La hipercalcemia hipocalciúrica familiar (HHF) constituye una causa poco común de hipercalcemia. Su prevalencia se estima que es de 1:78.000 personas. Mostramos un caso con presentación atípica con confirmación mediante diagnóstico genético.Se trata de una mujer de 74 años con osteoporosis remitida al servicio de Endocrinología por sospecha de hiperparatiroidismo primario (HPTP). Presentaba una hormona paratiroidea (PTH) elevada (96,3 pg/ml-límites normales (LN): 15-65 pg/ml) con niveles de calcio, fósforo y magnesio normales, además de una calciuria elevada.Posteriormente presentó cifras de PTH normales, niveles de calcio elevados junto con calciurias normales-elevadas. El cociente aclaramiento de calcio/aclaramiento de creatinina (CCCR, en mmol/l) osciló entre 0,011-0,02 mmol/l. El CCCR 0,02 de HPTP; este cociente se ve limitado en el rango entre 0,01-0,02 mmol/l, motivo por el que se justifica solicitar la prueba genética en todos los pacientes con PTH normales o altas, hipercalcemia y CCCR

Se determinaron las condiciones para disolver la cáscara de piña en polietilénglicol 400. Se utilizó un tamaño de partícula entre 125 y 150 micrómetros, una relación 1/1 en peso de sustrato/PEG, temperatura 225°C y un tiempo de disolución de 3 horas. El análisis de la solución resultante fue [OH]= 3,6 ± 0,2 mmol/g, [COOH]= 1,10 ± 0,03 mmol/g y contenido de agua de 2,70 ± 0,05 mg/mL .

RESUMEN Introducción: En Chile, los efectos maternos y perinatales de la pandemia por SARS-CoV-2 son aún desconocidos. GESTACOVID es un estudio multicéntrico que incluye embarazadas y puérperas hasta el día 42 con COVID-19. El objetivo de este estudio es presentar un informe preliminar, describiendo el impacto de la enfermedad en las embarazadas, factores de riesgo asociados y resultados perinatales. Métodos: Estudio de cohorte descriptivo que incluye 661 pacientes enroladas entre el 7 de marzo y el 6 de julio de 2020, en 23 centros hospitalarios del país. Se analizaron variables demográficas, comorbilidades, características clínicas y del diagnóstico de COVID-19 y resultado materno y perinatal. Resultados: Las pacientes hospitalizadas por COVID-19 tuvieron mayor prevalencia de hipertensión arterial crónica [10% vs 3%; OR=3,1 (1,5–6,79); p=0,003] y de diabetes tipo 1 y 2 [7% vs 2%; OR=3,2 (1,3–7,7); p=0,009] que las pacientes manejadas ambulatoriamente. Un IMC >40 kg/mt2 se asoció con un riesgo dos veces mayor de requerir manejo hospitalizado [OR=2,4 (1,2 – 4,6); p=0,009]. Aproximadamente la mitad de las pacientes (54%) tuvo un parto por cesárea, y un 8% de las interrupciones del embarazo fueron por COVID-19. Hasta la fecha de esta publicación, 38% de las pacientes continuaban embarazadas. Hubo 21 PCR positivas en 316 neonatos (6,6%), la mayoría (17/21) en pacientes diagnosticadas por cribado universal. Conclusiones: Las embarazadas con COVID-19 y comorbilidades como diabetes, hipertensión crónica y obesidad mórbida deben ser manejadas atentamente y deberán ser objeto de mayor investigación. La tasa de transmisión vertical requiere una mayor evaluación para diferenciar el mecanismo y tipo de infección involucrada.

