Resumen Algunos estudios sugieren que existe una relación entre el uso de antipsicóticos y el riesgo de tromboembolismo venoso (TEV) y embolia pulmonar (EP). Sin embargo, los resultados siguen sin ser concluyentes. Se trata del caso de un Masculino de 23 años con antecedentes de Esquizofrenia y Depresión tratado quetiapina 800 mg, el cual es encontrado muerto en la cama de un hotel. En la necropsia sin lesiones traumáticas visibles, hallazgos histológicos de tromboembolismo pulmonar masivo con infartos pulmonares secundarios. Laboratorio de Toxicología detectó la presencia de quetiapina, no se detectó alcohol o drogas de abusos. Mediante el Algoritmo De Karch & Lasagna Modificado el tromboembolismo pulmonar fue una reacción adversa con una probabilidad de relación causal posible. Se han informado muchos casos de muerte súbita causada por EP con la exposición a antipsicóticos, pero la relación de su uso y el riesgo de TEV y EP sigue siendo controvertida. Abstract Some studies suggest a relationship between antipsychotic use and the risk of venous thromboembolism (VTE) and pulmonary embolism (PE). However, the results remain inconclusive. This is the case of a 23-year-old male with a history of schizophrenia and depression treated with quetiapine 800 mg, who was found dead in a hotel bed. At necropsy with no visible traumatic lesions, histological findings of massive pulmonary thromboembolism with secondary pulmonary infarcts. Toxicology laboratory detected the presence of quetiapine, no alcohol or drugs of abuse were detected. Using the Modified Karch & Lasagna Algorithm, pulmonary thromboembolism was an adverse reaction with a probable causal relationship. Many cases of sudden death caused by PE have been reported with exposure to antipsychotics, but the relationship of their use and the risk of VTE and PE remains controversial.

La quetiapina es un antipsicótico de segunda generación que tiene un amplio espectro de empleo en el tratamiento de varias enfermedades psiquiátricas, incluyendo a los trastornos por consumo de sustancias. A pesar de su eficacia y usos aprobados, hay varios informes de su potencial de abuso. En este artículo se reporta el primer caso en Hispanoamérica sobre uso riesgoso de quetiapina. Además, se incluye una revisión de la literatura que toma en cuenta reportes de caso, estudios epidemiológicos y estudios en modelos animales. El uso riesgoso o patrón de búsqueda-uso compulsivo de la quetiapina se reporta en individuos vulnerables con historial de trastornos por uso de sustancias, a menudo por consumo de cocaína. Se discuten las hipótesis sobre este fenómeno. A pesar de que la evidencia más fuerte de este tema procede de estudios preclínicos y no de mejores fuentes, los informes de casos como este deben ser tomados en cuenta en la evaluación de los pacientes, en cuanto a la utilidad de esta droga y sus posibles riesgos en el tratamiento de trastornos por uso de sustancias.

Approved uses of the atypical antipsychotic quetiapine by the FDA are limited to schizophrenia and bipolar disorder. Howeverin clinical settings quetiapine has many uses outside of these approved indications. In particular increasingly elderly patients are medicated with this drug for the management of behavioral and psychological symptoms associated with dementia. Starting from a clinical vignette, the author questions whether in patients with a diagnosis of dementia, the use of quetiapine contributes positively to the management of behavioral and psychological symptoms associated with it. After running a bibliographic search, four relevant articles are summarized.It concluded that there is no evidence to support the routine use of quetiapine or any of the atypical antipsychotics for any of the behavioral symptoms associated with dementia. Non-pharmacological strategies must be the first line of treatment in all cases. However, the progressive feature of these symptoms, the impact on the quality of life of patients and their household and the lack of pharmacological alternatives, put this group of drugs in the small arsenal with which doctors can manage the behavioral symptoms associated with dementia. Considering these aspects, individually use is recommended, always evaluating the risks and benefits for patients.

Los corticoides son fármacos muy usados en la práctica clínica habitual que, sin embargo, se asocian a frecuentes efectos adversos neuropsiquiátricos. Se describe el caso clínico de un varón de setenta y un años que desarrolló síntomas maníacos tras toma de corticoides y que fue tratado de forma satisfactoria con quetiapina. A partir de este caso, se realiza una revisión de la manía inducida por corticoides. Corticosteroids are drugs widely used in routine clinical practice that are however associated with neuropsychiatric adverse effects. This paper describes the case of a seventy-one- year man who developed manic symptoms after taking with corticosteroids and was successfully treated with quetiapine and, on the other hand, reviews corticos-teroid-induced mania.

