The obesity epidemic is a phenomenon that has been widely studied in recent decades but is still incompletely understood, and its control is far from the desirable level in view of the increasing prevalence figures observed worldwide. This paper conducts a narrative review with the aim of providing updated evidence on the global obesity epidemic, and particularly on the situation in Latin America and Argentina, identifying the main challenges and future directions for addressing this public health problem. It first describes the current burden and increasing trends in the prevalence of obesity, in the overall population and by population groups, and its possible association with genetic and epigenetic aspects. It also summarizes the direct and indirect socioeconomic consequences of this epidemic, as well as recent strategies and initiatives focused on obesity prevention, with special attention to those reported as the most efficient in the Latin American context. This review identified some pending challenges in the region, the integrated approach to the double burden of malnutrition and the growing childhood overweight; and it points out some emerging approaches, such as the syndemic approach, as potentially useful to understand and address this complex problem in the current context. In conclusion, it highlights the importance of implementing renewed, more efficient and evidence-based strategies to control the growing prevalence of obesity, which would also impact on the burden of related chronic diseases, and thus on the economy and well-being of Latin American societies.

Treatment-Resistant Depression (TRD) prevalence varies considerable between regions and epidemiology of TRD in Argentina is lacking. Based on the Treatment-Resistant Depression in America Latina (TRAL) study, epidemiology and burden of TRD in MDD patients from Argentina is reported in this paper. A sample of adult MDD patients (n=396) from 5 sites in Argentina, with clinical diagnosis were included. Patients with psychosis, schizophrenia, bipolar disorder, schizoaffective disorder, dementia, with severe chemical dependence or currently participating in another clinical trial were excluded. Patient reported outcomes and clinical assessment scales were used as outcomes. The prevalence of TRD in MDD patients in Argentina is 33.2%, based on TRAL data. Patients in TRD are older compared to those without TRD, and was more evident in married/consensual union MDD patients. Higher suicidality, greater comorbidity based on MINI, and worse scores in MADRS and PHQ-9 were identified in TRD patients. The prevalence identified in TRAL study for Argentina is substantial comparing with other Latin American countries and worldwide prevalence. TRD represents a disproportional burden to society, and efforts should be placed on reducing the burden of MDD and TRD in Argentina by improving early diagnosis, therapeutic management and ensuring that all patients have better access to mental healthcare.

Coagulopathy and thrombosis associated with coronavirus disease 2019 (COVID-19) represent a major issue in the management of this disease. In the past months, clinical studies have demonstrated that COVID-19 patients present with a particular hypercoagulable state, in which a markedly increased D-dimer concomitant with increased levels of fibrinogen are observed. This hypercoagulable state leads to an increased risk of thrombosis, which seems to be higher among those patients with critical symptoms of COVID-19. The best therapeutic approach to prevent thrombotic events in COVID-19 has not been determined yet and several questions regarding thromboprophylaxis therapy, such as the time to initiate anticoagulation, type of anticoagulant and dose regimen, have emerged among physicians. To address these concerns, several medical societies have published position papers to provide the opinion of thrombosis experts on the management of coagulopathy and thrombosis associated with COVID-19. In line with this, the Latin America Cooperative Group of Hemostasis and Thrombosis (Grupo CLAHT) has constituted a panel of experts in thrombosis and hemostasis to discuss the available data on this topic. The aim of this review is to summarize the current evidence regarding hemostatic impairment and thrombotic risk in COVID-19 and to provide a carefully revised opinion of Latin American experts on the thromboprophylaxis and management of thrombotic events and coagulopathy in patients with suspected COVID-19.

The incidence of the coronavirus disease 2019 (COVID-19) in Latin America and Spain and its impact particularly on hospital emergency departments have been great, sustained, and unpredictable. Unfortunately, this situation will continue in the medium term, regardless of the diverse concepts and definitions used to identify cases or hypotheses about the role of staff. In the context of the worldwide pandemic, a multinational group of experts from the Latin American Working Group to Improve Care for Patients With Infection (GT-LATINFURG) has drafted various opinion papers for use by emergency care systems in the member countries. The GT-LATINFURG is comprised of representatives from the 13 scientific associations affiliated with the Latin American Federation for Emergency Medicine (FLAME). Experts from the Spanish Society of Emergency Medicine (SEMES) also participated. The present consensus statement offers protocols and recommendations to facilitate the work of hospital emergency departments with regard to key issues the group identified, namely, the need for reorganization, triage, and routine test availability. Additional issues discussed include biomarkers; clinical, laboratory, radiologic, and microbiologic criteria for identifying patients with COVID-19; and risk and prognostic factors for mortality that emergency staff can use to quickly detect severe cases in our settings.

The current COVID-19 pandemic has the potential to overwhelm the capacity of hospitals and Intensive Care Units in Chile and Latin America. Thus local authorities have an ethical obligation to be prepared by implementing pertinent measures to prevent a situation of rationing of scarce healthcare resources, and by defining ethically acceptable and socially legitimate criteria for the allocation of these resources. This paper responds to recent ethical guidelines issued by a Chilean academic institution and discusses the main moral principles for the ethical foundations of criteria for rationing during the present crisis. It argues that under exceptional circumstances such as the current pandemic, the traditional patient-centered morality of medicine needs to be balanced with ethical principles formulated from a public health perspective, including the principles of social utility, social justice and equity, among others. The paper concludes with some recommendations regarding how to reach an agreement about rationing criteria and about their implementation in clinical practice.

