The results presented in this paper were obtained from a test carried out during the last series of a research project regarding the creep behaviour of Argentinean Eucalyptus grandis loaded in dry condition. A test with a sample containing 15 structural-sized beams was carried out during 470 days. 7 pieces were free of pith and another 8 contained it. The results allow to compare the creep behaviour of these beams with previous data reported for both structural-sized and small-clear specimens of this timber species and also with those reported for other species. The effectiveness of the criteria adopted by overseas standards as well as those of Latin-American countries for calculating the creep deflections of this timber species under a long-term loading is analysed. The creep deflections obtained in the present research are higher than those calculated by means of the procedures of the Brazilian standard NBR 7190, the European standard Eurocode 5, the American standard National Design Specification for Wood Construction and the Peruvian standard. On the contrary, creep results obtained in this test are lower than those calculated according to the New Zealand standard NZS 3603, the Chilean standard NCh 1198 and the former German standard DIN 1052

La deformación en vigas de E. grandis cultivado en la Mesopotamia Argentina, fue estudiada en función del tiempo, con el objetivo de determinar su comportamiento bajo esfuerzos de flexión de larga duración. Cuarenta cuerpos de prueba de dimensiones pequeñas, libres de defectos, fueron sometidos a la acción de una carga concentrada en el centro, durante un año. El valor de las tensiones internas alcanzó los niveles normales para estructuras en servicio. Las condiciones de servicio en el lugar de realización de los ensayos fueron las habituales para ambientes interiores en la región. Se registraron periódicamente los valores de las deformaciones en el centro para cada probeta. El cociente entre la deformación obtenida luego de una semana y la instantánea alcanzó el valor de 1,48. La misma relación fue de 2,25 a los seis meses, manteniéndose constante hasta el año. Los resultados se comparan con los publicados para otras especies, y con los que surgen de la aplicación de las recomendaciones del Eurocódigo 5. Los valores obtenidos son congruentes con los correspondientes a otras investigaciones y mayores que los calculados utilizando las prescripciones de la normaCreep of Argentinean Eucaplytus grandis was studied for determining its behaviour under long-term bending stresses. For this purpose an empirical research project with forty small clear specimens randomly selected were subjected to a middle span constant load during one year. Level of stresses and climate conditions were the usual for typical roof structures in the region. Deformations were periodically registered at the center of each specimen. The ratio of the total deformation to the instantaneous one reached 1,48 after a week. The same ratio was 2,25 after six month and also after one year. Results are discussed and compared with the ones found by other researchers and with the recommendations of Eurocode 5. Values of creep obtained in this work are congruent with the results of other investigations and greater than the ones obtained applying the prescriptions of the above cited code

4. Preferencia de hábitat del murciélago hortelano meridional Eptesicus isabellinus (Temminck, 1840) en ambientes mediterráneos semiáridos

Diversos estudios moleculares han revelado recientemente la existencia de una segunda especie del género Eptesicus en la península ibérica: el murciélago hortelano meridional E. isabellinus. Dicha especie se encuentra en el sur de la península y parece tener una distribución alopátrica con su especie gemela E. serotinus. Debido a la reciente separación entre ambas, es necesario realizar estudios que amplíen el conocimiento sobre la ecología de E. isabellinus. En este trabajo se han empleado datos sobre la presencia de la especie en la Región de Murcia (SE de España) para elaborar modelos de nicho ecológico (MNE) y el correspondiente mapa de idoneidad de hábitat. Los resultados revelan que la especie tiene un índice de marginalidad bajo, lo que sugiere una buena adaptación a las condiciones ambientales semiáridas de la zona de estudio. El principal hábitat utilizado por la especie son los cursos de agua, las zonas de matorral y, en general, los ecosistemas con una elevada productividad. Por el contrario, la especie parece evitar las zonas agrícolas de secano y no muestra especial preferencia por las zonas más humanizadas ni los cultivos de regadío. La información proporcionada por este estudio contribuye al conocimiento de la ecología de E. isabellinus y los resultados del modelo de distribución permiten discutir sobre los aspectos importantes para su conservación.

Objetivo: Determinar la prevalencia de anomalías oculares no corregidas y su asociación con cervicalgias en grupo de trabajadores del sector industrial. Metodología: Estudio descriptivo, correlacional, de corte transversal en 83 trabajadores del área de producción de una fábrica de lámparas en Cali, año 2011. Se realizaron encuestas de datos demográficos, laborales y defectos visuales previos. Los defectos visuales no detectados se evaluaron con, optotipo Snellen E y examen directo interno y externo para detección de anomalías estructurales. La presencia de dolor osteomuscular en cuello y espalda alta se evaluó con el cuestionario nórdico. Resultados: El defecto visual predominante fue presbicia (43,4%). El síntoma osteomuscularde mayor prevalencia fue dolor de cuello (44,6%). Se encontró una asociación significativa entre tener algún defecto visual y dolor en cuello (P=0,041). Conclusiones: Se encontró una relación entre los defectos visuales y molestias osteomusculares en cuello. Se recomienda realizar exámenes visuales periódicos para evitar la aparición de enfermedad osteomusculares.

