Background: Elucidating the determinants of species’ geographic distributions is a fundamental goal of ecology and biogeography, as they can inform about key biological processes, with implications for conservation. Goal: Evaluate whether realized climatic amplitude (a proxy for niche breadth) is an ecological correlate of geographic range size in morning glories in Mexico, a highly diverse group of plants of worldwide socio-economic relevance, but still poorly studied. Methods: Based on a dataset of > 30,000 records of which > 7,000 were manually georeferenced, we calculated geographic range size (using convex hull polygons and buffered occurrence points), and climatic amplitude (using three first axes of Principal Components based on 19 Worldclim variables, and focusing on temperature, precipitation, and seasonality), and explored their relationship using general linear models and phylogenetic generalized least squares. Results: All models show that climatic amplitude explains a high proportion of variation in geographic range size for our dataset (up to > 69 %). We find no evidence of tradeoffs related to temperature and precipitation niche breadths. Through review of reported records, we update the number of species of Ipomoea reported for Mexico to 178. Conclusions: Much of the variation in geographic range size in Mexican Ipomoea can be explained by realized climatic amplitude, and this result is not driven by phylogenetic history. We discuss cases where despite large geographic range sizes, narrow climatic amplitudes can signify higher risks for species in the face of changing environments.

Introduction and Objective: Assessment of urethral stricture (US) management in a specific group of professionals, Urology Residents, in a specific region (Spain), seems to be important to determine the quality of the educational program and design educational interventions to improve it. We aim to investigate diagnosis and therapeutics practices among Urology Residents for the US management., Materials and Methods: 20-question on-line survey was conducted among residents and junior consultants registered on the mailing list of residents and young urologists of the Spanish Association of Urology (RAEU) group of the educational period 2018-2023. We evaluated demographic, educational, surgical technics and experience data during the training period. 290 questionnaires were mailed between May-August 2023. Data was collected in a prospective way between May-December 2023., Results: The survey obtained 86 responders, with 29,7% (86/290) response rate. Two first sections were answered by all the responders, however, from the assessment section on, only 57 responders completed the survey, which represents 66,3% of them., Conclusions: Educational program in Reconstructive Urology among Urology Residents in Spain has an improvement margin. Nephrolithiasis is a globally prevalent urologic condition associated with significant morbidity and patient discomfort. Current management of kidney stones includes both surgical and pharmacologic interventions. Though surgery may be necessary under certain circumstances, pharmacologic treatment is a more affordable, readily available, and a less invasive option for patients. A comprehensive scoping review was conducted to summarize the available literature on the pharmacologic strategies for managing the predominant stone types including calcium oxalate, calcium phosphate, uric acid, struvite, and cystine stones. Central to these therapeutic approaches is the regulation of factors such as urine pH, stone crystallization, and patient metabolics that precipitate stone development and growth. This review highlights the pharmacological options available for treating each kidney stone type, emphasizing the importance of patient tailored medical management that should be considered by every physician., (Copyright © 2023 The Author(s). Objective: To design and assess a novel penile fracture simulation model for teaching penile fracture repair., Methods: We used a validated circumcision simulator to create a model. Foreskin for a circumcision was divided into two halves. A transverse slit ("simulated fracture") was created on one part of the first half of the foreskin (mimicking "tunica") and was applied over the penile model. A red jelly tablet ("clot") was placed underneath the cut. A second full-length of foreskin was applied over it to cover the defect. The model was assessed by participants and expert faculty at the Urology Simulation Boot Camp. Evaluation was performed using a 5-point Likert Scale questionnaire. Data was analysed using Microsoft Excel and IBM SPSS Statistics V25. The intra-class correlation was calculated using a "One-way random model"., Results: Twenty-two urology trainees and four experts participated in the evaluation. The majority of trainees strongly agreed (59%, n = 13) the model is useful for training with experts similarly agreeing in 75% of cases. The appearance of penile fracture was considered good by both trainees (68%, n = 14) and faculty (75%). Overall, the ability of the model to represent a realistic simulation of the task was considered excellent by 23% of participants and good by 64%. Personal confidence after simulation in managing a similar situation was considered high among trainees. The main difficulties reported were related to fascial planes and urethra., Conclusion: This is the first simulation model for penile fracture repair and has demonstrated face validity at a national urology bootcamp., (Crown Copyright © 2021. Objectives: To develop a suprapubic catheter (SPC) training model as no specific realistic training model exists to replicate SPC exchange where a catheter tract is present., Materials and Methods: We describe a novel, anatomically realistic, animal simulator model for use in SPC training, which was trialed at a national urology simulation boot camp by new urology trainees and validated by expert urologists. A scale reproduction of an abdominal wall was created using a porcine abdominal wall. A segment of small bowel was stitched around a size 16F Foley catheter to form a tract. Abdominal wall tissue was excised cylindrically to create an opening, and the small bowel tract was passed through the abdominal wall and sutured anteriorly, producing a realistic SPC tract: inferiorly, the tract was anastomosed to a porcine urinary bladder. This model was evaluated by 10 expert urologists for content validity with an 8-item 5-point rating scale used to evaluate domains relevant to the simulator., Results: The domains were scored between 1 and 5 by 10 expert urologists, 1 being «strongly disagree» and 5 being «strongly agree». The average expert ratings of the domains were then calculated and tabulated following the training course. There was an average global rating of 4.2/5 for the model, with an average usefulness for training score of 4.6/5., Conclusion: The feedback from experts and trainees (informal) was overwhelmingly positive. Faecal immunochemical tests for haemoglobin (FIT), as an adjunct to clinical information, assist in the triage of patients presenting in primary care with lower abdominal symptoms. Controversy remains regarding whether and which qualitative and quantitative FIT can be used, which groups of patients would benefit most from FIT, whether FIT should be done in primary and/or secondary care, and how FIT should be incorporated into diagnostic pathways. Controversy also exists as to the optimum cut-off used for referral for colonoscopy. A single sample of faeces may be sufficient. Reporting of results requires consideration. FIT provide a good rule in test for colorectal cancer and a good rule out test for significant bowel disease, but robust safety-netting is required for patients with negative results and ongoing symptoms. Risk scoring models have been developed, but their value is unclear as yet. Further evaluation of these topics is required to inform good practice., (Copyright © 2018 Elsevier España, S.L.U. Objective: this study explores the process of the development of an intention to leave bedside nursing., Method: the process was studied from the perspective of 21 nurses using the grounded theory method. Data were collected using semi-structured interviews and the constant comparative method of Corbin and Strauss was used for data analysis., Results: according to the participants, the two main categories, "social image of nursing", and "culture and structure of the bedside", were the contextual factors that influence why nurses are leaving bedside care provision. Disappointment with a perceived lack of progress or improvement in the clinical experience formed primary psychosocial concerns for the participants. Competence and a process of self-control were steps taken by the participants. These, associated with interventional conditions produced the outcomes of the loss of professional commitment and desire to leave bedside nursing. "Failure to integrate personal expectations with organizational expectations: in search of escape" was the central category of the study that linked the categories together., Conclusion: the findings of this study provide useful information about the needs of nurses for overcoming the intention to leave bedside care. The identification of this process can help in recognizing emerging problems and providing solutions for them.