Informe final de Proyecto de Investigación. Código del proyecto: 1401057 La torrefacción de biomasa consiste en someter el material a temperaturas que varían entre 200°C a 300°C, en ausencia de oxigeno por periodos que varían de 5 a 35 minutos. Durante esas condiciones la biomasa pierde una serie de sustancias volátiles y aumenta su poder energético. Su poder calórico puede alcanzar hasta 23 MJ/kg, a diferencia de la biomasa sin torrefactar que su contenido energético, es de aproximadamente 18,5 MJ/kg. Además de este aumento en el contenido energético, la torrefacción permite que los productos fabricados tengan una mayor resistencia mecánica, soporte la manipulación y tenga resistencia a la humedad, a diferencia de la biomasa sin este proceso, ya que es afectada por estas condiciones. Es por ello que el presente proyecto desarrolla la base de conocimiento para aplicar el proceso de torrefacción a biomasa a 5 especies de madera de Costa Rica, específicamente maderas utilizadas en la forestación comercial (Cupressus lusitanica, Dipteryx panamensis, Gmelina arborea, Tectona grandis y Vochysia ferruginea). Primeramente se analizó el proceso de torrefacción de biomasa con el análisis termogravimétrico (TGA) para establecer cuáles son las condiciones de temperatura y tiempo para llevar el proceso de torrefacción en cada especie de madera. En esta etapa la biomasa se caracterizó desde su composición química hasta su composición física. En una segunda etapa se aplicó la torrefacción en un modelo de reactor de una capacidad que permita obtener material para fabricar pellets, aproximadamente 3 kg, en esta fueron probadas las condiciones anteriormente establecidas. Finalmente se construyeron los pellets de la biomasa torrefactada y se caracterizaron con la aplicación de pruebas comunes para este producto. Biomass torrefaction involves subjecting the material to temperatures ranging from 200°C to 300°C, in the absence of oxygen for periods ranging from 5 to 35 minutes. During these conditions the biomass loses a number of volatile substances and increases its energy power, in some cases up to 23 MJ / kg, is approximately 18,5 MJ / kg. In addition to this increase in energy content, torrefaction allows the products manufactured to have a greater mechanical resistance, support the manipulation and has resistance to humidity, unlike the biomass without this process, since it is affected by these conditions. This is why the present project develops the knowledge base to apply the roasting process to biomass. Specifically, 5 species of wood used in the commercial forestation of Costa Rica (Cupressus lusitanica, Dipteryx panamensis, Gmelina arborea, Tectona grandis and Vochysia ferruginea). In this process the torrefied biomass was first analyzed with thermogravimetric analysis (TGA) to establish the temperature and time conditions for the roasting process in each wood species. At this stage the biomass was characterized, chemically and physically. In a second step the torrefaction was applied in a reactor model of a capacity to obtain material to make pellets, approximately 3 kg. Finally the torrefied biomass pellets were constructed and characterized by the application of common tests for this product.

12. POTENCIAL PROBIÓTICO DE CEPAS DE BACILLUS CON PROPIEDADES ANTIMICROBIANAS FRENTE A PATÓGENOS DE PECES

El objetivo de este trabajo ha sido aislar, identificar y caracterizar cepas bacterianas procedentes de ambiente marino, fitoplancton, zooplancton, huevos, larvas de lenguado y diferentes juveniles (branquias, intestino, piel), que pudieran tener actividad probiótica y actividad antibacteriana frente a distintos patógenos de peces. Se aislaron un total de 217 cepas bacterianas. Tras un primer cribado mediante ensayos de inhibición en placa frente a patógenos, se seleccionaron 9 cepas y se identificaron utilizando la secuenciación de su rRNA 16S. Los resultados mostraron que las mejores candidatas eran cepas del género Bacillus. Varios de estas posibles cepas candidatas se caracterizaron in vitro para determinar su potencial probiótico, mediante ensayos de inhibición en co-cultivo con V. anguillarum, resistencia a bilis de pez, y adherencia a mucus intestinal de pez. Nuestros resultados mostraron que algunas de las cepas seleccionadas pertenecientes al género Bacillus, podrían ser consideradas como potenciales probióticos frente a patógenos de peces, por lo que el beneficio de la administración de estas cepas a cultivos de peces será posteriormente estudiada.

Se determinó la susceptibilidad antibiótica de bacterias subgingivales en caninos con enfermedad periodontal moderada a severa. Se trabajó con 30 canes provenientes de cuatro albergues de la ciudad de Lima. Las muestras se tomaron con puntas de papel estéril colocadas en el fondo de la bolsa periodontal del cuarto premolar y canino del cuadrante superior derecho. El agente bacteriano aislado con mayor frecuencia fue Porphyromonas gingivalis (50%). Además, se hallaron Bifidobacterium spp (20%), Prevotella intermedia (16.7%), Staphylococcus spp (30%) y enterobacterias (60%). La mayor susceptibilidad antibiótica de las especies halladas fue frente al imipenem (100%), habiendo una susceptibilidad variable para los demás antibióticos dependiendo del agente bacteriano. The aim of this study was to determine the antibiotic susceptibility of subgingival bacteria in dogs with moderate to severe periodontal disease. Thirty dogs from four shelters were sampled using sterile paper sheet tips at the bottom of the periodontal pocket in the fourth premolar and canine teeth of the upper right quadrant. The most frequent bacteria isolated were Porphyromonas gingivalis (50%), Bifidobacterium spp (20%), Prevotella intermedia (16.7%), Staphylococcus spp (30%) and enterobacteria (60%). The highest antibiotic susceptibility from isolated bacteria species was against imipenem. The susceptibility against other antibiotics depended of the bacterial species.

