Introduction: Nanobiotechnology and synthetic biology are sciences that impact today with the launching of innovative and beneficial applications for the human being. These sciences have been amalgamated to manufacture new components for the construction of totally artificial cells and the creation of synthetic biomolecules. Objective: To know the applications of nanobiotechnology related to the use of the system CRISPR/Cas in the storage of bacterial DNA and therapeutic alternatives. Materials and methods: A bibliographical review on the main applications of nanobiotechnology was carried out in ScienceDirect, SciELO, PubMed databases and in magazines such as: Nature Biotechnology, Biochemistry, Science and Journal Microbiology. Results: The literature review describes and analyzes the new nanobiotechnology applications used to write information in the genetic code of bacterial cells, in which the system is used based on short grouped and regularly interspaced palindromic repetitions (CRISPR/Cas) and the production of synthetic DNA, as well as therapeutic alternatives related to gene therapy. Conclusion: Among the nanobiotechnology applications, two methods to record information in the DNA of bacterial cells Escherichia coli and Sulfolobus Tokodai have been shown, which are linked to the use of the system CRISPR/Cas and the production of synthetic DNA, as well as the use of CRISPR/Cas in gene and cellular therapy.

Introducción. La inmunodeficiencia común variable es un síndrome heterogéneo caracterizado por infecciones recurrentes, hipogammaglobulinemia y producción deficiente de anticuerpos específicos. Las anormalidades en subpoblaciones de linfocitos en sangre periférica, particularmente de linfocitos B, permiten la clasificación de los pacientes en grupos homogéneos. Objetivo. Caracterizar clínica e inmunológicamente los linfocitos B y tipificar sus subpoblaciones en doce pacientes colombianos con inmunodeficiencia común variable, para clasificarlos en grupos homogéneos. Materiales y métodos. Se revisaron las historias clínicas de los pacientes y se evaluaron las inmunoglobulinas séricas, la proliferación de linfocitos y la hipersensibilidad retardada, así como las subpoblaciones de linfocitos y de linfocitos B mediante citometría de flujo. Resultados. Todos los pacientes presentaron infecciones respiratorias o gastrointestinales recurrentes y, algunos, infecciones en otros sistemas. Además, todos presentaban disminución de la IgG, en tanto que la IgA y la IgM fueron bajas en nueve y diez pacientes, respectivamente. En todos hubo disminución de la proliferación de linfocitos inducida por mitógenos, pero fue normal frente a antígenos específicos. La tipificación de subpoblaciones reveló valores elevados de linfocitos T en tres pacientes; siete presentaron disminución en la relación CD4+/CD8+ y, cuatro, linfocitos NK bajos. El conteo de linfocitos B fue normal en once pacientes, ocho de los cuales presentaron linfocitos B de memoria bajos, en tanto que cuatro presentaron aumento de linfocitos B de transición o de linfocitos B CD21low. Conclusión. La tipificación de subpoblaciones de linfocitos solo permitió asignar a 11 de los pacientes a grupos homogéneos según los esquemas de clasificación internacionales, lo que indica la necesidad de agregar más criterios hasta lograr una clasificación ideal. Este estudio permitirá establecer mejores seguimientos médicos para pacientes con inmunodeficiencia común variable en grupos con alto riesgo de desarrollar complicaciones clínicas.

