 153.038 Mexicans affected by this disease. The state of Tlaxcala reports annually 25.000 new births, which represents 33 new cases. Our study includes activity of Cleft Lip and Palate Clinic of General Hospital of Tlaxcala and its impact on the social integration of the attended patients. Methods. We conduct an observational, descriptive, transversal and retrospective study to review the cases registered in our clinic from 2013 to 2019. Results. In Tlaxcala, the average scholarship of the population over 15 years is 9.3 years of formal study, which means a little more than middle school concluded. We can observe inside the population that is followed up inside the clinic, that the scholar shipper age group is above the media, and that 100% of the patients in follow-up are integrated to the society in an successful way. Conclusions. Our multidisciplinary experience, with a comprehensive approach to patients and long-term follow-up, offers a model of successful performance in countries where the incidence of cleft lip-palate is important. Cleft lip -with or without cleft palate- is the most frequently reported facial congenital malformation in the world that requires surgery -cheiloplasty- to be corrected. Unfortunately, the hypertrophic scar after cheiloplasty is a common result that negatively affects the patient, their social environment and even the specialists, because to date there is no specific treatment. In order to know the current status of the treatment of this problem, a literature review was conducted since 2000 in the different databases. It was found that the treatment and prevention options are similar: primary closure of the wound without tension, adhesive tape of microporous paper, injection of intralesional corticoid, silicone sheet or gel, flavonoid gel, and laser. It was concluded that the treatment of the hypertrophic scar after cheiloplasty is still unsatisfactory and the research is limited since it is not a replicable pathology in animal models. - Abstract
Background and Objective. For many plastic surgeons, procedure of election in the management of alveolar cleft is to take iliac crest as donor area as it is an easy surgical technique, with high availability of tissue and adequate reproducibility. The available literature regarding the use of olécranon as a donor area for alveolar cleft management is scarce. Our aim is to describe the results of olécranon cancellous bone graft management for patients with nasoalveolar cleft, compiling a more than 10-year experience performing this procedure and focusing on the low morbidity generated. Methods. We conduct a descriptive, cross-sectional study on patients with a diagnosis of unilateral or bilateral cleft lip and palate to whom olécranon bone grafts were performed preserving a bone cortical window to close de defect. Sociodemographic variables were collected such as age, sex, origin, diagnosis, number of grafts; and clinical variables such as surgery time, post-surgical pain, time of hospitalization, days of return to daily activities and complications. Patients were clinically evaluated at the first control 4 days after surgery. Results. Data were obtained from 111 patients, to whom 146 cancellous bone graft from olécranon were performed. Surgical time ranged from 1 to 1.5 hours. Return to daily activities ranged from 2 to 4 days after surgery. Five patients presented post-surgical pain in the donor area (grade III-IV in the pain scale according to the facies)., and only in the first and second time of grafts. Of those patients, 4 were diagnosed as grade III and 1 as grade IV according to Veau. Four returned to daily activities 4 days after surgery and 1 after 3 days. At the time of bone grafting, 1 patient was 4 years old, 2 were 5 years old, 1 was 6 years old, and another one was 8 years old. Conclusions. With the proposed modification we obtain only cancellous bone graft preserving cortical surface for ulterior close of the bone window created on the olécranon. Our serie of patients evidence a reduced surgical time, the presence of pain only in a low percentage of patients and their return to daily activities in 2-4 days after surgery. Among the most frequent congenital defects are cleft lip and/or palate (CL/P), with a prevalence of 1:1,000 live births. 