Resumen: Introducción: La determinación de pruebas metabólicas en los recién nacidos (hipotiroidismo y fenilcetonuria) mediante punción de talón es un método doloroso. La venopunción podría suponer un método alternativo de extracción. Objetivos: Evaluar la eficacia y la repuesta al dolor obtenida mediante punción venosa en el dorso de la mano en comparación con la punción de talón en la realización de pruebas metabólicas, y la detección de posibles diferencias en cuanto a los resultados de TSH. Material y métodos: Estudio observacional descriptivo aleatorizado con 607 recién nacidos consecutivos asignados a 2 grupos: grupo venopunción (n = 280), y grupo punción de talón (n = 327). Se estudió la respuesta al dolor utilizando una escala de dolor (escala NIPS), y latencia y duración del llanto, eficacia de la prueba valorando el número de punciones necesarias y duración de la prueba, y comparación de falsos positivos para el hipotiroidismo congénito. Objetivo: Determinar la prevalencia de bocio y déficit de yodo en comarcas del interior de la Comunidad Valenciana, dada la ausencia de datos en esta comunidad autónoma. Material y métodos: Se trata de un estudio epidemiológico, descriptivo, transversal, estratificado por sexo y edad en cuatro comarcas del interior de la provincia de Valencia. Se estudiaron escolares de 6 a 14 años en 20 centros. La exploración tiroidea se realizó mediante palpación e inspección (bocio ≥ 0B). Se determinó la yoduria en una muestra de orina casual. Se recogieron datos de filiación, antropométricos, y una encuesta sobre el estado nutricional de yodo. En niños con bocio se determinó la hormona tiroestimulante (TSH), la tiroxina libre (T4L) y los anticuerpos antitiroideos. Resultados: Se estudiaron 928 niños (478 varones y 450 mujeres). La prevalencia de bocio fue de 33,7 % (IC 95 %: 30,7-36,9). No se hallaron diferencias significativas por edad ni por sexo, aunque sí una correlación inversamente proporcional entre prevalencia de bocio y nivel instrucción paternos. La mediana de yoduria fue 155 mg/l, sin observar una correlación significativa con la prevalencia de bocio. Entre los niños con bocio 13 presentaron los anticuerpos antitiroideos positivos, 18 la TSH elevada (hipotiroidismo subclínico) y uno la TSH suprimida (hipertiroidismo subclínico). Conclusiones: Existe una endemia bociosa en las comarcas estudiadas de la Comunidad Valenciana. Los valores de yodurias normales podrían interpretarse como una fase de transición hacia una mejoría del déficit de yodo en esta zona. La patología autoinmune explicaría sólo un 4% de bocios. : Objective: To estimate the prevalence of goiter and iodine deficiency in a health district in the Autonomous Community of Valencia, given the absence of data in this region of Spain. Material and methods: We conducted a descriptive, epidemiologic, cross-sectional study, stratified by age and sex, in four interior regions of the province of Valencia. We selected students aged from 6 to 14 years old in 20 centers. Thyroid examination was performed by means of palpation and inspection (goiter ≥ 0B). Urinary iodine excretion was analyzed in a routine urine sample. Sociodemographic and anthropometric data, as well as nutritional iodine status, were recorded in a standardized survey. In children with goiter, thyroid-stimulating hormone (TSH), free T4, and antithyroid antibodies were determined. Results: We studied 928 children (478 boys and 450 girls). The prevalence of goiter was 33.7 % (95 % CI: 30.7-36.9 %). There were no significant differences in the prevalence of goiter by age or sex, but an inverse correlation was detected between the prevalence of goiter and parental socioeconomic position. Mean urinary iodine excretion was 155 mg/l, with no significant correlation with the prevalence of goiter. In children with goiter, 13 had positive antithyroid antibodies, 18 had high TSH (subclinical hypothyroidism), and one had suppressed TSH (subclinical hyperthyroidism). Conclusions: There is endemic goiter in the region studied. rinary iodine levels were in the normal range and could be interpreted as indicating a transition phase to an improvement in iodine deficiency. Autoimmune diseases would only explain 4% of cases of goiter.

This article constitutes a approach in the analysis of the catastrophic flood occurred on October 1875 in the Guadalentin Basin and Lower Segura river. In first place, event implications within scientist, political and socioeconomic contexts areis described, as well as the magnitude of the same, valued in terms of material damages and morphological effects. Next the physical factors that contributes to the disaster are studied, revising the pluviométric and hydrological data and deducing another new (kinetic energy of the rain, rainfall erosivity, hydraulic parameters for the methods of Manning and MEI). Finally the situation of the basin in relation to the risk is shown, emphasizing the effects derived of the works of defense and other civil structures like an clear indicator of the environmental fragility imposed for the human action., Este artículo constituye una nueva aproximación en el análisis de la catastrófica inundación ocurrida los días 14 y 15 de octubre de 1875 en la cuenca del Guadalentín y vegas media y baja del Segura, dedicando especial atención a esta última, por su extraordinaria gravedad y mayor grado de información. En primer lugar se describe la trascendencia del suceso dentro de los ámbitos científico, político y socio-económico, así como la magnitud del mismo, valorada en términos de daños materiales y efectos geo- morfológicos. A continuación se estudian los factores físicos que contribuyeron a la catástrofe, haciendo una revisión de los datos pluviométricos e hidrológicos existentes y deduciendo otros nuevos (energía cinética de la lluvia, erosividad pluvial, obtención de parámetros hidráulicos por los métodos de Manning y MEI). Finalmente se muestra la situación de la cuenca ante el riesgo, destacando los efectos derivados de las obras de defensa y de otras infraestructuras como un claro indicador de la fragilidad de este medio impuesta por el hombre.

The basic chemical composition and the fatty acid (FA) profile of Uruguayan Colonia cheese, consumed in the Mercosur region, were determined. 24 cheeses from different dairy farms: 10 elaborated in autumn and 14 in spring, were analyzed. Those cheeses are made from cow´s milk, whose diet was composed of 50% pasture system, 25% concentrate, and 25% silage reserves. The cheese making and ripening time were similar. Seasonal differences emerged in the profiles of saturated FA (SFA), unsaturated FA (UFA) and polyunsaturated FA (PUFA). The seasonal comparation of polyunsaturated FA (PUFA) as well as the CLA isomer such as C18:2 t10c12 showed significant differences (p < 0.05). CLA isomer such as C18:2 c9t11, showed no differences (p > 0.05). The atherogenesis index (AI) = 2.21 and thrombogenicity (IT) = 2.84, were calculated and discussed.

