JUSTIFICATIVA E OBJETIVOS: Desde a descoberta dos receptores opióides e um maior entendimento da neurofarmacologia da medula espinhal em relação à transmissão e inibição do impulso nociceptivo, tem aumentado o interesse na utilização de drogas por via subaracnóidea, em Anestesiologia e tratamento da dor. O presente estudo tem como finalidade avaliar prospectivamente a eficácia clínica da adição de fentanil (25 µg) e sufentanil (5 µg) à bupivacaína isobárica (10 mg),em pacientes submetidos à raquianestesia para tratamento cirúrgico de varizes de MMII. MÉTODO: Participaram do estudo 60 pacientes, estado físico ASA I e II, com idades abaixo de 60 anos, submetidos à raquianestesia para tratamento cirúrgico de varizes e divididos aleatoriamente em três grupos: B (bupivacaína isobárica 10 mg), BF (bupivacaína isobárica 10 mg e fentanil 25 µg) e BS (bupivacaína isobárica 10 mg e sufentanil 5 µg).A raquianestesia foi realizada em decúbito lateral esquerdo, punção lombar entre L3-L4, com agulha de Quincke 27G. Após a raquianestesia foram avaliados: tempo de latência, o nível de bloqueio sensitivo e bloqueio motor. No período per-operatório foram anotados: hipotensão arterial, bradicardia, depressão respiratória, diminuição da saturação periférica da hemoglobina pelo oxigênio, náuseas, vômitos, prurido e tremor. No período pós-operatório foi pesquisado o tempo de analgesia. RESULTADOS: Os grupos foram homogêneos. O tempo de latência, o bloqueio motor e o tempo de analgesia não apresentaram diferença significativa. O nível de bloqueio sensitivo mostrou diferença entre o grupo controle e os grupos contendo opióides em todos os momentos estudados. Prurido foi o efeito colateral mais freqüente nos grupos contendo opióides. CONCLUSÕES: A adição de fentanil (25 µg) e sufentanil (5 µg) à bupivacaína a 0,5% isobárica (10 mg) em raquianestesia altera o nível de bloqueio sensitivo.

Abstract Green areas in urban landscapes are under strong anthropogenic pressure, and, at the same time are fundamental to maintaining biodiversity, as they provide resources for many animal and plant species. Knowing these species is fundamental for its maintenance and conservation, and inventories are extremely important for monitoring fauna and conserving it. Therefore, the goal of this research is to inventory the butterflies species in the park of the Instituto Butantan (Ibu), located in an urban area in the city of São Paulo, southeast Brazil. The surveys of butterflies were conducted through visual censuses from August 2017 to July 2019 and recorded a total of 324 butterfly species. The most speciose family was Hesperiidae, followed by Nymphalidae, Lycaenidae, Pieridae, Riodinidae, and Papilionidae. Among the sampled species, there is Euselasia zara which is a new record for the state of São Paulo. Neither the species accumulation nor the richness estimator curves tended to reach an asymptote, suggesting that additional butterflies’ species will be recorded with more sampling effort on the site. Even with a flora composed mainly of exotic and ornamental plants, the park of Instituto Butantan exhibits a very rich butterfly community. This community exhibits a pattern of seasonally variation, with the peak of species richness related to the rainy season. When compared with Cidade Universitária Armando de Salles Oliveira (USP), another nearby urban green area, which is larger, more heterogeneous and sampled over a longer period, it is possible to notice that the Ibu butterfly community is a subsample of this larger one. These results highlight the potential that urban parks have for the maintenance and conservation of butterfly species.

A Lei nº 14.133, de 2021, conhecida como Nova Lei de Licitações, introduziu importantes alterações no modelo de contratações públicas brasileiro. Dentre as modalidades licitatórias, a nova Lei prevê que tanto o pregão quanto a concorrência poderão ser utilizados nas contratações de serviços comuns de engenharia. A escolha da modalidade licitatória reflete diretamente na eficácia da contratação, não só do ponto de vista do objeto a ser contratado como também na economicidade da contratação. A indicação, portanto, da modalidade licitatória mais adequada para a contratação de serviços comuns de engenharia propiciará uma escolha segura por parte do gestor sobre a modalidade a ser utilizada, que, por sua vez, repercutirá diretamente no serviço prestado à sociedade e na aplicação eficiente dos recursos públicos.

