The article presents an evaluation of the Sistema de selección de beneficiarios (Sisben) in its application to the subsidized health regime. It analyzes both the results of the subsidy assignation to the poor and the magnitude of the errors of inclusion and exclusion, as well as some changes in the behavior of different sectors of the population before the incentives promoted by the focalization and by the way it's implemented. It also examines the capacity of the Sisben index to select the population without capacity to pay, and some traits of the Sisben's state of application and it's instrumentation problems. It evaluates the grade of «focalization» achieved in the first subsidized regime, the results of coverage provision to the poor with the application of the ley 100 and the unfairness it has promoted in the access to health services. The article recognizes the contributions made by the «new» Sisben program the Departamento Nacional de Planeación has been implementing, abut it finds limitations in its orientation as its not placed in a context that looks for better long term solutions to carry out the focalization of health subsidies and to provide the instruments to the Sistema de seguridad social en salud, towards the unification of the services of the two regimes and the integration of the mechanisms to fix tariffs and partial subsidies. It criticizes the secretiveness surrounding the details of the methodologies of the «new» Sisben index, as it's soundness in legal terms is suspect, as well his effectiveness. With this procedure the reigning distrust is reaffirmed and rests legitimacy to the instrument, under the assumption that the population is passive and dishonest.
