Introduction and objective: Melanoma is one of the most malignant skin cancers originating from pigment cells, accounting for 5–7% of all skin cancers. The prognosis, especially in case of late diagnosis, is poor. The aim of the study is to systematise information on subungual melanoma. To raise awareness among the public and professionals working in the skin care sector of the diagnosis, early detection and treatment of subungual melanoma. Brief description of the state of knowledge: Subungual melanoma originates from subungual melanoma lentigines and constitutes from 0.7% to 3.5% of all malignant melanomas. Among dark-skinned populations, this subtype accounts for up to 30% and is the most common subtype of malignant melanoma among African Americans, Asians, and Latinos. Subungual melanoma occurs asymmetrically, most often affecting the big toe or thumb. In this disease, the starting point are melanocytes, which undergo malignant transformation within the nail matrix. A symptom of the disease may be melanonychia, a streak of pigment or irregular discoloration, eventually it takes the form of a triangle. This symptom indicates an advanced stage of the disease. The basis for making the diagnosis is dermoscopy and biopsy. Grazzini et al. suggest conducting a biopsy in each case where one nail plate is affected and the lesion persists for more than 6 months. An important element of diagnostics that helps diagnose of subungual melanoma is the ABCDEF algorithm. Knowledge of the above-mentioned helps to detect disturbing symptoms at an early stage of the disease, which improves the prognosis and increases the chances of cure. Summary: Early detection of melanoma gives a better chance of cure, reduces mortality and complications of the disease, therefore any action to increase public knowledge and awareness on the subject is important.

Resumen: La lectura de los parches de pruebas epicutáneas no es sencilla, presenta una gran variabilidad inter- e intraobservador y requiere experiencia. En ocasiones es realmente difícil determinar si se trata de una reacción alérgica de intensidad leve o si estamos ante una reacción irritativa. Recientemente se han publicado algunos trabajos que han estudiado las características dermatoscópicas de las distintas reacciones que se producen tras realizar pruebas epicutáneas. La característica dermatoscópica más frecuentemente observada en los parches alérgicos es el eritema homogéneo, si bien vasos puntiformes, vesículas o costras y áreas amarillo-anaranjadas también parecen indicar una reacción alérgica, guardando cierta similitud con lo observado en patología inflamatoria como en el eccema. Por otro lado, en cuanto a las reacciones irritativas, el patrón más indicativo sería el «patrón del poro», acompañado de eritema perifolicular. Estos hallazgos dermatoscópicos pueden ser de utilidad al clínico en su toma de decisiones ante una reacción dudosa. Abstract: Interpreting patch test reactions is not easy. It requires experience and is characterized by high intraobserver and interobserver variability. It can sometimes be truly difficult to discern between a weak allergic reaction and an irritant reaction. A number of recent studies have investigated the dermoscopic features of patch test reactions. Homogeneous erythema is the main feature observed in patients with a positive allergic reaction, although dotted vessels, vesicles, crusts and yellow-orange areas may also provide clues. These features are somewhat similar to those observed in inflammatory conditions, such as eczema. In patients with an irritant reaction, the most common dermoscopic findings are the pore reaction pattern and perifollicular erythema. Dermoscopy could be useful for establishing a diagnosis in the case of doubtful patch test reactions.

