In this communication we discuss the epidemiological panorama of dengue, which is being changed by influences of the ecological triad, from climatic changes and geographic hot spots that favor the interrelationship between arboviruses, vectors, and hosts, adjustments of the virus itself, with more serotypes predominating virulent and pathogenic than others, as well as the presence of other vector species with vec- tor capacity that ensure the distribution of the virus in regions not previously seen. Introduction: The characterization of Zika outbreaks is of great importance to determine the individual and collective risk factors involved in their occurrence and is the basis for the development of effective strategies for surveillance, prevention and control of Zika transmission. Objective: To describe epidemiologically the main characteristics of the first epidemic outbreak of Zika recorded in Panama, in the Guna Yala region. Materials and methods: A descriptive and retrospective study of the first Zika outbreak that occurred between 27 November 2015 and 22 January 2016 in Guna Yala, Panama, was designed and conducted. Results: A total of 68 serum samples were collected from suspected patients, 38 samples were positive by PCR for Zika in the Guna Yala Comarca, Ustupo registered the highest number of cases (n = 29, 76.3%), (n = 43; 86%) of the cases were younger than 59 years, (n = 37; 74%) of the cases were economically active population (14 to 65 years of age) and (n = 30; 60%) of the cases were female. Conclusion: This study has shown the ease with which arboviral diseases such as Zika can be introduced, spread and have an impact on the public health of the population, the health system and become established in the regions where these health events occur. Further studies strengthened epidemiological surveillance and consideration of risk factors are needed to ensure the sustainability of health interventions and to reduce the social and economic impact of this arbovirosis. In Cuba is verified the circulation of several flaviviruses such as Dengue, St. Louis encephalitis, West Nile, Zika, among others. Viruses of the Flaviviridae family are the cause of diseases in birds and mammals, including humans. Bats could be important reservoirs of these viruses; several flaviviruses have been isolated from infected bats, including St. Louis encephalitis virus and Japanese encephalitis. In addition, 26 species of bats inhabit the island, of which 10 can use architectural structures as daytime shelters, so their colonies could be in close contact with humans. However, until date, few studies have carried out related to the search for zoonotic viruses in these mammals or their ectoparasites. The objective of this study was the search for flaviviruses in bats in a peri-urban area of the Havana province. Fifty bats were captured, 40 classified as Artibeus jamaicensis (family Phyllostomidae) and 10 as Molossus milleri (family Molossidae). Samples taken from each individual were studied by reverse transcription polymerase chain reaction (RT-PCR) and nested (RT-nested PCR). The presence of flavivirus was determined in one of the captured individuals belonging to the species Molossus milleri. This finding reported for the first time in Cuba, is of great epidemiological importance since this bat is a synanthropic species, and therefore has direct contact with humans. The identification of the reservoir bat species could be an important advance to achieve an effective program in the control and risk reduction of arboviral diseases. [ABSTRACT FROM AUTHOR]

In Colombia, there is still little knowledge about the distribution of Aedes albopictus, an arbovirus vector, in rural areas. Its introduction into the country was first recorded in Leticia (Amazonas) in 1998 and from there its distribution has been expanded. The objective of this study was to determine the presence of Ae. albopictus in rural areas of some municipalities of Antioquia. In an active search for entomological surveillance of Aedes aegypti in some municipalities of Antioquia that are part of the area of influence of the Ituango Hydroelectric Project, specimens of Ae. albopictus were collected in the indoor, around the inside and outdoor of houses using manual aspirators with protected human bait for adult mosquitoes; the search for immature stages was also carried out on natural and artificial breeding sites. Specimens were identified using dichotomous keys at the Entomology Laboratory of the Facultad Nacional de Salud Pública de la Universidad de Antioquia. A total of 110 individuals