Objective: Oral health is an essential component of the general well-being and quality of life of the population. In Spain, access to dental services and emergency dental care for civilians is relatively straightforward. However, this is not the case for military personnel, especially those deployed on Navy ships that lack dental capabilities and in isolated areas where access to specialized healthcare services is complex and risky. The aim of this study was to evaluate the incidence and management of dental pathology among Spanish military personnel aboard the LHD 61 Juan Carlos I during its first International Peacekeeping Operation., Methods: A descriptive observational study was conducted on the accumulated incidences of dental pathology and treatments performed between May 1 and June 26, 2018. The population sample consisted of 448 Spanish military personnel aboard the ship. The data were obtained from the databases of the dental activity logbook and the military statistical and health data management systems, which serve to assess the actual health status of the force., Results: A total of 70 patients were treated for dental problems, with gingivitis and caries being the most prevalent conditions, accounting for 23.07% and 18.68% of cases, respectively. The most common treatments performed were dental reconstructions/fillings, making up 40% of the total, and tartar removal, accounting for 21%., Conclusions: Understanding the incidence of dental pathologies on Spanish Navy ships allows us to establish the training needs of the onboard medical personnel, as well as determine the material resources that should be provided to naval units. Background: The aim of this study was to determine incidence of COVID-19 in relationship to geographical distribution among Spain., Method: Cluster analysis taking into consideration the incidence of COVID-19 in the provinces and autonomous cities of Spain in each of the first six waves of the pandemic., Results: All the provinces of the Canary Islands, Catalonia and Andalusia form independent clusters. In Comunidad Valenciana, Galicia, País Vasco and Aragón two out of three provinces (three out of four in Galicia) were in the same cluster, with no other provinces., Discussion: The incidence of COVID-19 in Spain in the first six waves forms clusters that reproduce the territorial division of Spain into autonomous communities. Although this could be explained by greater mobility within a community, it cannot be ruled out that this distribution is due to differences in screening, diagnosis, registration or reporting of COVID-19 cases., (Copyright © 2023 FECA. Introduction: Resident physicians' proper use of nutritional support and knowledge about Clinical Nutrition is essential to ensuring that their patients receive suitable nutritional care., Materials and Method: An online survey was sent to resident physicians at our hospital in specialisations with hospital beds. The survey featured 20 multiple-choice questions scored from 1 to 10 (1 being "completely disagree" and 10 being "completely agree") across the following themes: nutritional assessment, diets, oral nutritional supplements, enteral nutrition and perception of the Nutrition Unit., Results: The survey was completed by 69% of resident physicians in medical specialisations and 70% of those in surgical specialisations. Overall, the average survey score was 6.28, with higher scores among medical residents than surgical residents (6.86 versus 5.38; p < 0.001), especially in the sections on nutritional assessment, diets and oral nutritional supplements. The respondents had a positive perception of the Nutrition Unit (mean score 7.6)., Discussion: Residents in medical specialisations afford greater importance to their patients' nutrition than residents in surgical specialisations, although in both groups the average score was rather low. There is much room for improvement in the training in Clinical Nutrition of this group, and it is important to include topics in Clinical Nutrition in training programmes for all residents in hospital specialisations., (Copyright © 2021 SEEN y SED. Introduction: A case report is a scientific article describing one or more patients with unusual clinical presentations. In recent years, the number of case reports in publications has decreased. In this study, we analyze the publication of case reports in journals of Endocrinology during the years 2010, 2015 and 2019., Materials and Methods: The Pubmed web was browsed for clinical journals of Endocrinology, those published in English and/or Spanish being selected, and the relevant variables analyzed., Results: Of 84 analyzed journals, 51 accepted cases for publication, 29 did not, and 4 did so only in exceptional cases. In 2010, 11,754 articles were published, of which 709 were clinical cases (6.9% of the total); in 2015, a total of 14,594 articles of which 655 were clinical cases (5.8% of the total); and in 2019 a total of 14,110 articles, of which 472 were clinical cases (4.6% of the total). In journals demanding payment for the publishing of clinical cases, case reports represented 9% of all articles, and in free journals, 3% (P<.05)., Conclusion: There has been a decline in publication of case reports in journals of Endocrinology in recent years, both in absolute and relative terms. Even though the cases described by these reports are, by definition, exceptional, the decline of their publication implies a significant loss of scientific information and clinical knowledge regarding certain pathologies., (Copyright © 2021 SEEN y SED. Introduction: Hypoglycemia is the major limiting factor in the glycemic management of type 1 diabetes. Severe hypoglycemia puts patients at risk of injury and death. Recurrent hypoglycemia leads to impaired awareness of hypoglycemia and this increases the risk of severe hypoglycemia. Recent studies have reported rates for severe hypoglycemia of 35% in type 1 diabetic patients., Objectives: To assess the prevalence of severe hypoglycemia in type 1 diabetes mellitus patients and to evaluate the relationship between this and impaired awareness of hypoglycemia according to the Clarke test., Patients and Methods: The following data were collected from a cohort of type 1 diabetic patients: age, gender, duration of type 1 diabetes, treatment (multiple daily insulin injection or continuous subcutaneous insulin infusion), glycemia self-control, HbA1c, episodes of severe hypoglycemia and impaired awareness of hypoglycemia., Results: Of the participants, 39.8% had had at least one episode of severe hypoglycemia (in the previous 6 months), 11.4% with loss of consciousness (in the previous 12 months). According to the Clark test, 40.9% had impaired awareness of hypoglycemia. Older age and longer duration of diabetes were associated with a higher prevalence of severe hypoglycemia with unconsciousness; older age and a lower level of HbA1c were associated with impaired awareness of hypoglycemia., Conclusions: Our study allows us to confirm the high rate of severe hypoglycemia and impaired awareness of hypoglycemia in patients with type 1 diabetes., (Copyright © 2020 SEEN y SED. Introduction: In Spain, the system used to select a medical specialty is the MIR (internal resident physician) exam. The MIR selection number may reflect the interest in a given specialty. Our study objective was to confirm the increase in the selection number and to analyze possible factors influencing the decision., Material and Method: To analyze change over time in the MIR number with which this specialty is chosen and to compare it with other related specialties, as well as the reasons why it is preferred using an anonymous survey to 108 MIRs of endocrinology., Results: The average number of MIR for Endocrinology and Nutrition has gradually increased to 2336 in year 2018, a trend that coincides with an increase in the number of places offered but is more marked as compared to other medical specialties. Respondents weighed different factors when choosing specialty. When asked about the most positive aspects of the specialty, the highest rated was that day to day activity was "less intense". The most commonly mentioned negative aspect was the low number of techniques. When asked if the specialty had met their expectations, respondents gave an average score of 8.7, although the perceived prestige of the specialty scored only 6.7 points., Conclusions: There is an obvious deterioration of the MIR selection number of our specialty that it is not so marked in other specialties., (Copyright © 2019 SEEN y SED. Caso clínico: presentamos una paciente de 33 años con anorexia nerviosa de 15 años de evolución con uno de los pocos casos reportados de fallo hepático agudo severo secundario a la desnutrición.Discusión: tras el soporte nutricional protocolizado para evitar el síndrome de realimentación y un adecuado manejo multidisciplinar, la paciente evoluciona favorablemente logrando normalizar los electrolitos, la función hepática y las alteraciones en la coagulación.