Objective: Little data is available on long-term functional and cognitive outcomes in patients with aneurysmal subarachnoid hemorrhage (ASH). The main objective of this study was to assess cognition, functional state, mood disorders, and quality of life in patients with SAH at least six months following the ASH., Patients and Methods: Cross-sectional study of 40 patients (aged 58.2 [SD 9.9] years) with ASH, discharged from a Neurologic Rehabilitation unit between January 2010 and July 2017., Main Outcome Variables: functional status (Barthel index), cognition (Pfeiffer questionnaire), depression (Hamilton scale), and health-related quality of life (European Quality of Life-5 Dimensions [EQ-5D]), as well as type and duration of therapeutic rehabilitation procedures after discharge., Results: From 35 patients with cognitive disorders, only 12 received cognitive therapy at hospital discharge. In the long-term follow-up, cognitive impairment persisted in 22 patients. When compared with those without cognitive impairment, they presented significantly worse mean differences in the Barthel index (15.5 [95% CI: 1.2-29.7]), Hamilton scale (-0.8 [95% CI: -1.27 to -0.37]), and EQ-5D (27.6 [95% CI: 12.4-19])., Conclusion: The prevalence of long-term cognitive impairments in survivors of a SAH episode is high, and their presence is associated with worse functional status, more depression and worse quality of life. The low percentage of subjects who received cognitive therapies through their recovery process and the clinical implications observed, support the need of including neuropsychological therapies in the rehabilitation programs after an SAH event., (Copyright © 2021 Sociedad Española de Rehabilitación y Medicina Física. Trichophyton mentagrophytes complex is the most frequent agent found in dermatophyte isolates after Trichophyton rubrum. It is divided into 3main groups: (1) Trichophyton benhamiae and related species; (2) Trichophyton simii and 2related species, Trichophyton quinckeanum and Trichophyton schoenleinii; and (3) T. mentagrophytes, T. interdigitale, and related species. They are all difficult to identify by morphophysiology. With the aim of classifying them at the species level, a descriptive study was performed on 17 isolated clinical strains identified in the Mycology Laboratory of the Universidad de Valparaíso as belonging to the T. mentagrophytes complex. They were compared with 3 reference strains (T. mentagrophytes CBS 318.56, T. interdigitale CBS 428.63, Trichophyton erinacei CBS 511.73). Morphophysiological and molecular tests were performed by high resolution melting analysis curves and ITS regions sequencing. Phenotypically, 3 species of the complex were identified. Molecular analyses reclassified all the species as belonging to T. interdigitale. In conclusion, no reliable morphophysiological patterns were established to differentiate between the species of the complex. The clubfoot is one of the most common conditions in Pediatric Orthopedics, may affect each part of the foot and ankle, equinus, varus, and internal rotation of the calcaneum, and true equinus of the ankle are common. The Ponseti method is an universally accepted treatment, consisting of three phases: manipulation and plaster, Achilles tendon tenotomy and maintenance phase. The highest percentage of relapses occurs in the maintenance phase and the deficient family member is associated in most cases. We present a clinical case of a patient with typical clubfoot treated with the Ponseti method on four occasions without the need of surgical treatment.

Spasticity is a common complication that occurs in those patients that have suffered a stroke. To identify those patients at high risk of having post-stroke spasticity and to start treatment at early stages would probably benefit the patient. The key aspects in the early management of post-stroke spasticity were review and the clinical implications and strength of evidences were also considered. The document drafted by the study coordinators was subsequently reviewed and then a validated document was developed. The experts recommend defining early treatment of spasticity as one that begins before the first three months after stroke. The panel considers very important to identify the risk factors associated with the onset of spasticity, since this might reduce its impact. Additionally, the most common conditions subsidiaries of early treatment of both upper and lower limb are defined. The panel recommends that the treatment with botulinum toxin A must only be given by specialists with experience in diagnosis and management of spasticity. In conclusion, the treatment of focal spasticity in the first three months after stroke is indicated in certain situations. These recommendations help to standardize the early management of post-stroke spasticity, with the consequent support to clinicians and patients.

