Resumen Introducción: La enfermedad renal crónica (ERC), produce una pérdida gradual e irreversible de la función de los riñones. Sus modalidades de tratamiento son invasivas y la existencia de un acceso vascular adecuado para hemodiálisis es fundamental para garantizar la supervivencia de los pacientes. Las diferencias de género existen con respecto a la epidemiología, la evolución y el pronóstico de las enfermedades crónicas del riñón, y pueden existir diferencias en las respuestas y perspectivas de las mujeres ante la ERC. Objetivos: Describir la experiencia de los pacientes con enfermedad renal crónica en hemodiálisis; su experiencia como portadores de un catéter venoso central (CVC), y si existen diferencias de género en las experiencias y vivencias de los portadores/as. Material y Método: Estudio cualitativo fenomenológico. Se aplicó un muestreo por propósito, se recogieron datos a través de entrevistas en profundidad, y se aplicó un análisis temático. Resultados: La enfermedad renal crónica supone un gran impacto en sus vidas. Todos los pacientes narran su preferencia sobre el CVC, son conscientes de su implicación en su tratamiento y en su expectativa vital, y se preocupan de su viabilidad y seguridad. Conclusiones: Existen diferencias de género, sobre todo en el impacto de la enfermedad y el tratamiento en el trabajo y en su nivel de autonomía y actividad. Abstract Introduction: Chronic Kidney Disease (CKD) is a disease that involves the gradual and irreversible loss of kidney function. The existence of adequate vascular access for hemodialysis is fundamental to guarantee a survival of the patients. Gender differences exist with respect to the epidemiology, evolution and prognosis of chronic kidney diseases. There are differences in the responses and perspectives of women on CKD. Objectives: To describe the experience of patients with CKD on hemodialysis; their experience as holders of a central venous catheter (CVC), and if there are gender differences in the responses and experiences of the holders. Material and Method: Qualitative phenomenological study was applied. Purposeful sampling was used and in-depth interviews was applied by researchers. Also, thematic analysis was used to qualitative data analysis. Results: CKD has a great impact on the patients’ lives. All patients report their preference about the use of CVC, are aware of the involvement in the treatment and their life expectancy, and are concerned about the viability and safety. Conclusions: There are gender differences, especially in the impact of the disease and treatment at work and in their level of autonomy and activity.

Resumen El objetivo del estudio fue explorar el estado psíquico de profesores de educación primaria de varios centros públicos y concertados de la Comunidad de Madrid. Se evaluó la sintomatología ansiosa y depresiva y el síndrome de burnout, mediante la Escala HADS y el Cuestionario de Burnout del Profesorado. Los resultados indicaron que, aunque los docentes de colegios concertados reportan más sintomatología ansiosa, los profesores de colegios públicos presentan mayor presencia del síndrome de burnout en las escalas de cuestiones organizacionales, supervisión y reconocimiento. Se evidencia que los profesores de educación primaria son profesionales de alto riesgo, a pesar de que los estudios se han centrado mayoritariamente en educación secundaria. Abstract The study objective was to explore the psychic state of Primary Education teachers of several public and concerted centers of the community of Madrid. We evaluated anxiety and depression symptoms and burnout syndrome using the HADS scale and the Burnout Questionnaire of teachers. The results indicated that, although concerted schools teachers report more symptoms anxious, teachers of public schools have a greater presence of the Burnout syndrome in the scales of organizational issues, supervision and recognition. It is evident that the primary education teachers are high-risk professionals, despite the fact that the studies have focused mainly in Secondary Education.