Resumen: Las consultas monográficas de dermatoscopia digital están dirigidas a la población con alto riesgo de melanoma. Conocer los motivos de derivación a estas consultas, así como las características epidemiológicas de los pacientes permite optimizar los recursos sanitarios, y determinar qué pacientes se benefician más de esta técnica. Se analizaron los motivos de derivación y las características epidemiológicas de 413 pacientes atendidos en una consulta monográfica de dermatoscopia en un período de 10 años, y que presentaban al menos un criterio de alto riesgo de melanoma. También se analizó el número necesario de extirpaciones (NNE) por cada melanoma diagnosticado, sus características histológicas y las variables no ambientales se asociaron con su diagnóstico. Los motivos de derivación más frecuentes fueron: antecedentes de melanoma previo (21,5%), cambios detectados por el paciente o su familia (20%), hallazgos clínicos y/o dermatoscópicos sospechosos de malignidad (19,4%) y antecedentes familiares de melanoma (17,4%). Setenta y seis de las 178 lesiones extirpadas fueron melanomas, obteniendo un NNE de 2,34. La edad avanzada fue el único factor de riesgo que se asoció de forma estadísticamente significativa con el desarrollo de melanoma. Abstract: Dedicated dermoscopy units assess individuals at high risk for melanoma. Understanding the reasons for referral to these units and the epidemiological profile of referred patients can help optimize health care resources and determine who benefits most from dermoscopic evaluation. We analyzed reasons for referral and epidemiological characteristics of 413 patients with at least 1 high-risk factor for melanoma seen at a dedicated dermoscopy unit over a period of 10 years. We also analyzed the number of necessary excisions (NNE) for each melanoma diagnosed, histologic features, and associations between nonenvironmental factors and diagnosis. The main reasons for referral were a past history of melanoma (21.5%), changes detected by the patient or a relative (20%), clinical and/or dermoscopic findings suggestive of malignancy (19.4%), and a family history of melanoma (17.4%). Seventy-six of the 178 excised lesions were melanomas (NNE per melanoma detected, 2.34). Older age was the only risk factor significantly associated with the development of melanoma.

Introduction and purpose: Number of new cases of skin cancers grows every year. Melanoma malignum is responsible for approximately 75 percent of death from skin cancer. General practitioners are often first medical professionals who see patients with suspicious skin lesions. Early diagnosis can benefit in improving survival rates. Dermoscopy is a useful tool for dermatological diagnostic purposes not only for dermatologists, but also for Primary Care Physicians.This review aims to analyze the role of easy 3-step Triage Amalgamated Dermoscopy Algorithm performed by family medicine physicians in skin cancers detection. Material and methods: We searched PubMed database in order to collect relevant studies about skin cancers diagnoses by family medicine physicians using dermoscopy algorithms. Results: An analysis of multiple studies shows that short Triage Amalgamated Dermoscopy Algorithm training taken by primary care physicians resulted in better diagnosis accuracy of skin cancer identification. After education about 3 benign skin lesion patterns and 7 signs of potential malignancy general practitioners reported more confidence in the diagnostic process and both sensitivity and specificity of examination increased post-training. Conclusion: Teaching Primary Care Physicians how to diagnose patients with suspicious skin lesions using Triage Amalgamated Dermoscopy Algorithm may have a beneficial effect on increasing the number of early-diagnosed skin cancers which may result in improving survival rates.

This research is proposed a design of architecture for melanoma (a kind of skin cancer) recognition by using a Convolutional Neural Network (CNN), work that will be useful for researchers in future projects in areas like biomedicine, machine learning, and others related moving forward with their studies and improving this proposal. CNN is mostly used in computer vision (a branch of artificial intelligence), applied to pattern recognition in skin moles and to determine the existence of malignant melanoma, or not, with a limited dataset. The CNN classifier designed and trained in this case was built through a couple of layers of convolution and pooling stacked to form a neural network of 6 layers followed by the fully connected to complete the architecture with an output classifier. The proposed database to train our CNN is the largest publicly collection of dermoscopic images of melanomas and other skin lesions, provided by the International Skin Imaging Collaboration (ISIC), sponsored by International Society for Digital Imaging of the Skin (ISDIS), an international effort to improve melanoma diagnosis. The purpose of this research was to design a Convolutional Neural Network with a high level of accuracy to help professionals in medicine with a melanoma diagnosis, in this case, it was possible to get accuracy up to 88.75 %.

