This paper seeks to find poetry in recreating, if not beauty, then aesthetic dimension in waste itself, to find perfection in imperfection itself. The underline hypothesis stems from questions such as what is the relationship between the automotive industry and the building industry? What if there exists a knowledge transfer mechanism between the two. What if the know-how of automakers be applied to the building industry? Beyond adaptive reuse of existing buildings and abandoned structures, the image of the city of Detroit as the model of industrialized American city could be reshaped by the knowledge of its memory. This manuscript provides a speculative approach to revitalize one of the most famous examples of an American industrial city. The approach attempts to highlight the importance of activating multi-industry pollination known as industrial symbiosis (IS) specifically between the automotive industry and the building industry using the city of Detroit as a case study. Ideas to test the vitality of this approach were explored and tested in a graduate architectural design studio where students were challenged with transforming the automotive industry by-products, waste streams, and flows into building products and façade systems.

Exercises for the core can be categorized as promoting core-stability, core-strength, or functionality, as these are crucial aspects of most sports activities. This study aimed to examine the effects of using core complex training (CCT), complex training (CT), and core exercise (CE) on some aspects of muscle strength and shooting performance in basketball players. The 36 participants were divided into three groups of 12 each, and then the experimental approach was applied to each group. The groups were labeled as follows: the core complex training group (N = 12; age, 18.58 ± 0.67 years; height, 178.08 ± 0.79 cm; weight, 76.42 ± 1.38 kg; training age, 7.42 ± 0.51 years); the complex training group (N = 12; age, 18.50 ± 0.52 years; height, 177.92 ± 1.31 cm; weight, 76.67 ± 1.78 kg; training age, 7.33 ± 0.49 years); and the core exercise group (n = 12; age, 18.42 ± 0.52 years; height, 177.75 ± 1.29 cm; weight, 76.58 ± 1.38 kg; training age, 7.42 ± 0.67 years). For ten weeks, each of the three groups participated in three training sessions every week. This study investigates the impact of core complex training on basketball shooting ability and muscle strength. The eight-week program, consisting of weight training, plyometric exercises, and core exercises, yielded improvements in muscle strength and shooting accuracy. In tests of muscular strength and basketball shooting ability, the CCT group outperformed the CE and CT groups. The F value varied from 3.75 to 58.77, which are function values with a p < 0.05 significance level. The core complex training group exhibited superior muscle strength to that of both the complex training group and the core exercise group, in some areas. This is shown in the results of the javelin quadrathlon medicine ball test, the core muscle strength and stability test, the sit-up abdomen test, the sit-up back test, the standing long jump test, the Sargent jump test, and the shooting test (p < 0.005). Due to the effect of the core complex training program on improving performance efficiency and muscle strength, which affects the results of matches, we have recommended using the proven basic strength training program at other age stages, with the objective of including the concept, importance, and design of compound basic strength training in training programs used by basketball coaches.

This study aimed to investigate the impact of a recreational behavioural programme on reducing the degree of anger among children with intellectual disabilities (ID) in the primary stage. The study was implemented with 24 children who were randomly divided into two groups: an experimental group (n = 12, age = 10.80 ± 1.03 years, IQ = 63.10 ± 4.43 scores, ASW = 55.50 ± 1.51 scores) and a control group (n = 12, age = 10.80 ± 0.92 years, IQ = 63.00 ± 4.16 scores, ASW = 56.00 ± 1.15 scores). We used the PROMIS anger scale with a modification that was used to measure the degree of anger, and the recreational behavioural programme was implemented three times per week for six weeks. The results of the research showed that the improvement percentages for Anger Triggers (AT), Inner Anger (IA), and External Anger (EA) were 9.73%, 9.04%, and 9.60%, respectively, and the Anger scale as a whole (ASW) rate was 9.46%. r = (0.89–0.91). The experimental group using the recreational behavioural programme also outperformed the control group, as the results indicated a decrease in the intensity of anger in the direction of the experimental group. The differences in the improvement percentages for Anger Triggers (AT) Inner Anger (IA), and External Anger (EA) were 32.97%, 31.03%, and 26.63%, respectively, and the Anger scale as a whole (ASW) rate was 30.09%, r = (0.82–0.86). The results of the study confirmed the effectiveness of the recreational activity programme in developing social interaction among children with intellectual disabilities, which indicates the success of the recreational behavioural programme in reducing the degree of anger among children with IDs. Therefore, the recreational behavioural programme had a positive effect in terms of reducing the degree of anger among children with IDs in the primary stage.