An urgent search is currently underway for alternatives to antibiotics to prevent infections, due to the accelerated evolution and increase in antibiotic resistance. This problem is more serious for patients with recurrent infections, since they have to use many cycles of antibiotics per year, so the risk for antibiotic resistance is higher and can be life-threatening. In recent years, the use of prophylactic vaccines via the mucosal route for these patients with recurrent infections has been demonstrated as a potentially beneficial and safe alternative to prevent infections. The new knowledge about mucosal immunity and trained immunity, a form of innate immunity memory that can enhance the response to different infectious threads, has made it easier to extend its use. The application of the new concepts of trained immunity may explain the simultaneous pro-tolerogenic and boosting effect or effects of these drugs on diverse immune cells for different infections. Common variable immunodeficiency (CVID) is the most common symptomatic immunodeficiency in adulthood. CVID diagnosis is by exclusion and should be considered in patients of any age who have hypogammaglobulinemia of unknown origin. Numerous patients with CVID show alterations in the development of B lymphocytes, both in plasma cells and memory cells. The absence of memory B cells suggests an insufficient germinal reaction, which can be associated with a blockade of the transition of T1 cells into T2 in patients with IDCV, owing to B-cell activating factor (BAFF) receptor deficiency. In patients with IDCV, memory B cell alterations with isotype change favor the development of concomitant comorbidities such as lymphadenopathy, splenomegaly, autoimmunity and granulomatous disease, and multiple classifications that use memory B cells in common have therefore been made trying to generate a classification of patients with IDCV, as well as to establish prognostic factors.

Introduction: Fingolimod, a sphingosine-1-phosphate receptor modulator, was the first oral therapy approved for relapsing-remitting multiple sclerosis, and shows a novel mechanism of action. Upon binding to S1P1 receptors in lymphocytes, the selective retention of naive and central memory T cells in secondary lymphoid tissues is promoted, preventing their egress to the central nervous system (CNS). In addition, fingolimod readily crosses the blood brain barrier, and several reports suggest a direct neuroprotective effect in the CNS., Aim: To review the available data on the central effects of fingolimod., Development: Imbalances between damage and repair processes are a reflection of chronic demyelination, axonal degeneration and gliosis, and seem to contribute to multiple sclerosis associated disability. Given fingolimod readily crosses the blood brain barrier, it can exert its action directly on S1P receptors present in CNS cells. Fingolimod occupies S1P receptors in oligodendrocytes, oligodendrocyte precursor cells, astrocytes, microglial cells and neurons, promoting remyelination, neuroprotection, and endogenous regeneration processes. Efficacy results from clinical trials are consistent with a mechanism of action that includes direct effects in CNS cells., Conclusions: Current evidence suggests that the efficacy of fingolimod in the treatment of Multiple Sclerosis is due to its dual action as an immunomodulatory molecule and as a direct modulator of S1PRs in the CNS. In fact, recent reports propose that fingolimod has neuroprotective effects in several models, and open new avenues of potential therapeutic applications, such as Alzheimer's disease, cerebral malaria, neuroblastoma and neuroprotection in cranial irradiation.

Background: Psoriasis is an immune-mediated disease typically associated with cutaneous neutrophilic infiltration and Munro microabscesses. Interleukin (IL)-8 is one of the main neutrophil-attracting chemokines. Although keratinocytes have traditionally been considered to be the principal source of IL-8 in psoriasis, we present data that suggest that cutaneous lymphocyte associated antigen (CLA) + T lymphocytes synthesize this cytokine., Material and Methods: Six patients with psoriasis and 6 healthy controls were studied. Immunomagnetic separation was used to isolate CLA+ and CLA- T lymphocytes and IL-8 and interferon (IFN)-gamma production was quantified for each cell subpopulation using enzyme-linked immunosorbent assay. Finally, gene expression of IL-8 was analyzed by reverse transcriptase-polymerase chain reaction., Results: CLA+ and CLA- T lymphocytes from patients with psoriasis and from controls showed a significantly increased production of IFN-gamma when activated, whereas only activated CLA+ T lymphocytes (from patients and controls) synthesized IL-8. The higher level of expression of IL-8 and IFN-gamma by CLA+T lymphocytes in comparison to CLA- cells was confirmed., Discussion: Previous studies have confirmed IL-8 production by T lymphocytes in inflammatory skin diseases with neutrophil-rich infiltrates, such as acute generalized exanthematous pustulosis, Behçet disease, and pustular psoriasis. We have confirmed the role of the subset of T lymphocytes with skin tropism (CLA+) in IL-8 production in nonpustular psoriasis.