Non-pigmented actinic keratoses are characterized by the "strawberry pattern". Pigmented actinic keratosis shows a significant dermatoscopic overlap with lentigo maligna, but the presence of pigmented scales, erythema, and prominent follicles favors its diagnosis. Bowen's disease is characterized by clustered glomerular vessels, white-yellowish scales, and brown or grey dots arranged in lines in its pigmented variant. Finally, dermoscopy allows us to detect invasive squamous cell carcinoma in its early stages and differentiate it from its precursors. Resumen: Antecedentes y objetivo: La incidencia del melanoma se ha incrementado significativamente y la forma más efectiva para disminuir su mortalidad es el diagnóstico precoz. La dermatoscopia aumenta la sensibilidad en el diagnóstico del melanoma, y por medio del análisis de las estructuras dermatoscópicas es posible estimar su grosor. Nuestro objetivo fue analizar la influencia del Breslow en las características dermatoscópicas del melanoma. Materiales y métodos: Estudio observacional de corte transversal. Se incluyeron pacientes con melanoma confirmado histológicamente y una imagen dermatoscópica del mismo. Se dividieron en tres grupos, melanoma in situ, melanoma fino (< 1 mm de Breslow) y melanoma grueso (≥ 1 mm de Breslow), y se analizaron el sexo, la edad, la localización, las características histológicas y las características dermatoscópicas. Resultados: Se analizaron 215 pacientes, 88 con melanoma in situ, 73 con melanoma fino y 54 con melanoma grueso. Las estructuras dermatoscópicas que incrementaron su frecuencia a medida que aumentó el Breslow del melanoma fueron el velo azul blanquecino (p < 0,001), las estructuras blanco brillantes (p < 0,001) y las áreas rojo lechosas (p < 0,003). Por otro lado, las líneas anguladas disminuyeron su frecuencia a medida que se incrementó el Breslow (p < 0,002). Conclusiones: La evaluación dermatoscópica tiene un importante rol, no solo en la precisión diagnóstica de las lesiones pigmentadas, sino también en ayudarnos a estimar el grosor preoperatorio del melanoma. Abstract: Background and objective: The incidence of melanoma has increased significantly, and early diagnosis is the most effective way to reduce associated deaths. Dermoscopy increases diagnostic accuracy in melanoma and analysis of dermoscopic structures can help in the estimation of tumor thickness. The aim of this study was to analyze the influence of Breslow thickness on the dermoscopic characteristics of melanoma. Material and methods: Observational, cross-sectional study of patients with histologically confirmed melanoma and dermoscopic images of the tumor. The patients were divided into three groups: melanoma in situ, thin melanoma (≥ 1 mm Breslow thickness), and thick melanoma (≥ 1 mm Breslow thickness). Age, sex, tumor location, and histologic and dermoscopic characteristics were analyzed in all cases. Results: We studied 215 patients: 88 with melanoma in situ, 73 with thin melanoma, and 54 with thick melanoma. The frequency of the following dermoscopic features increased with increasing Breslow thickness: the blue-white veil (p < 0.001), white shiny structures (p < 0.001), and milky-red areas (p < 0.003). Angulated lines, by contrast, became less common with increasing thickness (p < 0.002). Conclusions: Dermoscopy not only improves diagnostic accuracy for pigmented lesions but also helps in the preoperative assessment of Breslow thickness in melanoma.

Resumen: Objetivo: Reportar un caso de esporotricosis linfocutánea contraída en Chile y realizar una revisión de la literatura con énfasis en los hallazgos dermatoscópicos de las lesiones y en los casos reportados en el país. Caso Clínico: Paciente adulto mayor con lesiones nodulares ulceradas siguiendo trayecto linfático en extremidad superior derecha. Resultados: Estudio micológico confirmó la presencia de Sporothrix schenckii. Se inició tratamiento con antifúngico oral con buena respuesta terapéutica. Conclusiones: Existe un escaso número de reporte de esporotricosis linfocutánea contraída en Chile con confirmación micológica. Este es el único caso chileno con descripción dermatoscópica de las lesiones. Summary: Objective: To report a case of lymphocutaneous sporotrichosis adquired in Chile and provide a review of the literature with emphasis on dermoscopic findings and previous cases reported in the country. Clinical Case: An elderly patient presented with ulcerated nodular lesions following lymphatic vessels in his right upper limb. Results: Mycological work-up confirmed the presence of Sporothrix schenkii. Oral antifungal treatment was initiated with good therapeutic response. Conclusion: Few case reports of lymphocutaneous sporotricosis adquired in Chile can be found in literature. Until now this is the only chilean case with dermoscopic description of the lesions.