The purpose a call for papers on "History and health" at Salud Colectiva was to contribute to the development of a historical perspective on health-disease processes. We are therefore interested in calling on the perspectives of history and the social sciences to shed light on the fact that both the definition of health conditions as well as populations' possibilities for accessing essential healthcare services are the result of social processes in which knowledge, ideology, politics, and interests come into play - far from a simple reflection of objective biomedical conditions. This implies, on the one hand, that the definition of which of a population's health problems are to be attended to - as well as how, where, and who is responsible for them - is the result of relations among diverse actors (healthcare professionals, public officials, firms, users, among others), and these relations are not free of conflict. At the same time, these actors mobilize the institutional frameworks, professional knowhow, and economic interests that they represent. On the other hand, the unequal distribution of economic, symbolic, and cultural resources implies differential access to healthcare services, both for individuals and for social groups.

Caustic ingestion represents a serious social-medical problem due to the devastating and irreversible consequences it can produce in the upper digestive tract. In Ibero-America, there are no published reliable data on the incidence or prevalence of caustic-induced injuries, and most of the available information on clinical presentation, diagnosis, treatment, and prognosis is based on retrospective clinical series and, indeed, its clinical management is often based primarily on expert opinion. Re cently as an initiative of the Latin American Society for Pediatric Gastroenterology, Hepatology and Nutrition (LASPGHAN) and with the cooperation of the Spanish Society for Pediatric Gastroente rology, Hepatology and Nutrition (SEGHNP), we have designed a Clinical Practice Guideline that include a series of statements and recommendations aimed at optimizing patient medical care which is based on the systematic review of evidence. Two (2) separate papers focused on the evaluation of physiopathological and clinical-endoscopic diagnostic features of caustic esophagitis in children (1st. Paper) and, on the other hand, the most relevant therapeutic considerations (2nd. Paper). We expect this guideline to become a useful tool for the physician in the difficult decision-making process when assessing patients after caustic ingestion.

Ever since antimicrobial activity was observed at the end of the XIX century and antibiotics were produced on a large scale in the 40s, microorganisms have developed multiple resistance mechanisms, making treatment of infectious diseases difficult. For instance, several Gram-positive and Gram-negative bacteria lowered their sensitivity to ß-Lactam antibiotics as a result of their inadequate use and abuse. For this reason, microbial resistance to these drugs represents an increasing health problem in Latin America due to the emergence of drug-resistant bacterial strains. This review aims to summarize and analyze scientific literature reporting resistance to ß-lactam antibiotics in Latin America. We compiled scientific papers published during the last five years from PubMed, SciELO, and LILACS-BIREME. We found that: (i) it is common to identify resistance genes for ß-lactams in the soil and animal farms, and (ii) over 40% of strains isolated from clinical samples developed resistance against ß-lactam antibiotics.

Objective: this paper aims to describe the first phase of a project whose general goal was to develop a consensus-based set of advanced practice nurse competencies applicable to Latin American countries and, based on these competencies, produce an advanced practice nurse curricular prototype adapted to Latin American countries. The project was framed in a competency-based approach to advanced practice nursing education. The specific aims of the first phase of the project described in this paper were: 1) to identify a set of potential advanced practice nurse competencies that would serve as the template for Core Advanced Practice Nurse Competencies in Latin American countries and 2) to establish consensus for Core Advanced Practice Nurse Competencies in Latin American countries., Method: advanced practice nurse competencies were derived from a comprehensive review of published competencies and informed the development of a survey designed to assess the relevance of advanced practice nurse competencies in Latin American countries. The survey was distributed to nurse leaders and nurse educators. Data were analyzed using descriptive statistics., Results: consensus for Core Competencies was established., Conclusion: the Core Advanced Practice Nurse Competencies presented can provide a structured framework to build educational programs aligned to the needs of the regional environment.

Objetive: To describe the characteristics of the scientific production of midwives in Latin American obstetrics and gynecology journals indexed to Scopus during the period 2011 to 2016., Methods: This paper reports a descriptive bibliometric study, with intentional non-probabilistic sampling. We analyzed articles of research papers published by midwives in medical specialty journals in obstetrics and gynecology of Latin America indexed to Scopus, in the 2011 to 2016 period., Results: We found eight obstetrics and gynecology journals in Latin America indexed in Scopus. There were 1,696 articles published between 2011 and 2016, of which 4.9% were authored or co-authored by midwives. Of these publications, 93.8% were related to topics of the specialty, 62.5% had midwives as corresponding authors, 64.1% of papers were in Spanish, only 3.1% were published in English, and 57.8% of midwives worked in Chile at the time of publication. In 2016, there was one-fifth the number of publications compared to 2012., Conclusions: We found little scientific production by midwives in Latin American obstetrics and gynecology journals indexed in Scopus. Strategies that encourage and allow research and scientific production by midwives are needed.

En 2005 se publicaron recomendaciones para la tipificación de hemopatías malignas en Latinoamérica. Se consideró necesario realizar una reunión nacional para actualizarlas. Se convocaron y reunieron 95 profesionales expertos en el tema para analizar y contrastar alternativas y llegar a un consenso. Se alcanzaron opiniones de consenso en lo relativo a indicaciones, tipos y manejo de muestras, anticuerpos, nomenclatura e informe de resultados para el diagnóstico y seguimiento de las leucemias agudas. This paper describes and analyzes the psychiatric reform process in three Latin American countries (Brazil, Chile and Colombia) based on the Caracas Declaration of 1990. It compares the psychiatric reform processes in these three countries and highlights the role of national mental health laws in these processes. Our goal when investigating the experiences in other countries of Latin America is to draw conclusions for the Argentine psychiatric reform and for the future of such reforms in the region.

