- Abstract
A biosimilar (BS) is a biological drug that contains a version of the active substance of an already authorized original biological product. The BSs are marketed after patent period of the original drug has ended and once it has been demonstrated that the differences regarding the innovative medicine have no relevant effect on its safety or clinical efficacy. The Spanish Society of Rheumatology, in line with the European Medicines Agency, considers that because of its nature and complexity of production, a BS cannot be considered to be the same as a generic drug. The Spanish Society of Rheumatology expresses an unequivocal commitment to the sustainability of the health system in our country and our steadfast alignment with all measures designed to ensure continuity, without reducing the quality of care. Therefore, we believe that the advent of BSs will likely facilitate access of patients with rheumatic diseases to the biological drugs. This article reviews the European Medicines Agency requirements for authorization, the Spanish legal framework and controversies on BS and presents the position paper of the Spanish Society of Rheumatology on these drugs. Resumen Introducción El auge de la telemedicina en el campo de la salud mental está haciendo que el uso de instrumentos psicométricos, tradicionalmente basados en un soporte de «lápiz-y-papel», se adapte al formato electrónico. El objetivo de este trabajo es verificar si los 2 formatos de conocidos instrumentos como las escalas GHQ-12, WHO-5 y PHQ-9 son intercambiables. Material y métodos Cuarenta y siete voluntarios completaron los cuestionarios GHQ-12, WHO-5 y PHQ-9 en papel y en las siguientes 24 h en su versión electrónica a través de la web www.memind.net (24 en formato electrónico-Likert y 23 en formato electrónico-slider). Se midió la consistencia interna mediante el índice α-Cronbach y el coeficiente omega, la fiabilidad test-retest mediante el coeficiente de correlación intraclase (CCI), el grado de acuerdo de los ítems mediante el coeficiente kappa ponderado y comparamos la estructura dimensional de los 2 formatos con el índice de ajuste comparativo (IAC). Resultados La consistencia interna de los instrumentos fue mayor de 0,8 para todos los instrumentos a excepción del PHQ-9. Los CCI oscilaron entre 0,655 para PHQ-9 lápiz-y-papel/electrónico-slider y 0,901 para GHQ-12 lápiz-y-papel/electrónico-slider. El acuerdo entre los ítems en las versiones en lápiz-y-papel y electrónica-Likert fue variable, desde acuerdos muy bajos en el del ítem PHQ-1 (κ ponderada = 0,143; p = 0,384) hasta acuerdos altos en el ítem PHQ-5 (κ ponderada = 0,769; p = 0,000). La equivalencia arrojada con el IAC fue adecuada. Conclusiones Con la excepción del PHQ-9 electrónico-Likert, los cuestionarios mantienen su estructura en la versión electrónica. Encontramos discrepancias en el acuerdo entre los ítems, lo que apoya la idea de que el paso de un instrumento diseñado en papel al medio digital no es un proceso automático, sino que requiere un proceso de adaptación y de verificación sobre el tipo de diseño en formato electrónico. Introduction The increase in telemedicine in the mental health field has led to psychometric instruments changing from paper-and-pencil administration to an electronic format. A study is performed to determine if both formats are equivalent for well-known questionnaires such as GHQ-12, WHO-5, and PHQ-9. Material and methods Forty-seven volunteers completed GHQ-12, WHO-5 and PHQ-9 questionnaires in paper-and-pencil format, and in the following 24 h they completed their electronic versions via the web site www.memind.net . An electronic-Likert format was used by 24 participants, and 23 used an electronic-slider format. Internal consistency was measured by α-Cronbach index and omega coefficient, and test-retest was measured by the intraclass correlation coefficient (ICC). Agreement between individual items was compared using Weighted Kappa coefficients, and dimensional structure between formats using the Comparative Fit Index (CFI). Results Internal consistency was higher than 0.8 for GHQ-12 and WHO-5. The ICC ranged between 0.655 for PHQ-9 paper-and-pencil/electronic-slider and 0.901 for GHQ-12 paper-and-pencil/electronic- slider. Agreement for individual items in paper-and-pencil and electronic-Likert versions was variable, ranging from low agreement in PHQ-1 (weighted κ = 0.143; P = .384) to high agreement in PHQ-5 (weighted κ = 0.769; P = .000). The CFI results showed an adequate equivalence between formats. Conclusions Except for the PHQ-9 electronic-Likert, questionnaires keep their structure in electronic formats. Discrepancies were found in items agreement. This study supports previous works indicating that the change from paper-and-pencil to electronic formats is not an immediate process, and needs a proper adaptation. [ABSTRACT FROM AUTHOR]