Background: Different aspects of the doctor-patient relationship were associated with satisfaction and other outcomes of the consultation. However, measurements of the perception of communication quality are often limited by the "ceiling effect". To evaluate the relationship and differences between three ways to obtain information regarding patient satisfaction and perception of unmet needs in communication aspects with your doctor., Methods: An observational descriptive study was design. Patient perception regarding four communicational domains when attending family doctors measured by means of three different type of questions., Participation: 658 (91%) patients attending the consultation of 97 physicians in 58 Health Centres. 401 (62%) of them were females. Average age 52 years (TD:17,4; CI95%: 50,6-53,3 years), 401 females (62%: CI95%: 58,7-66,4%). In close ended questions, patients declared high rates of satisfaction for all the communicative domains studied: relationship 99,7% (CI95%: 98,8-99,9%), explaining reasons for consultation (CI95%: 94,4-97,7%), information regarding causes 96,2% (CI95%: 89,9- 94,3%), and therapeutic plan 97,9% (CI95%:96,7-99,1%). Patients declared as insatisfied gave high number of suggestions or showed more discrepancy of frequency (p<0.05); nevertheless, up to 28,9% of satisfied patients made suggestions for improving communication (p<0,001)., Conclusions: [corrected] Designing open-ended questions where patient can make suggestions, seems to be a good method for detecting unmet needs in doctor patient communication that close-ended question about satisfaction or those exploring possible discrepancy on the frequency of some behaviours.