The aim of this study is the methodological evaluation of Clinical Practice Guidelines (CPG) in hypertension. This is the first in a series of review articles, analysis, assessment in methodology and content of clinical practice guidelines in Cardiology. Of all clinical practice guidelines, three were selected and the Appraisal of Guidelines for Research and Evaluation (AGREE II) instrument was used to assess each guide. The guidelines obtained the lowest score in the domain of applicability (mean 43.8%); while the highest score was for clarity of presentation (mean 81.5%). The lowest percentage was found in the applicability domain (European guideline) and the highest of all scores was found in two domains: scope and purpose, and clarity of presentation (Canadian guideline). Assessing the quality of the clinical practice guidelines analyzed, the Canadian is one with the best scores obtained by applying the AGREE II instrument, and it is advised to be used without modifications.

Flying phobias are common. Mishaps and accidents to aircraft make many travellers apprehensive and fearful of flying, a condition which most overcome by rationalisation and thought blocking, others develop a phobia for flying. This condition responds well to hypnotherapy which can be however be time consuming. Limited session brief, focused, selective hypnotherapy can however cure disabling fear, disturbed conditioned responses and avoidance behaviour.

An opportunistic questionnaire study of peoples attitudes to, commercial flying and their behavioural responses after the events of September 11 2001 in the USA. Cohorts drawn from people attending a series of educational lectures, a specific leisure time activity and a travel health clinic 6 months after the disasters. More people appeared to worry about air travel 6 months after Sept 11 2001 than in reports prior to this date and the worried seem to experience a greater intensity of anxiety.

A detailed account is given of the occurrence of bovine spongiform encephalopathy (BSE), current research into the aetiology of this new disease of cattle, and the relationship between BSE, scrapie and other similar diseases. Epidemiology, clinical signs, pathology, diagnosis, prevention and control are described.

This review of internal parasitic diseases discusses methods of immune diagnosis, immune mechanisms of the host, and immunoprophylaxis against both protozoan and metazoan parasites of livestock. The limiting factors for immunodiagnosis are specificity and whether the host animals respond sufficiently to produce detectable serological responses. Because the array of antigens is so complex, the method of choice for helminths is to use excretory/secretory products of the parasite as antigens for microplate ELISA. Similarly, for protozoa, immunodiagnosis is facilitated by using a few, selected, immunodominant antigens rather than extracts of whole parasite. However, serodiagnosis does not always identify parasitised animals especially in the young, malnourished or sick host. Similarly, parasites may themselves modulate the response of the host and remain serologically undetectable. The production of recombinant, pure, parasite proteins is already facilitating diagnostic serology for several parasite species and, using recombinant DNA probes, it is now possible to distinguish inter- and intra-species differences. The central role of inflammatory cytokines, the regulatory proteins produced by activated T helper cells, in modulating both protective and potentially damaging inflammatory responses is discussed. Finally, the genetic control of the immune response and its relevance to immunoprophylaxis is considered. Several interesting developments in the production of sub-unit vaccines and the potential methods of delivery are appraised.