Introducción: El delírium es una alteración aguda de la conciencia y la cognición que ha empezado a manejarse con antipsicóticos atípicos (AA). Debido a que las facultades mentales, por definición, se ven afectadas, los estudios en esta población generan dilemas éticos respecto a la participación voluntaria de los pacientes y su estado de vulnerabilidad. Objetivo: Valorar si los estudios realizados con AA para el tratamiento del delírium obtuvieron una aprobación por un comité de ética en investigación en seres humanos (CEISH), si hubo aplicación de un consentimiento informado (CI), si este era verbal o escrito y quién dio la aprobación para la participación en el protocolo. Diseño: Revisión sistemática en MedLine de los estudios de delírium que emplearon quetiapina y olanzapina y valoración de la existencia de aprobación por un CEISH y de la aplicación de un CI. Resultados: Se detectaron 11 estudios (6 de quetiapina y 5 de olanzapina), de los cuales 5 contaron con la aprobación del protocolo por un CEISH. Conclusiones: La mayoría de los estudios para el tratamiento del delírium no fueron sometidos a su aprobación por parte de un CEISH ni obtuvieron (de forma exclusiva) el CI de participación por parte del apoderado legal del paciente. Es fundamental que los futuros estudios de antipsicóticos y otros fármacos cuenten con la aprobación del protocolo por parte de un CEISH y de la firma del CI por parte del representante legal del paciente. Introduction: Delirium is an acute alteration of consciousness and cognition. Atypical antipsychotics (AA) have recently become a main part of its treatment. Studies in this population generate a series of ethical dilemmas concerning the voluntary participation of patients and their state of vulnerability since their mental faculties are, by definition, compromised. Objective: To assess whether studies with AA for the treatment of delirium obtained an approval by an ethics committee on human research (ECHR), if an informed consent (IC) was obtained, whether the IC was verbal or written, and who gave the approval to participate. Method: Systematic review of Medline for studies of delirium where quetiapine and olanzapine were the main treatment, assessing the existence of an ECHR approval and implementation of an IC. Results: 11 studies were identified (6 of quetiapine and 5 of olanzapine). 5 had an ECHR approval. Conclusions: Most studies examining the treatment of delirium with quetiapine or olanzapine were not subject to approval by an ECHR and most of them did not obtain an IC from the patient's legal guardian. It is essential that future studies of antipsychotics and other drugs for the treatment of delirium have the protocol approved by an ECHR and a written IC signed by the patient's legal representative, since by definition delirium is a condition that compromises superior mental processes.

Introducción: La neutropenia es reconocida como un efecto adverso observado con los medicamentos antipsicóticos, principalmente la clozapina. Aunque rara, la administración de quetiapina puede llevar a la aparición de este efecto adverso con altas tasas de morbimortalidad. Objetivo: Describir un caso de neutropenia asociada con el uso de quetiapina en un paciente con delírium, quien posteriormente con la suspensión del medicamento presenta la resolución del cuadro. Método: Reporte de caso de un hombre de 38 años de edad. Resultado y conclusiones: Aun cuando el riesgo de neutropenia asociado con quetiapina es relativamente bajo, es importante la implementación rutinaria de un cuadro hemático tanto al inicio de la administración de quetiapina como de manera periódica. Siempre que sea posible se debe evitar combinar quetiapina con medicamentos con reconocida asociación con neutropenia. Introduction: Neutropenia is a side effect of the use of antipsychotic drugs, mainly clozapine. Although rare, the administration of quetiapine may lead to the development of this adverse effect with high rates of morbidity and mortality. Objective: To describe a case of neutropenia associated with the use of quetiapine in a patient with delirium and its subsequent resolution when the medication was withdrawn. Method: Case report. Results and conclusions: Although the risk of neutropenia associated with the use of quetiapine is relatively low, it is important to implement routine hemograms at the beginning of the treatment with quetiapine, and on a regular basis. Whenever possible, combinations of quetiapine and other drugs with netropenia as a recognized side effect must be avoided.

However, their use may be associated to a higher incidence of weight gain, dyslipidemia, metabolic syndrome, glucose intolerance and type 2 diabetes mellitus. We performed a systematic literature search to evaluate the risk of type 2 diabetes mellitus incidence associated to the use of atypical antipsychotic drugs, compared to conventional treatment. If users of all types of atypical antipsychotic drugs are compared with users of conventional treatment, no significant differences in the incidence of type 2 diabetes mellitus were observed. If individual drugs are evaluated, clozapine and risperidone are associated with a higher risk of diabetes than haloperidol. Quetiapine is associated with a lower risk of diabetes than conventional treatment. The quality of the evidence found was low; therefore, new studies should been performed.