Objective: To estimate the proportion of abstracts presented at meetings of the Latin American Society for Pediatric Research that are fully-published, to describe the reasons for not publishing papers, and to assess the impact of funding on the publication rate., Methods: Abstracts presented at meetings held between 2005 and 2009 were included. Authors were contacted and invited to take a survey on the publication of their work or the reasons not to do it., Results: Information was collected on 232 (71.4%) of the 325 abstracts presented. Of these, 58.6% were fully-published (136/232). Funded studies (40.0%) had more chances of publication (OR: 2.2; 95% CI: 1.2-3.9). "Lack of time" was the most common reason for failure to publish (35/96)., Conclusion: 58.6% of abstracts presented at meetings of the Latin American Society for Pediatric Research, were published as full-text articles; lack of time was the most common reason for failure to publish. Funded research had more chances of being published.

This paper presents only some of the most important contributions in the development of cultural psychiatry in Latin America. The continental efforts to understand the role that culture plays in the manifestation and treatment of mental disorders have been fruitful. Seguin, Hermilio Valdizán and Javier Mariátegui in Peru; Y. Bespaldi of Consens of Uruguay; Rojas and Carlos Malpica and Jacqueline Briceño Clarac of Venezuela.

Sensitization to food allergens, as well as the development of food allergies, is increasing worldwide, and cow's milk and hen's eggs are the main implicated foods. In most Latin American countries there are no management guidelines on the aforementioned topics; at their creation, such guidelines should be adapted to the conditions of the population in each region. This paper presents a review of the management of food allergy to milk and eggs useful for health personnel at all levels and some considerations of the factors found in Latin American developing countries.

Chagas disease is a chronic and systemic infection caused by Trypanosoma cruzi. According to estimates from WHO, 10 million people are affected by this parasite. In the last years, birthrate among the immigrant women from Latin America settled in the Comunidad Autónoma de Madrid has been increasing, and as T. cruzi can be transmitted from mother to child, in fact 11 cases of congenital Chagas disease have been confirmed. Therefore, the aim of this paper is encouraging improvements in the coverage of the anti-T. cruzi antibodies detection in pregnant women from endemic areas. By this strategy, an active search for infected pregnant women and early detection of her infected newborns could be conducted, and then an early specific treatment could be administrated. Thus, there could be an important contribution to the control of Chagas disease in non-endemic area.

The study was aimed at identifying Latin-American countries' public policy which has been related to the social determinants of health. A topic review was thus made of papers kept in the 22 Latin-American countries' databases and official documents issued by their multilateral organisations and ministries of health. The World Health Organization's concept of the social determinants of health has been summarised and a history given of the pertinent work developed worldwide in regions such as Europe and Latin-America. Public policy regarding the field of study in Argentina, Brazil, Bolivia, Chile, Colombia, Costa Rica, México and Venezuela has been described. It was concluded that Latin-America provides a panorama of inequality regarding the application of policy concerning the social determinants of health and that there was segmented intervention, mainly regarding intermediate determinants of health, without taking an integrated approach from different entrance points into account, according to the stated conceptual framework.

Latin America is adopting regulations that bear on medicinal costs and spending. The regulations have four main goals: i) to guarantee a competitive market, ii) to ensure affordability for individual consumers (commercial channel), iii) to contain public spending on medicines (institutional channel), and iv) to guarantee efficient spending on medicines. The experience of Latin America differs from that of countries in developed regions. In the latter, the countries tend to have similar policies, both in promoting generic medicines and in price control strategies, and in optimizing and containing pharmaceutical expense. In contrast, in Latin America, certain institutional weaknesses impede the consolidation and application of an effective regulatory policy. This paper reviews the experience gained through the adoption of economic regulations aimed at reducing spending and improving access to medicines, suggests lessons learned at the international level, and offers recommendations for the countries of Latin America. Its purpose is to offer key elements to decision-makers and the authorities of the countries concerned in pursuing economic regulation of medicines.

Homicides occur the world over, but they are not homogeneously distributed by geographical areas (continents, countries, regions), either over long or short periods of time, or in social groups, namely age, gender, social class or ethnicity. Why are there more homicides in some countries than in others? Why do killings increase in some countries, while they decrease in others? There are two fundamental schools of thought for social explanations of crime and violence: those attributing its origins to poverty and inequality and those blaming institutionalization or social norms. To discuss these theories, this paper analyzes and compares the changes in Colombia, Venezuela and Brazil in the first decade of the twenty-first century, where the homicide rate has decreased, increased and remained the same, respectively. Using the measurement of six variables (poverty, inequality, unemployment, national wealth, human development and the rule of law) and the technique of trajectory analysis, the results revealed that institutionalization is more to blame for the change than poverty and inequality. The text concludes that poverty and inequality affect crime and homicides, although not directly, but mediated by the institutions instead.

Introduction: Research is usually considered a reliable indicator of the degree of development. Research in a problematic area such as food and nutrition for a given region, should have an impact on scientific production in agreement with the importance of the problem, the research capacity and the available resources for generating such a research., Objective: To identify some indicators of Iberoamerican research in nutrition and food., Method: Retrospective study of Iberoamerican scientific production in nutrition and food in the last 25 years. The data were obtained from the bibliographic database Science Citation Index Expanded, Journal Citation Reports Science Edition Database 2011, both included in the Web of Knowledge (Thomson Reuters), and the database of the World Bank., Results: 49,808 papers were registered, the 3.20% of the Health Sciences collection in SCI. The evolution was fitted to an exponential model, N&D (R² 0.962) and FS&T (R² 0.995). The average production in N&D per average population was higher in Spain with 0.659 papers/million. The highest rates of productivity and profitability were found in Guatemala with 12.963 papers/1000 researchers and 1.486 papers/million $ respectively. The average production in FS&T of the different countries per average population was higher in Cuba with 21.624 papers/million. The productivity index was higher in Uruguay with 25.999 papers/thousand researchers. The profitability index was higher in Guatemala with 0.271 papers/million $., Conclusion: There is exponential growth in the two categories studied N&D and FS&T. Productivity and profitability was higher in countries with low R&D (Research & Development) budget.

