ABSTRACT Traumatic chiasmal syndrome is one of the rare etiologies of chiasmal syndrome, characterized by optic chiasm injury following head trauma. The main visual defect associated is bitemporal hemianopia with macular splitting; however, it can present with a variety of other visual defects and neurologic signs. The authors report a case of complete bitemporal hemianopia after head trauma, with multiple frontal and skull base fractures and no other neurologic deficits, or hypothalamic-pituitary abnormality. Most cases of traumatic chiasmal syndrome are caused by mechanical stretch or compression of the chiasma. Nevertheless, in this case, the radiologic findings excluded macroscopic disruption or external compression of the chiasma, raising the possibility of a contusion necrosis associated with functional impairment of the optic chiasma. Traumatic chiasmal syndrome must be considered in the differential diagnosis of patients presenting with complete bitemporal hemianopia after head injury caused by frontal and skull base fracture.

Objective: Understanding the relationship between the carotid artery, optic nerve and the anterior clinoid process is the basis of surgical approaches performed in the subchiasmal region. The location of the optic chiasm, the length of the optic nerves, and the distance and angle between the optic nerves determine the route of surgical approaches. We have determined the types of optic chiasm to study the relationship between vascular and neural structures in this region., Materials and Methods: Thirty autopsy specimens were investigated at the Bursa Forensic Medicine Institute for optic chiasm types and the relationship between the neural and vascular anatomical structures of the sellar-parasellar and subchiasmal region was examined between June 2016 and November 2016., Results: In this study, 4 prefix types (13%), 6 postfix types (20%), and 20 central types (67%) of chiasm were defined. Furthermore, we measured this angle between two optic nerves, which indirectly shows the location of chiasm according to the diaphragma sellae, and then detected the mean value of this angle as 87.1±11.6°. The "limit" value to designate a chiasm as prefix was measured in the current study as ≥101.1°. The angle between optic nerves ranged from a mean value of 69.9±3.7° in 6 cases with postfix chiasm, to a mean value of 104.0±2.1° in 4 cases with prefix chiasm and a mean value of 88.8±6.7° in 20 cases with central chiasm., Conclusion: In this study, we showed that the relationship among optic chiasma types, optic nerves and bony and vascular structures around the sellar area was effective at determining the surgical approach to this region., (Copyright © 2019 Sociedad Española de Neurocirugía. Background and Objectives: To describe the frequency of endocrine disorders in children with optic chiasm glioma and analyze related factors., Patients and Methods: Review of medical records by collecting sex, age, history of neurofibromatosis, clinical presentation, treatment of tumour, and presence of endocrine abnormalities. Statistical tests Wilcoxon and Fisher., Results: 14 patients (6 female) with age at diagnosis of 0.5 to 7.0 years (mean±standard deviation: 2.97±2.32) and follow-up of 10.64±3.30 years (range 6.0 to 16.0). 12/14 presented endocrinopathy at follow-up: 8 precocious puberty, 5 hypopituitarism, and 5 obesity. The onset of deficits was related to the neuroophthalmological symptoms under the age of five (P=.02)and treatment of the tumour was required.(P=.03)., Conclusions: Children with optic chiasm gliomas may present endocrine disorders from the time of diagnosis of the tumor and, in particular as they develop on. The most common of these is precocious puberty. Case Report: A 38-year-old female, at 20-weeks gestation, experienced a sudden visual loss and visual-field abnormalities. The neuroimaging tests showed a craniopharyngioma. Surgical removal was performed with a successful outcome as regards the pregnancy and visual function., Discussion: It is known that pituitary adenomas may grow during pregnancy; however this is unusual in craniopharyngiomas. They usually present with visual problems due to their suprasellar topography. Case Presentation: A 45-year-old woman with a history of seizures, headaches, nausea, vomiting, and decreased visual acuity of 5 years. Visual field detected a bitemporal heteronymous hemianopia. Magnetic resonance imaging revealed basal cistern arachnoiditis and supratentorial hydrocephalus. Cranial computed tomography revealed supratentorial calcifications, scolex in the left occipital region, and hydrocephalus secondary to entrapment of the fourth ventricle., Discussion: Neurocysticercosis can cause bitemporal hemianopsia due to chiasmatic compression secondary to obstructive hydrocephalus. The positivity of anti-cysticercus antibodies determined by ELISA evidence active disease. Objective: To determine the ability of frequency doubling technology (FDT) to evaluate retrochiasmatic lesions., Methods: 21 patients with characteristic retrochiasmatic visual field defects in automated perimetry (Humphrey field analyzer, 24-2 SITA) underwent FDT using C-20 screening and threshold tests. Two independent observers described the visual field defects, the degree of congruence and macular sparing of the three explorations., Results: The visual field defect found in the 24-2 test was inferred in 63.2% and 70% of the eyes from the C-20 screening and C-20 threshold test respectively. The retrochiasmatic lesions were congruent in 45% of the cases in automated perimetry, 5.6% in C-20 screening and 15% in C-20 threshold test. Macular sparing was present in 45% of cases with automated perimetry, 89% with C-20 screening and 82% with C-20 threshold test., Conclusions: The FDT allows us to suspect retrochiasmatic lesions in more than one-half of patients, but it tends to underestimate the defects. It is not useful for detecting macular sparing or the evaluation of visual field congruence. Both FDT strategies offer similar results, so the C-20 screening test could be more advisable because of its speed (mean: 70 seconds). However, in retrochiasmatic lesions the FDT accuracy is considerably lower than the 24-2 SITA strategy.

