Se evaluó la capacidad antioxidante y la calidad post-descongelación del semen equino criopreservado con quercetina y ergotioneina. Nueve eyaculados provenientes de tres caballos criollos colombianos se criopreservaron bajo tres tratamientos: ergotioneina (100 pM), quercetina (100 pM) y control (sin antioxidante). Posteriormente a la descongelación se evaluaron los siguientes parámetros: la capacidad antioxidante total (CAT) del semen mediante el ensayo del ácido 2,2'-azino-bis-[3-etilbenzotiazolina]-6-sulfónico (ABTS•+); la movilid ad total (MT); la movilidad progresiva (MP); la hiperactividad (HA) y las velocidades curvilínea (VCL), lineal (VSL) y media (VAP) mediante el sistema computarizado SCA ; además, la integridad estructural de la membrana y la integridad acrosómica por microscopia de fluorescencia mediante las sondas SYBR/IP y FITC/ PNA, respectivamente; la morfología mediante la tinción eosina-nigrosina y la integridad funcional de membrana a través de la prueba hipoosmótica (HOS). Se realizó el ajuste de modelos lineales generalizados (GLM) y la comparación de medias por Tukey. La CAT (pmol trolox/ml) del semen descongelado fue superior para la ergotioneina (4,0 ± 0,3) y la quercetina (3,9 ± 0,4), respecto del control (2,6 ± 1,5). Para la MT se encontró una media superior para la ergotioneina (70,3 ± 11,2 %), respecto a la quercetina (63 ± 10,5 %) y al control (66,1 ± 11,2 %) (P < 0,05). Para MP, HA, VCL, VSL y VAP, el tratamiento control presentó valores superiores a los tratamientos con antioxidantes (P < 0,05). Se concluye que la ergotioneina y la quercetina incrementan la CAT e influyen sobre la movilidad y la cinética post-descongelación del semen equino. The aim of this study was to evaluate the antioxidant capacity and post-thaw quality of stallion semen cryopreserved with quercetin and ergothioneine. Nine ejaculates from three Colombian Creole horses were cryopreserved under three treatments: ergothioneine (100 pM), quercetin (100 pM) and control (no antioxidant). Post-thaw were evaluated the parameters: total antioxidant capacity (TAC) of semen through the acid test of azino 2,2'-bis[3-ethylbenzothiazoline]-6-sulfonic acid (ABTS•+); total motility (MT), progressive motility (MP), hyperactivity (HA) and curvilinear (VCL), linear (VSL) and average path (VAP) velocities by the computerized system SCA ; structural membrane integrity and acrosome integrity by fluorescence microscopy using SYBR / IP and FITC / PNA probes, respectively; morphology by eosinnigrosin staining and functional membrane integrity by hypoosmotic swelling test (HOS). The adjustment of generalized linear models (GLM) and comparison of means by Tukey was performed. The TAC (pmol trolox/ml) of thawed semen was higher for ergothioneine (4.0 ± 0.3) and quercetin (3.9 ± 0.4), compared to control (2.6 ± 1.5). For MT a higher average for ergothioneine (70.3 ± 11.2%) compared to quercetin (63 ± 10.5%) and control (66.1 ± 11.2%) was found (P < 0.05). For MP, HA, VCL, VSL and VAP, the control showed higher values compared to the antioxidant treatments (P < 0.05). It is concluded that ergothioneine and quercetin increased the TAC and have influence on post-thawed motility and kinetics of stallion semen.

- Abstract
Los paradigmas de la pedagogía científica incorporan estrategias activas, pero, en el trabajo de aula sigue prevaleciendo el modelo pasivo. En contraposición a esto, el presente ensayo describe la experiencia que hemos acumulado desde 1999 con el desarrollo de una metodología denominada Proyectos Didácticos para la Enseñanza de la Ciencia que tiene por finalidad principal aportar diferentes propuestas metodológicas para abordar el trabajo de aula y fuera de ésta, para el caso especial de la enseñanza de las ciencias. Esta estrategia implica la participación activa del estudiante diseñando propuestas prácticas o juegos para el desarrollo de diversos contenidos típicos de la ciencia en el contexto de la formación inicial de docentes en la Universidad de Los Andes.

Este artículo presenta una alternativa para optimizar la calidad y frescura de las frutas y verduras que adquiere el consumidor final, visto desde la evaluación y optimización de los ciclos de retorno de las canastillas en los centros de distribución (CEDI) en Medellín y puntos de venta de una cadena de almacenes estudiada para la zona de Medellín y la Costa Atlántica. Para optimizar los ciclos de retorno se ha creído que sólo se debe estudiar la logística directa de los productos desde que salen del CEDI hasta que llegan a los almacenes, con lo cual sólo se está estudiando la mitad del problema. Los almacenes de cadena deben garantizar que las canastillas retornen al proveedor para que éste pueda empacar nuevamente sus productos, es decir, estudiar la logística inversa o de reversa, un nuevo concepto fundamental para todas aquellas empresas que tienen conciencia por el medio ambiente y se preocupan por la recuperación de los productos al final de su vida útil. This paper presents an alternative to optimize the quality and freshness of the fruits and vegetables bought by final consumer, according to the evaluation and optimization of the return cycles of the plastic containers in the distribution centers (CEDI) in Medellín and in points of sale of a store chain studied for the zone of Medellín and the Atlantic Coast. In order to optimize the return cycles, people has believed that it is only necessary to study the direct logistics of products since they leave the CEDI until they arrive at the stores, consequently, only half of the problem is being studied. Retailers must guarantee to their supplier that the plastic containers return so that he can pack his products again, that is to say, to study inverse or reverse logistic, a new fundamental concept for all those companies that they have consciousness about environment and they worry about the recovery of products at the end of their life utility.