Centro de Investigación en Biotecnología (CIB); Universidad Nacional de Costa Rica, 2011 La Ingeniería de Tejidos comprende los métodos utilizados para obtener y crecer tejidos humanos o animales para restaurar, mejorar o sustituir órganos humanos dañados. Este proyecto constituyó un esfuerzo pionero de las universidades públicas en ingeniería de tejidos en el país. Se diseñaron y ensayaron biomateriales tridimensionales compuestos a partir de colágeno de la dermis de tilapia y de quitosano del exosqueleto de camarón, para la realización de pruebas preliminares con la finalidad de demostrar su potencial terapéutico en afecciones epidérmicas. Las matrices extracelulares porosas fueron producidas por POLIUNA, a partir de colágeno y quitosano con la finalidad de estimular la proliferación y diferenciación celular, sirvieron de base estructural para el crecimiento de la línea celular de fibroblastos 3T3. Las membranas con proporciones de 2-8 y 6:4 C-Q, (Colágeno-Quitosano), mostraron el mayor crecimiento de fibroblastos. 5 Se probaron varios protocolos de esterilización (antibióticos, acetona, luz ultravioleta e irradiación de las membranas con rayos x). La irradiación fue el método más eficiente, este protocolo debe adaptarse según el grosor y composición del biomaterial. El diseño y confección de un soporte de anillos y la colocación de las membranas en anillos de sujeción, resultó favorable para el crecimiento celular, debido a que mantienen la membrana tensa. Se logró optimizar la adhesión celular al colocar la membrana al liofilizador antes de colocar las células, para que se adhiriera al pozo. El método de tinción más eficiente fue el MTT, permitió observar el crecimiento de fibroblastos sobre la membrana, ésta no se tiñó con el compuesto. Instituto Tecnológico de Costa Rica. Introducción: Aun cuando se ha ido incorporando la calidad de vida como un indicador comprensivo del estado de salud, no se tienen valores de referencia. Objetivo: Reportar valores que puedan ser utilizados como referencia en el campo clínico y de la salud pública. Método: Se evalúa a 1 678 niños y adolescentes de entre 8 y 18 años de establecimientos públicos, subvencionados y particulares de la ciudad de Antofagasta a través del cuestionario KIDSCREEN-27. Resultados: Se reportan las medias y deciles de cada una de las cinco dimensiones evaluadas (bienestar físico, bienestar psicológico, relación con los padres y autonomía, apoyo social y pares, ambiente escolar) para hombres y mujeres, rangos de edad y tipos de establecimiento educacional. Conclusión: Los valores presentados en las diferentes dimensiones evaluadas pueden ser utilizados como valores de referencia en población infanta juvenil chilena. En general, los hombres reportan una mejor calidad de vida que las mujeres, al igual que los rangos de menor edad y los establecimientos privados. Estos hallazgos son similares a los reportados en la población española utilizando similar instrumento. Background: Although quality of life has been incorporated as a comprehensive indicator of health status, there are no standards available. Objective: To report values that can be used as standards in clinical and public health areas. Methods: 1 678 children and adolescents (8 to 18 years old) were evaluated. They came from public, semiprivate and private schools in the city of Antofagasta (North of Chile) utilizing KIDSCREEN-27 questionnaire. Results: The study shows mean and decile values for each of the five domains assessed (physical wellbeing, psychological wellbeing, relationship with parents and autonomy, social support and peers, school environment) for gender, age range and type of school. Conclusion: Values were presented which can be used as standards for Chilean children and adolescents in the specified domains. In general, best quality of life was found among boys, younger age children, and those from private schools. These findings are similar to those reported in studies in Spain, utilizing the same instrument.

Introducción: La fibrosis quística (FQ) exhibe un deterioro progresivo de la función pulmonar (FP) asociada a infecciones recurrentes. Objetivo: Evaluar la FP en sujetos con FQ mediante espirometría, saturometría y pletismografía (PG) determinando alguna relación matemática. Pacientes y Métodos: Se identificaron 10 pacientes capaces de realizar CVF, CRF, VR, CPT, VEF1, y FEF25-75 %; comparándose con los valores de Zapletal y de 33 controles sanos. Resultados: Edad promedio 13,6 años (rango: 9-20). Sa0(2)y VEF1fueron normales en 10 y 5 sujetos, respectivamente (4 tenían VEF1entre 65-80%). La PG mostró 4 pacientes con VR/CPT > 30%, 3 con aumento de CRF y VR/CPT, sólo uno tuvo PG normal. Los promedios de CPT y VR fueron superiores a valores de referencia y controles (p < 0,05). Se encontró una relación inversa entre VEF1y VR/CPT (r = -0,642, p < 0,05) y entre FEF25-75 y VR/CPT (r = -0,803, p < 0,01). Conclusiones: Este estudio sugiere un incremento significativo de los volúmenes pulmonares en pacientes con FQ aún teniendo espirometría normal; así como una relación matemática inversa entre VEF1, FEF25-75 y VR/CPT. Objective: To evaluate PFT in CF patients using spirometry, oxygen saturometry (Sa0(2)) and whole plethysmography (WP) for determining possible mathematical relationships. Methods: FVC, FRC, RV, TLC, FEV1, FEF25.75, obtained in 10 CF patients were compared with Zapletal's reference values and with the values obtained in 33 healthy-school children. Results: Mean age 13.6 years-old (range: 9-20). All CF patients had normal Sa0(2), FEV1 was normal in five (four had FEV1 between 65-80%), 4 had RV/TLC > 30%, 3 had increased FRC and RV/TLC, only one CF patient had WP normal. The average of TLC and RV were higher than reference values and control group (p < 0.05). We found an inverse relationship between FEV1, FEF25-75 and RV/TLC [r = -0.642 (p < 0.05) and r = -0.803 (p < 0.01); respectively]. Conclusion: This study suggests an increase of pulmonary volume in CF patients with not necessarily abnormal spirometry and an inverse mathematical relationship between FEV1, FEF25-75 and RV/TLC.

