8. Implementación de guías de práctica clínica de la Asociación de Enfermeras de Ontario-RNAO: Una experiencia de enfermería en un hospital colombiano.

Introduction: The implementation or Clinical Practice Guidelines (CPG) is a strategy that whose goal is to increase the use of evidence in the context of decision making and building health policies. Nurses of The “Cardioinfantil-IC” Foundation (FCI-IC) established a collaborative agreement with the Registered Nurses Association of Ontario, Canada (RNAO), in order to implement and evaluate three of its GPC in adult inpatients. Objective: This paper describes the design, process and preliminary evaluation of an implementation plan between July 2012 and December 2014. Methodology: The selection of the CPG was made after an initial needs diagnosis and a feasibility analysis. Two CPGs were based on prevention: one about fall and another one about pressure ulcers (PU); and one more based on direct care related to assessment and management of diabetic foot ulcers. The indicators selected for CPG about prevention were frequency of evaluation and confirmation of risk and event rate (falls and UP). In the CPG about care was evaluated the percentage of assessed patients (lower limbs and feet) in admissions, the percentage of patient/family education, and the percentage of patients with foot injury reduction (50%). Results: The evaluation of the risk for falls and pressure ulcers with a reassessment ranging from 24 to 72 hours was performed in all patients that were admitted. There was a reduction in the use of guardrails, and a low reduction in falls. There was no change in the rate of pressure ulcers but the incidence of ulcers was limited to grade II. The assessment in lower limbs, the education, and care about foot ulcers in diabetic patients increased. Conclusions: The implementation process of GPC, as a result of the networking with the RNAO, has guided nursing care practice based on the assessment of risk and events. Some limitations in the level of evidence of some recommendations have promoted the development of a new research. Nowadays, balanced scorecards have updated traditional management systems in the business sector. In this way, Kaplan and Norton propose performance measurement through several perspectives with a logical sequence: internal processes and learning impact client services, so that financial performance is affected. The aim of the present paper is to analyze the main characteristics of balanced scorecard when it is applied to non-for-profit companies and, specifically to the health sector in the clinical nutrition field. This model improves the economic vision of management with clinical indicators that represent healthcare professional's perspective. The balanced scorecard would allow a proper monitoring and tracking system for the main healthcare indicators. This contributes to a better control in comparison with standards that are associated with adequate quality assistance. Owing to the role of management accounting and cost calculations, the definition of healthcare professionals as clients or users, and clinical results relevance, it is necessary to adapt the balanced scorecard to the specific characteristics of the clinical field, redefining both perspectives and indicators. Masseter muscle hypertrophy is an overgrowth of muscle mass with subclinical presentation and multifactorial etiology, with an incidence between 20 to 40 years and without sex predilection. In 1880 Legg describes this pathology for the first time and since then have been reported and attempted to formally multiple treatments, ranging from medical to surgical; medical treatment remains the most current and the most widely accepted is botulinum toxin. The objective of this paper is to review literature and to present a clinical case with unilateral masseteric hypertrophy, as well as the technique used and so provide a tool for the physician faced with masseter muscle hypertrophy. As diagnostic method we used CT of the head and facial mass with thin slices of 1 mm and of the masseteric region in the patient, and after that we performed botulinum toxin treatment according to the safety zone proposed by Nam-Ho Kim, based on anatomical sites indicated in the same protocol, with dose of 50 U at intervals of 3 months and measurement in the postoperative with CT images to evaluate the outcome of the applied treatment. Medical treatment with botulinum toxin are variable in dosage, applications and techniques so as in the products available in the market. In our case, the result was clinically satisfactory, covering the expectations required by the patient. We propose the use of protocols with patients, right dosages, time between applications, injection in correct anatomical sites, leading to the correction of the masseter muscle hypertrophy.