Resumen: Imiquimod tópico ha sido utilizado como monoterapia o tratamiento adyuvante fuera de indicación para el lentigo maligno (LM). Nuestro objetivo es describir las modalidades de tratamiento, los resultados clínicos y el manejo de la recidiva en los pacientes que reciben imiquimod para lentigo maligno.Se incluyó en este estudio a los pacientes de nuestra unidad con lentigo maligno o lentigo maligno melanoma tratados con imiquimod 5% en régimen de monoterapia o junto con cirugía.Se seleccionaron 14 casos (el 85,7% de lentigo maligno y el 14,3% de lentigo maligno melanoma). Ocho pacientes (57,1%) recibieron imiquimod sin cirugía, y seis (42,9%) fueron sometidos a resección antes de iniciar el tratamiento. Durante el periodo de seguimiento, reapareció la pigmentación en seis pacientes (cuatro con hiperpigmentación postinflamatoria y dos recidivas). Las recidivas fueron tratadas con un margen de resección muy estrecho (1 mm) y retratamiento con imiquimod 5%.Todas las modalidades de imiquimod reflejaron buena tolerancia de efectos secundarios y bajas tasas de recidiva. Imiquimod parece ser una opción muy versátil para tratar LM en candidatos idóneos. Abstract: Topical imiquimod has been used off-label as monotherapy or adjuvant treatment for lentigo maligna. Our aim is to describe treatment modalities, clinical outcomes, and management of recurrence in patients receiving imiquimod for lentigo maligna.Patients from our unit with lentigo maligna or lentigo maligna melanoma treated with imiquimod 5% as monotherapy or in combination with surgery were included in this study.Fourteen cases were recruited (85.7% lentigo maligna and 14.3% lentigo maligna melanoma). Eight patients (57.1%) received imiquimod without surgery, and six (42.9%) underwent narrow excision before beginning treatment. During the follow-up period, pigmentation reappeared in 6 patients (4 postinflammatory hyperpigmentation and 2 relapses). Relapses were managed with very narrow excision (1 mm margin) and retreatment with imiquimod 5%.All imiquimod modalities showed well-tolerated side effects and low recurrence rates, with long periods of follow-up. Imiquimod appears to be a versatile option for treating LM in suitable candidates.

Resumen: La foliculitis por Malassezia es una entidad que no está debidamente reconocida y que afecta normalmente a la cara y el tronco superior. El cuadro clínico remeda el acné vulgar, siendo a veces difícil su diagnóstico. Normalmente se realiza un examen con hidróxido de potasio al 10% para confirmar el mismo. El objetivo de este estudio fue describir las características dermatoscópicas de la foliculitis por Malassezia, incluyéndose en el mismo a los pacientes diagnosticados clínicamente y confirmados mediante examen con hidróxido de potasio al 10%. La dermatoscopia fue realizada con un videodermatoscopio (Dino-Lite AM413ZT; Polarizer) en la lesión más representativa. Se incluyó a un total de 45 pacientes (V:M = 1:0,8). Todos los pacientes tenían lesiones monomórficas papulopustulosas. El 64,4% de los pacientes presentó prurito. La dermatoscopia reveló foliculocentricidad (100%), eritema circundante perilesional (100%), vasos punteados/lineales/tortuosos (88,9%), escamas de color gris parduzco (77,8%), hipopigmentación del folículo piloso (64,4%), pelos en espiral/enrollados (57,8%) y rotura del pelo (13,3%).En conclusión, la dermatoscopia muestra las características típicas de la foliculitis por Malassezia, pudiendo servir de ayuda en consulta como herramienta para la identificación de esta entidad. Abstract: Malassezia folliculitis is an under-recognized entity commonly affecting the face and upper trunk. Clinical picture mimics acne vulgaris and diagnosis is challenging at times. Ten percent potassium hydroxide examination is usually performed to confirm the diagnosis. This study sought to describe the dermoscopic features in Malassezia folliculitis. Patients diagnosed clinically with Malassezia folliculitis and confirmed by 10% potassium hydroxide preparation were included in the study. Dermoscopy was performed with a videodermatoscope (Dino-Lite AM413ZT; Polarizer) from the most representative lesion. A total of 45 patients (M:F = 1:0.8) were recruited. All patients had monomorphic papulo-pustular lesions. Itching was present in 64.4% patients. Dermoscopy reveled folliculocentricity (100%), perilesional background erythema (100%), dotted/linear/tortous vessels (88.9%), dirty white scaling (77.8%), hypopigmentation of hair follicle (64.4%), coiled/looped hairs (57.8%) and broken hairs (13.3%).In conclusion, dermoscopy shows typical features in Malassezia folliculitis and can serve as a office-based tool for identification of this entity.