El procesamiento de materiales por láser tiene gran aplicación a escala industrial, para imprimir patrones gráficos sobre superficies cerámicas. La rápida implementación de este proceso se debe a que, respecto al proceso convencional de horneado, tiene ventajas tales como: se concentra el calor sólo en la zona irradiada, lo que conlleva un menor consumo de energía sin afectar significativamente el resto de la pieza; se obtienen patrones gráficos de alta resolución espacial; permite introducir cambios durante el proceso sin requerir largas paradas, y la generación de residuos es mínima o casi nula. En este estudio se presentan los resultados obtenidos en el vidriado con láser Nd:YAG (λ=1064 nm) sobre las superficies esmaltadas de azulejos, las cuales fueron previamente recubiertas con pigmento negro de cobalto. Con el láser operando en modo continuo (CW), se marcaron una serie de líneas, cuyo ancho y profundidad evidenciaron una dependencia directa con la variación de la potencia y velocidad de desplazamiento del haz láser sobre la superficie del azulejo. Estas líneas vidriadas fueron finalmente caracterizadas por microscopía óptica, espectroscopia de dispersión de energía (EDS) y difracción de rayos X. Laser materials processing has great application to industrial scale, to print graphic patterns on ceramics surfaces. The fast implementation of this process is due to the fact that, with respect to the conventional baking process, has advantages, such as: heat is concentrated only in the radiated zone that involves a smaller energy consumption without affecting significantly the rest of the piece, graphic patterns with of highly spatial resolution are obtained, it permits to introduce changes during the process without requiring long stops, and low or almost none generation of residues. In this study the results obtained from Nd:YAG (λ=1064 nm) laser glazing on the enameled surfaces of glazed tiles, which were previously covered with black of cobalt pigment, are presented. With the laser operating in continuous wave (CW), a series of lines were marked, whose width and depth showed a direct dependence with the variation in the power and scan speed of the laser beam on the surface of the glazed tile. Finally, these glazed lines were characterized by optical microscopy, energy dispersion spectroscopy (EDS), and X-ray diffraction (XRD).

En este documento se presenta la situación actual del equipamiento urbano en el Municipio de Envigado en el sector de la educación, específicamente en lo que respecta a su ocupación. Se inicia con la normativa internacional de los convenios que ha suscrito Colombia, tiene en cuenta la Constitución Política Colombiana que garantiza los derechos de niños y jóvenes a la educación básica y la obligación del Estado de satisfacer ese derecho. Se plantean las políticas establecidas en los planes de desarrollo de las dos últimas administraciones del municipio de Envigado, desde el 2003 hasta el 2006 (políticas educativas, inversiones en infraestructura y en funcionamiento, cobertura, transferencias de la Nación, asignaciones por estudiante), según el Sistema General de Participaciones y gastos del municipio por estudiante. Luego se evalúa la ocupación de todas las instituciones públicas urbanas de educación con respecto a su capacidad instalada, se evalúa el crecimiento de la población estudiantil de cada institución y de la zona en la que están localizadas y se compara con el crecimiento poblacional. Por último, y con base en las evaluaciones hechas, se sacan algunas conclusiones y se hacen recomendaciones. This document portrays the current situation of urban equipment in the municipality of Envigado in the education sector, specifically in reference to its occupation. It begins with the international agreements that has subscribed Colombia. It has into account the Colombian Political Constitution which guarantees the right of children and youngsters to basic education and the obligation of the government to fulfill this right; it follows with the policy established by the last two local Government’s Development Plans, from 2003 to 2006 (education policy, infrastructure and operation investment, coverage, transfers from the national government, allocations by students according to the General Contributions System and municipal expenses by student); then, the occupation of all educational public institutions in relation with their installed capacity is assessed, evaluating the growth of student population in each institution and in the zone in which they are located, compared with the population growth in the aforementioned zones; lastly, and based on the previous evaluations, several conclusions are drawn and some recommendations, made.