Se identifican las sequías climáticas ocurridas en el espacio y tiempo durante el siglo pasado para la Pampa Húmeda. Haciendo uso de índices de sequía mensual, trimestral y anual propuestos se analizan algunas de las propiedades estadísticas como la persistencia, marcha temporal y distribución espacial, entre otras. También se muestran las potenciales consecuencias de las sequías sobre la productividad de un cultivo de importancia regional como el maíz, muy sensible a la misma. Para ello se elabora y evalúa un modelo de diagnóstico entre este estado del clima y la productividad del maíz para analizar los impactos. This work seeks to identify droughts events in the space-time that occurred during the last century in Humid Pampas (Argentina). By means of the drought-index, statistical properties such as persistence, extremes and trends are analyzed. Since corn is drought sensitive, this study also attempts to show potential consequences of drought on the productivity of this important region crop. For that, an interaction model between droughts and productivity-impacts is elaborated and evaluated.

Introduction: During the past three decades there has been increasing interest in the anti-inflammatory effects of macrolides. Objective: To review current data about anti-inflammatory mechanisms of these drugs, and to examine their effect on modulation of the inflammatory pathways, neutrophil function, bronchoconstriction, and Pseudomonas biofilm. Methods: A systematic review considering original papers and review articles at the National Library of Medicine (MEDLINE/Pubmed), using as key words macrolides, azithromycin, clarithromycin, immunomodulating, antiinflammatory, airway, cystic fibrosis, asthma, sinusitis and children, were performed. Results: Low-dose macrolide therapy has dramatically increased survival in patients with diffuse panbronchiolitis. This has led to further investigation into the potential use of macrolides in chronic lung diseases with an inflammatory component such asthma, cystic fibrosis and bronchiectasis. However, the current evidence to guide this approach, should be considered as only recommendations. Conclusion: The anti-inflammatory effects of these drugs have been under extensive investigation, but randomized and controlled clinical trials in children with respiratory condition are just beginning. The benefits and potential side effects need to be determined before routine use can be advised.

PROPÓSITO: evaluar la eficacia y la seguridad a largo plazo de la braquiterapia con hilos extraesclerales de iridio-192 en melanoma uveal posterior. MATERIAL Y MÉTODOS: serie retrospectiva no controlada de 241 pacientes tratados entre enero de 1983 y junio de 2005 en un hospital de referencia nacional. RESULTADOS: seguimiento medio de 5,4 años, con 17% de pérdidas de seguimiento, probabilidades de supervivencia libre de enfermedad a 5 y 10 años del 77% (I.C. del 95%: 72; 83) y del 64% (I.C. del 95%: 56; 73), de conservar el ojo a 5 y 10 años del 74% (I.C. del 95 %: 68; 80) y del 63% (I.C. del 95%: 55; 71), de conservan una agudeza visual de al menos 0,05 a 1, 5 y 10 años del 64% (I.C. del 95%: 57; 71), del 28% (I.C. del 95%: 21; 35) y del 15% (I.C. del 95%: 8; 23). La probabilidad a 5 años de enucleación por complicaciones fue del 11% (I.C. del 95%: 6; 16) en melanomas de coroides y del 58% (I.C. del 95%: 27; 89) en melanomas de cuerpo ciliar (p < 0.001). CONCLUSIONES: la braquiterapia con hilos de iridio-192 consigue un control de la enfermedad similar a las placas de iodo-125; sin embargo, la incidencia de enucleación por complicaciones de la radiación es aparentemente mayor en la técnica con iridio-192, especialmente en melanomas de cuerpo ciliar. PURPOSE: To assess the long-term efficay and safeness of iridium-192 wires brachytherapy in posterior uveal melanoma. MATERIAL AND METHODS: Non-controlled retrospective series of 241 patients, treated from January 1983 to July 2005 in a national referral hospital. RESULTS: The mean follow-up was of 5.4 years, with 17% of the patients lost for follow-up. The 5-year and 10-year probability of disease-free survival was 77% (95% IC: 56; 73) and 64% (95% IC: 56; 73); and of eye conservation, 74% (95% IC: 68; 80) and 63% (95% IC: 55; 71). The 1-year, 5-year and 10-year probability of visual acuity conservation of at least 20/400 was 64% (95% IC: 57; 71), 28% (95% IC: 21; 35) and 15% (95% IC: 8; 23), respectively. The 5-year probability of enucleation due to complications was 11% (95% IC: 6; 16) for choroidal melanomas, and 58% (95% IC: 27; 89) for ciliary body melanomas (p < 0.001). CONCLUSION: Iridium-192 wire brachytherapy achieves a disease control similar to that obtained with iodine-125 plaques, but the rate of complications requiring a secondary enucleation seems to be greater with the iridium-192 procedure, especially in ciliary body melanomas.