of Ae. albopictus were collected, 55 larvae and 55 adults, respectively. The main breeding sites were the unusable ones. This finding is the first report in rural areas in eight municipalities of the department of Antioquia, which expands knowledge about the distribution of the Ae. albopictus vector and constitutes a potential risk in the emergence and transmission of diseases such as dengue, chikungunya and Zika. The chikungunya virus (CHIKV) is an Alphavirus that belongs to the Old World group. These arthritogenic viruses cause a febrile illness characterized by arthralgias and myalgias. Although fatal cases during CHIKV infection are rare, the disease may be disabling and generate a broad spectrum of atypical manifestations, such as cardiovascular, respiratory, eye, kidney, and skin complications, among others. When joint pain persists for three or more months, it results in the chronic form of the disease called post-chikungunya chronic inflammatory rheumatism, which constitutes the main disease sequel. CHIKV is not considered a neurotropic virus; however, it can affect the central nervous system, especially in children and the elderly, causing severe and permanent sequelae. CHIKV outbreaks had been previously reported in Africa, Asia, and Europe, but the virus introduction to the American continent was documented until the end of 2013. Since then, the virus has spread to 45 countries and territories causing near two million cases in just two years. This review describes the molecular biology, clinical manifestations, pathogenesis, and significant post-infection complications of CHIKV. Introduction: Arthropod-borne viruses (arboviruses) cause morbidity and mortality in humans and domestic animals worldwide. The percentage of population immunity or susceptibility to these viruses in Ecuador is unknown. Objectives: To investigate the proportion of Ecuadorian populations with IgG antibodies (Abs) (past exposure/immunity) and IgM Abs (current exposure) against flaviviruses and alphaviruses and to study the activity of these viruses in Ecuador. Materials and methods: During 2009-2011, we conducted a serosurvey for selected arboviruses in humans (n=1,842), equines (n=149), and sentinel hamsters (n=84) at two coastal locations and one in the Amazon basin (Eastern Ecuador) using enzyme-linked immunosorbent assay and hemagglutination inhibition test. Results: From 20.63% to 63.61% of humans showed IgG-antibodies for the flaviviruses: Dengue virus (DENV), yellow fever virus (YFV) Saint Louis encephalitis virus, and West Nile virus (WNV); from 4.67% to 8.63% showed IgG-Abs for the alphaviruses: Venezuelan equine encephalitis virus, eastern equine encephalitis virus, and western equine encephalitis virus. IgM-Abs were found for DENV and WNV. Equines and hamsters showed antibodies to alphaviruses in all locations; two hamsters seroconverted to YFV in the Amazonia. Conclusions: The results show a YFV vaccination history and suggest the activity of arboviruses not included in the current surveillance scheme. This study seeks to establish the applicability of the spatial analysis of the Aedes aegypti pupal index (PI) as a tool to evaluate the transmission risk of arbovirus in a specific locality. The PI and traditional infestation indexes of Aedes aegypti larvae were obtained for 366 households during the rainy season in 2018 in the La Gran Vía. With the data obtained, a geographic information system (GIS) was created, and the spatial interpolation of the variables included in the study was made. Some 1,211 inhabitants were counted, and 617 containers were inspected. According to the container classification, concrete tanks were the most frequent (51%) and the most productive - accounting for 93% of pupae. The estimated pupal index in the locality was 0.4. The spatial heterogeneity of the arbovirus transmission risk and its components is described. The implementation of the cartographic representation of the pupal index, in addition to an analysis of arbovirus transmission risk, according to the human vulnerability and the vectorial menace, provides important information for control and prevention strategies, and enables an enhanced management of resources by local authorities. This work explores in general the importance of Viral Ecology as a discipline will allow the viral influence on ecosystems. This topic of Virology analyzes the relationship between abiotic and biotic variables that have an effect on the presence in time and in space of the viruses, mechanisms of adaptation and resistance, virulence and replication in susceptible