Aims: To assess patient preferences their satisfaction level and their participation in decision making with nurses., Methods: Cross-sectional and mixed quantitative-qualitative study carried out in people attending the nursing services of 9 Health Centres in Andalusia. Patients were interviewed immediately after receiving nursing treatment using two different questionnaires for assessing their opinions, satisfaction and perception of involvement in the decisional process. A descriptive analysis using the χ(2) test (P<.05) was performed to assess the differences among close-ended and open-ended questions. Qualitative analysis: Open-ended questions were grouped into categories by a process involving three researchers independently., Results: A total of 235 patients took part, of whom 59% (138) preferred a collaborative role with the nurse when making decisions. In the closed questions, 96.2% (228) of the surveyed patients declared to be satisfied or very satisfied with the decision making process; nevertheless 17.4% (41) made specific suggestions for improving this process. For them the main improvement areas were related to: general communication skills or a more specific one such as: strategies for helping them make decisions, reaching common ground or giving advice., Conclusions: Nurses should be aware that most patients wish to be involved in decision making and in clinical practice this participation can be improved by obtaining specific communicational skills. Introduction and Objective: There is a significant lack of scientific evidence on the role of SAHS in the elderly despite the increasing ageing of the population. The objective of the present study is to analyse the current healthcare situation in Spain on the diagnosis and treatment of sleep apnea in the population ≥65 years and its progress over the last few years., Material and Method: Cross-sectional study. Healthcare information was collected on the diagnosis and treatment of patients of both sexes and ≥65 years suspected with having SAHS and referred to sleep units (SU) between 2002 and 2008., Results: There were 51,229 sleep studies performed in 16 SU. Of these, 24.3% were performed on subjects ≥ 65 years (64.9% males), of which 71.5% had an AHI (apnoea-hypopnoea index) >10 (68.6% treated with CPAP). There were no differences over time as regards mean age, mean AHI or percentage of studies done. A significant decrease was observed in the number of CPAP prescribed to males ≥65 years from 2002 to 2005 (p=0.01) which subsequently increased up to 2008 (p=0.01). This phenomenon was not observed in women ≥65 years., Conclusion: Despite the lack of evidence on the subject, healthcare activity due to suspected SAHS in the elderly population is intense, therefore it should be a priority to start clinical studies that may be able to answer key questions on the diagnosis and treatment of SAHS in this age group., (Copyright © 2009 SEPAR. Objective: To determine the sensitivity of plain-film radiography and computed tomography (CT) in the diagnosis of cecal volvulus., Material and Methods: We reviewed the clinical histories of 11 patients diagnosed with cecal volvulus at endoscopy or surgery. Two radiologists working in consensus analyzed the findings at plain-film radiography and at CT and calculated the sensitivities. The plain-film and CT studies were retrospectively classified as certain, probable, or indeterminate for cecal volvulus on the basis of the presence or absence of previously reported signs. Signs of wall suffering at CT were compared to the histologic findings., Results: The most sensitive findings at plain-film radiography were the presence of a disproportionately dilated bowel loop and a pattern of distal small bowel occlusion (91%), followed by a single air-fluid level in the cecum and collapse of the distal colon (82%). At CT, the "coffee bean" sign with a single air-fluid level and collapse of the left colon had a sensitivity of 100%. The whirl sign was present in 86%. Retrospectively, 36% of the plain-film studies and 86% of the CT studies were classified as certain for cecal volvulus. Although all cases with ischemia had signs of vascular compromise on CT, no significant correlation was observed between these variables., Conclusions: The plain-film signs reported for cecal volvulus enable a certain diagnosis in a third of all cases; the CT signs enable a certain diagnosis in most cases. The authors analyze the influence of smoking on the validity of measurements of arterial saturation using percutaneous oxygen-hemoglobin blood gas tests. The authors conclude that these methods can yield elevated oxygen saturation values in smokers.