No randomized controlled trials have been conducted in gallbladder cancer to establish standard treatments. We therefore conducted the first Latin American Consensus meeting for the management of gallbladder cancer. In this paper we report the conclusions of the experts' panel for (neo) adjuvant treatment of resectable gallbladder cancer. These are based on the review of the literature, the discussion of the participating experts and the vote of the assistants (surgical oncologists, medical oncologists, radiation oncologists and others). The reviewed topics were the role or adjuvant radiochemotherapy in T1 bN0M0, T2-3 N0-1M0 and T4 N0-1 M0 disease and doses, schedules and drugs for radiochemotherapy.

This paper explores some ethical dilemmas faced while doing fieldwork. Ethical norms are not enough to appraise the relationship between researchers and participants; a reflexivity practice is needed to understand the dilemmas aroused during this process. Here four issues faced during fieldwork are presented. The academic social time usually defines the schedule of fieldwork; hence, the researcher may decide to follow the academic schedule or to arrange a different schedule with informants. Researchers usually decide which part of their identity will be disclosed for introducing themselves to the informants; but may also decide to hide who really he/she is. Researchers may cope with the dilemma to disclose their private life to informants during the fieldwork. Researchers also may decide to behave according to academic cultural norms of reciprocity or according to the cultural norms of participants. Finally, a reflexivity movement should be developed in Iberoamerica to define an agenda on ethical issues and to develop decolonizing strategies to debate these ethical dilemmas.

This paper reviews the principal concepts of social class, occupation, and social stratification, and their contribution to the analysis of the social determinants of health (SDH), and reviews empirical studies conducted in Latin America that use employment relations as an SDH. The review focuses on studies of the relationship between health and social class based on neo-Weberian or neo-Marxist perspectives. A search of the BIREME Virtual Health Library and the SciELO database found 28 articles meeting these characteristics. This relative dearth contrasts with the profusion of papers that use these approaches written in Europe and in the United States, with a long tradition in the analysis of SDH. In this regard, the political and programmatic implications of research on social class and employment relations are different from and complementary to studies of health gradients associated with income and education. Globalization of employment relations requires the development of new concepts to explain and measure the mechanisms of action of the SDH going beyond what is strictly labor related; in particular, the importance in the current Latin American reality of the impact of informal work on health.

In this paper the historical-epistemological statute of nineteenth-century European positivism is discussed to further develop its scope in the constitution of Science in general and in the conformation of the disciplines tied to the human behavior in the time of the creation of the nation's institutions in Argentina. The ideological bias that this thinking legitimates in key moments of identity construction, such as the Centenary period, is analyzed. Finally, emphasis is made on the incidence of positivism in other countries in the region observing in every case the same ideological bias found in the Argentinean case, tinged by cultural and socio-political variables.

Cervical cancer continues to be a significant health problem in Latin America. The use of conventional cytology to detect precancerous cervical lesions has had almost no major impact on reducing cervical cancer incidence and mortality rates, which are still high in the region. The availability of new screening tools to detect precancerous lesions provide great opportunities for cervical cancer prevention in the region, as do highly efficacious HPV vaccines able to prevent nearly all lesions associated with HPV-16 and -18 when applied before viral exposure. This paper summarizes the scientific evidence and regional experiences related to: i) the use of HPV testing and visual inspection after the application of acetic acid (VIA) in primary screening and ii) the implementation of adolescent HPV vaccination programs. Finally, we outline a number of recommendations for different resource settings. The feasibility of implementing successful and sustainable national cervical cancer prevention programs in Latin American countries in the region will depend on health priorities and the availability of infrastructure and health personnel--as determined by rigorous local situational analysis.

The present paper was aimed at promoting countermeasures based on scientific evidence and international cooperation for evaluating the impact on health caused by exposure to asbestos fibres in the workplace and the environment. Scientific evidence regarding asbestos made available by the international scientific community, decades of experience gained in Italy on this issue and being aware that adopting measures to combat the health effects caused by asbestos exposure should be verified considering the specificity of various national and local contexts in Latin-America form the basis for identifying four main areas for intervention which may be developed in the field of technical and scientific cooperation between Italy and Latin-America countries: promoting access to information about asbestos, interventions for reducing exposure to asbestos, health surveillance of exposed subjects and mesothelioma detection. Integrating Colombian and Italian researchers' abilities may lead to such results being achieved, thereby contributing to banning asbestos, which is already underway in Latin-America.

We present a review of the biological systematic research in Latin America during the twentieth century, applying a bibliometric analysis to the information contained in international databases with the largest number of biological records: Biosis (since 1969), CAB (since 1910) and Science Citation Index (since 1900), to recognize certain patterns and trends regarding the document production. We obtained 19079 documents and 1387 journals for Biosis, 14326 and 2537 for CAB, 3257 and 1636 for SCI. Of the documents, 54.6% related to new species, 15.3% dealt with morphology, 14.9% keys, 12.5% descriptions, 10.6% cases of synonymies, 6% new genera, 4.9% new geographical records, 23.6% geographical distribution, 4.2% redescriptions, and 3.6% with new nomenclatural combinations. The regions mentioned were South America with 11.9%, Central America with 4% and America (all) with 2.56%. Nineteen Latin American countries appear, whereas outside this region we found the United States of America with 12.6% of representation and Canada with 3%. Animals (65.6%) were the most studied taxa, which was 1.7 times higher than what was published for plants (37%), 11 times higher than fungi (6%) and nearly 30 times higher than microorganisms (2.3%). Out of the 155 journals that produced 66% of the papers, 76.5% were better represented in Biosis, 21.4% in CAB and 2% in SCI. Twenty-nine journals published 33% of the articles, the maximum number of records obtained was 69% for Biosis, CAB 24% and 6.9% for SCI, three (10.3%) are in biology, 11 (37.9%) in botany, 13 (44.8%) zoology, and two (6.9%) paleontology; eight of these journals (27.5%) were published in Latin America and twenty were indexed in the Science Citation Index. In the last two years more journals of the region that publish on taxonomy have been indexed, but their impact factor is still low. However, the impact factor of a number of Latin American journals that published biodiversity increased with time. Countries that are more interested in studying the Latin American biota from the taxonomic point of view are Brazil, the United States, Argentina and Mexico. The most active institutions in this discipline were the Universidade de São Paulo, Universidad Nacional Autónoma de México, and the Universidade Federal do Rio de Janeiro; together they produced 24% of the documents.