Molluscum contagiosum (MC) is a common viral infection in children, immunocompromised, and sexually active adults. Its usual clinical presentation is 2-5 mm, whitish or skin-colored papules, with a shiny surface and central umbilication, generally clustered and randomly distributed over the skin surface. Dermoscopy reveals yellowish-white polylobulated structures with peripheral telangiectasia. Diagnosis is usually clinical supported by dermoscopy. However, in some cases, inflammatory manifestations can be associated with this infection and can mimic other dermatological conditions, making the diagnosis difficult and leading to unnecessary treatments. The objective of this article is to describe the main skin reactions associated with MC infection in order to provide a diagnostic and initial management tool for clinicians dealing with these conditions. Reported manifestations include the BOTE sign, perilesional eczema, Gianotti-Crosti syndrome-like reaction, ID reaction, erythema annulare centrifugum, erythema multiforme, folliculitis, white halo, and atypical manifestations (giant, disseminated, necrotic, polypoidal, and nodular lesions, pseudocysts, abscesses). In pediatric patients with the clinical manifestations described above, infection by molluscum contagiosum pox virus should be considered among the differential diagnoses, and referral to a dermatologist should be made in selected cases.

Dermoscopy is a non invasive technique which can be a fantastic diagnostic tool in dermatology. Here we present a series of six cases where a dermoscopy threw light on the clinical diagnosis of lesions. Case 1A, 1B and 1C presented with singular papular lesions which led us to differential diagnosis of viral infections, seborrheic keratoses etc. However, on dermoscopy all three cases exhibited a white mass with a keratin plug and peripheral hair pin blood vessels. Our second case presented with a nodule on the nose leading us to a diagnosis of an intradermal nevus. Dermoscopy of this lesion demonstrated the presence of arborizing vessels thus indicating something far more sinister than a nevus. Case 3 was a female who presented with a small asymptomatic lobulated mass leading us to consider either seborrheic keratosis or dermatofibroma as diagnosis. But the dermoscopy correctly showed the presence of lacunae and a whitish veil which established the final diagnosis. Our final case presented with a greyish nodule suggestive of dermatofibroma. On dermoscopy, the presence of cerebriform structures and comedo like openings on the background of a white scar like patch highlighted the superimposition of another lesion on the existing dermatofibroma. Our cases prove the utility of dermoscopy as a diagnostic tool in clinical dermatology.

Dermoscopy has certainly had a profound impact on the clinical diagnosis and management of both pigmented and nonpigmented skin lesions. Over the years, there has been much emphasis on the use of dermoscopy for the diagnosis of onychopathies. Traumatic onycholysis has been considered as one of the most common onychopathies. Although history and clinical examination remain the most crucial steps, dermoscopy apppears to be a rapid and effective tool in establishing the diagnosis of traumatic onycholysis. Here, we present two cases of traumatic onycholysis with classical and novel dermoscopic findings.