Objetivo: Evaluar el impacto del sexo en la presencia del síndrome de lipodistrofia en una población de pacientes infectados por el VIH y su asociación con los factores de riesgo cardiovascular. Pacientes y método: Estudio transversal que incluyó a los pacientes de 20 años de edad o mayores, con infección por el VIH atendidos en la consulta externa del Servicio de Medicina Interna y Enfermedades Infecciosas del Hospital del Mar de Barcelona durante el año 2.003. Se evaluaron las características clínico-epidemiológicas de la infección por el VIH, la presencia de lipodistrofía y los factores de riesgo cardiovascular. Resultados: De los 710 pacientes incluidos en el estudio, las mujeres representaban el 28% de la serie. Los varones con lipodistrofia presentaron una mayor prevalencia de hipertensión arterial, hipercolesterolemia, hipoalfalipoproteinemia e hipertrigliceridemia comparado con los que no presentaban lipodistrofia (22,8 vs. 11,2%, p = 0,000; 20,6 vs. 9,3%, p = 0,001; 39,7 vs. 30%, p = 0,03 y 56,6 vs. 40,9%, p = 0,0001, respectivamente). En las mujeres, la presencia de lipodistrofia se acompañó de una mayor prevalencia de obesidad central e hipertrigliceridemia con respecto a la ausencia de lipodistrofia. En los varones predominó el patrón de lipoatrofia (24,9 vs. 12,6%, p = 0,0001), y en las mujeres el lipoacúmulo (12,3 vs. 22,6%, p = 0,0001). Además, las mujeres eran más jóvenes, tenían una mayor prevalencia de tabaquismo, antecedentes familiares de cardiopatía isquémica prematura y de obesidad central, y una menor prevalencia de hipertensión arterial y de hipoalfalipoproteinemia que los varones con lipodistrofia (42,1 ± 8 años vs. 44,8 ± 9,9 años, p = 0,03; 77,5 vs. 64%, p = 0,04; 22,5 vs. 9%, p = 0,003; 31 vs. 8,5%, p = 0,0001; 9,9 vs. 22,8%, p = 0,01; 25,4 vs. 39,7%, p = 0,03, respectivamente). Conclusiones: El presente estudio demuestra que el impacto del sexo en la lipodistrofia de los pacientes con infección por el VIH no sólo afecta al patrón de lipodistrofia, sino también al perfil de riesgo cardiovascular. Objective: To evaluate the influence of sex in human immunodeficiency virus (HIV)-infected patients with lipodystrophy and its association with cardiovascular risk factors. Patients and method: A cross-sectional study was conducted in HIV-infected patients aged 20 or over managed at the outpatient Infectious Disease Unit in 2003. Clinical and epidemiological characteristics of HIV infection, lipodystrophy and cardiovascular risk factors were evaluated. Results: Of the 760 patients included in the study, women comprised 28%. Men with lipodystrophy had a higher prevalence of hypertension, hypercholesterolemia, hypoalphalipoproteinemia and hypertriglyceridemia than those without lipodystrophy. Women with lipodystrophy had a higher prevalence of central obesity and hypertriglyceridemia compared with those without lipodystrophy (22.8 vs. 11.2%, p = 0.000; 20.6 vs. 9.3%, p = 0.001; 39.7 vs. 30%, p = 0.03 y 56.6 vs. 40.9%, p = 0.0001, respectively). The lipoatrophy pattern was predominant in men (24.9 vs. 12.6%, p = 0.0001) and lipoaccumulation forms in women (12.3 vs. 22.6%, p = 0.0001). Furthermore, women were younger, had a higher prevalence of smoking, family history of premature coronary heart disease and central obesity, and a lower prevalence of hypertension and hypoalphalipoproteinemia than men with lipodystrophy (42.1 ± 8 years vs. 44.8 ± 9.9 years, p = 0.03; 77.5 vs. 64%, p = 0.04; 22.5 vs. 9%, p = 0.003; 31 vs. 8.5%, p = 0.0001; 9.9 vs. 22.8%, p = 0.01; 25.4 vs. 39.7%, p = 0.03). Conclusions: This study demonstrated that the influence of sex in lipodystrophy in HIV-infected patients affects not only the lipodystrophy pattern, but also the cardiovascular risk profile.

Se estudia la relación entre la lluvia y los deslizamientos reportados en el departamento de Antioquia para el período comprendido entre 1929 y 1999. Se realiza la georreferenciación de 405 deslizamientos posiblemente producidos por lluvia en la región de estudio y se efectúa el análisis de la ocurrencia de los deslizamientos durante las fases cálida y fría del fenómeno ENSO (El Niño Southern Oscillation) y el ciclo intraanual de la precipitación en Colombia. La Niña, en la escala interanual, y los dos períodos más lluviosos sobre la zona andina, en la escala intraanual, favorecen un incremento en el número de deslizamientos de tierra. Mediante análisis de la precipitación diaria acumulada en estaciones cercanas al sitio de cada deslizamiento, se propone un umbral de lluvia y se describen las posibles combinaciones de lluvia acumulada antecedente. Las condiciones de lluvia más comunes para el desencadenamiento de un movimiento de suelo tienen que ver con altas cantidades de lluvia acumulada en el mediano plazo (15 días o más). Este estudio constituye un primer acercamiento al modelamiento de la lluvia y su influencia sobre los deslizamientos de tierra, lo cual puede dar apoyo a las autoridades en prevención de desastres para declarar estados de alerta ante condiciones de superación de umbrales de lluvia. El análisis tiene que complementarse considerando las condiciones geotécnicas, morfológicas, hidráulicas y antropogénicas propias de cada lugar para tener una visión integral del fenómeno de los deslizamientos. Relationship between rain and reported landslides in the department of Antioquia for the period of 1929 to 1999 is studied. The georeference of 405 landslides possibly produced by rain is made, on the region of study. The analysis of the occurrence of landslides during the warm and cold phases of ENSO phenomenon (El Niño Southern Oscillation) and the behavior of the intra-annual cycle are described. La Niña, on interannual scale, and the two rainiest periods in Andean zone, on intra-annual scale, contribute to increase the number of landslides per year. Analysis of precipitation data associated with historical landslide events in Antioquia has resulted in the identification of precipitation thresholds for the initiation of landslides and the possible combinations of preceding accumulated rain are described. The data indicated that the cumulative precipitation (15 days or more) influences the amount of subsequent 3-day precipitation that is needed to initiate ground movement. This study constitutes a first intend toward rain modeling and its influence on landslides occurrence which can help authorities on disasters prevention to declare states of emergency by conditions of threshold exceed. The analysis must be complemented having into account geotechnical, morphological, hydraulics, and anthropogenic conditions own to each place to have an integral view of the landslide phenomenon.