Abdominal symptoms frequently affect patients with AIDS. Acute abdominal pain is a diagnostic challenge that may require elective or urgent surgical treatment, although information about the latter is scarce. In this study we analyzed the clinical findings and follow-up of acute abdominal pain complicating patients with AIDS. In a two-year period, we collected several variables from patients with AIDS and acute abdominal pain: demographic, laboratory, clinical symptoms, initial diagnosis, surgical findings, post-surgical and histopathological diagnosis and post-surgical complications. From 232 hospitalized patients, 34 had acute abdominal pain: 32 male and 2 women (median age = 32 years; range 26 to 58 years). Twenty-two patients required surgical treatment. Eight patients had a post-surgical complication; in five of them, six surgical re-interventions were performed. Three deaths occurred in the 30-day period after surgery. Survival for patients conservatively treated was 4 months (1 to 17 months), vs. 6.5 months (1 to 20 months) in the surgically treated group. Physicians should be aware about the several diagnostic possibilities of acute abdominal pain complicating patients with AIDS. Delay of surgery in these patients may be lethal. Surgery has an important role in the integral treatment of patients with AIDS.

We report the case of an asthmatic man in whom a solitary pulmonary nodule was discovered after an episode of self-limited hemoptysis. Infection was suspected after initial response to empirical antibiotic therapy, and the pathogen was later identified to be a rare mycobacterium. The pulmonary nodule resolved without surgery after oral quinolone therapy. Mycobacterium fortuitum should be added to the list of possible causes of solitary pulmonary nodule in Spain.

We conducted a retrospective analysis of the epidemiological, etiological, clinical and radiological characteristics, and mainly of those related to the diagnosis and treatment, of all the patients with radiological criteria of solitary pulmonary nodule (SPN) studied in our environment during a period of six years (1984-1989). The incidence of SPN was 3.1% (123/3953). Among the 117 cases of filiated etiology, 83 (71%) were malignant and 34 (29%), benign thoracotomy was used as diagnostic method in 11 (32%) benign cases and in 8 (9.6%) malignant cases. Due to several causes, only 31 out of the 75 (41.3%) malignant nodules diagnosed prior to the thoracotomy underwent resection surgery. According to the selection criteria applied in our medium, a high number of the SPN were malignant; in most of these cases, the diagnosis was established without the need of thoracotomy and in less that half of them, curative surgery was attempted.

To check the possible existence of long-term pulmonary functional sequelae in patients with pneumonia due to Mycoplasma pneumoniae, 17 patients who showed evident spirometric disorders during the recovery from the acute phase of a pneumonia due to Mycoplasma pneumoniae were studied. In the acute phase of the disease, 7 patients showed a ventilatory restriction, 6 of them had an obstructive pattern while in four patients a mixed pattern was observed. After a latency period, which mean length was 39 months, it was seen that there was a return to normal values of the spirometric conventional parameters initially altered in all cases. However 5 patients (33%) who did not smoke, showed a significant decrease on the maximum flows at low volume. These results suggest that in a high percentage of adult patients, as has been previously described in children, pneumonia due to M. pneumoniae could leave as a sequelae the persistent affectation of small airway.

With the aim of testing the cost-effectiveness of diagnostic fiberbronchoscopy in pleural effusions, the results obtained in a study of 46 patients in whom pleural effusion was the only indication for this diagnostic technique was analyzed. Patients were divided into two groups based on whether they presented pleural effusion of unkown ethiology (group A) or non-localized primary metastatic effusion (group B). Two out of 28 patients in group A, and 5 out of 18 in group B were diagnosed by means of fiberbronchoscopy. It is concluded that given the good tolerance and low morbidity of this exam, the diagnostic cost-effectiveness justifies its performance in such situations. Possible modifications in the indications that could influence its utility are discussed.