The term governance is used more and more often in the formulation, implementation, and evaluation of public policies. The purpose of this paper is twofold: to shed light on its meaning, and to study its applicability for the study of recent public health policies in Latin America. After discussing the differences between governance and the close concepts of management, governability and government, we propose the adoption of the notion of Giddens' duality of social structure for better understanding of the governance processes. Then we use this theoretical framework for reviewing the conceptual and empirical implications of the set of papers about governance and health in Latin America included in the present special issue of the Journal of Public Health. Recognizing that the debate on governance is not closed, we end the paper highlighting the main contributions of the revised works, namely: the importance of equity in decision-making processes, the appreciation of the political nature of governance, and the advantages and limitations of the clan mechanism of governance for regulating interactions among actors involved in the health policy decision-making processes.

Chagas disease is one of the main endemic illnesses in Latin America. Since 1969 up to the present, the treatment of chronic Chagas patients has been the object of a scientific-medical controversy mobilizing several bodies of knowledge and actors. The resolution of the controversy has direct effects on the health of patients; however, they have been systematically excluded of public debates. In this paper we identified and specified four different steps of the controversy, showing the different positions adopted by the actors implicated, analyzing their statements, the scientific knowledge mobilized during the debates and the consequences for the establishment of norms for the treatment. We conclude showing that what makes up the central issue of the controversy is not what the actors implicated explicitly state as it but the criteria (technical and cognitive) through which are measured and defined "illness" and "cure" states. This research is about a case study in Argentina.

This article discusses the main features of the national health research systems (NHRS) of Argentina, Bolivia, Brazil, Chile, Costa Rica, Cuba, Ecuador, El Salvador, Honduras, Panama, Paraguay, Peru, Uruguay, and Venezuela, based on documents prepared by their country experts who participated in the First Latin American Conference on Research and Innovation for Health held in April 2008, in Rio de Janeiro, Brazil. The review also includes sources cited in the reports, published scientific papers, and expert opinion, as well as regional secondary sources. Six countries reported having formal entities for health research governance and management: Brazil and Costa Rica's entities are led by their ministries of health; while Argentina, Cuba, Ecuador, and Venezuela have entities shared by their ministries of health and ministries of science and technology. Brazil and Ecuador each reported having a comprehensive national policy devoted specifically to health science, technology, and innovation. Argentina, Brazil, Costa Rica, Cuba, Ecuador, Panama, Paraguay, Peru, and Venezuela reported having established health research priorities. In conclusion, encouraging progress has been made, despite the structural and functional heterogeneity of the study countries' NHRS and their disparate levels of development. Instituting good NHRS governance/management is of utmost importance to how efficiently ministries of health, other government players, and society-at-large can tackle health research.

Clostridium difficile associated disease (CDAD) has shown a sustained increase worldwide over the last ten years. However, there are few studies on this topic in Latin America. We conducted a comprehensive literature review using medical databases of Latin American countries. We found only seven recent papers in which clinical characteristics and risk factors were analyzed; some included outcome variables. Of these articles, only one was prospective, while the rest were either retrospective, cross-sectional or case-control studies. Most studies were done among hospitalized adult patients, even though patients 13+ years were also included in some reports. Only two recent clinical studies used cell culture to determine a cytopathic effect and the rest included immunoenzymatic assays. In general, all the studies we reviewed showed that the use of fluorquinolones, clindamycin, and cephalosporins were the antibiotics mostly associated with CDAD. Treatment schedules generally included metronidazol, although vancomycin was reported in one. Attributable mortality was lower than the mortality described in previous reports from hospitals in developed countries. Studies where this outcome was included did not surpass 4%, a significant difference from the findings from developed countries. In Latin America there are few studies that describe this clinical problem, they generally include small sample sizes and most are retrospective. There is a clear need to design and carry out prospective studies that will allow us to determine the true prevalence of this health problem

The establishment of environmental health indicators for assessing the adverse effects of environmental changes on the population's health and quality of life is, as yet, a goal that has not been fully reached in Latin America and the Caribbean. As such, the United Nations Environment Program and the Pan American Health Organization (PAHO) have convened Region's institutions and experts to develop a comprehensive method for assessing environmental health. This paper evaluates several methodologies, comprehensive or otherwise, for assessing health and the environment, and describes the frameworks that have historically undergirded the key methods that have either structured or generated the environmental health indicators being used in Latin America and the Caribbean. The recurring, methodological limitations were identified: (a) relying heavily on secondary data, which points out the need for technological infrastructure that is rarely available in Latin America and the Caribbean today; and (b) a lack of clear criteria for developing inclusive tools that would facilitate the discussion of environmental health issues at the grass-roots level. Despite the progress made by the field of environmental health with regard to understanding its interdisciplinary complexities, intersectoral operations must be improved to favor open communication and implementation of integrated policies on environmental and health.