7. Supplemental nursing staff ́s experiences at a Spanish hospital: Qualitative phenomenology research

The objective of this study was to describe the Supplemental Nursing Staff ́s experiences at different hospital units. A qualitative phenomenological approach was conducted; a purposeful and theoretical sampling was implemented with supplemental nursing staff at Santa Barbara Hospital of Soria (Spain), to gain a more in-depth understanding of the Supplemental Nursing Staff ́s experience. Data were collected by in-depth interviews and through a field notebook. Data were analyzed using the Giorgi proposal. Twenty-one nurses with a mean age of 46 years were included. Three main topics emerged from the data analysis: building the first contact, carving out a niche and establishing interprofessional/interpersonal relationships. We conclude that the experience of hosting the supplemental nursing staff in changing clinical environments is conditioned by various factors. It is necessary for nurses and hospital managers to establish clear objectives with regard to the supplemental nursing staff ́s role in the units. El propósito de este estudio es describir la experiencia de las enfermeras “de apoyo” en las diferentes unidades de hospitalización. Seguimos un enfoque fenomenológico cualitativo. Realizamos un muestreo por propósito y teórico de dicho personal del Hospital Santa Bárbara de Soria (España), con el fin de obtener una comprensión más profunda del fenómeno. Los datos fueron recolectados a través de entrevistas en profundidad y diario de campo. Los datosfueron analizados usando la propuesta de Giorgi. Veintiuna enfermeras “de apoyo” con una media de edad de 46 años fueron incluidos. Tres temas principales que describen la experiencia de los enfermeros emergieron de los datos: construyendo el primer contacto, buscando un hueco y estableciendo relaciones interprofesionales/interpersonales. Concluimos que la experiencia de acoger dichas enfermeras en entornos clínicos cambiantes esta condicionada por diferentes factores. Es preciso establecer objetivos claros respecto al rol de las mismas en las unidades por enfermeras y gestores. O propósito deste estudo é descrever a experiência das enfermeiras de turno nas diferentes unidades hospitalares. Seguiuse um enfoque fenomenológico e qualitativo. Realizamos uma amostragem teórica com o pessoal de enfermagem de turno do Hospital Santa Bárbara de Soria (Espanha), com a finalidade de obter uma compreensão mais profunda do fenômeno. Os dados foram coletados a través de entrevistas e de diário de campo. Os dados foram analisados usando a proposta de Giorgi. 21 enfermeiros de turno com uma média de idade de 46 anos foram incluídos. Três temas principais, que descrevem a experiência das enfermeiras, emergiram dos dados: construindo o primeiro contato, buscando um espaço e estabelecendo relações interprofissionais/interpressoais. Concluimos que a experiência de acolher as enfermeiras de turno em ambientes clínicos mutáveis está condicionada por diferentes fatores. É preciso estabelecer objetivos claros a respeito do papel das enfermeiras de turno nas unidades por parte de enfermeiras e gestores.