Visual fields continue to be a key exploration for the diagnosis and follow-up of patients in neuro-ophthalmology. The pattern of visual field defects helps, and in many cases allows, the identification of brain damage location. Manual kinetic perimetry has been replaced by automated methods. 24-2 SITA (Humphrey Visual Field Analyser) and TOP (Octopus) are regarded as the standard perimetric explorations in neuro-ophthalmology. Goldmann perimetry remains as an useful exploration for temporal crescent detection in occipital lobe diseases, and it could be more accurate and consistent for studying lesions in the post-geniculate pathway. Frequency doubling perimetry could be useful for detecting neuro-ophthalmic visual field defects, but does not provide an accurate characterisation of the lesions. From the neuro-ophthalmic point of view, visual field defects could be divided in pre-chiasmatics, chiasmatics and post-chiasmatics. Pre-chiasmatic defects are strictly unilateral, do not respect the vertical meridian, often have a nasal step associated and are usually accompanied by ocular pathology detectable in an ophthalmic examination. The characteristic perimetric pattern of chiasmal disease is bi-temporal hemianopsia. Homonymous contralateral defects are the characteristic perimetric pattern of post-chiasmal disease, and their congruency increases when the lesions are closer to the occipital lobe. Neuroimage studies are mandatory in all patients with a perimetric defect pattern compatible with chiasmal or post-chiasmal lesions. Magnetic Resonance Imaging may be normal in a patient with homonymous defects in Alzheimer's disease, the Heidenhain variant of Creutzfeldt-Jakobs disease, carbon monoxide poisoning and mild occipital ischemia demonstrated by SPECT or PET imaging (Arch Soc Esp Oftalmol 2002; 77: 413-428).

A four-year-old girl suffered difficult-to-diagnose hyponatremia resistant to treatment following surgery for a suprasellar tumor. The final diagnosis was diabetes insipidus evolving in three stages. Hyponatremia is a common problem following surgery to remove brain tumors. Early diagnosis and treatment of this electrolytic imbalance are essential for preventing serious neurological symptoms or death. The conditions most closely related to hyponatremia are inappropriate antidiuretic hormone secretion syndrome (IADHSS) and cerebral salt wasting syndrome (CSWS). The latter has become more common in recent years among patients undergoing brain surgery. Whereas IADHSS is treated by restricting fluids, CSWS requires administration of salt and volume fluid volume. We believe that for differential diagnosis of postoperative hyponatremia, a fluid restriction test takes priority over of fluid loading following neurosurgery. The course of hyponatremia must be carefully monitored and a complete endocrinological workup must be performed to detect the possible presence of hypophyseal deficiencies, particularly hypothyroidism and suprarenal insufficiency.

Unlabelled: INTRODUCTION, MATERIAL AND METHODS: We studied 103 patients with hypophyseal tumors aged between 15 and 74, with a marked predominance of females. Results. Of these, 49 patients were diagnosed as having macroadenomas (47.5%), 27 with microadenomas (26.2%), 25 with arachnoidoceles (24.3%) and two with craniopharyngiomas (1.9%). Of the total there were 78.9% functioning tumors of which the commonest was prolactinoma., Conclusions: Kinetic and static perimetry complement each other as diagnostic methods to obtain information about damage to the visual pathway caused by these tumors.

Hypophyseal apoplexy is not always easy to confirm morphologically or analytically. We present a patient with Cushing's syndrome due to hypophyseal apoplexy and optochiasmic pachyarachnoiditis which produced serious visual alterations. The adenoma necrotized behind an area of hypophyseal apoplexy which cured the patient's Cushing's disease. The second case was admitted with hypophyseal apoplexy, presenting hypophyseal tumoration with signs of hemorrhage and subsequent panhypopituitarism.

We here present the case of a patient who starts with a progressive vision loss, observing in the radiologic exam herniation of optic chiasma in a primary empty sella turcica. Based on this case, we have performed a review of this clinical picture and point out the most important aspects in the literature.

Three cases of primary hypothyroidism with optic chiasm compression due to pituitary hyperplasia are reported. Patients consulted for amenorrhea, galactorrhea and visual disturbances. The study of these patients disclosed a chiasm syndrome, persistent hyperprolactinemia, decreased thyroxine and triiodothyronine associated with elevated thyrotropin stimulating hormone in blood. Computerized tomography scan revealed the presence of pituitary enlargement. Treatment with thyroid hormones resulted in total resolution of clinical features and disappearance of tomographic abnormalities.