Introduction: Among stroke consequences highlights the negative impact on the informal carers' health related quality of life (HRQoL)., Objectives: To identify HRQoL dimensions most greatly affected in stroke caregivers and establish relations with dependent variables of survivors and caregivers., Methods: Transversal study of 215 informal caregivers of stroke survivors (1:3 man/woman, mean age 63.6 (±11.03) years). Main outcome measure was health perception assessed with the Short Fom 36 (SF-36). Clinical and demographic data of stroke patients were also registered. Statistical tests used were Chi-square, t-Student test and Spearman correlation coefficient., Results: Vitality and general health were the most affected dimensions perceived by caregivers. In comparison with the Spanish general population, female caregivers aged 45-54 years presented a significant reduction in the SF-36 subscales of vitality (p=0.003), social function (p=0.01), emotional role (p=0.038) and mental health (p=0.002). Female caregivers aged 55-64 years also showed a significant reduction in social function (p=0.027) and emotional role (p=0.024). Male caregivers did not present significant lower HRQoL., Conclusions: Caregiving experience has a negative impact on HRQoL, specially middle-aged female caregivers. Some carer physical and psychological characteristics (depression, musculoskeletal pain, time spent on caring) and survivor disability shown to influence carers outcomes. Caregivers play an important role in the recovery of stroke patients, so that the identification of strategies which help them to cope with the stroke might avoid that the role of caring results in a loss of quality of life., (Copyright © 2009 SECA. Objectives: To assess patient and caregiver satisfaction with the regular rehabilitation care after stroke. To identify patient features or aspects of the rehabilitation programme related to lower rates of satisfaction in our area., Material and Methods: Cross-sectional study of 74 stroke in-patients admitted from June 2005 to June 2006 (70.3% men, age 65.4 + or - 12.2 years, Barthel discharge 70.5 + or - 27). A telephone Satisfaction questionnaire (Pound 1999) was administered at 13.6 + or - 3.1 months to assess satisfaction regarding in-patient care, therapy and recovery, and services after hospital discharge. Three items addressed to caregiver satisfaction were also included. A total of 84 caregivers were identified: 71% relatives (partner or children), of which 80.9% were women., Results: The satisfaction questionnaire was completed by 52.9% of patients. Over 80% of interviewed patients were completely satisfied with in-patient care, and 52.7% were satisfied with therapy and recovery during admission, but more patients were dissatisfied with the amount of therapy received. The lowest rate of satisfaction was related to the services after discharge, with less than 25% of patients satisfied. A total of 83.1% of caregivers completed the questionnaire, of which 74.1% were satisfied with the information provided. Stroke educational training was given to 63.5% of caregivers, and 42.4% were given support after discharge. Only the presence of depression during admission and time passed after stroke were significantly related to patient satisfaction. (p<0.05)., Conclusions: Dissatisfaction with some aspects of stroke rehabilitation is considerable in our care area. Despite general patient and caregiver satisfaction regarding information provided, high dissatisfaction rates are also detected related to the amount of therapy and the social support provided after hospital discharge. According to these results, systematic information and training programs for patients and caregivers should be included in stroke rehabilitation, as well as providing social support at discharge., (Copyright 2009 SECA. Objective: To determine the quality of in-hospital stroke care in public acute care hospitals in Catalonia before the implementation of a clinical practice guideline (CPG) on stroke by determining adherence to specific recommendations of the CPG., Methods: We retrospectively reviewed the case notes of consecutive patients with stroke (defined with ICD-9 codes: 431, 433.x1, 434.x1, and 436) admitted to 48 Catalan hospitals within the first half of 2005. Data were collected on indicators of the healthcare process selected on the basis of their scientific evidence and/or clinical relevance. The participating hospitals included 20, 40 or 60 stroke cases according to their annual stroke caseload. After random selection, up to 9.3% of all cases recruited at each study center were externally monitored to assess the quality of the data gathered. Indicators were grouped into six different dimensions related to distinct aspects of clinical practice., Results: We analyzed data from 1,791 stroke cases (53.9% men, mean age: 75.6 [12.4] years). Overall inter-observer agreement was 0.7. Compliance with the six dimensions was as follows (mean percentage [95%CI]): quality of medical records, 78.5% (77.5-79.4); initial interventions, 92.4% (91.5-93.2); neurological assessment, 38.3% (37.3-39.3); assessment of rehabilitation needs, 44.9% (43.2-46.7); prevention and management of medical complications, 68.4% (66.9-70), and initial preventive measures, 78.9% (77.3-80.4)., Conclusions: In the first half of 2005, in-hospital stroke care in Catalonia showed room for improvement particularly in aspects related to the neurological assessment and follow-up of patients and their rehabilitation process.