70% of CL/P are non-syndromic, which means that they are found as an isolated defect without additional anomalies. They have a complex etiology with both an environmental and genetic component. By the development of human genome sequencing technologies have been identified polymorphic variants that may be associated with the CL/P phenotype and therefore may contribute to the multifactorial etiology of these. This review describes the commonly associated variants and their role in the etiology of CL/P. The SNPs located in the genes IFR6, MSX1, VAX1, PAX9, CHD1, FGF1, GREM1 and WNT3 have been significantly related to the presence of CL/P, and the variants located in the genes APC, GSK3, DVL2, BMP4, ABCA4, BHMT, NTN1, TBX1, EPHA3, FAM49A, MGMT, MMP3, TIMP2 y NOG, although its association with the presence of orofacial fissures has been reported, its relationship with this phenotype is not yet clear. It is important to carry out studies to identify genetic variants that involve specific populations in order to understand the etiology of non-syndromic CL/P. [ABSTRACT FROM AUTHOR]

- Abstract
Background and Objective. The lip and palate clefts are the most frequent congenital craneofacial malformations. Their treatment must be multidisciplinary and sequential. Most cases need more than one reconstructive surgery during the patient's childhood and can extend to additional surgery in adulthood. Dental, auditory, speech, facial growth and psychological management are required. This is why all of the professionals that work in a cleft team should always communicate to obtain the most successful results. The objective of the present study is to standardize the management of lip and palate fissures in patients treated at the clinics located in Tijuana and Ensenada, and soon Mexicali and Hermosillo, cities located in the Northwest of Mexico. Methods. The patients involved have been treated in clinics for clefts in Tijuana, Ensenada, and very soon in Mexicali and Hermosillo, cities located in the Northwest of Mexico. In the cleft lip and palate clinics in Tijuana and Ensenada, the collaboration between Plastic Surgery, Pediatric Odontology, Orthodontics, Otorhinolaryngology, Speech Therapy, Family Therapy, Child Psychology and other disciplines has been crucial to treat and follow up on patients during the past 10 years. The esthetic and functional results have been optimal when the surgery is complemented by these other disciplines. Results. For this approach, the cleft lip and palate patient has to be evaluated as early as possible, and parents must be instructed in nutrition and other special needs, and in getting the patient ready for the first surgery which in most cases is the lip repair and the second, if needed, is the palate repair. It always has to be emphasized that the pediatric odontologist and the orthodontist play an important role in molding the nasal cartilages as well as the maxillary segments before the surgery, as well as dental health. The speech therapist will have an important role when the patient's speech development starts and an early audiologic assessment will guide the treatment with ear tubes placed by the otorhinolaryngologist in order to avoid auditory and speech problems. Conclusions. During their lifetime, these patients may need other surgical procedures such as lip and nasal revisions, alveolar bone graft, pharyngoplasties, pharyngeal flaps, maxillary distraction or orthognatic surgery; this last one when there have been facial growth problems or malocclusions. Before any of these surgeries, the exchange of assessment and therapeutic plans by the multidisciplinary team is necessary to obtain the best results. [ABSTRACT FROM AUTHOR]

Background: Of congenital malformations, cleft lip with or without cleft palate is obvious upon inspection and they may occur isolated or as part of syndromes of diverse etiology. In Mexico, for the years 2008-2012, the prevalence of this malformation was 5.08 per 10,000 live births and stillbirths. Objective: To analyze the trends in the prevalence of live newborns with cleft lip (CL) and cleft lip-palate (CLP) in Mexico, at the state and municipal level, in the period 2008-2014. Material and methods: Descriptive and retrospective study in which databases of birth certifi cates in Mexico for the period 2008-2014 were used; percentages, ratios and prevalence rates were calculated. Results: A database of 14,842,105 births was obtained. The overall prevalence rate was 5.3; for CL, 2.6 and for CLP, 2.7 per 10,000 live births. Ratio male: female was 1.6: 1. Conclusion: The prevalence rates obtained in Mexico are lower compared with other countries. [ABSTRACT FROM AUTHOR]