97 lactantes de bajo peso de nacimiento fueron seguidos en forma prospectiva desde los 3 hasta los 12 meses de edad, para comparar dos formas de administrar hierro. El grupo A recibió una leche fortificada con sulfato ferroso y ácido ascórbico que suministra Fe++ en dosis de 2,5 mg/kg/día, en el volumen de leche diario. El grupo B recibió una leche similar, pero sin fortificación, y se agregó sulfato ferroso medicinal (Fe++ 2,5 mg/kg/día) en una o dos dosis. Se compararon parámetros de nutrición de hierro en ambos grupos con la prueba t student. Resultados: al inicio, y a los 3 meses de estudio, se observaron valores hematológicos similares en ambos grupos para hemoglobina, saturación de transferrina y ferritina, en tanto que la protoporfirina tuvo un valor significativamente más alto en el grupo A. A los 6 - 9 y 12 meses se observó un valor significativamente mayor de hemoglobina y saturación de transferrina en el grupo A, en tanto que la protoporfirina a los 6 meses fue igual y a los 9 y 12 meses tuvo un valor significativamente menor en el grupo A. La ferritina fue similar durante todo el estudio en ambos grupos. Estos hallazgos indican una mejor nutrición de hierro para el grupo A de leche fortificada con hierro. En el grupo B de hierro medicinal hubo una progresiva disminución en la administración del medicamento a medida que el estudio progresaba, indicando que no es un procedimiento adecuado para suministrar hierro por un período prolongado de tiempo. Los resultados también muestran que la dosis de Fe++ debe ser aumentada antes de los 6 a 9 meses de edad, para evitar carencia de hierro en un subgrupo de lactantes de bajo peso de nacimiento 97 low birth weight babies were prospectively studied from 3 to 12 months of age. Group A received an iron fortified formula with ferrous sulphate and ascorbic acid with a dose of 2.5mg/kg/day of iron in their daily feed. Group B received a similar formula but not fortified, iron 2.5mg/kg/day in the form of ferrous sulphate was added in 1 or 2 doses. Using the t-test iron nutritional parameters were compared between groups A and B. Results: At 3 months the values for haemoglobin, transferrin saturation and ferritin were similar, the protoporphyrins were significantly higher in group. A. At 6-9 months and 12 months significantly higher values for haemoglobin and transferrin saturation were observed in group. A, while protoporphyrins were similar at 6 months, but significantly lower at 9 and 12 months in group A. The ferritin had similar values in all the study. A better iron nutrition was obtained in group A receiving the iron fortified formula, while in group B compliance decreased as the study progressed, demonstrating that it is an inadequate method of giving iron for prolonged periods. The results indicated in a subgroup of low birth weight infants aged less than 9 months higher doses of iron were necessary to treat iron deficiency

Iron bioavailability from a modified cow milk, formula (LPM) (which included additional iron as 5 mg/l ferrous sulfate and ascorbic acid, 50 mg/l) was examined in 11 adult women using the extrinsic radioactive tag method. The geometric mean absorption from LPM was 13% (reference dose absorption = 26%). The effect of this formula on iron nutrition of infants was studied in 43 healthy term infants weaned spontaneously before 3 months of age which were given LPM until 9 months of age. Another 45 infants (controls) were fed full fat, non fortified powdered cow's milk (LP) as delivered through the National Complementary Food Program (PNAC-Chile) and additional 45 infants were given fortified a cow's milk formula (LF) with ferrous sulfate (iron: 15 mg/l and ascorbic acid: 100 mg/l). Solid foods (vegetables and meat) were introduced to all these babies from age 4 months. At 9 months of age 4% infants fed LPM had evidence of anemia vs. 2% and 20% of infants given LF and LP formulae respectively. This results suggests that LPM is effective in preventing iron deficiency anemia in infants.