Kawasaki disease is an acute febrile multisystemic vasculitis affecting children that can affect the coronary arteries. Routine BCG vaccination in Mexico leads to a 99% coverage in infants younger than 1 year. We present a case of Kawasaki disease with skin lesions at the site of BCG. Clinicians should be aware of this clinical manifestation that could help diagnose atypical or incomplete cases of the disease.

Objective: To examine the role of naive and memory T cells (as determined by CD45 isoforms) in recent tuberculosis infection and in pulmonary and pleural forms to the disease. To relate such subpopulations to serum levels of soluble IL-2 receptor (sIL-2R) and interleukin-6 (IL-6)., Methods: T cell subpopulations (CD3, CD4, CD8), naive (CD45RA), memory (CD45RO) and activated CD24+ T cells (IL-2 receptor) were measured in the peripheral blood and pleura of patients with recent tuberculosis infection (n = 7), pulmonary tuberculosis (n = 22) and tuberculosis pleurisy (n = 12). Serum levels of sIL-2 and IL-6 were determined in 13 patients with pleural or pulmonary tuberculosis and their relation to CD45RA and CD45RO subsets was analyzed., Results: T lymphocytosis involving all the subpopulations studied, including naive and memory T cells, was detected, although the RA:RO ratio did not change in relation to control levels. Marked increases in memory T cells and CD24+ T cells were found for patients with tuberculous pleurisy. sIL-2 and IL-6 levels were unrelated to naive and memory T cell subpopulations., Conclusions: T lymphocytosis involving all T cell subsets can be observed in recent tuberculosis infection; both naive and memory T cells are implicated, although neither one predominates over the other. In tuberculous pleurisy there is compartmentalization of memory and activated T cells, which presumably play important roles in the local immune response to Mycobacterium tuberculosis.

The synthesis of IgE class antibodies takes place in plasma cells after activation of B lymphocytes by two different signals. The presence of interleukin-4 (IL-4) in the microenvironment and intimate contact between B and T cells through the CD-40 molecule and its specific ligand (CD-40L) are necessary. Lymphocyte activation also can be induced by mast cells. IgE does not cross the placenta, but fetuses are capable of synthesizing it. We attempted to identify children at high risk of atopy by determining IgE in umbilical cord blood. The system was not entirely satisfactory because many other factors are involved, but it demonstrated that intrauterine sensitization can occur. IgE-mediated allergy is conditioned by the predominance of Th2 lymphocytes (secretors of IL-4, IL-5, and IL-10) over Th1 lymphocytes (secretors of IL-12 and IFN gamma). This imbalance is physiological in the fetus and Th1 stimulation causes miscarriage. At birth, the functional predominance of Th2 continues, probably because of the immaturity of the dendritic antigen-presenting cells. An irregular maturation process takes place in the following months; for instance, while the intestinal mucosa tends towards tolerance and matures more rapidly, the respiratory mucosa tends toward the Th1 response and is slower. It is likely that sensitization during fetal and neonatal life is interesting because it generates type Th2 memory cells that can predispose toward atopic responses in the future.

120 patients with Giardia lamblia infection diagnosed by biliary drainage or jejunal mucosa smear are studied. 80 patients showed frequent reinfections by the parasites (recurrent giardiasis). A study is made of T and B lymphoid populations in peripheral blood, which were reduced in all patients with giardiasis in relation with the control group (p less than 0.01). Changes were more pronounced in relapsing than in non-relapsing patients. Retarded in vivo hypersensitivity tests with memory and recognizing antigens did not show significant differences between the experimental and control group. Serum levels of IgG, IgM, and IgA differed (p less than 0.01) between patients and controls at the expense of an IgA increase and IgG and IgM decrease, although these variations were not significant between relapsing and non-relapsing patients. It is concluded that the immunity status of the host is of interest in the clinical picture of patients with giardiasis and therefore must be a factor to be considered in the study of Giardia lamblia infected patients.