Objective: To describe the demography, clinical presentation and complications associated with traumatic brain injury (TBI) in a cohort of children, ages 0-10, treated at Hospital General de Medellín and Clínica Somer between 2010-2017. Methods: Cross sectional retrospective study chart review of all children with a diagnosis of TBI between 2010 and 2017. There were systematic review and recording of demographic information, mechanism of injury, clinical presentation, and associated complications. Results: A total of 224 patients with TBI were identified. Among them, 64.7% were males and mean age was 4.5 years. Injuries most frequently occurred at the patient’s home, during the afternoon hours and between Monday and Friday. The most common mechanisms of injury were falls (75%), followed by motor vehicle accidents (13.3%). Severity of injury, measured by Glasgow coma scale, found that 78% of cases were mild. Seven (3%) of cases had neuromuscular complications during hospitalization and one TBI related death was identified. Conclusion: Findings of this study are consistent with the known characteristics of pediatric TBI worldwide. This information may be used to improve prevention, diagnosis, and long term follow-up.

Introduction: According to official sources, disability affects up to 12 % of a country’s population. In Colombia, however, this percentage will continue to increase in the coming years regardless of the existing under-recording. Additionally, Pension fund administrators (PFAs) are in charge of efficiently managing the pension risk fund in Colombia. Objective: To characterize disability pension applications due to common diseases along with the main factors regarding their approval between 2006 and 2011 in a Colombian PFA. Materials and Methods: The PFA’s Unique Manual for Disability Assessment was used as a secondary source of information. This was a descriptive study with an inferential exploration of the factors concerning two outcomes: disability and application approval. Results: Being declared as a disabled person was associated with being male, having the highest schooling level, and the impairment motivating the application: diseases that have low prevalence but are very disabling. Having social support decreased the probability of being declared disabled. Discussion and conclusion: The factors related to disability, which are consistent with those reported by other studies, should be considered when proposing public policies, which may have an impact on the prevention of this outcome and the well-being of the working population.

Se describe un caso de fibrosarcoma esplénico primario, con metástasis múltiples, en un perro mestizo macho, de ocho años de edad. Clínicamente el animal presentaba un cuadro de insuficiencia hepática. Mediante la necropsia y el estudio histológico se llegó al diagnóstico definitivo. A case of primary splenic fibrosarcoma, with multiple metastasis, in a male eight-year old mixed-breed dog is described. The animal showed clinical signs corresponding with an hepatic failure. Necropsy and a subsequent histopathologic study lead to the final diagnosis.

Se realizó un estudio cualitativo con el propósito de descubrir las vivencias psicosociales de niños y niñas de 9 a 12 años que recibían tratamiento con quimioterapia por cáncer. La recolección de la información se hizo mediante encuentros lúdicos con el apoyo de algunas preguntas básicas que facilitaron las revelaciones narrativas. Tres aspectos se destacan en los hallazgos del estudio: vivencias desfavorables, vivencias relacionadas con la autoestima y vivencias favorables; las dos primeras, de diversa manera, afectan en estos niños su vida personal, familiar, escolar y, en general, todo su entorno. Es de resaltar que, como consecuencia de la caída del cabello, con frecuencia han sido objeto de burla y de rechazo por sus compañeros de estudio, lo que produce el más notorio efecto sobre su autoimagen; por otra parte, el ausentismo escolar conlleva dificultades académicas y sentimientos de tristeza en estos niños, situación que empeora cuando, por motivos de tratamiento, su lugar de residencia está alejado de la ciudad, viéndose sometidos a largos periodos de separación, no solo de su familia sino de sus pares. Se concluye que la mayor parte de las revelaciones son impactantes por su gran contenido de sufrimiento y dolor para estos niños, lo que demuestra la urgencia de retomar los hallazgos del estudio para orientar el cuidado de enfermería de manera más integral y apoyar para que se haga lo propio en el hogar y en las instituciones educativas.A qualitative study was carried out in order to discover psychosocial experiences of boys and girls receiving chemotherapy treatment for cancer. Information was collected through ludic encounters with the support of some basic questions to facilitate narrative disclosures. Three aspects stand out among findings in the study: unfavorable experiences, experiences related to self esteem and favorable experiences; the first two, in different ways, affect boys and girls’ personal, family and school life, and in general, all of their environment. It is worth highlighting that, as a consequence of hair loss, they are frequently the object of mockery and rejection by school mates, which produces the most notorious effect over their self image; on the other hand, school absenteeism causes academic difficulties and feelings of sadness in these children, situations get worse when, due to treatment, they have to live far away from the city, being subject to long periods of separation, not only from their families, but also from their peers. It is concluded that most disclosures are shocking due to the contents of suffering and pain experienced by these children, which proves the urge to retake findings in order to more integrally guide nursing care and other aides so that the correct practices are carried out at home and in educational institutions.O presente trabalho corresponde à elaboração de um estudo qualitativo que visa descobrir as vivências psicossociais de crianças entre 9 e 12 anos sendo tratadas com quimioterapia por câncer. A informação foi compilada através de encontros lúdicos suportados por algumas perguntas básicas que facilitaram as revelações narrativas. Entre os achados do estudo destacam-se três aspectos: vivencias desfavoráveis, vivências relacionadas com a auto-estima e vivências favoráveis; as duas primeiras afetam de forma diferente na vida pessoal, familiar, escolar e geral das crianças. É importante salientar que normalmente a queda de cabelo nas crianças provoca deboche e repulsão por parte de seus companheiros, o que produz o efeito mais notório sobre a imagem própria; por outro lado, o absentismo escolar traz dificuldades acadêmicas e sentimentos de tristeza nessas crianças, situação que se agrava quando, por causa do tratamento, elas devem viajar longe de seu domicilio e ficar afastados da sua família e seus amigos por muito tempo. Conclui-se que a maior parte das revelações são chocantes devido ao alto conteúdo de sofrimento e dor para essas crianças, o que demonstra a urgência de retomar os achados do estudo para orientar o cuidado de enfermagem para uma forma mais integral e fazer tudo que for preciso para fazer o mesmo no lar e nas instituições educativas.