Abstract: In his latest hermeneutics, the French philosopher “Paul Ricoeur” ended up designing an entire research program whose aim is to rethink the question of the subject in the light of a radically new concept, namely that of narrative identity. This concept served to include the element of otherness in a way that it could be considered as constitutive of identity in the sense of selfhood, and this through the negative experiences forming, ultimately, the experiences of the otherness such as those of the own body, of others and of moral conscience. Pour « l’instance de réception » du sens que nous sommes, tout procès sémiosique, tout ensemble signifiant suppose une « instance énonçante » qui se trouve à son origine et qui « énonce », à travers ce qu’elle signifie, un rapport au monde, le sien ou celui d’autrui, vécu ou imaginé, tout en « s’énonçant » en même temps comme dotée de telle ou telle « identité ». Il s’agit non pas d’une identité socio-historique mais d’une « identité sémiotique » car elle fait sens pour nous, récepteurs et interprètes. Cependant, la saisie de cette identité varie d’un interprète à un autre : certains, tablant sur le « principe d’immanence » typique de toute pensée scientifique, ne tiennent compte que du logos et des « prédicats cognitifs » qui le manifestent ; d’autres, au contraire, peuvent en plus, en faisant le leur un « principe de réalité », mettre en avant les « prédicats somatiques » et la phusis qu’ils traduisent. Ce sont ces différents types de réception et d’interprétation du « discours », effectuation du langage, modèle par excellence de toute signification, que la « théorie des instances énonçantes » (Coquet) essaie de penser, non pas dans le cadre commun hypothético-déductif et génératif, mais plutôt en tablant, à travers la mise au jour d’un certain nombre de régularités sémiotiques, sur un « parcours inductif » de nature interprétative conjuguant les manifestations discursives aussi bien du logos que de la phusis. Ce sont les articulations importantes de cette « forme » singulière donnée à la « sémiotique discursive » (Greimas) que nous allons tenter de cerner et d’expliciter ici.

Il s’agit d’essayer de penser un « entre-deux » entre les travaux novateurs de Gilbert Durand sur l’imaginaire et les conceptualisations sémiotiques de la signification. Bien que Durand semble rejeter toute forme d’« explication sémiologique » des images-symboles, sa conception de l’imaginaire est régie par une sémiosis et une rationalité spécifiques qui permettent d’entrevoir, même si l’intersection n’implique pas forcément la fusion, les contours d’un « entre-deux » entre son « archétypologie » et la sémiotique.

A “semiótica discursiva” fundada por Greimas estipula, a partir do ponto de vista antropológico que é o seu, que o discurso é a manifestação dos “jogos de coerções semióticas” próprios ao espírito humano. A partir dessa semiótica se constituiu progressivamente uma abordagem do discurso que se inscrevendo numa perspectiva fenomenológica, interroga os modos de participação do “sujeito enunciador” no seu dizer e no seu fazer, antes de mostrar que a significação discursiva é polifônica e que ela faz ouvir tanto a voz do logos quanto aquela da phusis.