6. Presencia de la Medicina Nuclear en revistas españolas de Medicina Interna y de otras especialidades (2000-2009).

Resumen Objetivo Describir cuantitativa y cualitativamente las publicaciones sobre Medicina Nuclear (MN) en revistas ajenas a la especialidad, entre los años 2000-2009. Material y métodos Estudio descriptivo retrospectivo durante 2000-2009 en 3 revistas de Medicina Interna (MI) y en 3 de especialidades afines (EA). Los criterios utilizados son que algunos de los autores esté ubicado profesionalmente en un servicio, unidad o centro de MN y/o que el título del artículo o al menos su contenido haga alusión a algún aspecto específico de la MN. Se ha recogido la fecha de publicación, sección de la revista, temática, datos de autores, provincia y hospital de referencia. Resultados Se han encontrado un total de 186 artículos, 81 en revistas de MI y 105 en EA. Los artículos de las revistas de MI se originaron en 43 hospitales/centros españoles. El de mayor producción fue Vall d’Hebron. Se han identificado 24 provincias, destacando Barcelona con 20 artículos y Madrid con 17. En las revistas de EA se hallaron 59 hospitales/centros, destacando el Vall d’Hebron con 21 artículos. Hay 9 artículos extranjeros. Se distribuyen en 19 provincias, destacando Barcelona con 32 trabajos y Madrid con 20. Conclusiones Los trabajos en las revistas de EA duplican, al menos, los de las revistas de MI. Los «originales» son los más frecuentes. Destacan la revista Clinical and Translational Oncology en EA y Medicina Clínica en MI. No hay una temática que destaque. Barcelona y Madrid son las provincias más productivas. Por hospitales el Vall d’Hebron es el más productor. Objective This article aims to provide a quantitative and qualitative description of the publications on Nuclear Medicine (NM) in journals from other disciplines, between 2000 and 2009. Material and methods A retrospective descriptive study was carried out including the years 2000-2009 in three internal medicine journals (IM) and in three related specialty journals (RS). The criteria used are that some of the authors were located professionally in a Service, Unit or Central MN and/or that the title of the article or at least its content made a reference to some specific aspect of NM. Date of publication, the magazine section, thematic, data of the authors, province and referral hospital were collected. Results A total of 186 articles were found, 81 in IM journals and 105 in RS. The IM journal articles came from 43 different hospitals. Vall d’Hebron (Barcelona, Spain) was the hospital with the largest volume. Twenty-four provinces were identified, Barcelona and Madrid standing out among them with 20 and 17 articles, respectively. In the RS journals, 59 hospitals/centers had participated, Vall d’Hebron standing out with 51 articles. There were 9 foreign articles. The articles were distributed into 19 provinces, Barcelona and Madrid standing out with 32 papers and 20 papers, respectively. Conclusions There are at least twice as many articles in the RS Journals than in the IM ones. «Original» articles are the most frequent. The Clinical and Translational Oncology journal in RS and Medicina Clínica in IM stand out with the highest number of articles. No specific topic prevailed. [ABSTRACT FROM AUTHOR]

Resumen Las técnicas de valoración cualitativa han sido el estándar tradicional de valoración diagnóstica en los estudios PET con 18 F-FDG. Desde los inicios de la técnica se han buscado parámetros cuantitativos, más exactos y con mejor precisión diagnóstica, que puedan ofrecer información relevante en cuanto al comportamiento, agresividad o pronóstico de los tumores. Cada vez hay más estudios con evidencia de alta calidad sobre la utilidad de diferentes parámetros metabólicos distintos al SUV máximo, cuyo uso es controvertido, a pesar de ser ampliamente utilizado en la práctica clínica, y del que muchos médicos desconocen todavía su significado real. El objetivo de este trabajo ha sido revisar los conceptos clave de estos parámetros metabólicos que pueden ser relevantes en la práctica habitual durante los próximos años. Se ha visto que existe mayor evidencia en la valoración del metabolismo de una lesión de forma completa a través de parámetros volumétricos que reflejan de forma más adecuada la carga tumoral que presenta un paciente. Básicamente, estos parámetros calculan el volumen de tumor que cumple unas características determinadas. Para ello se ha utilizado un software , disponible en la mayor parte de las estaciones de trabajo y procesado de estudios PET, que ha permitido calcular estos volúmenes utilizando criterios más o menos complejos. Los métodos de segmentación más simples, basados en umbrales, están disponibles en la mayor parte de los equipos, son fáciles de calcular y han demostrado en muchos trabajos tener un importante significado pronóstico. Qualitative techniques have traditionally been the standard for the diagnostic assessment with 18 F-FDG PET studies. Since the introduction of the technique, quantitative parameters have been sought, more accurate and with better diagnostic precision, that may offer relevant information of the behavior, aggressiveness or prognosis of tumors. Nowadays, more and more studies with high quality evidence show the utility of other metabolic parameters different from the SUV maximum, which despite being widely used in clinical practice is controversial and many physicians still do not know its real meaning. The objective of this paper has been to review the key concepts of these metabolic parameters that could be relevant in normal practice in the future. It has been seen that there is more evidence in the complete evaluation of the metabolism of a lesion, through volumetric parameters that more adequately reflect the patient's tumor burden. Basically, these parameters calculate the volume of tumor that fulfills certain characteristics. A software available in the majority of the workstations has been used for this purpose and it has allowed to calculate these volumes using more or less complex criteria. The simplest threshold-based segmentation methods are available in most equipments, they are easy to calculate and they have been shown in many studies to have an important prognostic significance. The aim of this descriptive study is to analyze the scientific productivity of doctorate programs in Clinical and/or Health Psychology in Spain. That productivity is assessed from teachers of doctorate programs. A second aim is to find out whether doctorate programs with quality mention have a higher scientific productivity than those programs without quality mention. Scientific productivity of 375 teachers that participated in 17 doctorate programs was examined. Specifically, papers published in journals included in Web of Science and IN-RECS data bases were analyzed as well as the number of dissertations registered in TESEO data base. A total of 2,772 papers of Web of Science, 2,479 papers of IN-RECS and 604 dissertations included in TESEO were obtained. According to these results, a ranking of doctorate programs was made for each criterion. Subsequently, a global ranking was made through adjusting each criterion according to the weight that was assigned by scientific community to each indicator. Finally, a comparison of global results of programs with and without quality mention was carried out. Results showed that programs with Quality Mention doubled scientific productivity in all criteria compared to programs without Quality Mention. [ABSTRACT FROM AUTHOR]