Background: Biophasic insulin aspart (InAsBi) is a mixture of 30% of rapid acting soluble aspart insulin and 70% aspart insulin retarded with protamine. The soluble portion reduces postprandial serum glucose rises and the retarded portion reduces basal glucose levels., Aim: To assess the efficacy of biphasic insulin aspart in diabetics with a bad metabolic control., Material and Methods: Multicentríc study that included diabetic patients with a glycosilated hemoglobin over 7% that were transferred to treatment with InAsBi, given in one to three daily doses, according to glycemic control and followed for 12 weeks. At the end of follow up, glycosilated hemoglobin levels (HbA1c) were measured again., Results: One hundred ninety six patients were enrolled and 154, age 59+/- 12 (84 females), completed the follow up. HbA 1 c levels decreased in at least 1% in 96 and increased in eight cases. In the total group HbA1c decreased from 10.1+/- 1.7 to 8.4+/-1.4% (p <0.01). Those with higher initial values and with oral therapy, had the greatest reductions. At the end of the observation period, 29 patients received one daily dose of InAsBi, 114 two doses and 11 three doses. Two patients had allergy, one systemic and one in the injection site., Conclusions: In this group of diabetic patients with a bad metabolic control, the use of InAsBi was associated with a significant reduction of glycosilated hemoglobin levels.

Introduction: Many studies show that subjects with sequelae after stroke have a worse health perception in western societies. Due to the shortage of papers on the Spanish population in this regard, a study was carried out to assess health perception in hemiplegic patients 2 years after stroke in comparison with that of the general population and correlate health perception with demographic, clinical and functional variables., Methods: Cross-sectional study of 212 community stroke survivors. The main variables collected were: health perception assessed with the Short Form 36 (SF-36), disability assessed with the motor Functional Independence Measure (FIM) and Geriatric Depression Scale (GDS). Statistical tests used: chi2, Student t test and Spearman correlation coefficient., Results: In comparison with the Spanish general population, health perception in hemiplegic male stroke patients is diminished in all areas. In hemiplegic female patients, physical function is the only subscale significantly affected in all age groups. Variables related to worse health perception are: presence of depression, dependence and the belief of the need for kind of social support, A significant correlation was observed between SF-36 values and the motor FIM (r = 0.737 with physical function) and the GDS (r = -0.821 with mental health)., Conclusions: Health perception in hemiplegic patients 2 years after stroke is diminished in comparison with that 348 of the general population. Factors which determine worst health perception are depression, motor dependence, need of caregiver assistance and need of social support.

Spontaneous cervical epidural hematoma (CEH) is a rare entity but one that can cause permanent neurologic deficit unless, it is widely believed, surgical evacuation is prompt. A diagnosis of CEH is confirmed by computed tomography and/or magnetic resonance imaging. Spontaneous remission is unusual, with only 7 such cases reported in the literature as of 1993. We present a case of CEH with clinical remission and confirmation by neuroimaging in a patient who died of an unrelated cause. We conclude that when neurologic deficit is incomplete and there are no signs of clinical progression after 48 h, surgical treatment should not be applied routinely.