Lichen sclerosus (LS), also known as lichen sclerosus et atrophicus is a chronic inflammatory dermatosis of unknown aetiology. It has both genital and extragenital presentations, nevertheless genital forms significantly outnumber extragenital LS. Dermoscopy is noninvasive diagnostic tool traditionally employed in pigmented lesions, however its usefulness in inflammatory skin conditions is becoming continuously more meaningful. Although the clinical diagnosis of fully developed LS rarely causes difficulties, unusual presentations require differentiation from the diseases such as lichen planus, morphea, extramammary Paget disease, SCC and others. In these cases histopathology contributes to the diagnosis. Studies on the use of dermoscopy in LS are sparse, nevertheless some dermoscopic features of LS has been described.

Background and Objective: Atypical nevus syndrome has been described as one of the main risk factors for melanoma. The aim of this study was to analyze dermoscopic changes observed in melanocytic lesions over a follow-up period of 5 years in patients with atypical nevus syndrome., Material and Methods: We conducted a retrospective follow-up study of a cohort of patients seen at a specialized skin cancer and digital body mapping clinic in Medellin, Colombia, between January 2017 and December 2022. We analyzed the dermoscopic changes observed during this period and explored their association with newly diagnosed melanoma., Results: A total of 368 patients (187 women) with a median (interquartile range) age of 43 (37-51) years were included. The dermoscopic features observed at 5 years were an atypical network (222 patients, 60.3%), asymmetric globules (163, 44.2%), white-gray regression areas (105, 28.5%), lesion regression (72, 19.5%), a negative pigment network (59, 16%), asymmetric eccentric pigmentation (28, 7.6%), asymmetric projections (21, 5.7%), and asymmetric vascular patterns (8, 2.1%). Melanoma was diagnosed in 12.2% of patients during follow-up. Features significantly associated with a shorter time to melanoma onset were grayish-white areas (P <.001), asymmetric globules (P=.011), asymmetric eccentric pigmentation (P=.047), and a negative pigment network (P=.001)., Conclusions: The main dermoscopic features of melanocytic lesions in patients with atypical nevus syndrome associated with progression to melanoma were grayish-white areas, asymmetric globules, asymmetric spots, and a negative pigment network., (Copyright © 2023 AEDV. Confluent and reticulated papillomatosis (CRP) is an uncommon, progressive, distinctive dermatosis. It is characterized by persistent brown, scaly macules, papules, patches and plaques. It appears as tinea versicolor or acanthosis nigricans resulting in delay in the diagnosis. Hence a diagnostic tool which could help to diagnose this condition is dermoscopy. We reported a 24 year old male with skin lesions on upper back, neck and shoulders. Dermoscopic examination demonstrated fissures and ridges. Whitish structures were present on the ridges which are surrounded by grayish globules. These globules were lined by brown lines at the periphery. Fissures were brown in color. Global appearance was ‘crocodile skin-like’ pattern. Ridges and furrows in dermoscopy of CRP correspond to rete ridges in the histology. Whitish structures were there around follicular ostia which represent hyperkeratosis and acanthosis of follicular epidermis. Authors also observed sparse hairs in the involved area and V-sign under dermoscopy. Hence, pathogenesis of CRP probably revolves around hair follicles. CRP demonstrates specific dermoscopic patterns which correlate well with histopathologic changes. Therefore, authors propose that these patterns would aide in the diagnosis of CRP.

Early detection of malignant skin tumors, particularly malignant melanoma in childhood, which is most malignant of all, is of utmost importance for the prevention, treatment and outcome of the disease. If an observed lesion is characterized as melanocytic, the second step can be performed in the so called “two-step dermoscopy” algorithm, which includes the differentiation between a benign pigmented lesion (nevus) and a malignant lesion (melanoma). Gigantic nevi, as a subgroup of large congenital nevi, may pose a huge psycho-social burden for children and their parents, primarily because of potential complications. Melanoma can develop in healthy children but also in those with identified genetic disorder or immune diseases. Dermoscopy, at the same time, has reduced the number of unnecessary biopsies and excisions of benign melanocytic lesions in childhood.