Antecedentes: el tratamiento endovascular ofrece la posibilidad de cubrir el origen de la disección y evitar que progrese hasta aneurisma de la aorta, y con ello demuestra una reducción en la mortalidad hasta del 16%. Objetivo: evaluar los resultados quirúrgicos en términos de morbi-mortalidad de los pacientes sometidos a manejo endovascular de las lesiones de la aorta torácica en la Fundación Cardiovascular de Colombia desde 2003 hasta 2005. Diseño-método: estudio longitudinal tipo descriptivo retrospectivo, en el que se evaluaron las historias clínicas de todos los pacientes sometidos a manejo endovascular de patología toracoabdominal; en éste sólo se incluyeron los pacientes con procedimientos de la aorta torácica, desde 2003 hasta 2005. El análisis de los datos se realizó en Stata/SE 8,0. Resultados: se realizaron procedimientos de aorta torácica en 16 pacientes. El 75% de los pacientes eran hombres con edad promedio de 55,9 ± 12,6 años. El 87,5% (14 pacientes) presentaban disección aórtica tipo A o B; un paciente transección traumática de la aorta y un paciente aneurisma de aorta torácica descendente. Las disecciones agudas se presentaron en 78,6% (11 pacientes) y las crónicas en 21,4% (3 pacientes). El promedio de endoprótesis usadas fue de 2,8 ± 1. La estancia en la unidad de cuidados intensivos fue de 3 ± 2,7 días. El 81,3% de los pacientes no presentaron complicaciones mayores. La mortalidad fue del 18,7% (3 pacientes). A todos se les realizó control post-operatorio con tomografía axial computarizada, con evolución satisfactoria. Conclusión: de acuerdo con los reportes de la literatura con mayor casuística, se considera que el manejo endovascular para el tratamiento de la disección, aneurisma o trauma aórtico es un procedimiento confiable que disminuye la morbi-mortalidad.Antecedents: endovascular treatment has the possibility of covering the dissection origin and to avoid its progression to aortic aneurysm, showing a 16% mortality reduction. Objective: evaluate surgical results in terms of morbid-mortality in patients submitted to endovascular management of thoracic aortic lesions in the Colombian Cardiovascular Foundation from 2003 to 2005. Design-Method: longitudinal descriptive retrospective study in which clinical histories of all patients submitted to endovascular treatment of thoracico-abdominal aortic lesions were evaluated. Only patients with thoracic aortic procedures between 2003 and 2005 were included. Data analysis was realized in Stata/SE 8,0. Results: thoracic aortic procedures were performed in 16 patients. 75% were male with mean age 55.9 ± 12.6 years. 87.5% (14) had type A or B aortic dissection; one patient had traumatic aortic transection and one had aneurysm of thoracic descendant aorta. Acute dissections were presented in 78.6% (11 patients) and chronic dissections in 21.4% (3 patients). Average of endoprosthesis employed was 2.8 ± 1. Intensive care unit stay was 3 ± 2.7 days. 81.3% had no mayor complications. Mortality was 18.7% (3 patients). All patients had post-operative computerized tomography scan, with satisfactory evolution. Conclusion: according to the largest casuistics literature reports, endovascular management of dissection, aneurysm or aortic trauma is considered a trustworthy procedure that diminishes morbid-mortality.

Antecedentes: el accidente cerebrovascular es la tercera causa de muerte y probablemente la causa más importante de discapacidad a largo plazo. La tasa de mortalidad está entre 15% y 35% con el primer ataque y se eleva a 65% para los accidentes cerebrovasculares subsiguientes. Los resultados a largo y mediano plazo avalan esta técnica quirúrgica para el tratamiento de la enfermedad carotídea, al demostrar altos perfiles de seguridad. Objetivo: determinar los resultados de morbilidad y mortalidad quirúrgica y a 3, 6 y 12 meses de seguimiento, en los pacientes sometidos a endarterectomía carotídea desde 1998 hasta septiembre de 2005. Diseño-método: se evaluaron las historias clínicas de los pacientes sometidos a endarterectomía carotídea, desde 1998 hasta septiembre de 2005. Resultados: se realizaron 42 procedimientos desde 1998 hasta septiembre de 2005; el 57% de los pacientes eran hombres, con edad promedio de 68,8 ± 9,9 años. Los antecedentes de importancia fueron: hipertensión arterial (82%), tabaquismo (61%) y dislipidemia (50%). El 82% de los pacientes mostraban síntomas de enfermedad carotídea. Once pacientes tuvieron lesión de carótida derecha, 16 de carótida izquierda y 15 lesión bilateral, con un promedio de obstrucción de 85 ± 11,4%. La mortalidad fue de 4,7% y no estaba relacionada con el procedimiento quirúrgico. A septiembre de 2005 el 57% de los pacientes egresados estaban libres de síntomas. Conclusiones: la mortalidad para este grupo de pacientes es comparable con los resultados publicados en todo el mundo. La seguridad del procedimiento avala esta técnica quirúrgica como la primera opción en el manejo de la patología carotídea.Antecedents: cerebrovascular disease is the third cause of death and probably the most common cause of significant long term disability. Mortality rate with first stroke is between 15% and 35% and goes up to 65% with subsequent strokes. Results at long and middle term endorse carotid endarterectomy for carotid artery disease treatment by demonstrating high safety profiles. Objective: to determine the surgical morbidity and mortality results at 3, 6 and 12 months of follow-up in patients submitted to carotid endarterectomy from 1998 to 2005. Design-Method: clinical histories of patients submitted to carotid endarterectomy from 1998 to September 2005 were evaluated. Results: 42 procedures were realized since 1998 until September 2005. 57% of the patients were men with mean age 68.8 ± 9.9 years. Important antecedents were arterial hypertension (82%), cigarette smoking (61) and dyslipidemia (50%). 82% showed symptoms of carotid disease. 11 patients had right carotid lesion and in 15 the lesion was bilateral, with mean obstruction of 85 ± 11.4%. Mortality was 4.7% and was not related to the surgical procedure. At September 2005, 57% of the discharged patients were asymptomatic. Conclusions: mortality for this group of patients is comparable to the worldwide published results. Procedure safety guarantees this surgical technique as the first option in this carotid pathology management.