Secondary cleft lip reconstruction is a very demanding reconstructive procedure, and our goal must be to achieve a complete restoration of lip function and appearance. The workhorse for secondary cleft lip repair is the Sbattini-Abbe flap. Several modifications to the original technique have been proposed to adress lip deformities found among these patients, which are not only found in the central segment of the upper lip, but in the lateral portions of the lip, and both in the vermillion and the cutaneous lip. We present a modification to the original Sabattini-Abbe flap, with the aim of reconstruction of all defects with a single flap and adhering to Plastic Surgery principle of recontruction of like with like. Resumen Introducción El padecimiento de labio paladar hendido es una de las alteraciones congénitas más comunes que afecta las estructuras de la cara. El objetivo de este trabajo fue generar el perfil epidemiológico y clínico de la población con LPH atendida en el Hospital de Especialidades del Niño y la Mujer Dr. Felipe Núñez Lara de la Secretaría de Salud del estado de Querétaro, México, en el periodo de 2011-2014, a través de la Clínica de Labio Paladar Hendido, para brindar tratamientos interdisciplinarios a los pacientes con esta afección con base en la información de los expedientes registrados en el periodo mencionado. Métodos Estudio observacional, transversal, retrospectivo usando análisis univariado con frecuencias para variables cualitativas, y estadísticas centrales y de dispersión para variables cuantitativas y perfil clínico. Se revisaron 100 expedientes; se descartaron 15 por tratarse de casos sindrómicos. Las variables estudiadas fueron socio-demográficas, epidemiológicas y clínicas. Resultados Se presentan los perfiles epidemiológicos (variables relativas al embarazo de la madre y salud del paciente al momento de su nacimiento, desarrollo nutricional y psicomotor; antecedentes heredo-familiares y presencia de adicciones; datos socioeconómicos familiares) y clínicos (clasificación del padecimiento por sexo, estructura y lado de afectación; clasificación de las cirugías y orden en que se efectuó el procedimiento) de la población considerada. Conclusiones Los resultados del estudio mostraron la necesidad de estandarizar el registro de datos en expedientes para mejorar el seguimiento y tratamiento de los pacientes y enfatizar en acciones preventivas que permitan mantener la baja incidencia del labio paladar hendido en Querétaro. Background One of the most common congenital disorders that affects the facial structures is the cleft lip palate (CLP). The aim of this study was to generate the clinical-epidemiological profile of CLP patients from Hospital de Especialidades del Niño y la Mujer (HENM) Dr. Felipe Nuñez Lara, from the Ministry of Health, Queretaro, Mexico, from 2011 to 2014, who received treatment from the Cleft Lip Palate Clinic in order to provide interdisciplinary treatments for CLP patients based on the information from the pediatric records. Methods Retrospective, cross-sectional, observational study using univariate analysis frequencies for qualitative variables; central statistical and dispersion for quantitative variables and clinical profile. One hundred records were reviewed, from which 15 were discarded for being syndromic cases. Epidemiological, clinical, and socio-demographic variables were studied. Results The epidemiological profile (variables associated with mother's pregnancy, patient's health at birth, nutritional and psychomotor development; family medical records, addictions, and socioeconomic factors) and clinical profile (disease classification by sex, structure, and side; surgeries classification and order in which they took place) of the treated population were registered. Conclusions The results showed the need to standardize the data registration on medical records to improve the monitoring and treatment of patients and emphasize actions to maintain low incidence of CLP in Queretaro. Introduction. The lip and palate remains a very topical issue, its comprehensive treatment demands a multidisciplinary, internationally well determined and applied attention. Objective. To determine the frequency of patients with (LPH) treated at the outpatient Central Military Hospital in the period from January 2009 to December 2013. Material and methods. Ambispective, descriptive and observational design rightful conducted with patients suffering from complete dossiers (LPH) seen in the outpatient clinic at the Central Military Hospital in a period from January 2009 to December 2013. Results. Records of 133 patients diagnosed (LPH), of which 59 patients were female (44.37%) were analyzed and 74 were men (55.63%). The frequency was reported (LPHUI) 44 cases with 17 women and 27 men being 33.1%, followed by (LPHUD) 38 cases with 13 women and 25 men (28.5%), the (LPHB) third with 29 