Abstract: The most recent results in the dynamics of infinite systems of particles have shown there is no necessary connection between the performance of a deterministic supertask and the non-conservation of energy. Much more difficult to prove is the non-existence of any necessary connection between the performance of an indeterministic supertask and the non-conservation of energy; however, this paper does just that. So the requirement that energy be conserved does not mean we can exclude as “non-physical” either of the two major types of supertask considered to date. The paper also proposes, in this context, a “hidden variables method” to prove general results of compatibility in dynamics. [Copyright &y& Elsevier]

Resumen El cáncer de próstata (CaP) representa el tumor maligno más frecuente en los varones, pero según las directrices de la European Association of Urology (EAU) no deben realizarse cribados masivos para el diagnóstico de CaP debido a problemas relacionados con el sobrediagnóstico y sobretratamiento. El diagnóstico clínico precoz es posible, principalmente basado en el tacto rectal y la determinación del antígeno prostático específico (PSA). Sin embargo, el único test que puede determinar la presencia de un CaP es la biopsia guiada por ecografía, obteniendo múltiples muestras, la cual tiene un elevado valor pronóstico. En este contexto, la imagen diagnóstica juega un importante papel tal como lo confirmó la EAU, que en una actualización de 2016 de su guía clínica sobre CaP estableció la importancia de la tomografía por emisión de positrones (PET) con 11 C- o 18 F-colina combinada con la tomografía computarizada (TC) para individualizar la recidiva local, la afectación de ganglios linfáticos y la diseminación metastásica en todos los estadios. En consecuencia, en 2017, la European Association of Nuclear Medicine (EANM) junto con la Society of Nuclear Medicine and Molecular Imaging (SNMMI) compartieron nuevas directrices para la PET/TC con 68 Ga-antígeno de membrana prostático específico (PSMA) para ayudar a los médicos en la recomendación, realización e interpretación de los estudios PET/TC en pacientes con CaP. De esta manera, el objetivo de este «artículo de evidencia» es definir el algoritmo diagnóstico actual en el CaP para incrementar el nivel de confianza global en el enfoque de un tema tan crucial. Prostate Cancer (PCa) represents the most common malignant tumor in men but according to the European Association of Urology (EAU) guidelines, a mass screening for PCa diagnosis should not be performed due to over-diagnosis and over-treatment related problems. An early clinical diagnosis is possible, mainly based on digital rectal examination and Prostatic Specific Agent (PSA) testing. However, the only mandatory test to define the presence of PCa is ultrasound guided-biopsy, obtained on multiple samples, which has also a high prognostic value. In this context, diagnostic imaging plays an important role as confirmed by EAU that in a 2016 update of their guidelines on PCa stated the importance of Positron Emission Tomography (PET) with 11 C- or 18 F-choline combined with computed tomography (CT) to identify local relapse, lymph node involvement and metastatic spread at all stages. Consequently, in 2017, the European Association of Nuclear Medicine (EANM) together with the Society of Nuclear Medicine and Molecular Imaging (SNMMI) published new guidelines for 68 Ga-Prostate Specific Membrane Antigen (PSMA) PET/CT to help physicians in the recommendation, execution and interpretation of PET/CT scans in patients with PCa. Thus, the aim of this ‘evidence paper’ is to define the current diagnostic algorithm in PCa in order to increase the general level of confidence in approaching such a crucial topic. 13. Gammagrafía de tránsito gastrointestinal en el estudio del estreñimiento crónico en edad pediátrica; una exploración infrecuente.

Resumen Objetivo El estreñimiento crónico es una enfermedad común en el niño. Nuestro objetivo fue poner de manifiesto la utilidad de la gammagrafía de tránsito gastrointestinal en el estudio de pacientes pediátricos con estreñimiento crónico y las ventajas que tiene respecto a otros estudios de imagen, a pesar de nuestra limitada experiencia. Material y métodos Se valoraron 5 pacientes remitidos a nuestro servicio con diagnóstico de estreñimiento crónico refractario al tratamiento. Se realizó un protocolo de estudio completo, incluyendo gammagrafía de vaciamiento gástrico para líquidos y tránsito de intestino delgado y grueso, utilizándose una única dosis de 111 In-DTPA. Siguiendo guías internacionales se definieron áreas de interés en estómago, íleon terminal y en 6 regiones del intestino grueso. Resultados Todos los enfermos presentaron estudios gammagráficos alterados, mostrando 4 de ellos exploraciones radiológicas normales. El estudio radioisotópico cambió el diagnóstico del paciente en 2 casos y en otros 2 contribuyó a aclararlo, ante la discordancia entre pruebas radiológicas normales y biopsia rectal patológica. Uno de los pacientes mostró concordancia entre los 2 estudios de imagen. Tras la gammagrafía se produjo cambio en el manejo terapéutico en 2 casos. Conclusiones Nuestra limitada experiencia coincide con los datos publicados en los que el estudio gammagráfico resulta ser un método reproducible y preciso. Proporciona una información fisiológica, cuantitativa y útil en el estudio del estreñimiento, siendo la única exploración que permite la determinación tanto global como regional del tiempo de tránsito gastrointestinal. Aim Chronic constipation is a common pathology in children. The aim of this paper was to show the usefulness of gastrointestinal transit scintigraphy in pediatric patients with chronic constipation, and the advantages with respect to other imaging techniques, despite our limited experience. Material and methods We evaluated 5 patients sent to our service with a diagnosis of chronic constipation refractory to treatment. We performed a complete study protocol, including liquid gastric emptying scintigraphy and small and large bowel transit times, using a single dose of 111 In-DTPA. Following international guidelines regions of interest were defined in stomach, terminal ileum and in 6 regions of the large intestine. Results All patients showed altered scintigraphy study, showing 4 of them normal radiological tests. Radioisotopic study changed diagnosis in 2 patients and in other 2 patients contributed to clarify it, since discordance between normal radiological tests and abnormal rectal biopsy. One of the patients showed concordance between each imaging modality. The results of the test changed the therapeutic management in 2 cases. Conclusions Our limited experience coincides with published data in which scintigraphy study turns out to be a reproducible and accurate method. It provides physiological, quantitative and useful information in the study of constipation, being the unique exploration that allows both global and regional gastrointestinal transit time determination. 15. Ausencia de conflicto neurovascular durante la microdescompresión vascular en el manejo de la neuralgia trigeminal esencial: ¿qué hacer? Revisión sistemática de la literatura.