Antecedentes: desde 1991 la técnica endovascular se ha aplicado con éxito en el manejo de los aneurismas de aorta infrarrenal, y se ha perfeccionado de manera tal que rápidamente se ha convertido en una alternativa para pacientes de alto riesgo para la cirugía convencional. Objetivo: describir los resultados institucionales en el manejo de las patologías de aorta abdominal e ilíacas mediante técnica endovascular desde 2003 a 2005. Diseño-Método: estudio descriptivo, longitudinal, retrospectivo, en el que se analizaron las historias clínicas de los pacientes sometidos a procedimiento endovascular de aorta abdominal e ilíacas. El análisis se realizó en Stata 8,0 S/E. Resultados: a 9 pacientes se les realizó exclusivamente manejo de lesiones en aorta abdominal e ilíacas. Todos los pacientes del estudio fueron hombres con edad media de 68,9 + 8,1 años. Los diagnósticos fueron aneurisma de aorta infrarrenal en 6 pacientes y aneurismas anastomóticos en los 3 restantes. Se evidenció requerimiento de endoprótesis en promedio de 1,9 + 0,8. Se realizó puente femoro-femoral como procedimiento simultáneo en 4 de los 9 pacientes. El 77,8% de los pacientes no tuvo complicaciones. La mortalidad por el procedimiento alcanzó el 22% (2 pacientes), si bien cabe anotar que las complicaciones se presentaron sólo en esos dos pacientes. Conclusiones: la exclusión de aneurismas de aorta y de ilíacas con endoprótesis modulares, se está implementando ampliamente como una opción válida de tratamiento, con resultados excelentes que evitan los riesgos de la intervención convencional y la morbilidad asociada.Antecedents: since 1991 endovascular technique has been successfully used in the management of infra-renal aortic aneurysms and it has been improved in such a way that it has quickly turned into an alternative for patients considered having high risk for conventional surgery. Objective: describe the institutional results in the management of abdominal aortic pathologies through endovascular technique from 2003 to 2005. Design-Method: descriptive, longitudinal, retrospective study in which clinical histories of patients that underwent an endovascular procedure of abdominal aorta and iliac arteries were analyzed. The analysis was performed in Stata 8,0 S/E. Results: 9 patients received exclusively treatment for abdominal aortic and iliac lesions. All were male individuals with mean age 68.9 ± 8.1 years. 6 patients had diagnosis of infra-renal aortic aneurysm and the other 3 had anastomotic aneurysms. Requirement of endoprosthesis was evidenced in an average of 1.9 ± 0.8. Femoro-femoral bypass surgery was performed as simultaneous procedure in 4 of the 9 patients. 77.8% of patients had no complications. Mortality due to the procedure was 22% (2 patients) and it is important to notice that only these 2 patients had complications. Conclusions: exclusion of aortic and iliac aneurysms with modular endoprosthesis is being widely implemented as a valid treatment option, with excellent results that avoid the risks of conventional surgery and its associated morbidity.

Este artículo constituye una nueva aproximación en el análisis de la catastrófica inundación ocurrida los días 14 y 15 de octubre de 1875 en la cuenca del Guadalentín y vegas media y baja del Segura, dedicando especial atención a esta última, por su extraordinaria gravedad y mayor grado de información. En primer lugar se describe la trascendencia del suceso dentro de los ámbitos científico, político y socio-económico, así como la magnitud del mismo, valorada en términos de daños materiales y efectos geo- morfológicos. A continuación se estudian los factores físicos que contribuyeron a la catástrofe, haciendo una revisión de los datos pluviométricos e hidrológicos existentes y deduciendo otros nuevos (energía cinética de la lluvia, erosividad pluvial, obtención de parámetros hidráulicos por los métodos de Manning y MEI). Finalmente se muestra la situación de la cuenca ante el riesgo, destacando los efectos derivados de las obras de defensa y de otras infraestructuras como un claro indicador de la fragilidad de este medio impuesta por el hombre.

Introduction: The aim was to determine the time elapsed between the start of treatment with antiparkinsonian agents and the modification of the pharmacological therapy, and to establish its related factors, in a group of patients with Parkinson's disease from Colombia., Patients and Methods: Retrospective cohort study that collected information about the treatment of patients with Parkinson's disease who started drug therapy between June, 2011 and December, 2013; a five-year follow-up was performed. Survival analyses for time to therapy modification were generated, and factors related to these changes were determined using Cox regression models., Results: A total of 3,224 patients (51.8% men) with a mean age of 73.1 ± 13.5 years started treatment with antiparkinsonian agents. After five years, 2,046 patients (63.5%) had modifications in drug therapy, in a mean time of 36.4 months (95% confidence interval: 35.7-37.1). A total of 1,216 patients (37.8%) required the addition of another active principle, while 830 (25.7%) had a switch to another drug. In the multivariate analysis, male sex, age over 65 years, and the start of amantadine were identified as factors that increased the likelihood of therapy modification. The use of bromocriptine, biperiden, and monotherapy as an initial treatment were associated with a reduction in this likelihood., Conclusions: After five years of treatment, 63.5% of the patients with Parkinson's disease required modifications to their therapy, with a mean time of three years. Male sex, age over 65 years, and receiving initial therapy with amantadine affected the likelihood of switching therapy in these patients in Colombia.