Los métodos cualitativos pueden ayudar a comprender y mejorar las intervenciones mé- dicas (clínica, educación, investigación y gestión) y las respuestas de los pacientes. Una de las fases clave en la investigación cualitativa es el análisis de los datos cualitativos. El análisis se fundamenta en la identificación de aquellos elementos relevantes de los participantes, sobre la base de su experiencia, situación social, entorno y expectativas. Objetivo: Describir las estrategias de análisis para los datos cualitativos, aplicados en investigación médica. Diseño: Revisión de la bibliografía de referencia sobre metodología cualitativa para el desarrollo de estrategias prácticas de análisis de datos cualitativos. Resultados: Se describen aproximaciones de análisis cualitativo que pueden aplicarse de manera deductiva e inductiva, junto al empleo de la codificación para guiar el análisis y la interpretación. Conclusión: Las investigaciones cualitativas pueden mejorar la descripción de fenómenos complejos en la investigación médica y en la práctica clínica. El análisis de datos cualitativos es un instrumento que ayuda a los médicos en su práctica clínica y en sus investigaciones. Qualitative methods can be used to help understand and improve medical interventions (clinical intervention, education, research and development), as well as patient responses. One of the critical phases in qualitative research is the analyses of qualitative data. These analyses are based on the identification of relevant features in participating patients. Their particular experiences, social situation, environment, and expectations should be considered. Objective: The purpose of this paper was to offer practical strategies for the analysis of qualitative data. Design: We reviewed qualitative methodological literature to describe practical approaches to qualitative data analysis. Results: We describe an approach to qualitative data analysis that applies the principles of inductive and deductive reasoning while also employing predetermined code types to guide data analysis and interpretation. Conclusions: Qualitative research can improve the description of complex phenomena in medical research and clinical practice. Desde hace una década, se ha observado un aumento progresivo del envejecimiento de la población, junto a un elevado consumo de recursos y demanda de asistencia sanitaria (Castellote Varona, 1999; Bazo & Domínguez Alcón, 1996) y social. Por lo que surge la necesidad de reorientar los servicios sanitarios hacia la atención de las personas mayores (Haan et al, 1997); la presencia de unidades de valoración y de atención geriátrica (Solano Jaurrieta et al, 1998; Abizanda Soler et al, 1998; Baztán Cortes et al, 2000), junto a los programas específicos en atención primaria son sólo una muestra. La presencia de una atención más especializada y centrada en los problemas y necesidades de las personas mayores se acentúa ante la presencia de un sector de la población mayor más vulnerable a lesiones y complicaciones, es la población mayor considerada frágil. Además de esta, es preciso definir y diferenciar conceptos clave en la atención a la persona mayor como la geriatría y gerontología, ya que una u otra harán referencia a una atención determinada e intervendrán unos determinados profesionales sanitarios y no sanitarios. Dentro de este capítulo se pretende: 1º. Definir los conceptos de geriatría y gerontología, 2º definir al anciano frágil y la fragilidad, 3º mostrar sus características y 4º explicar las características de la presentación de la enfermedad en las personas mayores. Introduction: Medical Records have a large number of abbreviations and doctors and nurses may not be aware of their meaning, which could compromise patient safety., Objective: To evaluate the knowledge of doctors and nurses of the clinical abbreviations in medical discharge reports., Methods: Observational-cross sectional study through a questionnaire developed ad hoc for doctors and nurses from Hospital Universitario de Fuenlabrada. The content and logical validity of the questionnaire was assessed. The questionnaire was completed anonymously and voluntarily. The questionnaire was also distributed online to the professionals' corporate emails. The questionnaire included sociodemographic variables and 14 abbreviations present in medical discharge reports. The data were obtained from the Electronic Clinical Record., Results: Out of a total of 756 professionals, the questionnaire was answered by 68 doctors and 86 nurses (n=154).The mean age of the professionals was 40.58 years (SD ±7.54), and the mean number of years of professional experience was 17.10s (SD ±7.37). The professionals gave an average percentage of correct answers of 35.84%. Doctors gave 55.94% of the correct answers, and nurses 23.17%. The abbreviations for which the most errors occurred were SNG, NPIM, EEA, RCP, with a success rate of 5.19%, 6.49%, 6.49% and 7.79%, respectively., Conclusions: The identification of the abbreviations in medical discharge reports by doctors is superior to that of nursing staff. Aim: To describe the experience of the parents of children hospitalized due to bronchiolitis., Method: Phenomenological qualitative study. Purposeful sampling was undertaken of parents with children diagnosed with bronchiolitis, from December 7, 2015 to January 8, 2016, and admitted to the paediatric department of a public hospital of the Madrid Health Service. Data collection strategies included focus groups and researchers' field notes. Afterwards, the data were analyzed using thematic analysis. We included 10 parents, aged between 30 and 39 years. Five themes were explored: perception of monitoring, need to know, perception of child fragility, coping strategies, and reorganization of the family environment., Results: Oxygen saturation monitoring through devices produced dependence and uncertainty in the parents. In addition, there was a need to know and understand the technical language, which encouraged searching for information on the Internet. The parents perceived the fragility of the hospitalized child through their appearance and physical state. In addition, they developed different coping strategies to seek and confirm information, to approach professionals and be present with their children. Finally, the family was reorganised to maintain the constant presence of the parents during hospitalization., Conclusions: The results obtained can help paediatric professionals manage the information given to parents and thus empower them., (Copyright © 2018 Elsevier España, S.L.U. Objective: To describe the difficulties perceived by nursing staff in the delivery of end-of-life care to critically ill patients within intensive care units (ICU)., Method: A descriptive phenomenological qualitative study was performed. A purposeful and snowball sampling of nursing staff with at least 1 year's previous experience working in an ICU was conducted. Twenty-two participants were enrolled. Data collection strategies included in-depth unstructured and semi-structured interviews and researcher's field notes. Data were analysed using the Giorgi proposal., Results: Three themes were identified: academic-cultural barriers, related to the care orientation of the ICU and lack of training in end of life care; architectural-structural barriers, related to the lack of space and privacy for the patient and family in the last moments of life; and psycho-emotional barriers, related to the use of emotional detachment as a strategy applied by nursing staff., Conclusions: Nursing staff need proper training on end-of-life care through the use of guidelines or protocols and the development of coping strategies, in addition to a change in the organisation of the ICU dedicated to the terminal care of critically ill patients and family support., (Copyright © 2017 SESPAS. There are different types of therapeutic limitation, and the patient can be classified according to measures likely to withdraw or not start. Among such measures include removal of stand mechanical ventilation with the analgesia and sedation applied to the end of life of the patient.

- Abstract
Objectives: Knowing the meaning of hospitalization for the family caregiver of the hospitalized sick person., Method: Qualitative study using grounded theory. Participants were included if they met the following criteria: a) being part of the family; b) the sick person was admitted into a hospital in the state of Chihuahua; and c) the hospitalized person was over 18 years old. Sampling was applied by purpose. Data collection was conducted through in-depth interviews with an opening question. Similarly, memos were applied in the process of gathering. The analysis applied was based on grounded theory, with an open, axial and selective coding, identifying the categories and subcategories. During the analysis process the method of constant comparison was applied., Results: Eighteen participants were included; 16 of whom were female. The mean age was 48.11 years old. The core category was identified: "the naked soul", formed by the categories; "I deal with the disease" which describes the situations experienced by the caregiver during and after the hospitalization process; "wartime" makes reference to the family problems that develop or worsen during the hospitalization process; and "dancing in the storm" focuses on all the positive changes occurring in the primary caregiver., Conclusions: Hospitalization causes uncertainty in the caregiver. 16. Nurses' experiences working with nursing students in a hospital: a phenomenological enquiry.