Resumen Introducción La presencia de conflicto neurovascular es la hipótesis aceptada en el desarrollo de la neuralgia trigeminal y la microdescompresión vascular, la técnica quirúrgica más empleada en su tratamiento. Sin embargo, pese a los avances diagnósticos, en ocasiones dicho conflicto es indetectable intraoperatoriamente. El objetivo de este trabajo es revisar sistemáticamente las opciones de manejo ante dicha situación. Resultados Existen diversas técnicas descritas que se pueden emplear ante la ausencia de conflicto neurovascular durante una microdescompresión vascular. Describimos dichas técnicas, sus tasas de éxito, la recurrencia del dolor y de complicaciones, y damos nuestras recomendaciones sobre la materia. Conclusiones No existe un consenso terapéutico, pero se dispone de diversas opciones quirúrgicas ante la ausencia de conflicto neurovascular. Creemos que las técnicas destructivas como la RPS deben relegarse a última línea terapéutica. Introduction Neurovascular conflict is the most accepted hypothesis for the cause for trigeminal neuralgia. Microvascular decompression of the trigeminal nerve is the most common surgical treatment for these patients. However, despite advances in diagnostic techniques, neurovascular conflict is sometimes not detected during surgery. The aim of this paper is to systematically review all the options available to best manage this scenario. Results Several techniques that could be used during microvascular decompression for trigeminal neuralgia in the absence of neurovascular conflict have been described. The success rates of these techniques, pain recurrence rates and rates of complications are also reported. Finally, we provide suggestions based on our experience. Conclusions There is no gold standard, but several techniques could be successfully used in the absence of neurovascular conflict. The use of destructive techniques, such as PSR, should be held as treatments of last resort. [ABSTRACT FROM AUTHOR]

Uno de los aspectos que ha alcanzado mayor sofisticación es el relacionado con el estímulo eléctrico, un conocimiento que es preciso consensuar, extender y llevar a nuestra práctica. Se revisa en este trabajo la información disponible sobre frecuencia y número de sesiones de TEC, y la utilidad de las distintas localizaciones de electrodos; se describen y valoran los métodos de cálculo de la carga a emplear, titulación o cálculo directo con base en la edad, y se destaca la insuficiencia de las medidas globales de magnitud del estímulo, siendo preciso ampliar nuestro conocimiento sobre los parámetros que configuran la carga: tiempo, intensidad, frecuencia y amplitud de pulso. The process of normalization electroconvulsive therapy (ECT) requires, among other actions, disseminating the latest information on this technique. One of the most complex aspects is the electrical stimulus, whose knowledge should be spread and put into practice. In this paper we review the available information about frequency and number of ECT sessions, and efficacy of each electrode placement. We also present two approaches to determine the ECT charge: stimulus titration versus age-based method; and the limitations of the summary metrics of charge, being necessary to expand our knowledge of the parameters that configure the stimulus: duration, current amplitude frequency and pulse width. Resumen El uso de medicación antitrombótica (antiagregante y/o anticoagulante) se encuentra ampliamente extendido. El manejo de los pacientes neuroquirúrgicos que reciben este tipo de terapia continúa siendo, a día de hoy, un problema de especial importancia. Los pacientes en tratamiento antitrombótico pueden necesitar atención neuroquirúrgica bien por presentar sangrados secundarios a dicho tratamiento, lesiones neuroquirúrgicas no hemorrágicas pero que precisen intervención urgente, o simplemente procedimientos neuroquirúrgicos electivos. Además, las consecuencias de la reintroducción de la anticoagulación temprana (sangrado o resangrado) o tardía (trombóticas o tromboembólicas) pueden ser devastadoras. En este documento presentamos el protocolo de consenso en el tratamiento antitrombótico durante el periodo perioperatorio y periprocedimiento, tanto en cirugía emergente como en procedimientos electivos de neurocirugía. The use of antithrombotic medication (antiplatelet and/or anticoagulant therapy) is widespread. Currently, the management of neurosurgical patients receiving this type of therapy continues to be a problem of special importance. Patients receiving antithrombotic treatment may need neurosurgical care because of bleeding secondary to such treatment, non-haemorrhagic neurosurgical lesions requiring urgent attention, or simply elective neurosurgical procedures. In addition, the consequences of reintroducing early (bleeding or rebleeding) or late (thrombotic or thromboembolic) anticoagulation can be devastating. In this paper we present the antithrombotic treatment consensus protocol during the perioperative and periprocedural period, both in emergent surgery and in elective neurosurgical procedures. 18. Tiempo de reacción, velocidad de procesamiento y atención sostenida en esquizofrenia: impacto sobre el funcionamiento social.