Introduction and Objectives: In patients with tricuspid regurgitation (TR), edge-to-edge transcatheter tricuspid valve repair (TTVR) is the strategy with the highest penetration worldwide. A dedicated edge-to-edge TTVR system has recently become available in Europe. The present study describes the initial experience with the system in Spain., Methods: This multicenter study collected individual data from the centers accepted for the use of the novel system within an initial limited release. Between June 2020 and March 2021, all patients undergoing an edge-to-edge TTVR using the TriClip system in Spain were included in the study. The primary endpoint was the achievement of a TR reduction of at least 1 grade at discharge., Results: We included 34 patients. Most of them reported a previous history of atrial fibrillation (91%) and only 1 had a pacemaker lead. The primary endpoint (TR reduction of at least 1 grade at discharge) was met in all patients. Most of the patients required 1 (47%) or 2 clips (44%) with a clear predominance of XT (87%) over NT (13%). The location of the first clip was anteroseptal in >90% of the patients. Only 1 patient had a partial detachment, which was stabilized with additional clips in the same procedure. At discharge, TR severity was≤2 in 91% of patients. At 3 months, mortality was nil. Overall, 88% of patients were in New York Heart Association functional class≤2 and 80% had residual TR≤2., Conclusions: Edge-to-edge TTVR seemed to be effective and safe with a sustained TR reduction at 3 months. Introduction and Objectives: The Spanish Registry of Acute Aortic Syndrome (RESA) was launched in 2005 to identify the characteristics of acute aortic syndrome (AAS) in Spain. The aim of this study was to analyze the differences in management and mortality in the 3 RESA iterations., Methods: We analyzed data from patients with AAS prospectively included by 24 to 30 tertiary centers during the 3 iterations of the registry: RESA I (2005-2006), RESA-II (2012-2013), and RESA III (2018-2019)., Results: AAS was diagnosed in 1902 patients (74% men; age, 60.7±12.5 years): 1329 (69.9%) type A and 573 (30.1%) type B. Comparison of the 3 periods revealed that the use of computed tomography increased as the first diagnostic technique (77.1%, 77.9%, and 84.2%, respectively; P=.001). In type A, surgical management increased (79.6%, 78.7%, and 84.5%; P=.045) and overall mortality decreased (41.2%, 34.5%, and 31.2%; P=.002), due to a reduction in surgical mortality (33.4%, 25.1%, and 23.9%; P=.003). In type B, endovascular treatment increased (22.8%, 32.8%, and 38.7%; P=.006), while medical and surgical treatment decreased. Overall type B mortality also decreased (21.6%, 16.1%, and 12.0%; P=.005) in line with a reduction in mortality with medical (16.8%, 13.8%, and 8.8%, P=.030) and endovascular (27.0%, 18.0%, and 9.2%; P=.009) treatments., Conclusions: The iterations of RESA show a decrease in mortality from type A AAS, coinciding with an increase in surgical treatment and a reduction in surgical mortality. In type B, the use of endovascular treatment was associated with improved survival, allowing better management in patients with complications., (Copyright © 2022. 24. When people who inject drugs speak: Qualitative thematic analysis of the perception of a mobile app for needle exchange programs.

- Abstract
Spain is the Western European country with the highest prevalence of Human Immunodeficiency Virus among people who inject drugs. The Hepatitis-C Virus affects over fifty per cent of this population. At the same time, the World Health Organization considers that the average coverage of injection material for drug user per year is low. Harm reduction programs and services have been deployed for over thirty years, and these could now incorporate the advantages of eHealth and mHealth to improve harm reduction. The aim of this qualitative and descriptive study is to analyze how people who inject drugs perceive an application for mobile devices. Fifty-one such drug users participated actively in five focus groups. The main results of the thematic content analysis indicated that the application was welcomed as easy and useful. Participants reported that the application contributed to improving access to injection material, reducing the stigma of drug-dependence and optimizing the organization of the ritual of injection. Excessive preventive information and problems downloading the web app were identified as aspects for improvement. In conclusion, the application was seen as a useful eHealth tool that complements the normal intervention of needle exchange programs.

- Abstract
This study aims to analyse the use of an instant messaging app (WhatsApp®) as a means of communication for reaching people who inject drugs. An eight-week prospective longitudinal and observational study with three observations was designed for five addiction centres in Catalonia. The participants were 105 people who inject drugs, distributed in five intervention groups. The results of the Risk Assessment Battery (RAB) were compared in the three levels of analysis pre-test, post intervention and one month after the intervention. The main results indicate a significative reduction in RAB scores after the intervention. The main conclusion was that the WhatsApp® intervention has great potential for developing harm reduction interventions and to reduce the HIV contagion risk.