Las enfermedades mitocondriales son un grupo de trastornos que están producidos por un fallo en el sistema de fosforilación oxidativa (sistema Oxphos), la ruta final del metabolismo energético mitocondrial, con la consiguiente deficiencia en la biosíntesis del trifosfato de adenosina (ATP, por sus siglas en inglés). Parte de los polipéptidos que componen este sistema están codificados en el ácido desoxirribonucleico (DNA) mitocondrial y, en los últimos años, se han descrito mutaciones que se han asociado con síndromes clínicos bien definidos. Las características genéticas del DNA mitocondrial, herencia materna, poliplasmia y segregación mitótica, confieren a estas enfermedades propiedades muy particulares. Las manifestaciones clínicas de estas enfermedades son muy heterogéneas y afectan a distintos órganos y tejidos por lo que su correcto diagnóstico implica la obtención de datos clínicos, morfológicos, bioquímicos y genéticos. 30. Construction of multilevel statistical models in health research: Foundations and generalities

- Abstract
This topic review aims to present a global vision of multilevel analysis models’ applicability to health research, explaining its theoretical, methodological, and statistical foundations. We describe the basic steps to build these models and examples of their application according to the data hierarchical structure. It ir worth noticing that before using these models, researchers must have a rationale for needing them, and a statistical evaluation accounting for the variance percentage explained by the observations grouping effect. The requirements to conduct this type of analysis depends on special conditions such as the type of variables, the number of units per level, or the type of hierarchical structure. We conclude that multilevel analysis models are a useful tool to integrate information, considering the complexity of the relationships and interactions involved in most health conditions, including the loss of independence between observation units.

Introduction: Introduction: obesity in the pediatric population is a public health problem. The correlation of uric acid and carotid intima media thickness in adults has been demonstrated. Objective: to identify the correlation of uric acid and carotid intima media thickness in adolescents with obesity. Material and methods: an observational, cross-sectional study was carried out. Patients aged ten to 16 years with a diagnosis of obesity were included. Uric acid, lipid profile and carotid intima media thickness were determined. In relation to the statistical analysis, carotid intima media thickness was correlated with uric acid levels through Spearman's correlation coefficient. Results: one hundred and sixty-nine adolescents were included with a median age of 13 years, without predominance of sex. A positive correlation of uric acid with carotid intima media thickness was identified (r = 0.242, p = 0.001). When stratified according to sex, there was no correlation in women (r = -0.187, p = 0.074), while in men it increased (r = 0.36, p = 0.001) and by pubertal stage, pubertal male adolescents had a positive correlation (p = 0.384, p = 0.002). Conclusion: a weak positive correlation was identified between carotid intimal thickness and uric acid in obese adolescents.

32. Health conditions of Hitnü indigenous people potentially exposed to crude oil in Arauca, Colombia

Introduction: The Hitnü indigenous people live in precarious sanitary conditions, with food insecurity and being victims of sociopolitical violence in Arauca, Colombia. In addition, it is possible that they may be affected by exposure to hydrocarbons found in oil. Objective: To identify the health outcomes of morbidity and mortality profiles of the Hitnü people that could be associated with the exposure to crude oil. Materials and methods: A cross-sectional study was carried out with Hitnü indigenous people, during February and March, 2021, time of drought. A household questionnaire was applied, and one individual to collect data from the environment around the house, occupations and other activities, as well as data from sociodemographic, signs, symptoms, and findings of a medical examination. The potential association with hydrocarbons was explored considering three groups: inhabitants in Arauca city, Aspejaná reserve (not exposed), and San José del Lipa and La Vorágine reserves (exposed by the Ele river and tributaries). With free listings, causes of death were explored. The study incorporated a rigorous intercultural management in all its components. Results: A total of 576 indigenous people from 16 settlements participated. The water consumed could serve as means of exposure to hydrocarbons. Health problems were very varied, including infectious and chronic diseases, malnutrition, and trauma. The masses on the neck were associated with residing in the ancestral reserves (PR = 3.86; CI95% 1.77-8.39), territories with potential exposure to crude-oil. The most relevant causes of death were homicide, tumors, and tuberculosis. Conclusion: For its possible association with exposure to hydrocarbons, it is a priority to start the intercultural study of lymphadenopathies in indigenous communities potentially exposed to crude oil.