Resumen Introducción Estudios previos han relacionado la velocidad de procesamiento con la funcionalidad y la cognición social de los pacientes con esquizofrenia. Se necesita un análisis más discriminativo de los diferentes componentes de este constructo neuropsicológico. El presente trabajo analiza el impacto de la velocidad de procesamiento, del tiempo de reacción y de la atención sostenida sobre el funcionamiento social. Material y métodos Un total de 98 pacientes ambulatorios de entre 18 y 65 años con diagnóstico DSM-5 de esquizofrenia, con un período de 3 meses de estabilidad clínica, fueron reclutados. Se recogieron datos sociodemográficos y clínicos, y se midieron: velocidad de procesamiento (Test del Trazo [TMT], codificación de símbolos [BACS], fluidez verbal), tiempos de reacción simple y electiva, atención sostenida, reconocimiento de emociones faciales y funcionalidad. Resultados Las medidas que se asociaron con funcionalidad fueron: velocidad de procesamiento (medida solo a través de la BACS), atención sostenida (CPT) y tiempo de reacción electiva (pero no simple). El reconocimiento de emociones faciales (FEIT) se correlacionó de forma significativa con la puntuación en las medidas de velocidad de procesamiento (BACS, Animales, TMT), atención sostenida (CPT) y tiempo de reacción simple. El modelo de regresión lineal mostró una relación significativa entre funcionalidad, reconocimiento de emociones (p = 0,015) y velocidad de procesamiento (p = 0,029). Conclusiones El déficit en velocidad de procesamiento y reconocimiento facial de emociones se asocia a peor funcionamiento global en pacientes con esquizofrenia. Introduction Previous studies have linked processing speed with social cognition and functioning of patients with schizophrenia. A discriminant analysis is needed to determine the different components of this neuropsychological construct. This paper analyzes the impact of processing speed, reaction time and sustained attention on social functioning. Material and methods 98 outpatients between 18 and 65 with DSM-5 diagnosis of schizophrenia, with a period of 3 months of clinical stability, were recruited. Sociodemographic and clinical data were collected, and the following variables were measured: processing speed (Trail Making Test [TMT], symbol coding [BACS], verbal fluency), simple and elective reaction time, sustained attention, recognition of facial emotions and global functioning. Results Processing speed (measured only through the BACS), sustained attention (CPT) and elective reaction time (but not simple) were associated with functioning. Recognizing facial emotions (FEIT) correlated significantly with scores on measures of processing speed (BACS, Animals, TMT), sustained attention (CPT) and reaction time. The linear regression model showed a significant relationship between functioning, emotion recognition ( P = .015) and processing speed ( P = .029). Conclusions A deficit in processing speed and facial emotion recognition are associated with worse global functioning in patients with schizophrenia. [ABSTRACT FROM AUTHOR]

Resumen Introducción El presente artículo describe la metodología general de un estudio transversal cuyo principal objetivo consiste en la detección de la prevalencia de los principales trastornos mentales en Andalucía, y el estudio de sus correlatos o posibles factores de riesgo mediante una amplia muestra representativa de adultos que vive en la comunidad. Materiales y métodos Este es un estudio transversal en el que desarrollamos un muestreo de varias fases utilizando distintos niveles de estratificación habituales y en el que teníamos como objetivo entrevistar a 4.518 participantes seleccionados al azar y representativos de las 8 provincias de la comunidad andaluza, con un enfoque de «llamada a la puerta». Como principal herramienta diagnóstica se utilizó la versión española de la entrevista neuropsiquiátrica internacional MINI, un instrumento válido de detección para el establecimiento de diagnósticos de trastorno mental compatibles con los criterios CIE-10/DSM-IV. Asimismo, se empleó una amplia batería de instrumentos para explorar la funcionalidad global, la comorbilidad médica, las características de la personalidad, la función cognitiva y la exposición a posibles factores de riesgo psicosocial. También se obtuvo una muestra de saliva para extraer ADN para un estudio de asociación genética. Entrevistadores entrenados llevaron a cabo las entrevistas, a pesar de que la mayoría de las medidas son compatibles con entrevistadores legos. Resultados De los 5.496 hogares seleccionados inicialmente, el 70,8% (3.892) tuvieron que sustituirse por falta de respuesta (37,7%) o por no incluir a ninguna persona que cumpliese las características requeridas (33%). Así, de las nuevas 5.496 personas, a las cuales finalmente se accedió, 4.507 (83,7%) consintieron participar, completaron la entrevista y se las incluyó finalmente en el estudio (n = 4.507), mientras que 4.286 (78%) también proporcionaron una muestra de saliva. Por su parte, 989 (16,3%) rechazaron participar. Discusión Se trata del estudio de epidemiología de salud mental más amplio desarrollado en la comunidad autónoma más grande y poblada de España (Andalucía). Las tasas de respuesta y representatividad de la muestra son bastante altas. El método empleado es muy completo