Objective: Homelessness, which is a severe social problem across Europe and on the rise over the past decade, is closely linked to organic and mental health problems. In front of the lack of diagnostic analysis studies in the Spanish territory, this research aimed to determine the sociodemographic characteristics of a sample of individuals experiencing homelessness (IEH), to determine the prevalence of diagnoses of substance use disorders, dual pathology and other mental disorders and to analyze the existence of diagnostic differences between IEH born in the country and immigrant IEH., Methods: A transversal and observational study was carried out, based on the analysis of mental health diagnostics of the medical histories of a sample of 1,072 IEH (453 locals and 619 immigrants) assisted between 2008 and 2017 in mental health services in Girona, northeastern Spain. For the statistical analysis, Student's t test was used to compare means and the chi-squared test was used for the analysis of qualitative variables. A binary logistic regression analysis was carried out to establish the predictor variables for the diagnosis of drug addiction and dual pathology., Results: IEH showed high rates of mental disorders, especially psychoactive substances (82.6% of diagnosed people). 43.9% of IEH showed alcohol dependence as the reason for their diagnosis, followed by cocaine addiction and opiates. Foreign born IEH had more diagnoses for alcohol dependence and less for opiates or dual pathology than spanish born IEH. IEH born in Spain showed a higher vulnerability and received more mental health diagnoses than immigrant IEH., Conclusions: Even though immigrant PESS show more diagnoses for alcohol dependence, PESS born in Spain show a higher vulnerability and receive more mental health diagnoses than immigrant PESS. - Abstract
Introduction: The aim of this study is to assess epidemiological, clinical and laboratory characteristics of primary infection by Epstein-Barr virus (EBV) in children without previous diagnosis of any immune disease and its relationship with clinical presentation., Patients and Methods: A retrospective study was conducted on all children from 0 to 15 years with IgM against viral capsid of EBV positive or indeterminate during a 22 month period. Epidemiological, clinical and laboratory data were analysed and compared between typical (mononucleosis syndrome) and non-typical clinical symptoms., Results: The study included a total of 103 children, with a median age of 7 years (3-12.5 years). Almost two-thirds (63%) of patients had typical clinical signs, with a mononucleosis syndrome, and 37% had a non-typical presentation. The non-typical clinical group had a lower age (P=.03) and took less antibiotic than the typical clinical group (P=.015). From laboratory studies, there were no differences between the groups, except in RCP, which was higher in typical clinical group (P=.04). Heterophile antibodies were positive in 33% of patients. An indeterminate IgM against viral capsid was present in 20% of the patients, and most of them had an oligosymptomatic or atypical presentation. An IgM positive for other viruses was found in 21%, and 3 of them were suspicious of false positive for EBV., Conclusions: In the studied population, a primary infection due to EBV is common in younger ages, and they have usually an oligosymptomatic clinical presentation. A very low percentage of positive heterophile antibodies were found. Cases with indeterminate IgM against viral capsid are more frequent in the non-typical clinical group. - Abstract
Introduction and Objectives: The PARIS score allows combined stratification of ischemic and hemorrhagic risk in patients with ischemic heart disease treated with coronary stenting and dual antiplatelet therapy (DAPT). Its usefulness in patients with acute coronary syndrome (ACS) treated with ticagrelor or prasugrel is unknown. We investigated this issue in an international registry., Methods: Retrospective multicenter study with voluntary participation of 11 centers in 6 European countries. We studied 4310 patients with ACS discharged with DAPT with ticagrelor or prasugrel. Ischemic events were defined as stent thrombosis or spontaneous myocardial infarction, and hemorrhagic events as BARC (Bleeding Academic Research Consortium) type 3 or 5 bleeding. Discrimination and calibration were calculated for both PARIS scores (PARIS
ischemic and PARIShemorrhagic ). The ischemic-hemorrhagic net benefit was obtained by the difference between the predicted probabilities of ischemic and bleeding events., Results: During a period of 17.2 ± 8.3 months, there were 80 ischemic events (1.9% per year) and 66 bleeding events (1.6% per year). PARISischemic and PARIShemorrhagic scores were associated with a risk of ischemic events (sHR, 1.27; 95%CI, 1.16-1.39) and bleeding events (sHR, 1.14; 95%CI, 1.01-1.30), respectively. The discrimination for ischemic events was modest (C index = 0.64) and was suboptimal for hemorrhagic events (C index = 0.56), whereas calibration was acceptable for both. The ischemic-hemorrhagic net benefit was negative (more hemorrhagic events) in patients at high hemorrhagic risk, and was positive (more ischemic events) in patients at high ischemic risk., Conclusions: In patients with ACS treated with DAPT with ticagrelor or prasugrel, the PARIS model helps to properly evaluate the ischemic-hemorrhagic risk., (Copyright © 2018 Sociedad Española de Cardiología. - Abstract
Introduction and Objectives: The impact on mortality of myocardial infarction (MI) compared with the specific degree of bleeding severity occurring after discharge in acute coronary syndrome is poorly characterized. Defining this relationship may help to achieve a favorable therapeutic risk-benefit balance., Methods: Using Cox-based shared frailty models, we assessed the relationship between mortality and postdischarge MI and bleeding severity-graded according to Bleeding Academic Research Consortium (BARC)-in 4229 acute coronary syndrome patients undergoing in-hospital coronary arteriography between January 2012 and December 2015., Results: Both MI (HR, 5.8; 95%CI, 3.7-9.8) and bleeding (HR, 5.1; 95%CI, 3.6-7.7) were associated with mortality. Myocardial infarction had a stronger impact on mortality than BARC type 2 and 3a bleedings: (RRr, 3.8 and 1.9; P < .05), respectively, but was equivalent to BARC type 3b (RRr, 0.9; P = .88). Mortality risk after MI was significantly lower than after BARC type 3c bleeding (RRr, 0.25; P < .001). Mortality was higher after an MI in patients on dual antiplatelet therapy (DAPT) at the time of the event (HR, 2.9; 95%CI, 1.8-4.5) than in those off-DAPT (HR, 1.5; 95%CI, 0.7-3.4). In contrast, mortality was lower after a bleeding event in patients on-DAPT (HR, 1.6; 95%CI, 1.1-2.6) than in those off-DAPT (HR, 3.2; 95%CI, 1.7-5.8)., Conclusions: The differential effect on mortality of a postdischarge MI vs bleeding largely depends on bleeding severity. Aim: The aim of this study is to define the risk factors associated with early discharge in out-patients clinics., Design: Cross-sectional and observational study., Setting: Substance abuse clinics in Girona (Catalonia, Spain)., Participants: A total of 264 individuals were included in the sample, and 34.8% of them abandoned the process within two months of starting the therapy (n=92)., Procedure: Clinical and socio-demographic variables of the clinical history were compared between participants with/without adherence., Main Measurements: The Student t test was used to measure the comparison, and the chi-squared test was used for the analysis of qualitative variables. A binary logistic regression model was adjusted, with adherence as the dependent variable., Results: The results indicated that attending the appointments unaccompanied (OR=3.13), being female (OR=2.44), having cocaine related issues (OR=1.14), and being younger (OR=0.89) are the factors which increase the risk early abandonment. Contrarily, being referred to specialists from a Primary Health Centre reduces the risk (OR=0.28)., Conclusions: It is concluded that special attention must be devoted to the patients' families, women, and young patients. Introduction and Objectives: Acute coronary syndrome (ACS) guidelines recommend the use of newer P2Y
12 inhibitors (prasugrel and ticagrelor) over clopidogrel in patients with moderate-to-high ischemic risk, unless they have an increased bleeding risk. The aim of our study was to assess the GRACE risk score and the CRUSADE bleeding risk score relative to prescription of newer P2Y12 inhibitors at discharge in ACS patients., Methods: Retrospective analysis of a multicenter ACS registry; 3515 consecutive patients were included. The association between risk scores and prescription of newer P2Y12 inhibitors was assessed by binary logistic regression analysis., Results: A total of 1021 patients (29%) were treated with prasugrel or ticagrelor. On multivariate analyses, both GRACE (OR per 10 points, 0.89; 95%CI, 0.86-0.92; P < .001) and CRUSADE (OR per 10 points, 0.96; 95%CI, 0.94-0.98; P < .001) risk scores were inversely associated with the use of newer P2Y12 inhibitors. Moreover, other factors not included in these scores (revascularization approach, in-hospital stent thrombosis, major bleeding, and concomitant indication for anticoagulation therapy) also predicted the use of newer P2Y12 inhibitors., Conclusions: New P2Y12 inhibitors were more frequently prescribed among ACS patients with lower CRUSADE bleeding risk. However, an ischemic risk paradox was found, with higher use of these agents in patients with lower ischemic risk based on GRACE risk score estimates. - Abstract
Improvements in early detection and treatment have markedly reduced cancer-related mortality. However survival not only depends on effectively cure cancer, but prevention, diagnosis and treatment of cancer-related complications is also needed. Cardiovascular toxicity is a widespread problem across many classes of therapeutic schemes, however scientific evidence in the management of cardiovascular complications of onco-hematological patients is scarce, as these patients have been systematically excluded from clinical trials and current recommendations are based on expert consensus. Multidisciplinary teams are mandatory to decrease morbidity and mortality from both cardiotoxicity and cancer itself. An excessive concern for the occurrence of cardiovascular toxicity, can avoid potentially curative therapies, while underestimating this risk, increases long-term mortality of cancer survivors. The objective of this consensus document, developed in collaboration of the Spanish Society of Cardiology, the Spanish Society of Medical Oncology, the Spanish Society of Radiation Oncology and the Spanish Society of Hematology, is to update the necessary concepts and expertise on cardio-onco-hematology that enable its application in daily clinical practice and to promote the development of local multidisciplinary teams, to improve the cardiovascular health of patients with cancer., (Copyright © 2017 Sociedad Española de Cardiología. Background: To adapt and to validate in a Spanish population the Perception of Restraint Use Questionnaire (PRUQ), which assesses the importance that professionals give to the use of physical restraints when caring for older adults., Methods: After a process of forward-back translation and linguistic adaptation, a Spanish version of the original questionnaire was obtained. A descriptive cross-sectional multicenter study was then carried out. Based on non-probability sampling, 20 centers from three Spanish regions were selected; 830 professionals agreed to participate., Results: 15 of the 17 items of the original questionnaire were translated literally and two required minor modifications.All were considered acceptable by an expert panel(content validity index of 0.89); a pilot study confirmed the adequate feasibility of the questionnaire. The principal components analysis identified three dimensions that explained 66.2% of variance. The confirmatory factor analysis of this tridimensional model showed an acceptable fit [CFI = 0.936; RMSEA = 0.080], being factor loadings and factor correlations statistically significant (p<0.001). The internal consistency (Cronbach's alpha) for the total score was 0.92 and the test-retest reliability (ICCa) was 0.87 (95%CI: 0.78 to 0.92) over an interval of three weeks., Conclusions: The Spanish version of the PRUQ shows good psychometric characteristics and is adapted to the cultural context of this country. It may be considered a useful tool to assess in which situations professionals consider the use of physical restraints most necessary, helping to design training activities aimed at rationalizing its application.