Introduction: More than 90% of children infected with COVID-19 worldwide developed mild to moderate disease. In Colombia, during 2020, COVID-19 infections in children stayed below 9.2% of the total cases, with no trends for age group or sex., Objective: To estimate the incidence of acute respiratory symptoms and COVID-19 in children from public schools in Bogotá, Colombia during the second semester of 2020., Material and Methods: A telephone survey was conducted in over 5,000 scholar children. Antecedents and use of health services were informed. Descriptive statistics were used., Results: A total of 151.470 persons per day accounting for an IR of 157,8 per 100,000 people; almost three times the rate reported by the official surveillance system in the city., Conclusion: A lack of diagnosis and consultation in children was found compared to the general population. Further research is needed to elucidate the true burden of the disease in children.

- Abstract
Introduction: Eslicarbazepine acetate is a novel sodium channel blocker for use in the treatment of focal onset seizures. Prospective studies on its effectiveness in monotherapy in patients with newly diagnosed partial epilepsy in routine clinical practice are scarce., Aim: To evaluate the effectiveness of eslicarbazepine as initial monotherapy in patients with newly diagnosed partial epilepsy in routine clinical practice., Patients and Methods: A prospective, multicentre, post-authorisation study. Patients with newly diagnosed partial epilepsy aged 18 years or older without previous treatment were included. The efficacy variables were: percentage of seizure-free patients, responders and reduction in monthly frequency of seizures. The safety variables analyse the 12-month retention rate and the occurrence of adverse effects., Results: Fifty-three patients were included. The retention rate was 77.4%. At the end of the observation period, 83% of patients were seizure-free and 92.5% had reduced their baseline frequency by 50% or more. In addition, 68% of the patients reported some adverse effect and 7.5% of them dropped out of the study for this reason. The effectiveness analysis of the subgroup of patients aged 65 years or more showed no differences with respect to the overall population., Conclusion: Eslicarbazepine monotherapy in patients with newly diagnosed partial epilepsy, both in the general population and in the population over 65 years old, is effective and safe in routine clinical practice.

- Abstract
Introduction: Rapid IgM-IgG combined antibody tests can play an important role in the COVID-19 surveillance by supporting the diagnosis of infection, assessing the immune response, and verifying the progress towards herd immunity. Objective: To evaluate the performance of rapid IgM-IgG combined antibody tests in COVID-19 occupational surveillance in a group of Colombian enterprises. Materials and methods: We used the occupational surveillance data from companies that had performed periodic serological tests on all personnel from the end of April to the beginning of July, 2020. Workers were organized in small groups (“social bubbles”) to prevent outbreaks and optimize surveillance. The sensitivity was estimated as if the sampling had a prospective design. We describe here the changes in serological testing through periodic rounds. Results: Data were obtained from 4,740 workers, of whom only 23 were symptomatic showing changes from IgM(-)/IgG(-) to IgM(+) and then to IgM(+)/IgG(+) and IgG(+). The sensitivity was 40.94% for IgM(+) and 47.95% for IgM(+)/IgG(+). This implies that a little less than half of the cases can be detected. Conclusion: Antibody rapid tests have a role in the diagnostic process of infection and they must be evaluated taking into account the moment of the epidemic, the type of test purchased, and the populations at risk since their results depend on the number of infections and cases. In the context of a health crisis, they can be optimized by organizing workers into “social bubbles”

- Abstract
Introduction: Public health surveillance together with good sanitary decisions is essential for the proper management of the SARS-CoV-2 pandemic. Objective: To compare the performance of Colombian departments based on the quality of the data and to build the national ranking. Materials and methods: We analyzed the accumulated cases published between March 6 and September 1, 2020, by the Instituto Nacional de Salud. To achieve comparability, the analyses considered the day the first case was diagnosed as the first analysis date for each department. The fulfillment of Benford’s law was assessed with p-values in the log-likelihood ratio or chi-square tests. The analysis was completed with the lethality observed in each department and then the performance ranking was established. Results: Bogotá and Valle del Cauca had optimal public health surveillance performance all along. The data suggest that Antioquia, Nariño, and Tolima had good containment and adequate public health surveillance after the economic opening beginning on June 1, 2020. Conclusion: We obtained the ranking of the departments regarding the quality of public health surveillance data. The best five departments can be case studies to identify the elements associated with good performance.

- Abstract
Amid the global pandemic crisis, international concern has centered on the control and prevention measures aimed at reducing the speed of the virus transmission while a more radical sanitary measure, such as vaccines, is achieved. Governmental and social efforts have had great impact on various sectors of society and their consequences have exceeded the sphere of health. This essay discusses the scope of specific measures in the sense of the appropriation of risk control measures and proposes the epidemiological method as an alternative that goes beyond the quantification of risks and the attribution of responsibilities. To conclude, the emphasis is placed on the need to promote information about socialization processes to better understand the consequences of individual acts favoring alternatives other than pandemic control based on the use of coercive measures.