para este tipo de estudios e incluye tanto valoraciones de personalidad (rasgos y trastorno) como valoración cognitiva, así como un amplio abanico de medidas de riesgo biopsicosocial. Introduction This is the general methods describing paper of a cross-sectional study that aims to detect the prevalence of major mental disorders in Andalusia (Southern Spain), and their correlates or potential risk factors, using a large representative sample of community-dwelling adults. Materials and methods This is a cross-sectional study. We undertook a multistage sampling using different standard stratification levels and aimed to interview 4,518 randomly selected participants living in all 8 provinces of the Andalusian region utilizing a door-knocking approach. The Spanish version of the MINI International Neuropsychiatric Interview, a valid screening instrument ascertaining ICD-10/DSM-IV compatible mental disorder diagnoses was used as our main diagnostic tool. A large battery of other instruments was used to explore global functionality, medical comorbidity, personality traits, cognitive function and exposure to psychosocial potential risk factors. A saliva sample for DNA extraction was also obtained for a sub-genetic study. The interviews were administered and completed by fully trained interviewers, despite most tools used are compatible with lay interviewer use. Results A total of 3,892 (70.8%) of 5,496 initially attempted households had to be substituted for equivalent ones due to either no response (37.7%) or not fulfilling the required participant quota (33%). Thence, out of 5,496 eligible participants finally approached, 4,507 (83.7%) agreed to take part in the study, completed the interview and were finally included in the study (n = 4,507) and 4,286 (78%) participants also agreed and consented to provide a saliva sample for DNA study. On the other hand, 989 (16.3%) approached potential participants refused to take part in the study. Discussion This is the largest mental health epidemiological study developed in the region of Spain (Andalusia). The response rates and representativeness of the sample obtained are fairly high. The method is particularly comprehensive for this sort of studies and includes both, personality and cognitive assessments, as well as a large array of bio-psycho-social risk measures. Resumen La craneotomía es el procedimiento neuroquirúrgico de apertura craneal amplia con el fin de realizar una actuación terapéutica quirúrgica en el espacio intracraneal. La técnica actual de la craneotomía es el resultado de la evolución de la misma desde su introducción a finales del siglo xix . El primer abordaje craneal amplio fue descrito por Wagner en 1889 como una «resección craneal temporal», que podría denominarse ahora «craneotomía osteoplástica con colgajo óseo pediculado». Para abrir el cráneo se han usado desde entonces múltiples sistemas manuales, mecánicos y motorizados que son revisados. El resultado final de este largo proceso de mejora es la «craneotomía osteoplástica de colgajo óseo libre» que se realiza en la actualidad de forma universal. En el presente trabajo revisamos la evolución histórica de la craneotomía desde el punto de vista de la técnica quirúrgica utilizada. Craniotomy can be defined as the neurosurgical procedure aimed at achieving a wide cranial opening with the final purpose of performing a surgical therapeutic manoeuvre within the intracranial space. The current surgical technique for craniotomy is the final result of the development of the procedure since its introduction at the end of the 19th century. The very first wide cranial approach was introduced in 1889 by Wagner, and described as a ‘temporary cranial resection’. This procedure could be named today as ‘osteoplastic craniotomy with pedicle bone flap’. The final result of the procedural development of the craniotomy is the ‘osteoplastic craniotomy with free bone flap’, used widely around the world. Resumen La cirugía radioguiada puede ser útil en la localización de tumores neuroendocrinos, detectando más lesiones y de menor tamaño que las pruebas de imagen prequirúrgicas y la palpación por el cirujano, detectando lesiones residuales e indicando una ruta más corta para acceder a la lesión. No obstante, su uso no se ha generalizado, ya que plantea dificultades técnicas, las series publicadas son limitadas, y no existe una uniformidad de criterios, debido a la gran variabilidad en cuanto al tipo de radiofármaco, dosis a emplear e intervalo entre la inyección del trazador y la cirugía. De estos aspectos nos ocupamos en esta revisión, describiendo la experiencia de distintos grupos, en los diversos tumores. Radioguided surgery can be a useful technique in the localization of neuroendocrine tumors. It detects more and smaller lesions compared to pre-surgical imaging and intraoperative digital palpation by the surgeon. It detects residual lesions and also indicates the shortest access route to the lesion. Nevertheless, its use has not become widespread because of technical difficulties. There is a limited number of published series, a lack of standardized protocol because of the great variability regarding type of radiopharmaceutical, dose of radiotracer, timing between injection and surgery. 25. Directrices prácticas para programas de apoyo entre personas con enfermedad mental.