cases 16 women and 13 men giving (21.8%). The (LH) found 4 males and 1 female corresponding to a (3.77%). Regarding the status of military and/or entitled 99 patients (DHMA) Troop 42 women and 57 men were found corresponding to (74.43%) followed by (DHMA) of officers with 22 patients 10 women and 12 men (16.54%) third (DHMA) chiefs 4 patients 2 women and 2 men accounts for (3.0%) of the total. Background: A retrospective transversal descriptive study was carried out with medical records of children with cleft lip and palate (CLP) seen at the Universidad del Valle's Hospital (Cali, Colombia) between 2002 and 2011. Purpose: To describe socio-demographic and maternal characteristics, pregnancy risks, and clinical characteristics of afected structures of children with CLP. Methods: 170 medical records of children with CLP (105 boys and 65 girls) were reviewed. A form was designed to collect information about associated syndromes, type of anomaly, medical background of the mother, socioeconomic status, and medications taken during the first trimester of pregnancy. Results: There were associated syndromes in 4 % of the patients. Left lip-palate fissure (LPF) was the most prevalent in both sexes. Hypertension was the main antecedent (9%) among mothers, prevalence of left FLP in both genders, medical history of hypertension in 9% of mothers. 42% of the population belonged to the stratum 1 (low-low) socioeconomic, which in Colombia is determined through the utility bills. There were not important findings regarding medications. Conclusions: Most of the cases of CLP were non-syndromic, males were most affected, and left unilateral FLP was the most prevalent. 38. Defectos del Tubo Neural y de Labio y Paladar Hendido: un Estudio Morfológico.

- Abstract
The objective of this study was to perform an external morphological evaluation of newborn (NB) and lactating children (LC) with diagnosis of neural tube defects (NTD) and cleft lip c/s palate (CL/s PH) to determine the type of defect, location, extent, classification and assess the proportion of infants with intrauterine growth retardation (IUGR) and malnourished infants. A descriptive study in 36 children from birth to 12 months of age, 20 with NTD and 16 with LH c / s PH was carried out. The study was conducted in five hospitals in the city of Chihuahua, Mexico. An external morphological assessment and anthropometry of children were performed. The DTN lesions were classified as open and closed, as defects high or low, depending on the model of multiple sites and ICD-10. The LH c / s PH were classified as unilateral or bilateral, complete or incomplete, and as isolated or multiple. IUGR was determined in the RN and malnutrition in infants. A statistic analysis was made with STATA 8.0 for Windows. We studied 20 cases of NTDs, 3 LCs and 17 RN. LH c/s PH were 8 LC and 8 RN. The DTN, 60% were myelomeningocele and corresponded to open lesions. Eighty five percent were located at high level. In the model of multiple closures, the Z1 was 80%. The LH c/s PH were more frequent with complete cleft (50%). The 35% of newborns with NTD had IUGR and 67% of LC had malnutrition. It is important to understand the mechanisms of development of congenital anomalies as this allows to specify the time the fault occurred and to study the underlying diseases to offer genetic counseling for possible prevention. The treatment and improvement of patients with bilateral lip and cleft palate (BLCP) requires multidisciplinary management, focused on providing optimal structural, functional, and aesthetic conditions so the patient can develop and achieve a better quality of life. The goal of the estomatological treatment is to help altered structures to reach a growth potential and suitable development improving results in surgical times, minimizing adverse effects. One of the treatments for nasal deformity in patients with BLCP is the uses of nasal splinting, which are attachments that work the nasal deformity using functional orthopedics; these help to: 1-avoid collapse and relapse of the nostril. 2-have more permeable airway. 