The present paper has two aims: firstly to analyse and compare body dissatisfaction of Spanish and Latin adolescents, and secondly to determine the impact of sociocultural factors and Body Mass Index (BMI) on dissatisfaction. The sample is made up of 403 adolescents, 191 Spaniards and 212 Latin-Americans, all of them students in Basque Country high schools. They were assessed by the Body Shape Questionnaire (BSQ), the CIMEC-26 (a questionnaire designed to assess the influence of the aesthetic body model), and the BMI. The results show that the Latin adolescents have a higher level of body dissatisfaction than the Spanish. This is due to the greater influence in the Latin adolescents of sociocultural factors.

Stunting affects 16% of children under 5 years old, in Latin America. This is a cumulative effect of poor diets and repeated episodes of infectious diseases. Our aim was to analyze the impact of nutrition programs oriented to increase food availability at the family level on the growth of under fives in Latin America. We included all evaluated and published interventions on food availability. An electronic and manual search of papers published between 1995-2005 in PUBMED, LILACS, SCIELO and international organizations, was performed. Three type of programs were identified: Conditioned Economical Transference Programs (CETP) (n= ), Complementary Feeding Programs (CFP) (n= ), and Food Security Programs (FSP) (n=). Operational deficiencies were reported, some of them being participants attrition, non acceptance of the food supplement or its dilution among other members of the family. Beneficiaries of CETP (<36 months) gained up to 1 cm while the CFP registered impact but only in the low socioeconomic children. Conversely, the (FSP) did not show the expected effects on growth. This reduced impact is understandable because in order to permit the development of the children's growth potential interventions should be directed to strike on poverty as the main mechanism of the children's hampered growth. Hence, the best results are obtained by coordinated programs that combat poverty optimizing the intervention's continuity and management and give priority to the most vulnerable groups.

Objective: To characterize frequency and type of use of theories or models on papers of a Latin-American journal in public health between 2000 and 2004., Methods: The Revista de Saúde Pública was chosen because of its history of periodic publication without interruption and current impact on the scientific communication of the area. A standard procedure was applied for reading and classifying articles in an arbitrary typology of four levels, according to the depth of the use of models or theoretical references to describe problems or issues, to formulate methods and to discuss results., Results: Of 482 articles included, 421 (87%) were research studies, 42 (9%) reviews or special contributions and 19 (4%) opinion texts or assays . Of 421 research studies, 286 (68%) had a quantitative focus, 110 (26%) qualitative and 25 (6%) mixed. Reference to theories or models is uncommon, only 90 (19%) articles mentioned a theory or model. According to the depth of the use, 29 (6%) were classified as type I, 9 (2%) as type II, 6 (1.3%) were type III and the 46 remaining texts (9.5%) were type IV., Conclusions: Reference to models was nine-fold more frequent than the use of theoretical references. The ideal use, type IV, occurred in one of every ten articles studied. It is of relevance to show theoretical and models frames used when approaching topics, formulating hypothesis, designing methods and discussing findings in papers.

Dengue and dengue hemorrhagic fever in the Americas have been on the rise throughout the 1990s, with the highest number -over one million cases- reported in 2002. This paper analyzed the situation of dengue in the region and discussed the determining factors that account for the rise of the disease, making emphasis on socioeconomic factors, such as poverty, inequality, migrations and the lack of access to basic services, which are the most influential in perpetuating this disease in most countries. Considering that a safe and accessible vaccine is now unavailable, basic principles of vector control combined with political willingness, inter-sectoral involvement, active community participation and the tightening of health legislation were also examined as the only viable solution at present.

A survey was conducted in a meeting sponsored by ALAT (Latin American Association of Thoracic Diseases). Each of the seven editors reported about their journal and answered a questionnaire. The improvement in knowledge divulgation is the main motivation of respiratory societies to edit their own journals. To disseminate medical knowledge and report experiences, are the main motivations of authors to submit papers. The most common deficiency of submitted manuscripts is a bad compliance with journal requirements. An improvement in the relationship between author-editor-reviewer should be the best strategy to enhance the quality of the manuscripts. Suggestions to improve the Latin American journals included to professionalize editorial work, to increase the meticulousness of manuscripts reviewers and to reinforce international norms for editing medical journals. Some major problems reported were a lack of a regular and adequate periodicity in publishing the issues, lack of original papers submitted that mean a "milestone" for the specialty a low percentage of submitted papers rejection and a high and frequent turnover of editors. Although several journals are available in electronic indices, they should be maintained in their printed form. Each journal should have printed its subscription fee, even considering that its subscription is included in the annual society membership fee. The feasibility to generate a multinational Latin American Journal on Respiratory Diseases should be explored.

At the 2003 meeting of the Directing Council of the Pan American Health Organization (PAHO), the PAHO Member States issued a mandate to strengthen primary health care (Resolution CD44. R6). The mandate led in 2005 to the document "Renewing Primary Health Care in the Americas. A Position Paper of the Pan American Health Organization/WHO [World Health Organization]," and it culminated in the Declaration of Montevideo, an agreement among the governments of the Region of the Americas to renew their commitment to primary health care (PHC). Scientific data have shown that PHC, regarded as the basis of all the health systems in the Region, is a key component of effective health systems and can be adapted to the range of diverse social, cultural, and economic conditions that exist. The new, global health paradigm has given rise to changes in the population's health care needs. Health services and systems must adapt to address these changes. Building on the legacy of the International Conference on Primary Health Care, held in 1978 in Alma-Ata (Kazakhstan, Union of Soviet Socialist Republics), PAHO proposes a group of strategies critical to adopting PHC-based health care systems based on the principles of equity, solidarity, and the right to the highest possible standard of health. The main objective of the strategies is to develop and/or strengthen PHC-based health systems in the entire Region of the Americas. A substantial effort will be required on the part of health professionals, citizens, governments, associations, and agencies. This document explains the strategies that must be employed at the national, subregional, Regional, and global levels.

