Salud Mental Global (SMG), en tanto que conceptualización relativamente reciente de aspectos teóricos, clínicos y heurísticos de la enfermedad mental, su etiopatogenia, variantes y modalidades de manejo desde la perspectiva de salud pública a lo largo del mundo, es materia de esta breve revisión. Su elaboración se basa fundamentalmente en las contribuciones de Duncan Pedersen, renombrado psiquiatra, antropólogo e investigador argentino-canadiense, docente de la Universidad de McGill y autor de numerosos estudios en el Perú y en varios otros países, recientemente fallecido. Su afronte enfatizó enfoques objetivos en el uso de recursos propios o locales combinados, con sello genuinamente humanístico, con avances científicos, tecnológicos y financieros y programas de justicia social. Trauma y sus numerosas secuelas globales a nivel físico, emocional, individual y colectivo, constituyó un factor primum inter pares de este enfoque,conduciendo luego a hallazgos de psicopatologías adicionales, comprobaciones epidemiológicas y discrepancias a veces insalvables con las concepciones de la medicina de Occidente. Modalidades culturales de manejo, estudio de temas tales como tratamiento inter- y multidisciplinario, renovación de conceptos éticos, resiliencia y otros dentro y más allá de factores neurobiologicos inducirán soluciones sistemáticas y sostenibles a los problemas actuales y futuros de la SMG. Global Mental Health (GMH), a relatively recent conceptualization of theoretical, clinical and heuristic aspects of mental illness, its etio-pathogenesis, variants and management modalities from the Public Health perspective across the world, is the subject of this brief review, primarily based on the contributions of Duncan Pedersen a recently deceased renowned Argentine-Canadian psychiatrist, anthropologist and researcher, Faculty member at McGill University and author of numerous studies in Perú and several other countries. His work emphasized objective and humanistically conceived approaches to the use of local resources in combination with scientific, technological and financial advances in the context of social justice-oriented programs. Trauma and its numerous global sequelae at physical, emotional, individual and collective levels, constituted a primum inter pares factor of this approach leading to the finding of additional psychopathologies, epidemiological proofs and, at times, powerful discrepancies with the principles of Western medicine. Culturally-based management modalities, and the study of topics such as interand multi-disciplinary treatment, renewal of ethic concepts, resilience and other within and beyond neurobiological factors will bring up systematic and sustainable solutions to current and future GMH problems.

Background: Chronic venous ulcers (CVU) represent a frequent condition, with difficult therapeutic approaches, that impact on patients’ quality of life, and generate an economic burden to patients and health systems. Aim: To perform the cultural adaptation and initial evaluation of the Charing Cross Venous Ulcer Questionnaire (CCVUQ) for Uruguay, and to study the health-related quality of life (HRQL) of patients with CVU. Material and Methods: The translated and culturally adapted version of the CCVUQ was applied to a convenience sample of 50 patients. In addition, the PROMIS Global Health Survey was included in the assessment. Results: Both questionnaires showed good internal consistency (Cronbach alfa > 0.70). A statistically significant association was observed between the CCVUQ total scores, its subscales and both dimensions of the PROMIS: Global Physical (GPH) and Global Mental Health (GMH) (rho ≥ 0.40). The CCVUQ mean score was 54.9 ± 42 points while GPH and GMH mean scores were 37.9 ± 29 points, and 43.1 ± 35.1 points respectively. Simple linear regression showed that patients with higher income reported better emotional well-being, while in younger patients, ulcers had a higher impact on Emotional Status and Cosmetics. Conclusions: The translated and adapted version of the CCVUQ was easy to comprehend and apply, showing good psychometric properties. When used in association with the PROMIS Global Health Measure it provides complementary information. HRQL was severely affected in the study sample.

La fobia social es un problema de salud mental a escala mundial que no es diagnosticada y atendida en forma adecuada. Debido a que México presenta una alta prevalencia del trastorno, se puso a prueba un programa de telepsicología para miedo a hablar en público, que anteriormente probó su eficacia en población española. La muestra se conformó por 43 participantes: 26 mujeres y 17 hombres, entre 19 y 60 años de edad (M = 32.51). Todos los participantes cumplían los criterios del dsm-iv para fobia social: miedo a hablar en público. Se empleó un ensayo clínico “aleatorizado” con tres condiciones de tratamiento: asistido por terapeuta (n = 16), auto aplicado (n = 14) y lista de espera (n = 13), con medidas repetidas intrasujeto (pretratamiento, postratamiento y seguimiento a tres meses). Los resultados muestran cambios significativos intragrupo en los indicadores fóbicos en ambos grupos de tratamiento, superiores al grupo control. Los datos obtenidos apoyan la generalidad del programa de telepsicología en población mexicana.