Objective: Rituximab-induced immunosuppression could be a risk factor for mortality from COVID-19. The aim of the study was to describe the prevalence of SARS-CoV-2 infection in patients who have received rituximab and its association with a persistent viral infection., Methods: Retrospective observational study of patients who received rituximab in the 6 months before to the onset of the pandemic. We analyzed the presence of infection and associated them with demographic variables, pathological history related to an increased risk of developing severe COVID-19, the doses of rituximab received, the type of ventilatory support, thromboembolic events, and the treatment received. A descriptive analysis of all the variables was carried out and infected and uninfected patients were compared., Results: We screened a total of 68 patients who had received rituximab (median cumulative dose: 4,161mg (2,611-8,187.5)). 54.4% men, mean age 60.8 years (15.7; 25-87)). C + was confirmed for 22 patients. Of these, 45.5% had high blood pressure, 36.4% Diabetes Mellitus, 31.8% smokers/ex-smoker, 22.7% lung disease, 13.6% heart disease and 4.5% obesity. There were no statistically significant differences between C+ and C-. Only 2 patients developed immunity. For 10 patients (45.5%) did not have a negative CRP until the end of the follow-up. There was no association with cumulative dose of rituximab. The mortality rate was 22.7% in the C+., Conclusions: We observe that the persistence of the infection leads to a worse evolution of COVID-19. The use of alternatives should be considered during the pandemic, because of patients with decreased B-cell function may have high risk of fatal progression from COVID-19., (©The Author 2022. - Abstract
The Catalonian Newborn Screening Program (CNSP) began in 1969, in Barcelona. It was promoted by Dr. Juan Sabater Tobella and supported by Barcelona Provincial Council and Juan March Foundation. That is how the Institute of Clinical Biochemistry was born, whose aims were diagnosis, research and teaching, along with the spirit of contributing to the prevention of mental retardation. The CNSP began with the detection of phenylketonuria (PKU), and, in 1982, the Program was expanded with the inclusion of congenital hypothyroidism detection. Towards 1990, the Program covered almost 100% of all newborns (NB) in Catalonia. In 1999, the CNSP was expanded with the incorporation of cystic fibrosis. It took fourteen years, until 2013, to make the largest expansion so far, with the incorporation of 19 metabolic diseases to the screening panel. The detection of sickle cell disease began in 2015 and in 2017 the detection of severe combined immunodeficiency was included. Currently, the CNSP includes 24 diseases in its main panel. Since 1969, 2,787,807 NBs have been screened, of whom 1,724 have been diagnosed with any of these diseases, and 252 of other disorders by differential diagnosis with those included in the main panel. The global prevalence is 1: 1,617 NBs affected by any of the diseases included in the CNSP and 1: 1,140 NBs if incidental findings diagnosed through the CNSP are included.

Introduction: Despite the frequency with which anaemia is present in patients with chronic kidney disease (CKD), its relationship with gastrointestinal lesions has not been studied., Method: A cross-sectional, analytical, observational study involving one year of recruitment was carried out to determine the prevalence of endoscopic gastrointestinal lesions and associated risk factors in asymptomatic patients with chronic kidney disease stages 1-5 and anaemia who had a positive qualitative immunochemical faecal occult blood test., Results: A total of 9,658 patients with CKD were analysed, of which 286 (2.9%) had anaemia; 198 had a positive faecal occult blood test (47% male, 71.1±11.8 years). The endoscopic study revealed 255 lesions, with at least one lesion in 68.2% of patients, with the most prevalent being: adenomatous colorectal polyps (39.6%), acute lesions of the gastric mucosa (22.6%), neoplastic lesions 15.1%), angiodysplasia (14.4%), oesophagitis (8.4%), inflammatory bowel disease (4.8%) and ischaemic colitis (3.1%). Uraemia and acetylsalicylic acid were identified as risk factors for acute gastric mucosal lesions. Angiodysplasia was associated with alcoholism, a more advanced stage of chronic kidney disease, anaemia, and lack of response to erythropoiesis-stimulating agents. Age and refractory anaemia were risk factors for adenomatous polyps and colorectal cancer., Conclusion: Renal patients with anaemia could benefit from an endoscopic study due to their high prevalence of gastrointestinal lesions, particularly adenomatous polyps and colorectal cancer, which are more common in those over 50 years of age with CKD stages 3-5., (Copyright © 2018 Sociedad Española de Nefrología. - Abstract
Introduction: Establishing a home enteral nutrition (HEN) that is adapted to the patient's needs and preferences can improve their quality of life and adherence, contributing to a higher treatment efficiency, better prognosis and cost reduction. As in most cases, patients need total or partial help from their caregivers, and there is a need for evaluating how well do caregivers know their patients' priorities., Objectives: Exploring patients' preferences for HEN characteristics in Spain, and the concordance between patients and caregivers., Methods: A cross-sectional observational study based on the discrete choice experiment methodology. A set of eight choice scenarios generated from six attributes with two levels each was presented along with an ad hoc questionnaire on the importance and satisfaction with HEN. The relative importance (RI) of each attribute and the patient-caregiver concordance was estimated in every question., Results: One hundred and forty-eight patients participated, and in 77 cases both the patient and the caregiver took the survey. The most important attributes (RI) for HEN were adaptation to comorbidities (33%), tolerability (33%), nutrients and calories (26%) and package characteristics (8%). Patients showed a high degree of satisfaction with the assistance received. Concordance between patient and caregiver was found in every question, being it moderate to good., Conclusions: According to patients, an ideal HEN product would be adaptable to comorbidities, easy to tolerate, providing the necessary nutrients and calories, with an easy to handle package. Caregivers know well their patients' preferences.

42. Spanish multicentre PIBHE study: Prevalence and immunization of chronic hepatitis B in haemodialysis patients in Spain.

