Abstract

Resumen: La exposición prenatal al alcohol es la principal causa prevenible del déficit cognitivo en los países desarrollados y puede dar lugar al trastorno del espectro alcohólico fetal (TEAF). Este término engloba una gran variedad de efectos físicos, mentales, conductuales y cognitivos que derivan del daño causado por la exposición al alcohol durante la vida intrauterina. El consumo de esta sustancia entre la población general es frecuente en los países de la Europa del Este y, especialmente, entre las mujeres en riesgo de exclusión social, que son las mayores afectadas en procesos de pérdida o renuncia de custodia de sus hijos. Un elevado número de estos niños son adoptados en España y muchos de ellos presentan alteraciones neurocognitivas y conductuales, convirtiendo el TEAF en un problema de salud pública en nuestro país. En muchas ocasiones, este cuadro clínico está infradiagnosticado debido a la superposición de los síntomas neuropsicológicos causados por el abandono y la falta de apego. Hasta el momento, no se ha descrito un perfil neurocognitivo y conductual específico del TEAF y muchos de los síntomas son comunes a otras etiologías. El objetivo de este trabajo es revisar el perfil neuropsicológico en el diagnóstico de TEAF. Abstract: Prenatal alcohol exposure is the leading preventable cause of cognitive deficit in developed countries and can lead to fetal alcohol spectrum disorder (FASD). This term encompasses a wide range of physical, mental, behavioral, and cognitive effects that result from damage caused by exposure to alcohol during intrauterine life. Alcohol consumption among the general population is common in Eastern European countries and especially among women at risk of social exclusion, who are the ones who lose or give up custody of their children. A high number of these children are adopted in Spain and many of them present neurocognitive and behavioral disorders, causing FASD to be a public health problem in our country. In many occasions this clinical spectrum is delayed or under-diagnosed due to the overlapping of neuropsychological symptoms caused by the abandonment. A neurocognitive and behavioral profile specific for FASD has not been defined and all the symptoms are common to other etiologies. The aim of this work is to review the neuropsychological profile in the diagnosis of TEAF.

espanolObjetivos: Nuestros trabajos previos han demostrado que el consumo abusivo de etanol durante la gestacion induce retraso en el desarrollo y alteraciones estructurales en los ojos y en la vision (Pinazo-Duran et al., Teratology ’93; Eur J Ophthalmol ’97; Stromland and Pinazo-Duran, Teratology ’94; Alcohol Alcoholism ’02). Nos proponemos estudiar a un nivel celular y molecular si el etanol modifica la expresion de proteinas marcadoras del desarrollo glial durante los estadios pre- y postnatal de la rata, que puedan contribuir a la dismorfogenesis del nervio optico. Metodo: Para estudiar las acciones del etanol sobre el desarrollo del nervio optico de ratas sometidas a un proceso de alcoholizacion y ratas controles, las madres fueron alimentadas con dieta liquida durante la gestacion y la lactancia, conteniendo alternativamente etanol (5% p/v; 35% del aporte calorico diario) o carbohidratos isocaloricos (35% aporte calorico diario). Los ojos fueron enucleados junto a los nervios opticos y procesados para tecnicas inmunocitoquimicas, morfologicas y western blot, utilizando anticuerpos contra la proteina fibrilar acida de la glia (GFAP), proteina de los neurofilamentos (NFP) y proteina basica de la mielina (MBP). Resultados: Constatamos tres diferencias principales entre el grupo tratado con etanol respecto al control: 1) el tamano del nervio optico fue significativamente inferior en el grupo expuesto al etanol, 2) objetivamos cambios estadisticamente significativos en el desarrollo de los astrocitos, oligodendrocitos, axones opticos y vainas de mielina de los nervios opticos de ambos grupos y 3) apreciamos el retraso y anomalias en la expresion de proteinas implicadas en su desarrollo. Conclusiones: Los resultados confirman, como en trabajos anteriores, los efectos adversos del alcohol en el desarrollo del sistema visual. Sugerimos que el etanol debe actuar negativamente sobre la expresion de genes implicados en el desarrollo del sistema visual. Estos resultados pueden extrapolarse a los hallazgos clinicos del sindrome alcoholico fetal y de las neuropatias opticas toxicas. EnglishPurpose: Previous work from our group demonstrated that regular high consumption of ethanol during pregnancy induces a delay in growth and structural changes in the developing eye and vision (Pinazo-Duran et al., Teratology ’93; Eur J Ophthalmol ’97; Stromland and Pinazo-Duran, Teratology ’94; Alcohol Alcoholism ’02). Our main goal is to study at a cellular and molecular level, whether or not the prenatal alcohol exposure may change the development of the glial cells and inducing the optic nerve dysmorphogenesis. We have used key protein markers to analyse the expression in the rat optic nerve throughout the pre- and postnatal periods. Methods: To better understanding the actions of ethanol on optic nerve development in alcohol-induced and control dams, these were fed a liquid diet during gestation and lactation, containing either ethanol (5% w/, 35% of the daily food intake) or isocaloric carobydrates (35% of the daily food intake). Eyes were enucleated and processed to immunocytochemical and morphological tecnhiques and western blot approaches, using antibodies against the glial fibrillary acidid protein (GFAP), neurofilament protein (NFP) and myelin basic protein (MBP). Results: Three main observations were made in the ethanol-exposed and control groups: 1) the optic nerve size was significantly lower in the ethanol group than in the control group, 2) there were statistically significant changes in optic nerve astrocytes and oligodendrocytes, optic axons and myelin sheaths and 3) a delay and altered expression of developmental proteins. Conclusions: All data support our earlier studies confirming the deleterious effects of ethanol on the developing visual system. We suggest that ethanol may alter the expression of precise genes involved in eye development and posterior remodelling. These results can be extrapolated to clinical advances in fetal alcohol syndrome and toxic optic neuropathies.