Objectives: To determine the prevalence of metabolic syndrome (MS) according to the definitions of the National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III) and the International Diabetes Federation (IDF) and its relation to cardiovascular disease (CVD) in hospitalized elderly patients., Material and Methods: This descriptive and prospective study (February-March 2011) included 200 consecutive patients hospitalized in a Geriatric Department. Sociodemographic, clinical and biochemical data was collected., Results: The prevalence of MS was 65% (NCEP-ATP III) and 67.5% (IDF) and was greater in women (NCEP-ATP III=72.8%, IDF=73.6%) than in men (NCEP-ATP III=50.7%; IDF=56.3%). The mean age of patients diagnosed with MS by both diagnostic criteria were similar: 84.7 years. MS was not associated with an increased prevalence of CVD., Conclusions: MS is highly prevalent in elderly hospitalized patients, being higher in women, with both diagnostic criteria (NCEP- ATP III and IDF). - Abstract
Health is one of the fundamental human rights. Recognizing it as a right means that the State has an obligation to ensure a minimum level of opportunities is maintained, and to restore it when lost. This minimum level may not be covered in periods of economic crisis, such as the one we are currently experiencing.Managed care, focused on economic questions, emerged after the crisis of 1973 in order to help make clinical decisions based on economic factors. In practice, the result of managed care was to turn economic cost control into an end in itself while forgetting about equity; something for which it has been challenged from an ethical perspective. Since then, many authors have attempted to reconcile efficiency and equity in health management, but the debate remains open.In this article, and basing our approach on the theories of P. Ruger and Norman Daniels, we argue that shared health governance and accountability for reasonableness can offer significant ethical contributions in the process of achieving an efficient and fair health system. In the model we propose, citizens, professionals and health institutions all play an active role in capacity building in the field of health. These capacities are related to healthy lifestyles, accessible and transparent information, the promotion of self-care, the acquisition of knowledge, skills and appropriate attitudes, leadership based on values and co-responsibility to achieve set goals in a reasonable way. If we develop these capacities, we will have used the current economic crisis as an opportunity for improving ethical practice in the field of health.