Introduction: The PIBHE study, promoted by the Spanish Liver and Kidney Association and the Dialysis Virus Group of the Spanish Society of Nephrology, is the first study to determine the status of haemodialysis patients with chronic HBV infection and the immunisation against the vaccine., Method: The study has a national multicentre, observational, cross-sectional design and was carried out between January 2013 and 2014. A data collection folder was sent to all the nephrology departments and outpatient haemodialysis units in Spain, to be completed based on patient medical files after informed consent. The data were recorded in a central database., Results: A total of 215 centres participated (15,645 patients), with an HBV prevalence of 1.03%. HCV or HIV was present in 7.2% of the HBV(+) patients. Viral load was below 2,000 IU/ml in 80%. GOT and GPT levels were 19.1±10.1 and 15.9±9.6 IU/ml, respectively. Liver biopsy was performed in 7.1%. Antiviral treatment was prescribed in 30% and suspended in 12.5%: entecavir (13.3%), lamivudine (10%), adefovir and tenofovir (6.7%), and interferon (3.3%). A total of 34.5% were candidates for renal transplantation and 6.9% had not been evaluated; 64.3% were followed up by a gastroenterologist; 27.2% of HBV(-) patients without immunisation had not been vaccinated. Fourteen different immunisation schedules had been used, with an immunisation rate of 58.8%. Mean anti-HBs stood at 165.7±297.8mIU/ml. A total of 72.7% of patients had received a vaccination course; 26.4%, 2 cycles; 1.0%, 3 cycles; and 11.6%, a booster dose. A total of 28.3% had a poor response (anti-HBs 10-99mIU/ml); 22.4%, an optimal response (anti-HBs 100-999mIU/ml); and 7.9%, an excellent response (anti-HBs ≥ 1,000mIU/ml). Age was significantly associated with response to vaccination; the mean age of nonresponders was significantly higher than patients who had a response of any kind (P<.05). The highest probability of an immune response was achieved with 4 doses of 40 mcg of adjuvanted vaccine (OR: 7.3; 95% CI 3.4 to 15.7), for the same age and number of cycles and boosters. Age, adjuvanted vaccine, dose and vaccination schedule influenced the immune response and the anti-HBs titres reached (P<.05)., Conclusion: The prevalence of chronic HBV infection in haemodialysis in Spain is low and so are the rates of immunisation against the virus. The vaccination schedules used are very diverse and have been observed to correlate with the immune response. It would therefore be necessary to establish a protocol for the most effective vaccination schedule to increase immunisation in these patients., (Copyright © 2015 Sociedad Española de Nefrología. Introduction: Current epidemiological transition from infectious to chronic-degenerative diseases is more and more demanding procedures requiring contrast medium, which turns out to be a risk factor for acute renal major damage in patients with chronic kidney disease. The aim of this article is to describe the frequency and evolution of Contrast-induced nephropathy in ambulatory patients with chronic kidney disease undergoing percutaneous coronary intervention., Methods: We included patients with renal damage at the time of exposure to the contrast medium. They took a risk and were adjusted prevention measures follow at 48 hrs. Statistical analysis was performed using descriptive statistics., Results: From 1236 cardiac catheterization procedure performed, 32 patients met the inclusion criteria. The 96.87% had chronic degenerative diseases such as diabetes or hypertension, overweight and obesity were a constant in 65.62% of cases, in 21.7% of patients the contrast dose was slightly higher than recommended. The 6.3% developed contrast-induced nephropathy with elevated creatinine greater than 25% at 48 hours after the procedure, despite preventive measures, however none of them required renal replacement., Conclusions: The frequency of contrast medium-induced nephropathy in patients with high risk factors was lower than that reported in the literature; this may correspond to that in all cases nephrology assessment was performed before the procedure.

- Abstract
Thalassiosira pseudonana is a marine Bacillariophyta commonly used as live feed in mariculture. The growth rate and biochemical composition of microalgae are highly influenced by environmental factors such as, irradiance and nutrient availability. The aim of this study was to investigate the influence of three irradiances (60, 120 and 180 microE/m2.s) and two culture media (Algal and Humus) on growth and biochemical composition of this diatom. The microalga was grown semicontinuously at a daily renewal rate of fresh media of 30%, 37 per thousand salinity, 25 +/- 1 degree C and constant aeration (200 mL/min). The cell densities (cel/mL) and contents of protein, lipid, carbohydrate, chlorophyll a, total carotenoids, and fatty acids, showed significant differences (p < 0.05) between treatments. During steady-state phase, the maximal cell density, and lipid and carbohydrate contents were of 4.62 x 10(6) cel/mL, 20.3 +/- 2.28% and 16.6 +/- 2.43%, respectively, and were achieved in Humus medium at 180 microE/ m2.s. Moreover, highest protein contents (45.0 +/- 5.05%) and total carotenoids (0.5 +/- 0.01%) were obtained in Algal medium at 180 microE/m2.s. Chlorophyll a (0.93 +/- 0.04%) was higher at low irradiances in Algal medium. In both media, the fatty acids unsaturation degree was lower with increasing irradiance, being eicosapentaenoic acid, 20:5 n-3 (EPA) most represented (6.20%) in Algal medium at 60 microE/m2.s. This strain of T. pseudonana showed multiple physiological responses to changes in culture conditions, and may be cultivated with an alternative medium, which reduced the operating costs and allowed a high nutritional biomass production value for animals under culture.