Introduction: The COVID pandemic is a challenge for public health surveillance and an opportunity to assess its strengths and weaknesses to improve the response. Objective: To evaluate the performance of the Colombian public health surveillance system during the first 50 days of the COVID-19 pandemic in the country. Materials and methods: We analyzed the data published between March 6 and April 24, 2020, by the Instituto Nacional de Salud and the World Health Organization (WHO). We evaluated: i) the quality of the data according to the fulfillment of Benford’s law, and ii) the timeliness of the information measured as the difference in dates between the data generated by the Instituto Nacional de Salud and WHO’s situational reports. We assessed the fulfillment of Benford’s law using the p values of the log-likelihood ratio, the chi square or Moreno’s exact tests. Results: Until April 24 there were 4,881 cases of COVID-19 in Colombia. During most of the first 50 days of the pandemic, Benford’s law was fulfilled except the first days of the epidemic. The difference between Instituto Nacional de Salud and WHO reports largely depends on the different reporting times. Conclusion: In general, the Colombian public health surveillance system fulfilled Benford’s law suggesting that there was quality in the data. Future studies comparing the performance of the departments and districts will improve the diagnosis of the Colombian surveillance system.

Background: Automated image classification is a promising branch of machine learning (ML) useful for skin cancer diagnosis, but little has been determined about its limitations for general usability in current clinical practice., Objective: To determine limitations in the selection of skin cancer images for ML analysis, particularly in melanoma., Methods: Retrospective cohort study design, including 2,849 consecutive high-quality dermoscopy images of skin tumors from 2010 to 2014, for evaluation by a ML system. Each dermoscopy image was assorted according to its eligibility for ML analysis., Results: Of the 2,849 images chosen from our database, 968 (34%) met the inclusion criteria for analysis by the ML system. Only 64.7% of nevi and 36.6% of melanoma met the inclusion criteria. Of the 528 melanomas, 335 (63.4%) were excluded. An absence of normal surrounding skin (40.5% of all melanomas from our database) and absence of pigmentation (14.2%) were the most common reasons for exclusion from ML analysis., Discussion: Only 36.6% of our melanomas were admissible for analysis by state-of-the-art ML systems. We conclude that future ML systems should be trained on larger datasets which include relevant non-ideal images from lesions evaluated in real clinical practice. Introduction: It has been described that menstruation is perceived as a natural but cumbersome event, with a negative impact on women's daily life. The objective of this study was to identify the perception of menstruation among female adolescents and the factors that may affect it., Material and Methods: Observational, crosssectional, comparative study in postmenarcheal adolescents with and without chronic conditions in two children's hospitals and two primary and secondary schools. Age, education level, type of population, date of menarche, and presence of dysmenorrhea were recorded. A previously validated questionnaire was administered to assess the perception of menstruation., Results: A total of 346 female adolescents were included. The most common perception of menstruation was negative in 65.6 % (n = 227), positive in 16 % (n = 55), discreet in 13 % (n = 45), and indefinite in 5.4 % (n = 19). The presence of a chronic condition demonstrated to be a protective factor against a negative perception of menstruation (odds ratio: 0.4 [95 % confidence interval: 0.20-0.78], p = 0.007)., Conclusions: More than half of female adolescents had a negative attitude toward menstruation, but the presence of a chronic condition improved it., Competing Interests: The authors report no conflicts of interest in this work., (Sociedad Argentina de Pediatría.)

Objective: Evaluate the origin of Fournier gangrene (FG) as a prognostic factor of morbidity and mortality., Material and Methods: Patients who came to our clinic with a diagnosis of FG from 2010 to 2017 were included retrospectively. Patients were categorized depending on the origin of the infection. Three severity factors were determined in each group: days of hospital stay, the FG severity index, and mortality. Logistic regression test was performed to analyze the data., Results: Of the 130 patients evaluated, the origin was established in 121 based on the clinical history and radiological and surgical findings. Thirty-five patients had an intestinal origin with a mortality of 20.68%, 46 patients had a testicular origin with a mortality of 2.22%, 12 patients had a urinary origin with a mortality of 0%, and 28 patients with a cutaneous origin with a mortality of 16.6%. The testicular origin was the most frequent (38%) in addition to presenting a lower hospital stay, a lower FG severity index, and a lower mortality than those with an intestinal origin (P=.022)., Conclusions: The origin of the infection has a significant prognostic value in the mortality of the patient., (Copyright © 2019 AEU. - Abstract
Objectives: To describe the sociodemographic and clinical characteristics of a cohort of patients with epilepsy from a reference centre in Colombia., Methods: Cross-sectional study including patients diagnosed with epilepsy who attended our epilepsy centre (Neurocentro) between 2013 and 2016. Data were gathered from patients' medical histories., Results: We gathered data from a total of 354 patients diagnosed with epilepsy. Median age was 37 years; 52% were men. Seizures were focal in 57% of the patients and generalised in 38%; seizure type was not determined in 6% of the sample. The most frequent aetiology was cryptogenic (21%), followed by traumatic (14%). Median time of disease progression and age at onset were 23 and 11 years, respectively. Psychiatric comorbidities were found in 18% of the patients and 40% had some degree of cognitive impairment. Around 40% of our sample reported adverse reactions to antiepileptic drugs at some point during treatment. Antiepileptic drugs were administered in monotherapy in 36% of the patients. Around 37% had drug-resistant epilepsy and 14% underwent surgery., Conclusions: Psychiatric comorbidities, cognitive impairment, adverse drug reactions, and drug-resistant epilepsy are common among epileptic patients in Colombia. - Abstract
Introduction: People living with HIV/AIDS in developing countries tend to have a lower quality of life and less social support compared with those in developed countries. Objective: To explore the association between affective social support or social support generating confidence and each dimension of the quality of life related to health among people with HIV/AIDS from Bogotá. Materials and methods: We conducted a cross-sectional study of people living with HIV/AIDS using convenience sampling of a care program in a hospital network in Bogotá. The quality of life questionnaire SF36 and the generic social support questionnaire Duke-UNC-11 were used, along with linear regression models for the analyses. Results: There was a direct relationship between the emotional well-being dimension of quality of life and the social support systems of affectivity (ß =7.36;95% CI: 1.04; 13.68) and those generating confidence (ß =11.63;95% CI: 5.30; 17.96). There was a correlation between the dimensions of physical function, emotional performance, pain, and perceived affective social support. Likewise, we detected relations between the perception of social support generating confidence and the dimensions of vitality and social function. Contrarily, we found an inverse relationship between the averages of the dimension of emotional performance and the perceived affective social support, as well as between the dimension of physical performance and general health with social support generating confidence. Conclusions: Subjects with higher levels of social support had higher levels of quality of life related to health. This finding offers an opportunity for the design and implementation of healthcare plans that incorporate clinical, para-clinical and environmental variables of the patient.