Congenital melanocytic nevi (CMN) are visible pigmented lesions in the skin that are present at birth. CMN are benign malformations resulting from defective development of melanocyte precursors in the embryo. Six MMs from six patients were analyzed by clinical and dermoscopic examination. Of the patients, 33.3% were female (N = 2) and 66.6% were male (N = 4). Of the MMs, four (66.6%) were superficial spreading MM and two (33.3 %) were in situ MM. A reticular pattern was present in the MMs of three patients (50%), a homogeneous pattern was present in the other patients (50%) at the base of the MMs. Superficial spreading melanomas and in situ melanomas with atypical dots and globules and a blue-white veil were the most common dermoscopic features of MMs found in CMN.

Also called as physiologic anemic macules, Bier spots are small, hypopigmented irregularly shaped macules against a background of diffuse erythema, which creates an appearance of speckled vascular mottling of the skin. Bier spots most commonly appear on distal portions of the limbs though there are case reports describing diffuse involvement, which also affect trunk and mucous membranes of the patient. Although the exact pathophysiological mechanisms underlying Bier spots still need to be elucidated, Bier spots have been suggested to be a vascular anomaly caused by vasoconstriction of small vessels. In addition, several diseases have been proposed to be associated with Bier spots, including scleroderma renal crisis, cryoglobulinemia, Peutz-Jeghers syndrome, alopecia areata and hypoplasia of the aorta, although it has not been shown whether these associations are casual or coincidental. The clinical presentation of Bier spots is quite typical. These tiny whitish macules easily become prominent when the affected limb is placed in a dependent position and fade away when the limb is raised. Here we report a case of Bier spots in a 32-year-old male patient with characteristical clinical manifestations.

Background and Objective: Clear cell acanthoma (CCA) is an uncommon lesion histologically characterized by the presence of epidermal acanthosis with keratinocytes containing clear cytoplasm. Although many single cases of CCA have been described, few case series have been published. The aim of this study was to describe the clinical characteristics of CCA in our practice setting., Material and Methods: Retrospective study of patients diagnosed with CCA at Hospital Universitario de Bellvitge in Barcelona, Spain, between 1995 and 2021. We conducted a chart review to record age, sex, number and location of lesions, diameter, time since onset, clinical characteristics, suspected clinical diagnosis, and treatment., Results: Seventy patients (30 women and 40 men) with a mean (SD) age of 62 (13) years were diagnosed with CCA during the study period. Median (interquartile range) time since onset was 2 (4) years and median lesion diameter was 6 (5) mm. One woman had multiple lesions. Lesions were located on the lower extremities in 57 patients (81%), the posterior aspect of the trunk in 8 (11%), the anterior aspect of the trunk in 4 (5%), and the upper extremities in 1 (1%). CCA was clinically suspected in 40% of patients seen by dermatologists., Conclusions: CCA presents as an erythematous, dome-shaped lesion with pinpoint vessels and an epidermal collarette. The accuracy of clinical diagnosis has improved relative to earlier series, possibly due to a better clinical understanding of this lesion and a greater use of dermoscopy., (Copyright © 2023 AEDV. The name is derived from French phrase peau chagrinee which is usually found on lower back, buttock and thigh. The major manifestations of Tuberous sclerosis include skin lesions in more than 95%, mental retardation in approximately 50%, autism, seizures in approximately 85%. The incidence at birth is estimated to be 1 in 5800. We report case of shagreen patch in a 27 year female which is present since birth. However there is no history of seizures or consanginous marriage in our case. Associated features are naevus comedonicus and naevus collagenosis, facial angiofibroma. Shagreen patch are present in mandibular area of face. Although, diagnosis is easy, it can be mistaken for inflammatory verrucous epidermal nevus, plaques of other inflammatory skin conditions. Diagnosis is usually on clinical background. Sometimes biopsy is necessary to confirm the diagnosis. Dermoscopy, a non-invasive, in vivo technique for the microscopic examination of pigmented skin lesions, has the potential to improve the diagnostic accuracy. Dermoscopy of Shagreen patch showed reddish-brown strands with white dots giving a cobblestone appearance It can be utilized as a diagnostic aide in the diagnosis of Shagreen patch. Authors evaluated the dermoscopic patterns of Shagreen patch and hence, it is useful in diagnosis.