hosts. It states how the breaking between them due to economic, social, biological and mainly anthropogenic factors contribute to appearance of emergent infection diseases. It is accentuated the importance of zoonotic viral disease with RNA genome, especially arbovirosis, and the circulation of some of them in Cuba. The article titled "Contributions to the approach of dengue and emerging arboviruses of medical importance in Chile, through the creation of a One Health network" presents the advances in the approach to dengue and emerging arboviruses in Chile through the creation of a comprehensive health network. Dengue is the most impactful arbovirus for public health and in Chile it is native to Easter Island and was reintroduced in Arica in 2016. Despite the implemented public policies, the vector continues to advance and new strategies have not been successfully incorporated. The objective of the study was to develop an early surveillance observatory in entomovirology that contributes to the prevention and control of dengue, incorporating research, development, and innovation. The results include the development of a web platform that contributes to education and research, as well as the development of a device for dengue virus detection and the optimization of Aedes aegypti detection through artificial intelligence. Objective: To describe the clinical and laboratory characteristics of the exanthematous disease by chikungunya virus (CHIKV). Material and methods: Review of clinical fi les with diagnosis of CHIKV occurred in the Children's Hospital of the State of Sonora, during the year 2015. Results: Five cases were resolved without complications in the acute phase and one case developed arthritis and arthralgia with three months of evolution without presenting orthopedic sequelae. One case with mild thrombocytopenia, four children developed lymphopenia and one case with neutropenia, leukopenia and lymphopenia. Objective. Estimate seroprevalence, clinical case rate, and proportion of subclinical infections from chikungunya. Methods. A cross-sectional study was conducted in October 2015 at 39 sites distributed across Nicaragua. Demographic and clinical information was compiled through a personal survey. Blood samples were collected to detect chikungunya antibodies using the ELISA inhibition method developed by Nicaragua's National Diagnostic and Reference Center. Results were analyzed using generalized linear models and multilevel Poisson models. Results. A total of 11 722 participants aged >2 years were enrolled and 11 280 samples were processed. National seroprevalence was 32.8% (95% CI [95% confidence interval]: 31.9-33.6), with a clinical case rate of 26.5% (95% CI: 25.7-27.3) and a proportion of subclinical infections of 19.1% (95% CI: 17.8-20.4). Seroprevalence varied among the 39 sites and was greater at sites with higher vector infestation indices. Individually, seroprevalence was higher in participants aged >11 years. Conclusion. Since its introduction, this is the first study on chikungunya seroprevalence in continental Latin America to determine national prevalence, clinical case rate, and proportion of subclinical infections. Chikungunya fever is an acute febril disease related with severe polyarthralgias, caused by Chikungunya Virus (CHIKV) an arthropode borne virus endemic in tropical áreas, trasmitted to humans through the bite of the infected mosquito Aedes spp. In the last decade, the virus has arthralgias in the African and Asian continent, as well as the islands of the Indian Ocean, spreading to numerous regions as The Americas and Europe. This importation of the virus can be related to several factors as epidemics and the vector's large distribution globally. This is disease that presents a severe morbility and fatality which is why more understanding of the former is needed. Mosquitoes transmit yellow fever, a viral infection characterised by haemorrhage and jaundice. Currently, it is endemic in African and South American countries whereas our country has been declared free of the disease since 1959 following the latest outbreaks and epidemics occurred in coastal cities from both the Gulf of Mexico and the Pacific coast that were registered from the Colony until the middle of century XX. In 1881, Carlos J Finlay, who was a Cuban physician, exposed the hypothesis concerning the transmission of yellow fever by vectors; such theory was corroborated in 1890. In 1903, Mexico started working to eradicate the disease through control of mosquitoes. Finally, in 1923 Mexico achieved the control of the disease