Introducción: En los últimos años se han incorporado al medio nacional diversos antipsicóticos de nueva generación, pero el modo de utilización práctica por los clínicos ha sido poco documentada. El objetivo de este estudio es caracterizar el patrón de prescripción de los antipsicóticos en pacientes hospitalizados en una institución psiquiátrica nacional. Métodos: Se estudió retrospectivamente las historias clínicas de los pacientes hospitalizados en la Clínica Psiquiátrica Universitaria entre Marzo y Septiembre de 2005. Se registraron los diversos antipsicóticos empleados y sus dosis para cada diagnóstico, tanto como indicación de uso permanente o esporádico. Resultados: En el período estudiado hubo 246 egresos y se empleó algún antipsicótico en el 80% de los casos. Su principal indicación fue en esquizofrenia, psicosis exógena y trastorno bipolar, pero también fueron utilizados con frecuencia en otros cuadros como en trastornos de la alimentación, de la personalidad y en depresión monopolar. Los más empleados fueron los antipsicóticos atípicos, solos o asociados con un neuroléptico de uso esporádico. Los antipsicóticos atípicos más prescritos fueron risperidona (41%) y quetiapina (34%). Las dosis utilizadas variaron ampliamente según los diversos diagnósticos. Conclusiones: Los antipsicóticos, en especial los atípicos, son ampliamente utilizados en el tratamiento de diversas patologías psiquiátricas en pacientes hospitalizados Introduction: During the past fewyears, several new generation antipsychotic drugs have been incorporated in our country. However, the manner in which these drugs are actually used by our clinicians has beenpoorly documented until now. This study aims to characterize the prescription pattern of antipsychotic drugs for inpatients at a Chilean psychiatric institution. Methods: We carried out a retrospective study using clinical records of inpatients at the University of Chile Psychiatric Clinic between May and September 2005. We registered every antipsychotic drug used and its dosagefor every diagnosis, including whether it was prescribed as permanent or as PRN. Results: During this period there were 246 hospital discharges and some kind of antipsychotic drug was used in 80% of cases. The main diagnoses associated with the use of antipsychotic drugs were schizophrenia, exogenous psychosis and bipolar disorder, but they were also used frequently in other diseases such as eating disorders, personality disorders, and unipolar depression. Atypical antipsychotic drugs were the most frequently used, whether alone or in PRN combination with a neuroleptic drug. Among atypical antipsychotics, the most frequently prescribed were risperidone (41%) and quetiapine (34%). The dosage used was widely different depending on the diagnosis. Conclusión: Antipsychotic drugs, particularly atypical ones, are broadly used in the treatment of inpatients with several psychiatric disorders

Objetivo: Hacer un estudio de la utilización de psicofármacos del grupo de antipsicóticos atípicos dentro del Centro Penitenciario de Málaga durante los años 2003/2004 y compararlo con el Distrito de Atención Primaria de referencia (Distrito Guadalhorce). Material y Métodos: Se ha realizado un estudio retrospectivo sobre la utilización de antipsicóticos atípicos en el Centro penitenciario de Málaga durante los años 2003/04 y se ha comparado con la utilización que de estos fármacos se hace en el Distrito de Atención Primaria (A.P.) de referencia. La información sobre consumo de medicación se obtuvo a partir de las ordenes médicas recibidas en el centro durante los años de estudio. Para la clasificación de principios activos se usó la clasificación ATC (Anatomical Therapeutic and Chemical Classifying System) y para el cálculo de envases, la base de datos propia del centro (SANIT 3.0). Para el cálculo de DDD se utilizó la DDD/1000 habitantes día. Resultados: Se ha incrementado la utilización de antipsicóticos atípicos a nivel del C.P. Existe una tendencia hacia la mayor utilización de quetiapina a dosis bajas mientras que ha disminuido la utilización de risperidona en el año 2004 respecto a 2003, aunque continúa siendo el de mayor utilización tanto en DDD como en envases consumidos. La situación en Atención Primaria es exactamente la contraria. Conclusión: El uso de este grupo de medicamentos en estudio ha disminuido a nivel de Atención Primaria (A.P.), posiblemente debido a medidas especiales de control médico (visado). En nuestro Centro Penitenciario se ha incrementado el uso, aunque se desconocen los motivos de dichas diferencias. Aim: we studied the use of psychotropic drugs belonging to the group of atypical neuroleptics in Malaga State Prison (Centro Penitenciario de Málaga) from 2003 to 2004. We also compared the results of this study with references taken from the Primary Health Care District of Guadalhorce. Materials and Methods: A retrospective study was carried out on the use of antipsychotic drugs from 2003 to 2004 in Malaga State Prison. A comparison was then made with the use of this type of medication in the Primary Health Care District of Guadalhorce. Data on medication consumption was taken from medical orders received at the prison during this study. The ATC (Anatomical Therapeutic and Chemical Classifying System) was used for classifying the active principles. The prison´s own data base (SANIT) was used for calculating the number of containers. For calculating the DDD, the ratio DDD/1000 inmates/day was utilised. Results: The use of atypical antipsychotic medication in the prison increased. There is an increasing trend towards the use of quetiapine in small doses. The use of risperidone went down during the period of this study, although it is still the most commonly used drug in DDD and in consumed containers. The Primary Health care results indicate a trend in the opposite direction. Conclusion: The use of the group of drugs in this study has decreased in the Primary Health Care area, possibly because of special medical control measures such as the control stamp. In Malaga Prison use of these drugs has increased. The reasons for this difference are as yet unknown.