3-eliminate surgical adhesions by secretions or contraction of healing. The aim of this review is present the case of a Female patient of 9-year-old with bilateral lip and cleft palate (BLCP), that presents nasal anomalies, like low nasal tip projection that use nasal retainer as a postoperative brace to improve the structural, functional and esthetic soft tissue deficiencies. An observational, descriptive and cross-sectional study was carried out in 98 patients with lip and palate malformations, attended by the multidisciplinary team of the Southern Children Hospital of Santiago de Cuba, from January 2000 to the same month of 2009, with the purpose of characterizing them according to clinical and epidemiological variates of interest to research. Standard deviation and percentage were used for calculating qualitative and quantitative variates, as well as the independence chi-square test to identify the statistically significant association between the criteria of interest. Congenital lip and alveolopalatal malformations (58.1%), females (55.1%), mother's diseases during pregnancy (18.4%) as the most common risk factor, and Santiago de Cuba municipality as the place of higher incidence predominated in the series. The cleft lip deformities present in a wide spectrum of variation and their treatment requires a very detail description of the morphology and a more individualized surgical treatment. Most of reference center around the world use one surgical technique or modifications of it, to address the different forms of cleft lip. During our first years of experience we repair all the cleft forms using only one technique (Millard for unilateral and Mulliken for bilateral) with good and bad results. This scenario stimulates us to classify the cleft lips and propose a new strategy for their surgical treatment, making them more individualized. The purpose of this study is to compare the surgical results obtained using two different protocols. This is an analytic and comparative study of 2 protocols: protocol A, using one surgical technique to address all cleft types, and protocol and B, using different surgical techniques according to our proposed classification based on number of bad results obtained using each one. To establish the type of result obtained we did anthropometric measurements in both sides of the repaired lip before the cleft palate repair under general anesthesia. In both cases, we have seen statistically significant differences in the number of bad results between the two protocols, on behalf of protocol B (p: 0.0001 and p: 0.002). We observed less number of bad results using the individualized protocol (protocol B) in the surgical treatment of unilateral and bilateral cleft lip showing the efficacy of the proposed classification and used techniques. The inhibition of the growth and development resulting of a surgical treatment in patients with cleft lip and palate is a widely discussed topic in the world. According to literature, tissue deficiency, probably due to the cleft width and position of the alveolar segments, is a considerable variable that affects the growth of the maxilla, which is also influenced by the surgical correction of the lip, the nose and the palate by scarring, types of surgical treatment, time of the surgery, surgeon skills and pre-surgical orthopedics. The purpose of this study was to determine the possible associations between the severity of cleft and maxillary growth in patients with non-syndromic unilateral cleft lip and palate, who were born in 2001 and treated at the Hospital Nacional de Niños "Dr. Carlos Sáenz Herrera", Caja Costarricense de Seguro Social, San José, Costa Rica. The study was retrospective and descriptive, based on data obtained from medical records, initial maxillary study casts of the newborn child, cephalometric radiograph and the present study casts of the child at the age of 5 years. The study sample comprised of 13 patients. The maxillary transverse arch of 12 cases was asymmetric, indicating an alteration of growth in this direction. There is a statistically significant positive correlation between the length of the arch and the SNA angle, indicating that a bigger length of the arch is related to a bigger SNA angle. The ANB mean value has a important deviation bigger than 2. There is no relationship between the position and length of the maxillary arch (SNA, BaNA and ANS-PNS) and the severity of the cleft. This means that the scar tissue, types of surgical treatment, time of the surgery, surgeon skills and pre-surgical orthopedics do not influence the anterior maxillary growth direction. In conclusion, the maxillary transverse asymmetry of the arch was the most important finding in this study. A new research study must be made in regards to the maxillary transversal dimension in children with cleft lip and palate, since this condition was affected. The anterior maxillary position and maxillary length were not influenced by the severity of clefting. It is important to consider, that the maxillary growth can be affected by individual factors, such as genetic facial pattern. A new evaluation should be delayed until the growth of the facial skeleton is complete. [ABSTRACT FROM AUTHOR]