Objective: this paper explores the experiences of registered nurses working with Spanish nursing students within the hospital., Methods: a qualitative phenomenological approach was followed. Purposeful sampling was employed. Twenty-one registered nurses, from a public hospital located in Spain, were included in the study. Data were collected by means of unstructured and semi-structured interviews and were analysed using Giorgi's proposal. The Consolidated Criteria for Reporting Qualitative Research were followed., Results: three main themes described the experience of registered nurses: "The nurse's relationship with nursing students"; most nurses emphasized the importance of the first contact with students and they considered students' attitude to be key. "Defining the role of the student in clinical practice"; it is necessary to unify the nurse's role and interventions to avoid misleading students and establish priorities in clinical practice. "Building bridges between clinical settings and the University"; the need to establish a common ground and connection between the university and hospital clinical settings was emphasized. Nurses felt that the training program should also be designed by the clinical settings themselves., Conclusions: understanding the meaning of nursing students with registered nurses might gain a deeper insight into their expectations.

17. [Physical activity in adult working population: results from the European National Health Survey for Spain (2009)].

Objectives: 1) To describe physical activity in the Spanish adult working population aged 16-70 years in 2009, and 2) to describe the prevalence of physical activity according to socio-demographic features, self-perceived health status, co-morbidity, and lifestyle habits., Design: An epidemiological population based descriptive study was conducted using individual data taken from the European Health Survey for Spain., Setting: Community., Participants: The number of subjects aged 16-70 years included in the study was 10,928 (5,628 women and 5,300 men)., Interventions: None., Main Variables: Physical activity and intensity were assessed from questions included in the interview-survey. An analysis was performed on the socio-demographic characteristics and self-rated physical and mental health, using multivariate logistic regression models., Results: Not having a partner (OR 1.44; P<.001), have university studies (OR 1.62; P<.001), non-smoker (OR 1.50; P<.001), and taking medications (OR 1.22; P<.5) were a predictor of intense physical activity in men. The first 3 factors are equal for intense activity in women. In contrast, obesity (OR 0.58; P<.001), and 36-50 years (OR 0.68; P<.001) were factors related to low activity in men. Aged between 36-50 years (OR 1.26; P<.01), suffering≥2 co-morbid conditions (OR 1.30; P<.001), and non-smoker (OR 1.17; P<.5) were also associated with higher probability of reporting moderate physical activity in women. The positive mental health was significant for physical activity in both sexes (OR 1.01; P<.5)., Conclusions: This study identified several factors that appear to influence physical activity in the Spanish adult working population, with potential implications for healthcare providers., (Copyright © 2014 Elsevier España, S.L.U. - Abstract
Objective: To describe the daily experience of patients with an automatic defibrillator (AD) implant and the adaptive changes of the patient., Method: Qualitative and phenomenological research. Collection of data through; initially unstructured interview with half of the informants, semi-structured interviews through an open questions guide after the initial unstructured interviews and use of personal narratives of the informants. Analysis of the data using the Van Manen proposal., Results: We analysed the interviews of 10 participants. We collected socio-demographic variables and identified the following themes, which respond to the question "How is life with an AD": It is life "with the two sides of the coin," living in constant wait and uncertainty, accepting change, developing adaptation strategies, renegotiating relationships and sexuality and it is to live transformed., Conclusions: The results of this study can be integrated into nurse clinical practice in areas such as valuation after discharge, changes in habits, control of treatment, notification of shocks, masking detection of symptoms and strategies that can jeopardise the bearer. Research needs to be developed that looks closer into the influence of other technological devices in people., (Copyright 2009 Elsevier España, S.L. All rights reserved.)

20. [Management of technology and its influence on nursing care].

- Abstract
Currently, much nursing care adheres to a specific biomedical paradigm within the positivist framework. However, sometimes nursing care cannot be adapted to numerous human or vital conditions affecting our patients, their families or the environment in which nurses work. An specific example of these nursing interventions are those applied in intensive care units (ICU) where there is a large amount of technology and nursing care is specialized. Several questions that arise are whether the above-mentioned specialization is inherent to nurse care, whether technology management forms part of nursing care, whether this care has a non-nursing origin, and what is the source of nursing knowledge. The present article aims to provide basic knowledge to distinguish the nursing care performed in the ICU within the 2 predominant paradigms in current nursing: the biomedical and the holistic paradigms. The characteristics of nursing care in both paradigms are described and an integrated vision of these 2 paradigms and of nursing care with and without the use of technology is provided.