Introduction and Objectives: CYP2C19*2 and CYP2C19*17 alleles appear to contribute to heterogeneous clopidogrel metabolism. The aims of the present study were to assess the phenotype-genotype relationship of CYP2C19*2 and *17 allele carriage and to explore the clinical impact of those polymorphisms at 6-month follow-up of an acute event in an unselected population of non-ST elevation acute coronary syndrome., Methods: Recruitment for the first and second objectives was 40 stable acute coronary syndrome patients under dual antiplatelet therapy at 12 months after coronary stent placement and an unselected population of 493 consecutive patients with non-ST elevation acute coronary syndrome, respectively. Platelet reactivity was assessed by optical aggregometry induced by adenosine diphosphate and thrombin receptor activating peptide, and by vasodilator-stimulated phosphoprotein phosphorylation measurement using flow cytometry. Genotypes were determined with a TaqMan assay., Results: Only the vasodilator-stimulated phosphoprotein phosphorylation measurement detected significant differences in on-clopidogrel platelet reactivity between the wild-type subjects and the CYP2C19*2 (P=.020) and *17 allele carriers (P=.048). No significant difference was found between CYP2C19*2 ([HR (95%CI): 1 (0.94-1.55)], P=.984) or *17 ([HR (95%CI): 0.93 (0.61-1.43)], P=.753) allele carriage and the occurrence of adverse events at 6-month follow-up., Conclusions: Even though CYP2C19 genotype is associated with variable on-clopidogrel platelet reactivity, it has no significant clinical influence. - Abstract
Rotifers are an important live feed for first feeding larvae of many fish species. The use of concentrated algae cells in the mass culture of the rotifer Brachionus plicatilis (Brachionidae) has opened new horizons for research on this organism. Pastes of Rhodomonas salina (Pyrenomonadaceae) obtained either by centrifugation or flocculation with chitosan were preserved, with or without vitamin C, at -20 degrees C for four weeks and were evaluated biochemically (proteins, lipids, pigments and fatty acids contents) and subsequently, were used to feed the rotifer Brachionus plicatilis at a ratio of 25 mg/L/day. Four different microalgae pastes were prepared: (1) centrifuged and preserved with vitamin C (CV), (2) centrifuged and preserved without vitamin C (C), (3) flocculated and with vitamin C (FV) and (4) flocculated without vitamin C (F). All treatments showed similar contents of proteins and total lipids with respect to control culture (a fresh culture of R. salina), with mean values of 40.0 +/- 2.32% and 12.0 +/- 1.45%, respectively. The pheophytin a/chlorophyll a ratio, a general indicator of the chemical status of microalgal concentrates, was similar (0.09-0.11) between centrifuged pastes and control culture, but was found to be higher in flocculated pastes (1.28-1.48). The fatty acid profile varied with respect to the control culture, mainly in the proportion of the essential polyunsaturated fatty acids (PUFAs): eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA). Total PUFAs, EPA and DHA contents were statistically similar between centrifuged pastes and control culture (PUFAs: 47%, EPA: 4% and DHA: 4.7%), whereas values obtained for flocculated pastes were significantly lower. The rotifers grew equally well when fed with centrifuged pastes or control culture (maximum density: 320 rotifers/mL; instantaneous growth rate: 0.23 rotifers/day, fecundity: 1.49 eggs/female and productivity: 43 x 10(3) rotifers/L/day. No significant effect of vitamin C was found when used as a paste preservative. We concluded that centrifugation is an effective harvesting method, and that freezing to -20 degrees C for four weeks (no vitamin added), may help maintain the nutritional quality of R. salina paste, similar to fresh microalgae and can be offered to Brachionus plicatilis.

- Abstract
Unlabelled: Incontinentia pigmenti is a genodermatosis described by Garrod and in 1920 by Bloch, Sulzberger, Siemens y Bardach. It is an ectodermic disorder that affects skin, teeth, eyes and may also have neurological problems. The IP2 name describes the histological characteristics, the incontinence of melanin into the melanocytes cells in the epidermal basal layer and its presence in superficial dermis. IP2 is an x-linked dominant condition but genetic heterogeneity may exist., Case Report: The patient was 4 yrs 5 months old when she came for the first time. In a physical exploration she presented sparse and thin hair, eyelashes and eyebrows, beaked nose, labial protrusion, the four central teeth have a conic crown and there was also a delayed eruption of other teeth, right eye strabismus, hipoacusia, language defects and a trunk, legs, feet, and face dermatosis characterized by grouped vesicles, hyperkeratotic and warty lesions and brownish-gray lesions in a lineal pattern. The patient s father had hypopigmented lesions in the posterior regions of both legs. The oral clinical and radiographic exams showed diverse anomalies. Both the patient's and the father's chromosomal studies were normal., Discussion: In the present case we can see that the father has IP2 without supernumeraries X, with the antecedent that his mother had something similar. It is possible that the inheritance was autosomic dominant or it is a different mutation of NEMO (NF-kappa-B essential modulator) gene to a classical one, which was found in some affected men. It is necessary to carry out a molecular study of these patients.