During the last 22 years, the Region of the Americas has seen an upward trend in dengue incidence, with epidemics peaking ever higher and recurring every 3-5 years, almost regularly. A major factor in the spread of the disease has been the diminished capacity of national programs to respond with dengue prevention and control. This paper evaluates the Integrated Strategy for Dengue Prevention and Control-approved by the 44th Directing Council of the Pan American Health Organization in September 2003- and its preliminary results. The Integrated Strategy for Dengue is a management model designed to strengthen national programs, with a focus on reducing morbidity, mortality, and the societal and economic burdens produced by outbreaks and epidemics. Currently, 11 of the countries in the Region have developed a plan for or implemented a national strategy. In addition, a sub-Regional plan has been developed for Central America and the Dominican Republic. The Integrated Strategy for Dengue is expected to produce a qualitative leap forward in prevention and control through stronger partnerships among the State, its various ministries, and governing bodies, at all levels; private companies; and the range of community and civil groups. Once implemented, this strategy will reduce risk factors for dengue transmission, establish an integrated epidemiological surveillance system, decrease Aedes aegypti mosquito populations, prepare laboratories to better detect and identify the virus, optimize diagnosis and treatment, and, as a result, decrease the frequency, magnitude, and severity of dengue outbreaks and epidemics.

From the data submitted to the Pan American Health Organization (PAHO) by nationwide programs against sexually transmitted diseases (STD), HIV infection, and AIDS (2002), one can estimate the overall prevalence of syphilis among pregnant women to be 3.1% and to range from 1.00% in Peru to 6.21% in Paraguay. According to these data, the incidence of congenital syphilis ranges from 1.4 per 1000 live births in El Salvador to 12.0 per 1000 live births in Honduras. Among men who engage in sex with other men, who often classify themselves as heterosexual, as well as in female sex workers, the prevalence of syphilis ranged from 5% to 15%. Factors that determine the persistence of congenital syphilis as public health problem include a lack of awareness of the seriousness of the problem among politicians, health officials, and health care providers, difficult access to prenatal care, and screening services, a low demand for the test among users, and the stigma and discrimination that surround sexually transmitted diseases (STD). This paper seeks to focus the attention of health professionals on maternal and congenital syphilis so they will undertake actions, using an interprogrammatic approach, to eliminate congenital syphilis from Latin America and the Caribbean. Eliminating congenital syphilis will only become possible if interventions targeting vulnerable groups are also implemented. PAHO's role in eliminating congenital syphilis includes determining the baseline situation in the Region as a whole and in each country, developing communication and procurement strategies, supporting nationwide programs, promoting operational research, and facilitating interprogrammatic coordination.

In the present paper, we standardized a microtitration assay for the determination of specific activity of glutathione-S-transferase using a Culex quinquefasciatus strain selected in lab with pyrethroid insecticide lambdacyhalothrin for 6 generations (SP6). The saturation values for reduced glutathione and 1-chlorine 2,4-dinitrobenzene were 15 mM and 40 mM, respectively; we also estimated that 3 minutes was the optimal time in order to differentiate resistant strain SP6 from the susceptible reference strain S-Lab (p>0.05). Additionally, the optimized microtitration method allowed testing 4 Culex strains from Cuba (SANTIAGO DE CUBA, SD4, QUIBU AND SP6), one from Venezuela (MIRANDA), one from Colombia (MEDELLIN) and the other from Brazil (RIO DE JANEIRO) for glutathione-S-transferase activity. MIRANDA showed the highest activity for this detoxifying enzyme. However, the mechanism of detoxyfication in all the strains was very unfrequent, which indicates that its role in resistance to insecticide for all the studied strains is not so important. Glutathione-S-transferase values were compared with non-specific esterase and altered acetylcholinesterase frequencies for each of the studied strains.

Exposure to lead is a widespread and serious threat to the health of children in Latin America. Health officials should monitor sources of exposure and health outcomes to design, implement, and evaluate prevention and control activities. To evaluate the magnitude of lead as a public health problem, three key elements must be defined: I) the potential sources of exposure, 2) the indicators to evaluate health effects and environmental exposure, and 3) the sampling methods for the population at risk. Several strategies can be used to select the study population depending on the study objectives, the time limitations, and the available resources. If the objective is to evaluate the magnitude and sources of the problem, the following sampling methods can be used: I) population-based random sampling; 2) facility-based random sampling within hospitals, daycare centers, or schools; 3) target sampling of high risk groups; 4) convenience sampling of volunteers; and 5) case reporting (which can lead to the identification of populations at risk and sources of exposures). For all sampling methods, information gathering should include the use of a questionnaire to collect general information on the participants and on potential local sources of exposure, as well as the collection of biological samples. In interpreting data, one should consider the type of sampling used and the non-response rates, as well as factors that might influence blood lead measurements, such as age and seasonal variability. This paper analyzes the Latin American experience of decentralizing health services within the context of health reform. We examine the meaning of the term decentralization and discuss the various modalities of this concept: geographical, institutional and functional. The objectives that, in general, these objectives have not been achieved. After reviewing the literature and drawing on our own fieldwork, we conclude that in many instances the Latin American decentralization programs have produced results opposite to those intended, i.e., these programs have increased inequality and the cost of services and have reduced efficiency and quality of care. We point out that existing information is insufficient to determine with exactitude whether the failure of decentralization is due to the selection of inappropriate policies or to failures in the implementation process. Decentralization is a complex political process, policy makers should decide on the modality of decentralization they wish to implement, estimate the costs, identify the potential obstacles that could surface during the implementation phase and provide solutions. We conclude by suggesting that the multilateral banks have erred by pressing governments to improvise the implementation of decentralization programs without taking into account the different historical, political and socioeconomic contexts.