with the last urban case registered, whereas the last jungle case was recorded in 1959. However, due to the resurgence of the disease in our continent, it is important to provide the clinician with a comprehensive review of the disease and to raise awareness of the possible occurrence of imported or autochthonous cases in our territory. In this paper, the neurotropism potential Zika virus is discussed, by comparison with viruses both RNA and DNA are neurotropic known, also it is said that compared with the new viruses that have affected the Americas, as the chikungunya, Zika has shown great affinity by brain tissue, manifested by a high incidence of acute neurological conditions, such as Guillain-Barré syndrome, among others, as well as the reported incidence of microcephaly that is abnormally high compared with the previous incidence, which, in a stillborn subject necropsied significant alterations demonstrated in brain tissue, identifying viral material and live virus in the fetoplacental complex, and demonstrating the impact both white matter and gray matter as well as basal ganglia, corpus callosum, ventricles and spinal cord, which could explain the microcephaly that concerns him. Background: Zika virus was introduced to the country and threatens to spread, so the health personnel must be able to identify the disease and face an operational definition given case; in this review the clinical manifestations of probable cases of Zika of the last five years were described. Methods: A search was performed in Google Scholar and PubMed with "Zika". He settled for a database and obtained simple frequencies and calculated the limits for proportions with an alpha of 0.05 through test Wilson. Results: 109 probable cases of fever came together by Zika, the clinical manifestations was heterogeneous, with predominant involvement to musculoskeletal, dermatological and systemic level. Conclusions: It is necessary to continue the documentation of the clinical manifestations of Zika virus, which will be achieved by strengthening epidemiological surveillance. [ABSTRACT FROM AUTHOR]

Infection Zika virus is an emerging disease in the Americas region, which is caused by Zika virus (ZIKV), an arbovirus of the flavivirus genus. The ZIKV is transmitted by the bite of Aedes mosquitoes, both in urban and jungle area. After the mosquito bite, the disease symptoms usually appear after an incubation period of three to twelve days. The infection may be asymptomatic or presented with fever and not purulent conjunctivitis, headache, myalgia, arthralgia, asthenia, maculopapular rash, edema in lower limbs and, less frequently, retro-orbital pain, anorexia, vomiting, diarrhea or pain abdominal. Symptoms last for 4-7 days and are self-limiting. Neurological and autoimmune complications are rare. Since 2014 it has been detected native circulation of Zika virus in the Americas. So far, there is no specific antiviral treatment or effective vaccine, so it's giving priority symptomatic and supportive treatment for the acute phase and make an early diagnosis of atypical and severe forms. Diseases produced by arboviruses are a major worldwide problem due to the fact that the surveillance and prevention of them implies the surveillance of vectors, which makes it very difficult to control them, and almost impossible to avoid their expansion to any tropical or subtropical country, and even some temperate regions. Globally, dengue is the most common arboviral disease, with 40% of the world population living in areas with dengue virus transmission. Of the estimated 390 million infections and 100 million cases annually, a small proportion of these cases progress to severe dengue. Approximately one in 2,000 dengue cases result in death. However, the case-fatality rate of patients with severe dengue can be reduced from almost 10% to less than 0.1% by the timing and quality of clinical care that patients receive. This, coupled with the recent and quick spread of Chikungunya virus and the outbreak of Zika virus in Latin American countries including Mexico, makes it necessary the medical update and review of bibliography concerning the prevention, control and management of arboviral-caused diseases. Aedes albopictus, a mosquito species from Southeast Asia, is considered an important vector of dengue in some countries of that continent. It is also a vector of viruses and parasites causing diseases such as yellow fever, encephalitis, and filariasis. The presence of this mosquito in different cities of our country poses challenges for the control of diseases such as dengue. Objective: