espanolIntroduccion: el cancer de vejiga es un problema de salud importante a nivel mundial, es el noveno cancer mas comun en hombres y mujeres y representa una causa considerable de morbimortalidad por esta enfermedad. El carcinoma escamocelular es una variedad histologica excepcional, clasificado dentro de los canceres de vejiga no uroteliales, representando menos del 5 % de todos los tumores de vejiga. Se presenta un caso de carcinoma escamocelular mal diferenciado primario de vejiga en un paciente con antecedente de nefrostomia bilateral y urolitiasis. Caso clinico: paciente masculino de 53 anos de edad, con antecedente de nefrostomia bilateral y lesion ocupante de espacio a nivel intravesical sin reporte de patologia, consulta con cuadro clinico de 6 horas de evolucion caracterizado por disfuncion de nefrostomia izquierda asociado a dolor en hipocondrio y fosa iliaca izquierda. Se realiza ecografia de vias urinarias que reporta lesion ocupante de espacio solida, heterogenea con calcificaciones en su interior que ocupa la totalidad de la topografia de la vejiga urinaria, con efecto de masa sobre las estructuras vecinas. Posteriormente, se realiza reseccion transuretral de lesion vesical, el estudio histopatologico reporta un carcinoma invasor mal diferenciado, confirmado por estudios de inmunohistoquimica como carcinoma escamocelular mal diferenciado. Conclusiones: el carcinoma escamocelular de vejiga es una neoplasia poco frecuente, tiende a ser localmente avanzada en el momento del diagnostico y es relativamente resistente a los regimenes de quimioterapia utilizados para el carcinoma urotelial. EnglishIntroduction: bladder cancer is a major health problem worldwide; it is the ninth most common cancer in men and women and represents a considerable cause of morbidity and mortality from this disease. Squamous cell carcinoma is an exceptional histological variety, classified within non-urothelial bladder cancers, representing less than 5% of all bladder tumors. We present a case of primary poorly differentiated squamous cell carcinoma of the bladder in a patient with a history of bilateral nephrostomy and urolithiasis. Case Report: a 53-year-old male patient with a history of bilateral nephrostomy in 2016 and a space-occupying lesion at the intravesical level without a report of pathology, who consulted with a clinical picture of 6 hours of evolution characterized by dysfunction of the left nephrostomy associated with pain in hypochondrium and left iliac fossa. Ultrasound of the urinary tract was performed, which reported a heterogeneous, solid space-occupying lesion with calcifications in its interior that occupies the entire topography of the urinary bladder, with a mass effect on neighboring structures. Subsequently, a transurethral resection of the bladder lesion was performed, and it was sent for histopathological study, which reported a poorly differentiated invasive carcinoma, confirmed. Conclusion: squamous cell carcinoma of the bladder is a rare neoplasm, tends to be locally advanced at the time of diagnosis, and is relatively resistant to chemotherapy regimens used for urothelial carcinoma.
