Abstract

Objective: To estimate the hospital cost of a sample of cases treated in seven hospitals of the National Health System in several Spanish cities., Method: Study based on 78 cases of occupational disease recognized by the social security, and previously treated in hospitals in Badalona, Barcelona, Ferrol, Gijón, Girona, Madrid and Vigo between 2017 and 2019., Results: The healthcare activity generated by these hospitals to attend these processes involved a total cost of 282,927€., Conclusions: It is urgent to improve the coordination between the two public health systems, the social security health care system and the National Health System., (Copyright © 2021 SESPAS. Abstract

A case of aortic valve endocarditis caused by Coxiella burnetii and operated on with success is reported. The patient is doing well at 18 months follow up. Diagnosis of Q-fever endocarditis was made by high antibodies against phase I Coxiella burnetii antigens titration and by demonstration of aortic valvular vegetations by bidimensional echocardiography. Our patient suffered emergency aortic valve substitution due to acute hemodynamic failure and started a long-term treatment with doxycycline and rifampicin. Some interesting aspects about the diagnosis and treatment of this patient are reviewed because long-term follow-up and serological controls are still rare in the literature.

Abstract

Testicular feminization is a syndrome due to androgen insensitivity. It occurs in a complete and incomplete form. The case of a 17 years old girl with complete androgen insensitivity syndrome is reported. Patient came to medical attention because of primary amenorrhea as the only pathology, with normal external, female genitalia. Diagnosis, findings, management and a five months follow up, are described, as well as a review of this subject.

Abstract

Three cases of primary hypothyroidism with optic chiasm compression due to pituitary hyperplasia are reported. Patients consulted for amenorrhea, galactorrhea and visual disturbances. The study of these patients disclosed a chiasm syndrome, persistent hyperprolactinemia, decreased thyroxine and triiodothyronine associated with elevated thyrotropin stimulating hormone in blood. Computerized tomography scan revealed the presence of pituitary enlargement. Treatment with thyroid hormones resulted in total resolution of clinical features and disappearance of tomographic abnormalities.

Abstract

A case of cutaneous alternariasis with appeared during the treatment with corticosteroids in a patient having a nephrotic syndrome, is presented. The diagnosis was verified by histological and mycological examination, isolating Alternaria alternata. The etiopathogenic mechanisms and the clinical and histological aspects of the lesions are discussed. Good results of treatment with ketoconazole and topic ciclopirox are reported.