This paper identifies the trends and recent progress in the generation and reproduction of knowledge on health economic evaluation. Analysis is organized along nine public health action fields, namely: health determinants and predictors, economic value of health, healthcare demand, healthcare supply, microeconomic evaluation of healthcare, healthcare market balance, evaluation of policy instruments, general evaluation of the health system, and healthcare planning, regulation and supervision. Each action field is defined to place the reader in the proper setting and level of analysis. In addition, thematic research topics developed in each action field are proposed and discussed. The generation and reproduction of knowledge on the different action fields was based on the review of the bibliographic databases MEDLINE and LILACS for the 1992-2000 period. Results lead to the conclusion that development and application of economic evaluation of healthcare has been uneven across different countries and that there is a growing increase of applications starting in 1994, the year of initiation of healthcare reform in Latin America.

This paper examines the narrative testimony of women survivors of two atrocious events that took place in the XX century: the Holocaust in the 40's and the HIV/AIDS pandemic in the 80's. The author recognizes similarities in thoughts, feelings, experiences and meaning regarding several issues of human suffering that emerge from these testimonies. It expounds the perceptions on death, motherhood, family separation, intimacy and sexuality. For the author approach of these issues from a female perspective can provide new meanings arrived at the development of a new discourse and new social practices. Promotes to think about reactions of indifference before human suffering. Concludes by questioning why technological, scientific advances and advances in social development have not been able to provide human responses to human problems.

The recent growth of urban violence obliges us to examine the multiple factors and the variety of participants, scenarios, and logics which simultaneously interact to give rise to the current state of violence occurring on a daily basis in our societies. This paper attempts to specify the dimensions of the complex problem of violence, introducing concepts and data developed in the course of several research projects and stressing social and criminal violence in preference to other kinds. It is structured in terms of four themes: impoverishment and expectations, vulnerability and the State, the spread of firearms, and the idea of respect among men. In the light of these four themes and of the unprecedented transformations taking place in world society at the present time, the paper concludes by articulating the need to understand today's violence as a new social phenomenon in Latin America.

Objective: This paper characterizes the current stage of traditional medicine in nine countries of Latin America and the Caribbean., Material and Methods: This qualitative study was conducted between March and December 1998. Data were collected on the components of traditional health systems in countries of Latin America and the Caribbean, by means of a network of individuals and institutions from different countries that acted as expert informants from different specialty areas., Results: Findings from the analysis of traditional medicine regulation are presented in three groups: a) Countries with some developments in the area of legislation; b) Countries where legislation is underway; and, c) Countries with no legislation or incipient regulation., Conclusions: Several stages of traditional medical practice legislation are found in the region. This heterogeneity shows the complexity involved in regulating the practice of providers with low levels of formal training, with different therapeutic practices, and with customs that are frequently difficult to include within the standards of the official health system. These findings are important for designing and implementing healthcare policies to adequate traditional medical practices to the needs of populations that commonly use them.

In the present study, based on data from nine Latin American countries, we found evidence of an association between the economic crisis and infant mortality during the last decades. The paper initially review previous studies on this issue and shows the need for a greater research focus on shorter time intervals. We then describe the deterioration and unequal conditions among the countries based on trends in selected social and economic indicators and the evolution of infant mortality rates. According to our statistical analysis, infant mortality bore an inverse association to short-term economic variations. We also found a significant and negative correlation between decreasing infant mortality rates and increasing poverty. The economic crisis displayed effects of varying intensity among the countries we analyzed, with social inequality appearing as the most probable explanatory variable.

In the early 1990s, most Latin American and Caribbean countries were beginning, or planning to begin, health sector reform processes. This paper presents the status and trends of health sector reform at the end of the 1990s. The authors relied on information in 20 health system and services profiles completed by the Pan American Health Organization between August 1998 and October 1999. The analysis, which follows a methodology that had been applied earlier, was organized on two different levels: (1) monitoring the reform processes (dynamics and content) and (2) evaluating their outcomes. In looking at the dynamics of the reform processes, the article examines the context in which they take place and the actors involved in their different phases: inception, design and negotiation, implementation, and evaluation. The description and analysis of the contents of health sector reform initiatives are organized into 12 broad thematic areas. Outcomes evaluation was only possible in the eight countries that provided enough pertinent information, and should be viewed as preliminary. Nevertheless, the article does present detailed information on the outcomes of health sector reform in terms of equity, effectiveness and quality, efficiency, sustainability, and societal participation and control. The article also discusses the potential causes and determining factors for the observed outcomes.

Many recent papers show the important role of bioactive phytochemicals to maintain a good health status. Among them the carotenoids are the best known. About 637 have been described and possibly 70 of them could have an important role in human health, 16 have been found in human brain in high amounts. Most of the studies have found relations between the carotenoids and chronic non-communicable diseases like several types of cancer, atherogenic disease and some degenerative pathology of the eye. This relation is mediated by genes and age. Studies of carotenoids are of scientific and economic interest for Latin America as many tropical products are high sources of these compounds. Therefore the first task is to analyze them and iniciate some evaluation on its metabolic availability. A coordinated regional work is proposed, in which 40 or 50 fruits and vegetables are analyzed in terms of the seven carotenoids most related to human health. At the same time it will be important to start epidemiological studies that will compare groups with different levels of consumption of fruits and vegetables and make chronic disease risk analysis. In some countries of the Latin American region, with the support of FAO and INFOODS, some courses and meetings are taking place so that in a short time period the carotenoid composition of the important regional foods will be completed and a carotenoid regional food composition table be published.