Abstract Objectives: We conducted this study to investigate the safety and analgesic efficacy of the addition of Ketamine to Bupivacaine in bilateral extra-oral infra-orbital nerve block in children undergoing cleft lip surgeries. Methods: Sixty patients were randomly allocated into two groups (n = 30), Group B received infra-orbital nerve block with 2 mL of 0.25% Bupivacaine and Group BK received 0.5 mg.kg-1 Ketamine for each side added to 1 mL of 0.5% Bupivacaine solution diluted up to 2 mL solution to 0.25% Bupivacaine concentration. Assessment parameters included; hemodynamics, recovery time, time to first oral intake, postoperative Faces Legs Activity Cry Consolability (FLACC) scores, Four-point Agitation scores, analgesic consumption and adverse effects. Results: Patients in Group BK showed lower postoperative FLACC scores during all recorded time points (p < 0.0001). Two patients in Group BK versus 12 in Group B requested for postoperative rescue analgesia (p < 0.001). There were no differences between groups in time, minutes (min), to first request for rescue analgesia. Patients in Group BK reported lower analgesic consumption (366.67 ± 45.67 vs. 240.0 ± 0.0 mg, p < 0.04). The time to first oral intake was significantly reduced in Group BK (87.67 ± 15.41 vs. 27.33 ± 8.68 min, p < 0.001). Lower postoperative Agitation scores were recorded in Group BK patients that reached a statistical significance at 45 min (0.86 ± 0.11 vs. 0.46 ± 0.16, p < 0.04) and in the first hour (h) postoperatively (1.40 ± 0.17 vs. 0.67 ± 0.14, p < 0.003). Higher parent satisfaction scores were recorded in Group BK (p < 0.04) without significant adverse effects. Conclusions: The addition of Ketamine to Bupivacaine has accentuated the analgesic efficacy of infra-orbital nerve block in children undergoing cleft lip repair surgeries.

Introduction: Laparoscopic radical nephrectomy(LRN) is a cornerstone in managing renal cancer and small renal masses. Twenty-first century surgical training faces challenges, thus must be efficient and safe so surgeons attain relevant skills, protecting patients and operative outcomes. This study aimed to systematically develop a tool for training and assessment in LRN and validate the developed tool for use by trainee urologists., Methods: This prospective, longitudinal, multi-institutional study was undertaken from September 2014 - June 2015. Healthcare Failure Mode and Effect Analysis was utilised for development and followed by validation where the assessment tool was distributed to five specialists to increase content validity. Four experts were observed as a multi-institutional approach. Hand-assisted, transperitoneal and retroperitoneal approaches were considered., Results: The LRN Assessment Tool comprised four phases, 17 processes, 41 sub-processes. Four surgeons and operating teams were observed across four hospitals for 19.5hours (5.75h hand-assisted, 8.75h trans-peritoneal, 5h retro-peritoneal). After hazard analysis, three checklists were constructed. Those for hand-assisted LRN and transperitoneal LRN contained four phases, 20 processes, 33 sub-processes and that for retroperitoneal LRN contained four phases, 20 processes, 30 sub-processes. These were merged to form one assessment tool. The final result was a four phase LRN Assessment Tool with 17 processes, 41 sub-processes. All participants agreed the final LRN Assessment Tool included pertinent steps., Conclusions: The LRN Assessment Tool was developed using Healthcare Failure Mode and Effect Analysis risk analysis to ensure hazardous procedural sub-steps were included. Introduction: Cognitive training is an important training modality which allows the user to rehearse a procedure without physically carrying it out. This has led to recent interests to incorporate cognitive training within surgical education but research is currently limited. The use of cognitive training in surgery is not clear-cut and so this study aimed to determine whether, relative to a control condition, the use of cognitive training improves technical surgical skills on a ureteroscopy simulator, and if so whether one cognitive training method is superior., Methods: This prospective, comparative study recruited 59 medical students and randomised them to one of three groups: control- simulation training only (n=20), flashcards cognitive training group (n=20) or mental imagery cognitive training group (n=19). All participants completed three tasks at baseline on the URO Mentor simulator followed by the cognitive intervention if randomised to receive it. Participants then returned to perform an assessment task on the simulator. Outcome measures from the URO Mentor performance report was used for analysis and a quantitative survey was given to all participants to assess usefulness of training received., Results: This study showed cognitive training to have minimal effects on technical skills of participants. The mental imagery group had fewer laser misfires in the assessment task when compared to both control and flashcards group (P=.017, P=.036, respectively). The flashcards group rated their preparation to be most useful when compared to control (P=.0125). Other parameters analysed between the groups did not reach statistical significance. Cognitive training was found to be feasible and cost effective when carried out in addition to simulation training., Conclusion: This study has shown that the role of cognitive training within acquisition of surgical skills is minimal and that no form of cognitive training was superior to another.