Resumen Introducción Este estudio tiene como objetivo determinar los principios orientadores para la implementación de programas de apoyo entre personas con problemas de salud mental en Portugal. Material y métodos Este estudio ha sido dividido en 2 fases. En la primera se realizó una revisión sistemática de 112 artículos indexados en las bases de datos de ISI y EBSCO (2001-2012). En la segunda fase se llevó a cabo un panel con metodología Delphi y 2 rondas formado por profesionales, investigadores y personas con problemas de salud mental. El objetivo era analizar los enunciados generados a partir de la revisión sistemática previa. Los datos fueron analizados con NVivo 9 y SPSS 19. Resultados Durante la ronda Delphi participaron inicialmente 72 expertos, de los cuales 44 participaron en la segunda ronda. Se alcanzó un consenso del 84% sobre las principales afirmaciones. A partir de estos resultados, se destacan 8 ámbitos principales que cubren las siguientes áreas: objetivos de apoyo de los iguales, selección de los proveedores de apoyo, capacitación y acreditación, papel de los profesionales de salud mental, papel de los proveedores de apoyo, acceso a los proveedores de apoyo, cuidado de los proveedores de apoyo, y programa de evaluación. Conclusiones El apoyo de iguales aplicado a la enfermedad mental está todavía poco explorado y rodeado de cierta controversia y ambigüedad. Sin embargo, si cuenta con una buena organización y supervisión parece mejorar la calidad de vida y la inclusión social de las personas con enfermedad mental. Este hecho demuestra la importancia de realizar estudios que incrementen nuestro conocimiento acerca de dichos programas y se elaboren directrices para su aplicación. Este consenso nacional se puede utilizar como punto de partida para el diseño e implementación de programas de apoyo de iguales en las organizaciones de salud mental. Introduction This study aims to determine the guiding principles for the implementation of peer support programmes in Portugal. Material and methods The study was divided in 2 phases. In the first phase a systematic review of 112 papers indexed in ISI and EBSCO databases (2001 to 2012) was conducted. In the second phase clinicians, researchers, and people with psychiatric disabilities were invited to take part in a two-round online survey based on the Delphi process to rate the importance of statements generated from the systematic review. Data were analysed with NVivo 9 and SPSS 19. Results During the Delphi round 72 experts were contacted, 44 participated in the second round. A consensus was achieved on major statements, with 84% of the sentences obtaining a consensus and 8 key recommendations covering goals of peer support, selection of peer supporters, training and accreditation, role of mental health professionals, role of peer supporters, access to peer supporters, looking after peer supporters, and programme evaluation were based on these statements. Conclusions Use of peer support for mental health problems is still underexplored and surrounded by some controversy and ambiguity. However, its organization and proper monitoring appears to enhance the quality of life and social inclusion of people with mental illness. This highlights the importance of conducting studies that increase our knowledge of these programmes and determining guidelines for their implementation. Resumen La sintomatología relacionada con la presencia de quistes aracnoideos en el sistema nervioso central dependerá del tamaño del quiste y de su velocidad de crecimiento, su localización y, en algunos casos, de la posible alteración de dinámica de LCR asociada. En ocasiones la clínica se presenta de forma brusca por rotura del quiste o sangrado agudo. Aunque se acepta de forma general que los quistes asintomáticos o paucisintomáticos no precisan tratamiento quirúrgico, no existe consenso respecto a la actitud terapéutica de elección en los casos sintomáticos. El objetivo del presente trabajo es el de revisar la literatura analizando los pros y contras de las 3 opciones quirúrgicas principales (microcirugía, neuroendoscopia y derivación de LCR) en función principalmente de la localización de la lesión. Aunque el tratamiento debe ser siempre individualizado para cada caso, pueden ofrecerse unas recomendaciones generales de manejo. The symptoms related to the presence of arachnoid cysts in the Central Nervous System depend on the size of the cyst and its growth rate, its location and, in some cases, the associated CSF dynamic disorder. Sometimes there is acute clinical presentation due to cyst rupture or acute bleeding. Although it is generally accepted that asymptomatic or paucisymptomatic cysts do not require surgical treatment, there is no consensus on the therapeutic approach of choice in symptomatic cases. The aim of this paper is to review the literature, analyzing the pros and cons of the three main surgical options (microsurgery, neuroendoscopy, and CSF shunt) based primarily on the location of the cyst. Resumen Una de las principales opciones de tratamiento en quistes aracnoideos sintomáticos es la craneotomía con fenestración de membranas. Las principales ventajas de la cirugía abierta incluyen la inspección directa del quiste, la toma de muestra para biopsia, la posibilidad de fenestración en quistes multitabicados y, en ciertas localizaciones, la comunicación del quiste con las cisternas de la base. El objetivo de este trabajo es el de repasar las ventajas e inconvenientes de esta modalidad de tratamiento para quistes de diferentes localizaciones anatómicas. Craniotomy and fenestration of membranes is one of the main treatment options for symptomatic arachnoid cysts. Open surgery advantages include, direct inspection of the cyst, biopsy sampling, fenestration in multilocular cysts and, in certain locations, cyst communication to basal cisterns. Resumen Objetivo El aumento de peso es un efecto secundario frecuente e importante de los antipsicóticos de segunda generación (ASG). Además, estos fármacos pueden inducir otros efectos secundarios que están asociados a un aumento de la morbimortalidad cardiovascular, tales como la resistencia a la insulina, la diabetes o el síndrome metabólico. Estudios preliminares indican que las diferencias genéticas interindividuales producen distintos grados de vulnerabilidad a los efectos secundarios inducidos por los ASG. El estudio SLiM (por sus siglas en inglés, Second-generation antipsychotic Long-term treatment Metabolic side effects ) tiene como objetivo identificar en pacientes no tratados previamente con ASG (pacientes naive), aquellos factores clínicos, genéticos