Introduction: Using heel puncture by lancets for the determination of metabolic tests (hypothyroidism and phenylketonuria) using heel puncture is a very painful procedure. Venepuncture can be used as an alternative., Objectives: To determine whether venepuncture in the dorsal side of the hand is less painful and more efficient than heel puncture in the screening test for inborn errors of metabolism, as well as comparing false positives in the hypothyroidism test., Material and Methods: We tested 607 consecutives newborns in an observational, randomised study (280 with venepuncture, and 327 with heel puncture). Pain response was assessed using the Neonatal/Infant Pain Scale (NIPS), and the delay and duration of the first cry. We measured the number of punctures needed, and the time taken to complete the test, as well as the number of false positives in the hypothyroidism test., Results: Venepuncture was less painful than heel prick (NIPS 2 vs 5, P<.001), as well as in the delay (crying in 57.8% vs 90.2%, P<.0001) and duration of the cry (58 vs 104 seconds). Venepuncture was also faster (60 vs 120seconds), and required less puncture attempts. There were no differences in cases with TSH greater than 5mU/mL, or greater than or equal to 9 mU/mL., Conclusions: Venepuncture is associated with less pain, is faster to perform and not is associated with more false positive cases., (Copyright © 2011 Asociación Española de Pediatría. - Abstract
Understanding the classic doctor-patient relationship has entered a crisis in medicine today. Communication difficulties, underestimation of empathy, or bringing economic criteria are leading to care styles that facilitate the objectification of patients, contempt for doctors, and indifference of both to collective measures of health. Basic principles of this relationship such as quality, justice, patient autonomy and beneficence may appear weak content. We intend to apply the concept of "reciprocal recognition" from philosophy to "recharge" content different aspects of the clinical relationship: a look "inside", dealing with the identity of the protagonists seek to promote it by using reciprocal recognition of both patient and the professional. In one aspect "external", focusing on the quality of care, a key objective of the clinical relationship. With reciprocal recognition, issues of justice and equity are reinforced through public health, building an identity of citizens with rights.

- Abstract
Quality of life is seriously compromised in both severe aortic stenosis and in hypertrophic obstructive cardiomyopathy. At advanced stages of disease, symptoms become incapacitating. Surgical correction is extremely effective, reducing symptoms and improving functional capacity. Percutaneous surgical techniques facilitate the treatment of patients in high-risk categories for whom conventional surgery is not an option. We report the case of a woman with severe symptomatic aortic stenosis and hypertrophic obstructive cardiomyopathy with associated mitral regurgitation. An aortic valve was implanted percutaneously. Later septal ablation was also performed percutaneously. The postoperative course was slow and the hospital stay was long. Percutaneous procedures provide a useful alternative way to treat severe symptomatic aortic stenosis and hypertrophic obstructive cardiomyopathy with mitral regurgitation, particularly when patients are at high risk for conventional surgery. These procedures are not without risk, however.

Background: To estimate the agreement between observers on the detection of goitre by palpation in the school population because it is considered a variability test., Methods: For five months, during 2001/2002, the presence of goitre was studied in across-section sample of 845 school children (for 6 to 14 years old) from 18 school centres in a Health Area in Valentian Community. The exploration was always carried out by the same two observers. The thyroid size was established in 6 degrees. It was considered goitre since 0B inclusive. The agreement was assessed in relation to age, sex, IBM, and the exploration date. The Kappa Index was used as a measure of agreement., Results: The global prevalence of goitre was 40,4% according to the first observer and 36,8% to the second one. The agreement between observers was high, with a Kappa Index of 0.83 and it was similar in relation to sex, IBM, and the exploration date. It was smaller in the youngest children (six and seven years old) than in the oldest ones (from 12 to 14)., Conclusions: An excellent interobserver agreement in clinic assessment of goitre by palpation in a school children population was achieved. The least concordance was seen in youngest group. It would be advisable to include the study of agreement in the protocol of endemic goitre study.

Introduction and Objectives: Because acute aortic syndrome (AAS) is associated with high mortality, early diagnosis and treatment are vital. The aim of the Spanish Acute Aortic Syndrome Study (RESA) was to investigate the effectiveness of current treatment of AAS in a broad range of tertiary care hospitals in Spain., Methods: Between January 2005 and December 2007, 24 tertiary care hospitals reported data on 519 patients with AAS (78% male, mean age 61 +/- 13 years, range 20-92 years): 357 had type-A AAS and 162 had type B., Results: The time delay between symptom onset and diagnosis was <24 hours in 67% of cases and >72 hours in 11%. Some 80% of patients with type-A AAS were treated surgically. The interval between diagnosis and surgery was <24 hours in 90% of cases. In patients with type-B AAS, 34% received invasive treatment: 11% had surgery and 23% underwent endovascular procedures. Mortality during hospitalization in patients with type-A disease was 33% in those treated surgically and 71% in those treated medically. Mortality in patients with type-B disease was 17% with medical treatment, 27% with endovascular treatment and 50% with surgical treatment., Conclusions: Despite significant advances in the diagnosis of AAS, in-hospital mortality remains high. The findings of this study are representative of a broad range of unselected patients undergoing treatment for the disease and support the need for continuing improvements in therapeutic approaches to AAS.

- Abstract
Objective: Several studies performed in Spain have reported iodine deficiency in pregnant women but data from Valencia are lacking. The aim of the present study was to determine the degree of implantation of iodine supplementation in pregnant women in our health area., Patients and Method: A total of 232 pregnant women were studied in the first trimester of pregnancy. Thyroid-stimulating hormone (TSH), free thyroxine, and antiperoxidase antibodies were measured. A survey was performed on intake of iodized salt and pharmacological iodine supplements. Neonatal TSH concentrations were measured, based on screening of metabolopathies, in the newborns of participating women., Results: Then mean age was 30.5+/-4.4 years. A total of 60.8% of the women consumed iodized salt, 51.3% took iodine polyvitamins and only 14.2% consumed potassium iodide. The median urinary iodine concentration was 100 microg/l; 66% had urinary iodine concentrations below the recommended level (150 microg/l). A positive association was found between pharmacological iodine intake and urinary iodine (p=0.016). Ten percent of the women had mild hypothyroxinemia, while 5.7% of the newborns had elevated TSH levels., Conclusions: Iodine intake in pregnant women in our health area is low, despite current recommendations. Our data support the need to promote the use of iodized salt in every home and to systematically administer potassium iodide supplements in all pregnant women.