Background: Cleft lip and cleft palate is the most common malformation of the head and neck of our environment, is a multifactorial disease determined by genetic and environmental factors. It occurs in one in every 900 live births, affecting males at a greater rate, today's advances in terms of proposed treatment from the multidisciplinary approach, starting with a strict evaluation by pediatric to ensure that patients are in the right conditions to initiate surgical procedures. Materials and Methods: A cross sectional study. We recorded demographic characteristics, clinical and treatment of 118 patients who attended the lip and palate clinic at Children's Hospital University of Manizales. Results: We found more frequently in males 55.1%, 51.7% from the urban area and most of the layers III and IV, the highest incidence is presented for cleft lip and palate grade III in 36.4% The most commonly used treatment regimen was palate surgery and cheiloplasty. Conclusions: The characterization of patients with cleft lip and cleft palate indicates predominance for patients with cleft lip and palate grade III, gender equality, urban hometown strata IV which has the highest frequency. Remarkable the importance of early diagnosis also appropriate multidisciplinary treatment, that has support in addition to the surgical staff will be reflected in a good performance and quality of life of patients [ABSTRACT FROM AUTHOR]

Cleft and palate is one of the most common congenital malformations, represents 2 - 3 % of these and is considered the most common craniofacial anomaly. Surgical repair of a patient with a cleft lip is not an emergency. Primary repair of cleft lip is done about 3 months old, consisting of a series of elective surgeries that are performed individualizing each case as required by the patient. The aim of this review is to address the innovative treatment alternatives that are being used in patients with cleft lip and palate, as hypodevelopment of occlusion, the diction and the growth of the jaws during childhood, also allowing correct swallowing to ensuregood nutrition. Introduction. Multiple techniques of repair of cleft lip have been developed. Achieving an excellent aesthetic result; However the surgical techniques to cleft lip repair have not been able to restore the function in the orbicularis muscle, obtaining plane and inexpressive lips. Material and methods. You forms a group of 6 rabbits New Zealand race from 2.5 to 3 kg of weight that underwent an electromyographic study to obtain the basal activity and the insert activity in a study pre and post operative (Cleft lip repair) to document electromyographic changes compatible with alteration in the function of the orbicularis muscle. Results. They underwent the results the test of t of Paired Student obtaining: The basal activity doesn't present a difference statistically significant to 95% of trust, in the opposing values (t = 2.23; 5 gl; p = 0.076), the insert activity presents a difference statistically significant to 95% of trust (t = 6.708; 5 gl; p = 0.001). Conclusion. Clinically the electromyographic changes demonstrate that a not specific inflammatory process exists, without functional damage after the cleft lip repair. [ABSTRACT FROM AUTHOR]

Objective. To determine the relationship between exclusive breastfeeding and fusion of palatal crests, in neonates with unilateral cleft lip palate. Material and Method. Longitudinal and cohort study, conformed of 2 groups: A) group of study: 4 neonates with unilateral cleft lip palate with exclusive breastfeeding; B) control group: 4 neonates with unilateral cleft lip palate with non-exclusive breastfeeding. To determine the level of fusion of the palatal crests a maxilla with an impression on 3 periods (start, day 10 and day 20) was registered. In each model obtained, the distance between the palatal crests was measured with a digital calibrate being the result obtained a representative one of the fusion level of palatal crests. Results. The level of fusion of the palatal crests in neonates with exclusive breastfeeding increased 2.44 mm from the start to the day 20. The level of fusion of palatal crests increased 2.28 mm in neonates with non-exclusive breastfeeding, from the start to the day 20. Although, in both groups the average difference wasn't statistically significative. Conclusions. The exclusive breastfeeding stimulates higher fusion of the palatal crests in comparison with the non-exclusive breastfeeding, although the difference is not statistically significative.