y ambientales que expliquen las diferencias interindividuales en relación con el aumento de peso y los cambios metabólicos generados tras 6 meses de tratamiento con estos fármacos. Material y métodos El estudio SLiM es un estudio farmacogenético multicéntrico, observacional, prospectivo, de 6 meses de duración, en el que se ha reclutado una cohorte de 307 pacientes pediátricos y adultos (rango de edad entre 8,8 a 90,1 años) naive a ASG y una cohorte de 150 controles sanos (rango de edad entre 7,8 y 73,2 años) emparejados por edad y sexo. Resultados En este artículo se presentan la justificación, los objetivos y el diseño del estudio y se ofrece una descripción de la muestra al inicio del estudio. Conclusiones Los resultados del estudio SLiM permitirán una mejor comprensión de los factores clínicos, ambientales y genéticos implicados en el aumento de peso y los trastornos metabólicos asociados al tratamiento con ASG. Aim Weight gain is an important and common side effect of second generation antipsychotics (SGAs). Furthermore, these drugs can induce other side effects associated with higher cardiovascular morbidity and mortality, such as insulin resistance, diabetes or metabolic syndrome. Preliminary studies show that inter-individual genetic differences produce varying degrees of vulnerability to the different SGA-induced side effects. The Second-generation antipsychotic Long-term treatment Metabolic side effects (SLiM) study aims to identify clinical, environmental and genetic factors that explain inter-individual differences in weight gain and metabolic changes in drug-naïve patients after six months of treatment with SGAs. Materials and methods The SLIM study is a multicenter, observational, six-month pharmacogenetic study where a cohort of 307 drug-naïve paediatric and adult patients (age range 8.8-90.1 years) and a cohort of 150 age- and sex- matched healthy controls (7.8-73.2 years) were recruited. Results This paper describes the rationale, objectives and design of the study and provides a description of the sample at baseline. Conclusions Results from the SLiM study will provide a better understanding of the clinical, environmental, and genetic factors involved in weight gain and metabolic disturbances associated with SGA treatment. This theoretical study deals with a review of empirical research and theoretical papers in which typologies of the men who are violent against their intimate partners are analyzed. First, the most relevant typologies and the aspects that have in common are described. Specifically, most studies about batterer men have distinguished three categories: a) Family only batterer; b) Borderline/Disforic; and c) Generally Violent/Antisocial. Recently, other studies have identified new subtypes of aggressors (for example, Low-level Antisocial Batterers) or they have focused on other variables of classification (personality and psychopathological characteristics or stages and processes of change). Second, some of the current questions on typologies (temporal stability of the typologies and categorical versus continuous antisocial type) are examined. Third, it is also considered the possibility of tailoring specific therapeutic interventions to specific typology. The aim of this paper was to evaluate an individual 20 sessions cognitive- behavioral programme in the treatment of male batterers after implementing it for ten years in a community setting. The sample consisted of 196 subjects who voluntarily participated in the study. An experimental design with repeated measures (pre-treatment, posttreatment and 1-, 3-, 6- and 12-month follow-up) was used. The success rate was of 88% in the treatment completers. In spite of the lost patients in the follow-ups (46% at the end of twelve months), the results were maintained in the contacted patients at follow-up (53% in the 12-month-follow-up), with a high impact on reducing recidivism. However, the rate of refusals and drop-outs of the treatment was rather high. Therefore it is necessary to develop strategies to improve motivation for treatment. Abstract: In this paper, a model of carnivore taphonomic action in the Argentinean Puna is applied to archaeological site Inca Cueva-cueva 4, on the Eastern Fringe of the Puna East of the Andes cordillera, which contains hunter–gatherer occupations dating to the early and middle Holocene. Different indicators, such as the low levels of bone modifications (ca. 2 to 8% NISP), consisting mainly in subtle marks, suggest a low intensity of the intervention of carnivores during the formation of these deposits. Variables such as the frequency of carnivore scats attest to repeated carnivore visits to the cave, but the “dilution effect” upon the small faunal assemblages these carnivores tend to accumulate in a context of larger zooarchaeological assemblages would have faded their traces away. Abstract: This paper employs artificial neural network and decision tree to derive knowledge about the job attitudes of Generation Xers. The sample frame consisted of 1000 large Manufacturing Industries and 500 large Service Industries, randomly selected from the Common Wealth Magazine 1000 index of Taiwan Manufacturing Industries and Service Firms. Then, we exploited the ART2 neural model to take the collected data as inputs and form performance classes according to their similarities. Finally, the decision tree was employed to determine definitions for each class, resulting in 52 rules associated with certainty factors. Abstract: Taxonomy of 57 bivalves, internal and external molds is described in this paper, corresponding to three paleotaxodontids genera Praectenodonta, Praenucula and Notonucula. Samples were collected from nine sites, where stratigraphic columns were measured in Silurian to Devonian series from Altiplano, Cordillera Oriental and Interandean of Bolivia. For the first time in Bolivia, Praectenodonta and Notonucula have been identified. Four species have been described, three of them are new: Praectenodonta boliviensis n. sp., Praenucula quichua n. sp. and Notonucula altiplanica n. sp. Most of the material review in this paper was included in the work of Sudre et al. (1983), some other provides from new sites. Two new species of primitive artiodactyls are described here: Diacodexis corsaensis n. sp. and Eygalayodon isavenaensis n. sp., which is considered the most plesiomorphic form of the subfamily Eurodexeinae and probable derived from Diacodexis corsaensis n. sp. It is for the first time that a representative of the Eurodexeinae is found in the Iberian Peninsula, where they might originate. [Copyright &y& Elsevier]