Apocrine hydrocystoma (AH) is a translucent, skin-colored to bluish dome shaped cyst on the face. AH mimics basal cell carcinoma (BCC), blue nevus, amelanotic melanoma requiring histopathological confirmation. Dermoscopy shows specific patterns in skin conditions. Dermoscopy of AH is not described in the literature. Authors evaluated dermoscopic patterns in AH and observed characteristic patterns corresponding to histological features. To the best of our knowledge, it is a first report in literature.

A 35-year-old man was referred to our hospital with a 3 months history of itchy cutaneous eruption on the trunk and asymptomatic cutaneous eruption on both groins. Physical examination revealed several, purplish-brown, scaly papules on the trunk and well-circumscribed, brown patchs in a linear distribution, on the bilateral inguinal regions. Dermoscopic examination of papules on the trunk revealed white crossing lines surrounded by brown dots; dermoscopic examination of patchs on groins revealed gray-brown dots and globules. According to histological, dermoscopical and clinical changes, the diagnoses of classical lichen planus (LP) for the lesions on the trunk; and lichen planus pigmentosus inversus (LPPI) for the lesions on the inguinal regions were made. Inhere we have described a rare case of LPPI and classical LP with dermoscopic features; and we suggest that LPPI is a variant of classical LP.

Lentigo maligna is an in situ cutaneous melanoma that arises in sun-damaged skin. Its most common presentation is a progressive, slow-growing, irregularly pigmented spot on the face of older patients. Although the exact percentage of Lentigo maligna that progresses to invasive tumors is unknown, it is thought to lie between 2% and 5%. Both the clinical and histologic diagnosis of Lentigo maligna can be challenging, especially in patients with early-stage or atypical disease. Treatment also holds challenges, because lesions are located in highly visible areas and are often large. Surgery can thus compromise cosmetic and sometimes functional outcomes. We review clinical and histopathological findings that can facilitate the diagnosis of Lentigo maligna. We also examine treatment options, with a focus on surgery., (Copyright © 2023 AEDV. Fixed drug eruption (FDE) usually appears as a solitary or a small number of pruritic, well circumscribed, erythematous macules that evolve into edematous plaques; these lesions typically resolve after discontinuation of the offending drug, leaving hyperpigmentation at the site of lesions. Fixed drug eruption has been mentioned previously as a disease model for elucidating the mechanism of how skin inflammation is caused by skin-resident T cells, a multistep process that results in eventual tissue damage. In this article we discuss the utility of dermoscopy as an additional tool which gives significant information aiding us to infer these complex processes seen in FDE and thus to confirm the diagnosis

Dermoscopy is valid, noninvasive technique for the visualization of different morphological structures within pigmented melanocytic skin lesion. In clinical practice, positive personal history is considered to be as an indication of increased melanoma risk. The thickness ofmelanoma (Breslow Index) is the most important independent predicting factor of survival for stage I patients.We aimed to present a two cases of head-melanoma and discuss prominent clinical and dermoscopic features across the spectrum of“hidden-melanoma” and differential diagnosis.

Interpreting patch test reactions is not easy. It requires experience and is characterized by high intraobserver and interobserver variability. It can sometimes be truly difficult to discern between a weak allergic reaction and an irritant reaction. A number of recent studies have investigated the dermoscopic features of patch test reactions. Homogeneous erythema is the main feature observed in patients with a positive allergic reaction, although dotted vessels, vesicles, crusts and yellow-orange areas may also provide clues. These features are somewhat similar to those observed in inflammatory conditions, such as eczema. In patients with an irritant reaction, the most common dermoscopic findings are the pore reaction pattern and perifollicular erythema. We aimed to desribed clinically widespread itchy rach vs scabies submited to dermoscopic examination. We confirmed diagnosis of scabies clinically and dermoscopically.