Objective: The comparison on mitochondrial function between severe septic patients and healthy control subjects according to mitochondrial deoxyribonucleic acid (mtDNA) haplogroup has not been previously reported; and this was the objective of the current study., Methods: Prospective, multicenter, observational study. We obtained blood samples from 198 severe septic patients at days 1, 4 and 8 of severe sepsis diagnosis and from 96 sex- and age-matched healthy controls to determine mtDNA haplogroup and platelet respiratory complex IV (CIV) specific activity. The endpoint of the study was 30-day mortality., Results: We included 198 severe septic patients (38 with mtDNA haplogroup JT and 160 with mtDNA haplogroup non-JT) and 96 healthy control subjects (16 with mtDNA haplogroup JT and 80 with mtDNA haplogroup non-JT). We have no found statistically significant differences in platelet CIV specific activity between healthy controls and survivor severe septic patients with mtDNA haplogroup JT at days 1, 4 and 8 of severe sepsis diagnosis; and the remaining severe septic patients showed lower platelet CIV specific activity than healthy controls with the same mtDNA haplogroup., Conclusions: The new finding of our study was that survivor severe septic patients and healthy controls with mtDNA haplogroup JT showed no different platelet Civ specific activity., (Copyright © 2017 Elsevier España, S.L.U. and Sociedad Española de Enfermedades Infecciosas y Microbiología Clínica. Background: There is a wide interindividual variability in the response to a period of exercise training. The science have reported that a minimum of participants could be non-responders for improving different health-related outcomes after training., Aim: To compare the effects of a 6-weeks exercise program on body composition, cardiovascular and metabolic outcomes patients with type 2 diabetes and hypertension., Material and Methods: Data from 23 trained subjects were used in a secondary analysis of the response to exercise. Of these, 14 were considered adherent to training and nine as non-adherent. Body mass, height, waist circumference, four skinfolds and their sum, blood pressure and plasma triglyceride levels were assessed before and after the training period., Results: Among adherent participants, significant reductions were observed in the sum of four skinfolds (30 ± 7 to 27 ± 6 mm, p ≤ 0.05), systolic blood pressure (133 ± 18 to 127 ± 20 mmHg; p ≤ 0.05) and plasma triglycerides (125 ± 58 to 102 ± 34 mg/dL; p ≤ 0.05). No changes were observed in weight or diastolic blood pressure. Among non-adherent participants, no changes of measured parameters were observed. Among adherent participants, 57% were considered as non-responders for waist circumference, 7% for the sum of skinfold thickness, 50% for systolic blood pressure, 64% for diastolic blood pressure and 57% for plasma triglycerides., Conclusions: Participants with a good adherence to a 6-weeks exercise training program experienced overall improvement in body composition, blood pressure and plasma triglycerides. The prevalence of non-responders varied considerably among measured outcomes.

Introducción: La enfermedad renal crónica (ERC) se asocia con alteraciones menstruales, y el manejo del sangrado uterino suele ser complejo por las condiciones de este grupo de pacientes. El objetivo de este trabajo fue describir la respuesta clínica al tratamiento hormonal de las alteraciones menstruales de adolescentes con ERC., Métodos: Se presentan los datos de una serie de casos de pacientes adolescentes con ERC que cursaron con alteraciones menstruales y que recibieron tratamiento desde el año 2008 al 2012. Se identificaron las características del trastorno menstrual, del tratamiento hormonal recibido y de la respuesta al mismo. El análisis estadístico fue descriptivo., Resultados: Se estudiaron 11 pacientes de sexo femenino con edad promedio de 14.5 años, que se encontraban en prediálisis (n = 1), diálisis peritoneal (n = 7) y hemodiálisis (n = 3). Las pacientes presentaron hiperpolimenorrea asociada a la opsomenorrea (n = 3), en su mayoría clasificadas como hemorragia uterina anormal secundaria. El tratamiento, en general, fue con progestágenos de manera inicial (clormadinona con o sin medroxiprogesterona) o bien con anticonceptivos combinados. En la mayoría de las pacientes se obtuvo una respuesta favorable; sin embargo, hubo casos en los que fue necesario modificar la dosis y el tiempo de tratamiento., Conclusiones: La mayor parte de las adolescentes con ERC que han sido tratadas por hemorragia uterina anormal en nuestro estudio tuvieron una respuesta favorable al tratamiento hormonal., Background: Chronic kidney disease (CKD) is associated with menstrual abnormalities and management of uterine bleeding is often complex because of the conditions in this group of patients.