to analyze the relevant aspects of this vector's biology along with its importance in human health while providing suggestions for vector control. Methodology: a literature review was performed using the PUBMED and MEDLINE databases and an appropriate search strategy. As a result of this, 83 documents were selected. Results: Aedes albopictus has shown rapid distribution in Colombia since it was first recorded in the Amazon in 1998, and is now present in several cities. Additionally, the biology of the vector favors the establishment of this species in sites whose eco-epidemiological conditions are suitable for the transmission of dengue and urban yellow fever. Conclusions: The presence of Ae. Albopictus in Colombia poses new challenges in public health because this species has been suggested as a competent vector in the experimental transmission of a wide variety of arboviruses. Objectives: To know the etiologic profile in patients with febrile syndrome reported as negative for Bartonella infection or with a negative result in thick smears for malaria in two pilot areas (Northern Coast and Eastern Jungle in Peru). Materials and methods: A longitudinal and descriptive study was performed between May 2000 and July 2001 in four health facilities: Chiclayito and Salitral in Piura, Yurimaguas Hospital, and San Juan Health Center in Loreto. Febrile patients between 5 and 65 years old were included with negative smears for Bartonella or negative thick smears for malaria. The following tests were performed in the samples obtained: indirect immunofluorescence for typhus (only in Piura), ELISA IgM for leptospirosis and ELISA IgM-IgG for dengue fever, yellow fever, Mayaro, Oropuche and Venezuelan equine encephalitis (VEE). Results: Isolation of dengue fever virus was achieved in 27 (6,3%) patients, VEE virus was found in five patients, and group C virus was found in one patient. Dengue fever 2 and 3 viruses were identified in Chiclayito. VEE and group C virus were isolated in San Juan patients. Anti-dengue fever IgM was found in 43 (9,6%) patients, and yellow fever IgM was found in 21 (4,7%) patients, most of them from Yurimaguas. Also, VEE IgM was found in seven patients, Mayaro virus IgM was found in one and Oropuche virus IgM was found in four. Leptospirosis was the second most frequent cause of febrile syndrome (3,8%); while typhus was confirmed using indirect immunofluorescence in five Chiclayito patients. Conclusions: Studying the etiology of febrile syndrome allowed us to detect circulation of EEV virus, the presence of dengue fever 3 virus in Peru and also the prevalence of arboviral infections as a cause of febrile syndrome in the Peruvian Northern Coast and Eastern Jungle in the absence of epidemic activity. Aedeomyia squamipennis (Lynch Arribalzaga), Culex acharistus Root, Culex chidesteri Dyar, Haemagogus spegazzinii Brèthes, Mansonia titillans (Walker) and Psorophora holmbergi Lynch Arribalzaga are listed for the first time for the region. Introducción. El dengue hace parte de los arbovirus de mayor incidencia en Colombia. En el departamento del Meta, se han reportado casos por encima de lo esperado durante el 2019 y finales del 2018; el municipio de Guamal a la semana epidemiológica 33, además de alta incidencia en morbilidad, presentó una letalidad de 1 por cada 100 casos reportados. Objetivo. Determinar los conocimientos, actitudes y prácticas en torno al dengue en la población urbana del municipio de Guamal. Materiales y métodos. Se realizó un estudio transversal con análisis descriptivo, tomando una muestra aleatoria de 167 viviendas, con un margen de error del 8 %, recolección de datos mediante una encuesta CAP estructurada y observación de la vivienda. Resultados. Se encuestaron 167 personas, 70 % hombres (117), entre los 15 y 84 años; 47 % (77) de la población pertenecía a la EAPB capital salud; 56 % de los entrevistados pertenecían al estrato 2, y hubo un promedio de siete habitantes por vivienda y disponibilidad de servicios básicos en el 100 %. El 97 % había escuchado hablar del dengue, un 35 % a parientes, amigos y vecinos; el lavado de tanques (48 %) es una de forma de prevención, la fiebre (89,2 %) el síntoma más identificado, el 68 % lo identifica como un problema grave, el 63 % considera que el gobierno debe hacer control químico, y el 67 % fumiga frecuentemente. Conclusiones. Reconocimiento del vector, medidas de eliminación, protección, sintomatología y tratamiento, clasificado como un problema grave que requiere control químico por parte del gobierno, siendo la práctica más frecuente en el hogar.
